Abstract:The aim of this paper was to study the effect of Wubi Shanyao Pills on sexual dysfunction in rats with kidney-Yang deficiency and to investigate its possible mechanism. Adenine(100 mg·kg~(-1)) was administered to male SD rats for 8 weeks to establish kidney-Yang deficiency model, and at the same time, Wubi Shanyao Pills(2, 1, 0.5 g·kg~(-1)) were administered to rats for 8 weeks. The syndrome manifestation of kidney-Yang deficiency was observed in rats and the scores of symptoms were evaluated. Sexual behavior indexes(incubation period and times of capture, straddle and ejaculation) were measured by mating experiment. The levels of serum testosterone(T), estradiol(E_2), follicle stimulating hormone(FSH), luteinizing hormone(LH), and gonadotropin releasing hormone(GnRH) were measured by radioimmunoassay. The wet weights of testis and seminal vesicle were measured. The content of fructose in seminal plasma was detected by UV spectrophotometry. The pathological changes of testis and epididymis were observed by HE staining. The expression levels of transforming growth factor(TGF-β1) and cytochrome P450 aromatase(CYP19) in testis were detected by immunohistochemistry and Western blot. The results showed that Wubi Shanyao Pills could significantly reduce the score of kidney-Yang deficiency syndrome, improve the symptoms of kidney-Yang deficiency syndrome, shorten capture, straddle and ejaculation latency, increase capture and straddle times, increase serum T, LH, FSH, E_2 and GnRH levels, increase the wet weight of testis and seminal vesicle and fructose content in seminal plasma, improve the pathological structure of testis and epididymis, and inhibit the expression of TGF-β1 and increase CYP19 in testis of the model rats. Therefore, Wubi Shanyao Pills can significantly improve sexual dysfunction in rats with kidney-Yang deficiency, and its mechanism may be related to regulating the low function of hypothalamus pituitary gonad(HPG) axis and improving the disorder of sex hormone secretion. In addition, it may be also related to inhibiting the expression of testicular TGF-β1, increasing the expression of CYP19 protein, and then regulating the amount of T converted to E_2.

[Pricking-reinforcing -reducing therapy improves semen quality and seminal plasma biochemical indexes in patients with varicocele infertility]

Abstract:Objective: To investigate the effect of pricking-reinforcing -reducing therapy (PRRT) on the semen quality and seminal plasma biochemical indexes of varicocele (VC) infertility patients. Methods: We randomly and equally assigned 160 patients with VC infertility into a PRRT and a control group, the former treated by PRRT and the latter with oral ShengjingCapsules. Before and after treatment, we obtained the semen parameters, sperm morphology, sperm survival rate, sperm acrosin activity, seminal plasma neutral α glucosidase and seminal plasma zinc in the patients and compared them between the two groups. Results: Before treatment, there were no statistically significant differences between the PRRT and control groups in sperm concentration (［16.81 ± 7.83］ vs ［16.80 ± 7.54］ ×106 /ml, P > 0.05), total sperm count (［42.01 ± 19.57］ vs ［41.99 ± 18.84］ ×106, P > 0.05), percentages of progressively motile sperm (PMS) (［15.37 ± 11.03］% vs ［14.68 ± 10.27］%, P > 0.05) and morphologically normal sperm ( MNS) (1.62 ± 1.51］% vs ［1.62 ± 1.13］%, P > 0.05), sperm survival rate (［28.11 ± 18.95］% vs ［28.23±18.38］%, P > 0.05) and sperm acrosin activity (［28.11 ± 14.64］ vs ［27.19 ± 14.07］ U/L, P > 0.05). After three months of treatment, all the patients showed evident increases in the above parameters (P < 0.05), even higher in the PRRT than in the control group, more significantly in sperm concentration (［38.88 ± 30.54］ vs ［25.60 ± 14.71］ ×106 /ml, P < 0.05), PMS (［32.60 ± 12.46］% vs ［27.67 ± 12.27］%, P < 0.05) and sperm acrosin activity (［65.74±31.81］ vs ［67.94±17.95］ U/L, P < 0.05), though not significantly in total sperm count (97.20 ± 76.35］ vs ［88.19 ± 39.56］ ×106, P > 0.05), MNS (［2.35 ± 1.83］% vs ［1.87 ± 1.20］%, P > 0.05) and sperm survival rate (［61.44 ± 20.02］% vs ［59.12 ± 22.48］%, P > 0.05). Compared with the baseline, after treatment, the patients in the PRRT group also exhibited elevated levels of neutral α-glucosidase (［14.42 ± 5.90］ vs ［28.43 ± 19.76］ U/L, P < 0.05) and seminal plasma zinc (［2.11 ± 1.22］ vs ［2.89 ± 1.23］ mmol/L, P < 0.05), and so did the controls (［14.44 ± 5.61］ vs ［26.66 ± 17.69］ U/L , P < 0.05) and (［2.09 ± 1.10］ vs ［2.82±1.08］ mmol/L, P < 0.05). No statistically significant difference, however, was observed between the two groups after treatment (P > 0.05). Conclusion: PRRT can significantly improve semen quality in patients with VC infertility, even more effective than ShengjingCapsules in improving sperm concentration, PMS, sperm survival rate, and sperm acrosin activity, which may be related to its effect of elevating the levels of seminal plasma neutral-α glucosidase and zinc providing sufficient energy for basic sperm metabolism, maturation, energy acquisition and motility.
Zhuangyao Jianshen pill ameliorates testosterone-induced benign postatic hyperplasia in rats

Abstract:Objective: To determine the therapeutic effects of the Zhuangyao Jianshen pill (, ZYJSP) against benign prostatic hyperplasia (BPH) and investigate the underlying mechanism. Methods: Forty-eight male Sprague-Dawley rats were randomly divided into six groups: Control group, BPH model group, finasteride-treated group, ZYJSP low, medium and high dose groups. Except for the control group, 40 rats were castrated and injected with testosterone propionate (TP) for 28 consecutive day to induce BPH. Meanwhile, the corresponding drugs were administered by gavage. The prostate wet weight, prostate index (PI), and the histopathological changes in the prostate were measured as the basis for examining the efficacy of ZYJSP against BPH. Levels of the serum sex hormones, oxidative stress markers, inflammatory markers, renal function markers, growth factors, and Cyclin D1 expression in prostate were measured to characterize the therapeutic mechanism of ZYJSP against BPH. Results: ZYJSP administration significantly reduced prostate wet weight and PI and ameliorated histological changes of the prostate in TP-treated castrated rats. TP markedly increased the levels of creatinine, blood urea nitrogen and growth factors in the serum as well as the expression of the Cyclin D1 in the prostate. Most of these markers were significantly decreased by ZYJSP. ZYJSP significantly restored the dysregulation of testosterone, estradiol, and dihydrotestosterone caused by TP. Furthermore, ZYJSP relieved TP-induced prostate injury and exhibited both anti-inflammatory and anti-oxidant activity by decreasing interleukin-6, interleukin-8, and malondialdehyde levels and increasing the activity of superoxide dismutase in the serum. Conclusion: These findings indicate that ZYJSP can effectively ameliorate BPH induced by TP in castrated rats, and the underlying mechanism might be related to regulating sex hormone balance, reducing oxidative stress, and inhibiting the inflammatory response.
In vivo and in vitro protective effects of the Wuzi Yanzong pill against experimental spermatogenesis disorder by promoting germ cell proliferation and suppressing apoptosis

Abstract:Ethnopharmacological relevanceWuzi Yanzong pill (WZYZP) is a classical traditional Chinese medicine (TCM) formula originated from the Tang dynasty. WZYZP has a long history of use for reinforcing kidney and alleviating male infertility in China.Aim of the studyThe effect of WZYZP on male infertility and the mechanism underlying this effect was not clarified clearly. Therefore, this study aimed to investigate the protective effect of WZYZP in experimental spermatogenesis disorder via in vivo and in vitro studies, to promote the use of this formula for the treatment of spermatogenesis disorder.Material and methodsMale SD rats were exposed to tripterygium glycosides to induce experimental spermatogenesis disorder, and WZYZP was subsequently administrated at different dosages for treatment. Sperm counts, sperm motility, and serum hormone levels were detected. HE staining and TUNEL staining were performed to evaluate the pathological lesions and apoptosis of testes, respectively. Next, germ cells were isolated from spermatogenesis disorder-model rats and treated with WZYZP- containing serum at different concentrations. CCK-8 assay and flow cytometry assay were performed to detect cell proliferation and apoptosis. Immunofluorescence assay, qRT-PCR and Western blotting analyses were performed to detect the expression of Beclin 1, LC3 and TGF-β-PI3k/AKT-mTOR pathway - related factors, including TGF-β, PI3K, AKT, mTOR, 4 EBP-1 and p70S6K.ResultsIn vivo experiments showed that WZYZP protected against spermatogenesis disorder in model rats by improving sperm count and motility, as well as restoring serum hormone levels. HE and TUNEL staining demonstrated that the pathological injuries and cell apoptosis in testes of the model rats were alleviated by WZYZP treatment. Moreover, in vitro experiments of germ cells isolated from spermatogenesis disorder-model rats showed that WZYZP treatment increased the cell proliferation, inhibited cell apoptosis and autophagy. qRT-PCR and Western blotting assay results showed that this protective effect was associated with the regulation of the TGF-β/PI3K/AKT/mTOR signaling pathway. The expression levels of p-PI3K/PI3K, p-AKT/AKT, p-mTOR/mTOR, 4 EBP-1 and p70S6K were increased, while TGF-β was inhibited in the WZYZP treated groups.ConclusionThe results showed that WZYZP could protect against experimental spermatogenesis disorder by increasing the germ cell proliferation and inhibiting their apoptosis. Our support the clinical use of this formula for the management of spermatogenesis disorder.

Abstract:Varicocele (VC) refers to expansion and tortuosity of spreading venous plexus in spermatic cord due to poor blood flow. This study aimed to investigate effects of Shugan Tongluo Qiangjing recipe (SGTL) on sperm DNA damage and oxidative stress in experimental VC (EVC) rats. EVC model was established by partial ligation of left renal vein. Spermatic vein diameter, testicular weight, sperm DNA fragmentation index (DFI) were evaluated. Telomere reverse transcriptase (TERT) expression, telomere gene transcription, and testicular tissue morphology were determined. H 2 O 2 , catalase, SOD, T-AOC were measured with colorimetry. SGTL significantly decreased spermatic vein diameter ( P =0.000) and increased testicular weight ( P =0.013) of rats compared those of EVC rats. SGTL maintained testicular tissue morphology in EVC rats. SGTL markedly reduced sperm DFI value in sperm of rats compared to EVC rats ( P =0.000). SGTL significantly enhanced TERT expression and telomere gene transcription ( P =0.028) in testis of rats compared to EVC rats. SGTL reduced H 2 O 2 levels (P=0.001) and promoted CAT activity (P=0.016), SOD activity (P=0.049), and T-AOC activity (P=0.047) of rats, compared to EVC rats. In conclusion, SGTL could reduce pathogenic process of EVC by reducing sperm DNA damage and regulating telomere length in EVC rats, which may be related to oxidative stress regulation.

The Therapeutic Effects of a Traditional Chinese Medicine Formula Wuzi Yanzong Pill for the Treatment of Oligoasthenozoospermia: A Meta-Analysis of Randomized Controlled Trials

Abstract:Oligoasthenozoospermia is a crucial factor in male infertility. Wuzi Yanzong (WZYZ) pill is a popular traditional Chinese medicine (TCM) formula which has been used for male infertility treatment for years. However, its effects on semen quality remain controversial. We conducted a preregistered meta-analysis to assess the effect of WZYZ pill for the therapeutic effects on oligoasthenozoospermia. Five randomized controlled trials including 960 participants were selected from databases of domains in North-East Asian regions, PubMed, Embase, and Cochrane Library. WZYZ pill group yielded a greater mean increment on sperm concentration (5 trials: MD 5.99, 95% CI 2.12–9.85, P = 0.002 ), sperm motility (5 trials: MD 4.57, 95% CI 0.47–8.68, P = 0.03 ), sperm morphology (2 trials: MD −1.93, 95% CI −4.87–1.01, P = 0.20 ), activity of acrosomal enzyme (2 trials: MD 28.27, 95% CI 12.41–44.14, P < 0.01 ), volume of semen (2 trials: MD 0.56, 95% CI 0.21–0.91, P = 0.002 ), and a decrement of sperm DNA fragmentation index (2 trials: MD −3.82, 95% CI −6.45–−1.19, P = 0.004 ). However, qualities of selected studies were generally unsatisfactory, and there was inherent heterogeneity among some of the outcomes. Despite these limitations, the WZYZ pill improved sperm quality by improving several semen parameters and decreasing DNA damage in oligoasthenozoospermia patients.

Abstract:The reliability of the L5178Y TK+/- forward mutation assay as a rapid screen for genotoxicity was evaluated by testing 63 coded chemicals. Replicate treatments were used, and at least two independent experiments were performed for each test condition. The test conditions consisted of no exogenous activation, activation by Aroclor 1254-induced Fischer 344 rat liver S9 homogenate, and in some cases activation by noninduced Fischer 344 rat liver S9. The results were organized into tables that show the mutant colony counts, mutant frequency, and toxicity for each test chemical treatment, positive control treatment, and solvent negative control cultures. The repeat experiments were highly consistent and yielded contradictory evaluations for only a few of the chemicals studied. Fifty-one of the chemicals (81%) were evaluated as mutagenic under one or both of the test conditions. A range in minimum effective concentrations of almost 10(6)-fold (0.008 to 5,000 micrograms/ml) was observed among the mutagenic chemicals. Nine chemicals (14%) were considered to be nonmutagenic. Three chemicals (progesterone, p-rosaniline HCl, and 1,1,1-trichloroethane) gave responses that were not easily evaluated under any test condition: evidence for mutagenesis was obtained in some experiments but not for all repeat studies. Under nonactivation conditions, specifically, the mutagenic activities of 4,4'-bis(dimethylamino)benzophenone, progesterone, and p-rosaniline HCl remained uncertain. With S9 activation, uncertain evidence for mutagenesis was obtained for 2-naphthylamine, progesterone, and 1,1,1-trichloroethane. In some cases, changes in the treatment conditions could lead to different evaluations of the mutagenic activity, and these possibilities are discussed in the descriptive evaluations of each chemical. Comparisons of the observed responses with published results were possible for 29 of the compounds and yielded highly confirmatory evaluations.
Effects of Zuogui pill on reproductive endocrine hormones in rats with oligoasthenospermia

Abstract:Objectives:To observe the preventive effect of Zuogui Pill on sperm parameters,testis/epididymal coefficient,reproductive hormone level and carnitine content in epididymis of rats with oligoasthenospermia induced by tripterygium wilfordii (GTW).Methods:36 male Sprague-Dawley rats were randomly divided into normal group,model group and Zuogui pill group,with 12 rats in each group,for a total of 8 weeks.The rats in model group were treated with deionized water for the first 4 weeks,and then the rats were treated with 40rag / (kg · d) Tripterygium wilfordii for 5-8 weeks.The rats in Zuogui pill group were treated with 6g/ (kg · d) Zuogui pill for preventive intervention for the first 4 weeks,and 40mg / (kg · d) of the tripterygium glycosides to intervene for 5-8 weeks.(LH),follicle-stimulating hormone (FSH),estrogen (E2),prolactin (PRL) and testosterone (T)were measured,and the testis weight was measured.Inhibin B (INHB) and epididymal carnitine (EC) were measured.Results:The differences in contents of FSH,LH,T,INHB and EC in testis between Zuogui pill group and model group were significantly different (P ＜ 0.05 or P ＜ 0.01),and the levels of INHB and EC in Zuogui pill group and model group were still different from these of normal group (P ＜ 0.05).Conclusion:Zuogui Pill can effectively reduce the damage degree of GTW on the reproductive organs of rats,including the effects of general state,testis/epididymis organ index,sperm quality and reproductive hormone levels,and the content of carnitine in epididymis.It is proved that Zuogui Pill has certain preventive effect on rats with oligoasthenospermia.
Yangjing Capsule Improves Oligoasthenozoospermia by Promoting Nitric Oxide Production Through PLCγ1/AKT/eNOS Pathway.

Abstract:Background: Oligoasthenozoospermia is an important factor leading to male infertility. Yangjing capsule (YC), a traditional Chinese preparation, displays beneficial effects on male infertility. However, whether YC could improve oligoasthenozoospermia remains unclear.Methods: In this study, we aimed to explore the effect of YC in the treatment of oligoasthenozoospermia. Male Sprague-Dawley (SD) rats were treated with 800 mg/kg ornidazole once daily for 30 days to induce in vivo oligoasthenozoospermia; primary Sertoli cells were treated with 400 μg/mL ornidazole for 24 h to induce in vitro oligoasthenozoospermia.Results: We found that YC improved the testicle and epididymis weight, sperm concentration, sperm progressive motility, serum testosterone, fertility rate and testis morphology in ornidazole-exposed rats and enhanced cell survival in ornidazole-stimulated primary Sertoli cells. YC also inhibited the ornidazole-caused decrease in nitric oxide (NO) generation and the phosphorylation of phospholipase C γ1 (PLCγ1), AKT, and eNOS in vivo and in vitro in oligoasthenozoospermia. Furthermore, the knockdown of PLCγ1 blunted the beneficial effects of YC in vitro.Conclusion: Collectively, our data suggested that YC protected against oligoasthenozoospermia by promoting NO levels through the PLCγ1/AKT/eNOS pathway.

Abstract:Abstracts Background Qilin pills (QLPs), a classic Traditional Chinese Medicine (TCM) formula for treating male infertility, effectively improve semen quality in clinical trials. This study was designed to evaluate the effects of QLPs on spermatogenesis, reproductive hormones, oxidative stress, and the testis-specific serinekinase-2 (TSSK2) gene in a rat model of oligoasthenospermia. Methods Forty adult male Sprague-Dawley (SD) rats were randomly divided into four groups. The rat model with oligoasthenospermia was generated by intragastric administration of tripterygium glycosides (TGs) once daily for 4 weeks. Then, two treatment groups were given different doses (1.62 g/kg and 3.24 g/kg) of QLPs once daily for 60 days. Sperm parameters, testicular histology and reproductive hormone measurements, oxidative stress tests, and TSSK2 expression tests were carried out. Results QLPs effectively improved semen parameters and testicular histology; restored the levels of FSH, LH, PRL, fT, and SHBG; reduced the levels of oxidative stress products (ROS and MDA); increased testicular SOD activity; and restored the expression of spermatogenesis-related gene TSSK2. Conclusion QLPs have a therapeutic effect on a rat model of oligoasthenospermia, and this effect is manifested as improvement of semen quality and testis histology, gonadal axis stability, decreased oxidative stress, and the regulation of testis-specific spermatogenesis-related gene TSSK2.

Abstract:Background: Varicocele (VC) is a relatively common and treatable cause of male infertility. Mailuoshutong pill (MLST), a traditional Chinese patent medicine, is widely used for treating varicose vein disease, but the underlying mechanism of MLST on varicocele-associated male infertility is unclear. Objective: To reveal the phytochemical characterisation and multitarget mechanism of MLST on varicocele-associated male infertility. Methods: The components in MLST were determined using UHPLC-MS/MS. Through network analysis, we constructed the “Drug-Components-Targets-Disease” network and predicted the potential biological functions and signaling pathways of MLST. Finally, the therapeutic effects and potential mechanisms of MLST were discovered by pharmacological experiments. Results: By network analysis, the “Drug-Components-Targets-Disease” network was constructed, 62 components such as apigenin, limonin, kaempferol, and obacunoic acid may be the main active components of MLST for varicocele-associated male infertility, 28 targets such as VEGFA, PIK3CA, AKT1, and MTOR are considered as hub targets, signaling pathways such as HIF-1, Estrogen, PI3K/Akt, and mTOR may be key pathways for MLST against varicocele-associated male infertility. Through pharmacological experiments, we found that MLST ameliorated VC-induced testicular atrophy. Further histomorphology showed that MLST reduced VC-induced damage to testicular spermatogonia and seminiferous tubule, while MLST reduced ROS and MDA levels and increased antioxidant enzymes (GSH, GSH-Px, SOD, and CAT) levels. TUNEL staining and immunofluorescence showed that MLST reduced VC-induced apoptosis in testicular tissue, decreased BAX, and increased BCL2. Western blot results showed that MLST decreased the phosphorylation of PI3K, AKT, and mTOR proteins, and decreased the expression of HIF1α. Conclusion: The phytochemical characterisation and multitarget mechanism of MLST on varicocele-associated male infertility were discovered using network analysis and pharmacological experiments. We verified that MLST can inhibit the activation of the PI3K/Akt/mTOR signaling pathway, reduce the expression of HIF1α, and further attenuate VC-induced oxidative stress and apoptosis in the testis. These findings provide evidence for the therapeutic role of MLST in varicocele-associated male infertility.

Abstract:Objective:To observe the effects of Zuogui Pills on the expression of Bcl-2 and Bax protein in testis of dyszoospermia rats rnduced by Tripterygium glycoside (GTW),and to preliminary explore the mechanism of GTW in improving the reproductive function of model rats.Methods:Thirty-six SD rats were randomly divided into the normal group (blank control),model group and Zuogui Pills group,12 rats in each group.Dyszoospermia rat models were established by gavaging GTW 40mg · kg-1 · d-1 for 4 weeks,while rats from Zuogui Pills group were treated by Zuogui Pills Solution 6g ·kg-1 ·d-1 for 4 weeks.After modelization,amount of sperms of each group were detected,and the expression of Bax and Bcl-2 protein and mRNA in the testes of each group were detected by immunohistochemistry,Western blot and RT-PCR respectively.The differences of different histones were analyzed and compared.Results:After treating with Zuogui Pills,the number of spermatozoa in Zuogui Pills group was significantly better than that of the model group (P＜0.05).The expression of Bcl-2 protein and mRNA were significantly increased,and showed high positive expression (P＜0.05);Bax protein and mRNA expression also increased to varying degrees with obvious trend,although no significant differences were found.Conclusion:Zuogui Pills can effectively reduce the GTW-induced damage of rat spermatogenic cells,up-regulate the expression of Bcl-2 protein,and inhibit the apoptosis of spermatogenic cells,which showed obvious therapeutic effect.

Abstract:Background. The testicular microcirculation was an important aspect of testicular physiology and it offered a stable environment for the transport of nutrients and secretary products in the testis. Yangjing capsule (YC), a traditional Chinese compound herbal prescription, has been proved as an effective drug to ameliorate spermatogenesis, promote testosterone synthesis in vivo, and cure spermatogenesis in clinical practice. Objective. This study was aimed at understanding the potential mechanisms of YC exerting angiogenic effects in the mouse spermatogenesis dysfunction model induced by cyclophosphamide (CP) and MLTC-1 cells. Materials and Methods. Balb/c mice were randomly divided into five groups: control, CP, CP plus YC (630 mg/kg), CP plus YC (1260 mg/kg), and CP plus YC (2520 mg/kg). After 30 days, mice were sacrificed and the expressions of endothelial marker CD34+, angiogenic marker VEGFA, VEGFR1, VEGFR2, and eNOS in the testes of the mice were examined; moreover, Leydig cell line MLTC-1 cells were cultured and treated with different concentrations of YC extracts (YCE), and the expressions of VEGFA, VEGFR1, VEGFR2, and eNOS, as well as the secretion of NO, were evaluated. Results. We observed that YC significantly increased the expressions of VEGFA, VEGFR1, VEGFR2, and eNOS in testes of CP-treated mice; moreover, YCE has led to increased expressions of VEGFA, VEGFR1, VEGFR2, and eNOS and secretion of NO in MLTC-1 in vitro. These data suggested that the YC might be an alternative treatment for the dysfunction of testicular microcirculation by promoting the angiogenesis in the testis.

Abstract:Objective To evaluate the effects of improved left varicocele (ELV) on testis enzymes and proliferating cell nuclear antigen (PCNA) in rats. Methods ELV models of SD rats were established by the improved method in 21 SD rats, and another 15 were included in a sham operation group as controls. At the 3rd month, the activity of LDH, SDH and G6PDH was measured by colorimetry. The concentration of the PCNA was determined by enzyme-linked immunosorbent assay ( ELISA). Results The activity of LDH and G6PDH (8. 17 ± 3. 47), (34. 00 ± 16. 29) U/mg in left testes of experimental group was reduced remarkably as compared with that in right testes (12.69 ±3.97), (78.03 ± 25. 28) U/mg and control group (11. 98 ± 2. 26) , (54. 88 ± 20. 87) U/mg (P 0. 05). Conclusion The reduction of LDH, SDH and G6PDH and the changes of PCNA in improved ELV may play an important role in the development of subfertility. Abstract:Background Varicocele (VC) is present in 35 - 40% of men with infertility. However, current surgical and antioxidant treatments are not completely effective. In addition to oxidative stress, it is likely that other factors such as testicular immune microenvironment disorder contribute to irreversible testicular. Evidence suggests that VC is associated with anti-sperm antibodies (ASAs), spermatogenesis and testosterone secretion abnormalities, and testicular cytokine production. Moreover, inhibition of inflammation can alleviate VC-mediated pathogenesis. The normal function of the testis depends on its immune tolerance mechanism. Testicular immune regulation is complex, and many infectious or non-infectious diseases may damage this precision system. Results The testicular immune microenvironment is composed of common immune cells and other cells involved in testicular immunity. The former includes testicular macrophages, T cells, dendritic cells (DCs), and mast cells, whereas the latter include Leydig cells and Sertoli cells (SCs). In animal models and in patients with VC, most studies have revealed an abnormal increase in the levels of ASAs and pro-inflammatory cytokines such as interleukin (IL)-1 and tumor necrosis factor (TNF)-alpha in the seminal plasma, testicular tissue, and even peripheral blood. It is also involved in the activation of potential inflammatory pathways, such as the nucleotide-binding oligomerization domain-like receptor family pyrin domain containing (NLRP)-3 pathway. Finally, the development of VC-mediated infertility (VMI) may be facilitated by abnormal permeability of proteins, such as claudin-11, that constitute the blood-testis barrier (BTB). Conclusions The testicular immune response, including the production of ASAs and inflammatory factors, activation of inflammatory pathways, and destruction of the BTB may be involved in the pathogenesis of VMI it is necessary to further explore how patient outcomes can be improved through immunotherapy.

Abstract:The crisis of male infertility is an issue of human reproductive health worldwide. The Wuzi Yanzong pill (WZYZP) is a traditional Chinese medicine prescription that shows efficacy in kidney reinforcement and essence benefit to ameliorate male reproductive dysfunctions. However, the pharmacological mechanisms of the WZYZP on male infertility have not been investigated and clarified clearly. This study was designed to investigate the effects of the WZYZP on spermatogenesis disorder and explore its underlying pharmacological mechanisms. First, based on a network pharmacology study, 39 bioactive compounds and 40 targets of the WZYZP associated with spermatogenesis disorder were obtained, forming a tight compound-target network. Molecular docking tests showed tight docking of these compounds with predicted targeted proteins. The protein–protein interaction (PPI) network identified TP53, TNF, AKT1, Bcl-XL, Bcl-2, and IκBA as hub targets. The Kyoto Encyclopedia of Genes and Genomes pathway network and pathway-target-compound network revealed that the apoptosis pathway was enriched by multiple signaling pathways and multiple targets, including the hub targets. Subsequently, the chemical characterization of WZYZP was analyzed using liquid chromatography to quadrupole/time-of-flight mass spectrometry, and 40 compounds in positive ion mode and 41 compounds in negative ion mode in the WZYZP were identified. Furthermore, based on the prediction of a network pharmacology study, a rat model of spermatogenesis disorder was established to evaluate the curative role and underlying mechanisms of the WZYZP. The results showed that WZYZP treatment improved rat sperm quality and attenuated serum hormone levels, reversed histopathological damage of the testis, reduced cell apoptosis in testis tissues, and ameliorated the expression of the predicted hub targets (TP53, TNF-α, AKT1, NFκB, and IκBA) and the apoptosis related proteins (Bcl-XL, Bcl-2, Bax, Caspase 3, and Caspase 9). These results indicated that the WZYZP has a protective effect on spermatogenesis disorder, suggesting that it could be an alternative choice for male infertility therapy.

Abstract:Objective To observe the effects of Zuogui pills on the expression levels of stem cell factor (SCF) protein and mRNA in the testis of oligoasthenozoospermia rats induced by tripterygium glycosides (TG), and based on this, to explore the mechanism of Zuogui pills on improving the reproductive function of these rats.Methods From March to April 2014, 36 SD, SPF grade rats were randomly divided into normal group, TG model group and Zuogui pill treatment group, with 12 rats in each. The normal group received intragastric administration of deionized water during the whole intervention period; for modelling, at 4 weeks before the experiment, both TG model group and Zuogui pill treatment group received oral administration of the suspension of TG 40 mg? kg-1? d-1, then, during the 5th-8th weeks of experiment, the TG model group received intragastric administration of deionized water, Zuogui pill treatment group received oral administration of the suspension of Zuogui pill 6 g? kg-1? d-1. At the end of the 8-week intervention, all the rats were sacrificed, and their testicular tissues were collected aseptically, fixed, embedded and sectioned. Immunohistochemistry was used to detect the expression level of SCF protein in the testicular tissue. Real-time PCR was employed to measure the expression level of SCF mRNA in the testicular tissue.Results Both the expression levels of SCF protein and mRNA in the testicular tissue were significantly higher compared with the TG model group (P<0.05).Conclusion Zuogugui pill can promote the division and proliferation of spermatogonial stem cells and inhibit the apoptosis of spermatogenic cells by up regulating the expression levels of SCF protein and mRNA.

Abstract:To investigate whether chemokine like factor (CKLF)-like myelin and lymphocyte and related proteins for vesicle trafficking and membrane link (MARVEL) transmembrane domain-containing protein 2 (CMTM2) is involved in spermatogenesis in varicocele induced subfertility rats and to discuss the possible mechanisms.Forty male SD rats (body weight: 220-330 g, age: 6-7 weeks) were randomly divided into 4 groups: varicocele for 4 weeks, varicocele for 12 weeks, sham operation for 4 weeks and sham operation for 12 weeks, with 10 rats in each group. These rats were introduced by partially ligating left kidney veins for the experimental groups, and the sham surgery groups as controls were executed with exactly the same surgery as in the experimental groups except for the ligation. The rats in control and experimental groups for 4 and 12 weeks were killed after laparotomy at the end of 4 and 12 weeks, respectively, the left testes and epididymis were taken out for counting the sperm, observing the seminiferous tubule change and immunochemistry for CMTM2. The changes included sperm density and motility, the outer diameter and inner diameter change and the changes of epithelium and the CMTM2 expression in immunochemistry.Compared with the control groups, the sperm density [(63.9±7.1)×10(6)/mL vs.(74.3±5.0)×10(6)/mL] and motility [(58.7%±7.9%) vs.(66.1%±4.3%)] were reduced slightly in group of varicoele for 4 weeks, respectively (t=1.432, 1.563; P=0.076, 0.059, respectively). Varicocele significantly caused a decrease in sperm concentration [(40.5±7.2) ×10(6)/mL vs.(71.1±4.5)×10(6)/mL] and motility [(35.2%±8.5%)vs. (63.4%±4.1%)] at 12 weeks, compared with the related sham groups (t=3.754, 3.933; P=0.004, 0.002, respectively). Additionally, testis CMTM2 exhibited the same disparity, that is, the CMTM2 protein expression in varicocele group was significantly reduced, with the ratio of sham group to varicocele group at the end of 12 weeks 2.3±0.4 (t=1.978; P=0.039). In the evaluation of seminiferous tubules diameter, the external [(198.2±10.2) μm vs. (255.8±12.7) μm, t=2.125, P=0.003] and epithelium diameter [(54.1±1.5) μm vs. (75.5±4.1) μm, t=2.246, P=0.021] were decreased compared with the sham-related groups and previous varicocele groups. In all the varicocele groups, all types of sperm motility decreased compared with the related sham-operated group (P<0.05).This study suggests varicocele has a detrimental effect on CMTM2 levels and decreases spermatogonia cell number, seminiferous tubules diameter, and sperm indices. CMTM2 is associated with sperm changes in rats with varicocele, and further studies are needed to study the mechanism.
