Abstract:Sir, Cerebral venous thrombosis (CVT) is a rare neurological condition that can occur after a stroke. Headache, seizures, or hemiplegia are common symptoms. Severe patients may present with altered mental status or a coma. Patients predisposed to CVT have suffered from multiple sclerosis,[1] cancer, and trauma. Here we report a case of CVT associated with IJV stenosis, a rare cause[2] to our knowledge. A 28-year-old primigravida (G1P0) with an uneventful pregnancy presented with seizures at 32+3 weeks of gestation. Her cranial magnetic resonance imaging (MRI) was unremarkable at that time. The seizure recurred at 39 weeks of gestation. She denied any medical history or substance use. On admission, her blood pressure was 121/74 mmHg, and the urine dipstick was negative. The neurological examination and 24-hour electroencephalogram were unremarkable. MRI and MR venography (MRV) showed multiple abnormal signals adjacent to one side of the cerebellum [Figure 1]. A healthy boy was delivered by cesarean section. Patient-controlled intravenous sufentanil (1.25 mcg/kg) and dezocine (1 mg/h) were administered postoperatively. Prophylactic magnesium sulfate was used to prevent eclampsia. Low molecular weight heparin (LMWH) was administered daily at 4000 IU. Subsequent CTA and CTV showed thrombosis at the junction of the straight and transverse sinuses. The jugular foramen of the affected side was smaller in diameter than the normal side (0.6 cm vs. 1.2 cm), suggesting congenital hypoplasia of the vessel [Figures 2 and 3]. On day 38 postpartum, she was started on oral levetiracetam 250 mg. No detectable venous collaterals were seenThe IJV is the main vessel that collects the outflow blood of the brain. It is an important access point to the central circulation in clinical practice. Physically, the diameter of the right IJV was larger than the left IJV in both men and women.[3,4] In general, there are very few cases of thrombosis caused by IJV stenosis due to collateral circulation. In the study, the patient did not report any complaints or symptoms related to IJV stenosis before pregnancy. Initially, the MRI did not show any localized deficits. We speculated that the hypercoagulable state in pregnancy exacerbated the slow blood stasis at the site of the vascular stenosis, which eventually led to thrombosis. Distinguishing CVT from eclampsia remains a challenge in pregnancy, especially when the chief complaints are headaches and seizures. In our experience, magnesium sulfate prophylaxis, or diazepam were the optimal initial emergency treatment for pregnant patients. Apart from that, our case highlighted the important role of radiological examination, such as head MRV, in pregnant women with seizures. The neurology consultant is very important for differential diagnosis and subsequent management.
