Asymptomatic Extrahepatic Portal Venous Obstruction: A Case Report

Abstract:This report describes a case of extrahepatic portal venous obstruction (EHPVO) with esophageal varices that would have led to significant bleeding if left untreated or inadequately managed. A 56-year-old diabetic and hypothyroid female visited our medical outpatient clinic to be assessed for pancytopenia and easy fatiguability. She experienced acute calculus cholecystitis 24 years ago, which was treated with a partial cholecystectomy. The laboratory tests showed indications of hypersplenism, characterized by anemia, leucopenia, and thrombocytopenia. The MRI results showed signs of long-term blockage of the portal vein outside the liver, with the liver tissue seeming normal. The upper gastrointestinal endoscopy showed grade IV esophageal varices, gastroesophageal varices 1, fundal varices, isolated gastric varices 2, and antral varices. The patient was diagnosed with EHPVO, and banding was performed as a preventive measure against upper gastrointestinal bleeding. Additionally, she was treated using conservative management techniques such as beta blockers. Endoscopic treatment is the standard of care for treating acute varices, while beta blockers are given as a secondary preventive measure. Despite cholelithiasis being a cause and/or sequelae to portal venous thrombosis, a clear explanation has not been offered to this patient while taking consent for cholecystectomy or thereafter. EHPVO is not frequently detected, and there is still a dearth of appropriate guidelines for managing this illness, even though it is a frequent cause of acute calculus cholecystitis and upper gastrointestinal bleeding.
A Rare Case of Extrahepatic Portal Venous Obstruction in a Nine-Year-Old Female and Its Management: A Case Report

Abstract:Extrahepatic portal vein obstruction (EHPVO) is a kind of liver vascular disease that causes structural abnormalities in the portal veins, including cavernomatous metamorphosis and obstruction. It is the most common cause of esophageal varices-related hematemesis in youngsters. Significant risk factors include congenital abnormalities, dehydration, sepsis, trauma, hypercoagulable conditions, and multiple transfusions. Acute extrahepatic portal vein blockage is often ignored because patients are usually asymptomatic. Subacute and chronic stages can cause symptoms including splenomegaly and hematemesis without hepatic decompensation. Imaging studies aid in the diagnosis; Doppler imaging is added to ultrasonography to visualize portal vein blood flow. MRI and CT scans are used to visualize portal vein blockage. Prevention of acute bleeding is the cornerstone in the management. Studies have shown that transhepatic thrombolysis is the preferred choice to avoid systemic side effects. Transjugular intrahepatic portosystemic shunt (TIPS) treats extrahepatic portal venous thrombosis and is typically followed by conservative variceal hemorrhage treatment. Liver transplantation is performed when other management measures fail. Here, we present a rare case of EHPVO in a nine-year-old female who was lost to follow-up for a long time and later showed signs of portal biliopathy and non-visualization of a surgically created splenorenal shunt. Re-shunting was performed after detailed conservative management, and the patient responded well to the treatment given.

Abstract:Esophageal varices develop as a sequalae of portal hypertension and have a high risk of fatality. Causes of portal hypertension can be divided into pre-hepatic, hepatic, and post-hepatic. Pre-hepatic causes include portal vein thrombosis and post-hepatic causes include heart failure. Hepatic causes can be pre-sinusoidal, sinusoidal, and post-sinusoidal. Of these, cirrhosis, which occurs at the level of sinusoids, is most common; 80%-90% of cases of portal hypertension are due to cirrhosis. Obliterative portal venopathy (OPV) is a non-cirrhotic cause of portal hypertension that occurs at the level of the pre-sinusoids. Despite its serious complications, OPV is not well understood and is sparingly reported in the literature. Here, we discuss a case of esophageal variceal bleed in setting of biopsy proven OPV due to 6-Mercaptopurine (6MP) use.

Abstract:Background & Aims: In patients with noncirrhotic chronic extra-hepatic portal vein obstruction (EHPVO), data on morbimortality of abdominal surgery are scarce. Approach & Results: We retrospectively analyzed the charts of 76 patients (78 interventions) with EHPVO undergoing abdominal surgery within the VALDIG network. Fourteen percent of the patients had ≥1 major bleeding (unrelated to portal hypertension) and 21% had ≥1 Dindo-Clavien grade ≥3 postoperative complication within 1 month after surgery. Fifteen percent had ≥1 portal hypertension related complication within 3 months after surgery. Three patients died within 12 months after surgery. An unfavorable outcome ( i.e. ≥1 above-mentioned complications or death) occurred in 37% of the patients and was associated with a history of ascites and with non-wall, non-cholecystectomy surgical intervention: 17% of the patients with none of these features had an unfavorable outcome, versus 48% and 100% when one or both features were present, respectively. We then compared 63/76 EHPVO patients with 126 matched (2:1) control patients without EHPVO but with similar surgical interventions. As compared with control patients, incidence of major bleeding ( p <0.001) and portal-hypertension related complication ( p <0.001) was significantly higher in patients with EHPVO, but not that of grade ≥3 postoperative complication nor of death. The incidence of unfavorable post-operative outcome was significantly higher in patients with EHPVO than in those without (33% vs. 18%, p =0.01). Conclusion: Patients with EHPVO are at high-risk of major peri- or postoperative bleeding and postoperative complications, especially in those with ascites or undergoing surgery other than wall surgery or cholecystectomy.

Abstract:BackgroundPortal hypertension is a key complication of portal hypertension, which is responsible for the development of varices, ascites, bleeding, and hepatic encephalopathy, which, in turn, cause a high mortality and requirement for liver transplantation.AimThis review deals with the present day state-of-the-art preventative treatments of portal hypertension in cirrhosis according to disease stage. Two main disease stages are considered, compensated and decompensated cirrhosis, the first having good prognosis and being mostly asymptomatic, and the second being heralded by the appearance of bleeding or non-bleeding complications of portal hypertension.ResultsThe aim of treatment in compensated cirrhosis is preventing clinical decompensation, the more frequent event being ascites, followed by variceal bleeding and hepatic encephalopathy. Complications are mainly driven by an increase of hepatic vein pressure gradient (HVPG) to values ≥10 mmHg (defining the presence of Clinically Significant Portal Hypertension, CSPH). Before CSPH, the treatment is limited to etiologic treatment of cirrhosis and healthy life style (abstain from alcohol, avoid/correct obesity…). When CSPH is present, association of a non-selective beta-blocker (NSBB), including carvedilol should be considered. NSBBs are mandatory if moderate/large varices are present. Patients should also enter a screening program for hepatocellular carcinoma. In decompensated patients, the goal is to prevent further bleeding if the only manifestation of decompensation was a bleeding episode, but to prevent liver transplantation and death in the common scenario where patients have manifested first non-bleeding complications. Treatment is based on the same principles (healthy life style..) associated with administration of NSBBs in combination if possible with endoscopic band ligation if there has been variceal bleeding, and complemented with simvastatin administration (20-40 mg per day in Child–Pugh A/B, 10–20 mg in Child C). Recurrence shall be treated with TIPS. TIPS might be indicated earlier in patients with: 1) Difficult/refractory ascites, who are not the best candidates for NSBBs, 2) patients having bleed under NSBBs or showing no HVPG response (decrease in HVPG of at least 20% of baseline or to values equal or below 12 mmHg). Decompensated patients shall all be considered as potential candidates for liver transplantation.ConclusionTreatment of portal hypertension has markedly improved in recent years. The present day therapy is based on accurate risk stratification according to disease stage.

Long-term Results of Variceal Bleeding Management in 302 Patients with Chronic Extrahepatic Portal Vein Obstruction.

Abstract:Background and Aim Treatment modalities of variceal bleeding or rebleeding for extrahepatic portal vein obstruction (EHPVO) are limited, and their long-term results and prognostic factors are unclear. This study aimed at assessing the long-term results of EHPVO treated with current recommendations and investigating the influencing factors. Methods Between 2009 and 2016, 302 consecutive patients with EHPVO were included. Watch-and-wait policy was applied for those with no (n = 59) or small varices (n = 55), nonselective beta-adrenergic blocker and nonselective beta-adrenergic blocker plus endotherapy were for primary (n = 115) and secondary prophylaxis (n = 87), transjugular intrahepatic portosystemic shunt (TIPS) or combination therapy was for those with recurrent bleeding (n = 92). Results The median follow up was 58.8 months. The 1-, 3-, and 5-year cumulative rates were 3.6%, 19.2%, 32.3% for small varices development and 4.5%, 30.9%, 53.4% for large varices development. The 1-, 3-, and 5-year cumulative rates were 11.1%, 20.9%, and 34.9% for first variceal bleeding and 16.0%, 26.9%, and 33.6% for variceal rebleeding. For those with recurrent variceal bleeding, only TIPS (n = 37, technical success rate: 90.2%) was associated with a reduced risk of variceal rebleeding (1-, 3-, and 5-year: 5.6%, 11.7%, and 21.9%). The 1-, 3-, and 5-year survival rates were 96.9%, 95.8%, and 91.9%. Prothrombotic factors and anticoagulation did not influence the risk of variceal bleeding, rebleeding, and survival. Conclusions By applying the same variceal management as recommended for patients with liver cirrhosis, patients with non-cirrhotic EHPVO showed a similar development of varices and variceal bleeding. This is also true for the beneficial effect of TIPS to prevent rebleeding.
Portal Vein Thrombosis Can Aggravate Esophageal Variceal Bleeding After Endoscopic Variceal Ligation

Abstract:Background: We aimed to evaluate whether portal vein thrombosis (PVT) and other factors influence esophageal variceal bleeding (EVB) after endoscopic variceal ligation (EVL) for liver cirrhosis patients. Methods: We retrospectively enrolled 255 patients with EVL in our hospital and parameters of age, gender, blood platelet, albumin, total bilirubin (TBil), D-dimer, activated partial thromboplastin time (APTT), prothrombin time (PT), Child-Pugh classification, portal vein thrombosis (PVT) and splenectomy were collected. Results: EVB patients had higher PVT diagnosed rate than non-EVB patients (P=0.009) and the rate of EVB had an increasing trend with the severity of ascites (P=0.034). On multivariate analysis, we still found PVT and ascites obviously affected incidence rate of EVB (P=0.026, P=0.027, respectively). Conclusion: PVT and ascites per se appeared to be the cause of EVB subsequent to EVL. If detected, anticoagulation or treatments refer to PVT and ascites needed to conducted in order to reduce the incidence of EVB after EVL.

Abstract:Abstract Background Portal hypertension is a major complication resulting from obstruction of portal blood flow, like cirrhosis or portal vein thrombosis, that leads to portal hypertension. MDCT angiography has become an important tool for investigation of the liver as well as potentially challenging varices by detailing the course of these tortuous vessels. This information is decisive for liver transplantation as well as for common procedures in which an unexpected varix can cause significant bleeding. Results This study included an assessment of 60 cases of portal hypertension (28 males and 32 females), their age ranged from 42 to 69 years (mean age = 57.2 ± 6.63). All patients were diagnosed with portal hypertension, underwent upper GI endoscopy followed by a triphasic CT scan with CT angiographic assessment for the screening of gastro-esophageal varices. CT is highly sensitive as compared to upper GI endoscopy (sensitivity 93%) in detecting esophageal varices. Gastric varices detected by CT in 22 patients (37%) compared to 14 patients (23%) detected by endoscopy. While paraesophageal varices were detected in 63% of patients and retro-gastric varices in 80% of patients that were not visualized by endoscopy. Our study reported that the commonest type of collaterals were the splenic collaterals, and we also found there is a significant correlation between the portal vein diameter and the number of collaterals as well as between the portal vein diameter and splenic vein diameter. Conclusions Multi-slice CT serves as an important non-invasive imaging modality in the diagnosis of collaterals in cases of portal hypertension. CT portography can replace endoscopy in the detection of high-risk varices. It also proved that there is a correlation between portal vein diameter, splenic vein diameter, and number of collaterals.

Abstract:Obliterative portal venopathy (OPV) is a rare cause of portal hypertension in the absence of cirrhosis. It involves a pathologic obliteration of the intrahepatic portal venous branches, leading to increased splanchnic blood flow. It typically manifests with variceal bleeding in young patients without evidence of liver insufficiency or cirrhosis. Differentiation from cirrhosis is paramount since major differences exist in managing both pathologies. OPV is diagnosed by liver biopsy. Since it is a rare condition, at times, it can be wrongly categorized as cryptogenic cirrhosis. There is no established cause of OPV, although it has been associated with HIV infection, Schistosomiasis, prothrombotic states, and medications like Didanosine and Azathioprine.

Abstract:Ectopic varices account to about 5% of all variceal bleeding with a mortality rate of 3%-4%. A 43-year-old man presented to the emergency room (ER) with diarrhea for 2 months and intermittent bleeding. Labs significant for hemoglobin 2.9 g/dl, MCV 71.8 fl. Colonoscopy showed a partially obstructing fungating, friable mass in the sigmoid colon. Biopsies showed adenocarcinoma. CT abdomen showed innumerable lesions in the liver and biopsies consistent with adenocarcinoma. He was treated with chemotherapy but lost to follow up. He presented to the emergency room about 5 months after with a large bowel obstruction and a colostomy was performed. Two months later imaging showing progression of hepatic metastatic disease, retroperitoneal and pulmonary metastases. He then presented to the ER with melena through the colostomy. Upper endoscopy and colonoscopy through the ostomy site both of which were negative for any active signs of bleeding or any etiology identified for bleeding. He had pancytopenia. Ultrasound of the liver noted patent portal vein and hepatopetal flow. CT showing sequalae of portal hypertension including splenomegaly, ascites with gastroesophageal and large ectopic peristomal varices. Bleeding recurred and despite careful endoscopic inspection, varices were not visible near the ostomy. Given clinical concern for peristomal variceal bleeding, he had an intervention radiology (IR) guided percutaneous plug and coil assisted antegrade transhepatic obliteration (PAATO and CAATO) of 3 different feeding vessels that was successful in preventing further bleeding. After discharge, he was continued on chemotherapy. No bleeding noted since.

Abstract:Background. Variceal esophageal-gastric bleeding is considered to be a life-threatening complication of portal hypertension in patients with cirrhosis. In some cases, only portosystemic shunt can serve as a life-saving intervention for the patient. Aim. To justify the forced expediency of transjugular intrahepatic portosystemic shunt (TIPS) in case of ineffective drug and endoscopic hemostasis or a high risk of early recurrence of variceal bleeding. Methods. A single-center observational controlled study was conducted to analyze the results of shunt procedure in 62 patients during the period of 2017–2023. The patients were divided into 2 groups: the main group ( n = 32) with patients who underwent “early” shunt procedure in a “salvage” variant ( n = 10) with continued bleeding and in a “pre-emptive” variant ( n = 22) with a high risk of early recurrence of hemorrhage, and the control group ( n = 30) with patients who underwent planned shunt procedure. The value of the portosystemic pressure gradient was calculated by subtracting the value of the pressure in the inferior vena cava recorded at the initial stage of the operation from the value of the initial pressure in the portal vein, measured by direct manometry. The authors compared the dynamics of the portosystemic pressure gradient in the study groups at similar stages of the intervention — initial, after embolization of the veins of portal blood flow to the varix, after shunt stenting. The study involved the analysis of mortality rates (6-week, one-year, for the entire observation period) and complications. Statistica-for-Windows 12.0 (StatSoft®, USA) and Excel (Microsoft, USA) were used to calculate descriptive statistics. Results. Patients of both groups achieved normalization of pressure in the portal vein system and, accordingly, the portosystemic pressure gradient as a result of shunt surgery. Effective portal decompression was confirmed by their significant reduction. In the portal vein, the pressure decreased from 33.84 ± 2.70 to 20.53 ± 1.27 mmHg ( t = 4.46; p < 0.001) in the main group and from 32.80 ± 3.07 to 20.10 ± 1.60 mmHg ( t = 3.67; p < 0.001) in the control group. The dynamics of the portosystemic pressure gradient showed a significant decrease from 26.16 ± 2.69 to 10.06 ± 0.88 mm Hg ( t = 5.69; p < 0.001) in the main group, and from 24.83 ± 2.73 to 9.67 ± 1.21 mm Hg ( t = 5.08; p < 0.001) in the control group. Together with embolization of the vessels of the hepatofugal inflow of portal blood to the varices, this led to a stable and long-term cessation of variceal bleeding. When comparing the results of shunt procedure in the study groups, no significant differences were found in terms of differentiated mortality rates and complications in both the early and long-term periods. In the main group, the 6-week and one-year mortality rates accounted for 6.3% ( n = 2) and 15.6% ( n = 5), in the control group — 6.7% ( n = 2) and 13.3% ( n = 4), respectively ( p = 0.917). Mortality in the main group comprised 25.0% ( n = 8) over 58 months, in the control group — 23.3% ( n = 7) over 60 months ( p = 0.886). Conclusion. The transjugular shunt provides a significant reduction in the portosystemic pressure gradient. “Salvage” and “pre-emptive” shunt options can prevent death and early recurrence of bleeding. Their effectiveness increases due to endovascular blockage of vessels that ensures the overflow of esophagogastric varices with portal blood.
Splenectomy And Selectively Devascularization For Esophageal Bleeding Secondary To Portal Hypertension

Abstract:Aim: Portal hypertension is a common disease with high mortality and poor prognosis, especially in the cases of bleeding due to portal hypertension, and secondary hypersplenism. Herein, we report a case-series of 6 patients who had bleeding from esophagus varices due to complicated portal hypertension and subsequently underwent splenectomy and selectively devascularization. Our objective was to evaluate the effects of this technique on the treatment of acute recurrent esophageal bleeding secondary to portal hypertension. Methods: We retrospectively analyzed the results of 6 patients with portal hypertension who underwent splenectomy and selectively devascularization from Jan 2010 to Mar 2014. The patients had undergone repeated endoscopic hemostatic treatment, transjugular intrahepatic portosystemic shunt (TIPS), liver transplantation or portal-vena cava shunt operation prior to splenectomy and selectively devascularization. Results: All the patients had splenectomy and selectively devascularization. The operative mortality for this procedure was null. Esophagus varices bleeding were controlled after operation in all patients. After a median follow-up of 17 months (range 3-36 months), one patient died of liver failure after 8 months; one patient had re-bleeding after 6 months and was easily controlled by medical treatment. For liver function improvement, 4 patients improved from preoperative Child class C to B. The other 2 patients improved to Child class A after surgery. Five patients had gastroscopy 6 months post surgery, and gastro-esophageal varices disappeared in 3 patients. Conclusion: Our results suggest the efficacy of splenectomy and selectively devascularization in the treatment esophagus varices bleeding due to complicated portal hypertension.

Abstract:Background. Portal vein (PV) thrombosis (PVT) is a common complication of liver cirrhosis and can refer to thrombosis within the PV that can extend to its left or right branches and in some cases to the superior mesenteric vein or the splenic vein (Chawla and Bodh, 2015). For severe PVT patients, there are possibilities of increasing PV resistance and reduction of the blood flow though PV towards liver, which exacerbate liver function damage meanwhile elevating the gastrointestinal variceal bleeding risk. Endoscopic Variceal band ligation (EVL) is often used to prevent esophageal variceal bleeding; postoperative complications such as severe gastrointestinal bleeding and bleeding-related death, fever, retrosternal pain, and esophageal stenosis may appear. There was absence of the research which evaluated the impact of PVT in liver cirrhosis on the complication of endoscopic Variceal band ligation for now. We herein aimed to compare cirrhosis patients with and without PVT of recent complications after EVL. Method. We established the retrospective investigation on 144 consecutive cirrhosis patients (excluding patients with hepatocellular carcinoma and who received portal vein-systemic circulation devascularization or shunt surgery, splenectomy, hepatectomy, liver transplantation, transjugular intrahepatic portal vein stent shunt (TIPS), endoscopic varices Variceal ligation, or sclerotherapy before) who have received first endoscopic esophageal varices band ligation in Gastrointestinal Endoscopy Center of the First Affiliated Hospital, College of Medicine, ZheJiang University, between January 2014 and December 2017. Portal vein Doppler ultrasonography, liver computerized tomography (CT), and angiography or liver-enhanced magnetic resonance imaging (MRI) were applied to evaluate the portal vein thrombosis of each patient before EVL. There were 18 patients confirmed with portal vein thrombosis while the other 126 patients without PVT. The primary end point for this research is the upper gastrointestinal hemorrhage and related death occurred from the date of ligation until leaving hospital, and the secondary end point is the appearance of postoperative fever and retrosternal pain. Results. There are no significant differences of gastrointestinal bleeding, bleeding-related death, fever, or retrosternal pain after EVL and the length of hospital stays between cirrhotic patients with or without PVT (P=0.34,0.51,0.58,0.61,0.88). Conclusion. Liver cirrhosis with portal vein thrombosis did not increase incidence of recent complications of the endoscopic Variceal band ligation.
Extrahepatic Portal Vein Thrombosis, an Important Cause of Portal Hypertension in Children

Abstract:One of the most important causes of portal hypertension among children is extrahepatic portal vein thrombosis (EHPVT). The most common risk factors for EHPVT are neonatal umbilical vein catheterization, transfusions, bacterial infections, dehydration, and thrombophilia. Our study aimed to describe the clinical manifestations, treatment, evolution, and risk factors of children with EHPVT. Methods: We analyzed retrospectively all children admitted and followed in our hospital with EHPVT between January 2011–December 2020. The diagnosis was made by ultrasound or contrast magnetic resonance imaging. We evaluated the onset symptoms, complications, therapeutic methods, and risk factors. Results: A total of 63 children, mean age 5.14 ± 4.90 (33 boys, 52.38%), were evaluated for EHPVT during the study period. The first symptoms were upper gastrointestinal bleeding (31 children, 49.21%) and splenomegaly (22 children, 34.92%). Thrombocytopenia was present in 44 children (69.84%). The most frequent risk factors were umbilical vein catheterization (46 children, 73.02%) and bacterial infections during the neonatal period (30 children, 47.62%). Protein C, protein S, antithrombin III levels were decreased in 44 of the 48 patients tested. In 42 of these cases, mutations for thrombophilia were tested, and 37 were positive. Upper digestive endoscopy was performed in all cases, revealing esophageal varices in 56 children (88.89%). All children with gastrointestinal bleeding received an octreotide infusion. In 26 children (41.27%), variceal ligation was performed, and in 5 children (7.94%), sclerotherapy. Porto-systemic shunt was performed in 11 children (17.46%), and Meso-Rex shunt was done in 4 children (6.35%). The evolution was favorable in 62 cases (98.41%). Only one child died secondary to severe sepsis. Conclusions: EHPVT is frequently diagnosed in the last period in our region due to the increased use of umbilical vein catheterization. Furthermore, genetic predisposition, neonatal bacterial infections, and prematurity certainly play an important role in this condition. A proactive ultrasound assessment of children with risk factors for EHPVT should be encouraged for early diagnosis and treatment.

Abstract:Introduction: Portal hypertensive enteropathy (PHE) is a well-known small-bowel lesion that occurs with liver cirrhosis (LC) and is associated with increased hepatic venous pressure. However, it is unclear how esophageal varices (EV) treatment actually affects PHE. We aimed to analyze the effects of EV treatment on PHE. Methods: Among LC patients who underwent capsule endoscopy (CE) more than twice to confirm PHE at our hospital from February 2009 to September 2018, we targeted those with no change in the Child-Pugh classification after EV treatment for the prevention of bleeding. Patients were assigned to groups based on the EV treatment method (endoscopic injection sclerotherapy [EIS] group or endoscopic variceal ligation [EVL] group). We analyzed the PHE findings before and after treatment, and we investigated changes in the collateral vein using computed tomography imaging. Results: We analyzed 42 LC patients (27 men; mean age, 65.3 years); 20 were in the EIS group and 22 were in the EVL group. Exacerbation of PHE was significantly prevalent in the EIS group (90%) compared to that in the EVL group (50%). CE of PHE showed significantly prevalent exacerbation of villous edema in the EIS group (65%) compared to that in the EVL group (23%). Stenosis or disappearance of the feeding vessel (55%) and exacerbation of the hepatofugal collateral vein (35%) were significantly prevalent in the EIS group. Conclusions: EIS exacerbates PHE, especially villous edema, in LC patients with changes in collateral circulation.Digestion

Abstract:Fitness and rates of extinction were compared among populations of the housefly, Musca domestica L., kept either at constant effective sizes of 50, 500 or 1500 or passed through extreme founder events reducing effective size to 5. Populations were maintained for 24 generations, which for small to medium‐sized mammals would be less than the 200 years suggested by Souléet al. (1986) as necessary for maintaining viable populations of endangered species. The results demonstrate that effective population sizes have to be greater than the 50 individuals suggested by Franklin (1980) to retain fitness and escape extinction, even in the short term. In contrast to populations of constant size that exhibited monotonic decreases in fitness through time, populations established with few founders rebounded from initial inbreeding depression. However, they were less adaptable to environmental stress than constant size populations, suggesting that populations founded with few numbers may do well within a single environment but may do far less well if they are reintroduced to natural environments or exposed to rapid environmental changes.

Abstract:Left-sided portal hypertension (LSPH) is a special type of extrahepatic portal hypertension, which is relatively rare in clinical practice and accounts for about 5% of all portal hypertension. [ 1 ] Among them, bleeding from gastric varices is the most serious complication, posing a serious threat to the patient life. At present, the main treatment method for LSPH is splenectomy. Surgical excision, however, is an invasive procedure that requires general anesthesia and a relatively long hospital stay. Splenic venoplasty, as a minimally invasive interventional treatment method, can restore normal vascular anatomy while preserving spleen function, making it an alternative method for splenectomy. [ 2 ] However, the success rate of splenic venoplasty is relatively low, especially for some cases of long segment splenic vein occlusion caused by chronic pancreatitis. It is difficult to use only a single portal vein, jugular vein, or splenic vein approach for recanalization. In this study, we report a cases of LSPH with gastric varices in which percutaneous intervention with a combined transhepatic and transsplenic approach was successful.

Abstract:Background: Sinistral, or left-sided, portal hypertension (SPH) is a rare cause of upper gastrointestinal (GI) hemorrhage resulting from obstruction of the splenic vein. Venous drainage from the spleen via collaterals can result in venous hemorrhage into both the retroperitoneal and intra-abdominal spaces due to increased venous blood pressure in peripancreatic and gastroduodenal vasculature. SPH can occur secondary to pancreatitis with thrombosis of the splenic vein. Another possible cause is the surgical ligation of the splenic vein as part of pancreaticoduodenectomy (PD). Although splenectomy has been traditionally considered as the treatment of choice to relieve venous hypertension, individual concepts for each patient have to be developed. Considering the venous collateral drainage pathways, a comprehensive approach involving surgical, endoscopic, and interventional radiology interventions may be necessary to address the underlying cause of variceal bleeding. Among these approaches, splenic artery embolization (SAE) has demonstrated efficacy in mitigating the adverse effects associated with elevated venous outflow pressure. Summary: This review summarizes key imaging findings in SPH patients after PD and highlights the potential of minimally invasive embolization for curative treatment of variceal hemorrhage. Key Messages: (i) SPH is a potential consequence after major pancreas surgery. (ii) Collateral flow can lead to life-threatening abdominal bleeding. (iii) Depending on the origin and localization of the bleeding, a dedicated management is required, frequently involving interventional radiology techniques.

Abstract:Purpose This study aimed to assess the safety and efficacy of endovascular managements, including splenic vein recanalization (SVR), partial splenic embolization (PSE), and percutaneous transsplenic gastric varices embolization combined with PSE (PSE+GVE), for management of SPH-related variceal hemorrhage (VH). Methods A total of 61 patients with SPH-related VH from three hospitals were enrolled and classified into three groups: the SVR group (Group 1, n =24), the PSE+GVE group (Group 2, n =17), and the PSE group (Group 3, n =20). Baseline characteristics and clinical outcomes were compared among the groups. Results The technical success rates for transhepatic and transsplenic SVR were 27.8% and 34.6%, respectively. No major complications were observed during any of the procedures. The median follow-up period was 53.2 months. The 2-year GI rebleeding rates for Group 1, 2, and 3 were 0%, 5.9%, and 35%, respectively. Groups 1 and 2 have a lower GI rebleeding rate ( p = 0.002, p = 0.048, respectively) and better results of the degree of GV ( p = 0.003, p = 0.044, respectively) compared to Group 3. No significant differences were found in 2-year GI rebleeding rates and the degree of GV between Group 1 and 2 ( p = 0.415, p = 0.352, respectively). Conclusion SVR, PSE+GVE, and PSE seem safe and effective for management of SPH-related VH. SVR appears to be the superior treatment option. Transsplenic access may further increase the SVR success rate. PSE+GVE seems to have comparable outcomes in GV control and GI rebleeding rates compared to SVR, while superior to PSE.
Unusual case of left-sided portal hypertension induced variceal bleeding after pancreaticoduodenectomy treated with transplenic embolization

Abstract:With the advances in surgical technique and the accumulation of experiences, pancreatic cancer with portal-superior mesenteric vein (PV-SMV) invasion is no longer considered as an absolute contraindication for surgical resection. After resection of the PV-SMV confluence, congestion of the splenic vein (SV) may develop, resulting in splenomegaly and variceal formation, also known as left-sided portal hypertension (LPH). Along with improved postoperative prognosis, LPH induced varices are given enough time to develop and eventually bleed, which can be lethal. We present a 59-year-old woman who underwent pancreaticoduodenectomy (PD) for pancreatic cancer with a concomitant PV-SMV resection. Massive upper gastrointestinal bleeding and hypovolemic shock occurred 15 months after the surgery. Various exams, including endoscopy, dynamic computed tomography (CT) imaging, celiac, and superior mesenteric artery (SMA) angiography, were performed. However, the exact location of the bleeding could not be identified. LPH-induced varices bleeding was suspected and diagnosed by venography. The varices were embolized with n-BCA and lipiodol mixture by trans-splenic venous approach with complete cessation of bleeding. It is important to identify potential life-threatening LPH-induced varices bleeding, especially if certain clinical histories or classic imaging findings are presented. As for treatment, interventional radiology methods could be considered as the first choice.
