Abstract:Recent advancements in urology are transforming patient care by integrating artificial intelligence (AI), genomic insights, and novel therapeutic approaches. This review explores the significant strides made in the field, particularly the application of AI in enhancing diagnostic precision and treatment strategies. The role of genomics in identifying molecular targets and pathways has enabled the development of targeted and personalized treatments, revolutionizing the management of urological disorders. We highlight novel pharmacological and non-pharmacological treatments that have shown efficacy in improving patient outcomes. Additionally, advancements in diagnostic methodologies, including non-invasive tests and precision imaging, have facilitated early and accurate disease detection. Abstract:IntroductionUreteroscopy is now the most frequent treatment used around the world for stone disease. Technological advancement, efficiency, safety, and minimally invasiveness of this procedure are some of the reasons for this change of trend.Materials and methodsIn this review of the literature, a search of the PubMed database was conducted to identify articles related to ureteroscopy and accessories. The committee assigned by the International Consultation on Urological Disease reviewed all the data and produced a consensus statement relating to the ureteroscopy and all the particularities around this procedure.ConclusionThis manuscript provides literatures and recommendations for endourologists to keep them informed in regard to the preoperative, intraoperative, and postoperative consideration in regard of a ureteroscopy.

Abstract:In recent decades, the development of surgical systems which minimize patient impact has been a major focus for surgeons and researchers, leading to the advent of robotic systems for minimally invasive surgery. These technologies offer significant patient benefits, including enhanced outcome quality and accuracy, reduced invasiveness, lower blood loss, decreased postoperative pain, diminished infection risk, and shorter hospitalization and recovery times. Surgeons benefit from the elimination of human tremor, ergonomic advantages, improved vision systems, better access to challenging anatomical areas, and magnified 3DHD visualization of the operating field. Since 2000, Intuitive Surgical has developed multiple generations of master-slave multi-arm robots, securing over 7000 patents, which created significant barriers for competitors. This monopoly resulted in the widespread adoption of their technology, now used in over 11 million surgeries globally. With the expiration of key patents, new robotic platforms featuring innovative designs, such as modular systems, are emerging. This review examines advancements in robotic surgery within the fields of general, urological, and gynecological surgery. The objective is to analyze the current robotic surgical platforms, their technological progress, and their impact on surgical practices. By examining these platforms, this review provides insights into their development, potential benefits, and future directions in robotic-assisted surgery.

Abstract:The aim of the present review is to summarize recent developments in the field of urologic robotic surgery. A nonsystematic literature review was performed to retrieve publications related to robotic surgery in urology and evidence-based critical analysis was conducted by focusing on the literature of the past 5 years. The use of the da Vinci Surgical System, a robotic surgical system, has been implemented for the entire spectrum of extirpative and reconstructive laparoscopic kidney procedures. The robotic approach can be applied for a range of adrenal indications as well as for ureteral diseases, including benign and malignant conditions affecting the proximal, mid, and distal ureter. Current evidence suggests that robotic prostatectomy is associated with less blood loss compared with the open surgery. Besides prostate cancer, robotics has been used for simple prostatectomy in patients with symptomatic benign prostatic hyperplasia. Recent studies suggest that minimally invasive radical cystectomy provides encouraging oncologic outcomes mirroring those reported for open surgery. In recent years, the evolution of robotic surgery has enabled urologic surgeons to perform urinary diversions intracorporeally. Robotic vasectomy reversal and several other robotic andrological applications are being explored. In summary, robotic-assisted surgery is an emerging and safe technology for most urologic operations. The acceptance of robotic prostatectomy during the past decade has paved the way for urologists to explore the entire spectrum of extirpative and reconstructive urologic procedures. Cost remains a significant issue that could be solved by wider dissemination of the technology.

Translational Andrology and Urology

Abstract:Research on upper tract urothelial carcinoma (UTUC) has become a point of emphasis in the uro-oncologic community as of late. Over the past several decades, surgical and medical innovation has transformed the diagnosis and treatment of UTUC. In fact, survival rates have increased thanks to myriad of reasons including (I) technological advances in surgical technique, (II) focus on nephron sparing-surgery where applicable, and (III) multi-disciplinary management by using chemotherapy in the neoadjuvant or adjuvant setting. Despite the low incidence and prevalence of this disease, data from recent high-quality publications and randomized clinical trials has rapidly transformed clinical practice. This special series of terrific and well-written review articles authored by some of the leaders and experts in the field captures the many advancements and innovations in UTUC. Newer advances in ureteroscopy, specifically from fiber-optic to digital ureteroscopes, has led to earlier and more seamless detection of upper tract tumors allowing high quality biopsies. The European Association of Urology (EAU) and American Urological Association (AUA) have updated guidelines based on the latest evidence-based medicine. Advances in cross sectional imaging along with cytological and histopathological diagnosis help in proper risk-stratification, which is critical when considering treatment options. Speaking of treatments, advances in drug development as well as surgical innovation has allowed the possibility of nephron-sparing surgery. Where once cancer in the upper tract demanded removal the entire healthy kidney and ureter, newer techniques of distal ureterectomy, segmental ureterectomy, and ileal interposition has preserved renal function and limited the development of chronic kidney disease. Chemoablation of the upper tract using UGN-101 in select patients has emerged as a therapeutic alternative to nephroureterectomy for select indications. Moreover, the development of novel laser therapies such as holmium/yttrium-aluminum-garnet (YAG), neodymium/YAG, and thulium/YAG has also resulted in ability to preserve renal function. Additional topics found in this series are technique based with a step-by-step overview of robotic nephroureterectomy (considered as the gold standard) and a separate article on how to best tackle the distal ureter and bladder cuff—thought to be the most technically challenging part of the surgery. With the robotic approach becoming more en vogue , one topic explores the role, if any, of open surgery in the management of UTUC. Regardless of approach, sound oncologic principles should be followed when managing patients with UTUC. Three separate review articles explore the oncologic outcomes following radical nephroureterectomy including the role of adjuvant/neoadjuvant chemotherapy and timing/evidence of intravesical therapy at the time of surgery. Editing this series has led us to take a step back and marvel upon the remarkable growth of evidence-based research and practice-changing guidelines in the field of UTUC. Review of these high-yield topics can help our readers achieve a thorough understanding of this rare but impactful cancer. Despite advances in management of upper tract urothelial cancer over three decades, work is still in progress to help our patients. We are excited to see how artificial intelligence can transform the uro-oncology space by predicting prognostic patterns, confirming variant histologies using digital pathologies, and predict treatment survival based on preoperative characteristics. Obtaining an accurate diagnosis ureteroscopically can be challenging and perhaps computed tomography (CT) urogram findings coupled with patient-specific patterns can help predict likelihood of high-risk malignancy. A more personalized and patient-centered treatment plan may be achieved using specific molecular footprints that are yet to be discovered. We are excited to learn of the future directions in UTUC diagnosis and management. Abstract:This review discusses recent advances in robotic surgery for urologic tumors, focusing on three key areas: robotic systems, assistive technologies, and artificial intelligence. The Da Vinci SP system has enhanced the minimally invasive nature of robotic surgeries, while the Senhance system offers advantages such as tactile feedback and eye-tracking capabilities. Technologies like 3D reconstruction combined with augmented reality and fluorescence imaging aid surgeons in precisely identifying the anatomical relationships between tumors and surrounding structures, improving surgical efficiency and outcomes. Additionally, the development of artificial intelligence lays the groundwork for automated robotics. As these technologies continue to evolve, we are entering an era of minimally invasive, precise, and intelligent robotic surgery.

Abstract:In nearly three decades to the direction of the journal Annali Italiani di Chirurgia I could add to my personal experiences as surgeon and University Professor of Surgery, those very special in the rather different field of scientific literature from the specific point of view of Chief Editor - English term that identifies the "Director" of a scientific journal. The original wish to enhance and promote the qualities of researches and professional activity of Italian surgery within the journal, after an initial phase of normal management has expanded ambitiously, and over the last decade has been taken new editorial strategic decisions to promote more and more the cultural value and the diffusion of the magazine, for the benefit of its subscribers and its readers. Therefore, since 2007 the preferential adoption of English was introduced, then it became the exclusive language from 2010. Later in 2008 there was the authoritative introduction of the magazine in the "informatics network" with the activation of a "web site" to constitute the parallel Digital Edition of Annali Italiani di Chirurgia, ISSN credited with the CNR, to constitute a large interactive library where there are nowadays collected in free reading all articles published in press from 2000 up to those published in the first half of 2015 - with subsequent to follow. In parallel it was inaugurated the section reserved to the individual "Clinical cases", published exclusively online in the same Digital Edition, with preservation of iconography colors. The own search motor of the "site" (www.annitalchir.com) enables the identification of all articles published in the Digital Edition, whether published in print or exclusively online, proceeding by title, by author, by keyword, and of course by date of publication. The functionality of the purchase mechanism of each article with the procedure of "e-commerce" right from the site and the possibility for all readers to interactively express the own evaluation of published articles, has transformed the site into a "portal" constantly updated, and this has led to the growing number of foreign authors asking access with their papers to the magazine, especially from the Mediterranean area and Eastern Europe. Considering the variety and quality of the studies performed not only in Italy but also in such a vast geographical and cultural area, evident in the articles already published, it seems appropriate not only the substantial analysis by the Scientific Committee of the articles received to be published, but also to underline the care of the formal aspects of those proposed to the press, and to express a series of suggestions, to urge the Authors to improve the formal aspects in the preparation and exposure for their articles, to benefit of their image of Authors and for prestige of the magazine in an international perspective. This is the meaning of the Leading Article of this first issue 2016. I conclude by listing the qualitative preferences adopted for accepting articles to be published, provided that the individual clinical cases - the surgical "chronicle" - have a privileged place in the Digital Edition of Annali Italiani di Chirurgia, preserving the colors of the iconography, but that only exceptionally will be published in the printed issues. Preoperative patient optimization for endourological procedures: the current best clinical practice

Abstract:Purpose of review: Despite technological advancements in endourological surgery, there is room for improvement in preoperative patient optimization strategies. This review updates recent best clinical practices that can be implemented for optimal surgical outcomes. Recent findings: Outcome and complication predictions using novel scoring systems and techniques have shown to assist clinical decision-making and patient counseling. Innovative preoperative simulation and localization methods for percutaneous nephrolithotomy have been evaluated to minimize puncture-associated adverse events. Novel antibiotic prophylaxis strategies and further recognition of risk factors that attribute to postoperative infections have shown the potential to minimize perioperative morbidity. Accumulating data on the roles of preoperative stenting and selective oral alpha-blockers adds evidence to the current paradigm of preventive measures for ureteral injury. Summary: Ample tools and technologies exist that can be utilized preoperatively to improve surgical outcomes. The combination of these innovations, along with validation in larger-scale studies, presents the cornerstone of future urolithiasis management.

Abstract:<h3 class="Heading">Context</h3><p class="Para">Despite the current era of precision surgery in robotics, an unmet need still remains for optimal surgical planning and navigation for most genitourinary diseases. 3D virtual reconstruction of 2D cross-sectional imaging has been increasingly adopted to help surgeons better understand the surgical anatomy.</p><h3 class="Heading">Objectives</h3><p class="Para">To provide a short overview of the most recent evidence on current applications of 3D imaging in robotic urologic surgery.</p><h3 class="Heading">Evidence acquisition</h3><p class="Para">A non-systematic review of the literature was performed. Medline, PubMed, the Cochrane Database and Embase were screened for studies regarding the use of 3D models in robotic urology.</p><h3 class="Heading">Evidence synthesis</h3><p class="Para">3D reconstruction technology creates 3D virtual and printed models that first appeared in urology to aid surgical planning and intraoperative navigation, especially in the treatment of oncological diseases of the prostate and kidneys. The latest revolution in the field involves models overlapping onto the real anatomy and performing augmented reality procedures.</p><h3 class="Heading">Conclusion</h3><p class="Para">3D virtual/printing technology has entered daily practice in some tertiary centres, especially for the management of urological tumours. The 3D models can be virtual or printed, and can help the surgeon in surgical planning, physician education and training, and patient counselling. Moreover, integration of robotic platforms with the 3D models and the possibility of performing augmented reality surgeries increase the surgeon's confidence with the pathology, with potential benefits in precision and tailoring of the procedures.</p>

Abstract:The field of genitourinary prosthetics has evolved rapidly to become the standard of care for conditions such as incontinence and refractory erectile dysfunction. Its scope has expanded to encompass newer indications such as Peyronie's disease and gender-affirming surgeries. This review, based on the Urological Society of India's Best Essay Award 2022, aims to elaborate on the advances in the field of urological prosthetics in the past 20 years as well as to provide an insight into ongoing research and what one can expect to see in the next decade, particularly in the area of penile and testicular prosthetics as well as treatment of incontinence. A PubMed and patent search was performed to achieve these objectives. Future considerations include improving acceptance, reliability, making them more accessible for developing countries and improving training and education to improve outcomes.

Abstract:Enhanced Recovery After Surgery (ERAS) constitutes the application of a series of perioperative measures based on the evidence, in order to achieve a better recovery of the patient and a decrease of the complications and the mortality. These ERAS programs initially proved their advantages in the field of colorectal surgery being progressively adopted by other surgical areas within the general surgery and other surgical specialties. The main excluding factor for the application of such programs has been the urgent clinical presentation, which has caused that despite the large volume of existing literature on ERAS in elective surgery, there are few studies that have investigated the effectiveness of these programs in surgical patients in emergencies. The aim of this article is to show ERAS measures currently available according to the existing evidence for emergency surgery.
Endourological society fellowship program: a real contribution to improve surgical outcomes of stone patients treated with ureteroscopy

Abstract:Purpose: We assessed the impact of the one-year endourological society fellowship program (ESFP) on the achievement of optimal surgical outcomes (OSO) in stone patients treated with ureteroscopy (URS). Materials and methods: We identified 303 stone patients treated with URS from January 2018 to June 2022 by five experienced surgeons (ES). Of those, two attended ESFP. OSO was defined as the presence of residual fragments ≤ 4 mm at 1-month post-operative imaging (Ultrasound + X - Ray or CT scan). Descriptive statistics explored patients' characteristics and the rate of OSO according to the attendance of ESFP. Uni- and multivariate logistic regression analyses (UVA and MVA) tested the impact of stone characteristics, surgical data, ESFP, and imaging technique on the rate of OSO. The LOWESS curve explored the graphical association between stone size and the multivariable-adjusted probability of OSO in the two groups of surgeons. Results: Of 303 patients, 208 (69%) were treated by the two surgeons who attended ESFP. OSO was achieved in 66% and 52% of patients treated by ES with and without ESFP, respectively (p = 0.01). At UVA, ESFP (OR = 1.78; 95% CI = 1.09-2.90), stone diameter (OR = 0.92; 95% CI = 0.88-0.96), stone location (kidney vs. ureter; OR = 0.34; 95% CI = 0.21-0.58), imaging technique (CT scan vs. Ultrasound + X-Ray; OR = 0.28; 95% CI = 0.16-0.47) predicted OSO achievement (all p < 0.05). At MVA analyses, ESFP was associated with OSO (OR = 2.24; 95% CI = 1.29-3.88; p < 0.05), along with the other aforementioned variables. The LOWESS curve showed that the greater the stone size, the greater the difference in OSO in the two groups of surgeons. Conclusions: ESFP positively affects OSO achievement after URS, especially in patients with a high stone burden.

Abstract:The new scientific developments and technological opportunities that have led to significant changes all build up the digital era. In medicine, the use of new technologies in patient diagnosis and treatment processes has opened new horizons for physicians and patients. As considering for the medical training, 3-dimensional modeling opportunities, virtual reality, augmented reality, and various simulators offered by the new technologies of the digital era have become a new hope. The 3-dimensional scanning and modeling, 3-dimensional medical printing, virtual reality technologies applications and simulators in urology are very recent and valuable. Besides, the exoscope-assisted 3-dimensional open surgery provides high-resolution 3-dimensional images to surgeons with high comfort as compared with old-fashioned operating microscopes. New modalities that tried to be integrated in robotic surgery are 3-dimensional reconstruction, usage of indocyanine green, augmented reality, contrast ultrasound, haptic feedback, and availability of single port. Some new companies announced their new robotic systems in the market. The use of these new technological applications during medical training, especially at the beginning of the education curve for various surgical interventions, may be beneficial in terms of reducing possible complications that may be encountered due to inexperience at the beginning of the education process and increasing patient safety. Urology will also stay at the futuristic approach in medicine, while 3-dimensional technologies used more widely in this field.
