Abstract:Objective: To evaluate the diagnostic value of the contrast enhanced ultrasound (CEUS) in the thyroid nodule less than 0.5 cm with an initial inconclusive diagnostic fine needle aspiration cytology (FNA). Methods: A retrospective analysis of the clinical data of 61 patients was performed, including 63 thyroid nodules with L<0.5 cm. All the thyroid nodules had an initial inconclusive diagnostic FNA and undergone CEUS, a repeat FNA after 3 months and finally confirmed by pathology. All the imaging data of preoperative FNAs and CEUS were collected. Pursuant to which, the detection rate of malignant nodules (DROMN), the detection rate of begin nodules (DROBN), the false negative rate (FNR), the false positive rate (FPR), the accuracy of diagnosis, the per-sistently inconclusive diagnostic rate (PIDR) and the malignant proportion by repeat FNA and CEUS were ana-lyzed respectively. Results: In CEUS group, the DROMN, DROBN, FNR, FPR, the accuracy of diagnosis, PIDR and the malignant rate were 84.8%, 36.7%, 15.3%, 12.5%, 86.7%, 28.5% and 16.7% respectively. In the repeat FNA group, DROMN, DROBN, FNR, FPR, the accuracy of diagnosis, PIDR and the malignant proportion were 24.2%, 33.3%, 41.2%, 0.0%, 72.0%, 60.3%, 47.4%. The DROMN, PIDR and the malignant rate in the group CEUS were statistically higher than the repeat FNA group (P<0.05). Conclusion: CEUS has a better diagnostic efficiency in the thyroid nodule less than 0.5 cm with an initial inconclusive diagnostic FNA than the repeat FNA. The non-invasive CEUS examination can be suggested.

Abstract:Ultrasound often represents the first diagnostic step for thyroid nodule evaluation in clinical practice, but baseline US alone is not always effective enough to achieve thyroid nodule characterization. In the last decades new ultrasound techniques, such as CEUS, have been introduced to evaluate thyroid parenchyma as recommended by EFSUMB guidelines, for use in clinical research field, although its role is not yet clear. Several papers show the potential utility of CEUS in the differential diagnosis of benign and malignant thyroid nodules and in the analysis of lymph node involvement in neoplastic pathology. Therefore, we carried out an evaluation of the literature concerning the role of CEUS in three specific areas: the characterization of the thyroid nodule, the evaluation of minimally invasive treatment and loco-regional staging of the lymph node in proven thyroid cancer. According to evidence reported, CEUS can also play an operative role in nodular thyroid pathology as it is able to guide ablation procedures on thyroid nodule and metastatic lymph nodes, to assess the radicality of surgery, to evaluate disease relapse at the level of the margins of ablated regions and to monitor the clinical evolution of necrotic areas in immediate post-treatment setting.
Utility of Quantitative Contrast-Enhanced Ultrasound for the Prediction of Lymph Node Metastasis in Patients with Papillary Thyroid Carcinoma

Abstract:OBJECTIVES: The aim of this study was to find the optimal parameters and cutoffs to differentiate metastatic lymph nodes (LNs) from benign LNs in the patients with papillary thyroid carcinoma (PTC) on the quantitative contrast-enhanced ultrasound (CEUS) features. METHODS: A total of 134 LNs in 105 patients with PTCs were retrospectively enrolled. All LNs were evaluated by conventional ultrasound (US) and CEUS before biopsy or surgery. The diagnostic efficacy of CEUS parameters was analyzed. RESULTS: Univariate analysis indicated that metastatic LNs more often manifested centripetal or asynchronous perfusion, hyper-enhancement, heterogeneous enhancement, ring-enhancing margins, higher PI, larger AUC, longer TTP and DT/2 than benign LNs at pre-operative CEUS (p < 0.001, for all). Multivariate analysis showed that centripetal or asynchronous perfusion (OR = 3.163; 95% CI, 1.721-5.812), hyper-enhancement(OR = 0.371; 95% CI, 0.150-0.917), DT/2 (OR = 7.408; 95% confidence interval CI, 1.496-36.673), and AUC (OR = 8.340; 95% CI, 2.677-25.984) were predictive for the presence of metastatic LNs. The sensitivity and accuracy of the quantitative CEUS were higher than qualitative CEUS (75% vs 55 % and 83.6% vs 76.1 %, respectively). CONCLUSIONS: Quantitative CEUS parameters can provide more information to distinguish metastatic from benign LNs in PTC patients; In particular, DT/2 and AUC have a higher sensitivity and accuracy in predicting the presence of metastatic LNs and reduce unnecessary sampling of benign LNs.
Evaluation of Thyroid Ultrasound Report Quality and Assessing Effect of Adherence to Risk Stratification Criteria on Referral for Thyroid Nodule Biopsy

Abstract:Purpose: This study aims to evaluate the quality of diagnostic thyroid ultrasound reports and determine the impact of consistent adherence to 2015 American Thyroid Association (ATA) and 2017 American College of Radiology (ACR) Thyroid Imaging Reporting and Data System (TI-RADS) on reducing unnecessary referrals for thyroid nodule biopsy. Materials and Methods: Reports from 291 referrals for thyroid nodule biopsy were included for retrospective report evaluation (males: 42; mean age: 56) according to 2015 ATA and ACR TI-RADS lexicon. Cytology results were collected for each patient. Two radiologists blinded to cytology results independently, retrospectively reviewed imaging of the referrals, and rescored them according to 2015 ATA and 2017 ACR TI-RADS risk stratification systems. Statistical analysis was completed using chi-square analysis and calculation of κ statistic for interobserver variability. Results: No report completely addressed all features associated with malignancy. Over half of the reports did not include descriptors on echogenicity, shape, margin, or echogenic foci. In all, 9.3% of biopsies showed malignant histology. Rescoring of referrals demonstrated decrease in biopsy referrals by 55% as per 2017 ACR TI-RADS and 14% as per 2015 ATA ( P < .0001). There was no impact on detection of malignancy with adherence to ATA or ACR criteria and less interobserver variability with application of 2017 ACR TI-RADS compared to 2015 ATA. Conclusion: Thyroid ultrasound report quality was found variable with respect to nodule description. Reports recommended biopsy based on nodule size with no detailed description of other imaging features. Adherence to risk stratification system would have resulted in significant reduction in the number of unnecessary biopsy referrals.
The diagnostic performance of neck ultrasound in follow-up of advanced stage differentiated thyroid cancer

Abstract:Background: Differentiated thyroid cancer (DTC) requires long-term follow-up due to the risk of delayed recurrence. Follow-up surveillance involves serial neck ultrasound (US) and thyroglobulin (Tg); however, the optimal frequency and diagnostic performance of neck US outside of specialized thyroid cancer centres in higher risk patients is not well defined. We sought to evaluate the diagnostic performance of US and serial Tg in advanced stage DTC. Methods: We retrospectively reviewed our thyroid cancer database for patients with stage III and IV DTC from 2006 to 2018, total thyroidectomy, and at least 2 years follow-up to assess recurrence rates. Those with hemi-thyroidectomy or anti-Tg antibodies were excluded. Diagnostic performance of US and Tg were assessed using a composite reference standard of follow-up imaging and pathology. All relevant US were reviewed by a blinded expert radiologist for uniformity. Results: Of 136 included patients (91 females, mean age 58.9), 26 (19%) had recurrence of DTC over median follow-up of 6.6 years (IQR 5.3-9.3). The sensitivity and specificity of US in diagnosing cervical recurrence were 73.3% (95% CI 0.51-0.96) and 68.3% (95% CI 0.60-0.77) based on historical reports, respectively, and 80% (95% CI 0.60-1.00) and 87.8% (95% CI 0.82-0.93) based on blinded expert review, respectively. Tg had a sensitivity of 95.5% (95% CI 0.89-1.0) and specificity of 96.2% (95% CI 0.92-0.99) in detecting cervical recurrence or distant metastases. False positive US findings on historical US and subsequent review occurred in 38 (28%) and 15 (11%) patients, respectively, while 5 (3.6%) had false positive Tg results. Conclusion: Serial Tg has better sensitivity and specificity than US for detecting recurrence of advanced stage DTC. Furthermore, re-interpretation of abnormal findings using structured US reporting with a subspecialized reader may improve diagnostic performance of US and improve its utility in clinical care.

Abstract:Purpose: The study aimed to assess the postoperative outcomes of pediatric thyroid nodules with Atypia of Undetermined Significance (AUS/FLUS) or Suspicious for a Follicular Neoplasm (SFN) and their EU-TIRADS scoring.Methods: The study retrospectively reviewed 44 patients at a single center with thyroid nodules classified as Atypia of Undetermined Significance or Suspicious for a Follicular Neoplasm from August 2019 to December 2022. Data on demographics, thyroid function, nodule size, and ultrasonographic features were collected. Postoperative pathologies were categorized into benign, low-risk, and malignant neoplasms according to WHO 2022 criteria, with EU-TIRADS used for radiologic scoring.Results: Among 21 pediatric patients, 72% had Bethesda 3 and 28% had Bethesda 4 thyroid nodules. Pathological outcomes post-surgery classified 43% as benign, 19% as low-risk, and 38% as malignant. Notably, EU-TIRADS 3 and 5 scores were present in 44% and 56% of benign cases, respectively. Malignant cases showed a prevalence of higher EU-TIRADS scores, with 64% rated as EU-TIRADS 5. Bethesda category 4 nodules had a 66% malignancy rate, significantly higher than the 27% in category 3.Conclusion: The investigation revealed that EU-TIRADS scoring showed a substantial proportion of benign cases were classified as EU-TIRADS 5, suggesting that EU-TIRADS may lead to unnecessary biopsies in benign cases. Malignant cases were more likely to have a higher EU-TIRADS score, indicating a positive correlation with malignancy risk, particularly in Bethesda 4 cases. However, the EU-TIRADS system's predictive value for malignancy in Bethesda 3 cases was less definitive.

Abstract:Objective: Gray-scale ultrasound (US) is the standard-of-care for evaluating thyroid nodules (TNs). However, the performance is better for the identification of hypoechoic malignant TNs (such as classic papillary thyroid cancer) than isoechoic malignant TNs. Quantitative ultrasound (QUS) utilizes information from raw ultrasonic radiofrequency (RF) echo signal to assess properties of tissue microarchitecture. The purpose of this study is to determine if QUS can improve the cancer risk stratification of isoechoic TNs. Methods: Patients scheduled for TN fine needle biopsy (FNB) were recruited from the Thyroid Health Clinic at Boston Medical Center. B-mode US and RF data (to generate QUS parameters) were collected in 274 TNs (163 isoechoic, 111 hypoechoic). A linear combination of QUS parameters (CQP) was trained and tested for isoechoic [CQP(i)] and hypoechoic [CQP(h)] TNs separately and compared with the performance of conventional B-mode US risk stratification systems. Results: CQP(i) produced an ROC AUC value of 0.937+/- 0.043 compared to a value of 0.717 +/- 0.145 (p >0.05) for the American College of Radiology Thyroid Imaging, Reporting and Data System (ACR TI-RADS) and 0.589 +/- 0.173 (p >0.05) for the American Thyroid Association (ATA) risk stratification system. In this study, CQP(i) avoids unnecessary FNBs in 73% of TNs compared to 55.8% and 11.8% when using ACR TI-RADS and ATA classification system. Conclusion: This data supports that a unique QUS-based classifier may be superior to conventional US stratification systems to evaluate isoechoic TNs for cancer and should be explored further in larger studies.

The Role of Ultrasonography in Monitoring Chemotherapeutic Effects for Primary Thyroid Lymphoma: A Single-center Retrospective Study

Abstract:Abstract Background: This study aimed to assess the utility of ultrasonography in monitoring the chemotherapeutic effects in primary thyroid lymphoma (PTL). Methods: This retrospective analysis included 17 patients with PTL who underwent chemotherapy from 2012–2022 (>10 years). Sonographic features were examined pre- and post-treatment using ultrasound to monitor treatment response at 2, 4, and end cycles of chemotherapy along with follow-up. Progression-free survival (PFS) and overall survival (OS) were analyzed. Results: Sonographic findings in all patients indicated diffuse or nodular infiltration with markedly hypoechoic masses. “Strip-shaped” high echoes and posterior acoustic enhancement were observed. In the diffuse group, complete response (CR) rates were 0%, 80%, and 80%, while partial response (PR) rates were 100%, 20%, and 0% at 2, 4, and end cycles post-chemotherapy, respectively. For the nodular group, CR rates were 0%, 20%, and 66.7%, with PR rates of 86.7%, 53.3%, and 13.3% at the same corresponding time points. After the end cycle of chemotherapy, the lesion sizes had significantly decreased compared to baseline (p <0.05). Post-chemotherapy, Adler’s blood flow grades decreased significantly, with 80% graded as 0–1. Among the 10 patients with cervical lymph node enlargement, 70% showed reduced lesion sizes and blood flow signals. The cumulative 5-year PFS and OS rates were 80% and 80% for the diffuse type, and 82.5% and 78.8% for the nodular type, respectively (p >0.05). Conclusions: Ultrasound examination can be used to monitor PTL treatment response post-chemotherapy, especially for efficacy evaluation after 2 cycles. Trial registration: retrospectively registered

Abstract:OBJECTIVES: To assess the yield of neck ultrasound (US) when serum thyroglobulin (Tg) is undetectable (<0.1 ng/mL) compared to elevated serum Tg in patients with differentiated papillary thyroid carcinoma (PTC) treated with thyroidectomy and radioactive iodine 131 (RAI) ablation.METHODS: A retrospective chart review was conducted from 2010 through 2015 at an academic institution evaluating US results in patients with serum Tg levels obtained within 6 months of a neck US examination after thyroidectomy and RAI. The reference standard for recurrence was pathologic results from US-guided fine-needle aspiration (FNA) or follow-up for at least 1 year.RESULTS: Among 76 patients with undetectable serum Tg levels, there were 19 examinations in 18 patients in which US raised the possibility of recurrence. None of these 18 patients had recurrence by FNA (n = 8) or clinical follow-up of at least 1 year (n = 10). Among 65 patients with elevated serum Tg levels, there were 24 examinations in 22 patients in which US raised the possibility of recurrence. Twelve patients underwent FNA, with 9 patients (34.6%) showing PTC; 7 patients had follow-up neck US examinations showing stability of findings; and 3 patients were lost to follow up. The yield of neck US was significantly lower when serum Tg was undetectable compared to when levels were elevated (P = .001).CONCLUSIONS: Neck US did not identify recurrent PTC when the serum Tg level was undetectable in patients who underwent total thyroidectomy and RAI therapy. Eliminating neck US when serum TG levels are undetectable could decrease unnecessary imaging examinations without negatively affecting the ability to detect recurrent disease.

Abstract:Background: Radiofrequency ablation is an effective minimally invasive technique for benign thyroid nodules, with European and Asian studies reporting improved quality of life following treatment. We aimed to assess the thyroid-related quality of life of patients with benign thyroid nodules treated with radiofrequency ablation in the United States. Methods: This is a prospective single-institution study of patients treated with radiofrequency ablation over a 4-year period. Nodule characteristics and Thyroid-Related Patient-Reported Outcome (ThyPRO)-39 scores were assessed at baseline, 3 months post-procedure, and last follow-up (range: 3-12 months). Wilcoxon signed-rank test was used to assess differences in scores before and after radiofrequency ablation with standardized effect size analysis. Results: A total of 56 patients with 76 nodules were treated. The median volume reduction ratio at 1, 3, 6, and 12 months was 44.9% (interquartile range: 27.3-57.3), 65.2% (53.5-73.1), 69.7% (63.5-81.9), and 79.6% (66.2-88.9), respectively. At 3-month follow-up, significant improvement (P < .05) was observed for goiter symptoms (28.9 vs 10.8), anxiety (30.7 vs 19.3), and appearance (29.9 vs 10.8). At last follow-up, significant improvement (P < .05) was observed for goiter symptoms (28.9 vs 8.9), anxiety (30.7 vs 22.3), appearance (29.9 vs 10.8), and overall quality of life (30.3 vs 11.1). Effect sizes were moderate for appearance (standardized effect size 0.5-0.8) at last follow-up. Conclusion: In the largest US study to date assessing quality of life following radiofrequency ablation, we observed an expected volume reduction ratio and improvements in thyroid-specific quality of life, particularly in the goiter, anxiety, and appearance domains.

Abstract:Abstract Background Molecular testing can refine the risk of malignancy in cytologically indeterminate thyroid nodules and can reduce the need for diagnostic thyroidectomy. However, quality of life (QOL) in patients mananged with molecular testing is not well studied. Objective We aimed to assess the QOL of patients undergoing surveillance after a benign molecular test result, or thyroidectomy after a suspicious molecular test result. Methods This prospective longitudinal follow-up of the Effectiveness of Molecular Testing Techniques for Diagnosis of Indeterminate Thyroid Nodules randomized trial utilized the Thyroid-Related Patient-Reported Outcome, 39-item version (ThyPro-39) to assess the QOL of patients with indeterminate cytology on thyroid fine needle aspiration (FNA) biopsy. All patients underwent molecular testing at the time of initial FNA. A mixed-effect model was used to determine changes in QOL over time. Results Of 252 eligible patients, 174 completed the assessment (69% response rate). Molecular test results included 72% ( n = 124) benign and 28% ( n = 50) suspicious. ThyPro-39 scores of benign molecular test patients were unchanged from baseline (following initial FNA and molecular test results) to 18 months of ultrasound surveillance. Baseline symptoms of goiter, anxiety, and depression were more severe for patients with suspicious compared with benign molecular test results. At a median of 8 months after thyroidectomy, suspicious molecular test patients reported improved symptoms of goiter, anxiety, and depression. Conclusion A benign molecular test provides sustained QOL throughout ultrasound surveillance, without worsening anxiety or depression relating to the risk of malignancy. Definitive surgery results in improvement of QOL in patients with suspicious molecular tests.

Abstract:BACKGROUND: Thyroid nodules are common, the majority benign. The small risk of malignancy leads to excessive workup. Thyroid ultrasound is essential for risk stratification and management guidance. Without an organised reporting guideline, reports do not add significant value to referring clinicians. The American College of Radiology Thyroid Imaging Reporting and Data System (ACR TI-RADS) was developed to aid ultrasound reporting, lessen excessive biopsies and diagnose thyroid cancers. OBJECTIVES: To standardise reporting of thyroid ultrasounds by utilising an organised reporting guideline based on ACR TI-RADS. METHOD: Thyroid ultrasound reports generated by radiology registrars at an academic hospital were studied in two phases. In Phase 1, the reports were generated as free text, and in Phase 2, using a guideline based on ACR TI-RADS. The percentages of reports that described the maximum size, the five ACR TI-RADS features and a management recommendation were compared. RESULTS: A total of 130 reports were studied. Significant improvement was observed in the description of all five ACR TI-RADS categories (p < 0.0001) from Phase 1 to Phase 2. Of all the reports, 89% included a management recommendation. Reports including an ACR TI-RADS-based recommendation increased from 48% to 75% (p < 0.05). Recommendation for biopsy increased from 35.4% to 53.8% (p < 0.05). CONCLUSION: Introduction of an organised reporting guideline based on ACR TI-RADS, standardised reporting of thyroid ultrasounds by increasing description of thyroid nodule features and ensuring appropriate management recommendations. This, in future, will prevent underdiagnosis of thyroid cancer and unnecessary workup of benign nodules.
Application of Neck Technetium Uptake of the Neck in Post-Operation PTC Patients for Detection of Forthcoming Iodine Ablation Response to Therapy

Abstract:Thyroid remnant after thyroidectomy is a major predictor for future recurrence of thyroid cancers; for which assessment, the accuracy of imaging is not optimal. To evaluate the value of the technetium uptake in the neck compared with neck ultrasonography, this application is used to predict future recurrence of differentiated thyroid cancer (DTC). The neck technetium uptake (NTU) value was acquired 20 min after injection of 1 mCi technetium by a camera. Patients were scheduled for forthcoming iodine ablation. Baseline risk factors were collected. Patients were followed for 10.5 (7 to 12) months when the records of the whole body scan, thyroglobulin, anti-thyroglobulin, neck ultrasonography, and the history of repeated iodine and surgical treatments were collected. The correlation of NTU and recurrence were studied. Out of 73 participants, 63 patients completed the study with established recurrence status and a valid NTU assessment. Baseline lymph node invasion (34.6% vs 80.0%; all 41.9%), extra-thyroid extension (17.0% vs 50.0%; all 22.2%), and tumor size (2.3 ± 2.1 vs 5.2 ± 2.4; all 2.7 ± 2.4) were higher in patients with future recurrence. But the NTU was similar in subjects with and without recurrence (−0.2 ± 0.7 vs 1.7 ± 6.3; all 59 ± 5.3). NTU was significantly lower in subjects with surgeries more extensive than thyroidectomy (<i>P</i> = 0.03). Recurrence cannot be predicted by NTU, but NTU may be used as a surrogate factor to determine the extent and completeness of the thyroidectomy surgery.

Sufficiency of a Single Negative Thyroglobulin Standard for Judging the Success of Ablation in Low- and Intermediate-risk Differentiated Thyroid Cancer: A Retrospective Study

Abstract:Background:: We investigated how reduced successful ablation criteria may be used to evaluate radioiodine remnant ablation in patients with low- and intermediate-differentiated thyroid carcinoma (DTC). Methods:: Overall, 254 low- and intermediate-risk patients with DTC were categorized into three groups (positive, weak, positive, and negative) on the basis of a visual study of thyroid imaging performed before postoperative iodine treatment. Semi-quantitative analysis parameters were incorporated into the positive Tc-99m pertechnetate scanning to further examine the clinical use of thyroid imaging. We investigated the value of successful judgment criteria and the influencing factors of radioiodine ablation. At the same time, the predictive value of thyroglobulin (Tg) for radioiodine treatment and the overall clinical efficacy were assessed. Results:: A total of 250 (98.43%) patients were identified as having functional thyroid tissue residue on the Tx-whole-body scan, and 137 (53.94%) patients had positive Tc-99m pertechnetate scans using semi-quantitative analysis. The single Tg standard could not substitute the double standard (χ! !=22.042, p<0.001) for patients with residual thyroid weight by a semiquantitative analysis. However, the semi-quantitative analysis revealed no association between 99mTcO4−thyroid scan and ablation treatment using semi-quantitative analysis; only preablation sTg levels were related with success in the multivariate logistic regression analysis, with a cut-off value of 2.88 ng/mL. The pre-ablation stimulated Tg level was also the primary factor of satisfactory response following follow-up with an optimal cut-off of 6.506 ng/mL. Conclusion:: Even in low- and intermediate-risk patients with DTC, a single negative Tg standard also requires receiving some restrictions in the evaluation of ablation success and is inadequate. Conventional 99mTcO4 thyroid imaging combined with a quantitative analysis program can improve the clinical practice of single negative Tg standard.

Diagnostic Performance Evaluation of Practice Guidelines, Elastography and Their Combined Results for Thyroid Nodules: A Multicenter Study

Abstract:<p>The purpose of this multicenter study was to compare the differential diagnostic value of the 2015 American Thyroid Association (ATA) and 2017 American College of Radiology (ACR) practice guidelines and elastography in thyroid nodules. This study also investigated whether the diagnostic value of practice guidelines can be improved, and the unnecessary biopsy rate decreased in combination with elastography. A total of 498 thyroid nodules were evaluated using the ATA and the ACR guidelines. Strain elastography, acoustic radiation force impulse imaging and point-shear wave elastography were used to assess the nodules. The suspicious levels were downgraded or upgraded after combination and unnecessary biopsy rates were calculated, respectively. The diagnostic performance of the practice guidelines was better than that of elastography. The ACR guidelines had a lower unnecessary biopsy rate and similar diagnostic performance compared with the ATA guidelines. The unnecessary biopsy rates significantly decreased when the ACR guidelines were combined with elastography, but the rates did not decrease when the ATA guidelines were combined with elastography.</p>

Abstract:BACKGROUND The goal of this study was to evaluate a self-learning algorithm for the computer classification of information extracted from flow cytometric immunophenotype list mode files from high-grade non-Hodgkin's lymphoma (NHL), Hodgkin's disease (HD), and multiple myeloma (MM). Materials and Methods Bone marrow aspirates (BMA) were obtained from untreated NHL (n = 51), HD (n = 9), or MM (n = 13) patients. Bone marrow aspirates were not infiltrated in NHL and HD patients as confirmed by thorough histologic and cytologic investigation; however, MM patients showed an infiltration rate >50% by malignant myeloma cells. Peripheral blood leukocyte (PBL) samples were taken from age-matched healthy volunteers (n = 44) as easily available control material. A second control group of 15 healthy volunteers, from whom BMA and PBL samples were available, allowed us to differentiate whether the observed classification results on malignant samples were due to the malignant process or simply to the inherent differences between BMA and PBL. Bone marrow aspirates and PBL were analyzed by the same immunophenotyping antibody panel (CD45/14/20, CD4/8/3, kappa/CD19/5, lambda/CD19/5). The acquired list mode data files were analyzed and classified by the self-learning triple matrix classification algorithms CLASSIF1 following a priori separation of the data into a learning set and unknown test set. After completion of the learning phase, known patient samples were reclassified and unknown samples prospectively classified by the algorithm. RESULTS Highly discriminatory information was extracted for the various lymphoma entities. The most discriminating information was encountered in antibody binding, antibody binding ratios, and relative antibody surface density parameters of leukocytes rather than in percentage frequencies of discrete leukocyte subpopulations. Samples from healthy controls were classified as normal in 97.2% of the cases, whereas those of NHL, HD, and MM patients were on average correctly classified in 80. 8% of the cases. CONCLUSIONS Although no detectable lymphoma cells were present in BMA of NHL and HD patients, the CLASSIF1 classification of the immunophenotypes of morphologically normal cells provided a surprisingly good disease discrimination equal or better than that obtained by examining pathological lymph nodes according to the respective literature. The results are suggestive for a lymphoma-related and disease-specific antigen expression shift on normal hematopoietic bone marrow cells that can be used to discriminate the underlying disease (specificity of unspecific changes), i.e., in this case NHL from HD. Multiple myeloma patients were discriminated by changes on malignant as well as on normal bone marrow cells.
