Abstract:BACKGROUND CONTEXT Opioids are commonly prescribed to treat spinal pain, especially those undergoing surgery. Cannabis has been suggested as an agent that can modulate opioid needs in these patients. PURPOSE To investigate the effect of cannabis use on perioperative opioid requirement and revision surgery rate in patients undergoing lumbar fusion. STUDY DESIGN Retrospective cross-sectional study PATIENT SAMPLE A total of 48,499 patients from PearlDiver national database who underwent posterior lumbar interbody fusion. OUTCOME MEASURES Opioid-use rates, MME, length of stay, and revision rates. METHODS Using PearlDiver, we identified patients for posterior lumbar interbody fusion (PLIF), cannabis use disorder, revision lumbar fusion, demographics, and comorbidities. Cannabis users and non-users were propensity matched for age, sex, and tobacco use. Pre- and postoperative cumulative morphine milli equivalence (MME) were calculated. Opioid-use rates, MME, length of stay (LOS), and revision rates were compared using univariate analysis. Revision rates were compared using Kaplan-Meyer log-rank analysis, and logistic and cox regression. RESULTS Of 48,499 patients undergoing PLIF, 3.4% were identified as chronic cannabis users. They were younger, and more likely to be male and use tobacco. They had a higher rate and amount of opioid use within 90-days preoperatively, and 90- and 365-days postoperatively, after controlling for age, sex, and tobacco use. Cannabis users had longer LOS (4.4 vs. 4.0 days), and a higher rate of revision surgery (6.9% vs. 3.2%). Log-rank analysis, as well as logistic and cox regression confirmed an increased revision rate. Concurrent tobacco and cannabis use also had an additive effect on revision rate to 8.1%, compared with those who used only cannabis (5.4%) or tobacco (4.5%). CONCLUSIONS Chronic cannabis use is associated with an increased preoperative and long-term postoperative opioid use, a longer length of stay, and an increased revision rate.

Abstract:INTRODUCTION: Opioids are critical for pain management following spine surgery. Given the devastating consequences of the opioid epidemic, strategies to minimize dosing and chronic use of opioids should be developed. METHODS: This retrospective study used medical coding to identify patients who had undergone 1-3 level lumbar fusions between January 2010 and October 2020. Propensity score matching based on demographics and comorbidities created two equal cohorts concerning cannabis use history. Opioid utilization rates (morphine milligram equivalents/day) and overuse rates six months after the index procedure were assessed. RESULTS: 1,216 patients were matched and distributed equally into cannabis and non-cannabis user cohorts. Lower rates of opioid utilization were found among cannabis users versus control as early as two months after fusion (47.7% versus 41.1%, p < 0.05), a relationship which persisted at six months as well (46.2% versus 37.7%, p < 0.01). Lower rates of high-dose opioid utilization (=100 MME per day) were also found in the cannabis user cohort during the initial period of 14-30 days following surgery (6.91% versus 3.79%, p < 0.05). This relationship also approached significance 121-150 days after surgery (3.13% versus 1.32%, p = 0.051). CONCLUSIONS: In this study, patients with a history of cannabis use were less likely to use opioids as early as two months postoperatively. They had lower rates of high-dose opioid utilization in the immediate postoperative period. While this study cannot say whether cannabis is a beneficial adjunctive therapy for pain, clearly physicians should be aware of their patients’ cannabis use patterns to provide appropriate dose titration over time.

Abstract:Monoamine oxidase- and catechol-Omethyl transferase-inhibited superfused (60 min) rat vasa deferentia were preincubated with either (−)- or (+)-14C-norepinephrine (200 ng/ml, 30 min). As compared to that in the control tissue, the radioactivity from the reserpine-pretreated tissue disappeared at a faster rate. The rates of disappearance of the total radioactivity from the tissues either preincubated with (−)-14C-norepinephrine or (+)-14C-norepinephrine were identical. In the reserpine-pretreated tissues, phenoxybenzamine enhanced the disappearance of the radioactivity. Tyramine produced equal contractions in the normal vasa deferentia preincubated with either (−)- or (+)-14C-norepinephrine, but released 25–50% more radioactivity from the tissue pre-incubated with the(+)-isomer than that released from the tissue incubated with the (−)-isomer. In vasa deferentia obtained from reserpine-pretreated rats, tyramine released equal amounts of radioactivity when tissues were either preincubated with (−)- or (+)-isomer. However, the response of vasa deferentia preincubated with the (−)-isomer was approximately 7-fold greater than of those preincubated with the (+)-isomer. The results are discussed in relation to the drug-induced release of the isomers.
Association between history of cannabis use and outcomes after total hip or knee arthroplasty: a systematic review and meta-analysis

Abstract:Background: Cannabis use may be increasing as countries legalize it and it becomes socially acceptable. A history of cannabis use may increase risk of complications after various kinds of surgery and compromise functional recovery. Here we systematically reviewed and meta-analyzed available evidence on how history of cannabis use affects recovery after hip or knee arthroplasty (THA/TKA). Methods: The PubMed, EMBASE, and Web of Science databases were comprehensively searched and studies were selected and analyzed in accordance with the PRISMA guidelines. The methodological quality of included studies was assessed based on the Newcastle-Ottawa Scale, while quality of evidence was evaluated according to the "Grading of recommendations assessment, development, and evaluation" system. Data on various outcomes were pooled when appropriate and meta-analyzed. Results: The systematic review included 16 cohort studies involving 5.91 million patients. Meta-analysis linked history of cannabis use to higher risk of the following outcomes: revision (RR 1.68, 95% CI 1.31–2.16), mechanical loosening (RR 1.77, 95% CI 1.52–2.07), periprosthetic fracture (RR 1.85, 95% CI 1.38–2.48), dislocation (RR 2.10, 95% CI 1.18–3.73), cardiovascular events (RR 2.49, 95% CI 1.22–5.08), cerebrovascular events (RR 3.15, 95% CI 2.54–3.91), pneumonia (RR 3.97, 95% CI 3.49–4.51), respiratory failure (RR 4.10, 95% CI 3.38–4.97), urinary tract infection (RR 2.46, 95% CI 1.84–3.28), acute kidney injury (RR 3.25, 95% CI 2.94–3.60), venous thromboembolism (RR 1.48, 95% CI 1.34–1.63), and deep vein thrombosis (RR 1.42, 95% CI 1.19–1.70). In addition, cannabis use was associated with significantly greater risk of postoperative transfusion (RR 2.23, 95% CI 1.83–2.71) as well as higher hospitalization costs. Conclusion: History of cannabis use significantly increases the risk of numerous complications and transfusion after THA or TKA, leading to greater healthcare costs. Clinicians should consider these factors when treating cannabis users, and pre-surgical protocols should give special consideration to patients with history of cannbis use.

Cannabis Use Disorder Associated With Increased Risk of Postoperative Complications After Hip or Knee Arthroplasties: A Meta-analysis of Observational Studies

Abstract:Introduction: With the legalization of marijuana in the United States, the number of patients with cannabis use disorder (CUD) in the joint arthroplasty population has increased markedly. The primary purpose of this meta-analysis was to determine whether there were differences in clinical and economic outcomes after total joint arthroplasty (TJA) between patients with and without perioperative CUD. Methods: We searched PubMed, Embase, Scopus, and Web of Science databases up to July 2018 to identify all eligible studies investigating the association of CUD with postoperative outcomes in patients undergoing TJA. Postoperative outcomes assessed consisted of complications, readmission, length of stay (LOS), implant revision, and cost of care. For dichotomous outcomes, pooled odds ratios (OR) with 95% confidence intervals (CI) were calculated using a random effects model. Results: We identified 10 retrospective cohort studies with a total of 17,981,628 study participants. Patients with CUD had significantly higher odds of medical complications (OR 1.33 [95% CI 1.07 to 1.66], P = 0.01) and implant-related complications (OR 1.75 [95% CI: 1.64 to 1.88], P < 0.00001) than noncannabis users. Specifically, CUD was associated with significantly increased odds of cardiac complications (OR 1.95 [95% CI 1.50 to 2.54], P < 0.00001), cerebrovascular accidents (OR 2.06 [95% CI 1.66 to 2.57], P < 0.00001), postoperative infections (OR 1.68 [95% CI 1.34 to 2.10], P < 0.00001), periprosthetic fracture (OR 1.42 [95% CI 1.19 to 1.70], P < 0.0001), mechanical loosening (OR 1.54 [95% CI 1.42 to 1.66], P < 0.00001), and dislocation/instability (OR 1.88 [95% CI 1.32 to 2.68], P = 0.0005). Longer LOS and higher cost of care were also found in patients with CUD. Conclusion: This study strengthens the body of evidence that patients with CUD face higher risk of postoperative complications and greater financial burden after knee and hip arthroplasties. Physicians should inform patients about adverse outcomes and undertake appropriate risk adjustments before elective orthopaedic surgery. Level of evidence: Level III.

We found similar results.

Abstract:Although epidural steroid injections are commonly used to treat radicular pain [1, 2], a paucity of data exists with regard to the long-term impact on pain after subsequent spine surgery. We hypothesized that in a proportion of patients, preoperative epidural injections might delay surgery and efficient analgesia, with potential worsening of pain pathology, or could indicate severe disease and thus would be associated with postoperative chronic opioid use. Study approval was obtained by our Institutional Review Board (IRB #2017-0169). We conducted a retrospective study of patients undergoing anterior cervical discectomy and fusion (ACDF) and microdiscectomy captured in the Truven Health MarketScan database (Copyright ©2017 Truven Health Analytics, Inc.) in 2012–2016 by using International Classification of Diseases 9th/10th revision (ICD-9/10) procedure codes (Supplementary Data Appendix 1). The main exposure of interest, preoperative epidural steroid injection, was identified on the basis of Current Procedural Terminology (CPT) codes reported in Supplementary Data Appendix 2 and included visits with a diagnosis code for arm/neck pain (patients undergoing ACDF) or back pain (patients undergoing microdiscectomy). The outcome postoperative chronic opioid use was defined as filling prescriptions for at least 120 pills or filling 10 or more prescriptions for opioids within the period from 90 days to 1 year after surgery [3, 4].
Antibodies against autologous tumor cell proteins in patients with small-cell lung cancer: association with improved survival.

Abstract:BACKGROUND The frequency and clinical relevance of human antitumor immune responses is not well known, and few target antigens have been identified. PURPOSE This study was designed to determine the frequency of antibodies reactive against extracts of autologous tumor cell lines and to correlate these data with survival. METHODS Serum samples were obtained from 40 lung cancer patients treated on National Cancer Institute protocols. These sera were used as probes in immunoblots against protein extracts from tumor cell lines derived from each of these patients. RESULTS We detected serum antibodies against autologous tumor cell proteins in 21 (58%) of the 36 patients with small-cell lung cancer (SCLC) and three (75%) of the four with non-small-cell lung cancer (NSCLC). Two patients' sera detected the p53 tumor suppressor gene product and two detected the product of the HuD gene (associated with paraneoplastic neurological syndromes) in their autologous tumor cell lysates. SCLC patients with antibodies against autologous tumor cell proteins had improved survivals compared with those in the antibody-negative group (P = .059). All patients who lived longer than 36 weeks were antitumor antibody positive. Sera from six (86%) of seven patients with limited disease were positive for antibodies that reacted against autologous tumor cells, compared with 15 of 29 (52%) of sera from patients with extensive disease. CONCLUSIONS Our results suggest that the sera from patients with SCLC frequently contain antibodies against tumor cell proteins and that these antibodies are associated with improved survival. IMPLICATIONS These data suggest that an antitumor immune response may affect tumor growth, and that the anonymous proteins detected by antitumor antibodies in lung cancer patient sera may represent proteins involved in the development of lung cancer or in its clinical manifestations.

Abstract:Homocytotrophic antibodies (HCA) were produced in rats by primary injection of streptokinase-streptodornase (SK-SD) after the antigen was absorbed on alminum hydroxide gel. The properties of rat SK-SD-specific HCA were qualitatively similar to SK-SD skin-sensitizing antibodies found in sera of human beings, namely, homocytotropic activity (48 h homologous PCA reaction), heat lability at 56 degrees C, and inactivation by reduction with 2-mercaptoethanol followed by alkylation with iodoacetamide. In neutralization tests, D-SK-SD, a low molecular hapten-like substance contained in SK-SD preparation, absorbed the antibody against ND-SK-SD antigen-specifically. The neutralizing activity mostly resided in fraction 1 of D-SK-SD separated through DEAE-Sephadex A-25 ion-exchange column, opposing the inhibitory activity of fraction 3 on lymphocyte transformation. The possible implication of these findings in the immunological treatment of streptococcal disorders is discussed.
Marijuana's Impact On Implant-based Breast Reconstruction: A Retrospective Cohort Study

Abstract:Background: Studies have shown that chronic marijuana use is associated with increased vascular inflammation, endothelial damage, myocardial infarctions, strokes, arteritis, and cardiomyopathies; however, cannabis's effect on wound healing in immediate direct-to-implant (DTI) breast reconstruction is unknown. With the increasing prevalence of marijuana use, it is imperative to understand its effects on surgical outcomes. Methods: We performed a retrospective cohort study of consecutive patients in a quaternary-care breast cancer center undergoing immediate DTI reconstruction. Patient demographics, operative details, and surgical complications were extracted through chart review. Active cannabis use was defined as use within 12 weeks of operation. Univariate and multivariable analyses were performed. Results: In total, 243 consecutive patients underwent immediate DTI reconstruction, and 12 reported active cannabis use. There were no significant differences in patient demographics, cancer treatment, or operative details. Active marijuana users demonstrated higher rates of cellulitis treated with IV antibiotics (P = 0.004), explantation for infection (P = 0.004), emergency department visits (P = 0.028), readmission (P = 0.037), takeback to the operating room in 90 days (P < 0.001), and overall major complications (P < 0.001). Multivariable analysis demonstrated that active marijuana users were more likely to experience cellulitis treated with IV antibiotics [odds ratio (OR) = 3.55, P = 0.024], takeback to the OR within 90 days of operation (OR = 4.75, P = 0.001), and major complications (OR = 2.26, P = 0.048). Conclusions: The consumption of cannabis in the perioperative setting is associated with increased rates of complications in patients undergoing immediate DTI reconstruction; however, an analysis with a larger patient population is needed to conclude that abstinence from its use should be highly encouraged.
347 Short-Term Pain Outcomes and Pain Medication Utilization Among Urine Toxicology-Identified Opioid and Marijuana Users Following Elective Spine Surgery

Abstract:INTRODUCTION: Postoperative pain outcomes may be influenced by preoperative substance use, which is often underreported due to associated stigma. METHODS: Patients undergoing elective spinal surgery between September 2020 and May 2022 were recruited for this prospective cohort study. Detailed chart review was completed to collect demographic, urine toxicology, VAS, and pain medication regimen data. Comparisons between self-reported and urine toxicology-identified substance use, pre- and post-operative VAS ratings, and postoperative pain medication use were made using χ 2 tests, student’s t-tests, and logistic regression, respectively. Models were adjusted for age, sex, and race. RESULTS: Among 111 participants (mean age 58 years, 59% female, 95% with >1 comorbidity), urine toxicology overestimated drug use (47% vs 16%, p < 0.001) and underestimated alcohol use (16% vs 56%, p < 0.001) at preoperative baseline relative to patient reports. Two-weeks postoperatively, participants on preoperative opioids as determined by urine toxicology reported no significant improvements in pain from baseline (6.67 preop vs 5.92 postop, p = 0.288) unlike non-users (6.56 preop vs 4.61 postop, p < 0.001). They also had worse postoperative VAS (5.92 vs 4.61, p = 0.030) and heavier reliance on opioid medications (OR = 3.09, 95% CI = 1.21-7.89, p = 0.019). Conversely, participants on preoperative marijuana reported similar improvements in pain from baseline (users: 6.88 preop vs 4.36 postop, p = 0.001; non-users: 6.49 preop vs 5.07 postop, p = 0.001), similar postoperative pain (4.36 vs 5.07, p = 0.238), and similar postoperative reliance on opioid medications (OR = 0.96, 95% CI = 0.38-2.44, p = 0.928). CONCLUSIONS: While urine toxicology-identified preoperative opioid use was associated with poor postoperative pain relief and continued reliance on postoperative opioids for pain management following elective spinal surgery, preoperative marijuana use was not. Preoperative marijuana use, hence, should not delay or be a contraindication to elective spinal surgery.

In-hospital outcomes of inflammatory bowel disease in cannabis users: a nationwide propensity-matched analysis in the United States

Abstract:Background: Literature suggests the role of cannabis (marijuana) as an anti-inflammatory agent. However, the impact of recreational marijuana usage on in-hospital outcomes of inflammatory bowel disease (IBD) remains indistinct. We assessed the outcomes of Crohn's disease (CD) as well as ulcerative colitis (UC) with vs. without recreational marijuana usage using a nationally illustrative propensity-matched sample. Methods: The Nationwide Inpatient Sample datasets (2010-2014) were queried to identify adults with CD and UC hospitalizations with cannabis use and linked complications using ICD-9 CM codes. Categorical and continuous variables were compared between propensity-matched cohorts using Chi-square and Student's t-test, respectively. Primary endpoints were in-hospital complications, whereas secondary endpoints were the discharge disposition, mean length of stay (LOS) and hospital charges. Results: Propensity-matched cohorts included 6,002 CD (2,999 cannabis users & 3,003 non-users) and 1,481 UC (742 cannabis users & 739 non-users) hospitalizations. In CD patients, prevalence of colorectal cancer (0.3% vs. 1.2%, P<0.001), need for parenteral nutrition (3.0% vs. 4.7%, P=0.001) and anemia (25.6% vs. 30.1%, P<0.001) were lower in cannabis users. However, active fistulizing disease or intraabdominal abscess formation (8.6% vs. 5.9%, P<0.001), unspecific lower gastrointestinal (GI) hemorrhage (4.0% vs. 2.7%, P=0.004) and hypovolemia (1.2% vs. 0.5%, P=0.004) were higher with recreational cannabis use. The mean hospital stay was shorter (4.2 vs. 5.0 days) with less hospital charges ($28,956 vs. $35,180, P<0.001) in cannabis users. In patients with UC, cannabis users faced the higher frequency of fluid and electrolyte disorders (45.1% vs. 29.6%, P<0.001), and hypovolemia (2.7% vs. <11) with relatively lower frequency of postoperative infections (<11 vs. 3.4%, P=0.010). No other complications were significant enough for comparison between the cannabis users and non-users in this group. Like CD, UC-cannabis patients had shorter mean hospital stay (LOS) (4.3 vs. 5.7 days, P<0.001) and faced less financial burden ($30,393 vs. $41,308, P<0.001). Conclusions: We found a lower frequency of colorectal cancer, parenteral nutrition, anemia but a higher occurrences of active fistulizing disease or intraabdominal abscess formation, lower GI hemorrhage and hypovolemia in the CD cohort with cannabis usage. In patients with UC, frequency of complications could not be compared between the two cohorts, except a higher frequency of fluid and electrolyte disorders and hypovolemia, and a lower frequency of postoperative infections with cannabis use. A shorter LOS and lesser hospital charges were observed in both groups with recreational marijuana usage.
Development and Validation of a Risk Prediction Model for Pseudarthrosis Following Transforaminal Lumbar Interbody Fusion: A Retrospective Analysis

Abstract:Background: Pseudarthrosis is a common complication of transforaminal lumbar interbody fusion (TLIF) that can affect the long-term prognosis and increase revision surgery risk. Therefore, it is crucial to investigate the risk factors associated with pseudarthrosis and develop a predictive model. Methods: Patients who underwent TLIF at a single hospital between January 2019 and June 2021 were included. Each patient's fusion status was determined based on thin-section lumbar spine computed tomography scans performed at least 1 year postoperatively. A nomogram was subsequently established to predict the probability of pseudarthrosis based on the results of least absolute shrinkage and selection operator and multivariable analysis. Receiver operating characteristic curve, calibration curve, and decision curve analysis were applied to evaluate the prediction effect and clinical value of the model. Results: In total, 284 patients (131 men [46.1%]; mean [standard deviation, SD] age, 63.0 [12.1] years) were included in the analysis (mean [SD] follow-up, 19.2 [9.2] months). The incidence of pseudarthrosis was 13.4% (38/284). In least absolute shrinkage and selection operator and multivariable analysis, independent risk factors for pseudarthrosis included history of smoking (odds ratio [OR]:2.966, 95% confidence interval [CI]: 1.135-7.750, P = 0.027), osteoporosis (OR = 6.362, 95% CI: 2.433-16.636, P < 0.001), and the change of mean disc height (OR = 2.401, 95% CI: 1.638-3.519, P < 0.001). The areas under the curves in the training and validation cohorts were 0.870 and 0.809, respectively. Conclusions: History of smoking, osteoporosis, and change of mean disc height are all independent risk factors of pseudarthrosis following TLIF surgery; a nomogram based on these may help predict the probability of pseudarthrosis.

Abstract:Study Design: Retrospective cohort study. Objective: The goal of this study is to evaluate the effects of preoperative lumbar epidural steroid injection on the rate of pseudarthrosis following lumbar spine fusion surgery. Summary of Background Data: Epidural corticosteroids help to reduce nerve root edema and suppress proinflammatory cytokines in patients with radiculopathy. Corticosteroids may inhibit bone formation and reduce bone matrix synthesis rates. Thus, there is concern that corticosteroids may reduce lumbar fusion capability, potentially resulting in increased rates of symptomatic pseudarthrosis. Materials and Methods: We identified all patients who underwent 1-level or 2-level lumbar fusion surgery between 2018 and 2022. Patients were categorized into one of 3 groups: no preoperative epidural steroid injection (ESI) history (group 0), preoperative ESI within 90 days of surgery (group 1), or most recent ESI >90 days before surgery (group 2). The primary outcome of this study was pseudarthrosis. Binominal regression analyses were performed to determine the relationships between potential risk factors (sex, age, body mass index, smoking history, diabetes status, history of systemic steroid use, preoperative ESI, perioperative intravenous steroid administration, type of surgery, and postoperative ESI within 6 mo) and the development of postoperative pseudarthrosis. Results: A total of 446 patients were included in this study. Of those, 106 patients (23.7%) did not have a preoperative ESI (group 0), 132 patients (29.5%) had an ESI within 90 days of surgery (group 1), and 208 patients (46.6%) had their most recent ESI >90 days before surgery (group 2). The overall incidence of pseudarthrosis following lumbar fusion was 8.7% (39 of 446). Although the incidence of pseudarthrosis following ESI at any time point was higher than in our control cohort (group 0), this difference was not statistically significant. Conclusions: This study found no increased risk of postoperative pseudarthrosis in patients who underwent 1-level or 2-level lumbar fusions after preoperative ESI. Level of Evidence: Level III.

Abstract:This study presents a systematic literature review and meta-analysis of pseudarthrosis risk factors following lumbar fusion procedures. The odds ratio (OR) and 95% confidence interval (95% CI) were used for outcome measurements. The objective of this study was to identify the independent risk factors for pseudarthrosis after lumbar spinal fusion, which is crucial for mitigating morbidity and reoperation. Systematic searches in PubMed, Embase, and Scopus (1990–July 2021) were conducted using specific terms. The inclusion criteria included prospective and retrospective cohorts and case‒control series reporting ORs with 95% CIs from multivariate analysis. The quality assessment utilized the Newcastle–Ottawa scale. Meta-analysis, employing OR and 95% CI, assessed pseudarthrosis risk factors in lumbar fusion surgery, depicted in a forest plot. Of the 568 abstracts identified, 12 met the inclusion criteria (9 retrospective, 2006–2021). The 17 risk factors were categorized into clinical, radiographic, surgical, and bone turnover marker factors. The meta-analysis highlighted two significant clinical risk factors: age (95% CI 1.02–1.11; p = 0.005) and smoking (95% CI 1.68–5.44; p = 0.0002). The sole significant surgical risk factor was the number of fused levels (pooled OR 1.35; 95% CI 1.17–1.55; p < 0.0001). This study identified 17 risk factors for pseudarthrosis after lumbar fusion surgery, emphasizing age, smoking status, and the number of fusion levels. Prospective studies are warranted to explore additional risk factors and assess the impact of surgery and graft type.

Abstract:Background: Retrospective studies have found that daily opioid use pre-arthroplasty predicts worse longer-term service, clinical and patient-reported outcomes. Prospective studies are needed to confirm these observations. This prospective, exploratory study aimed to determine: the proportion of total knee or hip arthroplasty (TKA, THA) patients who use opioids regularly (daily) pre-surgery; if opioid use pre-surgery is associated with acute and sub-acute outcomes to 12-weeks post-surgery. Methods: Consecutive patients undergoing primary TKA or THA were prospectively enrolled pre-surgery and followed-up by telephone to 12-weeks post-surgery. Acute-care (oral morphine equivalent dosage (OMED), length of stay, discharge to inpatient rehabilitation, complications) and 12-week outcomes (Oxford Knee or Hip Score, Euroqol 'today' health score, current use of opioids, and complications including readmissions) were monitored. Unadjusted and adjusted Odds Ratios (ORs) (95% Confidence Interval, CI), Rate Ratios and β coefficients (standard error) were calculated. Results: Five Hundred Twenty-One patients were included (TKA n = 381). 15.7% (95%CI 12.6 to 18.9) used opioids regularly pre-surgery. 86.8% (452/521) were available for follow-up at 12-weeks. In unadjusted analyses, pre-surgical opioid use was significantly associated with higher average acute daily OMED [β 0.40 (0.07), p < 0.001], presence of an acute complication [OR 1.75 (1.02 to 3.00)], and ongoing use of opioids at 12-weeks [OR 5.06 (2.86 to 8.93)]. After adjusting for covariates, opioid use pre-surgery remained significantly associated with average acute daily OMED [β 0.40 (0.07), p < 0.001] and ongoing use at 12-weeks [OR 5.38 (2.89 to 9.99)]. Conclusion: People who take daily opioids pre-surgery have significantly greater odds for greater opioid consumption acutely and ongoing use post-surgery. Adequately powered prospective studies are required to confirm whether pre-surgical opioid use is or is not associated with poorer joint and quality of life scores or a complication in the short-term.

Abstract:Background Conflicting evidence exists evaluating associations between cannabis (THC) and post-traumatic DVT. Methods Retrospective analysis (2014–2023) of patients ≥15yrs from two Level I trauma centers with robust VTE surveillance and prophylaxis protocols. Multivariable hierarchical regression assessed the association between THC and DVT risk. THC + patients were direct matched to other drug use categories on VTE risk markers and hospital length of stay. Results Of 7365 patients, 3719 were drug-, 575 were THC + only, 2583 were other drug+, and 488 were TCH+/other drug+. DVT rates by exposure group did not differ. TCH + only patients had higher GCS scores, shorter hospital length of stay, and the lowest pelvic fracture and mortality rates. A total of 458 drug-, 453 other drug+, and 232 THC+/other drug + patients were matched to 458, 453, and 232 THC + only patients. There were no differences in DVT event rates in any paired sub-cohort set. Additionally, iteratively adjusted paired models did not show an association between THC and DVT. Conclusions THC does not appear to be associated with increased DVT risk in patients with strict trauma chemoprophylaxis. Toxicology testing is useful for identifying substance abuse intervention opportunities, but not for DVT risk stratification in THC + patients.

Abstract:Introduction: Opioids and cannabis possess immunomodulatory properties. However, the effect of opioid receptor activity on immune function is complicated, facilitated by several mechanisms and can lead to enhancement or inhibition. Similarly, the relation between cannabinoid ligands of diverse types and endocannabinoid receptors on various immune cells is complex. We hereby delineate the effect of prescription opioids and recreational cannabis use on immune-related adverse events (IRAEs) during immune checkpoint inhibitor (ICI) therapy in various cancers, as well as efficacy outcomes in advanced non-small cell lung cancer (aNSCLC). Methods: This is a retrospective cohort study of adult patients with advanced cancers who received ICI with or without chemotherapy for at least two months at an urban safety net hospital from January 2016 to June 2022. All patients, as well as a subset of patients with aNSCLC, were divided into two arms based on whether they were prescribed opioids during ICI therapy or not (OP vs. NO). Similarly, they were grouped based on whether they reported recreational cannabis use during ICI therapy (since initiation of therapy) or not (CA vs. NC). Incidence of IRAEs, median overall survival (mOS) and median progression-free survival (mPFS) calculated per RECIST 1.1 were the outcome variables. Multivariate logistic regression and alternative log-rank test were utilized for analysis with adjustments for demographics, insurance status, BMI, ECOG, PD-L1, TMB, smoking and concurrent chemotherapy. Results: 230 patients received ICI-based therapy; 47% were female, 34% were African American and 6.5% were Hispanic. There was no significant difference in the incidence of IRAEs between OP (n=144) and NO (n=86; p=0.225). However, CA (n=44) had higher IRAEs than NC (n=186) with an odds ratio estimate of 2.023 (X2=4.18, CI 1.03-3.97, p=0.04). Among 110 patients with aNSCLC, 46% were female. Patients with aNSCLC in the OP arm (n=69) had no significant difference in mOS (HR=1.34, CI 0.8-2.25, p=0.26) or mPFS (HR=1.21, CI 0.69-2.1, p=0.51) compared to patients in the NO arm (n=51). Similarly, the two arms CA (n=21) and NC (n=89) had no difference in mOS (HR:0.96, CI 0.5-1.83, p=0.89) or mPFS (HR:1.18, CI 0.61-2.29, p=0.63) for aNSCLC. Conclusion: Recreational cannabis use during ICI therapy increased the incidence of IRAEs. Prescription opioids and recreational cannabis use did not significantly affect survival outcomes for ICI in aNSCLC. Prescription opioid use also did not significantly influence the occurrence of IRAEs. This study suggests that recreational cannabis may need to be used with caution in this patient population due to its influence on IRAEs. On the other hand, opioids appear to be a safe option for pain management in these patients. Larger studies are warranted to shed more insight into the effect of these substances on ICI therapy.
