Abstract:From 10/81 to 1/87, the National Cancer Institute (NCI) of the U.S. Department of Public Health Services (PHS) contracted with four institutions to evaluate hyperthermia technology to be applied in the treatment of human malignancy. During the five year period 1981-1986, data were collected which now reside in a consensus database representing treatments of 792 separate sites or fields in a total of 573 patients. These patients were treated with one or more of 49 devices by the four institutions. Sixteen ultrasound (US) devices were evaluated in 195 sites. Nine magnetic induction (MI) devices were evaluated in 208 sites. Twenty radiative electromagnetic (EM) devices were evaluated in 488 sites. Four interstitial (IRF) devices were evaluated in 37 sites. Many sites were treated with multiple hyperthermia modalities. This first in a series of 13 reports describes the general objectives of the Contractors' Group, basic methods of device evaluation and brief details of the large variety of devices tested.
Psychosocial aspects of health-related quality of life and the association with patient-reported bladder symptoms and satisfaction after spinal cord injury

Abstract:Study design: Prospective, multi-centered, observational. Objectives: To characterize the relationship between psychosocial aspects of health-related quality of life (HRQoL) and patient-reported bladder outcomes. Setting: Multi-institutional sites in the United States, cohort drawn from North America. Methods: We performed a cross-sectional analysis of data collected as part of the multicenter, prospective Neurogenic Bladder Research Group Spinal Cord Injury (SCI) Registry. Outcomes were: Neurogenic Bladder Symptom Score (NBSS), Neurogenic Bladder Symptom Score Satisfaction (NBSS-Satisfaction), and SCI-QoL Bladder Management Difficulties (SCI-QoL Difficulties). Adjusted multiple linear regression models were used with variables including demographic, injury characteristics, and the following psychosocial HRQoL measures; SCI-QoL Pain Interference (Pain), SCI-QoL Independence, and SCI-QoL Positive Affect and Well-being (Positive Affect). Psychosocial variables were sub-divided by tertiles for the analysis. Results: There were 1479 participants, 57% had paraplegia, 60% were men, and 51% managed their bladder with clean intermittent catheterization. On multivariate analysis, higher tertiles of SCI-QoL Pain were associated with worse bladder symptoms, satisfaction, and bladder management difficulties; upper tertile SCI-QoL Pain (NBSS 3.8, p < 0.001; NBSS-satisfaction 0.6, p < 0.001; SCI-QoL Difficulties 2.4, p < 0.001). In contrast, upper tertiles of SCI-QoL Independence and SCI-QoL Positive Affect were associated with improved bladder-related outcomes; upper tertile SCI-QoL Independence (NBSS -2.3, p = 0.03; NBSS-satisfaction -0.4, p < 0.001) and upper tertile SCI-QoL Positive Affect (NBSS -2.8, p < 0.001; NBSS-satisfaction -0.7, p < 0.001; SCI-QoL Difficulties -0.7, p < 0.001). Conclusions: In individuals with SCI, there is an association between psychosocial HRQoL and bladder-related QoL outcomes. Clinician awareness of this relationship can provide insight into optimizing long-term management after SCI.
Resilience as an Independent Predictor of Bowel Related Quality of Life After Spinal Cord Injury

Abstract:Bowel dysfunction remains a prominent priority in the rehabilitation of patients with spinal cord injuries (SCIs). However, our understanding of the factors that influence bowel-related quality of life (QoL) in this population remains limited. This study aimed to investigate the potential role of resilience, defined as an individual's capacity to cope with and adapt to adversity, as a predictor of bowel-related QoL among the patients with SCI. A cohort of adult patients with SCI who received bowel and bladder care at an outpatient clinic within a large rehabilitation hospital was identified through a prospectively collected database. Resilience was measured using the Spinal Cord Injury-Quality of Life (SCI-QOL) Resilience Short Form, whereas bowel QoL was assessed using the Irritable Bowel Syndrome-Quality of Life (IBS-QoL) questionnaire. Univariate and multivariate regression analyses were employed to identify predictors of bowel-related QoL. The examined variables included age, gender, level and completeness of injury, time since injury, hand function, resilience, and the severity of bowel dysfunction as measured using the Neurogenic Bowel Dysfunction Score (NBDS). A total of 73 patients participated in this study, with a mean age of 44.01 ± 13.43 years and comprising mostly men (n = 57, 78%). The results revealed a significant correlation between resilience scores and the total score of IBS-QoL (ρ = -0.47, p < 0.0001). The multivariate analysis demonstrated that both resilience and the severity of bowel dysfunction were significant independent predictors of bowel-related QoL, with resilience demonstrating a stronger association. Overall, this study elucidates the importance of resilience in shaping patients' perceptions of their bowel health within the SCI population. In addition to the more expected determinants of bowel-related QoL, such as the severity of bowel dysfunction, resilience emerged as a notable factor. Accordingly, integrating interventions that enhance resilience within bowel rehabilitation programs may yield improvements in patients' perceived bowel health beyond the benefits achievable through bowel function enhancement alone.

Abstract:The mode of action of bulgecin was investigated by examining its bactericidal and bacteriolytic activities, its effect on bacterial morphology, and its interaction with penicillin-binding proteins (PBPs). Bulgecin alone did not show any antibacterial activity against Escherichia coli and Serratia marcescens, but in concert with cefmenoxime, it induced potent growth-inhibitory and bactericidal activities. Electron microscopic examination of E. coli cells exposed to bulgecin combined with cefmenoxime revealed that a bulge developed in the middle of the cell, and additional smaller bulges were formed halfway between the central bulge and the polar ends. At the site of bulge development, vesicular mesosomelike structures appeared in the cytoplasm, the peptidoglycan layer facing them became faint, and the outer membrane protruded to form blebs. These morphological changes were quite different from those caused by the mecillinam-cefmenoxime combination that produces big bulges in E. coli. When S. marcescens was exposed to the combination of bulgecin and cefmenoxime, not only bulge formation, but also branching of the cells was observed. Bulgecin neither showed affinity for any PBPs of E. coli nor affected the binding of cefmenoxime or mecillinam to the PBPs.
[General surgical procedures in patients with electrical pacemaker].

Abstract:Work of the implanted electric pacemaker (EP) was assessed in 99 patients, aged 62.4±9.6 years, during non-cardiological surgery. Inhibition of the EP stimuli was registered in 9 (9.1%) patients, short episodes of uneffective stimulation with synchronization disturbation--in 2 (2%) patients and change of stimulation regimen was registered in the same number of patients by electocoagulation. Episodes of myopotential inhibition not assotiated with electrocoagulation was registered in 4 cases. The ascertained rhythm disturbances require a thorough preoperative check-up, intraoperative ECG control and short use of monopolar electrocoagulation.

Abstract:INTRODUCTION: Chronic pain does not reside within an individual. Pain is affected by and affects larger systems (e.g., families). We examined longitudinal, bidirectional associations between chronic pain and family functioning in spina bifida (SB). We hypothesized a bidirectional association between pain and family functioning (cohesion and conflict); youth chronic pain status would be associated with maladaptive family functioning and maladaptive family functioning would be associated with youth chronic pain status.METHOD: Participants were from an ongoing longitudinal study of adolescents with SB (<i>N</i> = 140, 53.6% female, ages 8-15 at Time 1). Data were collected every 2 years, with this study using data from Times 1 and 2 (T1, T2; T1: 2006-2009, T2: 2008-2011). Parents reported on youth demographics and family functioning. Youth reported on family functioning and pain; pain was dichotomized into a categorical variable (chronic pain vs. no chronic pain). Family functioning (cohesion and conflict) was also assessed using observational data.RESULTS: Youth chronic pain status was associated with decreased family conflict (child report) and increased cohesion (parent report) over time. Increased family conflict (parent report) was associated with a greater likelihood of reporting chronic pain 2 years later. Findings were nonsignificant between observed family conflict or cohesion and chronic pain in either direction.DISCUSSION: With the presence of an additional stressor (chronic pain) families increase cohesion and reduce conflict. Increased conflict may increase a child's vulnerability of developing chronic pain. It is critical that interventions for both pain and family functioning in SB are guided by a strengths-based model. An Exploratory Risk Perception Study of Attitudes Toward Homeland Security Systems

Abstract:Understanding the issues surrounding public acceptance of homeland security systems is important for balancing security needs and potential civil liberties infringements. A psychometric survey was used in an exploratory study of attitudes regarding homeland security systems. Psychometric rating data were obtained from 182 respondents on psychological attributes associated with 12 distinct types of homeland security systems. An inverse relationship was observed for the overall rating attributes of acceptability and risk of civil liberties infringement. Principal components analysis (PCA) yielded a two‐factor solution with the rating scale loading pattern suggesting factors of perceived effectiveness and perceived intrusiveness. These factors also showed an inverse relationship. The 12 different homeland security systems showed significantly different scores on the rating scales and PCA factors. Of the 12 systems studied, airport screening, canine detectors, and radiation monitoring at borders were found to be the most acceptable, while email monitoring, data mining, and global positioning satellite (GPS) tracking were found to be least acceptable. Students rated several systems as more effective than professionals, but the overall pattern of results for both types of subjects was similar. The data suggest that risk perception research and the psychometric paradigm are useful approaches for quantifying attitudes regarding homeland security systems and policies and can be used to anticipate potentially significant public acceptance issues.
Mental health, pain, and sleep factors associated with subjective cognitive difficulties in individuals with spinal cord injury.

Abstract:Objective: We examined the relationships between self-reported demographic, mental health, physical health, and health behavior factors and subjective cognitive difficulties in a unique cohort of individuals with spinal cord injury (SCI) who averaged more than 30 years postinjury at assessment. Method: The study involved cross-sectional analyses of self-report assessment data collected during the most recent data-collection period (2018–2019) of a 45-year multicohort longitudinal study. Participants (n = 553) were identified from a specialty hospital in the southeastern United States and two midwestern university hospitals and were initially enrolled in the study in 1973–1974, 1984–1985, 1993–1994, or 2003–2004. The Patient-Reported Outcomes Measurement Information System (PROMIS) Cognitive Function-Short Form 8a v2.0 was the main outcome measure used to assess subjective cognitive difficulties. Results: Bivariate analyses indicated that greater subjective cognitive difficulties were associated with being female, elevated anxiety and depressive symptoms, sleep disturbance, cardiovascular disease, worse pain, polypharmacy, worse self-rated diet, and tobacco use. A hierarchical linear regression analysis including sociodemographic and SCI-related variables (Step 1) and physical, mental health, and health variables (added in Step 2) explained 33% of the variance in subjective cognitive difficulties, F(16, 421) = 13.45, p ≤ .001. Depressive, anxiety, and pain symptoms; sleep disturbance; and injury level remained significant predictors of variance in subjective cognitive symptoms when considering all variables simultaneously. Conclusion: Improving mental health symptoms, pain experiences, and sleep difficulties may be promising to improve subjective cognitive symptoms in individuals with SCI. O-012 Psychosocial Functioning among Youth With Inflammatory Bowel Disease: Understanding the Roles of Medical Traumatic Stress, Cognitive Avoidance, and Physiological Arousal

Abstract:BACKGROUND:While most youth with pediatric inflammatory bowel disease (IBD) are psychologically healthy, a cohort of these patients is at risk for poor psychosocial function. Given the medical, social, and psychological complexity of these illnesses, the factors that may confer psychosocial risk are both diverse and numerous. This study aimed to (1) assess the psychosocial function of children and adolescents with IBD, and (2) identify factors that contribute to psychosocial function among youth with Crohn’s disease (CD) and ulcerative colitis (UC). METHODS:Data was collected prospectively from a cohort of 69 child and adolescent patients. Measures were completed online. Assessment included validated measures of psychosocial function (Pediatric Symptom Checklist), social connectedness (Social Connectedness Scale), perceived loneliness (UCLA Loneliness Scale), medical traumatic stress (Child Revised Impact of Events Scale), prior medical history, and current disease activity. RESULTS:Thirty-six participants (mean age 13.6 years; SD = 1.9; R = 11–18) completed the assessment measures. Sixty-nine percent of patients reported a diagnosis of CD. Fifty-eight percent of patients reported prior IBD-related hospitalizations and 28% endorsed prior IBD-related surgery. Eight percent of participants reported clinically significant psychosocial symptoms (Pediatric Symptom Checklist—M = 17.11; SD = 9.0; R = 2–33). Psychosocial function was significantly associated with symptoms of traumatic stress (r = 0.63; P < 0.01), perceived loneliness (r = 0.70; P < 0.01), and overall health rating (r = −0.36; P = 0.03). Thirty-three percent of participants reported clinically significant symptoms of traumatic stress secondary to their IBD diagnosis. These symptoms were unrelated to type of IBD, duration of disease, and current self-reported disease activity. When symptoms of traumatic stress were controlled for, the relationship between psychosocial function and overall health was no longer significant (r = −0.07; P = 0.69). Both symptoms of avoidance and arousal were significantly associated with psychosocial function (r = 0.54, P < 0.01; r = 0.70, P < 0.01) and moderated the relationship between psychosocial function and symptoms of traumatic stress. Youth with CD reported higher rates of avoidance (t (34) = 2.0, P = 0.05) than youth diagnosed with UC. Physiological arousal was significantly related to self-reported general health. CONCLUSIONS:Among children and adolescents with IBD, psychosocial function is significantly associated with symptoms of traumatic stress secondary to their medical diagnosis. The role and significance of these symptoms is noteworthy given their presence and magnitude within this patient population. One third of patients in this sample reported clinically significant symptoms of traumatic stress related to their IBD diagnosis. The moderating effect of post-traumatic symptoms suggests that psychological adjustment to illness may play a salient role in the psychosocial function of children and adolescents with IBD. Clinicians should be mindful that psychosocial wellbeing might be dependent on factors related to the child’s experience of illness rather than simply illness itself.

This paper describes experiments that separate the isopachics of a birefringent object by using the immersion method real-time polarization holography. The sign of the isopachics is accurately determined in real time by inserting a wave front retarder in the optical path of the illuminating beam and observing the shift of the fringe pattern.

Abstract:Background Patients with anxiety disorders suffer marked functional impairment in their activities of daily living. Many studies have documented that improvements in anxiety symptom severity predict functioning improvements. However, no studies have investigated how improvements in functioning simultaneously predict symptom reduction. We hypothesized that symptom levels at a given time point will predict functioning at the subsequent time point, and simultaneously that functioning at a given time point will predict symptom levels at a subsequent time point. Method Patients were recruited from primary-care centers for the Coordinated Anxiety Learning and Management (CALM) study and were randomized to receive either computer-assisted cognitive-behavioral therapy and/or medication management (ITV) or usual care (UC). A cross-lagged panel design examined the relationship between functional impairment and anxiety and depression symptom severity at baseline, 6-, 12-, and 18-month follow-up assessments. Results Prospective prediction of functioning from symptoms and symptoms from functioning were both important in modeling these associations. Anxiety and depression predicted functioning as strongly as functioning predicted anxiety and depression. There were some differences in these associations between UC and ITV. Where differences emerged, the UC group was best modeled with prospective paths predicting functioning from symptoms, whereas symptoms and functioning were both important predictors in the ITV group. Conclusions Treatment outcome is best captured by measures of functional impairment as well as symptom severity. Implications for treatment are discussed, as well as future directions of research.

Abstract:BackgroundThe majority of spinal cord injury (SCI) patients have urinary issues, such as incontinence, retention, and frequency. These problems place a significant burden on patients’ physical health and quality of life (QoL). There are a wide variety of bladder management strategies available to patients with no clear guidelines on appropriate selection. Inappropriate bladder management can cause hospitalizations and serious complications, such as urosepsis and renal failure. Patients believe that both independence and ability to carry out daily activities are just as important as physical health in selecting the right bladder-management strategy but little is known about patient’s QoL with different bladder managements. Our study’s aim is to assess patient reported QoL measures with various bladder managements after SCI. This manuscript describes the approach, study design and common data elements for our central study.MethodsThis is a multi-institutional prospective cohort study comparing three different bladder-management strategies (clean intermittent catheterization, indwelling catheters, and surgery). Information collected from participants includes demographics, past medical and surgical history, injury characteristics, current and past bladder management, and SCI /bladder-related complications. Patient reported outcomes and QoL questionnaires were administered at enrollment and every 3 months for 1 year. Prediction of Psychological Distress Among Persons With Spinal Cord Injury or Acquired Brain Injury and Their Significant Others

Abstract:Objectives: To identify intra- and interpersonal sociodemographic, injury-related, and psychological variables measured at admission of inpatient rehabilitation that predict psychological distress among dyads of individuals with spinal cord injury (SCI) or acquired brain injury (ABI) and their significant others (ie, individuals close to the individual with a disability, mostly family members) 6 months after discharge. Differences in predictors were investigated for persons with SCI or ABI and their significant others and were compared between diagnoses. Design: Prospective longitudinal study. Setting: Twelve Dutch rehabilitation centers. Participants: Dyads (N=157) consisting of adults with SCI or ABI who were admitted to inpatient rehabilitation and their adult significant others. Interventions: Not applicable. Main outcome measures: Psychological distress (Hospital Anxiety and Depression Scale). Results: Sociodemographic and injury-related variables were not or were only weakly associated with psychological distress among individuals with SCI or ABI and their significant others 6 months after discharge. Bivariately, higher baseline psychological distress, lower scores on adaptive psychological characteristics (combination of self-efficacy, proactive coping, purpose in life, resilience), and higher scores on maladaptive psychological characteristics (combination of passive coping, neuroticism, appraisals of threat and loss) were related to higher psychological distress, as well as crosswise between individuals with SCI or ABI and their significant others, although less strongly. Combined prediction models showed that psychological distress among persons with SCI or ABI was predicted by education level of their significant other, their own baseline psychological distress, and their own maladaptive psychological characteristics (explained variance, 41.9%). Among significant others, only their own baseline psychological distress predicted psychological distress (explained variance, 40.4%). Results were comparable across diagnoses. Conclusions: Although a dyadic connection was shown, primarily one's own baseline psychological distress and psychological characteristics were important in the prediction of later psychological distress among both individuals with SCI or ABI and their significant others. Screening based on these variables could help to identify persons at risk for psychological distress.

Abstract:Study design: Cross-sectional study. Objectives: (1) To assess the relationship between bladder management methods and the health-related quality of life (HRQL) in patients with spinal cord injury (SCI). (2) To identify any correlation between the two questionnaires used to assess the quality of life (one validated for SCI and one validated for bladder symptoms). Setting: Spinal Cord Injury Centre, Royal National Orthopaedic Hospital, Middlesex, UK. Methods: This study is based on two questionnaires with results collected from 142 people with SCI. The two questionnaires were based on information from the Short-Form 36-Item Health Survey (SF-36) and the King's Health Questionnaire and included demographic characteristics, bladder management methods and the frequency of incontinence. Results: There is a moderate correlation between the results of the SF-36 and the King's Health Questionnaire. Only 21% SCI patients report normal voiding without any other form of bladder management. The type of bladder management may influence the HRQL in patients with SCI. Clean intermittent catheterization by attendant, indwelling transurethral catheterization and indwelling suprapubic catheterization are the three groups with the worst mental status. In addition, the frequency of incontinence is a strong influence on HRQL. Conclusions: The results of this study may provide a general baseline HRQL for patients with SCI. Our findings show the relationships between bladder management methods and quality of life in patients with SCI. In addition, the impact of incontinence on quality of life was also confirmed.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>To conduct a conjoint analysis experiment to better understand the psychosocial priorities related to bladder management in individuals after spinal cord injury (SCI).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>We developed a conjoint analysis survey that included 11 psychosocial attributes phrased in the context of bladder management (including attributes for urinary infections, and incontinence). We then performed a multi-center prospective cross-sectional study of adults with existing SCI which consisted of a baseline interview, followed by the online conjoint analysis survey (delivered through Sawtooth software). Hierarchical Bayes random effects regression analysis was used to determine the relative importance of the attributes.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>A total of 345 people complete the study. There was good representation of both men and women, and individuals with cervical and thoracic or lower lesions. The most important attribute was the frequency of urinary infections. Age, sex, and level of SCI were generally not related to the attributes measured in the study. In the subgroup of 256 patients who used a catheter for bladder management, significantly more importance was placed on urinary tract infections, time, fluid intake, and social life among indwelling catheter users compared to intermittent catheter users.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Most bladder-related psychosocial priorities are not impacted by a patient's age, sex or level of SCI. Differences in psychosocial priorities between indwelling and intermittent catheter users may represent factors that should be focused on to optimize bladder management after SCI.</p></section>

Investigating physiological symptoms associated with mental health symptoms in youth with cerebral palsy: An observational study

Abstract:Over 50 % of children and youth with cerebral palsy (CP) experience mental health challenges, with anxiety and depression most common. Youth with CP also experience several physiological symptoms such as fatigue, pain, sedentary lifestyle, and sleep disturbances that impact their daily living; however, little is known about the impact of these symptoms on mental health outcomes in these youth. This study addressed this gap and examined the individual and cumulative impacts of physiological symptoms on anxiety and depression symptoms in youth with CP. Forty youth with CP aged 8 to 18 years, and their caregiver, participated in this cross-sectional observational study. Pain, fatigue, anxiety, and depressive symptoms were measured using caregiver- and self-reported questionnaires and participants wore accelerometers for seven consecutive days, providing non-invasive physical activity and sleep pattern data. Youth with CP experienced substantial physiological symptoms and elevated anxiety and depression symptoms. Linear regression models determined that all physiological factors were predictive of caregiver-reported youth anxiety (R2 = 0.23) and youth depressive symptoms (R2 = 0.48). Fatigue, pain severity, sleep efficiency, and physical activity outcomes individually and cumulatively contributed to caregiver-reported youth anxiety and depression symptoms. These findings highlight the important role of physiological symptoms as potential risk factors and potential targets for intervention for mental health issues for in youth with CP.
Association between functional outcomes and psychological variables in persons with spinal cord injury

Abstract:We aimed to explore the association of functional outcomes with psychological variables, including depression, anxiety, sleep quality, and suicide risk, in persons with spinal cord injuries (SCIs). The secondary aim was to determine specific functions related to the psychological variables. This retrospective study included 259 persons with SCIs who were admitted to the Korean National Rehabilitation Center between 2019 and 2021. The participants were interviewed by a psychiatrist and completed questionnaires, including the Korean Beck Depression Inventory II (K-BDI-II), Korean Beck Anxiety Index, Insomnia Severity Index, and Mini International Neuropsychiatric Interview. To assess functional outcomes, the Spinal Cord Independence Measure III (SCIM III) and Walking Index for Spinal Cord Injury were determined by a physical therapist. The findings revealed a negative correlation of SCIM III subdivisions 1 and 3 with K-BDI-II. Specifically, feeding and mobility in bed and actions to prevent pressure injuries were functional factors associated with all four psychological variables. Our findings can guide clinicians to focus on improving functional independence and activities of daily living during the management of persons with SCI to prevent psychological consequences. Developing devices that aid in improving functional independence is crucial and may improve psychological problems in such individuals.
Child and Parent Risk and Resilience Factors as Predictors of Long-term Recovery in Youths Undergoing Spinal Fusion Surgery

Abstract:Objectives: Undertreated pediatric post-surgical pain negatively affects quality-of-life and functioning and may lead to chronic post-surgical pain (CPSP). Predictors of recovery have been identified but more research is needed, particularly regarding resilience and social factors, and long-term effects. The aim of the present study was to investigate child and parent risk and resilience factors as predictors of long-term post-surgical recovery for adolescents. Methods: Participants were patients with Adolescent Idiopathic Scoliosis (AIS),12-18 years, undergoing spinal fusion, and their parents. Recruitment occurred at the orthopaedic units at four hospitals in Belgium. Data were collected before surgery (T0), at three (T1) and six weeks (T2), six months (T3) and one year (T4) post-surgery. Multiple regression models were used to evaluate the predictive effect of pain intensity, pain catastrophizing, psychological flexibility, and pain acceptance on long-term functioning, quality-of-life, and pain. Results: The sample comprised 100 adolescents and 61 parents. Pain at T0, T1, and T3 and adolescent pain catastrophizing (T0) predicted quality-of-life, functioning, and pain at T4 (while pain at T2 predicted quality-of-life and pain). Parent pain catastrophizing predicted pain at T4. Adolescent and parental psychological flexibility predicted quality-of-life, and parent psychological flexibility also predicted pain at T4. Adolescent acceptance at T1 predicted pain, and acceptance at T2 predicted quality-of-life, at T4. Discussion: The study identified pain and adolescent pain catastrophizing as risk factors, and adolescent and parental psychological flexibility and adolescent pain acceptance as resilience factors, for long-term recovery in youths undergoing spinal fusion. Post-surgical pain management targeting these factors may therefore promote recovery for these adolescents.

Abstract:OBJECTIVE: Pediatric brain tumor survivors (PBTSs) are at risk for impairments in social adjustment and psychological distress. This study investigated longitudinal associations between symptoms of social withdrawal and anxiety/depression in PBTS, as well as medical, demographic, and personal characteristics that may also influence reports of social withdrawal and anxiety/depression.METHOD: About 91 PBTS (51% male, mean age 11.21 years, off-treatment) participated. At baseline and 8 months follow-up, primary caregivers of PBTS completed measures of social withdrawal, anxiety, and depression symptoms. Medical information (e.g. tumor type and location, cranial irradiation therapy) and child personal characteristics (e.g. child's age and gender, executive function, social skills) were obtained at baseline.RESULTS: Baseline reports of depression symptoms and social skills predicted social withdrawal 8 months later. Social withdrawal at baseline predicted greater combined anxiety and depression symptoms 8 months later. Depression alone predicted greater anxiety symptoms at follow-up. Anxiety symptoms and poor global executive functioning predicted greater depression symptoms at follow-up.CONCLUSIONS: The social adjustment and psychological distress of PBTSs are interrelated and can influence each other across time. These findings support the importance of multifaceted interventions targeting both psychological distress and social adjustment, in order to support the optimal psychosocial adjustment of PBTSs.
