Abstract:Objective To investigate the clinicopathologic features of uterine tumor resembling ovarian sex-cord tumor (UTROST) to avoid misdiagnosis.Methods The clinical findings,morphologic features,immunohistochemical markers and gene mutation analysis in one case of UTROSCT were studied.Results A 20-year-old student presented with abnormal vaginal bleeding for one year and anemia for three months.The tumor was about 12 cm in maximum diameter with yellow appearance.Microscopically,the tumor had cord,trabeculae,nest,solid,and poseudo-adenoid pattern;the tumor cells were epithelioid partly with foam-like cytoplasm,prominent nucleoli and mitotic figures (3/10 HPF).The immunohistochemical staining showed that tumor cells were positive for CK,CK8,CK19,vimentin,CD99,bcl-2,AR,ER and PR,focal positive for calretinin and CD56,but negative for Inhibin-α,Melan-A,WT-1,FOXL2,SF-1,S-100,PAX-8,CD10,CK7,CK20,CK5,CK14,CK34βE12,p16,EMA,HMB45,SMA,Desmin,Calponin,S-100,CD117,CD34,FLI-1,MC,CD133,CD44,CgA and Syn;P53 showed wild type and Ki-67 index number was about 5 percent.FOXL2 and DICER1 mutation were absent.The patient was alive after follow up for 4 months after hysterectomy.Conclusions UTROSCT is a very rare tumor and FOXL2 and DICER1 mutation might be absent.The definite diagnosis depends on morphological observation and immunohistochemical analysis.
Uterine tumors resembling ovarian sex cord tumors: A retrospective analysis of 7 cases from a single institution

Abstract:To investigate the clinicopathological features, diagnosis, surgical treatment and prognosis of uterine tumors similar to ovarian sex cord tumors (UTROSCT). The clinical data, surgical approach, histopathological, and immunohistochemical features of 7 cases of UTROSCTs were retrospectively reviewed and followed up. All 4 patients were premenopausal women. The most common clinical presentation was menorrhagia (n = 4) followed by postmenopausal lower abdominal mass (n = 2) and postmenopausal bleeding (n = 1). Gynecological ultrasonography suggested uterine fibroids in 4 cases, adenomyosis with uterine fibroids in 2 cases, and an intrauterine mass in 1 case. Pelvic MRI was performed preoperatively in only 2 cases, and both indicated uterine fibroid degeneration, including 1 patient with suspected malignancy. Preoperative serum tumor markers were measured in 6 patients, and only 1 patient had elevated CA125 levels, up to 158 U/mL. Total hysterectomy with bilateral adnexectomy or salpingectomy was the most common treatment pattern (n = 6). The tumors were located within the myometrium (n = 4), submucosa (n = 1), and isthmus to external cervical os (n = 1), with a range of 2 to 12 (mean = 8) cm. Edema and degeneration were observed in 2 cases, and necrosis in 1 case. Postoperative follow-up ranged from 31 to 82 (mean = 43) months. Unfortunately, 1 patient died at 54 months of follow-up without undergoing hysterectomy. The remaining 6 cases showed no tumor recurrence or metastasis after surgery. Histological examination revealed a tumor composed of epithelioid tumor-like cells arranged in cords, trabeculae, and nests. All 7 tumors showed expression of 2 sex cord differentiation markers. Furthermore, all tumors expressed the smooth muscle marker, while epithelial marker CK (4/7). endometrial stromal marker CD10(0/7). The Ki-67 proliferation index was found to be <5% (5/7). The option of total hysterectomy may be considered for women who do not have any fertility requirements. However, for young women who desire to maintain their reproductive capacity, surgery to preserve the uterus may be an alternative, although it necessitates careful postoperative monitoring. In terms of follow-up monitoring, MRI is more suitable than ultrasound. The diagnosis of UTROSCT heavily relies on histopathological examination and immunohistochemical analysis.

Abstract:We report a case of a 65‐year‐old postmenopausal female patient who presented with 1 day of vaginal bleeding. Imaging studies diagnosed a uterine tumor lesion, and the patient underwent a total hysterectomy and bilateral salpingo‐oophorectomy. The excised specimen was sent for pathological examination, and based on immunohistochemical analysis, the patient was ultimately diagnosed with Uterine tumor resembling ovarian sex cord tumor (UTROSCT). Postoperative adjuvant chemotherapy was administered, and the patient has been in good condition during the follow‐up period.

Abstract:Uterine tumor resembling ovarian sex cord tumor (UTROSCT) is a rare mesenchymal neoplasm, of uncertain lineage, that shows predominantly sex cord-like differentiation with a broad range of histologic appearances and polyphenotypic immunohistochemical features. Although generally having a favorable prognosis, a subset can recur/metastasize. Most recently, several studies of UTROSCT have described novel fusion genes involving ESR1 and GREB1 as the 5 partner, and NCOA1-3 as the 3 partner. Genotype and phenotype correlation has suggested that GREB1-rearranged tumors may have a higher tendency to behave aggressively. Herein, we report a UTROSCT with aggressive histologic features harboring a GREB1-NCOA2 fusion. A 51-yr-old woman presented with menometrorrhagia and progressive dysmenorrhea and was found to have a submucous uterine lesion by ultrasonography. Gross examination of the hysterectomy specimen showed an 8.5-cm, polypoid, soft, intracavitary mass. Histologic examination revealed a deeply invasive neoplasm composed of uniform round to plump spindle cells, arranged predominantly in diffuse sheets and fascicles and focally in anastomosing cords patterns. Groups of rhabdoid tumor cells were occasionally noted. Worrisome features, including increased mitotic figures (up to 3/10 high power fields), geographic necrosis, and lymphovascular invasion, were evident. Immunohistochemical analysis showed variable positivity for epithelial, smooth muscle, neuroendocrine, and sex cord markers, as well as hormone receptors. RNA sequencing revealed an in-frame fusion between exon 3 of GREB1 and exon 14 of NCOA2. Fluorescence in situ hybridization analyses confirmed rearrangements of both the GREB1 and NCOA2 loci. Our case lends further supports that GREB1-rearranged UTROSCTs frequently exhibit aggressive histological features with inconspicuous sex cord-like differentiation.
Clinicopathological features and molecular genetic changes in 17 cases of uterine tumor resembling ovarian sex cord tumor

Abstract:Uterine tumor resembling ovarian sex cord tumor (UTROSCT) is a rare mesenchymal neoplasm that was recently reported to exhibit recurrent NCOA1-3rearrangement with the most frequent partners ESR1 and GREB1. In this study, the clinicopathological characteristics of 17 UTROSCT cases were summarized; among them, the fusion genes of 12 cases were retrospectively analyzed by targeted RNA sequencing. The mean age of our cohort was 47 years (19-67 y). Although the majority of UTROSCTs had clear boundaries on gross examination, microscopic infiltration into the myometrium was observed in 82.4 % of cases. The tumor cells showed diffuse, trabecular, nested, reticular, pseudopapillary, hollow and solid tubular patterns, expressing sex cord, epithelial, and myogenic markers. Six fusion genes, including ESR1::NCOA3 (n = 4), ESR1::NCOA2 (n = 2), ESR1::CITED2 (n = 2), GREB1::NCOA2 (n = 2), GREB1::NCOA1 (n = 1), and GREB1::NCOA3 (n = 1), were identified. The fusion genes of the three cases with recurrence and metastasis were GREB1::NCOA2, ESR1::NCOA3, and ESR1::CITED2. All 3 cases of recurrent tumors showed infiltrative growth, with moderate to severe dysplasia of tumor cells and different degrees of rhabdomyoid differentiation. This is the first report of the ESR1::CITED2 fusion genes in UTROSCT, and one of the two patients had recurrence and metastasis. Compared with UTROSCT withESR1 rearrangement, UTROSCT with GREB1 rearrangement was more common in elderly patientsand was more likely to present with intramural masses, less sex cord differentiation, poor prognosis, and relapse and metastasis.

Abstract:Uterine tumor resembling ovarian sex-cord tumors (UTROSCT) is an extremely rare type of uterine tumor, and its clinical characteristics are not fully understood. A systematic literature search was conducted in PubMed and MEDLINE using the keywords, "uterine tumors resembling ovarian sex cord tumors", limited to case reports. Clinico-pathological characteristics and survival data were abstracted and evaluated for the analysis. Among 43 cases reporting UTROSCT, Type I (endometrial stromal tumors with sex cord-like elements, ESTSCLE) and Type II (classic UTROSCT) were reported in 5 (11.6%) and 17 (39.5%), respectively, and nearly half of reported UTROSCT did not subcategorize the histology pattern into Type I or II (unspecified, n=21, 48.8%). Mean age was 52.2. The two most common symptoms were postmenopausal vaginal bleeding (44.2%) and abnormal menstruation (39.5%). The majority underwent total hysterectomy with adnexectomy (65.1%) followed by hysterectomy alone (18.6%) and tumor resection alone (14.0%). Mean tumor size was 6.2cm, and extra-uterine spread was seen in 7.0%. By immunohistochemistry, calretinin expression was significantly correlated with CAM5.2, inhibin, and progesterone receptor expression (all, p<0.05). In survival analysis, disease-free survival (DFS) rates for all 43 cases at 1, 2, and 5 years for all cases were 97.0%, 92.7%, and 69.7%, respectively. Among recurrent cases, median time to recur was 24 months (range 9-48). Decreased DFS was significantly associated with pelvic pain (2-year rate, 81.8% versus 94.7%, p=0.006), histology subcategory (Type I versus II, 23.8% versus 100%, p=0.006), tumor size ≥10cm (75.0% versus 100%, p=0.046), cervical/extra-uterine metastasis (46.7% versus 100%, p=0.024), and lymphovascular space involvement (50% versus 100%, p=0.002). Treatment patterns were not statistically associated with DFS (hysterectomy, p=0.28; and adnexectomy, p=0.38). When histology patterns were examined, Type II disease was associated with less aggressive tumor behavior when compared to Type I disease: extra-uterine spread (Type I versus II, 40% versus 5.9%, p=0.007) and lymphovascular space invasion (50% versus 6.7%, p=0.012). Among 17 cases of Type II disease, disease recurrence was reported in 1 (5.9%) case at 3 years after the initial treatment. In conclusion, our study showed that UTROSCT was often not subcategorized. Because classic UTROSCT has a distinct clinical outcome and characteristic histological patterns when compared to ESTSCLE, distinguishing UTROSCT from ESTSCLE is an integral component of the diagnosis. While classic UTROSCT typically has a favorable prognosis, it has been known to develop a late recurrence. If risk factors for recurrence are absent, both hysterectomy and mass resection alone are possible options for management.
Effect of erythrocyte heat treatment on pulmonary vascular resistance.

Abstract:The effect of red blood cell deformability on the pulmonary vascular resistance was studied in isolated dog and rat lungs. Blood cells were incubated at 49 degrees C for 1 hr, to render them rigid. The resistance to blood flow in the lung was assessed either by calculating the pulmonary vascular resistance (PVR = arterial - venous pressure difference divided by flow rate) or by examining the vascular pressure-flow relationship for changes in slope and intercept. The resistance in the lung was first assessed during perfusion with normal blood and again during perfusion with rigid cells. The results showed that PVR in dog lungs increased by 15% during perfusion with heat-treated blood and that this increase in PVR was associated with a significant increase in the middle segment resistance (arterial-venous occlusion technique) and with an increase in critical closing pressure (pressure intercept of the pressure-flow curve). In contrast to the small effect in dog lungs, the PVR in rat lungs rose more than 400% during perfusion with heat-treated blood. The marked increase of PVR in rat lungs was prevented with papaverine (PVR increased only 58%), suggesting that vasoconstriction was a primary event in rat lungs. The rise in vascular resistance in rat lungs was further shown to be primarily due to the presence of rigid erythrocytes (RBC). The increase in PVR in the rat lungs was not due to mechanical obstruction of the vasculature but rather to constriction of arteries and veins (double occlusion technique). The conclusion from this study is that RBC deformability plays an important role in the pulmonary vasculature, primarily because of release of vasoactive substances and partially because of the potential mechanical obstruction of capillaries. These events are apparently species dependent and are attributed mostly to red blood cell deformability which decreases during heat treatment.
Uterine tumors mimicking ovarian sex cord tumors with rhabdoid differentiation: a clinicopathologic study of 4 cases: A case series analysis

Abstract:Rationale: Uterine tumors resembling ovarian sex cord tumors (UTROSCT) with rhabdoid features are uncommon mesenchymal neoplasms exhibiting diverse histological patterns, including significant rhabdoid morphology. A thorough comprehension of their clinicopathologic features is crucial for precise diagnosis and effective management. Patient concerns: This study presents 4 cases of UTROSCT with rhabdoid features, diagnosed in patients aged 31 to 58. Varied recurrence patterns were observed, including similar recurrent lesions to the primary tumors with subsequent mortality, initial invasion and lymph node metastasis, and presence of only primary tumor. Diagnoses: Histopathological examination revealed diverse morphological patterns, prominently featuring rhabdoid differentiation. Immunohistochemical analysis showed expression of hormone receptors, sex cord, smooth muscle, and epithelial markers, notably WT1, CD56, and CD99. Molecular analysis identified ESR1-NCOA2 fusions and ESR1 and NCOA2/3 rearrangements, indicating a potential association between these genetic alterations and extensive rhabdoid differentiation. Interventions: Various treatments were administered post-recurrence, including chemotherapy and targeted therapies. However, poor clinical outcomes were observed in all cases. Outcomes: Despite aggressive treatments, including chemotherapy and targeted therapies, poor clinical outcomes were observed, highlighting the aggressive nature of UTROSCT with significant rhabdoid differentiation. Lessons: This case series emphasizes the importance of detailed pathological reporting, comprehensive molecular testing, and thorough tumor staging in UTROSCT cases with rhabdoid features. Enhanced understanding of the clinicopathologic characteristics of UTROSCT with rhabdoid differentiation is crucial for accurate diagnosis, prognostication, and management strategies.

ETIOLOGICAL INVESTIGATION OF DEAFNESS IN NEONATES SCREENED IN A UNIVERSAL NEWBORN

Abstract:Purpose: to describe the results of etiology of deaf in neonates screened in a universal newborn hearing screening program. Methods: a descriptive, cross-sectional and prospective study. The study included all newborns diagnosed with hearing loss identified in a universal newborn hearing screening program from August 2003 to December 2006. The etiology of deaf was determined after detailed anamnesis performed by the otorhinolaryngologist; survey of serological tests for toxoplasmosis, rubella, cytomegalovirus, herpes, syphilis and HIV; tomography of the temporal bone and genetic tests. Results: 17 neonates were diagnosed with hearing loss in the period studied. 64.7% of cases presented as probable causes prenatal etiology, 29.4% perinatal causes and one child (5.9%) had unknown etiology. Of prenatal causes, 36.4% had confirmed genetic origin and 36.4% presumed etiology of heredity. We confirmed the presence of congenital infections in 18.2% of cases and one child (9%) had craniofacial anomalies as a possible etiology. The degree of hearing loss more frequently observed in the subjects studied was the profound (47.1%). Conclusion: the increased occurrence of etiologies in this study was of prenatal origin, followed by perinatal origin.
Uterine Tumor Resembling Ovarian Sex Cord Tumors: 23 Cases Indicating Molecular Heterogeneity With Variable Biological Behavior

Abstract:Uterine tumor resembling ovarian sex cord tumor (UTROSCT) is a rare mesenchymal neoplasm that mainly harbors NCOA1-3 rearrangements with partner genes ESR1 or GREB1 . Here, we explored 23 UTROSCTs by targeted RNA sequencing. The association between molecular diversity and clinicopathologic features was investigated. The mean age of our cohort was 43 years (23-65 y). Only 15 patients (65%) were originally diagnosed with UTROSCTs. Mitotic figures ranged from 1 to 7/10 high power fields, of primary tumors and increased from 1 to 9/10 high power fields in recurrent tumors. Five types of gene fusions were identified in these patients, including GREB1::NCOA2 (n=7), GREB1::NCOA1 (n=5), ESR1::NCOA2 (n=3), ESR1::NCOA3 (n=7), and GTF2A1::NCOA2 (n=1). To our knowledge, our group included the largest cohort of tumors with GREB1::NCOA2 fusions. Recurrences were most common in patients with GREB1::NCOA2 fusion (57%), followed by 40% ( GREB1::NCOA1 ), 33% ( ESR1::NCOA2 ), and 14% ( ESR1::NCOA3 ). The recurrent patient who harbored an ESR1::NCOA2 fusion was characterized by extensive rhabdoid features. Both of the recurrent patients who harbored GREB1::NCOA1 and ESR1::NCOA3 had the largest tumor sizes in their own gene alteration groups, and another recurrent GREB1::NCOA1 patient had extrauterine involvement. The GREB1 -rearranged patients were of older age, larger tumor size, and higher stage than non- GREB1 -rearranged patients ( P =0.004, 0.028, and 0.016, respectively). In addition, the GREB1 -rearranged tumors presented more commonly as intramural masses rather than non- GREB1 -rearranged tumors presenting as polypoid/submucosal masses ( P =0.021). Microscopically, nested and whorled patterns were frequently seen in GREB1- rearranged patients ( P =0.006). Of note, estrogen receptor expression was weaker than progesterone receptor in all 12 GREB1- rearranged tumors, whereas the similar staining intensity of estrogen receptor and progesterone receptor was observed in all 11 non- GREB1- rearranged tumors ( P <0.0001). This study demonstrated that UTROSCTs were present at a younger age in the Chinese population. The genetic heterogeneity of UTROSCTs was correlated with variable recurrence rate. Tumors with GREB1::NCOA2 fusions are more likely to recur compared with those with other genetic alterations.
Bcl-2 and MALT1 Genes are not involved in the oncogenesis of uterine tumors resembling ovarian sex cord tumors

Abstract:Uterine tumors resembling ovarian sex cord tumors (UTROSCT) are rare entities. They were described by Clement and Scully in 1976 who classified them into groups I and II. Group I comprises typical endometrial stromal neoplasms with focal areas resembling ovarian sex cord elements and group II are predominantly or completely composed of ovarian sex cord-like elements. We report a case of UTROSCT type II with cytogenetic analysis. The tumor occurred in a 76-year-old woman who presented with vaginal bleeding. The tumor was lobulated, firm, yellow and histologically composed of sex cord-like elements. Tumor cells expressed vimentin, CD10, CD99 and alpha-actin. Cytogenetic analysis in a previously reported case detected translocation t(4;18)(q21.1;q21.3) in the majority of cells. Bcl-2 and MALT1 genes are located at or near the translocation breakpoints, and the aim of this study was to determine whether these genes were involved in chromosomal translocation or tumorigenesis. We did not find IgH translocation or the most common MALT translocations. Bcl-2 was also not involved in this oncogenesis.

Abstract:Unclassified mixed germ cell-sex cord-stromal tumor (UMGC-SCST) is a rare ovarian neoplasm composed of germ cells and sex cord elements, which occurs in genetically and phenotypically normal women without the usual histological features seen in gonadoblastoma. Few cases have been reported in the literature so far. The age of presentation is more frequent in girls younger than 10 years of age, although it can also occur in adult women. It can be associated with isosexual pseudoprecocity. The preferred management is the resection of the gonad that contains the tumor and the conservation of the opposite ovary and tube. This is a case of a 14-year-old patient, with precocious puberty and normal phenotype, diagnosed with this kind of ovarian tumor. A fertility preserving surgery with the resection of the right ovarian tumor and tube was performed. The patient was classified as stage IA according to the 2014 International Federation of Gynecology and Obstetrics (FIGO). She received adjuvant chemotherapy with bleomycin-etoposide-cisplatin for three cycles. After a follow-up of 24 months, she was found to be asymptomatic and free of relapse.

Abstract:OBJECTIVE: Uterine tumours resembling ovarian sex cord tumours (UTROSCTs) are extremely rare. To date, most patients with UTROSCTs have undergone hysterectomy and had a benign clinical course. Fertility-preserving surgery should be considered because some patients with UTROSCTs are aged < 40 years. This paper reviews the treatment and prognosis for patients with UTROSCTs, with a focus on fertility.METHODS: PubMed, MEDLINE and Scopus were searched systematically for case reports and case series of UTROSCTs published in English from inception to December 2022, and initial treatment and recurrence rates were compared. The following data were extracted: age; symptoms; initial therapy; metastasis at diagnosis; disease-free survival (DFS); and recurrence.RESULTS: In total, 147 patients (72 studies) reporting the clinical course of UTROSCTs were analysed. The median age at diagnosis was 50 years, and 28 (19.0 %) patients were aged < 40 years. Most patients (n = 125, 85.0 %) underwent hysterectomy as the initial surgery, with a recurrence rate of 17.6 % (n = 22). The recurrence rate was 30 % (n = 6) in patients who underwent mass resection (n = 20). Among the 15 patients who underwent mass resection aged < 40 years, seven went on to achieve pregnancy (46.7 %) and six had successful deliveries (40.0 %). No significant differences in 5- and 10-year DFS were found between the hysterectomy and mass resection groups (p = 0.123 and 0.0612, respectively). Bilateral salpingo-oophorectomy in addition to hysterectomy was not significantly associated with 10-year DFS (p = 0.548).CONCLUSION: While total hysterectomy is the recommended treatment for UTROSCTs based on recurrence rates, mass resection is an acceptable treatment option for patients who wish to retain their childbearing potential. It is recommended that these women should plan for pregnancy and delivery as soon as possible after mass resection, and should undergo hysterectomy within 5 years.

Malignant Intra-Mesosalpinx Extra-Ovarian Sex-Cord Like Tumor: A Case Report

Abstract:Uterine sex cord-like tumor (USCLTs) is a type of very rare cancer. It's originally divided into two subtypes: type I and type II, and it is very similar with real sex cord tumor. A 47-year-old Chinese woman; 7 years after total hysterectomy, pelvic masses had been found for 6 years. Transabdominal bilateral adnexa resection, greater omentum resection and pelvic lymph node dissection were performed simultaneously. In the follow up visit, no trend of tumor recurrence showed, according to all previous gynecologic examinations. Eleven months later, abdominal radical right hemicolectomy was performed because of 'stool characters have changed for several years'. Immunohistochemical staining and hematoxylin and eosin (H&E) staining were performed for pathological study. This type of tumor is not a result of primary ovarian sex cord stromal tumor's metastasizing inside the mesosalpinx, neither Wolffian adnexal tumor, but an extra-ovarian sex-cord like stromal tumor that generated in mesosalpinx, which is reported here for the first time.
A 1.8 to 2.4 GHz stacked power amplifier implemented in 0.25/SPL mu/m CMOS SOS technology

Abstract:A fully-integrated power amplifier (PA) designed with 8 stacked transistors is implemented in a 0.25/SPL mu/m Ultra-CMOS Silicon-on-Sapphire (SOS) technology. The stacked Cascode configuration allows the PA to deliver high gain and high output power while maintaining the PA stability. At 2.2 GHz, the PA under a bias supply of 16 V (2V per transistor) measures a saturated output power PSAT of 28.5 dBm (0.7 Watt) and a linear gain of 21.7 dB with a peak power-added efficiency (PAE) and drain efficiency (DE) of 16% and 18.5%, respectively. PAE and DE increase to 25.5% and 29%, respectively, when the PA is biased with a 13 V power supply. In the frequency range of 1.8 to 2.4 GHz the PSAT was above 27.6 dBm.
Ovarian Microcystic Stromal Tumor with Significant Nestin Expression: A Unique Case

Abstract:Ovary microcystic stromal tumor (MCST) is an extremely rare subtype of sex cord-stromal neoplasm, and only 57 cases have been reported. We herein report a unique case of ovarian MCST with positive nestin expression in a 39-year-old Chinese woman. The tumor showed microcystic stromal histological structures and characteristically expressed the CD10, WT-1, and Ki67 proteins. A molecular analysis identified a point mutation (c.110C > T) in exon 3 of the CTNNB1 gene. To our knowledge, no report has described a case of ovarian MCST with positive staining for nestin protein. Our study provides new insights into the tumor biology of ovarian MCST.

Abstract:the technique of laparoscopic suturing is unclear. The time interval between hysterectomy and vault dehiscence is shorter in the laparoscopic group (Agdi et al. 2009). Precipitating factors for evisceration include sexual intercourse (Nopdonrattakoon 2010), vaginal instrumentation (Rubin et al. 2010; Codd et al. 2010) and raised intra-abdominal pressure (Cardosi et al. 1999). Vaginal atrophy and surgical devascularisation may play a significant role in the pathogenesis of this condition in postmenopausal women (Kowalski et al. 1996). Surgical alteration of the anatomical axis of the vagina has been implicated and the condition has been described following sacrocolpopexy (Minguez et al. 2011). Inadvertent shortening of the vagina during hysterectomy and making the vagina more vertical may predispose to evisceration as this leaves the vaginal apex abnormally unsupported over the urogenital hiatus in the pelvic diaphragm. Irradiation could be a predisposing factor (Kang et al. 2009) and laboratory studies have shown decreased quantity and quality of tissue repair cells as well as the suppression of proliferation and increased apoptosis in wounds following irradiation (Qu et al. 2003). Repair of the vault defect may be performed through a vaginal, abdominal or laparoscopic approach (Sinclair et al. 2010). A transabdominal approach is recommended when the viability of the bowel is in question. Synthetic mesh and porcine small intestinal mucosal matrix (Surgisis) have also been used for the repair of vaginal vault dehiscence, in particular when closure by approximation of the vault edges is not feasible (Ogutu et al. 2010; Jurus et al. 2009). There have been recent changes in gynaecological oncological practice that may increase the propensity to vaginal vault dehiscence, and these should increase surgeons’ awareness of vaginal evisceration. First, with advances in perioperative care and improved outcomes of treatment, surgery is now undertaken more frequently on older patients with multiple co-morbidities and advanced malignancy, and with higher risk of complications and impaired wound healing. Second, chemotherapy can have a detrimental effect on wound healing. For many years, primary debulking surgery before initiation of chemotherapy has been the standard of care for patients with advanced ovarian cancer. Recently, there has been a move towards neoadjuvant platinum-based chemotherapy with interval debulking surgery being scheduled midway during the treatment. It is uncertain if this practice could affect wound healing, as evidence of increased surgical complications with neo-adjuvant chemotherapy in other cancers is equivocal. A further consideration is that the wider adoption of laparoscopic approach to hysterectomy and the implementation of enhanced recovery programmes in gynaecological oncology surgery. Allowing early resumption of regular activities – including sexual intercourse – might contribute to the possibility of vaginal vault weakness and evisceration. There is also a tendency not to perform a routine pelvic or speculum examination for women in the postoperative period or prior to discharge. Thus, early vault weaknesses might go unnoticed until later dates. There is a variation in practice between centres as regards vaginal vault closure. In the authors’ centre, the vaginal vault is routinely closed following hysterectomy. A systematic review undertaken by Boesch and Umek (2009) suggests that leaving the vaginal vault open after vaginal or abdominal hysterectomy is as safe as closing it, in terms of wound healing. However, there was no comment on the risk and incidence of evisceration. When closing the vaginal vault, it is not clear if the choice of suture material makes a difference. Whether a delayed absorbable – or non-absorbable suture – might be advantageous in patients at higher risk of dehiscence, is an area that requires further research.

Abstract:Rationale:The incidence of severe hyperandrogenism associated with masculinity in women is very low. While rare and difficult to diagnose, androgen secreting tumors should be suspected in women with hyperandrogenism and hirsutism, especially in the postmenopausal population. Herein we present one case of ovarian Leydig cell tumor (LCT) with markedly elevated serum testosterone levels and frank hirsutism.Patient concerns:A 60-year-old woman, presented with increased hair growth and androgenic alopecia and the hormonal laboratory examination showed that she had elevated serum testosterone level and normal dehydroepiandrosterone sulfate (DHEAS), androstenedione, 17- hydroxyprogesterone, cortisol and thyroid stimulating hormone (TSH).Diagnoses:The diagnosis of possible testosterone secreting tumor was performed when pelvic computed tomography (CT) and magnetic resonance image (MRI) showed a right adnexal mass of 15mmx16mm indicative of sex cord- stromal tumors.Interventions:The patient received laparoscopic total abdominal hysterectomy and bilateral salpingo-oophorectomy.Outcomes:After operation, testosterone got back to the normal level and clinical symptoms subsided.Lessons:It is common that postmenopausal androgen excess is a state of relative or absolute androgen excess originating from the adrenal gland and/or ovaries. In either case, doctors need to assess such patients and exclude relatively rare potential causes of tumors. Any woman who has hirsutism or frank evidence of markedly increased testosterone should exclude this kind of possibility of androgen producing tumors. It is possible to determine the origin of androgen hypersecretion with the severity of symptoms, the extent of androgen excess, and the relevant imaging studies. Since LCT are rare ovarian sex-cord stromal tumors, it can be beneficial for diagnosis with careful research of patient history of the defeminization followed by virilization, and a CT and MRI image.
Non-Peutz-Jeghers syndrome associated ovarian sex cord tumor with annular tubules: a case report

Abstract:Objective: Sex cord tumors with annular tubules (SCTAT) are a rare subtype of sex cord stromal tumor of the ovary. An evidence-based management plan with follow-up evaluations is difficult to outline because of the rarity of these tumors. We describe the case of a premenarcheal patient with a SCTAT. Design: Case report. Setting: The patient was encountered during routine patient care process. Patient(s): The patient presented with a pelvic mass and precocious puberty. Her condition was diagnosed as SCTAT. Her clinical presentation was consistent with an estrogen-secreting tumor, resulting in early menarche and premature breast development. Inhibin and estradiol levels were markedly elevated preoperatively and normalized 5 weeks after surgical removal of the tumor. The preoperative computed tomography scan demonstrated a 12-cm abdominopelvic mass, which appeared to be mostly cystic. Intervention(s): The patient was treated surgically. She underwent laparotomy, right salpingo-oophorectomy, ipsilateral pelvic and paraaortic lymph node sampling, and partial omentectomy. Peritoneal biopsy samples were obtained from the abdomen and pelvis. Main outcome measure(s): The patient did well postoperatively. She is being observed with serial examinations and serum inhibin measurements. Result(s): Normalization of serum estradiol and inhibin along with cessation of menstruation were seen 5 weeks postoperatively, with persistence of morphologic signs of precocious puberty and advanced bone age at 11 months after the diagnosis. Conclusion(s): The diagnosis of SCTAT was established on final pathology examination based on morphologic features of the tumor microscopically and the marker expression profile on immunohistochemistry. Primary management was surgical.
Characterization of an Unusually Aggressive Ovarian Malignant Sex Cord Stromal Tumor in an Adolescent Female With Peutz-Jeghers Syndrome

Abstract:Abstract Peutz-Jeghers syndrome (PJS) is a rare autosomal dominant disorder with mutations in SKT11 (19p13.3), mucocutaneous hyperpigmentation, gastrointestinal hamartomatous polyposis, and increased cancer predisposition. A PJS-associated distinctive ovarian tumor is sex cord tumor with annular tubules (SCTAT), which is multifocal, microscopic, bilateral, and almost always clinically benign. Non-SCTAT ovarian tumors with malignant behavior are exceptionally rare. We describe a malignant, multiply recurrent, widely metastatic sex cord stromal tumor in PJS. Case Report: A 17-year-old female presented with new complaints of unintentional weight loss, alopecia, and abdominal fullness. She had a PJS diagnosis at age 7 with mucocutaneous pigmentation, gastrointestinal polyposis, and STK11 mutation. CT showed a 23-cm abdominopelvic mass with diffuse peritoneal metastases and malignant pleural effusion. CA-125 was elevated. CEA, CA 19-9, β-hCG, AFP, and LDH were normal. Laparoscopic biopsy pathology revealed a tumor composed of sheets and loose aggregates of mitotic spindled and round heterogenous eosinophilic and basophilic cells within fibromyxoid stroma. Rare tubules with eosinophilic cytoplasm and focal foamy cells were also seen. IHC was significant for inhibin, calretinin, CD56 and WT1 positivity, and cytokeratin and EMA negativity, consistent with poorly differentiated sex cord stromal tumor. Interestingly, EM showed neurosecretory dense core granules and neurite-like processes. After neoadjuvant therapy, the resected left adnexal mass was 25.5 cm, purple-gray/tan-red, heterogeneous, diffusely cystic, diffusely edematous, rubbery to spongy. In the left ovary were also found unassociated microscopic SCTAT foci. During adjuvant chemotherapy, the patient became pregnant. During the pregnancy, the patient developed liver and pelvic peritoneal recurrence and continued altered chemotherapy. She delivered a healthy late-preterm infant but also concurrently had diffuse peritoneal recurrence. She underwent two more cycles of adjuvant chemotherapy. Approximately 2 years after finishing adjuvant chemotherapy, she again developed peritoneal recurrences, which were resected, and partial chemotherapy response was seen on pathology. She is currently undergoing additional chemotherapy cycles 4 years since the initial diagnosis.
