Abstract:In recent years, with the development of artificial intelligence, deep learning model has achieved initial success in ECG data analysis, especially the detection of atrial fibrillation. In order to solve the problems of ignoring the correlation between contexts and gradient dispersion in traditional deep convolution neural network model, the hybrid attention-based deep learning network (HADLN) method is proposed to implement arrhythmia classification. The HADLN can make full use of the advantages of residual network (ResNet) and bidirectional long–short-term memory (Bi-LSTM) architecture to obtain fusion features containing local and global information and improve the interpretability of the model through the attention mechanism. The method is trained and verified by using the PhysioNet 2017 challenge dataset. Without loss of generality, the ECG signal is classified into four categories, including atrial fibrillation, noise, other, and normal signals. By combining the fusion features and the attention mechanism, the learned model has a great improvement in classification performance and certain interpretability. The experimental results show that the proposed HADLN method can achieve precision of 0.866, recall of 0.859, accuracy of 0.867, and F1-score of 0.880 on 10-fold cross-validation.

Abstract:Long-term Electrocardiogram (ECG) analysis has become a common means of diagnosing cardiovascular diseases. In order to reduce the workload of cardiologists and accelerate diagnosis, an automated patient-specific heartbeat classification method based on a customized convolutional neural network (CNN) is proposed in this paper. The parallel convolutional layers with kernels of different receptive fields in the network are responsible for extracting multi-spatial deep features of the heartbeats, and the channel-wise attention module is adopted to selectively emphasize the informative features, which are beneficial to distinguish different classes of beats. To facilitate the extraction and emphasis of the important features, each heartbeat is segmented and stacked to form the multi-channel network input according to the basic temporal characteristics of the main components (P wave, QRS complex, and T wave) in the ECG. Besides, to further improve the network generalization and achieve better performance on various ECG of new patients, a method of intra-record sample clustering is proposed to select the representative heartbeats to construct the training set. The proposed method classifies heartbeats into five classes (normal beat (N), supraventricular ectopic beat (SVEB), ventricular ectopic beat (VEB), fusion beat (F), and unclassifiable beat (Q)). Validated on the MIT-BIH arrhythmia database, our approach demonstrates performance superior to several state-of-the-art methods. In addition to an average accuracy and specificity of over 99%, this method achieves a sensitivity of 95.4% and a positive predictivity of 97.1% for VEB class, and a sensitivity of 81.1% and a positive predictivity of 90.0% for SVEB class. With high classification performance and pathological heartbeat detection accuracy, the proposed method is promising for clinical device applications.

Abstract:This paper presents an automated patient-specific ECG classification algorithm, which integrates long short-term memory (LSTM) and convolutional neural networks (CNN). While LSTM extracts the temporal features, such as the heart rate variance (HRV) and beat-to-beat correlation from sequential heartbeats, CNN captures detailed morphological characteristics of the current heartbeat. To further improve the classification performance, adaptive segmentation and re-sampling are applied to align the heartbeats of different patients with various heart rates. In addition, a novel clustering method is proposed to identify the most representative patterns from the common training data. Evaluated on the MIT-BIH arrhythmia database, our algorithm shows the superior accuracy for both ventricular ectopic beats (VEB) and supraventricular ectopic beats (SVEB) recognition. In particular, the sensitivity and positive predictive rate for SVEB increase by more than 8.2% and 8.8%, respectively, compared with the prior works. Since our patient-specific classification does not require manual feature extraction, it is potentially applicable to embedded devices for automatic and accurate arrhythmia monitoring.

Abstract:Nowadays, a series of social problems caused by cardiovascular diseases are becoming increasingly serious. Accurate and efficient classification of arrhythmias according to an electrocardiogram is of positive significance for improving the health status of people all over the world. In this paper, a new neural network structure based on the most common 12-lead electrocardiograms was proposed to realize the classification of nine arrhythmias, which consists of Inception and GRU (Gated Recurrent Units) primarily. Moreover, a new attention mechanism is added to the model, which makes sense for data symmetry. The average F1 score obtained from three different test sets was over 0.886 and the highest was 0.919. The accuracy, sensitivity, and specificity obtained from the PhysioNet public database were 0.928, 0.901, and 0.984, respectively. As a whole, this deep neural network performed well in the multi-label classification of 12-lead ECG signals and showed better stability than other methods in the case of more test samples.

Abstract:Cardiac arrhythmias are the leading cause of death and pose a huge health and economic burden globally. Electrocardiography (ECG) is an effective technique for the diagnosis of cardiovascular diseases because of its noninvasive and cost-effective advantages. However, traditional ECG analysis relies heavily on the clinical experience of physicians, which can be challenging and time-consuming to produce valid diagnostic results. This work proposes a new hybrid deep learning model that combines convolutional neural network (CNN) and bidirectional gated recurrent unit (BiGRU) with multi-head attention (CBGM model). Specifically, the model consists of seven convolutional layers with varying filter sizes (4, 16, 32, and 64) and three pooling layers, respectively, while the BiGRU module includes two layers with 64 units each followed by multi-head attention (8-heads). The combination of CNN and BiGRU effectively captures spatio-temporal features of ECG signals, with multi-head attention comprehensively extracted global correlations among multiple segments of ECG signals. The validation in the MIT-BIH arrhythmia database achieved an accuracy of 99.41%, a precision of 99.15%, a specificity of 99.68%, and an F1-Score of 99.21%, indicating its robust performance across different evaluation metrics. Additionally, the model's performance was evaluated on the PTB Diagnostic ECG Database, where it achieved an accuracy of 98.82%, demonstrating its generalization capability. Comparative analysis against previous methods revealed that our proposed CBGM model exhibits more higher performance in automatic classification of arrhythmia and can be helpful for assisting clinicians by enabling real-time detection of cardiac arrhythmias during routine ECG screenings.

Abstract:Early diagnosis of abnormal electrocardiogram (ECG) signals can provide useful information for the prevention and detection of arrhythmia diseases. Due to the similarities in Normal beat ( N ) and Supraventricular Premature Beat ( S ) categories and imbalance of ECG categories, arrhythmia classification cannot achieve satisfactory classification results under the inter-patient assessment paradigm. In this paper, a multi-path parallel deep convolutional neural network was proposed for arrhythmia classification. Furthermore, a global average RR interval was introduced to address the issue of similarities between N vs. S categories, and a weighted loss function was developed to solve the imbalance problem using the dynamically adjusted weights based on the proportion of each class in the input batch. The MIT-BIH arrhythmia dataset was used to validate the classification performances of the proposed method. Experimental results under the intra-patient evaluation paradigm and inter-patient evaluation paradigm showed that the proposed method could achieve better classification results than other methods. Among them, the accuracy, average sensitivity, average precision, and average specificity under the intra-patient paradigm were 98.73%, 94.89%, 89.38%, and 98.24%, respectively. The accuracy, average sensitivity, average precision, and average specificity under the inter-patient paradigm were 91.22%, 89.91%, 68.23%, and 95.23%, respectively.

Abstract:Electrocardiogram (ECG) is a simple non-invasive measure to identify heart-related issues such as irregular heartbeats known as arrhythmias. While artificial intelligence and machine learning is being utilized in a wide range of healthcare related applications and datasets, many arrhythmia classifiers using deep learning methods have been proposed in recent years. However, sizes of the available datasets from which to build and assess machine learning models is often very small and the lack of well-annotated public ECG datasets is evident. In this paper, we propose a deep transfer learning framework that is aimed to perform classification on a small size training dataset. The proposed method is to fine-tune a general-purpose image classifier ResNet-18 with MIT-BIH arrhythmia dataset in accordance with the AAMI EC57 standard. This paper further investigates many existing deep learning models that have failed to avoid data leakage against AAMI recommendations. We compare how different data split methods impact the model performance. This comparison study implies that future work in arrhythmia classification should follow the AAMI EC57 standard when using any including MIT-BIH arrhythmia dataset.

Abstract:Arrhythmia is one of the common cardiovascular diseases. Nowadays, many methods identify arrhythmias from electrocardiograms (ECGs) by computer-aided systems. However, computer-aided systems could not identify arrhythmias effectively due to various the morphological change of abnormal ECG data. This paper proposes a deep method to classify ECG samples. Firstly, ECG features are extracted through continuous wavelet transform. Then, our method realizes the arrhythmia classification based on the new lightweight context transform blocks. The block is proposed by improving the linear content transform block by squeeze-and-excitation network and linear transformation. Finally, the proposed method is validated on the MIT-BIH arrhythmia database. The experimental results show that the proposed method can achieve a high accuracy on arrhythmia classification.

Abstract:Cardiovascular diseases have become more and more prominent in recent years, which have proven to be a major threat to people's health. Accurate detection of arrhythmia in patients has important implications for clinical treatment. The aim of this study was to propose a novel automatic classification method for arrhythmia in order to improve classification accuracy. The electrocardiogram (ECG) signal was subjected preprocessing for denoising purposes using a wavelet transform. Then, the local and global characteristics of the beat, which contained RR interval features according with the clinical diagnosis criterion, morphology features based on wavelet packet decomposition and statistical features along with kurtosis coefficient, skewness coefficient and variance are exploited and fused. Meanwhile, the dimensionality of wavelet packet coefficients were reduced via principal component analysis (PCA). Finally, these features were used as the input of the random forest classifier to train the model and were then compared with the support vector machine (SVM) and back propagation (BP) neural networks. Based on 100,647 beats from the MIT-BIH database, the proposed method achieved an average accuracy, specificity and sensitivity of 99.08%, 99.00% and 89.31%, respectively, using the intra-patient beats, and 92.31%, 89.98% and 37.47%, respectively, using the inter-patient beats. Moreover, two classification schemes, namely, inter-patient and intra-patient scheme, were validated. Compared with the other methods referred to in this paper, the performance of the novel method yielded better results.
Res-BiANet: A Hybrid Deep Learning Model for Arrhythmia Detection Based on PPG Signal

Abstract:Arrhythmias are among the most prevalent cardiac conditions and frequently serve as a direct cause of sudden cardiac death. Hence, the automated detection of arrhythmias holds significant importance for assisting in the diagnosis of heart conditions. Recently, the photoplethysmography (PPG) signal, capable of conveying heartbeat information, has found application in the field of arrhythmia detection research. This work proposes a hybrid deep learning model, Res-BiANet, designed for the detection and classification of multiple types of arrhythmias. The improved ResNet and BiLSTM models are connected in parallel, and spatial and temporal features of PPG signals are extracted using ResNet and BiLSTM, respectively. Subsequent to BiLSTM, a multi-head self-attention mechanism was incorporated to enhance the extraction of global temporal correlation features over long distances. The model classifies five types of arrhythmia rhythms (premature ventricular contractions, premature atrial contractions, ventricular tachycardia, supraventricular tachycardia, and atrial fibrillation) and normal rhythm (sinus rhythm). Based on this foundation, experiments were conducted utilizing publicly accessible datasets, encompassing a total of 46,827 PPG signal fragments from 91 patients with arrhythmias. The experimental results demonstrate that Res-BiANet achieved exceptional classification performance, including an F1 score of 86.88%, overall accuracy of 92.38%, and precision, sensitivity, and specificity of 88.46%, 85.15%, and 98.43%, respectively. The outstanding performance of the Res-BiANet model suggests significant potential in supporting the auxiliary diagnosis of multiple types of arrhythmias.

Abstract:ECG signal is of great importance in the clinical diagnosis of various heart diseases. The abnormal origin or conduction of excitation is the electrophysiological mechanism leading to arrhythmia, but the type and frequency of arrhythmia is an important indicator reflecting the stability of cardiac electrical activity. In clinical practice, arrhythmic signals can be classified according to the origin of excitation, the frequency of excitation, or the transmission of excitation. Traditional heart disease diagnosis depends on doctors, and it is influenced by doctors' professional skills and the department's specialty. ECG signal has the characteristics of weak signal, low frequency, large variation, and easy to be interfered. In this investigation, an ECG anomaly automatic classification system based on the convolutional neural network is proposed. The training sets of the convolutional neural network are ECG beats extracted from the MIT-BIH database as training sets. A 36-layer convolutional neural network (CNN) is trained based on Caffe framework to classify ECG signals automatically. The experimental results show that it can reach or even exceed the level of a senior cardiologist in judging three diseases: FIB, AFL and IVR.

Abstract:Arrhythmia classification based on electrocardiogram (ECG) is crucial in automatic cardiovascular disease diagnosis. The classification methods used in the current practice largely depend on hand-crafted manual features. However, extracting hand-crafted manual features may introduce significant computational complexity, especially in the transform domains. In this study, an accurate method for patient-specific ECG beat classification is proposed, which adopts morphological features and timing information. As to the morphological features of heartbeat, an attention-based two-level 1-D CNN is incorporated in the proposed method to extract different grained features automatically by focusing on various parts of a heartbeat. As to the timing information, the difference between previous and post RR intervels is computed as a dynamic feature. Both the extracted morphological features and the interval difference are used by multi-layer perceptron (MLP) for classifing ECG signals. In addition, to reduce memory storage of ECG data and denoise to some extent, an adaptive heartbeat normalization technique is adopted which includes amplitude unification, resolution modification, and signal difference. Based on the MIT-BIH arrhythmia database, the proposed classification method achieved sensitivity Sen = 93.4% and positive predictivity Ppr = 94.9% in ventricular ectopic beat (VEB) detection, sensitivity Sen = 86.3% and positive predictivity Ppr = 80.0% in supraventricular ectopic beat (SVEB) detection, and overall accuracy OA = 97.8% under 6-bit ECG signal resolution. Compared with the state-of-the-art automatic ECG classification methods, these results show that the proposed method acquires comparable accuracy of heartbeat classification though ECG signals are represented by lower resolution.

Abstract:Group A streptococcus (GAS) is responsible for a number of diseases ranging from uncomplicated pharyngitis through to life-treating invasive and post-infectious diseases such as necrotizing fasciitis and rheumatic heart disease. GAS associated diseases occur globally and are serious problems in many developing nations and indigenous populations of many developed nations. This, and the resurgence in industrialized countries, and increased virulence of GAS in the 1980s highlight the need of cost-effective control strategies. Here we highlight the GAS diseases that are still a problem in many populations and discuss potentially useful strategies to combat GAS infections and disease.
Automated arrhythmia classification using depthwise separable convolutional neural network with focal loss

Abstract:<p>Arrhythmia was one of the primary causes of morbidity and mortality among cardiac patients. Early diagnosis was essential in providing intervention for patients suffering from cardiac arrhythmia. Convolution neural network (CNN) was widely used for electrocardiogram (ECG) classification. However, the conventional CNN method only worked well for balanced dataset. Therefore, a depthwise separable convolutional neural network with focal loss (DSC-FL-CNN) method was proposed for automated arrhythmia classification with imbalance ECG dataset. The focal loss contributed to improving the arrhythmia classification performances with imbalance dataset, especially for those arrhythmias with small samples. Meanwhile, the DSC-FL-CNN could reduce the number of parameters. The model was trained on the MIT-BIH arrhythmia database and it evaluated the performance of 17 categories of arrhythmia classification. Comparing with state-of-the-art methods, the experimental results showed that the proposed model reached an overall macro average F1-score with 0.79, which achieved an improvement for arrhythmia classification.</p>

Abstract:Our blood vessels show signs of aging as we grow older, which leads to various cardiovascular diseases. Arrhythmia is usually the symptom of patients with early cardiovascular diseases. Early detection of arrhythmia is of great significance to the mortality of cardiovascular diseases. Applying deep learning to arrhythmia detection can help doctors discover cardiovascular diseases in time. At present, the performance of arrhythmia classification algorithms based on convolutional neural networks has far surpassed traditional methods. However, the imbalance of arrhythmia data will seriously affect the performance of the classification algorithm. To better apply the convolutional neural network to the arrhythmia classification, a large amount of labeled ECG data is required. Therefore, this article proposes ECG Deep Convolution Generative Adversarial Networks (ECG-DCGAN) to expand the scarce data in the arrhythmia dataset and solve the problem of arrhythmia data imbalance. In addition, the convolution neural network (CNN) model is used to automatically classify the ECG signals without artificial feature extraction. Experimental results show that the classification method proposed in this paper improves the accuracy of arrhythmia diagnosis to 98.7% and that the algorithm used in this paper has good recognition performance and high clinical application value.

Abstract:Electrocardiogram (ECG) is widely used in the diagnosis of heart disease because of its noninvasiveness and simplicity. The time series signals contained in the signal are usually obtained by the professional medical staff and used for the classification of heartbeat diagnosis. Professional physicians can use the electrocardiogram to know whether the patient has serious congenital heart disease and whether there is an abnormal heart structure. A lot of work has been done to achieve automatic classification of arrhythmia types. For example, Autoencoder can obtain the time series characteristics of ECG signals and be used for ECG signal classification. However, some traditional methods are abstruse and difficult to understand in principle. In the classification of arrhythmias carried out in recent years, some researchers only use Autoencoder to provide structural characteristics, without giving too much explanation to the design reasons. Therefore, we optimized a new network layer design based on LSTM to obtain the autoencoder structure. This structure can cooperate with the ECG preprocessing process designed by us to obtain better arrhythmia classification effect. This method enables direct input of ECG signals into the model without complicated preprocessing such as manual parameter input. Also, it eliminates the gradient vanishing problem existing in traditional convolutional neural network. We used five different types of ECG data in MIT-BIH arrhythmia database and MIT-BIH supraventricular arrhythmia database: atrial premature beats (APB), left bundle branch block (LBBB), normal heartbeat (NSR), right bundle branch block (RBBB) and ventricular premature beats (PVC). High accuracy, precision and recall were obtained. Compared with traditional methods, this method has better performance in arrhythmia classification.

Abstract:Early detection of arrhythmia and effective treatment can prevent deaths caused by cardiovascular disease (CVD). In clinical practice, the diagnosis is made by checking the electrocardiogram (ECG) beat-by-beat, but this is usually time-consuming and laborious. In the paper, we propose an automatic ECG classification method based on Continuous Wavelet Transform (CWT) and Convolutional Neural Network (CNN). CWT is used to decompose ECG signals to obtain different time-frequency components, and CNN is used to extract features from the 2D-scalogram composed of the above time-frequency components. Considering the surrounding R peak interval (also called RR interval) is also useful for the diagnosis of arrhythmia, four RR interval features are extracted and combined with the CNN features to input into a fully connected layer for ECG classification. By testing in the MIT-BIH arrhythmia database, our method achieves an overall performance of 70.75%, 67.47%, 68.76%, and 98.74% for positive predictive value, sensitivity, F1-score, and accuracy, respectively. Compared with existing methods, the overall F1-score of our method is increased by 4.75~16.85%. Because our method is simple and highly accurate, it can potentially be used as a clinical auxiliary diagnostic tool.

Abstract:A generative adversarial network (GAN) based fully automatic electrocardiogram (ECG) arrhythmia classification system with high performance is presented in this paper. The generator (G) in our GAN is designed to generate various coupling matrix inputs conditioned on different arrhythmia classes for data augmentation. Our designed discriminator (D) is trained on both real and generated ECG coupling matrix inputs, and is extracted as an arrhythmia classifier upon completion of training for our GAN. After fine-tuning the D by including patient-specific normal beats estimated using an unsupervised algorithm, and generated abnormal beats by G that are usually rare to obtain, our fully automatic system showed superior overall classification performance for both supraventricular ectopic beats (SVEB or S beats) and ventricular ectopic beats (VEB or V beats) on the MIT-BIH arrhythmia database. It surpassed several state-of-art automatic classifiers and can perform on similar levels as some expert-assisted methods. In particular, the F1 score of SVEB has been improved by up to 10% over the top-performing automatic systems. Moreover, high sensitivity for both SVEB (87%) and VEB (93%) detection has been achieved, which is of great value for practical diagnosis. We, therefore, suggest our ACE-GAN (Generative Adversarial Network with Auxiliary Classifier for Electrocardiogram) based automatic system can be a promising and reliable tool for high throughput clinical screening practice, without any need of manual intervene or expert assisted labeling.

Abstract:Although convolutional neural networks (CNNs) can be used to classify electrocardiogram (ECG) beats in the diagnosis of cardiovascular disease, ECG signals are typically processed as one-dimensional signals while CNNs are better suited to multidimensional pattern or image recognition applications. In this study, the morphology and rhythm of heartbeats are fused into a two-dimensional information vector for subsequent processing by CNNs that include adaptive learning rate and biased dropout methods. The results demonstrate that the proposed CNN model is effective for detecting irregular heartbeats or arrhythmias via automatic feature extraction. When the proposed model was tested on the MIT-BIH arrhythmia database, the model achieved higher performance than other state-of-the-art methods for five and eight heartbeat categories (the average accuracy was 99.1% and 97%). In particular, the proposed system had better performance in terms of the sensitivity and positive predictive rate for V beats by more than 4.3% and 5.4%, respectively, and also for S beats by more than 22.6% and 25.9%, respectively, when compared to existing algorithms. It is anticipated that the proposed method will be suitable for implementation on portable devices for the e-home health monitoring of cardiovascular disease.
