Abstract:ObjectivesTo develop and evaluate the performance of a deep learning–based system for automatic patellar height measurements using knee radiographs.MethodsThe deep learning–based algorithm was developed with a data set consisting of 1018 left knee radiographs for the prediction of patellar height parameters, specifically the Insall-Salvati index (ISI), Caton-Deschamps index (CDI), modified Caton-Deschamps index (MCDI), and Keerati index (KI). The performance and generalizability of the algorithm were tested with 200 left knee and 200 right knee radiographs, respectively. The intra-class correlation coefficient (ICC), Pearson correlation coefficient, mean absolute difference (MAD), root mean square (RMS), and Bland-Altman plots for predictions by the system were evaluated in comparison with manual measurements as the reference standard.ResultsCompared with the reference standard, the deep learning–based algorithm showed high accuracy in predicting the ISI, CDI, and KI (left knee ICC = 0.91–0.95, r = 0.84–0.91, MAD = 0.02–0.05, RMS = 0.02–0.07; right knee ICC = 0.87–0.96, r = 0.78–0.92, MAD = 0.02–0.06, RMS = 0.02–0.10), but not the MCDI (left knee ICC = 0.65, r = 0.50, MAD = 0.14, RMS = 0.18; right knee ICC = 0.62, r = 0.47, MAD = 0.15, RMS = 0.20). The performance of the algorithm met or exceeded that of manual determination of ISI, CDI, and KI by radiologists.ConclusionsIn its current state, the developed system can predict the ISI, CDI, and KI for both left and right knee radiographs as accurately as radiologists. Training the system further with more data would increase its utility in helping radiologists measure patellar height in clinical practice.Key Points• Objective and reliable measurement of patellar height parameters is important for clinical diagnosis and the development of a treatment strategy.• Deep learning can be used to create an automatic patellar height measurement system based on knee radiographs.• The deep learning–based patellar height measurement system achieves comparable performance to radiologists in measuring ISI, CDI, and KI.

Abstract:Background: An increased posterior tibial slope (PTS) corresponds with an increased risk of graft failure after anterior cruciate ligament (ACL) reconstruction (ACLR). Validated methods of manual PTS measurements are subject to potential interobserver variability and can be inefficient on large datasets. Purpose/hypothesis: To develop a deep learning artificial intelligence technique for automated PTS measurement from standard lateral knee radiographs. It was hypothesized that this deep learning tool would be able to measure the PTS on a high volume of radiographs expeditiously and that these measurements would be similar to previously validated manual measurements. Study design: Cohort study (diagnosis); Level of evidence, 2. Methods: A deep learning U-Net model was developed on a cohort of 300 postoperative short-leg lateral radiographs from patients who underwent ACLR to segment the tibial shaft, tibial joint surface, and tibial tuberosity. The model was trained via a random split after an 80 to 20 train-validation scheme. Masks for training images were manually segmented, and the model was trained for 400 epochs. An image processing pipeline was then deployed to annotate and measure the PTS using the predicted segmentation masks. Finally, the performance of this combined pipeline was compared with human measurements performed by 2 study personnel using a previously validated manual technique for measuring the PTS on short-leg lateral radiographs on an independent test set consisting of both pre- and postoperative images. Results: The U-Net semantic segmentation model achieved a mean Dice similarity coefficient of 0.885 on the validation cohort. The mean difference between the human-made and computer-vision measurements was 1.92° (σ = 2.81° [P = .24]). Extreme disagreements between the human and machine measurements, as defined by ≥5° differences, occurred <5% of the time. The model was incorporated into a web-based digital application front-end for demonstration purposes, which can measure a single uploaded image in Portable Network Graphics format in a mean time of 5 seconds. Conclusion: We developed an efficient and reliable deep learning computer vision algorithm to automate the PTS measurement on short-leg lateral knee radiographs. This tool, which demonstrated good agreement with human annotations, represents an effective clinical adjunct for measuring the PTS as part of the preoperative assessment of patients with ACL injuries.
Automatic Knee Osteoarthritis Diagnosis from Plain Radiographs: A Deep Learning-Based Approach

Abstract:Knee osteoarthritis (OA) is the most common musculoskeletal disorder. OA diagnosis is currently conducted by assessing symptoms and evaluating plain radiographs, but this process suffers from subjectivity. In this study, we present a new transparent computer-aided diagnosis method based on the Deep Siamese Convolutional Neural Network to automatically score knee OA severity according to the Kellgren-Lawrence grading scale. We trained our method using the data solely from the Multicenter Osteoarthritis Study and validated it on randomly selected 3,000 subjects (5,960 knees) from Osteoarthritis Initiative dataset. Our method yielded a quadratic Kappa coefficient of 0.83 and average multiclass accuracy of 66.71% compared to the annotations given by a committee of clinical experts. Here, we also report a radiological OA diagnosis area under the ROC curve of 0.93. Besides this, we present attention maps highlighting the radiological features affecting the network decision. Such information makes the decision process transparent for the practitioner, which builds better trust toward automatic methods. We believe that our model is useful for clinical decision making and for OA research; therefore, we openly release our training codes and the data set created in this study.

Automated analysis of knee joint alignment using detailed angular values in long leg radiographs based on deep learning

Abstract:Malalignment in the lower limb structure occurs due to various causes. Accurately evaluating limb alignment in situations where malalignment needs correction is necessary. To create an automated support system to evaluate lower limb alignment by quantifying mechanical tibiofemoral angle (mTFA), mechanical lateral distal femoral angle (mLDFA), medial proximal tibial angle (MPTA), and joint line convergence angle (JLCA) on full-length weight-bearing radiographs of both lower extremities. In this retrospective study, we analysed 404 radiographs from one hospital for algorithm development and testing and 30 radiographs from another hospital for external validation. The performance of segmentation algorithm was compared to that of manual segmentation using the dice similarity coefficient (DSC). The agreement of alignment parameters was assessed using the intraclass correlation coefficient (ICC) for internal and external validation. The time taken to load the data and measure the four alignment parameters was recorded. The segmentation algorithm demonstrated excellent agreement with human-annotated segmentation for all anatomical regions (average similarity: 89–97%). Internal validation yielded good to very good agreement for all the alignment parameters (ICC ranges: 0.7213–0.9865). Interobserver correlations between manual and automatic measurements in external validation were good to very good (ICC scores: 0.7126–0.9695). The computer-aided measurement was 3.44 times faster than was the manual measurement. Our deep learning-based automated measurement algorithm accurately quantified lower limb alignment from radiographs and was faster than manual measurement.

Automating Linear and Angular Measurements for the Hip and Knee After Computed Tomography: Validation of a Three-Stage Deep Learning and Computer Vision-Based Pipeline for Pathoanatomic Assessment

Abstract:Background: Variability in the bony morphology of pathologic hips/knees is a challenge in automating preoperative computed tomography (CT) scan measurements. With the increasing prevalence of CT for advanced preoperative planning, processing this data represents a critical bottleneck in presurgical planning, research, and development. The purpose of this study was to demonstrate a reproducible and scalable methodology for analyzing CT-based anatomy to process hip and knee anatomy for perioperative planning and execution. Methods: One hundred patients with preoperative CT scans undergoing total knee arthroplasty for osteoarthritis were processed. A two-step deep learning pipeline of classification and segmentation models was developed that identifies landmark images and then generates contour representations. We utilized an open-source computer vision library to compute measurements. Classification models were assessed by accuracy, precision, and recall. Segmentation models were evaluated using dice and mean Intersection over Union (IOU) metrics. Contour measurements were compared against manual measurements to validate posterior condylar axis angle, sulcus angle, trochlear groove-tibial tuberosity distance, acetabular anteversion, and femoral version. Results: Classifiers identified landmark images with accuracy of 0.91 and 0.88 for hip and knee models, respectively. Segmentation models demonstrated mean IOU scores above 0.95 with the highest dice coefficient of 0.957 [0.954-0.961] (UNet3+) and the highest mean IOU of 0.965 [0.961-0.969] (Attention U-Net). There were no statistically significant differences for the measurements taken automatically vs manually (P > 0.05). Average time for the pipeline to preprocess (48.65 +/- 4.41 sec), classify/retrieve landmark images (8.36 +/- 3.40 sec), segment images (<1 sec), and obtain measurements was 2.58 (+/- 1.92) minutes. Conclusions: A fully automated three-stage deep learning and computer vision-based pipeline of classification and segmentation models accurately localized, segmented, and measured landmark hip and knee images for patients undergoing total knee arthroplasty. Incorporation of clinical parameters, like patient-reported outcome measures and instability risk, will be important considerations alongside anatomic parameters.
Automatic Diagnosis and Grading of Patellofemoral Osteoarthritis from the Axial Radiographic View: a Deep Learning-Based Approach

Abstract:Background Patellofemoral osteoarthritis (PFOA) has a high prevalence and is assessed on axial radiography of the patellofemoral joint (PFJ). A deep learning (DL)-based approach could help radiologists automatically diagnose and grade PFOA via interpreting axial radiographs. Purpose To develop and assess the performance of a DL-based approach for diagnosing and grading PFOA on axial radiographs. Material and Methods A total of 1280 (dataset 1) axial radiographs were retrospectively collected and utilized to develop the high-resolution network (HRNet)-based classification models. The ground truth was the interpretation from two experienced radiologists in consensus according to the K-L grading system. A binary-class model was trained to diagnose the presence (K-L 2 similar to 4) or absence (K-L 0 similar to 1) of PFOA. A multi-class model was used to grade the stage of PFOA, i.e. from K-L 0 to K-L 4. Model performances were evaluated using the receiver operating characteristics (ROC), confusion matrix, and the corresponding evaluation metrics (positive predictive value [PPV], negative predictive value [NPV], F1 score, sensitivity, specificity, accuracy) of the internal test set (n = 129) from dataset 1 and an external validation set (dataset 2, n = 187). Results For the binary-class model, the area under the curve (AUC) was 0.91 in the internal test set and 0.90 in the external validation set. For grading PFOA, moderate to severe stage of PFOA exhibited a good performance in these two datasets (AUC = 0.91-0.98, PPV = 0.69-0.90, NPV = 0.92-0.99, F1 score = 0.72-0.87, sensitivity = 0.75-0.87, specificity = 0.90-0.99, accuracy = 0.87-0.98). Conclusion The HRNet-based approach performed well in diagnosing and grading radiographic PFOA, especially for the moderate to severe cases.
MedKnee: A New Deep Learning-Based Software for Automated Prediction of Radiographic Knee Osteoarthritis

Abstract:In computer-aided medical diagnosis, deep learning techniques have shown that it is possible to offer performance similar to that of experienced medical specialists in the diagnosis of knee osteoarthritis. In this study, a new deep learning (DL) software, called "MedKnee" is developed to assist physicians in the diagnosis process of knee osteoarthritis according to the Kellgren and Lawrence (KL) score. To accomplish this task, 5000 knee X-ray images obtained from the Osteoarthritis Initiative public dataset (OAI) were divided into train, valid, and test datasets in a ratio of 7:1:2 with a balanced distribution across each KL grade. The pre-trained Xception model is used for transfer learning and then deployed in a Graphical User Interface (GUI) developed with Tkinter and Python. The suggested software was validated on an external public database, Medical Expert, and compared with a rheumatologist's diagnosis on a local database, with the involvement of a radiologist for arbitration. The MedKnee achieved an accuracy of 95.36% when tested on Medical Expert-I and 94.94% on Medical Expert-II. In the local dataset, the developed tool and the rheumatologist agreed on 23 images out of 30 images (74%). The MedKnee's satisfactory performance makes it an effective assistant for doctors in the assessment of knee osteoarthritis.

Abstract:A fully-automated deep learning algorithm matched performance of radiologists in assessment of knee osteoarthritis severity in radiographs using the Kellgren-Lawrence grading system. To develop an automated deep learning-based algorithm that jointly uses Posterior-Anterior (PA) and Lateral (LAT) views of knee radiographs to assess knee osteoarthritis severity according to the Kellgren-Lawrence grading system. We used a dataset of 9739 exams from 2802 patients from Multicenter Osteoarthritis Study (MOST). The dataset was divided into a training set of 2040 patients, a validation set of 259 patients and a test set of 503 patients. A novel deep learning-based method was utilized for assessment of knee OA in two steps: (1) localization of knee joints in the images, (2) classification according to the KL grading system. Our method used both PA and LAT views as the input to the model. The scores generated by the algorithm were compared to the grades provided in the MOST dataset for the entire test set as well as grades provided by 5 radiologists at our institution for a subset of the test set. The model obtained a multi-class accuracy of 71.90% on the entire test set when compared to the ratings provided in the MOST dataset. The quadratic weighted Kappa coefficient for this set was 0.9066. The average quadratic weighted Kappa between all pairs of radiologists from our institution who took a part of study was 0.748. The average quadratic-weighted Kappa between the algorithm and the radiologists at our institution was 0.769. The proposed model performed demonstrated equivalency of KL classification to MSK radiologists, but clearly superior reproducibility. Our model also agreed with radiologists at our institution to the same extent as the radiologists with each other. The algorithm could be used to provide reproducible assessment of knee osteoarthritis severity.

Abstract:The assessment of knee osteoarthritis (KOA) severity on knee X-rays is a central criteria for the use of total knee arthroplasty. However, this assessment suffers from imprecise standards and a remarkably high inter-reader variability. An algorithmic, automated assessment of KOA severity could improve overall outcomes of knee replacement procedures by increasing the appropriateness of its use. We propose a novel deep learning-based five-step algorithm to automatically grade KOA from posterior-anterior (PA) views of radiographs: (1) image preprocessing (2) localization of knees joints in the image using the YOLO v3-Tiny model, (3) initial assessment of the severity of osteoarthritis using a convolutional neural network-based classifier, (4) segmentation of the joints and calculation of the joint space narrowing (JSN), and (5), a combination of the JSN and the initial assessment to determine a final Kellgren-Lawrence (KL) score. Furthermore, by displaying the segmentation masks used to make the assessment, our algorithm demonstrates a higher degree of transparency compared to typical "black box" deep learning classifiers. We perform a comprehensive evaluation using two public datasets and one dataset from our institution, and show that our algorithm reaches state-of-the art performance. Moreover, we also collected ratings from multiple radiologists at our institution and showed that our algorithm performs at the radiologist level. Bacterial cell division: a moveable feast.

Abstract:Recent advances in fluorescence microscopy technologies have revolutionized our views on bacterial cell organization. A prolific area of this revolution in tiny cell visualization has been cell division. Most bacteria divide in their middle, implying that they are aware of at least one particular site of themselves, the midcell division site. This raises the fundamental question: How do bacterial cells identify midcell? It all started with the immunolocalization studies of the FtsZ division protein, which showed that FtsZ assembles at midcell in a ring-like structure (1). Since then, additional cell division proteins have been shown to localize to midcell (2). In a recent issue of the Proceedings , Raskin and de Boer (3) extend this concept of site-specific protein targeting by describing the unique dynamic properties of the localization of the MinD division inhibitor. This discovery adds a new dimension to our exploration of the unexpected complexity in the spatial organization of bacterial cells. One cannot talk about bacterial cell division without considering the FtsZ protein, which shares properties with cytoskeletal molecules. FtsZ, which binds GTP and has a GTPase activity (4–6), plays a central role in cytokinesis as a major component of a contractile ring (1). The assembly of the FtsZ ring at midcell occurs well before the constriction is initiated (1). In addition to ftsZ (7), many more genes are specifically involved in cell division (2, 8). The other cell division proteins are later recruited to the FtsZ ring to form the membrane-associated septal ring that mediates septation. The answer to the problem of midsite selection is still largely unknown, but important insights have come from studies on minicell-producing ( min ) mutants (9). Automatic segmentation of knee CT images of tibial plateau fractures based on three-dimensional U-Net: Assisting junior physicians with Schatzker classification

Abstract:Purpose: This study aimed to automatically segment knee computed tomography (CT) images of tibial plateau fractures using a three-dimensional (3D) U-net-based method, accurately construct 3D maps of tibial plateau fractures, and examine their usefulness for Schatzker classification in clinical practice. Methods: We retrospectively enrolled 234 cases with tibial plateau fractures from our hospital in this study. The four constituent bones of the knee were manually annotated using ITK-SNAP software. Finally, image features were extracted using deep learning. The usefulness of the results for Schatzker classification was examined by an orthopaedic and a radiology resident. Results: On average, our model required < 40 s to process a 3D CT scan of the knee. The average Dice coefficient for all four knee bones was higher than 0.950, and highly accurate 3D maps of the tibia were produced. With the aid of the results of our model, the accuracy, sensitivity, and specificity of the Schatzker classification of both residents improved. Conclusions: The proposed method can rapidly and accurately segment knee CT images of tibial plateau fractures and assist residents with Schatzker classification, which can help improve diagnostic efficiency and reduce the workload of junior doctors in clinical practice.
Comparison of tibial alignment parameters based on clinically relevant anatomical landmarks : a deep learning radiological analysis

Abstract:Aims: Accurate identification of the ankle joint centre is critical for estimating tibial coronal alignment in total knee arthroplasty (TKA). The purpose of the current study was to leverage artificial intelligence (AI) to determine the accuracy and effect of using different radiological anatomical landmarks to quantify mechanical alignment in relation to a traditionally defined radiological ankle centre. Methods: Patients with full-limb radiographs from the Osteoarthritis Initiative were included. A sub-cohort of 250 radiographs were annotated for landmarks relevant to knee alignment and used to train a deep learning (U-Net) workflow for angle calculation on the entire database. The radiological ankle centre was defined as the midpoint of the superior talus edge/tibial plafond. Knee alignment (hip-knee-ankle angle) was compared against 1) midpoint of the most prominent malleoli points, 2) midpoint of the soft-tissue overlying malleoli, and 3) midpoint of the soft-tissue sulcus above the malleoli. Results: A total of 932 bilateral full-limb radiographs (1,864 knees) were measured at a rate of 20.63 seconds/image. The knee alignment using the radiological ankle centre was accurate against ground truth radiologist measurements (inter-class correlation coefficient (ICC) = 0.99 (0.98 to 0.99)). Compared to the radiological ankle centre, the mean midpoint of the malleoli was 2.3 mm (SD 1.3) lateral and 5.2 mm (SD 2.4) distal, shifting alignment by 0.34o (SD 2.4o) valgus, whereas the midpoint of the soft-tissue sulcus was 4.69 mm (SD 3.55) lateral and 32.4 mm (SD 12.4) proximal, shifting alignment by 0.65o (SD 0.55o) valgus. On the intermalleolar line, measuring a point at 46% (SD 2%) of the intermalleolar width from the medial malleoli (2.38 mm medial adjustment from midpoint) resulted in knee alignment identical to using the radiological ankle centre. Conclusion: The current study leveraged AI to create a consistent and objective model that can estimate patient-specific adjustments necessary for optimal landmark usage in extramedullary and computer-guided navigation for tibial coronal alignment to match radiological planning.Cite this article: Bone Jt Open 2022;3(10):767-776.

Abstract:Knee pain is undoubtedly the most common musculoskeletal symptom that impairs quality of life, confines mobility and functionality across all ages. Knee pain is clinically evaluated by routine radiographs, where the widespread adoption of radiographic images and their availability at low cost, make them the principle component in the assessment of knee pain and knee pathologies, such as arthritis, trauma, and sport injuries. However, interpretation of the knee radiographs is still highly subjective, and overlapping structures within the radiographs and the large volume of images needing to be analyzed on a daily basis, make interpretation challenging for both naive and experienced practitioners. There is thus a need to implement an artificial intelligence strategy to objectively and automatically interpret knee radiographs, facilitating triage of abnormal radiographs in a timely fashion. The current work proposes an accurate and effective pipeline for autonomous detection, localization, and classification of knee joint area in plain radiographs combining the You Only Look Once (YOLO v3) deep convolutional neural network with a large and fully-annotated knee radiographs dataset. The present work is expected to stimulate more interest from the deep learning computer vision community to this pragmatic and clinical application.

Abstract:Background: Assessment of long leg radiographs (LLRs) in pediatric orthopedic patients is an important but time-consuming routine task for clinicians. The goal of this study was to evaluate the performance of artificial intelligence (AI)-based leg angle measurement assistant software (LAMA) in measuring LLRs in pediatric patients, compared to traditional manual measurements. Methods: Eligible patients, aged 11 to 18 years old, referred for LLR between January and March 2022 were included. The study comprised 29 patients (58 legs, 377 measurements). The femur length, tibia length, full leg length (FLL), leg length discrepancy (LLD), hip-knee-ankle angle (HKA), mechanical lateral distal femoral angle (mLDFA), and mechanical medial proximal tibial angle (mMPTA) were measured automatically using LAMA and compared to manual measurements of a senior pediatric orthopedic surgeon and an advanced practitioner in radiography. Results: Correct landmark placement with AI was achieved in 76% of the cases for LLD measurements, 88% for FLL and femur length, 91% for mLDFA, 97% for HKA, 98% for mMPTA, and 100% for tibia length. Intraclass correlation coefficients (ICCs) indicated moderate to excellent agreement between AI and manual measurements, ranging from 0.73 (95% confidence interval (CI): 0.54 to 0.84) to 1.00 (95%CI: 1.00 to 1.00). Conclusion: In cases of correct landmark placement, AI-based algorithm measurements on LLRs of pediatric patients showed high agreement with manual measurements.
Automatic detection and classification of knee osteoarthritis using deep learning approach

Abstract:PurposeWe developed a tool for locating and grading knee osteoarthritis (OA) from digital X-ray images and illustrate the possibility of deep learning techniques to predict knee OA as per the Kellgren-Lawrence (KL) grading system. The purpose of the project is to see how effectively an artificial intelligence (AI)-based deep learning approach can locate and diagnose the severity of knee OA in digital X-ray images.MethodsSelection criteria: Patients above 50 years old with OA symptoms (knee joint pain, stiffness, crepitus, and functional limitations) were included in the study. Medical experts excluded patients with post-surgical evaluation, trauma, and infection from the study. We used 3172 Anterior–posterior view knee joint digital X-ray images. We have trained the Faster RCNN architecture to locate the knee joint space width (JSW) region in digital X-ray images and we incorporate ResNet-50 with transfer learning to extract the features. We have used another pre-trained network (AlexNet with transfer learning) for the classification of knee OA severity. We trained the region proposal network (RPN) using manual extract knee area as the ground truth image and the medical experts graded the knee joint digital X-ray images based on the Kellgren-Lawrence score. An X-ray image is an input for the final model, and the output is a Kellgren-Lawrence grading value.ResultsThe proposed model identified the minimal knee JSW area with a maximum accuracy of 98.516%, and the overall knee OA severity classification accuracy was 98.90%.ConclusionsToday numerous diagnostic methods are available, but tools are not transparent and automated analysis of OA remains a problem. The performance of the proposed model increases while fine-tuning the network and it is higher than the existing works. We will extend this work to grade OA in MRI data in the future.

Abstract:Purpose Multifaceted factors contribute to inferior outcomes following anterior cruciate ligament (ACL) reconstruction surgery. A particular focus is placed on the posterior tibial slope (PTS). This study introduces the integration of machine learning and artificial intelligence (AI) for efficient measurements of tibial slopes on magnetic resonance imaging images as a promising solution. This advancement aims to enhance risk stratification, diagnostic insights, intervention prognosis and surgical planning for ACL injuries. Methods Images and demographic information from 120 patients who underwent ACL reconstruction surgery were used for this study. An AI‐driven model was developed to measure the posterior lateral tibial slope using the YOLOv8 algorithm. The accuracy of the lateral tibial slope, medial tibial slope and tibial longitudinal axis measurements was assessed, and the results reached high levels of reliability. This study employed machine learning and AI techniques to provide objective, consistent and efficient measurements of tibial slopes on MR images. Results Three distinct models were developed to derive AI‐based measurements. The study results revealed a substantial correlation between the measurements obtained from the AI models and those obtained by the orthopaedic surgeon across three parameters: lateral tibial slope, medial tibial slope and tibial longitudinal axis. Specifically, the Pearson correlation coefficients were 0.673, 0.850 and 0.839, respectively. The Spearman rank correlation coefficients were 0.736, 0.861 and 0.738, respectively. Additionally, the interclass correlation coefficients were 0.63, 0.84 and 0.84, respectively. Conclusion This study establishes that the deep learning‐based method for measuring posterior tibial slopes strongly correlates with the evaluations of expert orthopaedic surgeons. The time efficiency and consistency of this technique suggest its utility in clinical practice, promising to enhance workflow, risk assessment and the customization of patient treatment plans. Abstract:Background Knee cartilage is the most crucial structure in the knee, and the reduction of cartilage thickness is a significant factor in the occurrence and development of osteoarthritis. Measuring cartilage thickness allows for a more accurate assessment of cartilage wear, but this process is relatively time-consuming. Our objectives encompass using various DL methods to segment knee cartilage from MRIs taken with different equipment and parameters, building a DL-based model for measuring and grading knee cartilage, and establishing a standardized database of knee cartilage thickness. Methods In this retrospective study, we selected a mixed knee MRI dataset consisting of 700 cases from four datasets with varying cartilage thickness. We employed four convolutional neural networks—UNet, UNet++, ResUNet, and TransUNet—to train and segment the mixed dataset, leveraging an extensive array of labeled data for effective supervised learning. Subsequently, we measured and graded the thickness of knee cartilage in 12 regions. Finally, a standard knee cartilage thickness dataset was established using 291 cases with ages ranging from 20 to 45 years and a Kellgren–Lawrence grading of 0. Results The validation results of network segmentation showed that TransUNet performed the best in the mixed dataset, with an overall dice similarity coefficient of 0.813 and an Intersection over Union of 0.692. The model’s mean absolute percentage error for automatic measurement and grading after segmentation was 0.831. The experiment also yielded standard knee cartilage thickness, with an average thickness of 1.98 mm for the femoral cartilage and 2.14 mm for the tibial cartilage. Conclusion By selecting the best knee cartilage segmentation network, we built a model with a stronger generalization ability to automatically segment, measure, and grade cartilage thickness. This model can assist surgeons in more accurately and efficiently diagnosing changes in patients’ cartilage thickness.

Abstract:Knee osteoarthritis (OA) is the most common joint disorder and a leading cause of disability. Diagnosing OA severity typically requires expert assessment of X-ray images and is commonly based on the Kellgren-Lawrence grading system, a time-intensive process. This study aimed to develop an automated deep learning model to classify knee OA severity, reducing the need for expert evaluation. First, we evaluated ten state-of-the-art deep learning models, achieving a top accuracy of 0.69 with individual models. To address class imbalance, we employed weighted sampling, improving accuracy to 0.70. We further applied Smooth-GradCAM++ to visualize decision-influencing regions, enhancing the explainability of the best-performing model. Finally, we developed ensemble models using majority voting and a shallow neural network. Our ensemble model, KneeXNet, achieved the highest accuracy of 0.72, demonstrating its potential as an automated tool for knee OA assessment.
