Abstract:People with fast eating habits have been reported to have an increased risk of diabetes and obesity. To explore whether the speed of eating a test meal (tomato, broccoli, fried fish, and boiled white rice) influences postprandial blood glucose, insulin, triglyceride, and free fatty acid levels, 18 young, healthy women consumed a 671 kcal breakfast at fast speed (10 min) and slow speed (20 min) with vegetables first and slow speed (20 min) with carbohydrate first on three separate days. This study was conducted using a within-participants cross-over design in which all participants consumed identical meals of three different eating speeds and food orders. Significant ameliorations of both fast and slow eating with vegetables first regimen on postprandial blood glucose and insulin levels at 30 and 60 min were observed compared with those of slow eating with carbohydrates first. In addition, the standard deviation, large amplitude of excursion, and incremental area under the curve for blood glucose and insulin in both fast and slow eating with vegetables first were all significantly lower than those of slow eating with carbohydrate first. Interestingly, there was no significant difference between fast and slow eating on postprandial blood glucose and insulin levels as long as vegetables were consumed first, although postprandial blood glucose at 30 min was significantly lower in slow eating with vegetables first than that of fast eating with the same food order. These results suggest that food order with vegetables first and carbohydrate last ameliorates postprandial blood glucose and insulin concentrations even if the meal was consumed at fast speed.

Abstract:The aim of this review was to evaluate whether eating vegetables before carbohydrates could reduce the postprandial glucose, insulin, and improve long-term glycemic control in Japanese patients with type 2 diabetes. We studied the effect of eating vegetables before carbohydrates on postprandial plasma glucose, insulin, and glycemic control for 2.5 y in patients with type 2 diabetes. The postprandial glucose and insulin levels decreased significantly when the patients ate vegetables before carbohydrates compared to the reverse regimen, and the improvement of glycemic control was observed for 2.5 y. We also compared the postprandial glucose and glucose fluctuations assessed by continuous glucose monitoring system for 72-h in patients with type 2 diabetes and subjects with normal glucose tolerance when subjects ate vegetables before carbohydrates and carbohydrates before vegetables in a randomized crossover design. The glycemic excursions and incremental glucose peak were significantly lower when the subjects ate vegetables before carbohydrates compared to the reverse regimen. This evidence supports the effectiveness of eating vegetables before carbohydrates on glucose excursions in the short-term and glycemic control in the long-term in patients with type 2 diabetes.

Abstract:The purpose of this study was to examine how green leaf juice drinking affect the postprandial blood glucose. Postprandial hyperglycemia causes vascular endothelial damage and chronic inflammation, promoting atherosclerosis, regardless of the presence of diabetes. Some ingredients in greenleaf juice have been reported to suppress blood glucose levels; however, the effect of greenleaf juice on reducing blood glucose levels in healthy individuals is unclear. We observed changes in postprandial blood glucose levels in 13 healthy young women who drank greenleaf juice before or concomitantly with rice. Compared to water, greenleaf juice consumption reduced blood glucose levels at 90 and 120 min after rice consumption, with no difference regardless of the time of greenleaf juice consumption. Greenleaf juice may be one of the most convenient and cost-effective methods for reducing postprandial blood glucose in healthy people.

Abstract:Intestinal α-glucosidases are the key enzymes responsible for starch digestion and absorption and their inhibition has been proven effective in both preventing and treating diabetes through improvement of postprandial hyperglycaemia. This study, for the first time, identified that a Norton grape skin extract (GSE) significantly inhibited mammalian intestinal α-glucosidases but not other digestive enzymes including structurally relevant pancreatic α-amylase. Norton GSE inhibited rat intestinal α-glucosidases through a competitive mode with an IC50 of 0.384mg/ml. Further animal study revealed that the oral intake of Norton GSE (400mg/kg) significantly reduced postprandial blood glucose by 30.9% in the streptozocin-treated male C57BL/6J mice following starch challenge. These findings suggest that Norton GSE may have a unique property of suppressing postprandial blood glucose through a mechanism involving the inhibition of α-glucosidases, thereby providing a novel dietary opportunity for diabetes management.

Abstract:Although thorough chewing lowers postprandial glucose concentrations, research on the effectiveness of chewing vegetables in different forms on postprandial glucose metabolism remains limited. This study examined the effects of vegetables consumed in solid versus puree forms on postprandial glucose metabolism. Nineteen healthy young men completed two 180-min trials on separate days in a random order: the chewing trial involved the consumption of shredded cabbage with chewing and the non-chewing trial involved the consumption of pureed cabbage without chewing. Energy jelly was consumed immediately after the consumption of shredded or puree cabbage. Blood samples were collected at 0, 30, 45, 60, 90, 120 and 180 min. Circulating concentrations of glucose, insulin, total glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic peptide (GIP) concentrations were measured from the plasma. Although plasma glucose concentrations did not differ between the trials, the plasma insulin and GIP incremental area under the curve values were higher in the chewing than in the non-chewing trial. Postprandial total GLP-1 concentrations were higher in the chewing than in the non-chewing trial at 45, 60 and 90 min. This study demonstrates that consuming shredded cabbage while chewing enhances postprandial incretin secretion but has no effect on postprandial glucose concentration.

Abstract:Abstract Background The starch-removing rice cooker (SRR) is an innovative kitchen appliance which uses multiple water drainage techniques passing the rice to reduce starch from rice. Many manufacturers claimed their success in starch reduction from the rice. We hypothesized that the rice from SRR cooker might have a lower postprandial glucose than normal rice cooker. Objective To assess acute glycemic and insulin responses after ingesting rice from SRR versus normal rice cooker in healthy adults. Material and Methods We conducted a first pilot randomized crossover study to compare the postprandial glucose and insulin levels between rice from SRR versus normal rice cooker in healthy adults. This randomized crossover study was done in healthy volunteers aged 18 years and older. The participants were instructed to ingest 150 grams of rice which were randomly assigned to ingest rice from SRR or rice from normal rice cooker. Plasma glucose and insulin measurements were taken at 30, 60 and 120 minutes. After 1 week, the participants were cross over to another rice and plasma glucose and insulin levels were obtained as the same. The other metabolic parameters, satisfaction score and fullness were recorded to compare between SRR and normal rice. Results Plasma glucose at 30 minutes was significantly lower in the SRR group compared with the control group (120.45 ± 14.48 vs 137.27 ± 22.21; p <0. 05). And Likewise, Plasma glucose at 60 minutes was also significantly lower in the SRR group compared with the control group (110.82 ± 18. 08, 122.64 ± 21. 08; p <0. 05). However, There is no statistically significant difference in insulin and triglyceride levels between the SRR and control groups. Furthermore, there was no significant difference in appetite survey between SRR and control groups. Abstract:Background: Rice crackers are composed of carbohydrates refined from rice and enjoyed as a snack in Japan. Most rice crackers are crafted from white rice, and the potential postprandial blood glucose rise following their consumption may pose a clinical concern. Brown rice contains more dietary fiber than white rice and has been reported to suppress elevations of blood glucose. Dietary fiber-enhanced brown rice crackers have a significantly higher dietary fiber content than regular brown rice crackers and can be expected to suppress blood glucose elevations more reliably.Methods: We conducted a crossover trial in humans using white rice crackers and dietary fiber-enhanced brown rice crackers to investigate the effect of dietary fiber-enhanced brown rice crackers on postprandial blood glucose elevations. Participants ingested the 100 g rice crackers with 200 mL water for 10 min, and blood was collected from the fingertip at 7 time points: baseline, and at 15, 30, 45, 60, 90, and 120 min after ingestion of the rice cracker. The primary outcome measure was the blood glucose levels, and the secondary outcome measure was the incremental area under curve of blood glucose.Results: Glucose levels and incremental areas under curve at 60 min and 120 min after intake were significantly lower with dietary fiber-enhanced brown rice crackers than with white rice crackers. Incremental area under curves at 60 min and 120 min after intake of rice crackers were also significantly lower in dietary fiber-enhanced brown rice crackers than white rice crackers.Conclusions: Ingesting dietary fiber-enhanced brown rice crackers instead of white rice crackers could be useful for achieving better glycemic control.Keywords: Brown rice crackers, Dietary fiber, Postprandial blood glucose

Abstract:In a recent study, we showed that konjac glucomannan (KGM) inhibits rice gruel-induced postprandial increases in plasma glucose and insulin levels. To extend this research, we investigated the effects of KGM addition to rice gruel on pre- and postprandial concentrations of circulating lipoprotein lipase (LPL), glycosylphosphatidylinositol-anchored high-density lipoprotein-binding protein 1 (GPIHBP1), hepatic triglyceride lipase (HTGL), free fatty acids (FFA), and triglycerides (TG). A total of 13 Japanese men, without diabetes, dyslipidemia, or gastrointestinal diseases, interchangeably ingested rice gruel containing no KGM (0%G), rice gruel supplemented with 0.4% KGM (0.4%G), and rice gruel supplemented with 0.8% KGM (0.8%G), every Sunday for 3 weeks. Blood samples were obtained at baseline and at 30, 60, and 120 min after ingestion to measure the abovementioned lipid parameters. Lipid parameters showed small, but significant, changes. Significant reductions were found in circulating FFA levels among all participants. Circulating TG levels significantly declined at 30 min and then remained nearly constant in the 0.8%G group but exhibited no significant difference in the 0%G and 0.4%G groups. Although circulating levels of LPL and GPIHBP1 significantly decreased in the 0%G and 0.4%G groups, they increased at 120 min in the 0.8%G group. Participants in the 0%G and 0.4%G groups showed significant decreases in circulating HTGL levels, which was not observed in the 0.8%G group. Our results demonstrate the novel pleiotropic effects of KGM. Supplementation of rice gruel with KGM powder led to TG reduction accompanied by LPL and GPIHBP1 elevation and HTGL stabilization, thereby attenuating TG metabolism.

Effects of cooked rice containing high resistant starch on postprandial plasma glucose, insulin, and incretin in patients with type 2 diabetes

Abstract:Background and objectives: Few studies exist on resistant starch in rice grains. The Okinawa Institute of Science and Technology Graduate University (OIST) has developed a new rice (OIST rice, OR) rich in resistant starch. This study aimed to clarify the effect of OR on postprandial glucose concentrations. Methods and study design: This single-center, open, randomized, crossover comparative study included 17 patients with type 2 diabetes. All participants completed two meal tolerance tests using OR and white rice (WR). Results: The median age of the participants was 70.0 [59.0-73.0] years, and the mean body mass index was 25.9±3.1 kg/m2. The difference in total area under the curve (AUC) of plasma glucose was -8223 (95% confidence interval [CI]: -10100 to -6346, p<0.001) mg·min/dL. The postprandial plasma glucose was significantly lower with OR than with WR. The difference in the AUC of insulin was -1139 (95% CI: -1839 to -438, p=0.004) µU·min/mL. The difference in the AUC of total gastric inhibitory peptide (GIP) and total glucagon-like peptide-1 (GLP-1) was -4886 (95% CI: -8456 to -1317, p=0.011) and -171 (95% CI: -1034 to 691, p=0.673) pmol·min/L, respectively. Conclusions: OR can be ingested as rice grains and significantly reduced postprandial plasma glucose compared to WR independent of insulin secretion in patients with type 2 diabetes. OR could have escaped absorption not only from the upper small intestine but also from the lower small intestine.
Inter-Individual Variation in Postprandial Glycemic Responses in Women Co-Ingesting Green Leafy Vegetables with a Carbohydrate Meal: Interactions with the Sirtuin System

Abstract:Scope: Green leafy vegetables (GLV) may improve postprandial glycemic responses (PGR) and metabolic health. However, inter-individual variations (IIV) preclude conclusive evidence. Sirtuin system is emerging as a key player in blood glucose control. This study investigates IIV in PGR in women co-ingesting GLV with a carbohydrate meal and interactions with the sirtuin system. Methods and results: Volunteers (n = 31 women) consume rice, rice with bok choy, or spinach (75g available carbohydrate) on separate occasions. Postprandial glucose, insulin, adropin, and lipid levels are measured. Anthropometric measurements and sex hormones are measured. GeXP assay measures whole blood postprandial gene expression profiles of 25 markers involved in sirtuin signaling. GLV consumption has no significant effect on PGR, which shows high variation. PGR correlated with age, but no other consistent associations are observed. Sirtuin gene expression profiles reveal distinct stratified subgroups associated with PGR, lipid, insulin, fat mass, waist/hip circumferences, and adropin levels. Conclusion: PGR to co-ingesting GLV with a carbohydrate meal are highly variable in this cohort and fail to reveal a significant reduction in PGR. Variable responses are largely independent of menopausal status and meal consumed. However, lower expression of sirtuin gene targets is associated with higher PGR and with markers linked to health status.
Fruit and Vegetable Effect Changes in Blood Sugar Levels and Food Glycemic Index

Abstract:BACKGROUND: Foods consumed with fiber sources will be different from foods not consumed with fiber sources because fiber has a mechanism to lower blood sugar levels. However, the difference or the correlation factor is not yet known. AIM: This study aims to determine the differences in changes in blood sugar levels in the fruit or vegetable intervention and to determine the correction factor for changes in blood sugar levels and the glycemic index (GI) of food. METHODS: The research design used was experimental with a pre-post design; the sampling technique used purposive sampling totaling 13 people divided into two groups, namely, the fruit and vegetable group. The sample was selected under normal circumstances and had no history of diabetes mellitus. Blood sugar levels taken were fasting blood sugar levels at 0’, 30’, 60’, 90’, and 120’ after the intervention to see a graph of changes in blood sugar levels. The statistical analysis used is ANOVA/Kruskal–Wallis. RESULTS: The results showed a significant difference (p < 0.005) in comparing standard bread with bread and avocado and bread and beans at 30’ after the intervention. After the intervention, bread with watermelon showed significant results at 120’ and bread with carrots at 90’. The highest GI and the lowest correction factor for changes in blood sugar levels were shown in watermelon. In contrast, the lowest GI and the highest correction factor were shown in avocado. CONCLUSION: When compared between the four interventions, giving avocados were more able to lower blood sugar levels.

Abstract:The mechanism of the suppressive effect of rice albumin (RA) on postprandial hyperglycemia after glucose loading was investigated. In order to examine if small peptides derived from RA are responsible for prevention of postprandial hyperglycemia, RA was hydrolysed to indigestible high-molecular-weight peptides (HMP) and low-molecular-weight peptides (LMP) by trypsin, HMP and LMP being fractionated by gel-filtration chromatography. In oral glucose tolerance test (OGTT), both HMP and LMP suppressed the increase in blood glucose levels. In vitro diffusion test results showed that HMP adsorbed glucose and retarded its diffusion rate like dietary fibers. On the other hand, LMP suppressed the expression of sodium-dependent glucose transporter-1 (SGLT1) in STC-1 cells. Therefore, the suppressive effect of RA on postprandial hyperglycemia would be due to the dual actions of HMP and LMP. Our results demonstrated the potential use of RA as a functional food material to prevent diabetes.

Abstract:This study evaluates whether blood glucose response differs upon consuming rice cooked in a carbohydrate (carb)-reducing rice cooker. Rice cooked this way exhibited 19% reduced total carbohydrate (34.0 ± 0.3 vs. 27.6 ± 0.9 g/100 g rice) and 20% reduced total calorie (149.0 ± 1.0 vs. 120.8 ± 3.7 kcal/100 g rice) contents. We measured the blood glucose response (at 0, 15, 30, 45, 60, 90, and 120 min) in 13 healthy participants after consuming 6 different rice types: regular white rice (regular WR, 50 g of available carbohydrate (AC)), low-carb WR with equivalent weight as regular WR (low-carb WR (EW)), low-carb WR with equivalent carb as regular WR (low-carb WR (EC), regular mixed-grain rice (regular MR), low-carb MR (EW) as regular MR, and low-carb MR (EC) as regular MR. All rice samples were prepared in an electric carb-reducing rice cooker. Postprandial blood glucose, sensory, and appetite were assessed after each test meal. The incremental area under the curve of 15 and 30 min after rice consumption was significantly lower in low-carb WR (EW) than that in regular WR. These results suggest possible health benefits of low-carb WR in reducing early postprandial spikes in blood glucose level without significant differences in satiety and satisfaction.

Abstract:Regarding the circadian rhythm of glucose metabolism in humans and animals, glucose tolerance and insulin sensitivity show circadian rhythm with higher in the morning than in the evening. Blood glucose levels are higher in oral glucose tolerance tests or a mixed meal consumption test conducted in the afternoon and evening than in those conducted in the morning, whereas insulin secretions, especially early-phase of insulin secretion, are higher in the morning than in the evening. Epidemiological studies demonstrate that eating behaviors such as time of meal and number of chew food are associated with risk of obesity and type2 diabetes. Slow eating and chewing food well increase dietary induced thermogenesis (DIT) and appetite-reducing hormone secretions (PYY and GLP1), and decrease hunger hormone (ghrelin). In recently, we could reveal that the effect of mastication on postprandial glucose metabolism is dependent on the time of day and frequency of mastication. In this study, healthy male participants consumed a high carbohydrate food (200g rice) with 10 or 40 chews per mouthful at 0800 h and 2000 h. The results reveal that morning mastication but not evening decreases postprandial blood glucose concentrations and increases insulin secretion at 30 min and so-called the insulinogenic index as a marker of early-phase β-cell function. Clinically, impairment of early-phase insulin secretion is considered an early marker of β-cell dysfunction and the development of type 2 diabetes. Thus, the present findings suggest that morning mastication improves early-phase insulin secretion after rice consumption.

Abstract:Both glycemic index and dietary fiber are associated with cardiovascular disease risk, which may be related in part to postprandial vascular effects. We examined the effects of both glycemic index (GI) and dietary (mainly cereal) fiber on postprandial endothelial function. Eleven adults (5 men; 6 women; age = 42.4 ± 16.1 years; weight = 70.5 ± 10.7 kg; height = 173.7 ± 8.7 cm) consumed four different breakfast meals on separate, randomized occasions: High-Fiber, Low-GI (HF-LGI: Fiber = 20.4 g; GI = 44); Low-Fiber, Low-GI (LF-LGI: Fiber = 4.3 g; GI = 43); Low-Fiber, High-GI (LF-HGI: Fiber = 3.6 g; GI = 70); High-Fiber, High-GI (HF-HGI: Fiber = 20.3 g; GI = 71). Meals were equal in total kcal (~600) and macronutrient composition (~90 g digestible carbohydrate; ~21 g protein; ~15 g fat). The HF-LGI meal resulted in a significant increase in flow-mediated dilation (FMD) 4 hours after meal ingestion (7.8% ± 5.9% to 13.2% ± 5.5%; p = 0.02). FMD was not changed after the other meals. Regardless of fiber content, low-GI meals resulted in ~9% lower 4-hour glucose area under curve (AUC) (p < 0.05). The HF-LGI meal produced the lowest 4-hour insulin AUC, which was ~43% lower than LF-HGI and HF-HGI (p < 0.001), and 28% lower than LF-LGI (p = 0.02). We conclude that in healthy adults, a meal with low GI and high in cereal fiber enhances postprandial endothelial function. Although the effect of a low-GI meal on reducing postprandial glucose AUC was independent of fiber, the effect of a low-GI meal on reducing postprandial insulin AUC was augmented by cereal fiber.
Adequate Vegetable Intake Improves Metabolic Indices in Healthy Japanese Participants: A Randomized Crossover Study

Abstract:Aim: We aimed to elucidate the effect of a healthy diet containing adequate amounts of protein and vegetables on metabolic indices. Methods: In this randomized crossover study, twenty-two healthy Japanese participants ingested two different test meals: fish diet (F) or fish diet with adequate vegetable content (FV). Each 5-day diet load test was separated by a washout period of at least seven days. Metabolic indices were measured in fasting blood and 24-h urine samples. Results: The delta (Δ) plasma glucose and Δserum low-density lipoprotein (LDL) cholesterol concentrations were significantly larger in the participants in group FV than in group F (p=0.042, p=0.013, respectively). The urinary pH in participants in group F on day 6 was significantly lower than on day 1 (p=0.008), and the Δurinary pH and Δnet gastrointestinal absorption of alkali of participants in group FV tended to be smaller than in group F (p=0.070, p=0.075, respectively). Conclusions: This study showed that a healthy diet containing adequate protein and vegetables reduced the dietary acid load and improved plasma glucose and serum LDL concentrations in healthy Japanese participants.

Abstract:BackgroundLimited research has been conducted on the effect of intake of fruits and vegetables (FV) on hypertriglyceridemia (HTG), especially in Asia.MethodsWe conducted a cross‐sectional evaluation of 7934 adults aged 19–64 years from the fourth Korean Health and Nutrition Examination Survey. FV intake was estimated from food frequency questionnaires for the past year. Subgroups of FV included citrus/non‐citrus fruits, cruciferous/green leafy vegetables and carotene‐rich FV. HTG was defined as fasting blood triglycerides level ≥150mg/dl.ResultsThere were 2001(25.2%) cases of HTG. Total fruit intake was significantly inversely associated with the prevalence of HTG; the multivariate odds ratios (95% CIs) of HTG across increasing quintiles of total fruit intake were 1.00 (ref), 0.64 (0.53, 0.77), 0.67 (0.56, 0.80), 0.69 (0.58, 0.82), and 0.77 (0.65, 0.91). Similar inverse associations were found for citrus/non‐citrus fruits. However we found no significant association between intake of total vegetables/vegetable subgroups and HTG, the odds ratio for top vs. bottom quintile was 0.95 (0.79–1.13) for total vegetables intake.ConclusionsIntake of fruits and fruits subgroup (but not vegetables) was strongly associated with reduced prevalence of hypertriglyceridemia. Our findings support a potential beneficial role of fruit consumption to reduce blood triglyceride levels in Asian population.Grant Funding Source: None

Abstract:Objective: This study aimed to investigate the suppressive effect of a single dose of water-soluble α- glycosylated rutin (monoglucosyl rutin; MR) on postprandial blood glucose elevation in healthy subjects with relatively high fasting blood glucose levels.Methods: This randomized, placebo-controlled, double-blind crossover study enrolled 34 healthy Japanese adult subjects with relatively high fasting blood glucose levels. The study period ran from November 13, 2019, to March 19, 2020. All subjects were randomly allocated to either sequence A or sequence B (n = 17 per group) using a computerized random number generator. The washout period was at least one week between periods I and II. In period I, the subjects took either MR or placebo tablets. In period II, subjects took different tablets from the ones they had taken in period I. We evaluated their blood glucose and insulin levels after glucose loading (150 g of cooked rice). The incremental area under the curve (IAUC) of the postprandial blood glucose level was determined as the primary outcome. The blood glucose and insulin levels at maximum (maximum blood concentration; Cmax), each measurement point, and IAUC of the blood insulin level after glucose loading were the secondary outcomes.Results: Out of 33 subjects, 16 in sequence A (11 men and 5 women, 54.5 ± 9.8 years) and 17 in sequence B (9 men and 8 women, 58.8 ± 9.4 years) were analyzed as a per-protocol dataset. The glucose IAUC after MR consumption was significantly lower than that of the placebo (P = 0.034). Results of the other outcomes were not observed with significant treatment effects. There were no adverse events attributable to the test foods.Conclusions: We suggest that MR has a suppressive effect on the elevation of postprandial blood glucose in healthy adults with relatively high fasting blood glucose levels.Trial registration: UMIN-CTR: UMIN000038515. Abstract:We study robust pitch synchronous parameters that are derived from envelopeand instantaneous frequenciesestimated via a bank of cochlear filters. Closed set Speaker Identification experiments are performed on the SPIDRE corpus with matched and mismatchedhandsetsconditions. The recognizer is basedon a hybrid Linear Vector Quantization and Single Layer Perceptron (LVQSLP). Experiments are reported with different codebooksizes. In mismatched condition, the Mel Frequency Cepstral Coefficients (MFCC) yield slightly better rating (68%) than Envelope (58%) and Instantaneous Frequency (65%) parameters when used independently. When the MFCC based recognizer is used in conjunction with the envelope based recognizer, the recognition rate increases to 80%. We also report identification rates based on two classes: women and men. In another experiment, listeners were asked to discriminate speakerson a subset of ten females. We discuss their performance. We also discuss the potential of the approachand of judicious combination of the parameters to improve Speaker Identification Systems.
