Abstract:Introduction: MCL is a rare, aggressive subtype of B-cell non-Hodgkin lymphoma associated with a poor prognosis. A comprehensive understanding of the global epidemiology and economic and humanistic burden of MCL is needed to quantify the burden of disease in patients with MCL. Objective/Methods: To examine the burden of MCL, a systematic literature review was conducted to identify publications between the years 2010 to 2019 reporting on the epidemiology (incidence, prevalence, mortality, survival), economic (direct [e.g., drug costs] and indirect [e.g., lost productivity, missed days of work] costs and healthcare resource utilization [HRU; e.g., hospitalization, length of stay, emergency room use, outpatient visits]) and humanistic (e.g., quality of life [QOL], patient-reported outcome measures) burden of patients with MCL. Embase and Medline were searched via ProQuest and the Cochrane Controlled Register of Trials (CENTRAL) via the Cochrane Library. Results: A total of 2206 publications were identified, 177 full-text references screened, and 12 studies reporting epidemiology outcomes in MCL identified. Epidemiology data (1995-2016) were identified for the US (n = 5), UK (n = 2), Belgium (n = 1), France (n = 1), Netherlands (n = 1), Sweden/Denmark (n = 1), and Taiwan (n = 1). Incidence (n = 9) was lower in Taiwan (0.12 persons per 100,000) than in the US (0.8-1.15) and Europe (0.7-1.27). Six studies reported incidence over multiple time periods with 4 studies showing an increase in total incidence or incidence in men over time and 2 studies showing no change or a decrease in incidence in women over time. MCL prevalence was 3.0 per 100,000 persons in the UK (year 2011) and 3.6 per 100,000 persons in Belgium (year 2013). Mortality, reported in 1 US study, showed a consistent decrease from 1995-2013. Three-year all-cause mortality was 38% and MCL-related mortality 30%; 5-year all-cause mortality was 51% and MCL-related mortality 38%. Median overall survival (OS, n = 5) ranged from 28.8 (years 2004-2017; Europe) to 52.0 (years 1999-2013; US) months. Three-year OS (n = 2) was reported as 43.9% in the UK (2004-2017) and 61.0% in Sweden/Denmark (2006-2011). Increasing trends in survival over time were reported in 3 studies (US, Danish/Swedish, Netherlands; n = 1 each); increases in survival in the Netherlands were greatest in younger (age < 65 years) vs older patients (age ≥ 65 years). Twelve studies reported on the economic burden of MCL (Table). An increase in all costs and HRU, including outpatient encounters, emergency room visits, and days spent in the hospital, were found following vs pre MCL diagnosis. Length of hospital stays ranged from 3.0 to 8.3 days. All cause mean monthly costs for MCL patients were $5131 to $16,117 per patient month (PPM). Treatment cost varied with costs generally higher with chemoimmunotherapy and lower with targeted therapies. Adverse events (AEs) were associated with increased economic burden. Mean all-cause costs in patients with no AEs were $5131 PPM compared with $13,560 PPM in patients with ≥6 AEs. Patients experiencing 3-4 AEs during first-line therapy were almost 7-times more likely to require an inpatient admission than those experiencing only 1-2 AEs (OR, 6.9; 95% CI, 4.0-11.93). Patients with ≥6 AEs had inpatient stays lasting 2.2 days longer than those with no AEs. Common AE costs ranged from $5751 (atrial fibrillation) to $19,645 (hepatotoxicity). No studies reporting on indirect costs or the QOL impact of MCL or MCL treatment were identified. Conclusions: The increasing incidence and survival of patients with MCL along with the high costs of disease and treatment suggest an increasing economic burden. Though limited data are available on costs and HRU in populations with MCL and outcomes reported differed across studies, available data shows variability of costs among treatments influenced by AE costs. Some evidence suggests lower costs of therapy with targeted compared with chemoimmunotherapy regimens. The economic burden of MCL and MCL treatment on costs, beyond direct medical costs, needs to be quantified. Furthermore, the humanistic burden of MCL and impact on patient QOL warrants investigation. Additional epidemiology data are needed globally for MCL as well as the need for economic analyses that reflect the real-world treatment of patients with MCL. Treatment patterns and outcomes of older patients with mantle cell lymphoma in an Asian population

Abstract:Abstract Background Significant progress has been made in the treatment outcomes of mantle cell lymphoma (MCL) since the introduction of cytarabine and rituximab in modern regimens. However, older patients may not readily tolerate these agents nor derive benefit. We investigated the impact of age on treatment patterns and clinical outcomes of MCL patients in an Asian population. Methods A retrospective study was conducted on patients ( n = 66) diagnosed with MCL at the National Cancer Centre Singapore between 1998 and 2018. The median follow-up duration was 40 months. Survival analyses were performed using the Kaplan-Meier method and multivariate Cox proportional models. Results The median age of the cohort was 59 years (range, 26–84), with a male predominance (73%). The majority (86%) had advanced stage 3–4 disease at diagnosis. Compared with younger patients, older patients aged ≥60 years ( n = 32; 48.5%) presented more frequently with B-symptoms (75% vs 38%, p = 0.0028), anaemia (75% vs 35%, p = 0.0013), and carried higher prognostic risk scores (sMIPI high risk 84% vs 56%, p = 0.016). Non-cytarabine-based induction chemotherapy was more commonly administered in older patients (76% vs 32%, p = 0.0012). The 5-year overall survival (OS) and progression-free survival (PFS) was 68 and 25% respectively. In a multivariable model, older age (HR 3.42, 95%CI 1.48–7.92, p = 0.004) and anemia (HR 2.56, 95%CI 1.10–5.96, p = 0.029) were independently associated with poorer OS while older age (HR 2.24, 95%CI 1.21–4.14, p = 0.010) and hypoalbuminemia (HR 2.20, 95%CI 1.17–4.13, p = 0.014) were independently associated with poorer PFS. In an exploratory analysis, maintenance rituximab following induction chemotherapy improved PFS in younger patients, with median PFS of 131 months and 45 months with or without maintenance therapy respectively (HR 0.39, 95%CI 0.16–0.93, p = 0.035). In contrast, no survival benefit was observed in older patients. Conclusions We demonstrated in our analysis that older patients with MCL may harbor adverse clinical features and may not derive benefit from maintenance rituximab, highlighting the need for further research in this area of need.

Real-World Treatment Patterns, Survival, and Economic Burden Among Elderly MCL Patients Previously Treated With cBTKis

Abstract:Background: While covalent Bruton's tyrosine kinase inhibitors (cBTKis) have become a standard of care treatment for relapsed/refractory mantle cell lymphoma (R/R MCL), response duration is limited and resistance to BTKi and/or adverse events develop in a subset of patients. However, little real-world evidence on post-cBTKi clinical and economic outcomes exists for these patients. Patients and methods: This retrospective study used 2010 to 2019 U.S. Medicare claims, to identify elderly (≥ 66 years) patients with newly-diagnosed MCL who received third-line (3L) treatment and had evidence of cBTKi use in a prior line of therapy. Outcomes were assessed ≥ 12-months post 3L-treatment initiation and included treatment patterns, all-cause and MCL-related HRU and costs, and overall survival. Results: The final sample contained 230 elderly patients with R/R MCL receiving 3L treatment who had cBTKi use in a prior line of therapy (mean age 75.0, 21.7% age > 80 years; 67.4% male; 93.9% White). Common 3L treatments included chemotherapy (26.1%), lenalidomide (18.7%), and bortezomib (18.3%); 1-quarter (25.7%) of patients received a cBTKi (17.8% ibrutinib; 7.8% acalabrutinib). Overall survival was poor from 3L treatment initiation (median OS = 9.4 months; 1-years survival rate = 43.7%). Patients exhibited high rates of HRU (73.6% experienced hospitalization) and substantial costs ($145,726) in the 12-months after 3L initiation. Conclusion: A large unmet need exists in this patient subpopulation, highlighting the importance of ongoing development of novel therapeutics.
Impact of Diagnosis-to-Treatment Interval on Overall Survival in Patients with Mantle Cell Lymphoma: A National Cancer Database Analysis

Abstract:Abstract Introduction: The clinical presentation of mantle cell lymphoma (MCL) is heterogeneous, ranging from indolent disease that does not require immediate treatment to rapidly progressive disease that responds poorly to aggressive immunochemotherapy. Several small retrospective studies suggested that active surveillance or delayed treatment is feasible in select patients with an indolent disease course. A shorter diagnosis-to-treatment interval (DTI) is a strong predictor for inferior survival in patients with aggressive non-Hodgkin lymphomas such as diffuse large B-cell lymphoma (DLBCL) and peripheral T-cell lymphoma (PTCL), which likely correlates with an aggressive disease biology and clinical presentation. However, the impact of DTI on survival has not been well studied in patients with MCL. In this study, we utilized the National Cancer Database to investigate the feasibility of active surveillance or delayed treatment in patients with MCL and to explore the potential impact of DTI on survival of MCL patients requiring treatment. Methods: The National Cancer Database was used to identify adult patients (≥ 18 years) with MCL diagnosed from 2004 through 2017. Patients were divided into three groups based on their time to initial treatment: early treatment [ET] (treatment started within 3 months), delayed treatment [DT] (treatment started after 3 months) and surveillance [SUR] (watch and wait without treatment). Baseline clinical characteristics were compared between the 3 groups using Chi-square test. Survival analysis was performed using the Kaplan-Meier method and log-rank test. Statistical analysis was performed using SPSS version 25. Results: A total of 27,867 patients with newly diagnosed MCL were included in this study, 995 patients (4%) in the SUR group, 21,425 patients (77%) in the ET group, and 1,052 patients (4%) in the DT group. There were no major differences in the baseline characteristics among the three groups including demographics (age, sex, ethnicity), socioeconomic factors (annual income, insurance status, place of residence), comorbidity scores, and Ann Arbor stage. Compared to patients in the SUR and DT groups, patients in the ET group were more likely to have B symptoms (ET=27% vs DT=16% vs SUR=8%, p<0.001) and were less likely to be managed at an academic center (ET=40% vs DT=53% vs SUR= 51%, p<0.001) (Table 1). The median follow-up was 38.2 months for the entire cohort. There was a significant difference in OS among the three groups, with a median OS of 107 months (95% CI = 92.7-122.7) in the SUR group, 84.9 months (95% CI = 74.2-95.7) in the DT group, and 68.9 months (95% CI = 66.8-70.9) in the ET group (p=0.001) (Figure 1A). Among patients in the DT group, a shorter DTI was associated with a trend of inferior OS, with a median OS of 82 months (95% CI = 73.2-90.8) for those with a DTI of 3-12 months and not reached (95% CI not calculable) for those with a DTI > 12 months (p=0.095) (Figure 1B). Among the ET group, a shorter DTI was associated with shorter OS, with a median OS of 53 months (95% CI = 50.2-55.8) for patients with a DTI ≤ 15 days, 71.7 months (95% CI = 67.1-76.3) for those with a DTI of 16-30 days, and 87.1 months (95% CI= 82.8-91.4) for patients with a DTI of 31-90 days (p<0.001) (Figure 1C) Conclusion: In this retrospective analysis, patients with newly diagnosed MCL who were managed with an active surveillance strategy or delayed therapy had a better survival compared to patients who underwent early therapy. These findings validate the feasibility of delayed therapy in select patients with MCL. In patients who undergo early therapy, a shorter DTI is associated with an inferior survival outcome, mirroring findings in DLBCL and PTCL. A shorter DTI is likely associated with more aggressive disease biology and clinical presentation. Figure 1 Figure 1. Abstract:e19587 Background: The treatment landscape in patients with mantle cell lymphoma (MCL) is changing with the introduction of Bruton’s tyrosine kinase inhibitors (BTKis) and bendamustine in Australia. We sought to analyze the practice impact of the introduction of publicly funded novel agents in MCL. The objective of this study was to describe the evolving treatment patterns of Australian patients with MCL over the last 10 years using population-wide prescription records. Methods: Patients who initiated a treatment for MCL between 01/01/2011 and 07/31/2021 were extracted from the Services Australia 10% Pharmaceuticals Benefits Scheme (PBS) dataset, which includes the dispensing records for 10% of the Australian population. This dataset captures all publicly funded treatments in Australia. The index date was defined as the commencement of any drug for MCL. First-line (1L) therapy was defined as the first treatments prescribed for MCL. A patient was defined as relapsed/refractory (R/R) if they had commenced a drug which was in a different therapeutic category, or if they re-started the same regimen after a gap of more than 180 days. Descriptive analyses were conducted to examine the use of treatment regimens for the overall 10-year population by line of therapy. Analyses by calendar year were also performed to assess the changes in the treatment patterns. Results: Overall, 241 patients with MCL were identified over the study period. The majority of patients were male (68.4%), age > 60 years (84.9%), and most being aged 70-79 years (42.1% of total). Many patients were receiving comedications at baseline, including antihypertensives (44.1%), anticoagulants (14.5%), and/or antipsychotics or antidepressants (12.5%). In the overall population (2011-2021), the majority of patients had received 1L treatment with bendamustine-rituximab (BR, 53.9%), rituximab plus other regimens (27.6%), or rituximab monotherapy (11.2%). The most commonly used regimens in R/R patients at any subsequent episode of treatment included BTKi (66.3%), rituximab monotherapy (52.8%) or rituximab plus other regimens (31.5%). A trend in adoption of novel agents was observed throughout the years following their PBS listing. Analysis by calendar year showed that from 2011 to 2020 for 1L therapy, the use of BR increased from 0% to 50% and use of rituximab-containing regimen except BR decreased from 100% to 16.7%; use of BTKi increased in R/R patients from 0% to 74.7%. Conclusions: MCL treatment patterns have significantly changed in Australia since the introduction of BTKis and bendamustine-containing regimens. The use of rituximab-containing regimens except BR in 1L MCL has decreased and use of BTKis in R/R patients has increased.

A Prospective Disease Registry of Patients with Mantle Cell Lymphoma Treated with Novel Agents: Treatment Patterns, Outcomes, and Patient-Reported Health-Related Quality of Life

Abstract:Background: Mantle cell lymphoma (MCL) is a heterogeneous and often aggressive malignancy characterized by a varied median overall survival ranging from 2 years to under 10 years based on risk criteria (Hoster E, et al. JCO, 2016). While response rates to front-line chemoimmunotherapy strategies can exceed 90%, patients inevitably relapse. Novel therapies, including Bruton tyrosine kinase inhibitors and immunomodulatory agents, have been approved in the relapsed/refractory settings, yet the impact of these agents on outcomes and costs in real world populations are unknown. In an era of rapidly rising costs of therapies for hematologic malignancies, it is important to understand not only treatment patterns with the integration of novel therapies into MCL algorithms but also value, defined as incremental costs per improvement in health outcomes. Accordingly, we have designed a national prospective study of MCL patients who are initiating novel therapy. Methods: This study (NCT03816683) is an observational, prospective cohort study of patients with MCL treated in clinical practice. Eligibility includes persons ages 18 and older who have initiated standard or novel therapy within 3 months of the index enrollment date. 250 patients will be recruited from 50 academic (60%) and community (40%) settings in the United States. Recruitment will occur over 3 years, with a maximum follow-up of 60 months. The primary measures of the registry are to understand treatment patterns and value with novel therapies in MCL. The secondary measures include toxicity and efficacy of these agents when administered outside of a clinical trial and their impact on health-related quality of life and healthcare resource use. The study employs a novel patient portal that will collect patient outcomes and resource utilization over time. Analyses will be conducted from the patient, physician, and payer perspectives. Our hypothesis is that novel agents are cost-effective - as measured by costs per quality-adjusted year of life (QALY) - when used in clinical practice. Outcome measures include health state utilities (EQ-5D), (QLC-C30), survival, and healthcare resource use (HCRU), treatment modification based on adverse events, and grade 3-5 treatment-related adverse events. Uniform price weights (e.g., Medicare reimbursements) will be applied to HCRU to estimate costs. Incremental cost-utility estimates will be calculated both for the within-study period and over a lifetime, the latter using modeling. Conclusions: As the first observational study specifically designed to evaluate the clinical and economic outcomes of novel therapies for MCL in clinical practice, the findings will have high relevance for clinicians, payers, and patients. Abstract:7521 Background: In MCL, outcomes are heterogeneous and the clinical significance of the timing of relapse following autoHCT and its impact on OS is not well defined. Using the CIBMTR database, we evaluate the effect of post-autoHCT time to relapse on OS over time. Methods: Adult MCL pts treated with up to two lines of rituximab-based induction therapy followed by first autoHCT within 1-year of diagnosis were identified between 2000-2018. Primary outcomes included OS and post-relapse OS. A dynamic landmark analysis was performed at 6-month intervals following autoHCT to evaluate the impact of relapse on OS while adjusting for significant patient- and disease-related variables. Post-relapse OS was evaluated in pts who experienced relapse. Results: Of the 461 pts included in the analysis, the median age was 60 years (range 29-78), 57% had a KPS of ≥ 90, 83% had stage III-IV disease at diagnosis, and 76% had extranodal involvement. BEAM was the most common conditioning regimen (58%) and 23% of pts received post-autoHCT maintenance rituximab. With a median follow-up of 67 months, the 5-year progression-free survival was 45.8% with a 5-year OS of 69.6%. On multivariate analysis, age ≥60 years was associated with worse OS (HR= 1.55, 95% CI 1.08-2.24, p=0.0191) at all landmark timepoints. Additionally, the impact of relapse on OS varied with time (p=0.006) and was greatest at the 6-month (HR=7.68), 12-month (HR=6.68), and 18-month (HR=5.81) landmark timepoints. The risk of death for relapsing pts decreased with time and was mirrored by an improvement in adjusted OS for both relapsing and non-relapsing pts. In total, 9.3% of patients relapsed prior to the 18-month landmark timepoint. Relapse at the 6-month, 12-month, and 18-month landmark timepoints correlated with a poor median post-relapse OS of 9 months, 24 months, and 34 months, respectively (Table). Conversely, patients relapsing after the 18-month landmark timepoint experienced a median post-relapse OS ranging from 44-67 months. Conclusions: In MCL, early relapse (< 18-months) following autoHCT defines a high-risk group with inferior post-relapse OS. This population should be considered for clinical trials or novel therapeutic approaches including early utilization of chimeric antigen receptor T-cell therapy.[Table: see text]

Abstract:Background Mantle cell lymphoma (MCL) is an uncommon heterogeneous subtype of B cell non‐Hodgkin lymphoma, and clinical features in MCL appear regional characteristics. MCL treatment opinions are not uniform between countries or regions within Asia and China, and Asian patient‐specific data for MCL treatment are fewer. The study aims to explore the clinical characteristics, treatment patterns and prognosis of MCL patients in China. Methods A total of 805 patients diagnosed with MCL between April 1999 and December 2019 at 19 comprehensive hospitals in China were included in this retrospective analysis. Kaplan‐Meier method coupled with the log‐rank test was used for univariate analysis, and COX proportional hazards model was used for multivariate analysis (MVA). p < 0.05 was consided statistically significant. All outputs were produced using R version 4.1.0. Results The median age of the cohort was 60.0 years with a male‐to‐female ratio of 3.36:1. Five‐year progression‐free survival (PFS) and overall survival (OS) rates were 30.9% and 65.0%, respectively. High‐intermediate/high‐risk group according to MIPI‐c, without high‐dose cytarabine, lack of Auto‐SCT as consolidation and maintenance treatment and SD/PD in initial treatment remained statistically relevant to poor PFS on MVA, and ki67 ≥50%, B symptoms, high‐intermediate/high risk group according to MIPI‐c, without high‐dose cytarabine, lack of maintenance treatment, SD/PD in initial treatment and relapse/refractory state were independently associated with poorer OS on MVA. Conclusions First‐line high dose cytarabine exposure, auto‐SCT as consolidation therapy obtained survival benefits in Chinese population. Our study further confirmed the value of maintenance treatment and explored the application of new drug treatment and bendamustine in R/R MCL patients.

Abstract:Purpose The aim of the study was to delineate the disease characteristics, the initial treatment patterns, and survival in patients with mantle cell lymphoma (MCL) managed in the real world. Methods Data of 518 MCL patients from 5 major Chinese Hematology Centers in the period from 2007 to 2017 were retrospectively analyzed. Results The median age was 58 years. Of the patients, 88.6% had Eastern Cooperative Oncology Group Performance Status (ECOG PS) 0–1 and 80.7% had advanced-stage disease. Ki67 expression was <30% in 39.6% of the patients, and 43.2% of patients were categorized into a low-risk group based on the Mantle Cell Lymphoma International Prognostic Index (MIPI) scoring system. Overall, 73.4% of the patients received rituximab as their first-line therapy. The most commonly used chemotherapy was the CHOP-like (cyclophosphamide, hydroxydaunomycin, oncovin, and prednisone) regimen (45.2%), followed by high-dose cytarabine-containing chemotherapy (31.3%) and bendamustine (3.3%). Of the patients, 13.7% ( n = 71) underwent consolidative autologous stem cell transplantation (ASCT), and 19.3% ( n = 100) received novel agents containing first-line regimens. With a median follow-up time of 52 months, the 3- and 5-year overall survival (OS) rates were 73.7% and 61.4%, respectively. Age ≤60 years, ECOG PS 0–1, stages I–II, normal lactate dehydrogenase (LDH), absence of bone marrow involvement, Ki67 <30%, and lower-risk IPI/MIPI scores were significantly associated with improved OS ( p < 0.05). The inclusion of rituximab improved the 5-year OS, with borderline significance (62.5% vs . 55.2%, p = 0.076). High-dose cytarabine-containing chemotherapy showed significant clinical benefit in 5-year OS (72.1% vs . 55.9%, p = 0.010). Patients with ASCT had better 5-year OS in the younger (≤60 years) age group (87.2% vs . 64.8%, p = 0.002). Conclusion This large retrospective dataset unequivocally confirmed the survival advantage afforded by cytarabine-containing regimen and ASCT in a first-line setting under real-world management in the rituximab era.

Abstract:Progress in mantle cell lymphoma (MCL) has led to significant improvement in outcomes of patients even in the real world (RW) setting albeit to a lesser degree. In parallel to the demonstration of benefit using combination therapy with rituximab plus high-dose cytarabine (R-AraC) as well as dose intensive therapy-autologous stem cell transplantation (DIT-ASCT) consolidation and maintenance, it became clear over the last 2 decades that MCL is a highly heterogenous disease at the molecular level, explaining differences observed in clinical behavior and response to therapy. While clinical prognostic factors and models have helped stratify patients with distinct outcomes, they failed to help guide therapy. The identification of molecular high-risk (HR) features, in particular, but not only, p53 aberrations (including mutations and deletions [del]), as well as complex karyotype (CK), has allowed to identify subsets of patients with poorer outcomes (median overall survival [OS] < 2 years) regardless of conventional therapies used. The constant pattern of relapse seen in MCL has fueled sustained and productive efforts, with 7 novel agents approved in the United States (US), showing high and durable efficacy even in HR and chemo-refractory patients and likely curing a subset of patients in the relapsed or refractory (R/R) setting. Progress in diagnostics, in particular next-generation sequencing (NGS), which is accessible in routine practice nowadays, can help recognize patients with HR features, well beyond MIPI or Ki-67 prognostication, although the impact on decision making is still unclear. The era of integrating novel agents into our prior standard of care (SOC) has begun with a confirmed benefit, for example, ibrutinib (Ib) in the TRIANGLE study, defining the first new potential SOC in younger patients in over 30 years. Expanding on novel agents, either in combination, sequentially or to replace chemotherapy altogether, using biological doublets or triplets has led to a median progression-free survival (PFS) in excess of 72 months, certainly competitive with prior SOC and will continue to reshape the management of MCL patients. Achieving minimal residual disease negative (MRD-ve) status is becoming a new endpoint in MCL, and customizing maintenance and/or de-escalation/consolidation strategies is within reach, although it will require prospective, built-in MRD-based approaches, with the goal of eliminating subclinical disease and not simply delaying time to relapse. Taking into account the biological diversity of MCL is now feasible in routine clinical practice and has already helped recognize what not to do for HR patients (i.e., avoid intensive induction chemotherapy and/or ASCT for p53 mutated patients) as well as identify promising novel options. Ongoing and future work will help expand on these dedicated approaches, to further improve the management and outcomes of all MCL patients.

Abstract:Background: MCL remains incurable and its management is highly heterogeneous due to the lack of standard treatment. Since the results from prospective clinical trials are not always reproducible in real-life patients, the aim of this study was to analyze the management in clinical practice of patients with MCL in Spain and its impact on their survival. Methods: We retrospectively analyzed the clinical-biological characteristics and treatment patterns of patients (pts) with MCL from Spanish GELTAMO centers and their impact on overall (OS) and progression-free survival (PFS). First-line treatments (1L) were grouped as HiDAC (high-dose cytarabine or HiDAC-based) or non-HiDAC; second-line treatments (2L) as BTKi (BTK inhibitors), CIT (chemoimmunotherapy) or other (targeted agents other than BTKi). In patients receiving 1L at diagnosis, early progression/relapse (E-POD) was considered when occurring within 24 months (m) from diagnosis and late or L-POD beyond 24 m or no relapse at data cut-off. A follow-up time (FU) of ≥24m was required for patients to be evaluable for POD, except for those dying from MCL <24m, recorded as E-POD. Statistical analyses were performed with the R-4.1.0 software. Results: From January 2000-March 2023, 505 patients were diagnosed with MCL in 17 GELTAMO centers, their baseline characteristics are summarized in Table 1. At data-cut-off, 242 (47.9%) were alive, 17 (3.4%) lost to FU and 246 (48.7%) had died, being the lymphoma the main cause of death (50.4%). Sixty-five (12.8%) pts did not receive treatment at diagnosis, out of which 18 (28%) remain on observation at a median (M) FU=26 m (3-92). Out of 479 pts receiving 1L, 245 (51%) received 2L, 143 (30%) 3L, 75 (15.6%) pts 4L and <10% received 5-8 lines. Among 432 pts treated upfront (no observation prior to 1L), 423 were evaluable for response: 190 (44.9%) HiDAC and 233 (55%) non-HiDAC, with a predominance of anthracycline-based treatments (Figure 1). Overall (OR) and complete responses (CR) were higher with HiDAC vs. non-HiDAC (91%/78.4% vs. 82.8%/65.2%, respectively). In 394/432 pts evaluable for POD (149 E-POD and 245 L-POD), HiDAC was more frequently associated with L-POD (73.2% L-POD vs. 26.8% E-POD) than non-HiDAC (53.1% L-POD vs. 46.9% E.POD). Rituximab maintenance (RM) was given to 167 pts (prior HiDAC 48.5%, non-HiDAC 51.5%) and 80.7% of them were L-POD. ASCT was performed in 135 pts (prior HiDAC 83.7%, non-HiDAC 16.3%, followed by RM in 46%) and 86.1% were L-POD. In 2L, 232/245 were evaluable, 92 (39.6%) received iBTK (60 monotherapy, 11 with CT and/or rituximab), 122 (52.6%) CIT (bendamustine-based 24.6%, gemcitabine-based 36.5%) and 18 (7.75%) other salvage options (Figure 1), with no significant differences between iBTK and CIT regarding baseline characteristics or efficacy (OR/CR 60.8%/39.1% vs. 72%/56.5% for BTKi and CIT, respectively). With a M FU= 87.6m, M OS= 6.56 years (5.6-8.2) and M PFS= 34.3 m (29.86-43.43). Age>65 at diagnosis (HR=2.09, p<.001), blastoid variant (HR=1.77, p=.012), Ki67≥30% (HR=1.93, p=.001), non-CR after 1L (HR=2.20, p<.001) and E-POD (HR=5.21, p<.001) were associated with shorter OS (MVA) and blastoid variant (HR=1.83, p=.001) and Ki67≥30% (HR=1.65, p=.003) with PFS. ASCT and RM were significantly associated with longer PFS (HR=0.33 and HR=0.32, p<.001) in the MVA but only MR had impact on OS (HR=0.62, p=.031). After 2L, M OS-2= 30.01 m (25.23-46.42) and PFS-2= 12.42m (8.97-17.02), with no impact of BTKi in 2L (vs. CIT) on OS-2 (HR=0.64, p=.12) or PFS-2 (HR=0.73, p=.12). E-POD and HR-MIPI, however, were significantly associated with shorter OS-2 (HR=1.72, p=.03 and HR=2.08, p=.03, respectively) and PFS-2 (HR=1.91, p<.001 and HR=1.88, p=.005 respectively). Conclusions: The impact on survival of known risk factors such as older age at diagnosis, high Ki67, HR-MIPI, blastoid morphology, non-CR after 1L or E-POD was confirmed in our series, as well as the advantage in efficacy and risk of E-POD of HiDAC-based regimens vs. non-HiDAC in 1L. Regarding RM and ASCT in 1L, our results are in line with other studies, with RM associated with longer PFS and OS and ASCT showing advantage in PFS but not in OS. In 2L, the rate of OR/CR with BTKi in our cohort is like that reported in pivotal studies, but we did not find the survival advantage of BTKi over CIT observed in other retrospective studies.

Abstract:Aim: To examine real-world treatment patterns, survival, healthcare resource use and costs in elderly Medicare beneficiaries with diffuse large B-cell lymphoma (DLBCL). Methods: 11,880 Medicare patients aged ≥66 years with DLBCL between 1 October 2015 and 31 December 2018 were followed for ≥12 months after initiating front-line treatment. Results: Two-thirds (61.2%) of the patients received standard-of-care R-CHOP as first-line treatment. Hospitalization was common (57%) in the 12-months after initiation of 1L treatment; the mean DLCBL-related total costs were US$84,416 during the same period. Over a median follow-up of 2.1 years, 17.8% received at least 2L treatment. Overall survival was lower among later lines of treatment (median overall survival from initiation of 1L: not reached; 2L: 19.9 months; 3L: 9.8 months; 4L: 5.5 months). Conclusion: A large unmet need exists for more efficacious and well-tolerated therapies for older adults with DLBCL.
Randomized Bendamustine-Rituximab(R) + Bortezomib Induction and R + Lenalidomide Maintenance for Mantle Cell Lymphoma

Abstract:MCL therapy with BR-based induction and rituximab-based maintenance gave 88% overall response rate, MRD- rate 91%, and median PFS 6.9 years Neither adding bortezomib to induction nor lenalidomide to maintenance or both improved PFS in this older (87% over 59 years) cohort While initial therapy of mantle cell lymphoma (MCL) is not standardized, bendamustine-rituximab (BR) is commonly used in older patients. Rituximab (R) maintenance following induction is often utilized. Thus, the open-label, randomized phase II ECOG-ACRIN Cancer Research Group E1411 trial was designed to test two questions: 1) Does addition of bortezomib to BR induction (BVR) and/or 2) addition of lenalidomide to rituximab (LR) maintenance improve progression-free survival (PFS) in patients with treatment-naïve MCL? From 2012-2016, 373 previously untreated patients, 87% ≥ 60 years old, were enrolled in this trial. At a median follow up of 7.5 years, there is no difference in the median PFS of BR compared to BVR (5.5 yrs vs. 6.4 yrs, HR 0.90, 90% CI 0.70, 1.16). There were no unexpected additional toxicities with BVR treatment compared to BR, with no impact on total dose/duration of treatment received. Independent of the induction treatment, addition of lenalidomide to rituximab did not significantly improve PFS, with median PFS in R vs LR (5.9 yrs vs 7.2 yrs, HR 0.84 90% CI 0.62, 1.15). The majority of patients completed the planned 24 cycles of LR at the scheduled dose. In summary, adding bortezomib to BR induction does not prolong PFS in treatment-naïve MCL, and LR maintenance was not associated with longer PFS compared with rituximab alone following BR. Nonetheless, the > 5 year median PFS outcomes in this prospective cooperative group trial indicate the efficacy of BR followed by rituximab maintenance as highly effective initial therapy for older MCL patients. Front‐line and second‐line treatment for mantle cell lymphoma in clinical practice: A multicenter retrospective analysis

Abstract:Background There are few reports of clinical practice treatment patterns and efficacy in mantle cell lymphoma (MCL). Materials and Methods We retrospectively studied a large, multicenter, cohort of patients with MCL diagnosed between 2000 and 2020 in eight institutions. Results 536 patients were registered (73% male, median of 70 years). Front‐line treatment was based on high‐dose cytarabine, bendamustine, and anthracyclines in 42%, 12%, and 15%, respectively. The median PFS for all patients was 45 months; 68, 34, and 30 months for those who received high‐dose cytarabine‐based, bendamustine‐based and anthracycline‐based therapy. 204 patients received second‐line. Bendamustine‐based treatment was the most common second‐line regimen (36% of patients). The median second‐line PFS (sPFS) for the entire cohort was 14 months; 19, 24, and 31 for bendamustine‐, platinum‐, and high‐dose cytarabine‐based regimens, with broad confidence intervals for these latter estimates. Patients treated with cytarabine‐based therapies in the front‐line and those with front‐line PFS longer than 24 months had a substantially superior sPFS. Conclusion Front‐line treatment in this cohort of MCL was as expected and with a median PFS of over 3.5 years. Second‐line treatment strategies were heterogeneous and the median second‐line PFS was little over 1 year.

Methods Patients diagnosed with MCL between April 1999 and December 2019 at 19 comprehensive hospitals in China were included in this retrospective analysis. The median follow-up times was 36.0 months. The data of total 805 patients with mantle cell lymphoma were collected, 112 patients with incomplete clinical data, no chemotherapy and missing follow-up data were excluded. Finally, 693 patients with complete clinical data, treatment information and survival follow-up data were included in this study.Fully detailed information of patients, disease characteristics, treatments and outcomes were collected. Fisher's exact test or Pearson's Chi-squared test were used to compare categorical variables.Survival analysis was performed by Kaplan-Meier. Results The median age of the cohort was 60.0 years with a male-to-female ratio of 3.36:1. 477 patients (68.8%) were younger than 65 years at diagnosis.Advanced stage of diseases were 90.0%. Extranodal organ involvement was 83.4% and the most frequently involved extranodal organs were bone marrow(46.2%), spleen(36.1%), gastrointestinal tract(24.8) and oropharynx(17.7%). Blastic/pleomorphic mantle cell lymphoma accounted for 12.8%, and non-nodular mantle cell lymphoma accounted for 3.3%. Ki-67 more than 30% was 57.7% and Ki-67 ≥ 50% was 26.1%. The intermediate /high risk group was 49.8% according to MIPI score. The first-line therapeutic schedule is not completely unified, the most frequently regimen was CHOP/CHOP-like±R (n=312,45.0%) ,then high-dose cytarabine (n=222,32.0%), VR-CAP(n=44,6.3%), BR(n=30,4.3%), R-EPOCH (n = 17, 2.5%), FC / FCM±R regimen (n = 13,1.9%),and 55 patients(7.9%) uesd chemo-free regimen including IR / R2 / IR2 .Only 80 patients (11.5%) received autologous hematopoietic stem cell transplantation as consolidation therapy after chemotherapy remission. The ORR rate was 85.0%with 46.6% CR and 38.4% PR in the initial treatment.During the follow-up to June 2021(2-204months), 222 patients(32.0%) had died. The 5-year PFS and OS was 30.9% and 65.0%respectively.In the multivariate Cox regression model, ki67 ≥ 50% (HR 1.27, 95%CI 1.01-1.60, p = 0.038) ,stageIII/IV (HR1.56, 95%CI 1.05-2.33, p = 0.029),high- intermediate/high risk group accouding to MIPI-c (HR1.56, 95%CI 1.05-2.33, p = 0.008),spleen involvment (HR1.35, 95%CI 1.08-1.69, p = 0.009),without maintenance treatment (HR0.71, 95%CI 0.56-0.88, p = 0.003) ,SD/PD in inital treatment (HR6.20, 95%CI4.71-8.14, p < 0.001) were independently associated with poorer PFS while ki67 ≥50% (HR 1.74, 95%CI 1.31-2.31, p< 0.001),B symtom (HR 1.52, 95%CI 1.15-2.00, p = 0.003), high-intermediate/high risk group accouding to MIPI-c (HR1.43, 95%CI 1.24-1.64, p < 0.001),without high-dose cytarabine (HR0.47, 95%CI 0.31-0.70 ,p < 0.001) ,without maintenance treatment (HR0.40, 95%CI 0.28-0.58,p < 0.001)and relapse/refractory state (HR 4.04, 95%CI 2.21-7.39, p < 0.001) were independently associated with poorer OS. Conclusions This study describes the characteristics of mantle cell lymphoma in Chinese population with younger onset age and a higher tumor proliferation index. High dose cytarabine treatment and maintenance treatment have shown survival benefits in real-world.Recurrence and refractory is an important factor affecting survival,the strategy of combining molecular biological characteristics with novel strategies may improve outcome in these patients. Abstract:Mantle cell lymphoma (MCL) is an invasive B-cell lymphoma with significant individual differences. Currently, MCL international prognostic index (MIPI) score and tumor cell proliferation index Ki-67 have been proved to be the most important prognostic factors. But the prognostic effect of these factors in Asian population is uncertain. This study aimed to analyze the disease characteristics and prognostic factors of Chinese MCL patients. A total of 83 cases of newly-diagnosed MCL patients diagnosed by the Department of Pathology of our hospital between January 1, 2011, and May 31, 2016, were enrolled. The disease characteristics, treatment effects, and outcomes of the patients were collected and analyzed. According to our analysis, MCL cases accounted for 6.2% of non-Hodgkin lymphoma (NHL) cases and mainly occurred in elderly males. But the proportion of patients at stage IV by Ann Arbor staging system and high-risk group by simplified-MIPI (s-MIPI) were significantly lower than that among European patients. Immunochemotherapy containing rituximab was significantly more effective than chemotherapy (overall response rate, [ORR]: 88.5% vs 65.2%, P = .021) and significantly prolonged patient survival (progression free survival [PFS]: 45.5 m vs 16.2 m, P = .001; overall survival [OS]: 58.3 m vs 22.8 m, P = .001). The multivariate analysis showed that the B symptoms, s-MIPI and administration of immunochemotherapy were independent prognostic factors that affected PFS and OS of the patients. s-MIPI and B symptom make up s-MIPI-B stratification method, by which patients in low-risk group of s-MIPI without B symptom were classified as low-risk, patients in high-risk group of s-MIPI and patients in low-risk group of s-MIPI with B symptom as high-risk, the rest as middle-risk. 3-year PFS of the 3 groups were 74.9%, 43.4% and 16.1%, respectively (P = .001). 3 year OS were 84.4%, 62.2%, 27.6% (P < .001). Chinese MCL was male predominance. We have a minor proportion of late-stage and high-risk patients compared to European patients. Immunochemotherapy was proved to significantly improve the prognosis of MCL patients. B symptoms, s-MIPI, and administration of rituximab independently influenced the outcome. s-MIPI-B prognostic stratification method may better predict the prognosis of Asian MCL patients. Still, further confirmation in larger populations is needed.

Abstract:Mantle cell lymphoma (MCL) is a B-cell non-Hodgkin lymphoma; data indicates that blastoid and pleomorphic variants have a poor prognosis. We report characteristics and outcomes of patients with blastoid/pleomorphic variants of MCL. We retrospectively studied adults with newly diagnosed MCL treated from 2000-2015. Primary objectives were to describe progression-free survival (PFS) and overall survival (OS). Secondary objectives included characterization of patient characteristics and treatments. Of 1029 MCL patients studied, a total of 207 neoplasms were blastoid or pleomorphic variant. Median follow up was 82 months (range 0.1-174 months); median PFS was 38 months (95% confidence interval [CI] 28-66) and OS 68 months (95% CI 45-96). Factors associated with PFS were receipt of consolidative autologous hematopoietic transplant (auto-HCT) (hazard ratio [HR] 0.52, 95% CI 0.31-0.80, p < 0.05), MCL International Prognostic Index (MIPI) score intermediate (HR 2.3, 95% CI 1.2-4.3, p<0.02) and high (HR 3.8, 95% CI 2.0-7/4, p<0.01), and complete response to induction (HR 0.29 (95% CI 0.17-0.51). Receipt of auto-HCT was not associated with OS (HR 0.69, 95% CI 0.41-1.16, p = 0.16), but was with MIPI intermediate (HR 5.7, 95% CI 2.5-13.2, p< 0.01) and high (HR 10.8, 95% CI 4.7-24.9, p< 0.01). We report outcomes in a large cohort of patients with blastoid/pleomorphic variant MCL. Receipt of auto-HCT after induction was associated with improved PFS for eligible patients, but not OS. Higher MIPI score and auto-HCT ineligibility were associated with worse survival.

Abstract:Background - We investigated the efficacy and safety of using a combination of ibrutinib plus rituximab (IR) followed by short course (4 cycles) of R-HCVAD/MTX-ara-C as consolidation in previously untreated young (age ≤ 65 years) patients (pts) with mantle cell lymphoma (MCL). Using a chemo-free induction may reduce the toxicities, complications and the risk of second cancers which were observed with intensive chemoimmunotherapy regimen in MCL pts. Methods - We enrolled 131 previously untreated pts in this single institution, single arm, phase II clinical trial - NCT02427620. Pts received IR induction (part-A), until they achieved complete remission (CR) for up to a maximum of 12 cycles, followed by a maximum of 4 cycles of R-HCVAD/R-MTX-ara-C (part-B) as consolidation. None of the pts received stem cell transplant or maintenance therapy. The primary objective was to assess overall response rate (ORR), [defined as either a partial response (PR) or a complete response (CR)] after part A. Adverse events were coded as per CTCAE version 4. Among evaluable samples, minimal residual disease (MRD) by flow cytometry at best response, clonal evolution and MRD using circulating tumor DNA (ctDNA), whole exome (WES) and bulk RNA sequencing from baseline tissue samples was performed. Results - Among the 131 pts, the median age was 56 yrs (range - 35-65). High Ki-67 (≥30%) in 58/117 (49.5%) pts, 10 pts (8%) had high risk simplified MIPI score, 15 pts (11%) had aggressive MCL (blastoid/pleomorphic) and 114 pts (87%) had initial bone marrow involvement. Seventeen pts (13%) had had complex karyotype and 83% had positive SOX-11 expression. Baseline PET scan was available in 101 pts and 97 pts had PET positive disease. Median number of cycles on IR in part A was 7 (1-12). At week 16 on part A, the ORR was 95% (15% CR and 80% PR) and 5% pts had stable disease. Overall best response (ORR) on part A of therapy was 100% (88% CR and 12% PR) and at the time of last follow up after completion of part A and part B, ORR was 100% (98% CR). Median time to CR was 5 months (range 1-16). Pts with CR within 6 months are "early" CR (n=66) and those with CR ≥ 6 months were "late" CR (n=49) while pts who never had CR were grouped as no CR (n=13). With a median follow up of 37 months, the median PFS and OS were not reached (3 year 82% and 95% respectively). Twenty two pts (17%) relapsed after treatment, including 6 who transformed to aggressive MCL. PFS among pts with high and low Ki-67% was 58 months vs not reached (P=0.03) while no difference was observed in OS. PFS was significantly shorter in pts with aggressive histology (p=0.003) but not the OS. Overall 6 pts died (2 with progression, 2 due to disease transformation, one on study due to multiple complications including splenic hematoma, cardio-pulmonary arrest and progression and the last one expired outside and came off study due to encephalitis). Forty two pts came off study for various reasons [16 disease progression (including 4 transformation), 20 pt choice, 3 intolerance, two second cancer and one lost to follow up]. Grade 3-4 toxicities on part A were 4% myelosuppression and 8% each with fatigue, myalgia and rashes and 4% mucositis. None had grade 3-4 atrial fibrillation or bleeding. WES in 76 pts revealed deletions of BANK1, CCND1, TP53, UBR5, ATM, RB1, CDKN2A and BCL2 in late CR (n=14) (≥ 6 months) and no CR (n=12) pt groups vs early CR (< 6 months; n=30). KMT2C, NCOR2 SMARCA4 and NSD2 mutations were significantly different in late CR vs early CR groups. Furthermore, 68 pts had bulk RNA sequencing and a differential gene expression pattern was identified separating early vs late vs no CR groups. Immune response pathways were overexpressed in early CR pts while oncogenic signaling pathways were clustered in late CR and no CR groups. Conclusions - Chemo-free induction with IR induced durable and deep responses in young MCL pts in the frontline setting. Short course R-HCVAD chemotherapy minimized toxicities and consolidated responses. This combined modality treatment approach may significantly improve young MCL pts outcomes across all risk groups. Pathway analysis demonstrated immune response pathways to be associated with early CR on IR. Detailed analyses on MRD using ct-DNA will be reported. Abstract:Objective:To study the clinical features, therapeutic effects, survival time, and prognosis of patents with mantle cell lymphoma (MCL). Methods:Clinical data of 98 MCL patients admitted from January 2005 to December 2013 were retrospectively an-alyzed. Results:The median age was 61 years old, and the male-to-female ratio was 2.9∶1. Among these cases, 85 (86.8%) were in Ann Arbor stageⅢ-Ⅳ, 46 (46.9%) had bone marrow involvement, 25 (25.5%) had digestive tract involvement, and 53 chose R-CHOP as first-line treatment. The expected 3-year overall survival (OS) of these patients was only 61.4%. A total of 14 cases were treated with R-CHOP followed by ASCT. The expected 5-year OS was 92.3%, and the OS of the ASCT group was significantly higher than that of the R-CHOP group (75.5 months vs. 43.6 months, P=0.039). Elevated ESR,>60 years old, increased LDH level, B symptoms, and Ki-67≥25% were poor prognostic factors. Conclusion: Most patients with MCL were elder adults with bone marrow involvement. R-CHOP followed by ASCT had better clinical efficacy than conventional chemotherapy in the treatment of MCL.
