Abstract:This paper identifies uncertainties and problems in the practical application of the ICNIRP low frequency basic restriction on current density. This quantity should be averaged over a cross-section of 1 cm2 perpendicular to the current direction. The rationale and the sensitivity of the current density average are investigated. There are difficulties in finding a square centimetre of spinal cord over which to average. The consequences of including neighbouring tissues in the averages are investigated in the male and female voxel models NORMAN and NAOMI for applied uniform electric and magnetic fields at 50 Hz. Also the case of the non-uniform magnetic field from a horizontal current carrying conductor adjacent to the back of the body is investigated. The maximum and 99th percentile current density values are compared with the 1 cm2 average in the derivation of external field reference levels. Abstract:PURPOSE The purpose of this study was to develop and establish the psychometric properties of a clinical nursing competency evaluation tool to be utilized by clinical preceptors. METHODS The initial items were identified through in-depth literature review and field interviews based on a hybrid model. Content validation of the items was evaluated through three rounds of content validity testing. Participants were 34 clinical preceptors and 443 nursing students participating in clinical practice. Data were analyzed using exploratory and confirmatory factor analysis, convergence and discriminant validity, internal consistency and inter-rater reliability. RESULTS The final scale consisted of 23 items and four factors, fundamental nursing skills performance, critical thinking skills based on the nursing process, basic nursing knowledge, and professional attitude; these factor explained 69.7% of the total variance. The analysis with multi-trait/multi-item matrix correlation coefficients yielded 100.0% and 95.7 % convergence and discriminant validity, respectively. Cronbach's alpha for the total items was .95. The four subscale model tested by confirmatory factor analysis was satisfactory. Inter-rater reliability ranged from .912 to .967. CONCLUSION This scale was found to be a reliable and valid instrument that clinical preceptors can apply for evaluating the clinical nursing competency of nursing students in clinical settings.

Abstract:Abstract Background Adapting practices to respond sensitively to increasingly culturally diverse patients can be challenging. Therefore, cultural competence among nurses needs to be assessed to evaluate their current cultural competence and the need for interventions to improve daily nursing practice. Little is known about cultural competence of nurses in German-speaking countries, including Austria, as there is currently no validated tool in German to assess cultural competence in nurses. The aims of this study were to translate and cross-culturally adapt the Cultural Competence Assessment scale in German and to evaluate its psychometric properties. Methods This is a methodology study with a cross-sectional design. Conducting a convenience sampling, Austrian nurses working in the direct care of patients in acute care settings were invited to participate in this study. Data collection was conducted in March 2021. The translation and cross-cultural adaptation were conducted by following the guidelines of Sousa et al. (J Eval Clin Pract 17:268-74, 2011) and Beaton et al. (Spine 25:3186-91, 2000). The face and content validity, structural validity, and internal consistency reliability of the Cultural Competence Assessment scale, which consists of 25 items, was evaluated. Data were analysed using content validity index, confirmatory factor analyses as well as McDonald’s Omega. Descriptive statistics were computed with the statistical software IBM SPSS Statistics 26, while the confirmatory factor analysis was conducted with the R package Lavaan. Results Overall, 915 nurses completed the questionnaire. Twenty items had an acceptable item content validity index. Using confirmatory factor analyses, a two-factor model with 14 items yielded a good fit (x 2 /df = 3.16; CFI = .923; TLI = .908; RMSEA = .055 (.049–.062) and SRMR = .039). Internal consistency reliability was found to be acceptable, as indicated by a Omega of .87. Conclusion The German version of the Cultural Competence Assessment scale (CCA-G) can be recommended for measuring cultural competence behaviour of nurses in acute care settings. The 14-item scale showed strong construct validity and acceptable internal consistency. Further research using repeated measures could determine the cultural sensitivity and indicate if the tool is applicable in other healthcare settings and for other healthcare professionals.

Abstract:Summary: Magnetic resonance imaging of the breast in the diagnosis of silicone breast implant rupture is widely accepted to be the imaging study of choice for most women. Magnetic resonance imaging in the detection of silicone implant failure has been shown to have the highest sensitivity and specificity and has the ability to image the entire implant without the use of ionizing radiation. Unfortunately, some women are unable to have a magnetic resonance imaging examination of the breast because of contraindications such as cardiac pacemakers, aneurysm clips, and claustrophobia. Therefore, mammography, ultrasonography, and computed tomography will have roles in the diagnosis of silicone breast implant ruptures. This article illustrates the spectrum of imaging appearances of normal silicone gel implants and the appearances of silicone breast implant ruptures.

Abstract:BACKGROUND: Cultural competence of healthcare professionals (HCPs) is recognized as a strategy to reduce cultural disparities in healthcare. However, standardised, valid and reliable instruments to assess HCPs' cultural competence are notably lacking. The present study aims to 1) identify the core components of cultural competence from a healthcare perspective, 2) to develop a self-report instrument to assess cultural competence of HCPs and 3) to evaluate the psychometric properties of the new instrument.METHODS: The conceptual model and initial item pool, which were applied to the cross-cultural competence instrument for the healthcare profession (CCCHP), were derived from an expert survey (n = 23), interviews with HCPs (n = 12), and a broad narrative review on assessment instruments and conceptual models of cultural competence. The item pool was reduced systematically, which resulted in a 59-item instrument. A sample of 336 psychologists, in advanced psychotherapeutic training, and 409 medical students participated, in order to evaluate the construct validity and reliability of the CCCHP.RESULTS: Construct validity was supported by principal component analysis, which led to a 32-item six-component solution with 50% of the total variance explained. The different dimensions of HCPs' cultural competence are: Cross-Cultural Motivation/Curiosity, Cross-Cultural Attitudes, Cross-Cultural Skills, Cross-Cultural Knowledge/Awareness and Cross-Cultural Emotions/Empathy. For the total instrument, the internal consistency reliability was .87 and the dimension's Cronbach's α ranged from .54 to .84. The discriminating power of the CCCHP was indicated by statistically significant mean differences in CCCHP subscale scores between predefined groups.CONCLUSIONS: The 32-item CCCHP exhibits acceptable psychometric properties, particularly content and construct validity to examine HCPs' cultural competence. The CCCHP with its five dimensions offers a comprehensive assessment of HCPs' cultural competence, and has the ability to distinguish between groups that are expected to differ in cultural competence. This instrument can foster professional development through systematic self-assessment and thus contributes to improve the quality of patient care.

Abstract:Abstract Background In clinical environments, nursing students experience a range of stressors that can affect their health, learning, and quality of patient care. This study aimed to develop a Nursing Students’ Perceptions of Clinical Stressors Scale (NSPCSS) and to evaluate its psychometric properties. Methods This exploratory, sequential mixed-method study was conducted in 2 phases. In the qualitative (item generation) phase, NSPCSS items were generated using the data collected from semi-structured interviews and a literature review. In the quantitative (psychometric evaluation) phase, face, content, construct, convergent, and discriminant validity and reliability of the scale were tested. To evaluate construct validity, exploratory and confirmatory factor analyses were performed on the data collected from 430 nursing students. Reliability was also assessed through internal consistency and composite reliability. Results In this study, 6 factors were extracted from 30 itemes through exploratory factor analysis: (1) instructor’s limited competence in clinical environments, (2) inappropriate clinical environment, (3) inadequate knowledge and skills, (4) inefficient education in clinical planning, (5) instructor’s inappropriate conduct, and (6) concerns about the characteristics of nursing career. These factors accounted for 58.8% of the total variance. The results of the confirmatory factor analysis suggested the goodness-of-fit indices was acceptable. Furthermore, the internal consistency and composite reliability indices of all factors were greater than 0.7. Conclusions The NSPCSS is a valid and reliable instrument for assessing clinical stressors among nursing students.

Abstract:Background & Aims Nurses are the largest group of healthcare providers, especially during the COVID-19 pandemic. Patients with COVID-19 are in dire need of spiritual care due to being alone in quarantine, away from their families. The present study aims to assess the spiritual care competence of nurses in the special care units for COVID-19 in teaching hospitals in Arak, Iran Materials & Methods This is a descriptive cross-sectional study. Participants were 142 nurses working in the special care units for COVID-19 in hospitals affiliated to Arak University of Medical Sciences in 2022, who were selected by a census method. The data collection tool was the spiritual care competence scale. The data were analyzed in SPSS software, version 16. Results The mean total score of spiritual care competence was 100.71±14.45, which was higher than the median score of 81. The spiritual care competence had a significant relationship with marital status (P=0.044), willingness to work overtime (P<0.001), age (P=0.011), total work experience (P=0.03), and work experience in the special care unit (P=0.025), such that spiritual care competence was higher in older married nurses with a willingness to work overtime and a higher work experience. Conclusion The spiritual care competence of nurses in the study hospitals is at a favorable level. The results of this study can help future studies in improving the quality of nursing care and the competence of nurses in spiritual care.

Abstract:Background: Identiﬁcation of the inﬂuencing factors of nursing students’ clinical judgment (CJ) facilitates the achievement of this important educational outcome. However, no tool is so far designed to assess it in the ﬁeld of clinical education in Iran. Objectives: The current study aimed at designing sequential exploratory mixed method in 3 phases. Methods: It was a mixed method study. The 1st phase included reviewing literature, analyzing the relevant studies in Iran, and a qualitative content analysis. In the 2nd phase, validity and reliability of the tool were investigated. Results: Inthestudy,afterassessingtheface,content,andconstructvalidity,4factorsand38itemsobtained. Contentvalidityratio (CVR) ranged from 0.45 to 1.00 and content validity index (CVI) ranged from 0.6 to 1.00. Internal consistency of the total scale was goodwithCronbach’salphacoeﬃcient0.92,andintraclasscorrelationcoeﬃcient(ICC)was0.95thatindicatedgoodstability. Inthe constructvaliditybyexploratoryfactoranalysis,theKaiser-Meyer-Olkin(KMO)indexwashigherthan0.90andalsotheBartletttest of sphericity was signiﬁcant (P value < 0.001). Concerning the cumulative percent of the variance, the 4 factors determined 44.8% of the total variance. Conclusions: ThedevelopedquestionnaireinthecurrentstudywasasuitabletooltoassessthefactorsinﬂuencingtheCJof undergraduate nursing students.

Abstract:AIM: This study contributes to the development of a valid and reliable instrument, the spiritual care competence scale, as an instrument to assess nurses' competencies in providing spiritual care.BACKGROUND: Measuring these competencies and their development is important and the construction of a reliable and valid instrument is recommended in the literature.DESIGN: Survey.METHOD: The participants were students from Bachelor-level nursing schools in the Netherlands (n = 197) participating in a cross-sectional study. The items in the instrument were hypothesised from a competency profile regarding spiritual care. Construct validity was evaluated by factor analysis and internal consistency was estimated with Cronbach's alpha and the average inter-item correlation. In addition, the test-retest reliability of the instrument was determined at a two-week interval between baseline and follow-up (n = 109).RESULTS: The spiritual care competence scale comprises six spiritual-care-related nursing competencies. These domains were labelled: 1 assessment and implementation of spiritual care (Cronbach's alpha 0.82) 2 professionalisation and improving the quality of spiritual care (Cronbach's alpha 0.82) 3 personal support and patient counseling (Cronbach's alpha 0.81) 4 referral to professionals (Cronbach's alpha 0.79) 5 attitude towards the patient's spirituality (Cronbach's alpha 0.56) 6 communication (Cronbach's alpha 0.71). These subscales showed good homogeneity with average inter-item correlations >0.25 and a good test-retest reliability.CONCLUSION: This study conducted in a nursing-student population demonstrated valid and reliable scales for measuring spiritual care competencies. The psychometric quality of the instrument proved satisfactory. This study does have some methodological limitations that should be taken into account in any further development of the spiritual care competence scale.RELEVANCE TO CLINICAL PRACTICE: The spiritual care competence scale can be used to assess the areas in which nurses need to receive training in spiritual care and can be used to assess whether nurses have developed competencies in providing spiritual care.

Abstract:OBJECTIVES This study assessed the perceived impact of oral health conditions, and the relationship of two measures of self-reported outcome, the RAND SF-36 and the Oral Health Impact Profile (OHIP), to clinical indicators of oral health among inner-city adolescents. METHODS A convenience sample of 93 minority adolescents completed the RAND SF-36 and the OHIP and 76 of them completed a clinical dental examination assessing DMFS. RESULTS Participants averaged 14.4 (SD = 1.2) years old; 52 percent were female; and 86 percent were African-American. The mean DMFS was 8.8 (SD = 6.3). Participants reported relatively poor general health on the SF-36 as well as poor oral health on the OHIP. None of the SF-36 subscales were significantly related to DMFS. OHIP subscales were consistently related to DMFS--those with worse oral health reported more impacts. With the exception of the bodily pain subscale of the SF-36, the SF-36 and OHIP subscales were significantly correlated with Pearson's correlations ranging from -.21 to -.57 (P < .05). CONCLUSIONS Although the SF-36 and the OHIP were correlated, the OHIP appears to be more highly associated with the impacts of oral health conditions than the SF-36 among inner-city adolescents who reported low general and oral health quality of life.

Abstract:Interprofessional collaborative practice is a core competency and is the key to strengthening health practice systems in order to deliver safe and high-quality nursing practice. However, there is no Interprofessional Collaboration Practice Competency Scale (IPCPCS) for clinical nurses in Taiwan. Therefore, the purposes of this study were to develop an IPCPCS and to verify its reliability and validity. This was a psychometric study with a cross-sectional survey using convenience sampling to recruit nurses from the seven hospitals of a medical foundation. A self-designed structured IPCPCS was rolled out via a Google survey. The data were analyzed using descriptive statistics, principal-axis factoring (PAF) with Promax rotation, Pearson correlation, reliability analysis, and one-way ANOVA. PAF analysis found that three factors could explain 77.76% of cumulative variance. These were collaborative leadership and interprofessional conflict resolution, interprofessional communication and team functioning, and role clarification and client-centered care. The internal consistency of the three factors (Cronbach's α) was between 0.970 to 0.978, and the Pearson correlation coefficients were between 0.814 to 0.883. Significant differences were presented in the IPCPCS score by age, education level, total years of work experience, position on the nursing clinical ladder, and participation in interprofessional education. In conclusion, the three factors used in the IPCPCS have good reliability and construct validity. This scale can be used as an evaluation tool of in-service interprofessional education courses for clinical nurses.

Abstract:Proteinuria and progressive renal insufficiency are the primary manifestations of diabetic nephropathy. Accumulating evidence suggests that these clinical features can be linked, at least in part, to pathologic changes in the glomerular extracellular matrix. Most evidence suggests that glomerular basement membrane thickening and mesangial matrix expansion consist of at least three elements. These are (1) an accumulation of normal extracellular components; (2) an increase in the novel peptide chains of the normal components of Type IV collagen; and (3) an increase in matrix elements not normally expressed in the glomerulus. The pathogenetic features underlying these changes include increased synthesis and decreased degradation of matrix. Abnormal physico-chemical interactions among these matrix elements likely contribute to alterations in three-dimensional structure, leading to proteinuria and loss of glomerular basement membrane filtering surface area. Many of these changes may be explained in whole or in part by direct or secondary effects of hyperglycemia, as well as by hemodynamic changes.

Abstract:Background: Knowledge-sharing behaviors among academic members in Iran were believed to enhance organizational sustainability. However, the current status of these behaviors was unknown due to the lack of a valid scale to assess them. Objectives: The present study was conducted to validate the Knowledge Sharing Behavior Scale (KSBS) among faculty members of nursing and midwifery schools in Iran. Methods: This research was a cross-sectional study of scale psychometric type. The samples included 640 faculty members of nursing and midwifery schools across the country in 2021. Validation was performed through face validity, content validity by determining content validity index (CVI) and content validity ratio (CVR), construct validity by determining exploratory factor analysis (EFA) and confirmatory factor analysis (CFA), concurrent validity by determining the correlation between KSBS and Knowledge Management Questionnaire (KMQ) and Tacit Knowledge Sharing Scale (TEKS), internal consistency reliability (Cronbach's α coefficient), and stability reliability (test-retest) were determined. SPSS version 21 and LISREL version 8.8 were used for data analysis. The significance level was 0.01. Results: Items were edited based on the comments of the target community. Content validity ratio for 28 questions ranged from 0.73 to 1, and all items had I-CVI values greater than 0.78. The results of EFA showed four factors, and CFA approved the construct homogeneity. The correlation coefficient of KSBS with KMQ and TEKS were 0.72 and 0.87, respectively. Cronbach α coefficients for the whole scale and factors were higher than 0.90. Conclusions: Knowledge Sharing Behavior Scale was a valid and reliable scale to measure knowledge-sharing behaviour in the Iranian academic community.

Abstract:Background and Purpose: The "Hospital's Culture of Nursing Research Scale" (HCNRS) is a critical assessment tool in health care research. It was created in response to the rising acknowledgment of nursing research's critical role in improving patient care outcomes. This study aimed to develop the HCNRS to assess the nurses' perceptions of the hospital's culture of nursing research. Methods: This study utilized the instrument development design in developing the HCNRS. The study was conducted in Astana, Kazakhstan, from May to September 2023. The study followed three major stages of scale development: "item development," "scale development," and "scale evaluation." "Principal component analysis" (PCA; n = 387) and "confirmatory factor analysis" (CFA; n = 362) were performed in this study. Cronbach's coefficient α was computed for reliability. Results: The PCA on the 32-item HCNRS revealed five factors with an eigenvalue of above 1, contributing 66.0% of the model's total variance. The CFA revealed the following findings supporting the five subscales of the HCNRS-R: χ 2 /df = 2.65, root mean square error of approximation = .068 (90% CI [0.063, 0.072]), comparative fit index = 0.92, Tucker-Lewis index = 0.91, and standardized root mean square residual = .04. Based on the items that loaded in each factor, the subscales were labeled "Research Ethics, Integrity, and Supportive Culture," "Nursing Research Institutionalization," "Hospital's Financial Support and Incentives for Research Initiatives," "Nursing Research Engagement and Innovation," and "Nursing Research Integration and Career Support." The computed Cronbach's coefficient α of the entire scale was .963. For its subscales, the Cronbach's coefficient α ranged from .833 to .953. Conclusions: The study developed a five-factor HCNRS to evaluate the multifaceted aspects of nursing research culture in hospital settings. Health care institutions can utilize this scale to identify areas for improvement in research culture and implement strategies to foster research ethics, integrity, and innovation. This tool can also be used by hospital leaders, policymakers, and nurse researchers seeking to evaluate the research culture within their health care organizations.
