Abstract:This research is designed to examine the recurrence of non-small lung cancer (NSCLC) prediction using genomics information to reach the maximum accuracy. The raw gene data show very good performance but require more precise examination. This work is study about the way to reduce the complexity of the gene data with minimal information loss. This processed gene data tends to have the ability to archive the reasonable prediction result with faster process. This work presents a comparison of the operations of the two steps, including gene selection and gene quantization, Linear quantization and K-mean quantization, using associated gene selected from 88 patient sample from the open-access dataset of non-small cell lung cancer in The Cancer Imaging Archive Public Access. We use the different number of the group splitting and compare the performance of the recurrence prediction in both operations. The results of this study show us that the F-test method can provide us the best gene set that related to NSCLC recurrence. With F-test without quantization, accuracy of the prediction has been improved from 81.41% (using 5587 genes) to 91.83% (using selected 294 genes). With quantization methods, the suitable gene groups separation can maximize the accuracy to 93.42% using K-mean quantization.
Deep Learning-based Risk Prediction Model for Recurrence-free Survival in Patients with Hepatocellular Carcinoma Using Multi-phase CT Image

Abstract:Risk prediction for recurrence is a critical task for patients with hepatocellular carcinoma (HCC). Effective prediction can evaluate treatment options and guide personalized medicine. Traditional approaches use clinical data to construct cox-regression models to predict the risk of recurrence. Recently, prognosis analysis based on image information of patient often has better performance compared with traditional approaches. In particular, deep learning has demonstrated its superiority in medical image processing. In this paper, we propose a deep learning-based model to predict the risk of recurrence. We collected 292 patients with HCC from two independent centers, which were divided into a training set, an internal validation set, and an external validation set. Our models were compared with traditional clinical models and better performance was obtained. Our proposed method achieves a C-index performance of 0.627 and 0.630 for the internal validation set and external validation set, respectively.
Deep Learning-Based Radiomics Models for Early Recurrence Prediction of Hepatocellular Carcinoma with Multi-phase CT Images and Clinical Data.

Abstract:Hepatocellular carcinoma (HCC) is a primary liver cancer that produces a high mortality rate. It is one of the most common malignancies worldwide, especially in Asia, Africa, and southern Europe. Although surgical resection is an effective treatment, patients with HCC are at risk of recurrence after surgery. Preoperative early recurrence prediction for patients with liver cancer can help physicians develop treatment plans and will enable physicians to guide patients in postoperative follow-up. However, the conventional clinical data based methods ignore the imaging information of patients. Certain studies have used radiomic models for early recurrence prediction in HCC patients with good results, and the medical images of patients have been shown to be effective in predicting the recurrence of HCC. In recent years, deep learning models have demonstrated the potential to outperform the radiomics-based models. In this paper, we propose a prediction model based on deep learning that contains intra-phase attention and inter-phase attention. Intra-phase attention focuses on important information of different channels and space in the same phase, whereas inter-phase attention focuses on important information between different phases. We also propose a fusion model to combine the image features with clinical data. Our experiment results prove that our fusion model has superior performance over the models that use clinical data only or the CT image only. Our model achieved a prediction accuracy of 81.2%, and the area under the curve was 0.869.
A Unified Framework for Preoperative Early Recurrence Prediction of Hepatocellular Carcinoma with Multi-phase CT Images

Abstract:Early recurrence (ER) after surgery is one of the main causes of death in Hepatocellular Carcinoma (HCC). Making an effective prediction of the risk of ER is important in developing treatment strategies. The existing methods generally use radiomic features or simple deep learning features, which have insufficient feature representation capabilities. Here, we present a deep learning-based strategy for obtaining enriched multi-phase fusion features that can effectively represent feature information. We present an effective preoperative prediction model that uses multi-phase CT scans as input. Our proposed framework significantly utilizes and enhances multi-phase contextual information by effectively performing multi-phase feature fusion. We also present a novel backbone network with attention mechanisms called Inv-ResNet, that can extract more discriminative features from each CT phase using ResNet block and involution operator. Further, we design a Multi-Phase Feature Fusion Module that allows for global-Squeeze and phase-Excitation of important feature weights across all CT phases to selectively fuse features. In comparison to existing methods of preoperative prediction of ER of HCC, our method achieves 72.70% accuracy and a value of 0.7478 area under the ROC curve (AUC), which is the highest accuracy.

Abstract:Lung cancer is the leading cause of cancer related mortality by a significant margin. While new technologies, such as image segmentation, have been paramount to improved detection and earlier diagnoses, there are still significant challenges in treating the disease. In particular, despite an increased number of curative resections, many postoperative patients still develop recurrent lesions. Consequently, there is a significant need for prognostic tools that can more accurately predict a patient's risk for recurrence. In this paper, we explore the use of convolutional neural networks (CNNs) for the segmentation and recurrence risk prediction of lung tumors that are present in preoperative computed tomography (CT) images. First, expanding upon recent progress in medical image segmentation, a residual U-Net is used to localize and characterize each nodule. Then, the identified tumors are passed to a second CNN for recurrence risk prediction. The system's final results are produced with a random forest classifier that synthesizes the predictions of the second network with clinical attributes. The segmentation stage uses the LIDC-IDRI dataset and achieves a dice score of 70.3%. The recurrence risk stage uses the NLST dataset from the National Cancer institute and achieves an AUC of 73.0%. Our proposed framework demonstrates that first, automated nodule segmentation methods can generalize to enable pipelines for a wide range of multitask systems and second, that deep learning and image processing have the potential to improve current prognostic tools. To the best of our knowledge, it is the first fully automated segmentation and recurrence risk prediction system.
SAMA: A Self-and-Mutual Attention Network for Accurate Recurrence Prediction of Non-Small Cell Lung Cancer Using Genetic and CT Data

Abstract:Accurate preoperative recurrence prediction for non-small cell lung cancer (NSCLC) is a challenging issue in the medical field. Existing studies primarily conduct image and molecular analyses independently or directly fuse multimodal information through radiomics and genomics, which fail to fully exploit and effectively utilize the highly heterogeneous cross-modal information at different levels and model the complex relationships between modalities, resulting in poor fusion performance and becoming the bottleneck of precise recurrence prediction. To address these limitations, we propose a novel unified framework, the Self-and-Mutual Attention (SAMA) Network, designed to efficiently fuse and utilize macroscopic CT images and microscopic gene data for precise NSCLC recurrence prediction, integrating handcrafted features, deep features, and gene features. Specifically, we design a Self-and-Mutual Attention Module that performs three-stage fusion: the self-enhancement stage enhances modality-specific features; the gene-guided and CT-guided cross-modality fusion stages perform bidirectional cross-guidance on the self-enhanced features, complementing and refining each modality, enhancing heterogeneous feature expression; and the optimized feature aggregation stage ensures the refined interactive features for precise prediction. Extensive experiments on both publicly available datasets from The Cancer Imaging Archive (TCIA) and The Cancer Genome Atlas (TCGA) demonstrate that our method achieves state-of-the-art performance and exhibits broad applicability to various cancers.

Abstract:Purpose: Radical surgery is the primary treatment for early-stage resectable lung cancer, yet recurrence after curative surgery is not uncommon. Identifying patients at high risk of recurrence using preoperative computed tomography (CT) images could enable more aggressive surgical approaches, shorter surveillance intervals, and intensified adjuvant treatments. This study aims to analyze lung cancer sites in CT images to predict potential recurrences in high-risk individuals. Methods: We retrieved anonymized imaging and clinical data from an institutional database, focusing on patients who underwent curative pulmonary resections for non-small cell lung cancers. Our study used a deep learning model, the Mask Region-based Convolutional Neural Network (MRCNN), to predict cancer locations and assign recurrence classification scores. To find optimized trained weighted values in the model, we developed preprocessing python codes, adjusted dynamic learning rate, and modifying hyper parameter in the model. Results: The model training completed; we performed classifications using the validation dataset. The results, including the confusion matrix, demonstrated performance metrics: bounding box (0.390), classification (0.034), mask (0.266), Region Proposal Network (RPN) bounding box (0.341), and RPN classification (0.054). The model successfully identified lung cancer recurrence sites, which were then accurately mapped onto chest CT images to highlight areas of primary concern. Conclusion: The trained model allows clinicians to focus on lung regions where cancer recurrence is more likely, acting as a significant aid in the detection and diagnosis of lung cancer. Serving as a clinical decision support system, it offers substantial support in managing lung cancer patients.

Abstract:OBJECTIVE: To assist clinicians in arranging personalized treatment, planning follow-up programs and extending survival times for non-small cell lung cancer (NSCLC) patients, a method of deep learning combined with computed tomography (CT) imaging for survival prediction was designed.METHODS: Data were collected from 484 patients from four research centers. The data from 344 patients were utilized to build the A_CNN survival prognosis model to classify 2-year overall survival time ranges (730 days cut-off). Data from 140 patients, including independent internal and external test sets, were utilized for model testing. First, a series of preprocessing techniques were used to process the original CT images and generate training and test data sets from the axial, coronal, and sagittal planes. Second, the structure of the A_CNN model was designed based on asymmetric convolution, bottleneck blocks, the uniform cross-entropy (UC) loss function, and other advanced techniques. After that, the A_CNN model was trained, and numerous comparative experiments were designed to obtain the best prognostic survival model. Last, the model performance was evaluated, and the predicted survival curves were analyzed.RESULTS: The A_CNN survival prognosis model yielded a high patient-level accuracy of 88.8%, a patch-level accuracy of 82.9%, and an area under the receiver operating characteristic (ROC) curve (AUC) of 0.932. When tested on an external data set, the maximum patient-level accuracy was 80.0%.CONCLUSIONS: The results suggest that using a deep learning method can improve prognosis in patients with NSCLC and has important application value in establishing individualized prognostic models.

Abstract:Background: Prognostic risk factors for completely resected stage IA non-small-cell lung cancers (NSCLCs) have advanced minimally over recent decades. Although several biomarkers have been found to be associated with cancer recurrence, their added value to TNM staging and tumor grade are unclear. Methods: Features of preoperative low-dose CT image and histologic findings of hematoxylin- and eosin-stained tissue sections of resected lung tumor specimens were extracted from 182 stage IA NSCLC patients in the National Lung Screening Trial. These features were combined to predict the risk of tumor recurrence or progression through integrated deep learning evaluation (IDLE). Added values of IDLE to TNM staging and tumor grade in progression risk prediction and risk stratification were evaluated. Results: The 5-year AUC of IDLE was 0.817 ± 0.037 as compared to the AUC = 0.561 ± 0.042 and 0.573 ± 0.044 from the TNM stage and tumor grade, respectively. The IDLE score was significantly associated with cancer recurrence (p < 0.0001) even after adjusting for TNM staging and tumor grade. Synergy between chest CT image markers and histological markers was the driving force of the deep learning algorithm to produce a stronger prognostic predictor. Conclusions: Integrating markers from preoperative CT images and pathologist’s readings of resected lung specimens through deep learning can improve risk stratification of stage 1A NSCLC patients over TNM staging and tumor grade alone. Our study suggests that combining markers from nonoverlapping platforms can increase the cancer risk prediction accuracy.
Multimodal Fusion of Imaging and Genomics for Lung Cancer Recurrence Prediction

Abstract:Lung cancer has a high rate of recurrence in early-stage patients. Predicting the post-surgical recurrence in lung cancer patients has traditionally been approached using single modality information of genomics or radiology images. We investigate the potential of multimodal fusion for this task. By combining computed tomography (CT) images and genomics, we demonstrate improved prediction of recurrence using linear Cox proportional hazards models with elastic net regularization. We work on a recent non-small cell lung cancer (NSCLC) radiogenomics dataset of 130 patients and observe an increase in concordance-index values of up to 10%. Employing non-linear methods from the neural network literature, such as multi -layer perceptrons and visual-question answering fusion modules, did not improve performance consistently. This indicates the need for larger multimodal datasets and fusion techniques better adapted to this biological setting.

Abstract:It is critical for patients who cannot undergo eradicable surgery to predict the risk of lung cancer recurrence and metastasis; therefore, the physicians can design the appropriate adjuvant therapy plan. However, traditional circulating tumor cell (CTC) detection or next-generation sequencing (NGS)-based methods are usually expensive and time-inefficient, which urge the need for more efficient computational models. In this study, we have established a convolutional neural network (CNN) framework called DeepLRHE to predict the recurrence risk of lung cancer by analyzing histopathological images of patients. The steps for using DeepLRHE include automatic tumor region identification, image normalization, biomarker identification, and sample classification. In practice, we used 110 lung cancer samples downloaded from The Cancer Genome Atlas (TCGA) database to train and validate our CNN model and 101 samples as independent test dataset. The area under the receiver operating characteristic (ROC) curve (AUC) for test dataset was 0.79, suggesting a relatively good prediction performance. Our study demonstrates that the features extracted from histopathological images could be well used to predict lung cancer recurrence after surgical resection and help classify patients who should receive additional adjuvant therapy.

Abstract:Preoperative prediction of recurrence outcome in hepatocellular carcinoma (HCC) facilitates physicians’ clinical decision-making. Preoperative imaging and related clinical baseline data of patients are valuable for evaluating prognosis. With the widespread application of machine learning techniques, the present study proposed the ensemble learning method based on efficient feature representations to predict recurrence outcomes within three years after surgery. Radiomics features during arterial phase (AP) and clinical data were selected for training the ensemble models. In order to improve the efficiency of the process, the lesion area was automatically segmented by 3D U-Net. It was found that the mIoU of the segmentation model was 0.8874, and the Light Gradient Boosting Machine (LightGBM) was the most superior, with an average accuracy of 0.7600, a recall of 0.7673, a F 1 score of 0.7553, and an AUC of 0.8338 when inputting radiomics features during AP and clinical baseline indicators. Studies have shown that the proposed strategy can relatively accurately predict the recurrence outcome within three years, which is helpful for physicians to evaluate individual patients before surgery.

Abstract:Identification of patients with early stage non-small cell lung cancer (NSCLC) with high risk of recurrence could help identify patients who would receive additional benefit from adjuvant therapy. In this work, we present a computational histomorphometric image classifier using nuclear orientation, texture, shape, and tumor architecture to predict disease recurrence in early stage NSCLC from digitized H&E tissue microarray (TMA) slides. Using a retrospective cohort of early stage NSCLC patients (Cohort #1, n = 70), we constructed a supervised classification model involving the most predictive features associated with disease recurrence. This model was then validated on two independent sets of early stage NSCLC patients, Cohort #2 (n = 119) and Cohort #3 (n = 116). The model yielded an accuracy of 81% for prediction of recurrence in the training Cohort #1, 82% and 75% in the validation Cohorts #2 and #3 respectively. A multivariable Cox proportional hazard model of Cohort #2, incorporating gender and traditional prognostic variables such as nodal status and stage indicated that the computer extracted histomorphometric score was an independent prognostic factor (hazard ratio = 20.81, 95% CI: 6.42–67.52, P < 0.001).

Abstract:Non-small cell Lung Cancer (NSCLC) is one of the most common types of lung cancer, accounting for approximately 80% to 85% of lung cancer cases, and survival prediction in this cancer is an essential medical task. Traditional survival prediction methods, which only rely on demographic and clinical variables, fail to fully characterize patients' pathological characteristics and clinical factors, thus limiting the prediction effect. With the development of medical imaging technology and genomic methods, survival prediction studies have ushered in new analytical perspectives in recent years. However, most of the existing advanced techniques only rely on one class and few classes of medical data, which do not comprehensively characterize patients' pathological features and clinical conditions, and similarly limit the predictive effect. To this end, in this paper a new method is proposed to solve this problem, that is, using flexible interpretable graph structure to fuse and model the multimodal data (including clinical data and CT images, etc.) of patients, so as to solve the problem of fragmentation and one-sidedness among multi-class data of survival prediction. A new multi-modal fusion graph convolutional network (FGCN) is proposed according to the characteristics of multi-modal graph data. The main body of the network structure is composed of a SAGE graph convolution layer with the self-attention mechanism, which accelerates the convergence of the model compared to the traditional one and ensures that the risk predicted by the model is as close to the actual situation as possible. More importantly, this study is the first to introduce the TopKPooling strategy in this model to address the problems of feature redundancy and excessive noise features arising from multimodal data fusion and to reduce the model complexity. A large number of ablation and comparison experiments and analyses on public non-small cell lung cancer datasets show that the method proposed in this paper achieves better prediction results, with a C-index value of 0.76, which exceeds the currently known advanced techniques, and fully proves the effectiveness and superiority of the method in this paper.

Abstract:Purpose: Nasopharyngeal carcinoma is one of the most prevalent malignant tumors in Guangdong, China. In the field of medicine, predicting the recurrence of early -stage nasopharyngeal carcinoma after radiotherapy holds significant importance. Our objective was to develop a novel classification model aimed at predicting recurrence following radiotherapy for early nasopharyngeal carcinoma. Methods and Materials: This paper introduces an innovative approach by combining unsupervised diagnostic feature learning (biclustering algorithm) with Adaboost ensemble learning to create a novel classification model. Notably, the nasopharyngeal carcinoma dataset underwent biclustering algorithm application for the first time. Initially, the Borderline -SMOTE oversampling algorithm was employed to address the dataset's imbalance issue. Subsequently, the biclustering algorithm, which is based on an improved multi -objective genetic algorithm (NSGA-II), was utilized to seek bicluster outcomes exhibiting consistent representation patterns. The attributes of these bicluster outcomes were assessed and employed as diagnostic rules for constructing weak classifiers. Ultimately, the Adaboost ensemble learning technique was employed to amalgamate the weak classifiers into a robust classifier. Results: Following the application of 10 -fold cross -validation, the model exhibited an accuracy of 80.33%, sensitivity of 79.55%, specificity of 80.43%, and GMean of 79.99%, accompanied by an AUC value of 0.904. Conclusions: The presented classification model outperformed alternative classification models in terms of classification accuracy and generalization when applied to the nasopharyngeal carcinoma dataset. Consequently, it serves as a valuable tool to aid medical professionals in predicting the likelihood of recurrence in the cervical lymph nodes following radiotherapy for early -stage nasopharyngeal carcinoma.

Abstract:Non-small cell lung cancer (NSCLC) is a serious disease and has a high recurrence rate after the surgery. Recently, many machine learning methods have been proposed for recurrence prediction. The methods using gene data have high prediction accuracy but require high cost. Although the radiomics signatures using only CT image are not expensive, its accuracy is relatively low. In this paper, we propose a genotype-guided radiomics method (GGR) for obtaining high prediction accuracy with low cost. We used a public radiogenomics dataset of NSCLC, which includes CT images and gene data. The proposed method is a two-step method, which consists of two models. The first model is a gene estimation model, which is used to estimate the gene expression from radiomics features and deep features extracted from computer tomography (CT) image. The second model is used to predict the recurrence using the estimated gene expression data. The proposed GGR method designed based on hybrid features which is combination of handcrafted-based and deep learning-based. The experiments demonstrated that the prediction accuracy can be improved significantly from 78.61% (existing radiomics method) and 79.14% (deep learning method) to 83.28% by the proposed GGR.

Abstract:Rationale and Objectives We constructed a comprehensive model by combining the radiomics and clinical features of tumors to predict the recurrence risk of patients with operable stage IA–IIIA non-small cell lung cancer (NSCLC). Our aim was to improve the accuracy of prognostic prediction and provide personalized treatment plans to enhance patient outcomes. Materials and Methods We retrospectively analyzed 152 surgically treated patients with pathologically confirmed stage IA–IIIA NSCLC. These patients were randomly divided into a training cohort and a test cohort in an 8:2 ratio. Using the 3D Slicer image computing platform, we manually delineated the regions of interest (ROI) for all lesions and extracted radiomics features using Python. We used the Least Absolute Shrinkage and Selection Operator (LASSO) to select the radiomics features, while the COX multivariate regression model was employed to identify independent clinical risk factors for recurrence. Finally, we utilized logistic regression (LR) to build the model and validated it using the receiver operating characteristic curve (ROC). The predictive performance of the model was evaluated using the concordance index (C-index), and the clinical value of the model was compared through decision curve analysis (DCA). Results We extracted a total of 1562 radiomics features. After feature selection, we retained 29 features. The COX multivariate regression model demonstrated that the N stage was an independent risk factor for postoperative recurrence. In the training and test cohorts, the area under the curve (AUC) values of the radiomics-clinical comprehensive model were 0.972 and 0.937, respectively, while the C-index values were 0.815 and 0.847. These values surpassed those of the standalone clinical model or radiomics model. Conclusion Our study demonstrates that a comprehensive model based on CT radiomics and clinical features can effectively stratify the risk of postoperative recurrence in patients with operable NSCLC. It provides a powerful tool for accurately stratifying the risk of high-risk patients after surgery.

Abstract:Early diagnosis of lung cancer to increase the survival rate, which is currently at a low range of mid-30%, remains a critical need. Despite this, multi-omics data have rarely been applied to non-small-cell lung cancer (NSCLC) diagnosis. We developed a multi-omics data-affinitive artificial intelligence algorithm based on the graph convolutional network that integrates mRNA expression, DNA methylation, and DNA sequencing data. This NSCLC prediction model achieved a 93.7% macro F1-score, indicating that values for false positives and negatives were substantially low, which is desirable for accurate classification. Gene ontology enrichment and pathway analysis of features revealed that two major subtypes of NSCLC, lung adenocarcinoma and lung squamous cell carcinoma, have both specific and common GO biological processes. Numerous biomarkers (i.e., microRNA, long non-coding RNA, differentially methylated regions) were newly identified, whereas some biomarkers were consistent with previous findings in NSCLC (e.g., SPRR1B). Thus, using multi-omics data integration, we developed a promising cancer prediction algorithm.
