Abstract:Abstract Objective Our aim is to establish a machine-learning model that will enable us to investigate the key factors influencing the prevalence of myopia in students. Methods We performed a cross-sectional study that included 16,653 students from grades 1–3 across 17 cities in Hubei Province. We used questionnaires to discern levels of participation in potential factors contributing to the development of myopia. The relative importance of potential contributors was ranked using machine-learning methods. The students’ visual acuity (VA) was measured and those with logMAR VA of > 0.0 underwent a autorefraction test to determine students’ refraction status. Results The prevalence of myopia in grades 1, 2, and 3 was 14.70%, 20.54% and 28.93%, respectively. Myopia rates among primary school students in provincial capital city (32.35%) were higher than those in other urban (23.03%) and rural (14.82%) areas. Children with non-myopic parents, only one myopic parent, or both parents having myopia exhibited myopic rates of 16.36%, 25.18%, and 41.37%, respectively. Myopia prevalence was higher in the students who continued to use their eyes at close range for a long time and lower in those engaged longer in outdoor activities. The machine-learning model determined that the top three contributing factors were the students’ age (0.36), followed by place of residence (0.34), starting age of education (0.21). Conclusion The overall prevalence of myopia was 21.52%. Children’s age and place of residence were the important influencing factors, but genetics and environmental were also played key roles in myopia development.

Abstract:Objective To investigate the associated factors leading to myopia in primary school students in a relatively developed region in Hangzhou.Methods This was a cross-sectional study.By cluster sampling,primary students from 5 schools (grades 1-6) in Zhaohui District,Hangzhou,were recruited for this investigation.The right eye was examined and data were collected on spherical refraction,cylinder refraction,axial length and corneal curvature.Information was also obtained on family history,time spent on near reading after school and on weekends,and time spent on outdoor activities both after school and on weekends.A one-way ANOVA was used for analysis.Results There were significant differences in gender,family history,time spent on near reading and outdoor activity between myopic and non-myopic groups.Females were more likely to be myopic than males (63.7％ vs.67.4％).The students with myopic parents (F=10.559,P＜0.05) or those who spent more time on near reading on weekends (F=7.555,P＜0.05) and after school (F=2.492,P＜0.05) were more likely to be myopic.On the other hand,outdoor activities both after school (F=11.351,P＜0.05) and during weekends (F=6.387,P＜0.05) reduced the prevalence of myopia.Conclusion Myopia in primary school students is associated with gender,family history,near reading and outdoor activity.So both genetic and environmental factors contribute to the development of myopia.The effect of environmental factors,especially near reading,is time-dependent,and ultraviolet exposure during outdoor activity plays a significant role.

Abstract:With the rapid development of science and technology, the trend of low age myopia is becoming increasingly significant. The latest national survey done by the Chinese government found that more than 80% of Chinese teenagers suffer from myopia. Adolescent myopia is closely related to living environment, heredity, and living habits. Quantifying the relationship between myopia and living environment, heredity, and living habits is conductive to the prevention and intervention of adolescent myopia. In this study, we investigated the relationships between four main factors (environment, habits, parental vision, and demographic) and myopia status by analyzing the questionnaire data. Data were collected from Chengdu, China in 2021, including 2808 myopia samples and 5693 non-myopia samples, with a total of 22 features. Then, these 22 features were inputted into three machine learning algorithms to discriminate the two classes of samples. Results show that the computational model could produce an AUC of 0.768. To pick out the most important features which play important roles in classification, we used incremental feature selection strategy to screen the 22 features. As a result, we found that the 4 most influential features with XGBoost could achieve a competitive AUC of 0.764. To further investigate the risk and protective factors affecting adolescent myopia, we used OR values derived from MLE-LR to analyze the relationship between 22 features and adolescent myopia. Results showed that the age variable was the most significant risk factor for myopia, followed by the myopia status of parents. The most protective factor for eyesight is the measure taken by the children, followed by the distance between books and eyes when reading. These discoveries can guide the prevention and control of myopia in children and adolescents.

[Prevalence of myopia and influencing factors among primary and middle school students in 6 provinces of China]

Abstract:Objective: To understand the prevalence of myopia in primary and middle school students in 6 provinces and the possible influencing factors. Methods: Primary and middle school students were selected through multistage cluster sampling in 60 primary and middle schools in 6 provinces in China. The questionnaire survey and eyesight test were conducted among all the students selected according to the national student's physique and health survey protocol. Pearson chi-square test and binary multivariate logistic regression analysis were done to identify the influencing factors for myopia in students. Results: The prevalence of myopia among primary and middle school students surveyed was 55.7%, the gender specific difference was statistically significant (59.7% for girls, 51.9% for boys) (P<0.01). The prevalence of myopia increased with age obviously. The prevalence was 35.8% in age group 6-8 years, 58.9% in age group 10-12 years, 73.4% in age group 13-15 years and 81.2% in age group 16-18 years, the differences were statistically significant (P<0.001). Single factor and multivariate analysis showed that parents' myopia, distance between computer screen and eyes, distance less than 30 cm between eyes and book while reading, distance less than 10 cm between chest and the table edge while studying, distance less than 3 cm between fingers and pen tip, sleep time, average outdoor activity time during last week, school sport activities in the afternoon, the size of television set at home, time spent on watching TV and playing computer were the influencing factors for myopia. Conclusions: The prevalence of myopia is till high in primary and middle school students. Myopia is associated with both genetic factors and individual eye health related behaviors.

Abstract:Abstract Background Myopia is prevalent in children and adolescents. Understanding the effect of multiple behaviors and their latent patterns on ocular biometric parameters may help clinicians and public health practitioners understand the behavioral risk pattern of myopia from a person-centered perspective. The purpose of this study was to identify the patterns of four major behavioral risk factors associated with myopia, including time spent outdoors, digital screen time, sleep duration, and performance of Chinese eye exercises. The study also examined the relationships between these behavioral patterns and myopia as well as ocular biometric parameters in a sample of Chinese college students. Methods This study included 2014 students from the Dali University Students Eye Health Study. The average age of the subjects was 19.0 ± 0.9 years old, ranging from 15.7 to 25.1 years old. Each participant’s refractive status was measured using an autorefractor without cycloplegia and ocular biometric parameters were measured using an IOL Master. Behavioral risk factors were collected using a pre-designed self-administered questionnaire. Latent class analysis (LCA) was performed to identify cluster patterns of various behaviors. Results The prevalence of myopia was 91.8% in this population. The 2-class model was selected for the LCA based on goodness-of-fit evaluation metrics. Among the overall study sample, 41.1% and 58.9% were assigned into the high-risk and low-risk class, respectively. The risk of myopia [odds ratio (OR) = 2.12, 95% confidence interval (CI) = 1.52–3.14], high myopia (OR = 1.43, 95% CI = 1.14–1.78) and axial length/corneal radius (AL/CR) ratio of more than 3.0 (OR = 1.82, 95% CI = 1.22–2.72) were significantly higher in the high-risk compared with low-risk class. Conclusions Chinese university students showed differential risks of myopia and could be subdivided into high- and low-risk clusters based on four behavioral variables.

Abstract:The bispectral index (BIS) is widely used to measure anesthetic levels in the perioperative period. In our hospital, the BIS monitors (A-2000) are connected to bedside monitors and display BIS values on them through outside input. All datas on the bedside monitors are transmitted to anesthesia information management system automatically. We report cases that were impossible to measure BIS values with the displays of "Invalid Sensor" on the BIS A-2000 sensor, but BIS values were measurable on the bedside monitors. One possible reason is thought that the BIS Quatro Sensor transmits both EEG signals and sensor information to the BIS A-2000 monitor separately. BIS A-2000 monitors may process EEG signals and sensor information individually, and transfer BIS parameters to the external equipments, regardless of sensor information.

Abstract:According to literature, myopia has become the second most common eye disease in China, and the incidence of myopia is increasing year by year, and showing a trend of younger age. Previous researches have shown that the occurrence of myopia is mainly determined by poor eye habits, including reading and writing posture, eye length, and so on, and parents' heredity. In order to better prevent myopia in adolescents, this paper studies the influence of related factors on myopia incidence in adolescents based on machine learning method. A feature selection method based on both univariate correlation analysis and multivariate correlation analysis is used to better construct a feature sub-set for model training. A method based on GBRT is provided to help fill in missing items in the original data. The prediction model is built based on SVM model. Data transformation has been used to improve the prediction accuracy. Results show that our method could achieve reasonable performance and accuracy.

Abstract:120 children between ages 4 and 8 were administered a set of 4 visual perspective-taking tasks similar to ones employed in previous investigations. It was hypothesized that children's competence with the tasks would be a function of the number and type of spatial relationships embedded in the stimulus displays. 2 kinds of spatial relationships were described: (1) internal relationships--those between objects within a display; and (2) external relationships--those between the display and a frame of reference. Of the 4 task types, type A tasks necessitated awareness of possible variations in external relations, types B and C tasks required both awareness and computation of changes in those external relationships, and type D tasks required simultaneous coordination of internal and external spatial relationships. It was predicted that the tasks would sequence in terms of difficulty level as A less than B, B = C, and C less than D. Analysis of variance results on the obtained data revealed a good fit with the hypothesized predictions. The findings were discussed in terms of an information-processing analysis of children's performance on perspective-taking tasks.

Abstract:To analyze the prevalence and influencing factors of myopia among children and adolescents in Hangzhou city, and provide ideas for the formulation of myopia prevention and control measures. A cross-sectional school-based study using stratified cluster random sampling method was used to select 31,880 students from 113 schools in Hangzhou city. Myopia screening and questionnaires on the influencing factors of poor visual acuity were carried out. Univariate and multivariate logistic regression were used to screen possible risk factors of myopia. The overall prevalence of myopia was found to be 55.3% (17,630/31,880), with the myopia rates observed among students in kindergarten, primary school, junior high school, and senior high school, at 5.8%, 34.9%, 74.2%, and 85.0%, respectively. As the academic year progresses, there has been an observable increase in the prevalence of myopia (χ 2 = 8676.748, P <0.001), low myopia (χ 2 = 5444.040, P <0.001) and high myopia (χ 2 = 2171.073, P <0.001) among students, with the difference between the three groups being statistically significant. The myopia prevalence in girls is higher than that in boys (χ 2 = 119.917, P <0.001); the prevalence of myopia in urban students is higher than that in suburban students (χ 2 = 4.603, P = 0.032). The time spent on homework after school, the correctness of reading and writing postures, the duration of exposure to electronic screens, and the use of lighting in an appropriate manner all have an impact on the development of myopia (all P <0.05). The overall prevalence of myopia and high myopia in schools in eastern China, is notably high. The prevalence of myopia and high myopia increases with grade or age. Myopia is associated with increased age, parental myopia, extended periods of homework after school, poor reading and writing posture, prolonged exposure to electronic screens and inappropriate lighting. In light of the findings of the present survey, it is recommended that a comprehensive prevention and control intervention model for myopia in children and adolescents based on society-hospital-school-family-individual students with a view to preventing and controlling myopia.

A Study of the Predictive Effects of Machine Learning for the Relationship Between Axial Length Elongation and the Progression of Myopia in School-Aged Children

Abstract:Objective To investigate the relationship between axial length (AL)elongation and the progression of spherical equivalent refraction (SER) and its influential factors in school-aged children with myopia based on machine learning (ML).Methods A cross-sectional study evaluated 1 011 eyes of school-aged myopic children admitted to the optometry center of Peking University People's Hospital from January 2017 to December 2018,and data from the right eyes were used for analysis.All the collected data were used to train ML algorithms.When building predictive models,the input features included age,gender,central corneal thickness (CCT),mean K readings (K-mean),horizontal visible iris diameter (HIVD),lens power,and axial length (AL),and the output parameter was SER.A five-fold cross validation scheme randomly divided all the data into five groups,of which four were used as training data,and one group was used as validation data.This process was repeated five times so that all the data were validated by this model,which allowed a better prediction of the overall sample.The prediction accuracy of different models was evaluated by the R-value and R2.The best-performing algorithm was applied to investigate the relationship between AL elongation and the progression of SER and its influencing factors.Written informed consent was obtained from each guardian of each patient prior to entering the study cohort.This study followed the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Peking University People's Hospital (No.2019PHB280-01).Results In the comparison of the R-value and R2 of six ML algorithms based on five-fold cross validation,among all models,the best was the quadratic SVM regression model,with an R-value and R2 of 0.99 and 0.98,respectively.The results of Pearson correlation analysis showed that lens power was negatively correlated with age (r =-0.301,P＜0.01).According to the results calculated by the Bennett-Rabbetts formula,the average lens power of the 6-year-old myopic group was higher than that of the 18-year-old myopic group.According to this model,the SER change caused by AL elongation was not a constant value,which was estimated from plano to nearly-3.00 D,depending mainly on the time needed for 1-mm AL elongation.According to the results calculated by this model,the longer it took for the AL to grow by 1 mm,the smaller the corresponding SER change.In myopic children over an age span of one year,for example,from 6-7 years or 12-13 years,1-mm elongation of the AL corresponded to-2.50 D and-2.33 D of SER change,respectively.Over a three years span,for example,from 6-9 years,a 1-mm elongation of the AL corresponded to-1.77 D of SER change.Conclusions For myopic children,the longer the age span required for 1-mm elongation of the AL,the smaller the SER change.An ML algorithm can provide clinical practitioners with a relatively precise estimation for the relationship between AL elongation and myopia progression.
Accurate prediction of myopic progression and high myopia by machine learning

Abstract:Abstract Background Myopia is a leading cause of visual impairment in Asia and worldwide. However, accurately predicting the progression of myopia and the high risk of myopia remains a challenge. This study aims to develop a predictive model for the development of myopia. Methods We first retrospectively gathered 612 530 medical records from five independent cohorts, encompassing 227 543 patients ranging from infants to young adults. Subsequently, we developed a multivariate linear regression algorithm model to predict the progression of myopia and the risk of high myopia. Result The model to predict the progression of myopia achieved an R2 value of 0.964 vs a mean absolute error (MAE) of 0.119D [95% confidence interval (CI): 0.119, 1.146] in the internal validation set. It demonstrated strong generalizability, maintaining consistent performance across external validation sets: R2 = 0.950 vs MAE = 0.119D (95% CI: 0.119, 1.136) in validation study 1, R2 = 0.950 vs MAE = 0.121D (95% CI: 0.121, 1.144) in validation study 2, and R2 = 0.806 vs MAE = −0.066D (95% CI: −0.066, 0.569) in the Shanghai Children Myopia Study. In the Beijing Children Eye Study, the model achieved an R2 of 0.749 vs a MAE of 0.178D (95% CI: 0.178, 1.557). The model to predict the risk of high myopia achieved an area under the curve (AUC) of 0.99 in the internal validation set and consistently high area under the curve values of 0.99, 0.99, 0.96 and 0.99 in the respective external validation sets. Conclusion Our study demonstrates accurate prediction of myopia progression and risk of high myopia providing valuable insights for tailoring strategies to personalize and optimize the clinical management of myopia in children.

Abstract:Background Patients with high myopia have an increased lifetime risk of complications. The prevalence patterns of high myopia in children and adolescents in southern China are unclear. Early identification of high-risk individuals is critical for reducing the occurrence and development of high myopia and avoiding the resulting complications. Objective This study aimed to determine the prevalence of high myopia in children and adolescents in southern China via real-world screening data and to predict its onset by studying the risk factors for high myopia based on machine learning. Methods This retrospective school-based study was conducted in 13 cities with different gross domestic products in southern China. Through data acquisition and filtering, we analyzed the prevalence of high myopia and its association with age, school stage, gross domestic product, and risk factors. A random forest algorithm was used to predict high myopia among schoolchildren and then assessed in an independent hold-out group. Results There were 1,285,609 participants (mean age 11.80, SD 3.07, range 6-20 years), of whom 658,516 (51.2%) were male. The overall prevalence of high myopia was 4.48% (2019), 4.88% (2020), and 3.17% (2021), with an increasing trend from the age of 11 to 17 years. The rates of high myopia increased from elementary schools to high schools but decreased at all school stages from 2019 to 2021. The coastal and southern cities had a higher proportion of high myopia, with an overall prevalence between 2.60% and 5.83%. Age, uncorrected distance visual acuity, and spherical equivalents were predictive factors for high myopia onset in schoolchildren. The random forest algorithm achieved a high accuracy of 0.948. The area under the receiver operator characteristic curve (AUC) was 0.975. Both indicated sufficient model efficacy. The performance of the model was validated in an external test with high accuracy (0.971) and a high AUC (0.957). Conclusions High myopia had a high incidence in Guangdong Province. Its onset in children and adolescents was well predicted with the random forest algorithm. Efficient use of real-world data can contribute to the prevention and early diagnosis of high myopia.

Discrimination of indoor versus outdoor environmental state with machine learning algorithms in myopia observational studies

Abstract:Background: Wearable smart watches provide large amount of real-time data on the environmental state of the users and are useful to determine risk factors for onset and progression of myopia. We aim to evaluate the efficacy of machine learning algorithm in differentiating indoor and outdoor locations as collected by use of smart watches. Methods: Real time data on luminance, ultraviolet light levels and number of steps obtained with smart watches from dataset A: 12 adults from 8 scenes and manually recorded true locations. 70% of data was considered training set and support vector machine (SVM) algorithm generated using the variables to create a classification system. Data collected manually by the adults was the reference. The algorithm was used for predicting the location of the remaining 30% of dataset A. Accuracy was defined as the number of correct predictions divided by all. Similarly, data was corrected from dataset B: 172 children from 3 schools and 12 supervisors recorded true locations. Data collected by the supervisors was the reference. SVM model trained from dataset A was used to predict the location of dataset B for validation. Finally, we predicted the location of dataset B using the SVM model self-trained from dataset B. We repeated these three predictions with traditional univariate threshold segmentation method. Results: In both datasets, SVM outperformed the univariate threshold segmentation method. In dataset A, the accuracy and AUC of SVM were 99.55% and 0.99 as compared to 95.11% and 0.95 with the univariate threshold segmentation (p < 0.01). In validation, the accuracy and AUC of SVM were 82.67% and 0.90 compared to 80.88% and 0.85 with the univariate threshold segmentation method (p < 0.01). In dataset B, the accuracy and AUC of SVM and AUC were 92.43% and 0.96 compared to 80.88% and 0.85 with the univariate threshold segmentation (p < 0.01). Conclusions: Machine learning algorithm allows for discrimination of outdoor versus indoor environments with high accuracy and provides an opportunity to study and determine the role of environmental risk factors in onset and progression of myopia. The accuracy of machine learning algorithm could be improved if the model is trained with the dataset itself.

Abstract:Myopia is a common visual disorder that affects millions of people worldwide and its prevalence has been increasing in recent years. Environmental factors, such as reading time, viewing distance, and ambient lighting, have been identified as potential factors in the development of myopia. In this study, we investigated the relationship between three major factors and myopia in 120 adolescents. By collecting environmental images of the adolescents in the learning state as well as retinal fundus images, we proposed an environmental visual load (EVL) model to extract the potential information in these images. Through experimental data analysis, we found that these three major factors are closely related to the severity of myopia, and that the simultaneous exacerbation of these factors sharply increases the myopia of the eye. Our results suggest that interventions targeting these environmental factors may help prevent and manage myopia.
Influencing factors associated with high myopia in Chinese college students

Abstract:Background High myopia (HM) may elicit irreversible pathological changes in the fundus and severely impair visual quality, thereby becoming a major public health issue in China. However, the influencing factors associated with HM remain unknown in Chinese college students, whose visual quality is crucial to country development. Methods This is a cross-sectional observational study. Two thousand three hundred and fifteen undergraduate and graduate students were initially recruited from various majors in 3 universities in Tianjin, China. Under the principle of voluntary participation and informed consent, simple random sampling was conducted in the recruited subjects while maintaining balanced number of subjects from each major. After screening with inclusion and exclusion criteria, 96 undergraduate and graduate students (186 eyes) were finally included and divided into non-HM and HM groups. The eyes of subjects were examined by optical coherence tomography angiography (OCTA) for vessel density and structure thickness at the macula and optic disc, and the subjects were surveyed by an itemized questionnaire on lifestyles and study habits. Results The OCTA and questionnaire results revealed 10 factors, including hemodynamic and anatomic parameters and lifestyle metrics, with statistical significance between the non-HM and HM groups. Receiver operating characteristic curve analysis showed that vessel density of the inner retina at the macula, vessel density of the radial peripapillary capillary at the optic disc, smartphone usage time, continuous near work time, and sleeping after midnight had superior values of area under the curve (AUC > 0.700). Therefore, these 5 factors were selected for univariant and multivariant logistic regression analyses. A prediction model comprising the 5 influencing factors had an AUC of 0.940 and 95% CI of 0.908–0.972. Conclusion This study for the first time identified the vessel density of the inner retina at the macula, the vessel density of the radial peripapillary capillary at the optic disc, smartphone usage time, continuous near work time, and sleeping after midnight as influencing factors associated with HM in Chinese college students. A prediction model comprising the 5 influencing factors was proposed for calculating likelihood of a Chinese college student developing HM, based on which lifestyle improvement and medical intervention might be recommended.

Abstract:Objective To study influencing factors of myopia among pupils,and to provide basis for prevention and cure of myopia.Methods Five primary schools in Xuhui district of Shanghai were selected with cluster random sampling method,and 2 143 pupils in grade 3～5 were included.The students' behavioral and environmental factors related to myopia were investigated by the questionnaire survey.Multilevel model was used to explore the influencing factors of myopia.Results The significant influencing factors of myopia among pupils were grade,parent's eyesight and eye relaxation at individual-based level,gender,hours per week for outdoor activity,hours per day for reading and writing,hours per day for close-sight entertainment outside school and reading and writing posture were meaningful at school-based level.Conclusion Intervention measures for myopia among pupils should be taken on the basis of the actual situation in different schools.Instruct and supervise students' reading and writing posture should be emphasized by school and children's eye relaxation habit should be enhanced by parents.

Abstract:Purposes This study was to estimate the prevalence of myopia among primary school students in Hefei, China, and evaluate the association of educational environment with myopia. Methods This study was a cross-sectional study, and recruited primary school students in grades 1–6. Children underwent a stepwise ophthalmic examination, which included visual acuity and objective cycloplegic refraction to identify children with myopia. Under the guidance of parents, children completed a questionnaire, including gender, region, grade and several indicators related to education. The study analyzed the risk factors by using a logistic regression and assessed feature importance by using a random forest algorithm. Results A total of 3,596 primary school students were involved in this analysis, and the overall prevalence of myopia was 27.1%. Gender, grade, education level of the father, education level of the mother, academic level of children, hours of homework per day on weekends, number of after-school tutoring per week and frequency of extracurricular reading were significantly associated with myopia. There was no significant association between the amount of homework per day on school days and myopia after adjusting for covariates. In terms of educational environment, the top 3 factors were academic level of children, homework on weekends and after-school tutoring. Conclusions Educational environment with high educational loads was associated with the high prevalence of myopia. Reducing the burden of studying, especially that after class, was an effective way to prevent myopia.

Machine Learning Models for Predicting Cycloplegic Refractive Error and Myopia Status Based on Non-Cycloplegic Data in Chinese Students

Abstract:Purpose: To develop and validate machine learning (ML) models for predicting cycloplegic refractive error and myopia status using noncycloplegic refractive error and biometric data. Methods: Cross-sectional study of children aged five to 18 years who underwent biometry and autorefraction before and after cycloplegia. Myopia was defined as cycloplegic spherical equivalent refraction (SER) ≤-0.5 Diopter (D). Models were evaluated for predicting SER using R2 and mean absolute error (MAE) and myopia status using area under the receiver operating characteristic (ROC) curve (AUC). Best-performing models were further evaluated using sensitivity/specificity and comparison of observed versus predicted myopia prevalence rate overall and in each age group. Independent data sets were used for training (n = 1938) and validation (n = 1476). Results: In the validation dataset, ML models predicted cycloplegic SER with high R2 (0.913-0.935) and low MAE (0.393-0.480 D). The AUC for predicting myopia was high (0.984-0.987). The best-performing model for SER (XGBoost) had high sensitivity and specificity (91.1% and 97.2%). Random forest (RF), the best-performing model for myopia, had high sensitivity and specificity (92.2% and 96.9%). Within each age group, difference between predicted and actual myopia prevalence was within 4%. Conclusions: Using noncycloplegic refractive error and ocular biometric data, ML models performed well for predicting cycloplegic SER and myopia status. When measuring cycloplegic SER is not feasible, ML may provide a useful tool for estimating cycloplegic SER and myopia prevalence rate in epidemiological studies. Translational relevance: Using ML to predict cycloplegic refraction based on noncycloplegic data is a powerful tool for large, population-based studies of refractive error.
