Abstract:Introduction: The rising prevalence of obesity has become a global public health crisis. Traditional screening tools like body mass index (BMI) and waist circumference (WC) have limitations, prompting the need for simpler, more effective alternatives. Neck circumference (NC) has emerged as a promising tool due to its simplicity, affordability, and reliability. The study aimed to evaluate neck circumference as a screening tool for obesity among female adults, alongside measuring BMI, waist-to-hip ratio (WHR), WC, and NC, and establishing NC cut-off values using body fat percentage as the gold standard. Additionally, it sought to compare the predictive accuracy of these measures for assessing obesity. Methods: This cross-sectional study, conducted from March 2023 to October 2023, involved 362 female students from a health sciences university in Western Maharashtra, India. The participants provided informed consent and underwent anthropometric measurements, including height, weight, waist circumference, hip circumference, body fat percentage, and neck circumference. Body fat percentage, measured using a bioimpedance analyzer, served as the reference standard. Results: The study identified a neck circumference (NC) cut-off of 31.3 cm using receiver operating characteristic (ROC) analysis, showing robust sensitivity (71.23%) and specificity (79.02%) for detecting obesity defined by body fat percentage. Waist circumference (WC) showed the highest sensitivity (73.97%) for diagnosing obesity in females, followed by NC (71.23%). Conclusion: Neck circumference is a practical, cost-effective, and reliable screening tool for obesity, offering advantages over traditional methods. Its noninvasive nature and ease of measurement make it suitable for large-scale screening, contributing to the early detection and management of obesity-related health risks. This study supports the inclusion of NC in routine clinical assessments and public health initiatives.
Association Study on Neck Circumference and Central Obesity in Population Receiving Physical Examinations

Abstract:Objective To investigate the relationship between neck circumference(NC) and central obesity.Methods A cross-sectional investigation was conducted.4 236 subjects receiving physical health examination were divided into four subgroups according to their NC quartiles.The average variations of means of age,height,weight,body mass index(BMI),waist circumference(WC),hip circumferences(HC),and waist-to-hip ratio(WHR) were compared among the subgroups;the relationship of NC with obesity was analyzed between subgroups divided by three obesity standards according to WC,BMI and WHR.And the cut-off value of NC for obesity diagnosis was analyzed with ROC method.Results The NC of male subjects(37.40± 2.46) cm was larger than that of the female(32.46± 2.24) cm(t=47.20,P0.01).With the increase of NC quartiles,the age,height,weight,BMI,WC and HC in male and female also increased(P0.05),and the average variation of WC and weight were the largest.Among subgroups according to three obesity standards,the NC in the obesity subgroup was larger than that in the normal group(P0.01);For the people with central obesity,the cut-off value of NC was 36.8 cm for men and 33.1 cm for women.Conclusion NC is positively related to age,height,weight,BMI,WC and HC;NC is related to central obesity;NC36.8 cm in men and NC33.1cm in women may predict the risk for central obesity.
The Association of Wrist Circumference with Hypertension in Northeastern Chinese Residents in Comparison with Other Anthropometric Obesity Indices

Abstract:Background Wrist circumference (WrC) is an easily obtained measure in estimating the body frame and regional fat distribution, and has increasingly used as an obesity index. The aim of our study is to estimate the association of WrC with elevated blood pressure (BP) among northeastern Chinese community-dwelling residents, and compare the strength of this association to other anthropometric obesity indices. Methods A total of 2,331 adult participants (761 male participants, and 1,570 female participants) were included. WrC and other five generally used obesity indices, including body mass index (BMI), waist circumference (WC), waist-to-hip ratio (WHR), waist-to-height ratio (WHtR) and neck circumference (NC) were measured. Hypertension was defined as systolic blood pressure (SBP)/diastolic blood pressure (DBP) ≥140/90 mmHg or anti-hypertensive medication use. Multivariable linear and logistic regression models were performed to identify associations of BP and hypertension with per standard deviation (SD) increase of obesity indices. Areas under receiver operative characteristic curves (AUC) were calculated to compare the predicting capacity of WrC and other obesity indices on hypertension. Results All of the six obesity indices were positively associated with both SBP and DBP after adjustment for age and gender (P-values of associations of SBP with obesity indices were 0.043 for WrC, and <0.001 for other five indices; P-values of associations of DBP with obesity indices were 0.011 for WrC, 0.031 for WHR, and <0.001 for other four indices), while the association between SBP and WrC showed no statistically significant after further adjusted for life-style and metabolic risk factors (P-value was 0.062). The increases of both SBP and DBP per SD increase of BMI were the largest. The positive associations of five obesity indices but WHR with hypertension were observed after adjustment for all risk factors (P-values were 0.024 for WrC, 0.064 for WHR and <0.001 for other four indices). However, the odd ratios (OR) of WrC was the smallest, while BMI was the largest. Consistently, the AUC of BMI was the largest and statistically larger than that observed for WrC (P-value <0.001). Conclusions WrC was associated with hypertension among northeastern Chinese populations. However, the association was not stronger than the other generally used indices, particularly BMI.

Abstract:Measurement of waist circumference has substantial variability and some limitations, while neck circumference is a simple and reliable anthropometric measure. This study aimed to assess the association between neck circumference and waist circumference and to identify the best cutoff of neck circumference that could predict central obesity in prediabetic patients. This cross-sectional study included adult patients with prediabetes, defined as having fasting plasma glucose levels ranging from 100 to 125 mg/dL or HbA1c ranging from 5.7 to 6.49%, who visited the outpatient clinic of Family Medicine Department, Ramathibodi Hospital, Thailand, during October 2014 and March 2016. Neck circumference was measured from the level just below the laryngeal prominence perpendicular to the long axis of the neck. Central obesity was defined as having waist circumference measurements greater than 90 and 80 cm for males and females, respectively. The correlation between neck circumference and waist circumference was explored by applying pairwise correlation coefficient. Receiver operating characteristic (ROC) curve analysis was performed and Youden index equal to “sensitivity – (1-specificity)” was calculated. Neck circumference that yielded the maximum Youden index was determined as the optimal cutoff point for prediction of central obesity. There were 1,534 patients eligible for this study. After adjusting for covariables, neck circumference was found to be significantly associated with waist circumference in both females and males, with β -coefficients of 1.01 (95% CI: 0.83, 1.20) and 0.65 (95% CI: 0.46, 0.85), respectively. After applying the ROC analysis, neck circumferences ≥ 32 cm in females and ≥ 38 cm in males were determined as the best cutoff values to predict central obesity. Neck circumference is strongly correlated with waist circumference in prediabetics and should be considered as an alternative to the waist circumference measurement in screening for central obesity.

Abstract:Background: The leading cause of death in older people is cardiovascular diseases. Several studies have found that neck circumference (NC) is a simple anthropometric marker associated with adiposity. The aim of this study is to estimate and validate NC cut-off points as adiposity markers and analyze their association with cardiovascular and chronic metabolic diseases in older people. Methods: A cross-sectional study in 358 non-disabled, community-dwelling older people (71.7 ± 3.9 years) living in Santiago de Chile and participating in the HTSMayor study was conducted. Measurements of body composition and cardiovascular risks were evaluated. Receiver operating characteristic (ROC) curves and multiple logistic regression models were used to evaluate the association of NC with cardiovascular and chronic metabolic diseases. NC cut-off points were obtained to predict obesity, abdominal obesity, and adiposity. Results: The best performance values of neck circumference relative to obesity and adiposity were obtained with respect to abdominal obesity (40.6 cm in men and 34.2 cm in women). Higher NC values were associated with a higher area under the curve (AUC) for men and women (men: AUC = 0.84; women: AUC = 0.86). NC was significantly associated with a higher risk for diabetes mellitus (OR = 1.95), hypertension (OR = 2.42), acute myocardial infarction (OR = 4.36), and comorbidities (OR = 2.01), and a lower risk for sarcopenia (OR = 0.35). Conclusions: This study shows that NC is a useful tool for detecting abdominal obesity, obesity, and adiposity in older people and that a higher NC increases the risk of chronic diseases.

Abstract:Objective Central obesity is considered to be a central component of metabolic syndrome. Waist circumference (WC) has been widely used as a simple indicator of central obesity. This study is aimed to evaluate the sensitivity of WC cut-off values for predicting metabolic risk factors in middle-aged Chinese.Methods The study involved 923 subjects aged 40-65 years. The metabolic risk factors were defined according to the Chinese Joint Committee for Developing Chinese Guidelines on Prevention and Treatment of Dyslipidemia in Adults. WC cut-off 85-90 cm and >= 90 cm were used as cut-off values of central pre-obesity and central obesity in males, respectively, while WC 80-85 cm and >= 85 cm were used as cut-off values of central pre-obesity and central obesity in females.Results First, WC values corresponding to body mass index (BMI) 24 kg/m(2) and visceral fat area (VFA) 80 cm(2) were 88.55 cm and 88.51 cm in males, and 81.46 cm and 82.51 cm in females respectively. Second, receiver operating characteristic curves showed that the optimal WC cut-off of value was 88.75 cm in males, higher than that in females (81.75 cm). Third, the subjects with higher WC values were more likely to have accumulating metabolic risk factors. The prevalence of metabolic risk factors increased linearly and significantly in relation to WC levels.Conclusion WC cut-off values of central pre-/central obesity are optimal to predict multiple metabolic risk factors.

Abstract:ObjectiveWe aim to explore the relationship of neck circumference to metabolic syndrome (MetS) and obesity in Chinese elders and to establish cut-off points of the neck circumference for MetS and obesity. MethodsCross-sectional analysis for a population sample of 2092 individuals aged over 65years, representative of East China elders. MetS was identified according to the 2004 Chinese Diabetes Society criteria. ResultsNeck circumference was correlated with body mass index (r=0.70, p=0.000 in men, and r=0.73, p=0.000 in women) and waist circumference (waist circumference; r=0.70, p=0.000 in men, and r=0.72, p=0.000 in women). According to neck circumference quartile, the prevalence of MetS and its components were increased significantly from Q1 to Q4 (p for trend <0.001) in both genders. Binary logistic regression analysis revealed that both waist circumference and neck circumference were independent predictors of Mets, with odds ratios (95% confidence interval) 1.11 (1.08-1.15; p=0.044) and 1.17 (1.07-1.28; p=0.000), respectively, in men and 1.08 (1.05-1.10; p=0.000) and 1.31 (1.21-1.42; p=0.000), respectively, in women. As for obesity, the significant independent indicators in both genders were also waist circumference and neck circumference (all p<0.01). In the receiver operating characteristic curves, both the optimal cut-off points of neck circumference for MetS and obesity were 38cm in men and 35cm in women. ConclusionsThe neck circumference, as waist circumference, is also a valuable tool for identifying MetS and obesity, with established cut-off points for the prediction of MetS and obesity in Chinese elders. Exploring the Role of Adult Wrist Circumference as a Potential Indicator of Obesity

Abstract:OBJECTIVE: To explore the correlation of wrist circumference (WrC) with various obesity indices and to determine gender-specific optimal cut-off points of WrC for obesity.STUDY DESIGN: Cross-sectional study. Place and Duration of the Study: Dow University of Health Sciences, Karachi, Pakistan, from December 2021 to December 2022.METHODOLOGY: Two hundred and eighty residents of Karachi, aged 30-60 years, participated in this research. Established methods were used to measure body mass index (BMI), waist circumference, hip circumference, neck circumference, WrC, visceral fat level, waist-hip ratio, abdominal volume index, and body roundness index. Correlation of WrC with all obesity markers was done and scatterplots were obtained. Receiver Operating Characteristic curves were plotted to find the gender-specific cut-off value of WrC.RESULTS: Of the total 280 subjects, 191 (68%) were females and 89 (32%) were males. The overall mean age was 42.04 ± 9.12 years (range: 28-65 years). The mean WrC in males and females were 16.32 ± 1.01 cm and 14.82 ± 1.03 cm, respectively. The WrC correlated best with neck circumference in males (r = 0.758; p<0.001) and BMI in females (r=0.615; p<0.001). The scatterplots obtained showed positive linear relationships between WrC and anthropometric variables. The cut-off values of WrC for adult males and females when compared to waist circumference were 15.45 cm and 13.95 cm, respectively.CONCLUSION: Measurement of the WrC can be a helpful indicator to identify excess weight in the general population. Its ease of measurement makes it applicable in both routine clinical practice and extensive epidemiological research.KEY WORDS: Wrist circumference, BMI, Waist circumference, Obesity.

Abstract:Abstract Obesity increases the risk of developing cardiovascular disease and other metabolic diseases. We intended to compare three different anthropometric indicators of obesity, in predicting the incidence of cardiovascular events in Chinese type 2 diabetes. Beijing Community Diabetes Study was a prospective multi-center study conducted in Beijing community health centers. Type 2 diabetes patients from fourteen community health centers were enrolled at baseline. The primary endpoint was cardiovascular events. The upper quartile of neck circumference (NC) was set as greater NC. A total of 3299 diabetes patients were enrolled. In which, 941 (28.52%) had cardiovascular disease at baseline. Logistic analysis showed that central obesity (waist circumference (WC) above 90 cm in men and 85 cm in women) and greater NC were all related to baseline cardiovascular disease (adjusted OR = 1.49, and 1.55). After 10-year follow-up, 340 (10.31%) had cardiovascular events. Compared with patients without cardiovascular events, those having cardiovascular events had higher BMI, larger WC and NC. Cox regression analysis showed that greater WC and NC were all associated with the occurrence of cardiovascular events (adjusted HR = 1.41, and 1.38). A higher NC and WC might increase the risk of cardiovascular events by about 40% in type 2 diabetes patients in Beijing communities.

Abstract:Background: Waist circumference (WC) is strongly linked to obesity-associated risks. However, currently proposed WC risk thresholds are not based on associations with obesity-related risk factors but rather with body mass index (BMI; in kg/m(2)). Objective: The objective was to determine the relations of WC to obesity-associated risk factors in a representative sample of US whites and to derive comparable risk thresholds for WC and BMI. Design: Data on 9019 white participants of the third National Health and Nutrition Examination Survey were divided into 2 groups according to the presence of greater than or equal to 1 of 4 obesity-associated risk factors: low HDL cholesterol, high LDL cholesterol, high blood pressure, and high glucose. Odds ratio (OR) equations were derived from logistic regression models for WC and BMI with the use of the 25th percentile in the study population as the reference, Receiver operating characteristic curves for identifying risk factors were computed for WC and BMI. Results: At BMIs of 25 and 30, ORs were 1.19 (95% CI: 1.06, 1.35) and 2.37 (95% CI: 1.33, 4.22) for men and 1.56 (95% CI: 1.29, 1.91) and 3.16 (95% CI: 1.94, 5.28) for women, respectively. The corresponding ORs for WC were at 90 and 100 cm for men and at 83 and 93 cm for women, Minima on the receiver operating characteristic curves for men were at 96 cm for WC and at 26 for BMI and for women were at 86 cm for WC and 25 for BMI. Conclusion: WC is more closely linked to cardiovascular disease risk factors than is BMI.
Central markers of obesity affect heart rate variability independent of physical activity in young adults

Abstract:Introduction: Obesity is considered to be a risk factor for a variety of cardiovascular conditions. Various markers for obesity are used to evaluate effect of obesity on cardiovascular autonomic activity. In light of conflicting reports on effect of obesity on heart rate variability (HRV), use of obesity indices, and the effect of physical activity on HRV, we evaluated autonomic activity in young Indian obese adults using revised Indian and World Health Organization (WHO) body mass index (BMI) guidelines for obesity, waist circumference (WC), and waist-hip ratio (WHR) taking into consideration the level of physical activity.Methods: The study was conducted on 91 young healthy adults. Height, weight, waist, and hip circumference were recorded to determine BMI and WHR. Five-minute electrocardiogram (ECG) was recorded for assessment of HRV. Physical activity was assessed by the WHO Global Physical Activity Questionnaire (GPAQ).Results: Waist circumference showed a negative correlation with the time domain parameters of HRV and high frequency normalized units (HFnu) while a positive correlation with low frequency normalized units (LFnu). In multiple linear regression analysis, time domain indices, HFnu and total power decreased while LFnu increased with an increase in WC. The result was supported by the similar effect of waist-hip ratio categories on HRV in analysis of covariance (ANCOVA). Physical activity had no effect on HRV.Conclusion: Central obesity parameters are better predictors of effect of obesity on HRV independent of the effect of physical activity.

Is Neck Circumference Related to Other Anthropometric Measurements and Biochemical Parameters in Type 2 Diabetes?

Abstract:Background Obesity is one of the leading risk factors for developing type 2 diabetes mellitus (T2DM). Body mass index (BMI), waist circumference, and waist/hip ratio are anthropometric measurements used to diagnose obesity. In recent years, neck circumference, one of the anthropometric indicators used in assessing obesity, has come to the fore. This study investigates the relationship between neck circumference and other anthropometric measurements and specific biochemical parameters in T2DM. Methods Four hundred sixty-four individuals with type 2 diabetes were included in the study. Subjects' body weight, height, and other anthropometric measurements like circumferences of the waist, hip, and neck were measured. BMI, waist/hip, and waist/height ratio were calculated. The biochemical tests of the subjects in the previous month from the study were accessed from the hospital information system. The relationship between anthropometric measurements and biochemical parameters with neck circumference was evaluated. Results The mean age of the subjects was 54.6±8.51 years. 56.2% were female and 43.8% were male, and the time from T2DM diagnosis was 9.9±7.49 years. Most male subjects were overweight (49.8%), and approximately one-third of the women were first-degree obese (33.0%). Body weight, BMI, waist, hip, and neck circumferences, and waist/hip and waist/height ratios were significantly higher in both male and female subjects in the neck circumference high-risk group than those in the low-risk group (p<0.05). In terms of biochemical parameters, fasting blood glucose and HbA1c values of men with neck circumference at risk were significantly higher than those in the low-risk group. Neck circumference measurements of the participants were positively correlated with body weight (r=0.543; p<0.001), height (r=0.260; p<0.001), waist circumference (r=0.562; p<0.001), hip circumference (r=0.293; p<0.001), BMI (r=0.366; p<0.001), waist/hip ratio (r=0.428, p<0.001), and waist/height ratio (r=0.393, p<0.001). Neck circumference had a low positive correlation with fasting blood glucose (r=0.165; p<0.001), HbA1c (r=0.281; p<0.001), and triglyceride (r=0.231; p<0.001) and a negative relationship with low-density lipoprotein cholesterol (r=-0.118; p=0.001). When the relationship between neck circumference and biochemical parameters was examined, it was seen that this correlation was only in men. Conclusion Neck circumference measurement is a simple and reliable method and is not affected by external factors. It correlates with other anthropometric measurements and can be used as a good indicator of the distribution of upper subcutaneous adipose tissue in T2DM. However, more studies with larger samples are needed on this subject.
Association between neck circumference and pulmonary function in Korean adults

Abstract:Neck circumference (NC) is associated with various diseases. However, its association with pulmonary diseases remains unclear. This study aimed to investigate the association between NC and pulmonary function in Korean adults. Data from the 2019 Korean National Health and Nutrition Examination Survey were used, including forced expiratory volume in 1 second (FEV 1 ) and forced vital capacity (FVC). A total of 3818 adults aged 40 to 79 years were included in the analysis. Multivariable logistic regression models were used to examine factors associated with low pulmonary function (<80% of the predicted value) and calculate odds ratios (ORs) and 95% confidence intervals (CIs) for decreased lung function per 1-cm increase in NC. A negative correlation was observed between the NC and pulmonary function in both sexes. In multivariate logistic regression analyses, in contrast to men, women exhibited a notable relationship between increased NC and impaired pulmonary function, with a more pronounced effect observed in FVC (OR, 1.14; 95% CI, 1.05–1.24). Central obesity may play a role in pulmonary function decline. Understanding this relationship can inform preventive strategies and interventions to improve the respiratory health of individuals with central obesity.

Neck Circumference is Associated with Hyperuricemia: a Cross-Sectional Study.

Abstract:Neck circumference (NC) is associated with metabolic abnormalities, independent of other obesity indices. However, data are limited regarding the potential relation between NC and serum uric acid (UA) concentrations. Therefore, we evaluated the cross-sectional association between NC and UA concentration, and odds of having hyperuricemia in a community-based cohort. The current study included 87,782 participants (16,317 women and 71,465 men, 52.2 ± 14.1 y) of the Kailuan Study. NC and UA concentration were measured in 2014. We used generalized linear model to investigate the association between NC and serum UA concentration and logistic regression model to investigate the association between NC and likelihood of having hyperuricemia (≥ 7 mg/dl in men and ≥ 6 mg/dl in women), adjusting for demographic factor, anthropometric indices, plasma lipid profiles, blood glucose, blood pressure, physical exercise, snoring, smoking, diet quality, and alcohol consumption. Higher NC was associated with higher serum UA concentration, and higher odds of hyperuricemia in both men and women after adjusting for potential confounders (both p < 0.001). Each additional 5-cm increase in NC was associated with 6% higher likelihood of having hyperuricemia (adjusted OR = 1.06; 95% CI 1.02, 1.1) in men and 17% in women (adjusted OR = 1.17; 95% CI 1.06, 1.28) (p interaction = 0.01). Similar pattern was observed after excluding participants who reported use of anti-hypertensive drugs, participants with obesity or higher waist circumference, and participants with history of gout and chronic kidney diseases. Neck circumference as a predictor of all-cause mortality in middle-aged and older adults in rural Ecuador

Abstract:Abstract Background Neck circumference (NC) has been associated with mortality secondary to cardiovascular diseases and other conditions. However, information on this association in the population at large is limited. We aimed to assess this association in community dwellers living in rural Ecuador. Methods Individuals aged ≥40 y who were enrolled in the population-based Three Villages Study cohort were prospectively followed to estimate mortality risk according to baseline measurements of NC, after adjusting for relevant confounders. Results Analysis included 1521 individuals followed for a mean of 6.4±3.4 y. Mean NC was 36.2±3.7 cm, with 509 (33%) individuals allocated to the first (25–34 cm), 319 (21%) to the second (36–37 cm), 417 (27%) to the third (37–39 cm) and 276 (18%) to the fourth (40–50 cm) quartile. A total of 211 (14%) individuals died during the follow-up. Overall, the crude mortality rate was 2.3 per 100 person-years, which increased to 5.63 for those in the fourth NC quartile. An adjusted Cox-proportional hazards model showed that individuals in the fourth quartile of NC had higher mortality risk compared with the first quartile (HR: 2.98; 95% CI 1.77 to 5.02). Conclusion Larger NC increases mortality risk in middle-aged and older adults of indigenous ancestry living in rural Ecuador.

Abstract:Aims: To analyze the correlation between neck circumference (NC) and non- alcoholic fatty liver disease (NAFLD) and compare the predictive value of NC for NAFLD with that of other simple anthropometric measures and other biochemical profiles.Methods: 2761 subjects, undergoing a medical check-up at the Changhai Hospital between October 01, 2012 and November 30, 2012, were recruited to the study. NC, other simple anthropometric measures, and biochemical profiles were analyzed.Results: NC in NAFLD subjects with or without elevated ALT were 38.94 +/- 2.62 cm and 37.21 +/- 3.06 cm respectively, which was significantly higher than that in subjects with other metabolic disorders (NC: 35.33 +/- 3.03 cm) and in normal controls (NC: 32.60 +/- 2.37) (both P < 0.001). NC in women with NAFLD increased by 1 cm and fasting insulin (FINS) and homeostasis model assessment-insulin resistance (HOMA-IR) increased by 1.87 mIU/L and 1.43, respectively. Compared with other anthropometric measures, neck circumference-height ratio (NHtR) had a significant impact both on the incidence of NAFLD. After adjustment for sex, abdominal obesity and other influencing factors, the incidence of NAFLD still tended to positively correlate with NC. Optimal cut-off points of NC and NHtR for predicting NAFLD in males were 37.25 cm and 0.224, respectively, and such points in females were 32.90 cm and 0.208, respectively.Conclusion: NC was wider in NAFLD patients than in healthy subjects and other metabolic disorder sufferers. Combination Of Bmi And Waist Circumference For Identifying Cardiovascular Risk Factors In Whites

Abstract:Objective: BMI (kilograms per meters squared) and waist circumference (WC) (measured in centimeters) are each associated with the risk of developing cardiovascular disease (CVD). Therefore, a combination of the two may be more effective in identifying subjects at risk than either alone. The present study sought to identify the combination of BMI and WC that has the strongest association with CVD risk factors in whites.Research Methods and Procedures: Subjects were 8712 white men and women from the Third National Health and Nutrition Examination Survey. The optimal combination of BMI and WC was developed using logistic regression models with BMI and WC as predictors and CVD risk factors as outcomes. The combined measure of BMI and WC using current cut-off points was also examined. Sensitivity, specificity, and receiver operating characteristics curves were compared between the combined measures and BMI alone.Results: For white men, the optimal combination of BMI and WC for identifying CVD risk factors was 0.68 x BMI + 0.32 x WC. This combination generated a score that better estimated the odds of having CVD risk factors than either alone. For white women, WC alone largely determined the likelihood of having CVD risks. The combination of BMI and WC using current cut-off points may provide an improved measure of CVD risk. Combined measures showed a higher sensitivity or a shorter distance in receiver operating characteristic curves in the identification of CVD risk factors.Discussion: Combined measures of BMI and WC may provide a higher overall test performance for CVD risk factors and may be useful in some ethnic groups as an improved means of screening Subjects for further evaluation in the clinical setting.
Neck Circumference and Incidence of Diabetes Mellitus over 10 Years in the Korean Genome and Epidemiology Study (KoGES)

Abstract:Neck circumference, a proxy for upper-body fat, may be a unique fat depot that indicates metabolic risk beyond whole body fat. We investigated whether neck circumference is associated with development of diabetes mellitus (DM) in a subset of data with Korean Genome and Epidemiology Study (n = 3521, age range = 42-71 years). Nondiabetic subjects at the baseline were categorized into 4 groups (Q1-Q4) according to their neck circumference. Parameters related with β-cell function and insulin resistance including Epworth sleepiness scale and snoring habit were examined. The development of DM was confirmed biannually based on a 75-g oral glucose tolerance test. Over the 10 years, 2623 (74.5%) among 3521 subjects were followed-up. Among them, 632 (24.1%) developed DM. The incidence of DM increased from 17.6% in Q1 to 18.2% in Q2, to 25.4% in Q3, and to 36.0% in Q4 (P < 0.001). After adjusting for most risk factors related with DM, the relative risks of DM development were 0.989 (95% confidence interval, 0.638-1.578), 1.660 (1.025-2.687), and 1.746 (1.037-2.942) in men and 0.939 (0.540-1.769), 1.518 (0.808-2.853), and 2.077 (1.068-4.038) in women in Q2, Q3, and Q4, respectively when compared to Q1. This finding indicates negative impact from large neck circumference in the development of DM.

Abstract:Given the importance and continued interest in finding a simple, accessible, and universal measure which reflects both general and abdominal adiposity, this study tested for an association of the ratio of WC decile to BMI decile (WC-d/BMI-d) with all-cause mortality. Individuals aged 18–79 years who had participated in the National Health and Nutrition Examination Survey (NHANES) during the years 2007 to 2018 were included in the analysis. WC and BMI deciles were defined separately for males and females, while WC-d/BMI-d was calculated for each individual. The association of WC-d/BMI-d with mortality was assessed using logistic models for the total study population, and then again after stratification by sex, ethnicity, morbidity level, and BMI categories. Positive associations between WC-d/BMI-d and mortality were demonstrated for the total study population (adjusted OR = 1.545, 95%CI: 1.369–1.722) and within different sub-groups, including the population with a normal BMI level (adjusted OR = 1.32, 95%CI: 1.13–1.50). WC-d/BMI-d increased with age, with ~40 years representing a critical time point when WC-d surpasses BMI-d, with a sharper incline for males as compared to females. WC-d/BMI-d was significantly associated with all-cause mortality amongst NHANES American adults; thus, measurements of WC and its integration with BMI in this metric should be considered in clinical practice.
