Abstract:Myasthenia Gravis (MG) is an autoimmune neuromuscular disorder that heavily affects various daily actions of the suffers. The manifestations of MG disease are fluctuating muscle weakness and fatigue across different muscle units of human body, while ocular muscles involvement is regarded as primary symptom in a majority of the patients. Currently, the conventional clinical approach for MG assessment involves identifying and measuring key ocular structures manually, which is resource-intensive and prone to subjective errors. In our paper, a novel application of computer vision technique is introduced to assist in the quantitative evaluation of extraocular muscles in MG patients. By employing advanced segmentation algorithms, both prior feature-driven and deep feature-driven methods, we aims to automatically determine the ocular structures such as the iris, sclera, and pupil, etc. This pilot study serves as a proof of concept for using classical segmentation models to facilitate automatic and non-invasive MG assessments, enhancing clinical management and patient autonomy in monitoring their condition. Our approach not only aids physicians in obtaining more accurate measurements for better disease management but also reduces the necessity for frequent clinical visits, potentially improving adherence to treatment and overall quality of life for MG patients.
Trigger point--acupuncture point correlations revisited.

Abstract:UNLABELLED In 1977, Melzack and colleagues examined the possible correspondence of acupuncture points and trigger points for the treatment of pain. They claimed a 71% correspondence between these two classes of points. Their findings have influenced many researchers and practitioners but have not been examined since 1977. The current study explores the claim of a 71% correspondence between these two classes of points through a more extensive examination of the acupuncture literature. OBJECTIVES To investigate the claim of a 71% correspondence of acupuncture points and trigger points for the treatment of pain. METHODS The study involved a review of acupuncture texts published since 1977, focusing on five textbooks for the in-depth analyses and a broader range of texts for the more general analyses. RESULTS If trigger points correspond to any class of acupuncture points it would have to be to the a shi points rather than the "channel" or "extra" points with which the 1977 study attempted to find correlation. Approximately 35% of recommended acupuncture points in the treatment of pain are distant from the site of the pain, making assumptions about the infrequency of use of distant acupuncture points for pain suspect. Sixty-one percent (61%) of the acupuncture points that the 1977 study examined for the treatment of pain are not recommended at all for the treatment of pain, and 44% are not recommended in the treatment of any problem, while only 19% of the acupuncture points are frequently recommended for pain and 20% for all conditions. For the acupuncture points that corresponded in the 1977 study, the equivalent numbers are: 60% not recommended at all for pain, 47% not recommended for anything, 18% commonly recommended for pain, and 16% commonly recommended for anything. CONCLUSION The claimed 71% correspondence of trigger points to acupuncture points is conceptually not possible. Furthermore, even putting this conceptual problem aside, no more than 40% of the acupuncture points that the 1977 study examined could correlate for the treatment of pain, and more likely, only approximately 18%-19% correlate rather than the 71% that was claimed. However, this study found a probable correspondence of trigger points to a different class of acupuncture points, the a shi points, which appears to be an important finding. Researchers and clinicians who have assumed the conclusions of the 1977 study to be correct will need to reexamine the impact of the current findings on any claims that are dependent on the conclusions of that study.

Abstract:BackgroundAdvances in video image analysis and artificial intelligence provide opportunities to transform how patients are evaluated. In this study, we assessed the ability to quantify Zoom video recordings of a standardized neurological examination— the Myasthenia Gravis Core Examination (MG-CE)—designed for telemedicine evaluations.MethodsWe used Zoom (Zoom Video Communications) videos of patients with myasthenia gravis (MG) who underwent the MG-CE. Computer vision, in combination with artificial intelligence methods, was used to develop algorithms to analyze the videos, with a focus on eye and body motions. To assess the examinations involving vocalization, signal processing methods, such as natural language processing (NLP), were developed. A series of algorithms were developed to automatically compute the metrics of the MG-CE.ResultsA total of 51 patients with MG were assessed, with videos recorded twice on separate days, while 15 control subjects were evaluated once. We successfully quantified the positions of the lids, eyes, and arms and developed respiratory metrics based on breath counts. The cheek puff exercise was found to have limited value for quantification. Technical limitations included variations in illumination, bandwidth, and the fact that the recording was conducted from the examiner's side rather than the patient's side.ConclusionSeveral aspects of the MG-CE can be quantified to produce continuous measurements using standard Zoom video recordings. Further development of the technology will enable trained non-physician healthcare providers to conduct precise examinations of patients with MG outside of conventional clinical settings, including for the purpose of clinical trials.

Abstract:Meibomian glands dysfunction (MGD) is the main cause of dry eyes. Biological parameters of meibomian gland (MG) such as height, tortuosity and the degree of atrophy are closely related to its function. However, Thus, an effective quantitative diagnostic tool is needed for clinical diagnosis. Automatic quantification of MGs' morphological features could be a challenging task and play an important role in MGD diagnosis and classification. Our main objective is to develop an artificial intelligence (AI) system for evaluating MGs' morphology and explore the relationship between the morphological parameters and functions. We proposed a novel MGs extraction method based on convolutional neural network (CNN) with enhanced mini U-Net. A prospective study was conducted, 120 subjects were included and taken meibography. The training and validation sets encompassed 60 subjects; and the test set consisted of other 60 subjects with comprehensive examinations for ocular surface disease index questionnaire (OSDI), tear meniscus height (TMH), tear break-up time (TBUT), corneal fluorescein staining (CFS), lid margin score, and meibum expressibility score. The algorithm effectively extracted MGs from meibography even with this small training sample. As a result, while the intersection over union (IoU) achieved 0.9077, the repeatability was 100%. The processing time for each image was 100ms. Using this method, the investigators identified a significant and linear correlation between MG morphology and clinical parameters. This study provided a new method for quantification of MGs' morphological features obtained by meibography, which has advantages in reducing analysis time, improving diagnostic efficiency, and assisting ophthalmologists with limited clinical expertise.

Abstract:Importance: Machine learning (ML) approaches to facial landmark localization carry great clinical potential for quantitative assessment of facial function as they enable high-throughput automated quantification of relevant facial metrics from photographs. However, translation from research settings to clinical applications requires important improvements. Objective: To develop an ML algorithm for accurate facial landmarks localization in photographs of facial palsy patients, and use it as part of an automated computer-aided diagnosis system. Design, Setting, and Participants: Facial landmarks were manually localized in portrait photographs of eight expressions obtained from 200 facial palsy patients and 10 controls. A novel ML model for automated facial landmark localization was trained using this disease-specific database. Model output was compared to manual annotations and the output of a model trained using a larger database consisting only of healthy subjects. Model accuracy was evaluated by the normalized root mean square error (NRMSE) between algorithms' prediction and manual annotations. Results: Publicly available algorithms provide poor results when applied to patients compared to healthy controls (NRMSE, 8.56 +/- 2.16 vs. 7.09 +/- 2.34, p << 0.01). We found significant improvement in facial landmark localization accuracy for the clinical population when using a model trained with a relatively small number patients' photographs (1440) compared to a model trained using several thousand more images of healthy faces (NRMSE, 6.03 +/- 2.43 vs. 8.56 +/- 2.16, p << 0.01). Conclusions: Retraining a landmark detection model with a small number of clinical images significantly improved landmark detection performance in frontal view photographs of the clinical population. These results represent the first steps towards an automatic system for computer-aided assessment in facial palsy.

Abstract:Automated evaluation of facial palsy using machine learning offers a promising solution to the limitations of current assessment methods, which can be time-consuming, labor-intensive, and subject to clinician bias. Deep learning-driven systems have the potential to rapidly triage patients with varying levels of palsy severity and accurately track recovery over time. However, developing a clinically usable tool faces several challenges, such as data quality, inherent biases in machine learning algorithms, and explainability of decision-making processes. The development of the eFACE scale and its associated software has improved clinician scoring of facial palsy. Additionally, Emotrics is a semiautomated tool that provides quantitative data of facial landmarks on patient photographs. The ideal artificial intelligence (AI)-enabled system would analyze patient videos in real time, extracting anatomic landmark data to quantify symmetry and movement, and estimate clinical eFACE scores. This would not replace clinician eFACE scoring but would offer a rapid automated estimate of both anatomic data, similar to Emotrics, and clinical severity, similar to the eFACE. This review explores the current state of facial palsy assessment, recent advancements in AI, and the opportunities and challenges in developing an AI-driven solution. Automated Measurement of Ocular Movements Using Deep Learning-Based Image Analysis

Abstract:Purpose Clinical assessment of ocular movements is essential for the diagnosis and management of ocular motility disorders. This study aimed to propose a deep learning-based image analysis to automatically measure ocular movements based on photographs and to investigate the relationship between ocular movements and age. Methods 207 healthy volunteers (414 eyes) aged 5-60 years were enrolled in this study. Photographs were taken in the cardinal gaze positions. Ocular movements were manually measured based on a modified limbus test using ImageJ and automatically measured by our deep learning-based image analysis. Correlation analyses and Bland-Altman analyses were conducted to assess the agreement between manual and automated measurements. The relationship between ocular movements and age were analyzed using generalized estimating equations. Results The intraclass correlation coefficients between manual and automated measurements of six extraocular muscles ranged from 0.802 to 0.848 (P < 0.001), and the bias ranged from -0.63 mm to 0.71 mm. The average measurements were 8.62 +/- 1.07 mm for superior rectus, 7.77 +/- 1.24 mm for inferior oblique, 6.99 +/- 1.23 mm for lateral rectus, 6.71 +/- 1.22 mm for medial rectus, 6.81 +/- 1.20 mm for inferior rectus, and 6.63 +/- 1.37 mm for superior oblique, respectively. Ocular movements in each cardinal gaze position were negatively related to age (P < 0.05). Conclusions The automated measurements of ocular movements using a deep learning-based approach were in excellent agreement with the manual measurements. This new approach allows objective assessment of ocular movements and shows great potential in the diagnosis and management of ocular motility disorders.

Abstract:Background The number of patients with neurological diseases is currently increasing annually, which presents tremendous challenges for both patients and doctors. With the advent of advanced information technology, digital medical care is gradually changing the medical ecology. Numerous people are exploring new ways to receive a consultation, track their diseases, and receive rehabilitation training in more convenient and efficient ways. In this paper, we explore the use of facial expression recognition via artificial intelligence to diagnose a typical neurological system disease, Parkinson disease (PD). Objective This study proposes methods to diagnose PD through facial expression recognition. Methods We collected videos of facial expressions of people with PD and matched controls. We used relative coordinates and positional jitter to extract facial expression features (facial expression amplitude and shaking of small facial muscle groups) from the key points returned by Face++. Algorithms from traditional machine learning and advanced deep learning were utilized to diagnose PD. Results The experimental results showed our models can achieve outstanding facial expression recognition ability for PD diagnosis. Applying a long short-term model neural network to the positions of the key features, precision and F1 values of 86% and 75%, respectively, can be reached. Further, utilizing a support vector machine algorithm for the facial expression amplitude features and shaking of the small facial muscle groups, an F1 value of 99% can be achieved. Conclusions This study contributes to the digital diagnosis of PD based on facial expression recognition. The disease diagnosis model was validated through our experiment. The results can help doctors understand the real-time dynamics of the disease and even conduct remote diagnosis.

Abstract:Differences in expressing facial emotions are broadly observed in people with cognitive impairment. However, these differences have been difficult to objectively quantify and systematically evaluate among people with cognitive impairment across disease etiologies and severity. Therefore, a computer vision-based deep learning model for facial emotion recognition trained on 400.000 faces was utilized to analyze facial emotions expressed during a passive viewing memory test. In addition, this study was conducted on a large number of individuals (n = 493), including healthy controls and individuals with cognitive impairment due to diverse underlying etiologies and across different disease stages. Diagnoses included subjective cognitive impairment, Mild Cognitive Impairment (MCI) due to AD, MCI due to other etiologies, dementia due to Alzheimer’s diseases (AD), and dementia due to other etiologies (e.g., Vascular Dementia, Frontotemporal Dementia, Lewy Body Dementia, etc.). The Montreal Cognitive Assessment (MoCA) was used to evaluate cognitive performance across all participants. A participant with a score of less than or equal to 24 was considered cognitively impaired (CI). Compared to cognitively unimpaired (CU) participants, CI participants expressed significantly less positive emotions, more negative emotions, and higher facial expressiveness during the test. In addition, classification analysis revealed that facial emotions expressed during the test allowed effective differentiation of CI from CU participants, largely independent of sex, race, age, education level, mood, and eye movements (derived from an eye-tracking-based digital biomarker for cognitive impairment). No screening methods reliably differentiated the underlying etiology of the cognitive impairment. The findings provide quantitative and comprehensive evidence that the expression of facial emotions is significantly different in people with cognitive impairment, and suggests this may be a useful tool for passive screening of cognitive impairment.

Abstract:With the continuous progress of technology, the subject of life science plays an increasingly important role, among which the application of artificial intelligence in the medical field has attracted more and more attention. Bell facial palsy, a neurological ailment characterized by facial muscle weakness or paralysis, exerts a profound impact on patients' facial expressions and masticatory abilities, thereby inflicting considerable distress upon their overall quality of life and mental well-being. In this study, we designed a facial attribute recognition model specifically for individuals with Bell's facial palsy. The model utilizes an enhanced SSD network and scientific computing to perform a graded assessment of the patients' condition. By replacing the VGG network with a more efficient backbone, we improved the model's accuracy and significantly reduced its computational burden. The results show that the improved SSD network has an average precision of 87.9% in the classification of light, middle and severe facial palsy, and effectively performs the classification of patients with facial palsy, where scientific calculations also increase the precision of the classification. This is also one of the most significant contributions of this article, which provides intelligent means and objective data for future research on intelligent diagnosis and treatment as well as progressive rehabilitation.

Abstract:Introduction: Facial droop (FD) is a common symptom (45-60%) of stroke so early detection is critical for timely treatment. FD is a part of almost all stroke scales. Our study is to develop a machine learning (ML)/AI based tool that uses an automated detection of FD as one of the components. Our approach uses a normalized value of minor facial expressions called a blendshape. Methods: Google’s Mediapipe was used to create 52 different blendshape values (e.g., left eyebrow raised, right mouth lowered, etc.) from an input image, which were then used as inputs for a simple deep neural network (DNN). We trained this DNN using the blendshapes from 872 images of people with symptoms of facial palsy (FP) and 872 images of people without symptoms exhibiting multiple emotions (neutral, smiling, frowning, etc.). Results: The proposed approach was evaluated on a dataset of facial images of patients with FD and strokes. Our model showed promising results with over 91% test accuracy. This result suggests that the proposed approach is a promising new method for the early detection of FP with high accuracy. The approach is integrated in our ML/AI based tool to replicate different stroke scales. Conclusions: Our model shows that the use of Blendshapes as features in a ML classifier can produce great results in the detection of FP. For greater accuracy, a better dataset with higher quality images as well as unilateral facial expressions (winking, smirks, etc.) should be used. In a future study, a ML model that uses Blendshapes as inputs could be used to classify FP on a scale.

Abstract:We have previously described regulation of focal adhesion kinase (FAK) by its amino-terminal FERM-like domain through an autoinhibitory interaction with its kinase domain (Cooper, L. A., Shen, T. L., and Guan, J. L. (2003) Mol. Cell. Biol. 23, 8030–8041). Here we show that the first two subdomains of the FERM-like domain are independently capable of inhibiting phosphorylation of FAK in trans. We characterized several point mutations within the first subdomain of the FERM-like domain and find that mutation of Lys-38 to alanine results in a FAK mutant that is strongly hyperphosphorylated when expressed in mammalian cells, and promotes increased phosphorylation of the FAK substrate paxillin. A second mutation of Lys-78 to alanine results in a FAK mutant that is underphosphorylated, but can be activated by extracellular matrix stimuli. Like deletion of the amino terminus itself the K38A mutation is phosphorylated in suspension. The Δ375 truncation mutant of FAK is strongly phosphorylated both when Tyr-397 is mutated to phenylalanine, and in the presence of the Src inhibitor, PP2, suggesting that removal of the amino terminus can render FAK Src independent. This is in contrast to the K38A mutant that is not phosphorylated in the Y397F background, and which shows decreased phosphorylation in the presence of the Src inhibitor PP2, suggesting that regulation of FAK by Src is a secondary step in its activation. The K38A mutation weakens the interaction between the amino terminus of FAK and its own kinase domain, and disrupts the ability of the amino terminus to inhibit the phosphorylation of FAK in trans. The K38A mutation of FAK also increases the ability of FAK to promote cell cycle progression and cell migration, suggesting that hyperphosphorylation of this mutant can positively affect FAK function in cells. Together, these data strongly suggest a role for the first FAK subdomain of the FERM domain in its normal regulation and function in the cell.
Quantification of saccadic fatigability and diagnostic efficacy for myasthenia gravis

Abstract:Background and Objectives The diagnostic challenge of myasthenia gravis (MG) is exacerbated by the variable efficacy of current testing methodologies, necessitating innovative approaches to accurately identify the condition. This study aimed to assess ocular muscle fatigue in patients with MG using video-oculography (VOG) by examining repetitive saccadic eye movements and comparing these metrics to those of healthy control participants. Methods This prospective, cross-sectional study was conducted at a tertiary care center and involved 62 patients diagnosed with MG (48 with ocular MG and 14 with generalized MG) and a control group of 31 healthy individuals, matched for age and sex. The assessment involved recording saccadic eye movements within a ± 15° range, both horizontally and vertically, at a rate of 15 saccades per minute over a 5-min period, resulting in 75 cycles. Participants were afforded a 3-min rest interval between each set to mitigate cumulative fatigue. The primary outcome was the detection of oculomotor fatigue, assessed through changes in saccadic waveforms, range, peak velocity, latency, and the duration from onset to target, with a focus on comparing the second saccade against the average of the last five saccades. Results In the evaluation of repetitive saccadic movements, patients with MG exhibited a reduced saccadic range and a prolonged duration to reach the target, compared to healthy subjects. Furthermore, a significant elevation in the frequency of multistep saccades was observed among MG patients, with a marked rise observed over consecutive trials. Receiver operating characteristic (ROC) analysis revealed the discriminative performance of multistep saccade frequency, in conjunction with variations in saccadic range and duration from onset to target achievement between the second saccade and the mean of the final five saccades, as effective in distinguishing MG patients from healthy subjects. Although alterations in peak saccadic velocity and latency were less pronounced, they were nevertheless detectable. Discussion The utilization of VOG for repetitive saccadic testing in the diagnosis of MG has demonstrated considerable diagnostic precision. This methodology affords significant accuracy in evaluating ocular muscle fatigue in MG patients, providing class III evidence supportive of its clinical application.

Abstract:Objective: Distinguishing normal, neuropathic and myopathic electromyography (EMG) traces can be challenging. We aimed to create an automated time series classification algorithm. Methods: EMGs of healthy controls (HC, n = 25), patients with amyotrophic lateral sclerosis (ALS, n = 20) and inclusion body myositis (IBM, n = 20), were retrospectively selected based on longitudinal clinical follow-up data (ALS and HC) or muscle biopsy (IBM). A machine learning pipeline was applied based on 5-second EMG fragments of each muscle. Diagnostic yield expressed as area under the curve (AUC) of a receiver-operator characteristics curve, accuracy, sensitivity, and specificity were determined per muscle (muscle-level) and per patient (patient-level). Results: Diagnostic yield of the classification ALS vs. HC was: AUC 0.834 ± 0.014 at muscle-level and 0.856 ± 0.009 at patient-level. For the classification HC vs. IBM, AUC was 0.744 ± 0.043 at muscle-level and 0.735 ± 0.029 at patient-level. For the classification ALS vs. IBM, AUC was 0.569 ± 0.024 at muscle-level and 0.689 ± 0.035 at patient-level. Conclusions: An automated time series classification algorithm can distinguish EMGs from healthy individuals from those of patients with ALS with a high diagnostic yield. Using longer EMG fragments with different levels of muscle activation may improve performance. Significance: In the future, machine learning algorithms may help improve the diagnostic accuracy of EMG examinations.
A Quantitative Study of Factors Influencing Myasthenia Gravis Telehealth Examination Score

Abstract:Introduction: The increasing utilization of telemedicine offers an opportunity of video-recording to then perform post-hoc quantitative analysis of all aspects of the patient-examiner interaction. Presently, telemedicine examinations are adapted from the standard clinical visit, but their benefits and deficiencies have not been rigorously assessed. Methods: We utilized a bank of 54 subjects with myasthenia gravis (MG) each having two telemedicine examinations by neuromuscular experts with recording of the MG Core Examination and the MG Activities of Daily Living. We then applied a broad spectrum of artificial intelligence algorithms from computer vision and speech analysis to natural language processing to generate quantitative metrics of the digital records. Results: We successfully developed a technology to assess video examinations. Although the overall MG-CE examination scores were consistent across examiners, individual metrics showed significant variability, with up to a 25 percent variation in scoring within the MG-CE's range. There was wide range of compliance with MG ADL instructions across examiners. Discussion: We are able perform digital analysis of neuromuscular examinations and identified variations in outcome measure variations based on the examiner's instructions, video recording limitations, and the severity of the patient's disease. Particularly noteworthy is the high standard deviation in scores for examinations of patients with low disease severity.

Machine Learning Methods to Track Dynamic Facial Function in Facial Palsy

Abstract:For patients with facial paralysis, the wait for return of facial function and the resulting vision risk from poor eye closure, difficulty speaking and eating from flaccid oral sphincter muscles, as well as the psychological morbidity from the inability to smile or express emotions through facial movement can be devastating. There are limited methods to assess ongoing facial nerve regeneration: clinicians rely on subjective descriptions, imprecise scales, and static photographs to evaluate facial functional recovery and thus facial nerve regeneration remains poorly understood. We propose a more precise evaluation of dynamic facial function through video-based machine learning analysis which would facilitate a better understanding of the sometimes subtle onset of facial nerve recovery and improve guidance for facial reanimation surgery. Specifically, we here present machine learning methods employing likelihood ratio tests, optimal transport theory, and Mahalanobis distances to: 1) assess the use of defined facial landmarks for binary classification of different types of facial palsy; 2) identify regions of asymmetry and potential paralysis during specific facial cues; and 3) determining severity of abnormal facial function when compared to a reference class of normal facial function. Our work presents promising results of utilizing videos, rather than static photographs, to provide robust quantitative analyses of dynamic properties for various facial movements without requiring manual assessment. The long-term potential of this project is to enable clinicians to have more accurate and timely information to make decisions for facial reanimation surgery which will have drastic consequences on quality of life for affected patients.
