Abstract:Background: Breast cancer (BC) affects racial and ethnic groups differently, leading to disparities in clinical presentation and outcomes. It is unclear how Hispanic ethnicity affects BC outcomes based on geographic location and proximity to the United States (U.S.)/Mexico border. We hypothesized that the impact of race/ethnicity on BC outcomes depends on geographic location and country of origin within each BC subtype. Methods: We analyzed BC data from the Texas Cancer Registry by race/ethnicity/birthplace according to BC subtype (luminal A/luminal B/human epidermal growth factor receptor 2 [HER2]/triple-negative breast cancer[TNBC]). Other covariates included age, geographic location (U.S., Mexico), residency (border, non-border), treatments, and comorbidities. Crude and adjusted effects of race/ethnicity and birthplace on overall survival (OS) were analyzed using Cox regression methods. Results: Our analysis of 76,310 patient records with specific BC subtypes revealed that Hispanic and non-Hispanic Black (NHB) patients were diagnosed at a younger age compared with non-Hispanic White (NHW) patients for all BC subtypes. For the 19,748 BC patients with complete data on race/ethnicity/birthplace/residency, Hispanic patients had a higher mortality risk in the Luminal A subtype, regardless of birthplace, whereas U.S.-born Hispanics had a higher risk of death in the TNBC subtype. In contrast, NHB patients had a higher mortality risk in the Luminal A and HER2 subtypes. Residence along the U.S./Mexico border had little impact on OS, with better outcomes in Luminal A patients and worse outcomes in Luminal B patients aged 60-74 years. Conclusion: Race/ethnicity, geographic birth location, and residency were significant predictors of survival in BC. Migration, acculturation, and reduced healthcare access may contribute to outcome differences.

Abstract:e13698 Background: Despite advancements in breast cancer therapy leading to a decline in mortality rates within certain ethnic populations, there exists a concerning trend of increasing cancer incidence among African American (AA) and Hispanic women. This persistent rise highlights the urgent need to explore and comprehend the socio-demographic and breast cancer characteristics specific to these populations. Methods: We assembled a 10-year database of women diagnosed with breast cancer in the community hospital in Chicago. After excluding minority ethnic groups, specifically AA, Hispanic, and Caucasian cohorts were included in this study, comprising 786 patient records analyzed. Results: The distribution revealed a predominance of AA patients at 49.7%, Hispanics at 42.1%, and Caucasians at 8.14%. Noteworthy differences across the three ethnic groups were observed in age categories at cancer diagnosis, BMI categories, family history of first-degree relatives with breast cancer, marital status, smoking habits, menopausal status, and alcohol use at the time of diagnosis (p < 0.05). Within the molecular subtypes, 14.5% were identified as triple-negative breast cancer (TNBC), with a higher prevalence of 18.7% among the AA subgroups. 19.6% of patients had advanced disease, proportion was higher in AA ethnic group (22.8%; p = 0.83) compared to other ethnicities. Breast conservative procedures were prevalent in almost half of the patient population on average (52.1 %), with a slightly higher frequency of mastectomies noted in the Hispanic patient group (44.1%, p < 0.05). In logistic regression analysis, compared to the reference AA ethnicity, Hispanics exhibited higher odds of having HER-2 positive breast cancer (OR 1.74; CI 1.04 – 2.94; p < 0.05), as did Caucasians (OR 2.35; CI 1.05 – 5.04; p < 0.05). Conclusions: In summary, our study underscores substantial socio-demographic and breast cancer characteristic differences among AA and Hispanic populations compared to Caucasians, revealing notable ethnic disparities in breast cancer. Despite these variations, our findings highlight promising healthcare equity, with comparable access to surgical and chemotherapy interventions across ethnicities. TNBC demonstrates molecular heterogeneity and a higher prevalence among AA women, contributing to significant racial disparities.

Abstract:Breast cancer is the most commonly diagnosed cancer globally and the leading cause of cancer death in women, with ethnic disparities reported in cancer incidence, prognosis, diagnosis and therapeutic response. Although precision oncology holds the promise of revolutionising healthcare, it could exacerbate the racial disparities it seeks to eradicate unless rigorous efforts are made to address research biases. We evaluated the molecular and clinical effects of genetic ancestry in African and South Asian women using a combined cohort of 7,136 breast cancer patients available from four data sources — the 100,000 Genomes Project (UK), The Cancer Genome Atlas (US), the Breast Cancer Now Biobank (London, UK) and Genes & Health (UK). Using patients assigned to the European genetic ancestry as the baseline comparator for all analyses, we find that non-European patients present with breast cancer significantly earlier and die at a younger age. Patients within the African group also have an increased prevalence of higher grade and hormone receptor negative disease. South Asian patients show a small tendency towards lower stage at diagnosis, and a lower tumour mutational burden. We observed significant differences and similarities in the somatic mutational landscape of the non-European populations. Six genes, RBM5, OTOF, FBXW7, NCKAP5, NOTCH3 and GPR158, were differentially mutated across multiple cohorts. Furthermore, potential therapeutic candidates (BRIP1, CDKN2A, CHEK2, FBXW7, GPR158, KDM6A, RET, STK11) were found to be differentially mutated across the African and/or South Asian genetic ancestry groups. Genes with significant differences in germline mutation rates were identified in African and South Asian populations, including those used in current genetic testing (African: TP53, BRCA1, BRCA2, PALB2, p<0.001; South Asian: TP53, BRCA1, BRCA2, p<0.05) as well as those implicated in breast cancer predisposition in the literature, such as CDH1, CDK2A, ERCC3, EPCAM, FANCA, FANCC, POLE and PMS2. There is a higher propensity for BRCAness in the African population, with a lower rate in the South Asian population, serving as a potential prognostic indicator into the response to therapies such as PARP inhibitors. Our study confirms the under-representation of non-European ethnic minority groups within research studies, clinical applications and biobanks, with none of the resources able to recapitulate the ethnic diversity of their representative geographical locations (UK, London and US). Finally, our findings advocate for the implementation of ancestry-specific germline mutation breast cancer screening windows and germline screening panels. This study harnesses multimodal data to improve our understanding of ancestry-associated differences in breast cancer and highlight opportunities to advance health equity in breast cancer thus taking one step closer to achieving the promise of equitable precision oncology.
Racial Disparities in Young-Onset Patients with Colorectal, Breast and Testicular Cancer

Abstract:Aims: Racial disparities in cancer mortality persist despite rapid developments in cancer treatment strategies. In recent decades, an increased frequency of patients with young-onset cancer has been reported. However, few studies have assessed racial disparities in clinical features and overall survival among young-onset patients with colorectal, breast, and testicular cancer. Therefore, we evaluated racial disparities in cancer mortality for these three cancer types. Methods: We extracted the data of eligible patients from the Surveillance, Epidemiology and End Results (SEER) database from 1973 to 2014. Overall and cancer-specific survival rates were compared among races using Kaplan-Meier curves. Adjusted hazard ratios (HRs) with 95% confidence intervals (CIs) were calculated, and the association of race with survival was influenced by marital status, surgery and disease stage in Cox proportional hazard models. Results: We collected the data of 19,574 patients with colorectal cancer, 68,733 with breast cancer, and 26,410 with testicular cancer; all were aged 25-40 years. A higher proportion of Blacks presented with a distant stage at diagnosis compared to Whites and Others (colorectal cancer: 18.0%, 18.5% and 18.4%, respectively, P = 0.004; breast cancer: 3.5%, 6.3% and 4.0%, respectively, P < 0.001; testicular cancer: 6.9%, 10.8% and 8.6%, respectively, P < 0.001). Multivariate analysis showed that Blacks had the highest overall mortality rate (colorectal cancer, HR, 1.277, 95% CI: 1.198, 1.361, P < 0.001; breast cancer, HR, 1.471, 95% CI: 1.420, 1.525, P < 0.001; testicular cancer, HR, 1.887, 95% CI: 1.562, 2.281, P < 0.001). In stratified analyses, Unmarried Blacks had a higher mortality rates (colorectal cancer, HR, 1.318, 95% CI: 1.211, 1.435, P < 0.001; breast cancer, HR, 1.465, 95% CI: 1.394, 1.541, P < 0.001; testicular cancer, HR, 1.944, 95% CI: 1.544, 2.447, P < 0.001). Furthermore, Blacks with colorectal and breast cancer had a higher risk of mortality than Whites at every disease stage, with greatest disparities occurred among individuals at localized stage. The influence of racial disparities on survival was consistent among patients who accepted surgery, but was weak among those who did not undergo surgery for colorectal cancer (Blacks, HR, 1.027, 95% CI: 0.866, 1.219, P = 0.758; Others, HR, 0.919, 95% CI: 0.760, 1.112, P = 0.386) and testicular cancer (Blacks, HR, 1.039, 95% CI: 0.538, 2.007, P = 0.909; Others, HR, 0.772, 95% CI: 0.388, 1.533, P = 0.459). Conclusions: We demonstrated that Blacks had a worse prognosis for young-onset colorectal, breast, and testicular cancer. Marital status, cancer-directed surgery and disease stage may influence the association of race with the risk of mortality. Equal access to high-quality medical care among races, greater social support and comprehensive interventions are required. Moreover, further studies need to clarify the effects of biological properties like genetic differences between races on cancer patient survival.

Abstract:Abstract Abstract #2024 Background and Rational: Ethnic-specific disparities in breast cancer (BC) stage of presentation and survival rates are well documented. To further investigate possible ethnic-specific genetic contributions to these disparities, we are completing gene expression profiling studies in a multi-ethnic cohort consisting of thirty “Triple Negative” BC patients [10 each African-American (AA), Hispanic (His) and non-Hispanic white (Cauc) women] matched for age of diagnosis and hormone receptor status. The overall study aim is an increased understanding of the biological basis of ethnic-specific BC disparities, leading ultimately to individualized, ethnic-specific diagnostic and therapeutic approaches. Two immediate study goals are to demonstrate the utility of FFPE samples in obtaining consistent, reproducible data from gene expression arrays, and secondly, to identify differentially expressed genes between tumor and normal tissue that are common or unique among the three ethnic groups. Methods: Pathology specimens were freshly cut from FFPE blocks and marked by a pathologist as to normal vs. tumor tissue. RNA isolation, labeled cDNA preparation, and hybridization of tumor and normal cDNAs to a breast cancer focused gene expression microarray (Breast Cancer DSA Research Tool) was performed by Almac Diagnostics. Each patient was self-matched (tumor vs. normal tissue) for gene expression studies. Results: Using 36 matched tumor and normal FFPE samples from 18 patients, approximately 17516 transcripts were detected on the Breast Cancer DSA with intensity significantly greater than background. For normal and tumor tissue samples, 9399 and 10,296 transcripts respectively, were detected in all three ethnic groups. Importantly, a subset of transcripts (hundreds to one thousand) was detected in only one or two ethnic groups. Using two-way ANOVA (disease state and ethnicity), a subset of 6479 transcripts was identified with p-value less than 0.01 in the statistical test and was selected and further used in data quality control. Data QC indicated that patient samples clustered well with respect to both ethnicity and normal versus tumor tissue. Additional analytical methods included K-means 2-Dimensional clustering and Principal Component Analysis. From these analyses, we identified ethnic-specific expression patterns in the matched normal and tumor tissue samples. We are completing these studies by increasing sample size and matching for stage of diagnosis, mapping clusters of differentially-expressed genes in pathway analysis, and validation by real-time PCR. In the longer term, DNA copy number variation (CNV) and chromosomal alterations will be investigated by high density arrays. Summary: These preliminary analyses shows that high quality gene expression data can be generated from FFPE samples, and that ethnic specific gene expression differences can be detected in tumor and matched normal breast tissue samples across ethnic groups. Abstract:Although breast cancer is by far the most common cancer among all women worldwide, its incidence rates vary substantially in different populations. In the United States, rates are highest among white women, followed by African American (AA) and Hispanic American (HA) women. Among HA women, breast cancer incidence rates are higher among those born in the United States than among first-generation immigrants, who, in turn, have higher rates than their counterparts in their countries of origin. Among foreign-born HA women, breast cancer risk has been shown to increase with the duration of U.S. residency, degree of acculturation, and younger age at migration, underscoring the importance of environmental influence on breast cancer risk. On the other hand, overall breast cancer risk among HA women has been shown to increase with the degree of European contribution to their genetic admixture. Among AA women, a higher proportion of European ancestry has been associated with a less aggressive form of breast cancer (estrogen receptor [ER] positive/progesterone receptor [PR] positive and localized). These observations highlight the need for understanding the population genetic substructure in the evaluation of racial/ ethnic disparities in breast cancer incidence and prognosis. Although breast cancer is more likely to occur in white women, AA and HA breast cancer patients are more likely to die of their disease. The age at onset tends to be earlier in AA and HA women, and these cases tend to have more aggressive histologic characteristics, including the triplenegative breast cancer (TNBC) that lacks expression of ER, PR, and human epidermal growth factor receptor 2 (HER2). Interestingly, limited evidence suggests that TNBC is also overrepresented among Hispanic and African women in their countries of origin. The predisposition to more aggressive tumors likely has contributed to the higher mortality rates among young minority women. However, the poor prognosis of breast cancer among AA and HA women has also been attributed, in part, to inadequate health insurance, limited access to mammography, advanced disease stage at diagnosis, delayed and inadequate treatment, and a high prevalence of comorbidity.

Abstract:Background: In the U.S., there are stark racial and ethnic differences in the incidence and mortality of breast cancer that cannot be fully explained by lifestyle and environmental factors. The contribution of tumor biology to these differences in breast cancer outcomes across race and ethnicity is poorly understood. We conducted a preliminary analysis of the racial and ethnic differences in the gene expression profiles of biological signatures of breast tumors within in the Multiethnic Cohort Study (MEC). Methods: For 279 postmenopausal females with invasive breast cancer within the MEC, we examined 42 breast cancer signatures based on gene expression profiling of formalin-fixed paraffin-embedded breast tumors using the NanoString nCounter Breast Cancer 360TM (BC360) Panel. This included 36 African American, 62 Japanese American, 51 Latino, 65 Native Hawaiian, and 65 White females. Multivariable linear regression was conducted to examine racial and ethnic differences in gene expression levels, adjusting for age at diagnosis, stage (localized or regional/distant), and hormone receptor status (HR- positive, negative, or unknown). A Bonferroni corrected p-value threshold of 0.0019 (p=0.05/42 tests) was used to determine statistical significance. Results: We identified racial and ethnic differences in breast tumor gene expression signatures for four broad categories of breast tumor biology: breast cancer-specific, tumor, immune, and tumor microenvironment. Specifically, the differentially expressed signatures in these four categories were: breast cancer-specific (CDK4 p<0.001; ER-signaling p=0.013), tumor (apoptosis p=0.004; p53 p=0.002; proliferation p=0.026; cell adhesion p=0.029; SOX2 p=0.002), immune (B7-H3 p=0.024; CD8 T-cells p=0.037; MHC2 p=0.032) and tumor microenvironment (hypoxia p=0.022) signatures. Racial and ethnic differences in the expression levels of the cyclin-dependent kinase 4 (CDK4) signature was found to be statistically significant (p<0.001). In comparison to White females, increased expression of the CDK4 signature was observed in breast tumors of African American (p=0.001) and Latino (p=0.001) females and no differences were observed for Japanese American and Native Hawaiian females. Conclusion: Our preliminary results suggest racial and ethnic differences in breast tumor biology and the immune-microenvironment. As CDK4 is a key regulator of cell cycle progression, the increased expression of CDK4 in breast tumors from African American and Latino females compared to White females may reflect greater proliferation activity. CDK4 is a drug target of CDK4/6 inhibitors that could potentially serve as a basis for population-guided treatment strategies. Future work will examine additional breast tumor samples and evaluate association with clinico-pathological characteristics and survival across this multiethnic population. Abstract:Breast cancer (BCa) has long been a health burden to women across the globe. However, the burden is not equally carried across races. Though the manifestation and behavior of BCa differs among racial groups, the racial representation of models used in preclinical trials and clinical trial participants lacks this heterogeneity. Women of African Ancestry (WAA) are disproportionately afflicted by having an increased risk of developing BCas that are more aggressive in nature, and consequently suffer from poorer outcomes relative to women of European ancestry (WEA). Notwithstanding this, one of the most commonly used tools in studying BCa, cell lines, exhibit a sizeable gap in cell line derivatives of WEA relative to WAA. In this review, we summarize the available BCa cell lines grouped by race by major suppliers, American Type Culture Collection (ATCC) and the European Collection of Authenticated Cell Cultures (ECACC). Next, examined the enrollment of WAA in clinical trials for BCa. Of the cell lines found provided by ATCC and ECACC, those derived from WEA constituted approximately 80% and 94%, respectively. The disparity is mirrored in clinical trial enrollment where, on average, WEA made up more than 70% of participants in trials found where ancestry information was provided. As both experimental models and clinical trial participants primarily consist of WEA, results may have poorer translatability toward other races. This highlights the need for greater racial diversity at the preclinical and clinical levels to more accurately represent the population and strengthen the translatability of results.

Abstract:Background: In the U.S., breast cancer (BC) survival differs significantly by race and ethnicity, with Asian American (AA) patients consistently showing more favorable survival relative to other racial and ethnic groups. Factors contributing to the survival advantage of AA females, a heterogeneous population, are unknown. Methods: We included 5,294 AA female patients diagnosed with incident AJCC stage I-IV BC from 2000-2015 at Kaiser Permanente Northern California, and a random sample of 5,294 non-Latina White (NLW) patients. Follow-up was through 12/31/2020 for recurrence, second primary cancer, BC death, and all-cause death. Survival analysis was conducted using multivariable, cause-specific Cox proportional hazards models. Results: The AA cohort consisted of 41% Filipina, 27% Chinese, 9% Japanese, 9% South Asian, 7% Other Asian, 3% Korean, and 5% Southeast Asian females. AA and NLW females had similar stage (12% III/IV) and ER/PR status (15-18% ER-/PR-), yet more AA females were diagnosed with HER2+ or borderline disease (19% vs. 14%). More AA females had chemotherapy (48% vs. 39%) and mastectomy (47% vs. 36%) with less radiation therapy (38% vs. 46%) than NLW females. Receipt of endocrine therapy was similar between AA and NLW females (72-73%). Compared with NLW females, AA females were younger (mean=56.9 vs. 63.2 years), lived in higher SES neighborhoods (census tract-level education and income), had no comorbidities (45% vs. 36%), lower BMI (mean=25.5 vs. 28.6 kg/m2), and were never smokers (77% vs. 50%) at diagnosis. Fewer AA vs. NLW females met national guidelines of 150 minutes/week of moderate-vigorous exercise (27% vs. 33%). In multivariable Cox models adjusted for diagnosis year, diagnosis age, stage, ER/PR and HER2 status, chemotherapy, radiation therapy, endocrine therapy, and surgery type, AA females had 20% lower risk of BC death and all-cause death compared with NLW females. Similar lower risks were seen across AA ethnicities. Specifically, Japanese and Chinese females had lower risks of BC death (hazard ratio, HR=0.66; 95% confidence interval, CI: 0.47-0.93 and HR=0.78; 95% CI: 0.63-0.97) and all-cause death (HR=0.67; 95% CI: 0.53-0.84 and HR=0.78; 95% CI: 0.67-0.92). South Asian females also had lower risk of all-cause death (HR=0.71; 95% CI: 0.53-0.96). AA females had 17% lower risk of second primary cancer compared with NLW females, specifically for Japanese, South Asian, and Other Asian females (HR range: 0.60-0.66). No difference in risk of recurrence was observed for AA vs. NLW females (HR=1.08; 95% CI: 0.90-1.29), including by Asian ethnicities. Conclusions: In this large, multiethnic cohort in an integrated health care system, clear differences in sociodemographic, clinical, and lifestyle factors exist between AA and NLW BC patients. After accounting for these factors, AA females had a survival advantage over NLW females, mostly in Japanese, South Asian, and Chinese females. These findings identify contributors to outcomes that may be specific to Asian ethnicities and can inform improving survival in all BC patients. Abstract:Background: Despite advances in medical knowledge and treatment, disparities in breast cancer outcomes persist along demographic and socioeconomic lines. To quantify these disparities, the Area Deprivation Index (ADI) emerges as a powerful metric. It offers insights into the interplay between neighborhood socioeconomic status and breast cancer outcomes by encompassing factors pertaining to income, education, employment, and housing quality. There have been limited studies to examine the role of race and neighborhood disadvantage on specific breast cancer outcomes. Kentucky also faces a unique burden as it has the third highest breast cancer mortality rate in the United States. This study aims to examine the association between neighborhood socioeconomic disadvantage, and breast cancer outcomes including receipt of surgical treatment, radiation therapy, chemotherapy, and breast cancer-specific survival. While also examining how any associations are modified by race and ethnicity among Kentucky breast cancer patients. Methods: We conducted a retrospective cohort analysis using data from the Kentucky Cancer Registry (KCR) among breast cancer patients diagnosed between 2010 and 2017 with follow-up data through December 31, 2022. The KCR dataset was then linked with census tract data to examine ADI. We conducted consecutive multilevel logistic regression for binary outcomes and Cox Proportional hazard models using SAS (version 9.4, SAS Institute, Inc., Cary, North Carolina, USA) for time-to-event of breast cancer survival. Results: Women in the most disadvantaged quartile (4th quartile) were more likely to be Non-Hispanic Black (NH-Black), Appalachian residents, current tobacco users, divorced and using government insurance. Compared to women in the least disadvantaged neighborhoods (1st quartile) women living in the most disadvantaged neighborhoods (4th quartile) were 50% more likely to die from breast cancer (HR: 1.50, 95 CI: 1.28-1.76). Women in the most disadvantaged neighborhoods (4th quartile) were also more likely to be diagnosed at later stages (aOR:1.26, 95 CI:1.13-1.42). These associations were all observed after adjusting for age, race, tobacco use, marital status, insurance status, family history, and residence in Appalachia. Conclusions: Women living in areas of greater disadvantage had significantly increased odds of late-stage diagnosis and risk of breast cancer death regardless of race suggesting that neighborhood-level factors contribute to breast cancer disparities. Our findings underscore the need for neighborhood-level interventions to improve cancer outcomes. Citation Format: Breyanna E. Abstract:Ethnic disparities in breast cancer (BC) outcomes have been reported, with lower survival rates among black women compared to white. Additionally, the unfavorable triple negative (TNBC) sub-type is more prevalent in black women and within this population, risk of mortality is again higher compared to white women. Biological contributors to this disparity are less clearly understood than socio-economic factors. Recent studies recognize different breast tumour immune microenvironments between black and white BC patients, necessitating further research into ethnicity-related differences in anti-tumor immune responses. Such responses begin in regional lymph nodes (LNs) with tumor neoantigen presentation. Here, we generate single-cell spatial maps of involved TNBC LN cores in treatment naïve black and white women. To guide design of a phenotyping panel for LNs, we profiled blood samples (n=95) using high-dimensional flow cytometry from newly diagnosed black and white BC patients. We found differential skewing of NK cell subsets, lower memory B cell (identified as IgM+CD27+IgD- & IgM+CD27+IgD+ cells) abundance and higher CD4+ T effector memory presence in black patients compared to white. Additionally, higher antigen presentation capacity was observed with increased intermediate monocytes and conventional DCs, confirmed by MS-based proteomics. Using highly multiplexed imaging with a panel of 26 markers, we performed single-cell spatial immune profiling to characterize cellular phenotypes, neighborhoods and interactions within a cohort of LN core biopsies from black patients and white patients. A total of 536,113 cells were segmented for analysis. Cell abundance analysis revealed higher CD8+ T effector memory and CD4+ T cell subsets in white patients, except for CD4+ T effector memory cells which were higher in black patients, consistent with blood profiling. Cellular neighborhood analysis revealed black patient tumor-rich regions were enriched for M2 macrophages while white patients had no such enrichment. In these white patients, a neighborhood resembling canonical follicular structures almost exclusively containing B cells was identified. Interestingly in these black patients, analogous B cell rich regions also comprised a heterogeneous array of other immune cells. Further analysis to explore B cell zones is ongoing. Preliminary interaction analysis identified immunosuppressive crosstalk in black patients with enriched Treg & CD8+ T cell and Treg & NK interactions, not observed in white patients. These initial findings suggest ethnicity-based variation in LN architectures from treatment naive patients with TNBC, supporting the hypothesis that variation in immune programming could translate into differences in pro and anti-tumor immunity, contributing to disparities in outcome amongst black and white patients. Abstract:Simple Summary: Racial disparities in cancer incidence and outcome rates are prevalent in the US, with a variety of contributing factors such as socioeconomic status, differences in lifestyle and environmental exposures, and gene polymorphisms. The goal of this research was to broadly analyze public data to identify critical differences in molecular signatures and pathways between races. Additionally, to ensure the clinical translatability of our work, we analyzed the impact of these differences on patient survival. Our findings will help inform the use of novel biomarkers in clinical settings and the future development of precision therapies.Abstract:Epidemiological studies highlight a disparity in cancer incidence and outcome rates between racial groups in the United States. In our study, we investigated molecular differences among racial groups in 10 carcinoma types. We used publicly available data from The Cancer Genome Atlas to identify patterns of differential gene expression in tumors obtained from 4,112 White, Black/African American, and Asian patients. We identified race-dependent expression of numerous genes whose mRNA transcript levels were significantly correlated with patient survival. A small subset of these genes was differentially expressed in multiple carcinomas, including genes involved in cell cycle progression such asCCNB1,CCNE1,CCNE2, andFOXM1. In contrast, genes such as transcriptional factorETS1and apoptotic geneBAK1were differ-entially expressed and clinically significant only in specific cancer types. Our analyses also revealed race-dependent regulation of relevant pathways. Importantly, homology directed repair and ERBB4-mediated nuclear signaling were both upregulated in Black patients compared to Whites in four carcinoma types. This large-scale pan-cancer study refines our understanding of the cancer health disparity and can help inform the use of novel biomarkers in clinical settings as well as the future development of precision therapies.

Abstract:Intro: Lower socioeconomic status (SES) and African ancestry have been linked to poor overall survival (OS) in colorectal cancer (CRC). To better understand the interplay between these variables, we analyzed clinical and genomic real-world data from a large cohort of patients treated at a single tertiary center. Methods: Clinical annotations including sex, primary tumor location, age and stage at diagnosis were extracted from the electronic health record for 3,817 CRC patients who underwent clinical genomic sequencing at Memorial Sloan Kettering Cancer Center between 2014 and 2022. We also cataloged each patient's type of primary health insurance (commercial, Medicare, Medicaid or self-pay). Socioeconomic status was assessed from each patient's billing address using the Yost index (YI), which is a location-dependent percentile score based on household incomes, house values, rent, poverty, education, and employment statistics. Genetic ancestry was determined from DNA sequencing data using reference populations from the 1000 Genomes project, including European (EUR), African (AFR), East Asian (EAS), and Southeast Asian (SAS). Specific ancestries were assigned based on an ancestral fraction of >80%, otherwise patients were labeled as admixed (ADM). OS was measured from time of diagnosis to last follow-up. Results: Our cohort included 2,627 EUR, 195 EAS, 206 AFR, 61 SAS and 728 ADM patients. The median YI within our cohort was 18 [IQR: 30]. No significant associations were observed between YI and primary tumor location, age or stage at diagnosis. We observed significant differences in YI based on type of health insurance (median 15 for commercial vs. 29 for Medicaid, p<0.0001) and sex (median 15 in males vs. 17 in females, p=0.0113). YI was also strongly associated with genetic ancestry, with AFR patients exhibiting significantly higher YI values than other ancestries (median 45.5 vs. 14, p<0.0001). No significant differences in YI were observed among EUR, EAS and SAS patients. When all ancestries were analyzed together, patients within the bottom YI quartile (most affluent) had significantly longer OS than patients from the top YI quartile (most deprived) (median 52.3 vs. 61.6 months, p=0.023). No differences in OS were observed between EUR, EAS and SAS patients, but patients of AFR ancestry had significantly shorter OS than the rest (median 44.5 vs. 56.5 months, p= 0.016). When the analysis was restricted to patients of EUR, EAS and SAS ancestry, OS was still significantly shorter for patients within the top vs. bottom YI quartile (median 52.6 vs. 64.4 months, p=0.011). Conclusions: Patients of African ancestry exhibited lower socioeconomic status and shorter survival. Even when the analysis was restricted to other genetic ancestries, patients with the lowest socioeconomic status still had poorer clinical outcomes. The existence of these differences within a relatively homogeneous cohort of patients treated at the same institution highlights the need to better understand and address health disparities in CRC. Abstract:Cancer incidence and mortality rates can vary widely among different racial and ethnic groups, attributed to a complex interplay of genetic, environmental and social factors. Recently, substantial progress has been made in investigating hereditary genetic risk factors and in characterizing tumour genomes. However, most research has been conducted in individuals of European ancestries and, increasingly, in individuals of Asian ancestries. The study of germline and somatic genetics in cancer across racial and ethnic groups using omics technologies offers opportunities to identify similarities and differences in both heritable traits and the molecular features of cancer genomes. An improved understanding of population-specific cancer genomics, as well as translation of those findings across populations, will help reduce cancer disparities and ensure that personalized medicine and public health approaches are equitable across racial and ethnic groups.

Abstract:Purpose: In breast cancer, research has shown that race/ethnicity can be associated with disease predisposition, access to care, and outcomes. There are numerous studies that combine Hispanic and Haitian women of all races into a single group may overlook opportunities to identify important characteristics within this heterogeneous group that could more accurately predict outcomes and reveal opportunities to improve care. While these groups have been studied, there is limited data about the impact race and ethnicity has on young adult populations with breast cancer. In this study, we sought to compare tumor subtype, stage at diagnosis, time to surgery (TTS), and overall survival (OS) among Hispanic and Haitian patients to patients of non-Hispanic descent. Methods: Women 18 years of age or older who had been diagnosed with stage 0-IV breast cancer and who had undergone lumpectomy or mastectomy were identified in the National Cancer Database (2015-2021). Tumor subtype and stage at diagnosis were compared by race/ethnicity. Multivariable linear regression and Cox proportional hazards modeling were used to estimate associations between race/ethnicity and adjusted TTS and OS, respectively. Results: A total of 48,150 patients were included in this study. Age ranged from 18-40. (American Indian [AI]: 79 [0.2%]; Haitian: 15,011 [31.2%]; White: 20,109 [85.48%]; Hispanic: 13,030 [27%]. Hispanic women had higher rates of triple-negative disease (23.9%) than did Haitian women (11.5%) and white women (10.9%; P < .001). Hispanic women had higher rates of node-positive disease (26.2%) in comparison to Haitian women (17%) and Non-Haitian/Hispanic women (10% P < .001). Hispanic women had worse PFS when diagnosed at Stage II and III (37 and 39 months, respectively) than Haitian women (57 and 59 months, respectively) and Non-Haitian/Hispanic women (82 and 87 months). Hispanic women had worse OS when diagnosed at Stage II and III (45 and 49 months) than Black women (70 and 75 months) and non- women (95 and 99 months). Conclusion: Hispanic women had worse PFS versus non-Hispanic and Haitian women as well as present with high-risk characteristics and nodal disease at the time of presentation. This suggests that racial differences in tumor subtype and nodal stage among Hispanic women highlight the importance of developing additional studies which can investigate the disparity in patient outcomes with young Hispanic women in breast cancer research. Abstract:Background Breast Cancer (BC) is a complex disease with profound genomic aberrations. However, the underlying molecular disparity influenced by age and ethnicity remains elusive. Method We aimed to investigate the molecular properties of 843 primary and metastatic BC patients enrolled in the K-MASTER program. We explored the unique molecular profiles of breast cancer patients by categorizing them into two distinct age subgroups. The first subgroup comprised young breast cancer (YBC) patients, defined as women under the age of 40, while the second subgroup consisted of older breast cancer (OBC) patients. Our study aimed to characterize the molecular differences between these age-defined groups, providing insights into the underlying mechanisms of breast cancer in different age cohorts. Additionally, we leveraged large-scale genomic data from the TCGA and MSK-IMPACT studies to examine the ethnic-driven molecular and clinical disparities. Results The K-MASTER patients were mainly comprised of triple-negative breast cancer (TNBC) and HER2-positive tumors, while the TCGA and MSK-IMPACT cohorts exhibited a predominance of hormone receptor-positive (HR+) subtype. We observed a high prevalence of PI3KCA mutations in K-MASTER HER2+ tumors, particularly in older patients. Moreover, we identified increased mutation rates in DNA damage response molecules, including ARID1A, MSH6, and MLH1. Importantly, GATA3 mutations were less frequently observed in East Asian patients, which correlated with poor clinical outcomes. Our analysis also revealed specific associations between genomic aberrations, organotropisms, and metastatic breast cancer. In addition to characterizing the molecular disparities, we developed a gradient-boosting multivariable model to identify a new molecular signature that could predict the therapeutic response to platinum-based chemotherapy. Conclusion Our findings collectively provide unprecedented insights into the significance of age and ethnicity on the molecular and clinical characteristics of BC patients. Figure 1. Abstract:Background: Ovarian cancer is often associated with poor survival given that most patients are diagnosed at advanced stages when treatment is less effective. The disease is also very heterogeneous, consisting of various histologic subtypes that each have distinct epidemiologic and pathologic profiles. Given that some studies have noted ethnic-specific disparities in ovarian cancer survival when the Asian American population is disaggregated and certain tumor characteristics are associated with more favorable outcomes, we examined the relationship between Asian ethnic subgroup and ovarian cancer disease presentation. Methods: We included a total of 31,230 non-Hispanic White and Asian/Pacific Islander ovarian cancer cases diagnosed between 1996 and 2020 from the California Cancer Registry. Multivariable logistic regression with odds ratios (ORs) and 95% confidence intervals (CIs) was used to examine the likelihood of being diagnosed with tumors of a certain stage, grade, and histologic subtype, for each Asian ethnic subgroup compared to non-Hispanic Whites. Demographic and socioeconomic factors were considered in the analyses. Results: Most, but not all, Asian ethnic subgroups were less likely to be diagnosed with advanced stage ovarian cancer and more likely to be diagnosed with clear cell ovarian cancer compared to non-Hispanic Whites. For South East Asians, the odds of having advanced stage tumors were 43% lower than non-Hispanic Whites (OR=0.57, 95% CI 0.41-0.79) and the odds of having clear cell tumors were almost 200% higher than non-Hispanic Whites (OR=2.97, 95% CI 1.92-4.61); only South Asians did not show lower odds of an advanced stage tumor diagnosis (OR=1.06, 95% CI 0.86-1.29) and higher odds of a clear cell tumor diagnosis (OR=0.93, 95% CI 0.63-1.37). Conclusion: There are clear differences in ovarian cancer disease presentation when each Asian ethnic subgroup is examined separately with South Asians constituting the only subgroup who did not show a lower likelihood of an advanced stage diagnosis and a higher likelihood of a clear cell diagnosis when compared to non-Hispanic Whites. Given that tumor stage and histologic subtype are considered important prognostic factors for ovarian cancer, identifying the underlying factors that may contribute to being diagnosed with certain tumor characteristics could provide clues to help us improve the survival of this highly fatal malignancy. Abstract:Key PointsQuestionWhat are the differences in breast cancer between the East and the West? FindingsIn this review of breast cancer in East Asia, a greater proportion of breast cancers occur in younger women, and a higher frequency of luminal B subtype and TP53 mutations and a more immune-active microenvironment have been reported than in the West. The spectra of germline mutations and single-nucleotide polymorphisms contributing to breast cancer risk vary with ethnicity. Differences in tolerability of certain cytotoxic and targeted agents and other phenotypic characteristics may also influence treatment and imaging considerations. MeaningFurther research on ethnic diversity related to breast cancer is needed to help refine prevention, diagnostic, and management strategies. This review examines the differences in the epidemiology, tumor and host biology, and treatment of breast cancer in the East compared with the West. ImportanceDuring the past few decades, the incidence of breast cancer (BC) has been increasing rapidly in East Asia, and BC is currently the most common cancer in several countries. The rising incidence is likely related to changing lifestyle and environmental factors in addition to the increase in early diagnosis with BC awareness and screening. The understanding and management of BC are generally based on research and data from the West. However, emerging differences in BC epidemiology and tumor and host biology in Asian populations may be clinically relevant. ObservationsA higher proportion of premenopausal BCs occur in Asia, although this factor is possibly an age-cohort effect. Although the relative frequencies of different immunohistochemical subtypes of BC may be similar between the East and West, the higher prevalence of luminal B subtypes with more frequent mutations in TP53 may be confounded by disparities in early detection. In addition, Asian BCs appear to harbor a more immune-active microenvironment than BCs in the West. The spectra of germline mutations in BC predisposition genes and single-nucleotide polymorphisms contributing to BC risk vary with ethnicity as well. Differences in tolerability of certain cytotoxic and targeted agents used in BC treatment may be associated with pharmacogenomic factors, whereas the lower body mass of the average woman in East Asia may contribute to higher toxicities from drugs administered at fixed doses. Phenotypic characteristics, such as lower breast volume, may influence the type of surgery performed in East Asian women. On the other hand, increased breast density may affect the sensitivity of mammography in detecting BCs, limiting the benefits of screening mammography. Conclusions and RelevanceBreast cancer has become a major health problem in Asia. The inclusion of more women from Asia in clinical trials and epidemiologic and translational studies may help unravel the interethnic heterogeneity of BCs and elucidate the complex interplay between environmental and intrinsic factors in its pathogenesis. These insights may help to refine prevention, diagnosis, and management strategies for BC in the setting of ethnic diversity.

Abstract:Colorectal cancer (CRC) is the fourth most common cancer diagnosed in the United States (U.S.). This disease presents notable health disparities among racial and ethnic minorities in the U.S. Native Americans (NA) and non-Hispanic Blacks (NHB) exhibit the highest incidence and mortality rates for CRC compared to non-Hispanic Whites (NHW). These disparities are driven by a combination of factors, including socioeconomic inequalities, access to healthcare, lifestyle habits, and genetic susceptibilities. Minority populations often face delays in diagnosis and treatment, leading to advanced disease stages at presentation and poorer outcomes. Addressing these disparities through cancer genomics is essential for developing effective, personalized treatments that can improve outcomes for underrepresented populations. Comprehensive genomic studies can reveal genetic mutations and biomarkers unique to these groups, facilitating targeted therapies and preventive measures tailored to their specific needs. To address these disparities, we conducted an analysis of U.S. population-based colorectal cancer studies within 3 cancer genomic databases: the National Cancer Institute's Genomic Data Commons, the AACR Project GENIE, and cBioPortal. We identified 13 studies suitable for inclusion, calculating incidence percentages across all racial and ethnic categories. Our analysis encompassed a total of 20,295 patients. Among the analyzed patients, the racial distribution was as follows: NHW (68.5%), Asians (5.30%), NHB (7.7%), Other (5.6%), NA (0.2%), Pacific Islander/Native Hawaiian (0.07%), and not reported (12.7%). Ethnic data was missing for 24.1% (n=4,895) of the patients. Samples were predominantly from individuals identified as Not Hispanic (68.4%), while Hispanic individuals constituted 7.5%. Our results show significant underrepresentation of NHB, NA and Hispanic patients in CRC genomic databases highlighting a disparity between expected and actual representation. These findings underscore the critical need for inclusive research practices. Addressing these disparities in cancer genomics is imperative to develop equitable, effective treatments and improve health outcomes for all populations. Ensuring diverse representation in genomic databases will enhance the understanding of genetic differences across populations, inform targeted therapies, and ultimately contribute to reducing CRC disparities. By addressing these disparities, we can move towards a future where all populations benefit equally from advancements in CRC treatment and prevention, thus improving survival rates and quality of life for minority groups.
