Abstract:BACKGROUND The aim of this study was to quantitatively analyze the psychosocial characteristics of in vitro fertilization-embryo transfer (IVF-ET) couples and normal couples, and to identify the influencing factors of psychological characteristics and pregnancy outcomes. MATERIAL AND METHODS There were 260 infertile couples undergoing IVF-ET and 277 healthy couples of childbearing age in Shengjing Hospital of China Medical University recruited into 2 groups. Psychosocial characteristics were compared to analyze the influencing factors of pregnancy outcomes after IVF-ET. In-depth interviews (n=11) and infertility-related forum posts (n=12) were adopted to obtain the data related to the psychological experience and adjustment. Nvivo 11 software was utilized to collect and analyze the data. RESULTS The levels of anxiety and depression in the IVF-ET group were significantly higher (both P<0.01), the total scores of marital quality scale and social support scale were significantly lower (both P<0.05), immature defense mechanism score was significantly higher (P<0.05), and mature defense mechanism score (P<0.05) was significantly lower than those in the control group. Bod mass index (BMI), family's rural residence, marital quality, and immature and mature defense mechanisms were influencing factors of depression in IVF-ET female patients (all P<0.05). Marital quality, mature and immature defense mechanisms, concealment factors, and Harm Avoidance (HA) score were influencing factors of depression in IVF-ET males (all P<0.05). The age of the female patient was an independent influencing factor of IVF-ET pregnancy success rate (P<0.01). CONCLUSIONS The mental health levels of IVF-ET patients were worse than those of fertile couples. The younger the female infertile patient, the higher pregnancy rate of IVF-ET.

Abstract:Objective:To investigate the effect of Mindfulness-based stress reduction (MBSR) on perceived stress and mental state of patients with in vitro fertilization.Methods:83 cases women who were assisted reproduction with in vitro fertilization and embryo transfer (IVF-ET) in our hospital from May 2014 to December 2016 were selected as research objects.All the subjects were divided into control group and observation group according to the random number table method,with 41 cases in the control group and 42 cases in the observation group.Patients in the control group were given routine comprehensive nursing intervention,while the observation group was given MBSR intervention on the basis of the control group.The scores of self rating depression scale (SDS),self rating anxiety scale (SAS) and Chinese perceived stress scale (CPSS) before and after the intervention in the two groups were compared.Results:There was no significant difference in SDS score,SAS score and CPSS score between the two groups before intervention (P＞0.05).The scores of SDS,SAS and CPSS in observation group after intervention were (43.53± 4.89),(42.38± 4.34) and (27.53± 4.89),which were lower than (57.96± 7.30),(56.42± 5.49),(37.96± 7.30) before intervention,and the differences were statistically significant (P＜0.05).The scores ofSDS,SAS and CPSS in control group after intervention were (51.13± 5.52),(51.13± 5.52) and (31.13± 5.52),which were lower than (58.83± 7.28),(56.45± 5.51),(38.83± 7.28) before intervention,and the differences were statistically significant(P＜0.05).And those scores in were lower in observation group than in control group,the differences were statistically significant (P＜0.05).Conclusion:MBSR intervention can effectively improve the anxiety,depression and perceived stress of patients with in vitro fertilization,which is helpful in improving the success rate of IVF-ET,and it is worthy of clinical application.

Abstract:OBJECTIVE China was categorized as one of the 68 "countdown" countries to achieve the United Nations Millennium Development Goals (MDG) 5. This paper aimed to analyze the situation of maternal survival, and coverage of proven cost effective interventions in China, where specific attention was paid to disparities. METHODS National maternal and child mortality surveillance data were used to estimate maternal mortality ratio (MMR). Coverage for proven interventions was analyzed based on National Health Services Survey, where experts' consultations were made for complementation. RESULTS There had been a significant reduction of MMR in China, however great disparities existed, with rural II to IV areas experiencing 2 to 5 times higher maternal mortality risks than urban areas and accounting for over 70% maternal mortality burdens. Postpartum hemorrhage, pregnancy associated hypertension, embolism and sepsis were the leading causes, and over 75% of the maternal mortality was caused by preventable or curable causes. Maternal health services utilization decreased in accordance with region's development level. Socioeconomic factors like financial difficulties were the main obstacles hindering access of care. Even those who made deliveries in hospitals faced different probabilities in receiving qualified care according to their socioeconomic standings. CONCLUSION China is on track to achieve MDG 5, however great disparities exist. It is necessary to specifically target rural types II to IV areas. Major causes of maternal mortality which can be prevented or averted through the provision of essential obstetrical care. Yet as compared with maternity health needs, insufficient coverage of maternal and child health (MCH) care services and poor service quality are the leading predisposing factors contributing to maternal mortality in China.
Investigation on the psychological status of 803 infertile couples entering the IVF-ET treatment cycle

Abstract:Objectve To investigate the psychological status of infertile couples entering the in vitro fertilization-embryo transfer (IVF-ET) treatment cycle. Methods:In this cross-sectional study, convenience sampling method was used to selected couples who received IVF-ET treatment for the first time in the Reproductive Medicine Center of Peking University Third Hospital from October 2019 to August 2020. We used the basic questionnaire of infertility designed by ourselves, fertility problem inventory, the Chinese version of the fertility quality of life to evaluate the Status-Trait anxiety scale.Results:Totally 803 couples were included. The fertility stress score was 155.65 ±22.07 in female and 153.70 ±22.25 in male with no statistical difference ( P=0.101). The quality of life score was 90.60 ±18.27 in female and 84.24 ±16.47 in male. Female had better quality of life than male ( P<0.001). No statistical difference was observed in state anxiety scores ( P=0.074). The male trait anxiety score (43.38 ±6.91) was higher than that in female (41.93 ±6.89) ( P<0.001). Conclusion:When entering the IVF-ET treatment cycle, the fertility pressure level of the couple is the same, but the level of male anxiety characteristics is higher than that in women, and the quality of life is lower than that in women. Medical staff should strengthen the attention and intervention to the psychological state of men in the management of marital treatment.

Abstract:To evaluate the effects of high progesterone prior to oocyte retrieval on the genomic profile of peri-implantation endometrium, we conducted this single-center, prospective cohort study. Depending on whether or not the progesterone level on the day of hCG administration and the day after hCG administration were elevated, a total of 20 women undergoing IVF treatment who did not have fresh embryo transfer were included: Group 1 refers to subjects with normal progesterone level on both days; Group 2 refers to subjects with normal progesterone level on the day of hCG administration and high progesterone level on the day after hCG administration; Group 3 refers to subjects with high progesterone level on the day of hCG administration and normal progesterone level on the day after hCG administration; Group 4 refers to subjects with high progesterone level on both days. Five subjects were included in each group. Endometrial samples were obtained 7days after hCG administration. We found that high progesterone level prior to oocyte retrieval predominantly affected components of the NK cell mediated cytotoxicity pathway in the endometrium and that significant differences were only seen when progesterone measurements on both the day of and day after hCG administration were considered together.

Abstract:Study question: What are the risk factors and impacts of anxiety in women and men in heterosexual couples undergoing IVF as part of a randomised trial, with a delay in embryo transfer in one arm? Summary answer: Duration of infertility, ethnicity, and male partner's anxiety levels were associated with women's anxiety at the start of treatment, while initial anxiety score, partner's anxiety score at embryo transfer, ethnicity, and clinic location were associated with women's anxiety levels at embryo transfer; although women undergoing IVF were more anxious than their partners for slightly different reasons, their self-reported state anxiety was not associated with achieving clinical pregnancy, nor with switching from delayed frozen embryo transfer to fresh embryo transfer in an IVF trial. What is known already: Use of IVF treatment continues to rise and patients undergoing IVF are anxious. Participating in a randomised controlled trial (RCT) with uncertainty of arm randomisation might increase their anxiety, while a delay in treatment may add further to anxiety. Study design size duration: A mixed methods study was conducted using data from the multi-centre E-Freeze RCT cohort conducted across 13 clinics in the UK from 2016 to 2019. A regression analysis on anxiety scores of couples undergoing the IVF trial and a qualitative analysis of participant questionnaires were performed. Participants/materials setting methods: Six hundred and four couples participating in the E-Freeze trial, who had at least one useable State-Trait Anxiety Inventory (STAI) State Anxiety subscale (STAI-S) standardised self-report questionnaire for at least one of the partners, were included in the study. STAI-S scores were measured at consent for trial (T1) and again at embryo transfer (T2). Linear and log-binomial regression were used to explore the association between characteristics and STAI-S scores, and the associations between STAI-S scores and non-compliance and clinical pregnancy, respectively. Responses to the open text question were qualitatively analysed inductively using content analysis. Main results and the role of chance: Women's STAI-S scores at T1 (consent) were associated with their ethnicity, duration of infertility, and their male partner's STAI-S score at T1. Women's STAI-S scores at T2 (embryo transfer) were associated with their ethnicity, location of fertility clinic, their STAI-S score at consent, and their male partner's STAI-S score at embryo transfer. The adjusted coefficient (95% CI) for women's STAI-S scores at T2 was -4.75 (-7.29, -2.20, P < 0.001) for ethnic minority versus White, -2.87 (-4.85, -0.89, P = 0.005) for Scotland versus England, 0.47 (0.37, 0.56, P < 0.001) for each point increase in their own score at T1, and 0.30 (0.21, 0.40, P < 0.001) for each point increase in their male partner's score at T2. On average, women had higher STAI-S scores than men at both time points, and a larger increase of scores between the two time points. However, women's STAI-S scores were not associated with either non-compliance with trial allocation in the 'freeze-all' trial arm, or with chances of pregnancy. Both partners, but particularly women, described feeling anxious about the outcome of IVF, with women carrying the added worry of believing that feeling stressed might itself affect the outcome. Participants highlighted the important role of support from staff in helping them to manage their anxiety. Limitations reasons for caution: Data were not available on education level or social support, which might influence anxiety scores. Men's baseline characteristics were not collected. Wider implications of the findings: Identifying couples at increased risk of emotional distress may be improved by using standardised anxiety measures at the start of the fertility treatment. Women can be reassured that their self-reported state anxiety does not affect their chances of achieving clinical pregnancy through IVF, and this may help to reduce anxiety levels. The psychological wellbeing and experiences of couples undergoing IVF could be supported by patient-centred care: making information about the whole process of treatment and choices available to both partners in accessible formats; ensuring interactions with staff are kind and supportive; and acknowledging and addressing the different concerns of women undergoing IVF and their partners. Effects of group psychotherapy on mood in patients undergoing in vitro fertilization and embryo transfer

Abstract:Objective:To evaluate the effects of the group psychotherapy to the mood in women undergoing in vitro fertilization and embryo transfer(IVF-ET).Methods:Totally 100 women undergoing their IVF-ET treatment agreed and were divided into the routine-care control group(n=50)and the intervention group randomly(n=50).The intervention group(withdrawal 8)received group therapy which was performed about twice a week for 3 consecutive weeks(6 sessions),while the control group(withdrawal 3)did not receive any intervention.Both groups were assessed with the State-Trait Anxiety Inventory(SAI)and Self-Rating Depression Scale(SDS)before and after treatment.Results:The intervention group had not significant change in SAI and SDS scores after intervention(Ps>0.05).The SAI [(39.6±7.9)vs.(42.7±8.7),P<0.05] and SDS [(43.8±10.1)vs.(49.0±10.4),P<0.01] scores of the control group were significantly increased.Furthermore,the difference scores of SAI and SDS between post-test and pre-test in the two groups were compared.The difference scores of SAI were significantly higher in control group than in intervention group[(3.2±9.9)vs.(0.6±7.8),P<0.05].But for the difference scores of SDS,no statistically significant differences were found between the two groups(P>0.05).Compared with the control group,although there was a trend of a higher pregnancy rate in the intervention group,no significant group differences were found(35.7% vs.29.8%,P>0.05).Conclusion:The group psychological intervention is probably useful in alleviating anxiety level of women undergoing in vitro fertilization and embryo transfer treatment.

Abstract:Introduction: It is recognized that all over the world, infertile women undergoing in vitro fertilization (IVF) experience high levels of stress, but most remain undiagnosed and untreated. This study explored whether transcutaneous electrical acupoint stimulation (TEAS) could relieve anxiety levels in infertile women. Methods: A total of 360 infertile women with tubal blockage were randomized into four groups: control group, TEAS-2 Hz group, TEAS-100 Hz group and TEAS-2/100 Hz group. After excluding patients who did not complete embryo transfer, 84, 84, 80 and 86 cases respectively remained in the above groups. Anxiety levels were measured using the Spielberger's State Trait Anxiety Inventory (STAI) and the Amsterdam Preoperative Anxiety and Information Scale (APAIS). Results: We found that 2/100 Hz TEAS treatment significantly decreased the levels of state anxiety and preoperative anxiety (P < 0.05). In addition, there were indications that a100 Hz TEAS treatment could decrease the scores of APAIS. Conclusions: TEAS using a frequency of 2/100 Hz could help to alleviate state anxiety and preoperative anxiety levels.

Among them, 68 patients with severe oligoasthenospermia who used donor assisted pregnancy after ICSI failure were used as the study group. Sixty-eight patients with severe oligoasthenospermia who continued to receive ICSI assisted pregnancy were matched by propensity score 1:1 as control group. General data questionnaire, self-rating Anxiety Scale (SAS), self-rating Depression Scale (SDS), Chinese version of perceived Stress Scale (CPSS) and International Erectile Function Index Rating Scale (IIEF-5) were used to conduct a cross-sectional investigation to compare the mental health and erectile function status of the two groups. Results: There were statistical differences in age and occupational status between the two groups before PSM (P 0.05). The incidence of anxiety, depression, stress and erectile dysfunction (ED) in the study group was higher than that in the control group, and there were statistical differences between the two groups (P < 0.05). Conclusion: The use of donor sperm to assist pregnancy has a great impact on the mental health and erectile function of infertile men. Medical personnel should pay attention to the mental health and erectile function of these men, and formulate targeted measures to help these patients actively cope with infertility, so as to improve the life and marriage quality of this group and improve their mental health status. Abstract:Introduction: Although effects of acupuncture on pregnancy outcomes among women undergoing in vitro fertilization (IVF) have been analyzed Using systematic reviews and meta-analysis, few have been focused on psychological issues. The present systematic review and meta-analysis was designed to evaluate the efficacy of acupuncture on relieving anxiety and/or depression during IVF treatment. Methods: As of November 2020, randomized controlled trials (RCTs) and quasi-RCTs to observe the therapeutic effects of acupuncture used to relieve anxiety and/or depression during IVF treatment were identified from the following databases: Pubmed, Embase, the Cochrane Library, Web of Science; China National Knowledge Infrastructure, Wanfang, VIP and Chinese Biomedicine database. Study selection, data collection and quality assessment were carried out. Study characteristics, results on anxiety and/or depression relief, IVF outcomes and adverse events were summarized. Meta-analysis was performed using Review Manager 5.3 software. Results: Twelve studies with 2867 participants were included. A significant difference (3 studies, 547 participants, MD-9.26, 95%CI & minus; 12.01 to-6.51, P < 0.01) was observed for the relief of anxiety comparing acupuncture with controls during IVF treatment. No significant difference (9 studies, 1896 participants, RR = 1.30, 95%CI 1.03 to 1.64; P = 0.02) was found for clinical pregnancy rate. Only 2 out of 12 studies assessed remission of depression but there was no clear effect shown for acupuncture. Conclusions: Acupuncture may have a positive effect on anxiety relief during IVF treatment, while its effect on depression relief is inconclusive. More standardized, large-size, randomized and multicenter studies should be carried out on whether acupuncture can alleviate the anxiety and/or depression during IVF treatment.
A Study of Psychological Stress in Women Undergoing IVF-ET Treatment

Abstract:Objective Under the psychological stress framework,to investigate the changes of psychological stress before and during the IVF treatment,and the effects of psychological factors on IVF outcome. Methods We assessed factors of psychological stress before and during the IVF treatment with PSS,STAI,SDS and MCMQ. Results 324 women completed the questionnaires. The baseline SDS and PSS scores of 25.26±4.944 and 44.41±9.812,respectively were significantly higher than norm scores(P<0.05).The procedure SAI(F=21.688,P<0.01)and SDS score(F=4.938,P<0.01) had significantly differences,and both factors had an elevated trends among the four time points. Adjusting for general biomedical data of patients,neither baseline nor procedural state anxiety affected the ongoing pregnancy rates. The avoidance strategy was positively and significantly associated with pregnancy,but age and the resignation strategy were the relative risk factors for IVF outcome,with odds ratios of 1.124 (95% CI 1.033~1.222),0.944 (95% CI 0.895~0.996) and 0.903 (95% CI 0.827~0.987),respectively. Conclusion Before IVF treatment,patients had relatively higher depression level. The state anxiety and depression level increased during the course of the cycle,in which pregnancy test was the most stressful stages. The resignation coping may be a risk factor for pregnancy rates. The avoidance strategy may be beneficial during IVF treatment and is an recommend interventions aimed at promoting distraction strategies.

Abstract:BACKGROUND:Psychological distress may exert a negative influence on reproductive function of couples at reproductive age. Couples seeking assisted reproductive technology (ART) treatment may have a higher prevalence of psychological distress than fertile couples. However, whether psychological distress is associated with the outcome of ART treatment remains unknown. We aimed to investigate the association of pre-treatment psychological distress and clinical pregnancy rate among infertility couples undergoing in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) treatment.METHODS:This nested case-control study was conducted based on women who underwent their first fresh IVF or ICSI cycle in the Jiangsu Birth Cohort Study (JBC) between November 2015 and January 2019. A total of 150 women who did not obtain clinical pregnancy after first IVF or ICSI fresh embryo transfer were identified as cases, and a total of 300 age matched women who obtained clinical pregnancy were identified as controls. Conditional logistic regression analyses were used to investigate the association between psychological distress and the outcome of first IVF or ICSI treatment, adjusting for multiple potential confounders.RESULTS:No statistically significant association was observed between score of maternal symptoms of psychological distress and clinical pregnancy. Adjusted ORs of logistic regression were 1.00 (95% CI 0.97-1.03) for anxiety, 0.98 (95% CI 0.95-1.02) for depression, and 0.98 (95% CI 0.95-1.01) for perceived stress, respectively. When treat depression and anxiety as categorical variables, 62 (13.8%) were classified as clinical depression, 11 (2.4%) were classified as clinical anxiety, among 450 women in the present study. Psychological distress symptoms were also not associated with clinical pregnancy rate. Adjusted ORs of logistic regression were 0.27 (95% CI 0.03-2.33) for anxiety, 0.88 (95% CI 0.46-1.68) for depression, respectively.CONCLUSIONS:Our findings firstly indicated that psychological distress experienced prior to IVF/ICSI treatment was not associated with clinical pregnancy.

Abstract:This paper presents the research project IFUN Interest and Interdisciplinary Instruction in Science and Mathematics. As researchers in mathematics and science education we are basically interested in how students’ learning and understanding in these subjects might be improved. Many research studies indicate that students’ interest in science and mathematics declines during lower secondary school level (Hoffmann et al. 1998), but international surveys as well as other indicators do not indicate any general decrease of interest in science among the population in most countries (Sjøberg 2003). A simple assumption is that many young people find various aspects of science and mathematics interesting, but it is beyond the capacity of the school to catch and hold this interest. Until recently research in interest has not to any great extent addressed the relation between interest and education at the upper secondary level. The reason for this could be the assumption that students have developed relatively stable individual interest profiles during lower secondary school. According to Prenzel (1998) there are several factors that lead to questioning this assumption. The students are confronted with an abundance of topics, and although many of these do not correspond to the students’ individual interest they may lead to new areas of interest. At Danish upper secondary education the students’ student can choose between different subject packages and a number of optional subjects. The choices of students might among other things be based on interest. Also the students’ at upper secondary education have to make decisions about future education and profession. Personal interest is a key factor behind modern youth’s educational choice and career aspirations are used as indicators of interest in scientific fields (Gardner 1998). It is characteristic for research in students’ interests that the domains, which have been studied, are ones that belong to academically defined curricula with focus on a single subject, for instance physics or mathematics. In the light of current tendency towards increased interplay and integration between different subjects, interest research in the field of mathematics and science should also include broader and interdisciplinary domains that are associated with the applied, environmental, technological and socio-scientific content. It is the intention of the IFUN-project to fulfill these gaps in interest research. With focus on interdisciplinary domains of the disciplines mathematics, physics, chemistry, biology and technology the research questions of the IFUN-project are:
[Effects of family dignity intervention on male infertility patients and their spouses]

Abstract:Objective: To investigate the effects of family dignity intervention (FDI) on anxiety, depression, hope level and quality of life (QOL) of male infertility patients and their spouses. Methods: Using quasi-experimental design, we selected male infertility patients and their spouses undergoing human-assisted reproductive technology (ART) in our Center of Reproductive Medicine from June to December 2022 and divided them into an intervention group (38 couples) and a control group (40 couples). The former underwent a four-stage FDI, including ovulation promotion cycle assessment, family sharing, pre-transplantation interview and post-transplantation follow-up, while the latter received routine nursing. Using Hospital Anxiety and Depression Scale, Herth Hope Index and Fertility Quality of Life Scale, we evaluated the effects of FDI before and after transplantation. Results: After FDI, the anxiety and depression scores were significantly lower (P < 0.05) and the total scores on the hope level and all other dimensions remarkably higher in the intervention group than in the control (P < 0.05). The self-confidence of the couples in the intervention groups in ART treatment was markedly increased in comparison with that of the controls, and their scores on physical and mental health were significantly higher than those of the latter (P < 0.05). Conclusion: FDI can effectively relieve the anxiety and depression, raise the hope level and improve the quality of life of both male infertility patients and their spouses.

We enrolled 1247 infertile couples and assessed their anxiety and depression status before and during assisted reproductive technology (ART) treatment using the Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS). The Chi-square or fisher's exact test was used to analyze the prevalence of anxiety and depression in infertile couples. Multivariate logistical regression was performed to analyze the risk factors for anxiety and depression.

Abstract:Introduction: Dysregulation of the stress-regulatory hormone cortisol is associated with anxiety, but its potential impact on infertile women and in vitro fertilization (IVF) treatment remains unclear. This prospective cross-sectional study aimed at evaluating the dysregulation of cortisol and its correlation to anxiety in infertile women. The influence of stress on IVF outcomes was also investigated. Methods: A point-of-care test was used for the measurement of morning serum cortisol in 110 infertile women and 112 age-matching healthy individuals. A Self-Rating Anxiety Scale (SAS) was used for the anxiety assessment of infertile women, and 109 of them underwent IVF treatment starting with the GnRH-antagonist protocol. If clinical pregnancy was not achieved, more IVF cycles were conducted with adjusted protocols until the patients got pregnant or gave up. Results: Higher morning serum cortisol level was identified for infertile patients, especially for the elder. Women with no anxiety showed significant differences in cortisol levels, monthly income, and BMI compared with those with severe anxiety. A strong correlation was found between the morning cortisol level and the SAS score. When the cutoff value is 22.25 μg/dL, cortisol concentration could predict the onset of anxiety with high accuracy (95.45%) among infertile women. After IVF treatments, women with high SAS scores (>50) or cortisol levels (>22.25 μg/dL) demonstrated a lower rate of pregnancy (8.0%-10.3%) and more IVF cycles, although the impact of anxiety was not affirmative. Conclusion: Hypersecretion of cortisol related to anxiety was prevalent among infertile women, but the influence of anxiety on multi-cycle IVF treatment was not affirmative due to the complicated treatment procedures. This study suggested that the assessment of psychological disorders and stress hormone dysregulation should not be overlooked. An anxiety questionnaire and rapid cortisol test might be included in the treatment protocol to provide better medical care.

Abstract:Background Women undergoing infertility treatment have poor quality of life. This may cause them to withdraw from or refuse treatment. Women undergoing frozen embryo transfer have a treatment interval. The aim of this study was to investigate the status quo of the fertility quality of life in women undergoing frozen embryo transfer and analyse its predictors. Methods A cross-sectional survey was conducted from August 2019 to August 2020 among women undergoing frozen embryo transfer in a tertiary hospital reproductive centre in Beijing, China. The survey collected demographic characteristics and treatment data and included the fertility problem inventory, the fertility quality of life scale (FertiQoL) and the state-trait anxiety scale. Multiple linear stepwise regression was used to explore the predictors of fertility quality of life. Results In total, 1062 women completed the survey. Participants reported that they had high levels of fertility-related stress and anxiety during treatment. They also had lower fertility-related quality of life, and the Treatment FertiQoL scored the lowest. The regression results showed that social concern, trait anxiety, duration of treatment and age were risk factors for diminished fertility quality of life. Conclusion Chinese women undergoing frozen embryo transfer have relatively poor quality of life. The potential predictors of fertility quality of life include social concern, trait anxiety, duration of treatment and age.

Abstract:The study was to explore whether auricular acupressure (AA) can relieve anxiety during the period from trans-vaginal oocyte retrieval to the embryo transfer in IVF treatment and whether AA can improve the outcomes of IVF. 305 infertile patients with tubal blockage who were referred for IVF were included. The women were randomized into a control group with 102 cases, a Sham-AA group with 102 cases and an AA group with 101 cases. The anxiety levels were rated with Spielberger's State Trait Anxiety Inventory and the Amsterdam Preoperative Anxiety and Information Scale. Data of clinical pregnancy rate (CPR), implantation rate (IR) and live birth rate (LBR) were obtained. The levels of neuropeptide Y (NPY) and transforming growth factor alpha (TGF-alpha) in the follicular fluids were detected with ELISA. After treatment, in AA group, the levels of state anxiety, preoperative anxiety and need-for-information were significantly lower, whereas CPR, IR, LBR and NPY levels in the follicular fluids were markedly higher than Sham-AA group and control group. We concluded that AA could help to reduce anxiety levels associated with IVF and improves the outcomes of IVF partly through increasing the levels of NPY in the follicular fluids.

Identification of Psychological Symptom Clusters and Their Influencing Factors in Women Undergoing Assisted Reproductive Technology in China: a Cross-Sectional Study

Abstract:Background: There are multiple psychological symptoms in women undergoing assisted reproductive technology, which seriously affect health-related quality of life and even cause patients to stop treatment. Aim: This study aimed to identify psychological symptom clusters and their influencing factors in women undergoing assisted reproductive technology. Methods: A cross-sectional survey was conducted from June to November 2023 at the reproductive centers of Nanjing Women and Children's Healthcare Hospital. Data on demographic and clinical characteristics and Symptom Checklist-90 were collected. Exploratory factor analysis was performed to identify psychological symptom clusters. Univariate logistic regression analysis and multivariate logistic regression analysis were performed to explore influencing factors. Results: A total of 213 patients were recruited. The study found that the included participants scored higher on all SCL scales than the general Chinese females. The three most common were trouble remembering things (81.7%), feeling easily annoyed or irritated (81.2%), and feeling low in energy or slowed down (70.9%). Six symptom clusters were identified: paranoid ideation, depression, obsessive-compulsive disorder, interpersonal sensitivity, somatization, and sleep disorders. Multivariate logistic regression analysis showed that duration of infertility treatment (>12 months) was identified as a risk factor for sleep disorder cluster (OR=2.833, 95% CI:1.355~5.922), adverse pregnancy history was identified as a risk factor for paranoid ideation cluster (OR=2.961,95% CI:1.406~6.253), depression cluster (OR=2.404,95% CI:1.240~4.660), and obsessive-compulsive cluster (OR=1.810, 95% CI:1.016~3.233), financial burden during treatment was identified as risk factors for all symptom clusters[(OR=5.869, 95% CI:1.717~20.057),(OR=6.490,95% CI:2.210~19.063),(OR=3.034,95% CI:1.560~5.898),(OR=7.078,95% CI:2.420~20.698),(OR=4.532,95% CI:1.845~10.397),(OR=2.151,95% CI:1.129~4.098)]. Conclusion: Women undergoing ART experience various psychological symptoms that are interrelated and exist in the form of symptom clusters. More attention should be paid to the psychological status of patients with longer duration of infertility treatment, adverse pregnancy history, and financial burden during treatment. This study guides the development of targeted and effective psychological interventions to facilitate symptom management in women undergoing ART.
