Abstract:OBJECTIVE:To study the compliance of antihypertensive drugs and its related factors in inpatients and outpatients patients with hypertension.METHOD:Clinical data of 816 inpatients and 497 outpatients were reviewed to study the compliance of antihypertensive drugs and related factors as well as to investigate the utilization of anti-hypertensive medicament, the incidence of the side-effect of the medication and to explore the difference of the blood pressure control between the compliant group and the uncompliant group.RESULTS:The main risk factors of the hypertensive patients were in the order as: disorder of lipometabolism, smoking, left ventricular hypertrophy (LVH) and Diabetes mellitus. Sixty point nine percent of the inpatients were followed up. The incidences of compliance were 63.4% in the inpatients, 77.3% when being discharged, and 43.7% in the outpatients. The cause of uncompliance were as follows: side-effect of the medication, failure to control BP, financial problems, etc. Data gathered during follow up period, it was found the side effect of the calcium antagonist and the diuretics improved palpably. Those compliers controlled their BP well, while little fluctuation of BP was observed. However, the uncompliers showed different pictures, and the range of fluctuation of BP was large.CONCLUSIONS:The compliance of the medicament was best among the outpatients to the discharge order, and the compliance of the outpatients was the worst.
Treatment Adherence As a Mediator of Blood Pressure Control in Chinese Older Adults with Depression.

Abstract:ObjectivesBoth depression and hypertension (HTN) are prevalent, costly, and destructive, and frequently coexist among the aging population of China. This study aimed to examine the role that treatment adherence plays in blood pressure control in older adult Chinese with depression.MethodsData for these analyses were taken from a randomized control trial of a collaborative depression care management intervention conducted in rural villages of Zhejiang Province, China with older adults who had comorbid depression and HTN. They included baseline assessments of 2362 subjects ages ≥60 years, whose blood pressure and depression were measured using a calibrated manual sphygmomanometer and the Chinese version of the 17‐item Hamilton Depression Rating Scale (HDRS‐17), respectively. Treatment adherence was identified by a single question asking whether patients on occasion did not take their medicine.ResultsUncontrolled HTN was associated with older age (t = 3.10, P<0.01), higher HDRS‐17 score (t = 5.76, P<0.01), and higher rates of non‐adherence to HTN treatment (χ2 = 21.34, P<0.01). Logistic regression models indicated that adherence accounted for 39.4% of the total effect between depression and HTN. Specifically, those with poor adherence were at 1.417 greater odds of having their HTN uncontrolled compared with those with good adherence.ConclusionsHypertension control in older adults with depression is complicated by nonadherence to treatment. In addition to diagnosing and treating depression in their older adult patients, primary care physicians can optimize blood pressure control by identifying and addressing their patients' adherence to recommendations for HTN management.
Medication adherence and associated factors in newly diagnosed hypertensive patients in Japan: the LIFE study

Abstract:Hypertension is the leading cardiovascular risk factor worldwide. However, in Japan, only 30% of patients have their blood pressure controlled under 140/90 mmHg, and nonadherence to antihypertensives is thought to be a reason for the poor control of hypertension. We therefore sought to assess the adherence to hypertension treatment and to evaluate factors influencing patients' adherence in a large, representative sample of the Japanese population. To this end, we analyzed claims data from the LIFE Study database, which includes 112 506 Japanese adults with newly diagnosed hypertension. Medication adherence was measured for a year postdiagnosis using the proportion of days covered (PDC) method. Factors associated with adherence to antihypertensives were also assessed. Among the total 112 506 hypertensive patients, the nonadherence rate (PDC ≤ 80%) for antihypertensives during the first year after initiation of the treatment was 26.2%. Younger age [31–35 years: odds ratio (OR), 0.15; 95% confidence interval (95% CI), 0.12–0.19 compared with 71–74-year-old patients], male gender, monotherapy, and diuretics use [OR, 0.87; 95% CI, 0.82–0.91 compared with angiotensin II receptor blockers (ARBs)] were associated with poor adherence in the present study. Cancer comorbidity (OR, 0.84; 95% CI, 0.79–0.91 compared with no comorbidity), prescription at a hospital, and living in a medium-sized to regional city were also associated with poor adherence. Our present findings showing the current status of adherence to antihypertensive medications and its associated factors using claims data in Japan should help to improve adherence to antihypertensives and blood pressure control.

Physician and patient perspectives on hypertension management and factors associated with lifestyle modifications in Japan: results from an online survey

Abstract:Abstract We conducted a survey to examine the gaps between Japanese physician and patient perspectives on hypertension management and to investigate important factors that may help solve the “hypertension paradox” in Japan. Web-based surveys of patients and physicians were conducted in Japan between October 19 and 31, 2017. The data collected included physician and patient perspectives on hypertension education, adherence to lifestyle modifications and antihypertensive medication, and reasons for treatment adherence/nonadherence. Factors relating to specific patient behaviors (e.g., monitoring their home blood pressure [BP] daily) were analyzed by multivariate logistic regression analysis. Of the 541 physicians and 881 patients included in the analyses, both groups recognized that the extent of lifestyle changes was insufficient. Approximately 80% of physicians reported that they fully or sufficiently provided education to patients about reasons for hypertension treatment and its associated risks, target BP levels, and lifestyle modifications. Only 40–50% of patients considered those topics having been fully or sufficiently discussed. Logistic regression analyses revealed that positive lifestyle modifications (daily home BP monitoring, salt intake <6 g/day, and daily aerobic exercise for ≥30 min) were positively associated with receiving feedback from physicians about specific lifestyle modifications and patient motivation for maintaining their target BP. In conclusion, perception of the amount of education provided by physicians on hypertension management was lower in patients than in physicians. In addition to effective regular follow-up regarding lifestyle modifications, patient motivation by physicians is an important factor for improving lifestyle modifications and achieving effective hypertension management in Japan.

Abstract:The ideal management of a hypertensive patient include not only the pharmacological treatment but also a good compliance from the patient side. Some measures can be undertaken by the patient which can help to control or even reverse hypertension. Measures like decreasing weight, minimal or no salt intake, low carbs intake, physical exercise, etc. This article talks about a survey on patient compliance which was taken under Dr. Pravin Kahale in his clinic at Kokilaben Dhirubhai Ambani hospital, Mumbai (India). To understand the psychology of hypertensive patient regarding the disease of hypertension through a survey, a survey of 300 patient was done when they came to dr. Kahale's clinic for check-up/follow-up. Some survey forms were lled by doing home visits too. The questions in the survey highlighted the compliance of the patient towards the prescribed drugs. The questions are mentioned later in this article. There was a set of 17 questions which are discussed in the article along with their results But, vividly speaking hypertensive patients don't pay necessary attention when it comes to compliance. The data exclaimed that only 50% subjects were successful to decrease their body weight. Quite a good number of patients forget to take medicines on time. About one-third of our patients were frustrated to take medicines regularly. This study has revealed that a good number of people having hypertension are not compliant to their drug therapy. The article focuses on the importance of compliance in management of hypertension.
Current prescription status of antihypertensive drugs in Japanese patients with hypertension: Analysis by type of comorbidities

Abstract:In 2014, the Japanese Society of Hypertension (JSH) issued revised guidelines for hypertension management. To assess adherence to the guidelines, this retrospective study evaluated the real-world status of antihypertensive drug prescribing for Japanese patients with hypertension, classified by comorbidity: diabetes mellitus, dyslipidemia, gout/hyperuricemia and renal diseases. Data on 59,867 hypertensive patients who received their first prescription for antihypertensive therapy between April 2014 and March 2015, were obtained from a medical insurance claims database for hospitals participating in the Diagnosis Procedure Combination/Per-Diem payment system. The most common drugs prescribed for each comorbidity subgroup were calcium channel blockers (CCBs) and angiotensin II receptor blockers (ARBs), with prescription rates of around 60-70%. Contrary to JSH recommendations, diuretics and angiotensin-converting enzyme inhibitors were prescribed less often than β-blockers. Whereas diabetes mellitus is a compelling indication for use of renin-angiotensin system inhibitors, CCBs were commonly prescribed in this subgroup. The treatment pattern for patients with comorbid dyslipidemia closely resembled that for the overall patient population. Loop diuretics were prescribed more frequently for patients with renal diseases or gout/hyperuricemia than for those with diabetes mellitus or dyslipidemia. Although antihypertensive drug prescribing varied by comorbidity, JSH 2014 guidelines appeared not to be incorporated adequately into actual clinical practice.

Achievement rate of target blood pressure in patients with hypertension treated by hypertension specialists and non-specialists in a real-world setting

Abstract:Hypertension remains a major global healthcare issue. Considering that most Japanese patients with hypertension are managed by general practitioners, hypertension specialists should be involved in actual clinical practice. We investigated the blood pressure (BP), guidelines recommended for achievement rate of the target BP, and clinical variables of patients with hypertension treated by hypertension specialists and those treated by non-specialists in a real-world setting. Factors associated with the target BP achievement in this population were also investigated. Outpatients with hypertension from 12 medical facilities in Okinawa Prefecture were enrolled (n = 1469 [specialist group, 794; non-specialist group, 675]; mean age, 64.2 years; females, 45.8%). For all patients, BP and rate of the target BP achievement were 129.0 ± 15.5/74.6 ± 10.6 mmHg, and 51.8%, respectively. BP and the rate of target of BP achievement were 128.0 ± 15.1/73.4 ± 10.4 mmHg and 56.7% in the specialist group, and they were 130.1 ± 15.9/76.0 ± 10.8 mmHg and 46.1% in the non-specialist group. The urinary salt excretion and obesity rates were comparable between the specialist and non-specialist groups. Multivariable logistic analyses indicated that hypertension specialists and good medication adherence were positive factors, whereas obesity, chronic kidney disease, diabetes mellitus, and urinary salt excretion were inverse factors associated with target BP achievement in this population. Initiatives for salt reduction, medication adherence, and proper obesity management are crucial to improving BP management in patients with hypertension. Hypertension specialists are expected to play an essential role in them. For all patients, the target blood pressure (BP) achievement rate were 51.8%. Hypertension specialists and good medication adherence were positive factors in achieving target BP; conversely, obesity, diabetes mellitus, chronic kidney disease, and high urinary salt excretion were inverse factors in achieving target BP among patients with hypertension.

Abstract:Objective To collect and analyze the qualitative studies on patients' compliance with hypertension management in the community,and to summarize the factors influencing the compliance from the view of patients. Methods Eleven electronic databases and search strategies and words were determined through discussion by experts and the review group. After the selection and critical appraisal of the retrieved studies were performed by two reviewers independently,meta-summary was employed for the results of qualitative studies. Results Five studies were included. The results showed some factors had great impact on the compliance of patients,including patients' knowledge of hypertension and its treatment,doctor-patient relationship,patients' financial status,social support,designing of treatment plans,and patients' individual conditions. Conclusion To improve doctors' technical competence,building good doctor-patient relationship,strengthening patients' individualized health education,formulating the proper treatment plan,and making good use of the social support system would promote the compliance with hypertension management in the community.
Home blood pressure control and prescribing patterns of anti-hypertensive medications in a home blood pressure-based hypertension-specialized clinic in Japan: a sub-analysis of the Ohasama study

Abstract:Although the benefits of anti-hypertensive treatment are well known, the proportion of hypertensive patients with controlled blood pressure (BP) remains suboptimal. The present study aimed to compare BP control conditions in a hypertension-specialized clinic and non-hypertension-specialized clinics. This cross-sectional study used data from 379 treated patients who measured home BP in the Ohasama study between 2016 and 2019 (men: 43.0%, age: 71.6 years). Of those, 172 patients were managed at the hypertension-specialized clinic where physicians distributed home BP devices to each patient, evaluated the home BP data, and adjusted medications to maintain home BP values according to the recent Japanese guidelines. When we set morning home systolic/diastolic BP of <135/ < 85 mmHg as controlled BP, 93.6% of patients fulfilled the controlled home BP range, compared to 43.0% in non-specialized clinics ( n = 207). The proportion of the patients with home morning BP < 125/ < 75 mmHg was 73.3% in the hypertension-specialized clinic and 20.8% in the non-hypertension-specialized clinics. Hypertension-specialized clinics prescribed three or more anti-hypertensive drug classes to 41.9% of patients, compared to 15.2% in non-specialized clinics. In the hypertension-specialized clinic, angiotensin II receptor blockers were most commonly prescribed (86.6%), followed by dihydropyridine calcium channel blockers (77.9%), thiazide (including thiazide-like) diuretics (30.2%), mineralocorticoid receptor blockers (23.8%), and beta- and alpha-beta blockers (10.5%). In conclusion, the proportion of patients with controlled home BP was excellent in the hypertension-specialized clinic. Home BP-based hypertension practices, as recommended in the current Japanese guidelines, may be the key to achieving sufficient BP control.

[A Study on Relationship Between Compliance with Anti-Hypertension Therapy and Assessment of Quality of Health Care in Patients with Hypertension in Community].

Abstract:OBJECTIVE:To study the relationship between compliance with anti-hypertension therapy and assessment of quality of health care in hypertensive patients in community.METHODS:According to multi-stage cluster random sampling, we selected two communities from 36 streets in certain areas of Shanghai respectively using random number method on May, 2013, 72 communities in total, then we randomly selected 25 patients who were archived in the community health center from each community of the street.We conducted 1 800 questionnaires in total in which 1 172 patients were under medication study. The Morisky Medication Adherence Scale was used to evaluate medication compliance while the Patient Assessment of Chronic Illness Care Scale was applied to measuring the quality of patient's self-reported health service. The Spearman rank correlation analysis was used to evaluate the relationship between assessment of quality of health care for chronic illness and compliance with anti-hypertension therapy. Binary logistic analysis was applied to evaluate the factors which influenced patient's medicine compliance.RESULTS:There were 567(48.4%) respondents with good compliance. Age less than or equal to 65 accounted for 43.3% (245 people) while age greater than 65 accounted for 56.7% (321 people). The average scores of total and difference dimensions remained at a low level, ranging from 1.97 to 2.67 out of 5. The Assessment of quality of health care score was (2.67 ± 1.33)scores, in which the highest score of the dimensions was patient activation which was (2.56 ± 1.41)scores and the lowest was follow-up / collaboration which was (1.97 ± 1.49)scores. The Spearman rank correlation analysis showed that negative association between accumulate points of patient medicine compliance and patient activation was found (rs = -0.09, P < 0.01). The result of binary logistic analysis indicated that medication adherence of age greater than 65 was 1.49 times (95%CI: 1.11-2.00) higher than age less than or equal to 65. The patient activation was a protective factors of good medicine adherence (OR = 1.35, 95%CI:1.14-1.59).CONCLUSIONS:The medicine adherence of hypertension patients in community medication compliance is not very good at the moment. The self-rated quality of health care is still relative poor. Positive activation from interaction of physicians and patients can help enhancing patient's medicine compliance.

Abstract:Background: Low adherence to treatment is the most common cause of uncontrolled hypertension. Evidence suggests that illness perceptions and the physician-patient relationship may have a combined effect on treatment adherence. Aims: We investigated the roles of illness perceptions and the physician-patient relationship in medication and lifestyle modification adherence, and explained them using patients' experiences with essential hypertension.Method: In this mixed methods explanatory sequential study, we used questionnaires to assess illness perceptions, the physician-patient relationship, and adherence to medication and lifestyle modification guidance ( N = 112) in the first quantitative phase. Based on the results of regression analyses, the second qualitative phase was planned. We collected data from three patients through interviews, observations and journals, and conducted within-case and cross-case analyses. Finally, we integrated quantitative and qualitative findings. Results: The physician-patient relationship was an independent predictor of medication adherence (95% confidence interval [CI]: 0.29 to 5.89), whereas both illness perceptions (95% CI: −0.32 to −0.04) and the physician-patient relationship (95% CI: 1.05 to 9.48) were independent predictors of adherence to lifestyle modification guidance. Qualitative themes, reflecting the patient's experience of the concepts explored in this study, explained the quantitative results. Mixing methods revealed high consistency between quantitative and qualitative findings. Limitations: Limitations are the inability to infer causal associations among variables and indirect measurement of medication adherence. Conclusion: Shared decision-making on antihypertensive medications promotes medication adherence. Specifically, the illness perception, perceived illness controllability, and non-judgemental patients' acceptance within a satisfying physician-patient relationship empower patients to adhere to lifestyle modification guidance.

Abstract:Objective To survey the use of anti-hypertensive and to evaluate the compliance of patients at community. Method To interview patients at home with special questionnaire. Results There are 78.6% patients taking a kind of medicine in 973 patients. The five drugs of the most generally used,such as Zhengju in 34.12%,nifedipine in 23.12%, combination of drugs in 21.07%, captopril in 10.07%,metoprolol in 5.56%. The groups of the most commonly used medicine are compound preparation of antihypertension(58.97%), calcium antagonsis(34.05%), ACEI(12.82%). The compliance of taking one kinds of medicine group is better than taking two kinds medicine group(OR=1.72, 95%CI 1.13～2.72）. The compliance of sympathetic nerve blockers is significantly better than combined antihy pertensive drugs（OR=2.84，95%CI 1.18～6.92）. Conclusion:The compound medicine of antihypertension is mainly taken by patients in community, which is helpful to improve the compliance of patients. So some new long effect of compound drug must be developed in future.
Assessment of Compliance Status and Its Determinants Among Hypertensive Patients From County Areas in Zhejiang, China: A Cross‐Sectional Study

Abstract:Objectives Compliance is crucial for patients to control and manage their high blood pressure. This study assesses the compliance levels of hypertensive patients in China and explores the factors influencing compliance. Design A descriptive, cross‐sectional design was conducted. Sample A total of 371 hypertensive patients were recruited from six County hospitals and 12 township health centers in Zhejiang Province, China. Measure Patient compliance was measured using the Hypertensive Patient Scale (CHPS). Independent‐sample T‐test or variance analysis was applied to analyze CHPS scores by sociodemographic factors, and linear regression was used to explore the significant correlates of the total CHPS score. Results The total score of CHPS was 50.18 ± 6.12. Among the seven domains, drug treatment and lifestyle had the highest and lowest average scores, 3.59 ± 0.87 and 2.74 ± 0.73, respectively. The total score of CHPS positively associated with age (β = 0.075, p = 0.028), > 7 years of hypertension (β = 1.896, p = 0.022; Ref: < 3 years), and negatively associated with males (β = −2.224, p = 0.001; Ref: female) and rural area (β = −2.008, p = 0.007; Ref: urban area). Conclusion: These findings highlight the importance of related measurements of the local "health‐oriented" healthcare system. Primary health professionals should strengthen their health behavior intervention capacity and improve hypertension management among their patients.

Abstract:Non-compliance to treatment recommendations of patients with chronic diseases is a global medical problem that significantly affects the effectiveness of treatment, leading to serious medical and economic consequences. Sufficient research on the problem of non-compliance with medical recommendations has led to the creation of a holistic concept of medical compliance. The purpose of the work was to estimate the main factors influencing the compliance to antihypertensive therapy of middle age patients with Arterial hypertension. Material and methods. We investigated 40 patients 50-60 years old (mean age 56.6±4.5 years) with Essential Arterial hypertension. Socio-demographic, clinical, pathopsychological, psycho-diagnostic methods (Morisky Medication Adherence Scale, self-assessment anxiety scale Charles D. Spielberger – Y. L Hanin, the Scale of Internality in relation to health and illness, the study of the self-esteem of mental states by H. J. Eysenck, the study of the level of depression by the Beck Depression Inventory), mathematical and statistical methods were used. Results and discussion. With the Morisky Medication Adherence Scale survey, we distinguished three groups of patients: with high (22.5%), middle (27.5%) and low (50.0%) levels of adherence to the combined therapy of hypertension. Socio-demographic factors in patients with low adherence to treatment were determined by lower level of education and absence of continuous marital relations. Clinical characteristics of patients with low compliance level included the presence of 3rd degree of severity, I stage of the Arterial hypertension, often comorbidities (Diabetes Mellitus 2nd type, Chronic Obstructive Pulmonary Disease, Ischemic Heart disease), long tobacco smoking. According to the psychopathological and psycho-diagnostic examination, patients with an internal type of personality control, low anxiety and depression mostly showed the low level of compliance and also more frequent aggressiveness in the self-esteem of mental states (p <0.05). Conclusion. Thus, patients of middle age with Essential arterial hypertension very often (up to 78%) showed the insufficient level of adherence to antihypertensive treatment. Socio-demographic, clinical and psychopathological factors significantly affect the level of compliance in these patients. Improving the effectiveness of therapy of Arterial hypertension in such patients is possible due to optimization of treatment regimens, wide introduction of psycho-diagnosis and psycho-correction with the involvement of psychologists into this process

Abstract:Objective: Arterial hypertension (HTN) is responsible for the largest proportion of preventable mortality in the world. The primary cause of resistant hypertension (rHTN) is poor medication adherence. Blood or urine drug assays enable us to assess chemical adherence more accurately than self-reported questionnaires. We assayed anti-hypertensive drugs in patients with rHTN at our Center of Excellence in Hypertension, in collaboration with the Pharmacology and Toxicology Laboratory. Design and method: Patients with rHTN (office SBP/DBP >140/90 mmHg or out of office daytime >135/85 mmHg despite being prescribed 3 drugs at optimal dosage, including a diuretic, a renin angiotensin system inhibitor (RASI) and a calcium channel blocker (CCB), were informed of the study and agreed to have blood drugs concentrations measured. We describe the results for chemical adherence in the first 54 patients included between January 1, 2021 and September 1, 2023 in our center. Results: Of the 54 patients included, 38 (70%) were men, with a mean age of 65 +/- 11 years. The mean number of antihypertensive treatments prescribed was 5 +/- 1 (min 3, max 8). The proportions of treatments were RASI 87%, CCB (mainly dihydropyridines) 81%, diuretics 81%, beta-blockers 76%, alpha-blockers 57%, mineralocorticoid receptor antagonists (MRA) 37%, and central antihypertensives 26%. The best compliance was observed with alpha-blockers 81%, followed by CCB 50%, RASI 45%, diuretics 37% and beta-blockers 34%. The least compliant were central antihypertensives 0% and MRA 5%. All prescribed drugs were completely absent in the blood in 26% of patients, and 63% of patients had incomplete adherence (1 or more but not all drugs present in the blood), of whom 1/3 were non-compliant to 3 or more medications. Only for 9% of patients chemical adherence was excellent. Conclusions: Chemical adherence when measured at clinical visit was poor in this cohort of patients followed in a center of excellence. Limitations of this study are lack of follow-up for the same patient to assess variation in adherence over time, and small sample size. These results suggest the need to develop therapeutic education to improve patient involvement in the management of HTN and compliance.

Abstract:Nonadherence to antihypertensive drugs is a primary reason for suboptimal clinical outcomes among hypertensive patients. We assessed adherence to newly initiated antihypertensive medications in non-elderly Japanese patients and examined which patient and facility characteristics were associated with low adherence. We selected new oral antihypertensive drug users, aged 30–74 years, between 2014 and 2016 from a large administrative claims database. We measured adherence as the proportion of days covered (PDC) during a 1-year follow-up and divided patients into three groups of low (PDC < 40%), intermediate (PDC ≥ 40% to <80%), and high (PDC ≥ 80%) adherence. Factors associated with low adherence were assessed by logistic regression analysis with generalized estimating equations. Among 31,592 patients (mean age, 51.7 years; 41.2% female), the median 1-year PDC was 88.5% (IQR: 41.9–98.1%). In total, 59.2%, 16.6%, and 24.2% of patients were categorized as having high, intermediate, and low adherence, respectively. Female sex (odds ratio [OR] 1.15, 95% confidential interval [95% CI] 1.08–1.22), younger age, and the initiation of angiotensin-converting enzyme inhibitors (OR 1.37, 95% CI 1.12–1.66), beta-blockers and thiazide diuretics (OR 4.82, 95% CI 4.34–5.36 and OR 3.91, 95% CI 2.79–5.46, respectively; compared with angiotensin II receptor blockers) were associated with low adherence. Patients initiating antihypertensives at larger hospitals (≥200 beds) were more likely to be adherent. While adherence to antihypertensive drugs in non-elderly Japanese patients was relatively high compared with that reported in previous studies in Western countries, patients with intermediate-low adherence may benefit from targeted interventions.

Abstract:Background: Hypertension is a silent killer disease in the community. The success of hypertension therapy is influenced by adherence to medication. Three factors that shape compliance behaviour are knowledge and attitudes (these are predisposing factors) and family support (as a reinforcing factor). The majority of people in Indonesia choose healthcare centers when seeking hypertension treatment. Non-adherence to treatment has an impact on the incidence of complications. Objective: This current study was conducted on outpatients in the Tanggulangin Primary Healthcare Centre, Sidoarjo, Indonesia. Method: This study used a correlational design and a cross-sectional approach, involving 72 people in the sample. The variables measured were: knowledge, attitudes of patients, family support as well as patient compliance with hypertension treatment. Data was collected through a questionnaire instrument. Data analysis was done descriptively and correlatively. Results: The most widely accepted hypertension drug was amlodipine 78.6%, and a single therapy was given to 86.1% of patients. Most patients had suffered from hypertension for less than one year (38.9%); and half of the patients had family members who suffered from hypertension (52.1%). Patient knowledge of hypertension was moderate (69.4%). Most of the patients had a positive attitude (83.3%), good family support (76.5%), however, 94.5% did not comply with medication. The Spearman Rho test showed compliance with treatment was influenced by knowledge (5.9%) and attitude (14.8%). Meanwhile, the family support variable had no effect on compliance. Conclusion: This study concludes patient compliance to hypertension medication in the Tanggulangin Primary Healthcare Centre is only influenced by patient knowledge and attitudes.

This paper summarizes the improvement of compliance of patients with hypertension， analyzes the ideas and feasibility of assisting hypertension community prevention and control from three aspects： improving medication compliance， improving behavior compliance and digital intelligent management， and puts forward corresponding suggestions to improve the compliance of patients with hypertension.

Abstract:Adherence to antihypertensives is crucial for control of blood pressure. This study analyzed factors and interventions that could affect adherence to antihypertensives in the US. PubMed, Scopus, Web of Science, and Embase were searched on January 21, 2022 and December 25, 2023 for studies on the adherence to antihypertensives in the US. Nineteen studies and 23 545 747 patients were included in the analysis, which showed that adherence to antihypertensives was the highest among Whites (OR: 1.47, 95% CI 1.34–1.61 compared to African Americans). Employment status and sex were associated with insignificant differences in adherence rates. In contrast, marital status yielded a significant difference where unmarried patients demonstrated low adherence rates compared to married ones (OR: 0.8, 95% CI 0.67–0.95). On analysis of comorbidities, diabetic patients reported lower adherence to antihypertensives (OR: 0.95, 95% CI 0.92–0.97); furthermore, patients who did not have Alzheimer showed higher adherence rates. Different BMIs did not significantly affect the adherence rates. Patients without insurance reported significantly lower adherence rates than insured patients (OR: 3.93, 95% CI 3.43–4.51). Polypill users had higher adherence rates compared with the free‐dose combination (OR: 1.21, 95% CI 1.2–1.21), while telepharmacy did not prove to be as effective. Lower adherence rates were seen among African Americans, uninsured, or younger patients. Accordingly, interventions such as fixed‐dose combinations should be targeted at susceptible groups. Obesity and overweight did not affect the adherence to antihypertensives.
