Abstract:Background: Stillbirths and associated outcomes remain a significant concern in Sub-Saharan Africa (SSA), with approximately 44% of global stillbirths Methods: Using Demographic and Health Surveys (DHS) data, this study examined determinants of stillbirth among women in 29 SSA countries. Our cross-sectional analysis included a weighted sample of women 15-49 years of age who had given birth or experienced stillbirth. We used bivariate analyses and multilevel logistic regression approaches. Findings: Stillbirth prevalence was 319.06/1000 live births. Among individual-level factors, the risk increased with age. Higher maternal education levels were significantly associated with decreased stillbirth risk. Single women had significantly lower odds of stillbirth compared to those who no longer lived together/separated from their partner. Contextually, women with a job had an increased risk compared to women without a job, and living in a rural residential area was a significant factor. Interpretation: The complex interplay of individual-level factors and contextual factors influences stillbirth outcomes in SSA. Cross-sector holistic approaches to maternal and neonatal health are needed to address the multifaceted determinants of stillbirths
Epidemiology of intrapartum stillbirth and associated factors among women who gave childbirth in Ethiopia: systematic review and meta-analysis

Abstract:Background: Stillbirth always resulted in a multi-dimensional impact from the individual level to the country level at large. It causes psychological depression, social stigmatization, and decreased quality of life for women. Despite several studies conducted in Ethiopia, no national pooled estimates were done. Therefore, this systematic review and meta-analysis sought to assess intrapartum stillbirth and associated factors among women who had childbirth in Ethiopia using the available published evidence. Methods: The current review included studies conducted in Ethiopia. The databases used primarily were Medline/PubMed, Google Scholar, Scopus, Web of Science, Ethiopian University Repository Online, CINAHL, African Journals Online and Cochrane Library. All cross-sectional studies conducted in English and met eligibility criteria were included in the final review. A random-effects meta-analysis was performed. Data extraction and analysis were also performed using Microsoft Excel and STATA version 14 software respectively. Results: In the current review, eleven studies were included, and their quality was assessed before being chosen for the final review. The pooled prevalence of intrapartum stillbirth among women who had childbirth in Ethiopia was 9.21% [95% CI (7.03%, 11.39%); I 2 = 90.2, P = 0.000]. Women with a previous history of stillbirth [OR = 5.14, 95% CI (3.53-6.75), I 2 = 60.0%, p = 0.04] and had no use of antenatal care {[OR = 0.43, 95% CI (0.18-0.68) I 2 = 85.3%, p = 0.001]} were significantly associated with intrapartum stillbirth among women who gave childbirth. Conclusions: Nearly one-tenth of women who had childbirth in Ethiopia had an intrapartum SB. Revitalizing the existing health extension package particularly family health services with emphasis on focused antenatal care and counselling as well as with prompt referral system would reduce intrapartum SB. This review calls for the need to assess the quality of ANC provision and tailor targeted interventions to best improve the service quality.

Abstract:Background. The World Health Organization estimated that about 1.36 million pregnant women suffered from syphilis in 2008, and nearly 66% of adverse effects occurred in those who were not tested or treated. Syphilis infection is one of the most common maternal factors associated with stillbirth. Objective. This study aimed to determine the risk factors for stillbirth among pregnant women infected with syphilis. Methods. In this retrospective study, data on stillbirth and gestational syphilis from 2010 to 2016 were extracted from the prevention of mother-to-child transmission (PMTCT) program database in the Zhejiang province. A total of 8,724 pregnant women infected with syphilis were included. Multiple logistic regression analysis was performed to determine the degree of association between gestational syphilis and stillbirth. Results. We found that the stillbirth percentage among pregnant women infected with syphilis was 1.7% (152/8,724). Compared with live births, stillbirth was significantly associated with lower maternal age, not being married, lower gravidity, the history of syphilis, nonlatent syphilis stage, higher maternal serum titer for syphilis, inadequate treatment for syphilis, and later first antenatal care visit. In multiple logistic analysis, nonlatent syphilis (adjusted odds ratio (AOR) = 2.03; 95% CI = 1.17, 3.53) and maternal titers over 1 : 4 (AOR = 1.78; 95% CI = 1.25, 2.53) were risk factors for stillbirth, and adequate treatment was the only protective factor for stillbirth (AOR = 0.16; 95% CI = 0.10, 0.25). Conclusions. Nonlatent syphilis and maternal titers over 1 : 4 were risk factors for stillbirth, and adequate treatment was the only protective factor for stillbirth.
Causes of maternal mortality in Sub-Saharan Africa: A systematic review of studies published from 2015 to 2020

Abstract:BACKGROUND: Maternal deaths remain high in Sub-Saharan Africa (SSA) and their causes of maternal death must be analysed frequently in this region to guide interventions.METHODS: We conducted a systematic review of studies published from 2015 to 2020 that reported the causes of maternal deaths in 57 SSA countries. The objective was to identify the leading causes of maternal deaths using the international classification of disease - 10th revision, for maternal mortality (ICD-MM). We searched PubMed, WorldCat Discovery Libraries Worldwide (including Medline, Web of Science, LISTA and CNHAL databases), and Google Scholar databases and citations, using the search words "maternal mortality", "maternal death", "pregnancy-related death", "reproductive age mortality" and "causes" as MeSH terms or keywords. The last date of search from all databases was 21 May 2021. We included original research articles published in English and excluded articles that mentioned SSA country names without study results for those countries, studies that reported death from a single cause or assigned causes of death using computer models or incompletely broke down the causes of death. We exported, de-duplicated and screened the searches electronically in EndNote version 20. We selected the final articles by reading the titles, abstracts and full texts. Two authors searched the articles and assessed the risk of bias using a tool adapted from Montoya and others. Data from the articles were extracted onto an Excel worksheet and the deaths classified into ICD-MM groups. Proportions were calculated with 95% confidence intervals and compared for deaths attributed to each cause and ICD-MM group. We compared the results with WHO and Global Burden of Disease (GDB) estimates.RESULTS: We identified 38 studies that reported 11 427 maternal and four incidental deaths. Twenty-one of the third-eight studies were retrospective record reviews. The leading causes of death (proportions and 95% confidence intervals (CI)) were obstetric hemorrhage: 28.8% (95% CI = 26.5%-31.2%), hypertensive disorders in pregnancy: 22.1% (95% CI = 19.9%-24.2%), non-obstetric complications: 18.8% (95% CI = 16.4%-21.2%) and pregnancy-related infections: 11.5% (95% CI = 9.8%-13.2%). The studies reported few deaths of unknown/undetermined and incidental causes.CONCLUSIONS: Limitations of this review were the failure to access more data from government reports, but the study results compared well with WHO and GDB estimates. Obstetric hemorrhage, hypertensive disorders in pregnancy, non-obstetric complications, and pregnancy-related infections are the leading causes of maternal deaths in SSA. However, deaths from incidental causes are likely under-reported in this region. SSA countries must continue to invest in health information systems that collect and publishes comprehensive, quality, maternal death causes data. A publicly accessible repository of data sets and government reports for causes of maternal death will be helpful in future reviews. This review received no specific funding and was not registered.

Abstract:Background: Stillbirths are indicators of the quality of obstetrics care in health systems. Stillbirth rates and their associating factors vary by socio-economic and geographical settings. Published data on stillbirths and their associating factors in the Volta Region of Ghana are limited. This limits understanding of local factors that must be considered in designing appropriate interventions to mitigate the occurrence of stillbirths. This study determined the incidence of stillbirths and associated factors among deliveries at Ho Teaching Hospital (HTH) and contributes to understanding the consistent high stillbirths in the country and potentially in other low-resourced settings in sub-Saharan Africa. Method: This was a prospective cohort study involving pregnant women admitted for delivery at HTH between October 2019 and March 2020. Data on socio-demographic characteristics such as age and employment, obstetric factors including gestational age at delivery and delivery outcomes like birthweight were collected using a pretested structured questionnaire. The primary outcome was the incidence of stillbirths at the facility. Summary statistics were reported as frequencies, percentages and means. Logistic regression methods were used to assess for association between stillbirths and independent variables including age and birthweight. Odds ratios were reported with 95% confidence intervals and associations with p-values < 0.05 were considered statistically significant. Results: A total of 687 women and their 702 newborns contributed data for analysis. The mean age (SD) was 29.3 (6.3) years and close to two-thirds had had at least one delivery previously. Overall stillbirth incidence was 31.3 per 1000 births. Of the 22 stillbirths, 17 were antepartum. Pre-eclampsia was the most common hypertensive disorder of pregnancy observed (49.3%, 33/67). Among others, less than 3 antenatal visits and low birthweight increased the odds of stillbirths in the bivariate analysis. In the final multivariate model, pregnancy and delivery at 28-34 weeks gestation [AOR 9.37(95% CI 1.18-74.53); p = 0.034] and induction of labour [AOR 11.06 (95% CI 3.10-39.42); p < 0.001] remained significantly associated with stillbirths. Conclusion: Stillbirth incidence was 31.3 per 1000 births with more than half being antepartum stillbirths. Pregnancy/delivery at 28-34 weeks' gestation increased the odds of a stillbirth. Improving the quality of antenatal services, ensuring adherence to evidence-based protocols, accurate and prompt diagnosis and timely interventions of medical conditions in pregnancy particularly at 28-34 weeks' gestation could reduce incidence of stillbirths.

Abstract:Pulmonary alveolar proteinosis (PAP) is a rare disease characterized by the accumulation of lipoproteinaceous material within the alveoli. Though many studies addressed biochemical data in bronchoalveolar lavage (BAL) effluent, poor attention was payed to cell profile in PAP. We report the case of a 40 year old woman, presenting with progressive respiratory failure and bilateral pulmonary infiltrates. BAL and transbronchial biopsy confirmed PAP. Therapeutic whole lung lavage led to full clinical and radiological remission. Sequential BAL follow-up was started over a 4-year period. As previously reported, lymphocytes were increased prior to whole lung lavage. This pattern subsequently remained, whereas recurrence or superinfections did not occur. The mechanism of this disorder is unclear and deserves further investigation.

Abstract:Indicators of past wealth - 'old money' - typically bestow prestige and contribute to high status. Nobility, a culturally determined, hereditary status marker, might act as such an indicator, and thereby serve as a vehicle for the cultural transmission of economic standing. As an institution, nobility is an anachronism. Status, however, plays an important role in most societies, making individuals allocate valuable resources to status-enhancing activities. This suggests that even though nobility no longer entails formal privileges, it may continue to be coveted as a status marker. We examine the relative performance of nobility in the marriage market. Data on Swedish marriages provide an opportunity to test the hypothesis that the probability of hypergamy (marrying 'up') in terms of wealth increases when an individual belongs to the nobility. Our main finding is a significantly higher probability of hypergamy for members of the nobility, controlling for own wealth and other covariates. This 'nobility premium' is sizeable. The effect is statistically significant and robust to a number of different measures of hypergamy. This finding has implications for the intergenerational transmission of inequality, and for the longevity of the institution of nobility itself.
Independent and Cumulative Effects of Risk Factors Associated with Stillbirths in 50 Low- and Middle-Income Countries: A Multi-Country Cross-Sectional Study

Abstract:Background Early identification of high-risk pregnancies could reduce stillbirths, yet remains a challenge in low- and middle-income countries (LMICs). This study aims to estimate the associations between easily observable risk factors and stillbirths, and construct a risk score which could be adopted in LMICs to identify pregnancies with high risk of stillbirths. Methods Using the most recent Demographic and Health Surveys from 50 low- and middle-income countries (LMICs) with available data between January 1, 2010 and December 31, 2021, we analysed a total of 22 factors associated with stillbirths in a series of single-adjusted and mutually adjusted logistic regression models. Upon identification of the risk factors with the strongest associations, we constructed a risk score on the basis of the magnitude of the beta coefficient to examine the cumulative effects of risk factors on stillbirths. To assess whether the associations between risk scores and stillbirths were moderated by protective factors, we added an interaction term between the identified protective factor and risk scores to the regression model. We also conducted two sets of subgroup analyses for previous history of pregnancy and maternal age at pregnancy and four sets of supplementary analyses to test the robustness of the results. Findings Among the 795,642 women identified for analysis with at least one pregnancy within the five years before the survey, the most recent pregnancy of 8968 (1.13%) ended as stillbirths. Using a mutually adjusted regression model, we found that the top factors showing the strongest associations with stillbirths were short maternal height (odds ratio [OR]: 1.99, 95% confidence interval [CI]: 1.48-2.67, P < 0.001), interpregnancy interval less than six months (OR: 1.84, 95% CI: 1.42-2.38, P < 0.001), previous stillbirth history (OR: 1.55, 95% CI: 1.07-2.26, P < 0.020), low maternal education (OR: 1.50, 95% CI: 1.01-2.24, P = 0.045), and lowest household wealth (OR: 1.32, 95% CI: 1.08-1.61, P = 0.008). A female household head was a protective factor with an OR of 0.71 (95% CI: 0.55-0.90, P = 0.005). Single-adjusted models, subgroup analyses, and sensitivity analyses showed generally consistent results. We also found that the odds of stillbirths increased with a larger risk score with a P trend <0.001. Compared with women without any risk factors, women with a risk score of 5 or more were 4.11 (95% CI: 2.83-5.97, P < 0.001) times more likely to have their pregnancies ending up as stillbirths. However, these associations were weakened if the head of household was female. Interpretation Our study suggested that short maternal height, low socioeconomic status, previous stillbirth history, low maternal education, and very short interpregnancy interval had the strongest associations with stillbirths. The construction of risk scores using easily observable risk factors could be an effective way to identify high-risk pregnancies in resource-poor settings. Copyright (c) 2022 The Author(s). Abstract:Objective This study aimed to investigate the incidence of and risk factors for stillbirth in an Indian population. Methods We conducted a secondary data analysis of a hospital‐based cohort from the Maternal and Perinatal Health Research collaboration, India (MaatHRI), including pregnant women who gave birth between October 2018–September 2023. Data from 9823 singleton pregnancies recruited from 13 hospitals across six Indian states were included. Univariable and multivariable Poisson regression analysis were performed to examine the relationship between stillbirth and potential risk factors. Model prediction was assessed using the area under the receiver‐operating characteristic (AUROC) curve. Results There were 216 stillbirths (48 antepartum and 168 intrapartum) in the study population, representing an overall stillbirth rate of 22.0 per 1000 total births (95% confidence interval [CI]: 19.2–25.1). Modifiable risk factors for stillbirth were: receiving less than four antenatal check‐ups (adjusted relative risk [aRR]: 1.75, 95% CI: 1.25–2.47), not taking any iron and folic acid supplementation during pregnancy (aRR: 7.23, 95% CI: 2.12–45.33) and having severe anemia in the third trimester (aRR: 3.37, 95% CI: 1.97–6.11). Having pregnancy/fetal complications such as hypertensive disorders of pregnancy (aRR: 1.59, 95% CI: 1.03–2.36), preterm birth (aRR: 4.41, 95% CI: 3.21–6.08) and birth weight below the 10th percentile for gestational age (aRR: 1.35, 95% CI: 1.02–1.79) were also associated with an increased risk of stillbirth. Identified risk factors explained 78.2% (95% CI: 75.0%–81.4%) of the risk of stillbirth in the population. Conclusion Addressing potentially modifiable antenatal factors could reduce the risk of stillbirths in India.

Abstract:The coronary resistance, total heat production, oxygen consumption and isovolumic mechanical performance were measured simultaneously in the isolated beating rabbit heart. The direct effects of nitroglycerin (0.12 mg/l) were determined. The expected coronary dilatation occurred; coronary resistance fell by 21.5% independent of mechanical performance. There was a 14% fall in mechanical performance at the apex of the Frank-Starling curve and lesser reductions at less-than-apical diastolic volumes. Oxidative metabolism was not affected; there was no change in the calorific equivalent of oxygen (20.97 mJ/mul:O2) nor was there any significant anerobic metabolism. The ergonic cost of the force-independent metabolism which includes calcium activation energy was reduced by 17.7%. The unit metabolic cost of mechanical performance, or the performance-dependent metabolism, was unchanged resulting in a net increase in overall mechanical economy. It is concluded that it is against a background of a more metabolically efficient but less mechanically active myocardium that nitroglycerin exhibits its well known peripheral and reflex in the intact organism.

Abstract:Objective To assess stillbirth mortality by Robson ten‐group classification and the usefulness of this approach for understanding trends. Design Cross‐sectional study. Setting Prospectively collected perinatal e‐registry data from 16 hospitals in Benin, Malawi, Tanzania and Uganda. Population All women aged 13–49 years who gave birth to a live or stillborn baby weighting >1000 g between July 2021 and December 2022. Methods We compared stillbirth risk by Robson ten‐group classification, and across countries, and calculated proportional contributions to mortality. Main outcome measures Stillbirth mortality, defined as antepartum and intrapartum stillbirths. Results We included 80 663 babies born to 78 085 women; 3107 were stillborn. Stillbirth mortality by country were: 7.3% (Benin), 1.9% (Malawi), 1.6% (Tanzania) and 4.9% (Uganda). The largest contributor to stillbirths was Robson group 10 (preterm birth, 28.2%) followed by Robson group 3 (multipara with cephalic term singleton in spontaneous labour, 25.0%). The risk of dying was highest in births complicated by malpresentations, such as nullipara breech (11.0%), multipara breech (16.7%) and transverse/oblique lie (17.9%). Conclusions Our findings indicate that group 10 (preterm birth) and group 3 (multipara with cephalic term singleton in spontaneous labour) each contribute to a quarter of stillbirth mortality. High mortality risk was observed in births complicated by malpresentation, such as transverse lie or breech. The high mortality share of group 3 is unexpected, demanding case‐by‐case investigation. The high mortality rate observed for Robson groups 6–10 hints for a need to intensify actions to improve labour management, and the categorisation may support the regular review of labour progress.

Abstract:In patients with cystic fibrosis (CF), chronic colonization with a narrow but evolving spectrum of bacterial pathogens, leading to intermittent episodes of debilitating inflammatory exacerbations and progressive lung damage, are major influences on quality of life and life expectancy. Emerging pathogens also tend to be inherently resistant to available antibiotics depriving patients of effective antibiotic therapy. Thus infection control plays a critical role in the management of CF lung disease.

Abstract:Background: In Cambodia, stillbirths and their underlying factors have not been systematically studied. This study aimed to assess the proportion and trends in stillbirths between 2017 and 2020 in a large maternity referral hospital in the country and identify their key determinants to inform future prevention efforts. Methods: This was a retrospective cross-sectional analysis with a nested case-control study of women giving birth at the National Maternal and Child Health Centre (NMCHC) in Phnom Penh, 2017-2020. We calculated percentages of singleton births at ≥ 22 weeks' gestation resulting in stillbirth and annual stillbirth rates by timing: intrapartum (fresh) or antepartum (macerated). Multivariable logistic regression was used to explore factors associated with stillbirth, where cases were all women who gave birth to a singleton stillborn baby in the 4-year period. One singleton live birth immediately following each case served as an unmatched control. Multiple imputation was used to handle missing data for gestational age. Results: Between 2017 and 2020, 3.2% of singleton births ended in stillbirth (938/29,742). The stillbirth rate increased from 24.8 per 1000 births in 2017 to 38.1 per 1000 births in 2020, largely due to an increase in intrapartum stillbirth rates which rose from 18.8 to 27.4 per 1000 births in the same period. The case-control study included 938 cases (stillbirth) and 938 controls (livebirths). Factors independently associated with stillbirth were maternal age ≥ 35 years compared to < 20 years (aOR: 1.82, 95%CI: 1.39, 2.38), extreme (aOR: 3.29, 95%CI: 2.37, 4.55) or moderate (aOR: 2.45, 95%CI: 1.74, 3.46) prematurity compared with full term, and small-for-gestational age (SGA) (aOR: 2.32, 1.71, 3.14) compared to average size-for-age. Breech/transverse births had nearly four times greater odds of stillbirth (aOR: 3.84, 95%CI: 2.78, 5.29), while caesarean section reduced the odds by half compared with vaginal birth (aOR: 0.50, 95%CI: 0.39, 0.64). A history of abnormal vaginal discharge increased odds of stillbirth (aOR: 1.42, 95%CI: 1.11, 1.81) as did a history of stillbirth (aOR: 3.08, 95%CI: 1.5, 6.5). Conclusions: Stillbirth prevention in this maternity referral hospital in Cambodia requires strengthening preterm birth detection and management of SGA, intrapartum care, monitoring women with stillbirth history, management of breech births, and further investigation of high-risk referral cases.

Abstract:An estimated 2.6 million third trimester stillbirths occurred in 2015 (uncertainty range 2.4-3.0 million). The number of stillbirths has reduced more slowly than has maternal mortality or mortality in children younger than 5 years, which were explicitly targeted in the Millennium Development Goals. The Every Newborn Action Plan has the target of 12 or fewer stillbirths per 1000 births in every country by 2030. 94 mainly high-income countries and upper middle-income countries have already met this target, although with noticeable disparities. At least 56 countries, particularly in Africa and in areas affected by conflict, will have to more than double present progress to reach this target. Most (98%) stillbirths are in low-income and middle-income countries. Improved care at birth is essential to prevent 1.3 million (uncertainty range 1.2-1.6 million) intrapartum stillbirths, end preventable maternal and neonatal deaths, and improve child development. Estimates for stillbirth causation are impeded by various classification systems, but for 18 countries with reliable data, congenital abnormalities account for a median of only 7.4% of stillbirths. Many disorders associated with stillbirths are potentially modifiable and often coexist, such as maternal infections (population attributable fraction: malaria 8.0% and syphilis 7.7%), non-communicable diseases, nutrition and lifestyle factors (each about 10%), and maternal age older than 35 years (6.7%). Prolonged pregnancies contribute to 14.0% of stillbirths. Causal pathways for stillbirth frequently involve impaired placental function, either with fetal growth restriction or preterm labour, or both. Two-thirds of newborns have their births registered. However, less than 5% of neonatal deaths and even fewer stillbirths have death registration. Records and registrations of all births, stillbirths, neonatal, and maternal deaths in a health facility would substantially increase data availability. Improved data alone will not save lives but provide a way to target interventions to reach more than 7000 women every day worldwide who experience the reality of stillbirth.

Abstract:Introduction: Neonatal mortality during the first week of life is a global issue that is responsible for a large portion of deaths among children under the age of five. There are, however, very few reports about the issue in sub-Saharan Africa. For the sake of developing appropriate policies and initiatives that could aid in addressing the issue, it is important to study the prevalence of mortality during the early neonatal period and associated factors. Thus, the aim of this study was to ascertain the prevalence of and pinpoint the contributing factors to early neonatal mortality in sub-Saharan Africa. Method: Data from recent demographic and health surveys in sub-Saharan African countries was used for this study. The study included 262,763 live births in total. The determinants of early newborn mortality were identified using a multilevel mixed-effects logistic regression model. To determine the strength and significance of the association between outcome and explanatory variables, the adjusted odds ratio (AOR) at a 95% confidence interval (CI) was computed. Independent variables were deemed statistically significant when the p-value was less than the significance level (0.05). Result: Early neonatal mortality in sub-Saharan Africa was 22.94 deaths per 1,000 live births. It was found to be significantly associated with maternal age over 35 years (AOR = 1.77, 95% CI: 1.34-2.33), low birth weight (AOR = 3.27, 95% CI: 2.16, 4.94), less than four ANC visits (AOR = 1.12, 95% CI: 1.01, 1.33), delivery with caesarean section (AOR = 1.81, 95% CI: 1.30-2.5), not having any complications during pregnancy (AOR = 0.76, 95% CI: 0.61, 94), and community poverty (AOR = 1.32, 95% CI: 1.05-1.65). Conclusion: This study found that about twenty-three neonates out of one thousand live births died within the first week of life in sub-Saharan Africa. The age of mothers, birth weight, antenatal care service utilization, mode of delivery, multiple pregnancy, complications during pregnancy, and community poverty should be considered while designing policies and strategies targeting early neonatal mortality in sub-Saharan Africa.

This retrospective cohort study describes lifestyle and sociodemographic factors associated with stillbirth in South Australia (SA), between 1998 and 2016.

Abstract:A 10-year retrospective review was undertaken of patients presenting with "scorpion sting" to the Emergency Department of the King Fahad National Guard Hospital in Riyadh. The purpose of the study was 1) to assess the risk of morbidity and mortality following scorpion envenomation in the Riyadh area; 2) to define patient demographics of the study group and identify high risk groups for systemic toxicity; 3) to develop guidelines for patient management. Four hundred and thirty-four cases were studied. Local pain was the primary presenting complaint (92%). Systemic toxicity occurred in 25.6% (n=111). Anxiety-related presentations were the most common systemic symptoms (6%). Hypertension (17%) and/or tachycardia (4%) were the most common systemic signs. Only four patients (<1%) had signs of serious envenomation requiring admission. There were no deaths. The higher risk groups for systemic toxicity were identified as those with age less than eight years or greater than 30 years, with highest risk in those over 50 years, those with history of hypertension or diabetes, and patients with multiple stings. The great majority of patients can be treated with analgesia, local application of ice, and a period of observation dictated by clinical findings. The use of antivenin should be restricted to patients with signs of serious systemic toxicity only.
Rates and risk factors for antepartum and intrapartum stillbirths in 20 secondary hospitals in Imo state, Nigeria: A hospital-based case control study

Abstract:Despite Nigeria's stillbirth rate reducing from 28.6 to 22.5 per 1,000 births from 2000-2021, progress trails comparable indicators and regional variations persist. We assessed stillbirth incidences and associated risk factors in 20 secondary hospitals in Imo state, to generate essential local evidence to inform policymaking to reduce mortality. The total numbers of births and their outcomes were determined through hospital maternity registers. An unmatched case-control study was conducted. We collected retrospective data about 157 antepartum and 193 intrapartum stillbirths, and from 381 livebirths (controls). Potential risk factors were categorised into sociodemographic, obstetric and maternity care and biological determinants using a theoretical framework. Independent multivariable logistic regression models were used to investigate the association of risk factors with each stillbirth type. The overall stillbirth rate was 38 per 1,000 total births. The rate of antepartum and intrapartum stillbirths were 16 and 19 per 1,000 respectively. The risk factors independently associated with antepartum stillbirths were nulliparity (adjusted odds ratio (aOR) 1.87, 95%CI 1.04-3.36); preterm birth (aOR 14.29, 95%CI 6.31-32.38); being referred from another facility (aOR 3.75, 95%CI 1.96-7.17); unbooked pregnancy (aOR 2.58, 95%CI 1.37-4.85); and obstetric complications (aOR 4.04, 95%CI 2.35-6.94). For intrapartum stillbirths, associated factors were preterm birth (aOR 11.28, 95%CI 4.66-27.24); referral (aOR 2.50, 95%CI 1.19-5.24); not using a partogram (aOR 2.92, 95%CI 1.23-6.95) and obstetric complications (aOR 10.71, 95%CI 5.92-19.37). The findings highlight specific risk factors associated with antepartum and intrapartum stillbirths, shedding light on potential areas for targeted interventions.
