The Associations between Objectively Measured Physical Activity and Physical Function in Community-Dwelling Older Japanese Men and Women

Abstract:Objective: The relationships between physical activity (PA) and physical function (PF) among older Japanese adults have been examined before, with some studies reporting that moderate-to-vigorous physical activity (MVPA) is positively associated with PF. However, it is unclear whether the observed associations differ between men and women. In this study, we investigated the associations of objectively measured MVPA, light physical activity (LPA), and sedentary time (ST) with different PF levels in older Japanese men and women. Subjects and Methods: A total of 810 community-dwelling subjects aged 65–75 years were investigated (52.1% women, 47.9% men). The PF variables included grip strength, one-leg standing, usual and maximum walking speeds, and chair-standing time. PA (MVPA, LPA, and ST) and the number of steps taken daily were assessed for seven consecutive days by a tri-axial accelerometer. We determined the sex-specific quartiles of MVPA, LPA, and ST and analyzed their associations with physical function in separate models for each exposure measure. Results: In the crude analysis, MVPA was significantly associated with all PF variables in the men, and with the usual walking speed, max. walking speed, and chair-standing time in the women. Neither LPA nor ST was significantly associated with any PF variables. After adjusting potential confounding factors, significant associations between MVPA and usual walking speed remained in the men and women. Conclusions: Only greater moderate-to-vigorous physical activity (MVPA) was significantly associated with higher levels of PF variables in both men and women. Thus, time spent in moderate-to-vigorous physical activity (MVPA) can help older adults maintain or improve their physical function.

Differences in Accelerometer-Measured Physical Activity and Sedentary Behavior Between Middle-Aged Men and Women in Japan: A Compositional Data Analysis.

Abstract:Background : Differences in accelerometer-measured sedentary behavior and different physical activity (PA) intensities between men and women have been poorly described. The authors examined gender differences in time-use activity composition and total volume of PA. Methods : A cross-sectional mail survey was conducted from 2013 to 2015 with a randomized sample of 6000 middle-aged (40–64 y) community-dwelling Japanese adults living in urban and regional cities. Participants wore Active style Pro HJA-350IT on their waist for 7 consecutive days. Gender differences in activity time use was examined using compositional data analysis to control for time spent in all activity measures. Results : In total, 757 participants (303 men, 52.3 [7.1] y) with valid data were included in the analysis. Women spent on average 12.6% less time in sedentary behavior and 23.4% more time in light-intensity PA than men, whereas no significant difference was found for moderate to vigorous PA. Women accumulated a significantly greater volume of PA than men (17.8 vs 15.0 metabolic equivalent of task h/d). Conclusions : Japanese middle-aged women showed higher levels of PA than men because they spent more time in light-intensity PA. Given the health benefits of light-intensity PA, evaluating only moderate to vigorous PA may lead to an underestimation of women's participation in PA.

Normative Data of Objectively Measured Physical Activity and Sedentary Time in Community—Dwelling Older Japanese

Abstract:Background: The amounts of moderate-to-vigorous-intensity physical activity (MVPA), light-intensity physical activity (LPA), and sedentary time (ST) by sex, age, and body mass index (BMI) in older Japanese adults have not been known. We conducted this study to determine the actual physical activity (PA) and ST in this population. Subjects and Methods: A total of 3998 community-dwelling Japanese adults aged ≥65 years were investigated. Their levels of PA and ST and number of steps taken daily were assessed for seven consecutive days by a tri-axial accelerometer. Normative values of daily PA and ST were analyzed by age and BMI groups in the men and the women and are presented as mean, median, or decile. Results: The subjects generally adhered to the PA guideline, i.e., ≥10 metabolic equivalents (METs)·hour MVPA per week. Older age was associated with lower adherence to the PA guideline. Conclusions: Normative values (mean, median, or decile) were yielded for MVPA, LPA, and ST based on accelerometer readings in a large sample of older community-dwelling Japanese adults. One-half of the subjects’ waking time was spent being sedentary, and >70% of the subjects met the current PA guideline by engaging in MVPA.

Objective Measures of Physical Activity and Frailty in Ambulatory Adults Aged 85-89 Years in Kawasaki, Japan: A Cross-sectional Study

Abstract:Background: While physical activity (PA) can reduce physical frailty, the specific intensity and duration of PA required to prevent frailty are yet to be defined. Objectives: This study aimed to investigate the relationship between objectively measured PA or sedentary behavior (SB) and physical frailty and to explore the theoretical effect of replacing SB with different intensities of PA on the risk of frailty. Design: Cross-sectional study. Setting: Community-based assessments. Participants: Independently living adults aged 85-89 years participating in the Kawasaki Aging and Wellbeing Project. Measurements: PA was measured using a triaxial accelerometer for a minimum of 10 h per day over a minimum of 3 days. Physical frailty was measured using the revised Japanese version of the Cardiovascular Health Study criteria. Physical frailty was classified as non-frail, pre-frail, and frail. The relationship between PA and frailty was assessed using analysis of covariance. The effect of replacing SB with light-intensity PA (LPA) or moderate-to-vigorous-intensity PA (MVPA) on the risk of frailty was estimated using multinomial logistic regression and an isotemporal substitution model. Results: The analysis included 1004 participants (503 men and 501 women) of whom 242 were classified as frail. The mean step count, MVPA, and LPA were lower, and SB was higher in the frail group than in the non-frail group. SB replacement with MVPA for 10 min/day was associated with a significantly lower odds of frailty (odds ratio [95% confidence interval]: pre-frail men, 0.86 [0.77, 0.96]; frail men, 0.70 [0.60, 0.83]; pre-frail women, 0.77 [0.66, 0.90], frail women, 0.59 [0.47, 0.75]). SB replacement with LPA was not significantly associated with frailty odds in men or women. Conclusions: Replacing 10 min of SB with MVPA was associated with lower risk of frailty in adults aged 85-89 years. These findings require confirmation in longitudinal and intervention studies.

Abstract:Hyaluronic acid (HA) has a dose-related inhibiting effect on the migration and chemotaxis of polymorphonuclear leucocytes (PMN) in vitro. These effects were measured with a new indirect quantitative assay. On average 1 mg HA/ml causes an inhibition to about 80% of the control (spontaneous migration). This effect increased progressively with an increasing HA concentration, and with 4 mg HA/ml only about 19% of the PMN were able to migrate in the in vitro system. Similar results were obtained in the presence of a potent chemotactic factor (leukotriene B4 [LTB4]). In the mean 1 ng LTB4/ml alone stimulated the chemotaxis of PMN by a factor of 3 compared to the spontaneous migration. The highest HA concentration (4 mg/ml) reduced the number of migrating PMN cells to about 17%. From these experiments it may be concluded that the HA in the synovial fluid of the healthy joint has a protective effect against the invasion of PMN cells. This functions is disturbed in inflamed joints by the decrease in the HA concentration and possibly by its depolymerization. The intraarticular application of high molecular weight HA might be an important therapeutic regimen to restore the natural barrier against PMN migration, also in the presence of chemotactic factors and could therefore be helpful for interrupting the inflammatory cascade.

Abstract:[Purpose] The purpose of this study was to measure the physical activity of frail community-dwelling elderly using an accelerometer. [Participants and Methods] Thirty-two elderly persons (11 males and 21 females) who participated in a care prevention class were the subjects. The Mini Mental State Examination (MMSE), Life-Space Assessment (LSA), physical function (grip strength, 5 chair stand-ups, Timed Up and Go test (TUG), eyes open one leg standing time, 5-m maximum walking speed, and standing time from the long sitting position) and physical activity (PA) were evaluated, and standing, sitting, supine, sedentary behavior (SB), low, medium, and high intensity exercise times, and moderate-to-vigorous physical activity (MVPA) were measured using an accelerometer. [Results] Females showed significantly higher values for all physical function assessments except grip strength, significantly shorter sitting and SB times, and significantly longer exercise times in all intensity categories, and a negative correlation coefficient was found between their mobility ability including TUG and PA. [Conclusion] The results suggest that continuation of daily activities at home is necessary for maintaining physical function.
[A longitudinal study of the association between physical activity and apathy among community-dwelling older adults]

Abstract:Aim: This study clarified the association between the amount of physical activity and apathy after one year among community-dwelling older adults. Methods: Two hundred community-dwelling older adults (mean age: 74.3±5.0 years old, female: 52.5%) who participated in the 2018 and 2019 "Kasama Longevity Health Examination" were included. Apathy was assessed using the Apathy Scale (0-42 points; the higher the score, the lower the motivation), physical activity by Physical Activity Scale for the Elderly (PASE), and Geriatric Depression Scale-15 (GDS). The 2018 PASE data were grouped into tertiles. A multiple regression analysis was performed with the 2019 Apathy Scale as the dependent variable and the 2018 PASE as the independent variable, and the sex, age, years of education, economic situation, body mass index, chronic illness, smoking history, alcohol drinking habits, physical function, cognitive function, GDS, and the 2018 Apathy Scale as adjustment variables. The PASE subcategories (leisure-time, household, and work-related activities) were examined using a similar method. Results: The mean Apathy Scale in 2019 was 14.0±6.2 for the low physical activity group, 12.8±6.0 for the medium physical activity group, and 10.1±5.9 for the high physical activity group. The high physical activity group showed a significant negative association with the Apathy Scale (B=-1.56, 95% confidence interval=-2.91 to -0.21, p=0.023). No association was found for any activity of the PASE sub-items. Conclusions: A high level of physical activity may protect against apathy among community-dwelling older adults.
Factors that Decrease Sedentary Behavior in Community-Dwelling Elderly People: A Longitudinal Study

Abstract:Background and objectives: This study was to clarify how the frequency of participation in a health class affected the reduction in sedentary behavior after two years, and whether decreases in sedentary behavior in elderly people who participated in a health class persisted two years after the end of an intervention. Materials and Methods: This study was longitudinal, and the results of a previous study conducted in 2017 were added to the findings of a different year. The participants were elderly health class members at a community dwelling in Japan who participated between 2016 and 2018. Of the 86 participants that were enrolled, the data of 80 were collected in 2016. A year later, in a 2017 follow-up, the number of participants was 80, and two years later, in a 2018 follow-up, there were 71 participants. Results: There was a significant difference with regards to the reduction of the sedentary behavior rate between two different groups (a health class participation rate of more than 75% and the other with less than 75%) two years later. However, no difference in sedentary behavior rate was found between the two groups (intervention group and control group) at a two-year follow-up, despite observing differences after one year. Conclusions: Continuous participation in a health class may help reduce sedentary behavior. After two years, “Active Guide” brochures and documents may not help in reducing sedentary behavior, despite seeing improvements after one year. Persistent social participation may have a more lasting effect than one-off interventions in reducing the sedentary behavior ratio.

Abstract:Background : Several previous studies investigated physical activity of older adults using wearable devices, but more studies need to develop normative values for chronic disease conditions. This study aimed to investigate physical activity using a triaxial activity monitor in community-dwelling older Japanese adults with and without lifestyle-related diseases. Methods : Data from a total of 732 community-dwelling older Japanese men and women were collected and analyzed in a cross-sectional study. The participants’ physical activity was assessed for seven consecutive days by a triaxial accelerometer. Physical activity was assessed by number of lifestyle-related diseases and six lifestyle-related diseases categories by gender. Physical activity was assessed separately for total, locomotive, and nonlocomotive physical activity. Results : Participants with multiple (two or more) diseases had significantly lower total light-intensity physical activity (LPA; 278.5 ± 8.4 min/day) and nonlocomotive LPA (226.4 ± 7.0 min/day) versus without diseases in men. Compared in each disease category, total LPA and nonlocomotive LPA was significantly lower in men with hypertension and diabetes. Total sedentary time was significantly higher in men with hypertension, diabetes, and heart disease. Locomotive LPA was significantly lower in men with diabetes. In women, locomotive moderate- to vigorous-intensity physical activity was significantly higher in women with diabetes, and nonlocomotive moderate- to vigorous-intensity physical activity was significantly lower in women with heart disease. Conclusion : This study demonstrated that older Japanese men with multiple lifestyle-related diseases had lower physical activity. In each disease category, hypertension, diabetes, and heart disease affected lower physical activity, especially in men.

Abstract:Abstract Regular physical activity, dietary variety, and active social participation are modifiable and influential factors of adverse health outcomes. However, the cumulative effects of these behaviors are unknown. We examined these cumulative associations with active life loss in older adults. We analyzed 3-year longitudinal data from 7246 initially non-disabled residents aged 65-84 years from 18 districts of Ota City, Tokyo. Sufficiency of moderate- to vigorous-intensity physical activity (MVPA) of ≥150 minutes/week, dietary variety score (DVS) of ≥3, and social participation of ≥1 time/month were assessed using self-administered questionnaires. We operationally defined active life loss for individuals as being newly certified for long-term care insurance or death without prior certification. Multilevel survival analyses were applied to calculate hazard ratios (HRs) and 95% confidence intervals (CIs). During an average follow-up of 2.9 years, the cumulative incidence of active life loss was 11.3% (817 individuals: 650 new certifications and 167 deaths without prior certification). Multivariate-adjusted HRs (95% CIs) for active life loss were 0.73 (0.58-0.92) in only MVPA of ≥150 minutes/week, 0.88 (0.67-1.15) in only DVS of ≥3, 0.75 (0.51-1.09) in only social participation of ≥1 time/month, 0.56 (0.45-0.70) in the group satisfying any two, and 0.52 (0.40-0.67) in the group satisfying all three behaviors, compared with a reference group that did not satisfy any of the behaviors. Sensitivity analysis that excluded active life losses during the first year showed similar results. The combination of regular physical activity, dietary variety, and social participation further enhances the effects on active life expectancy than individual practices.
Sex and Age Differences in Association between Physical Activity and Metabolic Syndrome: Results from NHANES 2003-2006

Abstract:Objective: To examine whether and how sex and age modify the association between accelerometer-based physical activity (PA) and metabolic syndrome (MetS) among American (US) adults. Method: Adults aged ≥20 years old who participated in the mobile center examination during 2003-2006 in the National Health and Nutrition Examination Survey were included for analysis. The total minutes per day of moderate-to-vigorous PA (MVPA) was estimated using ActiGraph. Multivariable logistic regression was used to estimate the odds ratio (OR) of having MetS at an increasing MVPA time. The modification effects of gender and age on the association between MetS and MVPA time were examined by testing for two-way and three-way interaction terms of MVPA time, sex, and age in the model after adjusting for relevant covariates. Results: The prevalence of MetS generally decreased with the MVPA time and was lower in females than in males, although the sex difference varied across age groups. After adjusting for demographic and lifestyle covariates, there was a significant sex difference in how an increased MVPA time lowered the odds of MetS. This interactive effect also varied with age. MVPA benefitted young and middle-age populations up until about 65 years old for both sexes, and the protective effect weakened with age. Although the effect of MVPA was stronger for males than females at young ages, the rate at which it attenuated was quicker in males. The OR of MetS between males and females per unit change of MVPA time was 0.73 (95% CI: [0.57, 0.93]) at age = 25 years, compared to OR = 1.00 (95% CI: [0.88, 1.16]) at age = 60 years. Before the age of 50, the gender difference in the protective effect on MetS was larger at low MVPA levels and became smaller at higher MVPA levels. The male advantage was quite stable with an increasing MVPA time for ages 50-60, and no longer significant at older ages. Conclusions: Young and middle-age populations benefitted from MVPA, lowering the risk of MetS for both sexes. A longer MVPA time was associated with a greater decrease in the risk of MetS in young men than in women, but the sex difference reduced with age and was no longer apparent in older populations.
Physical Activity and Health of Middle-Aged and Older Japanese Across the COVID-19 Pandemic: Differential Outcomes Highlight a Problematic Life Stage

Abstract:Introduction : COVID-19 caused major disruptions across the super-aged nation of Japan, yet few studies explored temporal changes among middle-aged and older cohorts from baseline to the height of community transmission. Changes in physical activity and sedentary behavior during global pandemics may alter patterns of morbidity and mortality among susceptible aging populations. Objectives : This study investigated patterns of physical activity, sitting behavior, and health among representative samples of middle-aged and older adults in Tokyo before and during the pandemic. Methods : Repeated online surveys were conducted with quota samples of 800 Tokyo residents in 2019 and 2021 using validated Japanese-language measures, including the short form-International Physical Activity Questionnaire and the Basic Ecological Health Scale-6. Statistical analyses included comparative evaluations of activity parameters by age cohort, gender, and selected covariates. Statistical tests included the Kruskal–Wallis test, Mann–Whitney U test, chi-square test for Independence and Hierarchical Regression. Results : Over 34% of respondents were inactive at each data collection point, and 72% reported negative impacts of COVID-19 on their physical activities. Older adults showed no significant changes in their activity and sitting behavior and reported better health compared with those in middle age across the pandemic. Middle-aged males reported a significant decline in total activity of 33% across the pandemic period ( U = 16,958, z = −2.64, p = .008, r = .13). Middle-aged females reported the lowest levels of physical activity, and health, and showed a 29% increase in sitting behavior across the pandemic ( U = 16,925, z = −2.68, p = .007, r = .13). Subjective health status was consistently associated with higher overall activity and walking before and during the pandemic. Conclusion : Differential outcomes were identified between age and gender regarding health, physical activity, walking, and sitting across the pandemic with significantly worse impacts reported among middle-aged samples. Implications : These results have implications for healthy transitions to later life and the design of postpandemic interventions to address activity opportunities in Japan.

Abstract:Background: Insufficient or decreasing physical activity is common in older adults. Most studies on physical activity changes and mortality were conducted in adults younger than 80 years old in developed countries. We aimed to investigate the relationship between changes in physical activity and longevity in the oldest old (80 years or older) population using the Chinese Longitudinal Healthy Longevity Survey. Methods: Participants aged 80 or older at baseline were categorized into four groups: 1) remaining physically inactive (n = 14,287), 2) remaining physically active (n = 5411), 3) shifting from being inactive to active (n = 1364), and 4) shifting from being active to inactive (n = 1401). We fitted accelerated failure time Weibull survival regression models, adjusting for baseline sociodemographics, lifestyle factors and disease status. We further examined whether the associations differed by subgroups. Results: A total of 15,707 participants died during follow-up (median duration of follow-up = 3.0 years). Compared with participants who remained physically inactive, those who remained active (fully adjusted event time ratio (ETR): 1.14, 95%CI: 1.11-1.17) or shifted from being inactive to active (fully adjusted ETR: 1.14, 95% CI: 1.08-1.20) had statistically significant longer survival time. No significant association was observed between remaining physically inactive and shifting from being active to inactive. Subgroup analyses showed consistent associations in nearly all strata. Conclusion: Maintaining frequent physical activity or shifting from being physically inactive to active was consistently associated with longer survival time in the oldest old population. Our findings provide evidence for encouraging older adults to regularly engage in physical activity to gain longevity benefits.

Sex differences in the associations of accelerometer-determined physical activity with physical and cognitive function in older adults living in long-term care

Abstract:Older adults residing in long-term care often experience declines in physical and cognitive functions despite the access to in-house physical activity (PA) programs. This study aimed to evaluate the associations of PA with physical function and global cognitive function in older adults living in long-term care, while examining potential sex differences. A cross-sectional analysis of baseline data from a two-arm cluster randomized controlled trial was conducted, involving 67 participants (34 men, 33 women). PA levels were assessed using tri-axial accelerometers. Physical function, including muscular strength, postural sway, and Short Physical Performance Battery (SPPB) and cognitive function were measured. Spearman correlation analysis revealed no significant associations between PA metrics and muscular strength, postural sway, or global cognitive function across the entire samples ( p ≥ 0.091). Multiple linear regression models were developed for the entire sample, males, and females to examine the associations between PA and physical function measures and global cognitive function. After adjustments for confounders, light PA was significantly associated with higher SPPB sub-scores (gait: β = 0.600, p < 0.001; sit-to-stand: β = 0.574, p < 0.001), faster usual gait speed ( β = 0.659, p = 0.012), and shorter sit-to-stand times ( β = −0.305, p = 0.041) across the whole sample. Similar significant associations were observed in males between light PA and SPPB scores (total: β = 0.319, p = 0.040; gait: β = 0.532, p < 0.001; sit-to-stand: β = 0.417, p = 0.009), usual gait speed ( β = 0.762, p = 0.017), and sit-to-stand times ( β = −0.677, p < 0.001). In females, a significant association was found between light PA and global cognitive function ( β = 0.319, p = 0.012) after adjusting for confounders. This study highlights sex differences in the association of accelerometer-determined PA with physical and cognitive function in older adults living in long-term care, with LPA showing beneficial effects, especially for physical function in males and for cognitive function in females.

Abstract:PURPOSE: Physical activity (PA) is distinct from physical capacity (PC), even though they are correlated in old age. PC defines the limits for PA, while activities in daily life typically remain submaximal. Individuals whose intensity of daily activities is close to physical capacity may be better protected from future decline in physical function compared to those who do not, although prospective research to support this hypothesis is lacking. Therefore, this study compared changes in physical function over a four-year follow-up between community-dwelling older adults categorized based on their combined baseline PC and PA. METHODS: This was a four-year longitudinal follow-up study of older adults aged 75-85 years at baseline (N = 312, 60% women). Baseline PC was determined based on 5-second Mean Amplitude Deviation (MAD) epoch value during the maximal 10-meter walking test, and PA was determined based on the peak 75-minutes MAD intensity threshold from thigh-worn accelerometer monitoring over 3-7 days. Baseline values of PA and PC were categorized into lowPC-lowPA, lowPC-highPA, highPC-lowPA, and highPC-highPA profiles. Physical function was evaluated using the Short Physical Performance Battery (SPPB) at baseline and at the follow-up, with total score and 5 x Sit-To-Stand (5xSTS) test time as the primary outcomes. Nonparametric tests and generalized estimating equations were used for analyses. RESULTS: From baseline to follow-up, statistically significant changes in the SPPB total score and 5xSTS test time were observed in all profiles (p<0.05) except the low PC-high PA profile. Over the follow-up period, the decrease was greater for low versus high PA profiles within both PC profiles for SPPB total score (high PC: B -0.61, SE 0.24, 95% CI -1.08, -0.15; low PC: B -0.96, SE 0.35, 95% CI -1.62, -0.32), but not for 5xSTS time. No statistically significant difference was observed in the change in 5xSTS test time between the low and high PA profiles for either PC profile. CONCLUSIONS: The findings suggest that engaging in demanding PA regardless of baseline PC may help to protect against a decline in physical functioning in old age. Consequently, older adults should be encouraged to engage in physically demanding activities that could potentially enhance their functional capacity.
