Abstract:BACKGROUND:Identifying thyroid nodules' boundaries is crucial for making an accurate clinical assessment. However, manual segmentation is time-consuming. This paper utilized U-Net and its improved methods to automatically segment thyroid nodules and glands.METHODS:The 5822 ultrasound images used in the experiment came from two centers, 4658 images were used as the training dataset, and 1164 images were used as the independent mixed test dataset finally. Based on U-Net, deformable-pyramid split-attention residual U-Net (DSRU-Net) by introducing ResNeSt block, atrous spatial pyramid pooling, and deformable convolution v3 was proposed. This method combined context information and extracts features of interest better, and had advantages in segmenting nodules and glands of different shapes and sizes.RESULTS:DSRU-Net obtained 85.8% mean Intersection over Union, 92.5% mean dice coefficient and 94.1% nodule dice coefficient, which were increased by 1.8%, 1.3% and 1.9% compared with U-Net.CONCLUSIONS:Our method is more capable of identifying and segmenting glands and nodules than the original method, as shown by the results of correlational studies.
Hybrid transformer UNet for thyroid segmentation from ultrasound scans

Abstract:Deep learning based medical image segmentation methods have been widely used for thyroid gland segmentation from ultrasound images, which is of great importance for the diagnosis of thyroid disease since it can provide various valuable sonography features. However, existing thyroid gland segmentation models suffer from: (1) low-level features that are significant in depicting thyroid boundaries are gradually lost during the feature encoding process, (2) contextual features reflecting the changes of difference between thyroid and other anatomies in the ultrasound diagnosis process are either omitted by 2D convolutions or weakly represented by 3D convolutions due to high redundancy. In this work, we propose a novel hybrid transformer UNet (H-TUNet) to segment thyroid glands in ultrasound sequences, which consists of two parts: (1) a 2D Transformer UNet is proposed by utilizing a designed multi-scale cross-attention transformer (MSCAT) module on every skipped connection of the UNet, so that the low-level features from different encoding layers are integrated and refined according to the high-level features in the decoding scheme, leading to better representation of differences between anatomies in one ultrasound frame; (2) a 3D Transformer UNet is proposed by applying a 3D self-attention transformer (SAT) module to the very bottom layer of 3D UNet, so that the contextual features representing visual differences between regions and consistencies within regions could be strengthened from successive frames in the video. The learning process of the H-TUNet is formulated as a unified end-to-end network, so the intra-frame feature extraction and inter-frame feature aggregation can be learned and optimized jointly. The proposed method was evaluated on Thyroid Segmentation in Ultrasonography Dataset (TSUD) and TG3k Dataset. Experimental results have demonstrated that our method outperformed other state-of-the-art methods with respect to the certain benchmarks for thyroid gland segmentation.

Abstract:Ultrasound (US) technology is a widely utilized for clinical screening owing to its cost-effectiveness, painlessness, and convenience. However, the automatic segmentation of lesions or organs in US images remains to be challenging due to speckle artifacts, blurred boundaries, and low contrast. Recently, transformer-based methods have shown to be effective in long-range dependency, which is a good complement to Convolutional Neural Networks (CNN). In this article, we present a novel U-shape segmentation model based on a hybrid CNN-transformer structure that can effectively integrate CNN local features and transformer long-range contextual information of US images. To begin with, we design a coordinate residual block (CdRB) to encode the absolute position information of lesions. Further, we develop a channel enhanced self-attention-based transformer (ECAT) to help enhance the response of extracted global features. Finally, we adopt a comprehensive dual attention module (CDAM) to enhance skip connection features, which can learn feature correlations and capture more accurate edge features in US images. Results based on four public US datasets demonstrate that our method outperforms state-of-the-art segmentation methods, with 0.741 Dice on BUSI for breast lesion segmentation, 0.827 Dice on DDTI for thyroid lesion segmentation, 0.895 Dice on TN3k for thyroid lesion segmentation and 0.940 Dice on CAMUS for left ventricle segmentation. Furthermore, the robustness of our network is further demonstrated by an external validation dataset for breast lesion segmentation. In summary, our method showcases excellent adaptability and robustness in US image segmentation and can potentially be a general US segmentation tool.

Abstract:TO THE E D I T O R — I n April 2005, a case of reactivation of hepatitis B virus (HBV) infection occurred in a hemodialysis patient in an Irish tertiary referral center. In the absence of specific Irish dialysis guidelines at this time, most Irish units, including this center, followed UK guidelines; that is, HBV surface antigen (HBsAg) testing was performed prior to dialysis and then monthly, ideally, but at least every 3 months. The majority of units did not test for HBV core antibody (anti-HBc). This patient had tested negative for HBsAg prior to dialysis in November 2004 and was found to be HBsAg positive in April 2005. The patient became immunosuppressed over this period. Testing of an archived sample obtained prior to dialysis (November 2004) revealed that it was positive for anti-HBc. The April 2005 sample was positive for anti-HBc and negative for anti-HBc immunoglobulin (Ig) M.

Abstract:Breast cancer is a significant health concern that remains one of the leading causes of mortality in women worldwide. Convolutional Neural Networks (CNNs) have been shown to be effective in ultrasound breast image segmentation. Yet, because of the lack of long-distance dependence, the segmentation performance of CNNs is limited in addressing challenges typical of segmentation of ultrasound breast lesions, such as similar intensity distributions, the presence of irregular objects, and blurred boundaries. In order to overcome these issues, several studies have combined transformers and CNNs, to compensate for the shortcomings of CNNs with the ability of transformers to exploit long-distance dependence. Most of these studies limited themselves to rigidly plug transformer blocks into the CNN, lacking consistency in the process of feature extraction and therefore leading to poor performances in segmenting challenging medical images. In this paper, we propose HAU-Net(hierarchical attention-guided U-Net), a hybrid CNN-transformer framework that benefits from both the long-range dependency of transformers and the local detail representation of CNNs. To incorporate global context information, we introduce a L-G transformer block nested into the skip connections of the U shape architecture network. In addition, to further improve the segmentation performance, we added a cross attention block (CAB) module on the decoder side to allow different layers to interact. Extensive experimental results on three public datasets indicate that the proposed HAU-Net can achieve better performance than other state-of-the-art methods for breast lesions segmentation, with Dice coefficient of 83.11% for BUSI, 88.73% for UDIAT, and 89.48% for BLUI respectively.

Abstract:In recent years, for the purpose of integrating the individual strengths of convolutional neural networks (CNN) and Transformer, a network structure has been built to integrate the two methods in medical image segmentation. But most of the methods only integrate CNN and Transformer at a single level and cannot extract low-level detail features and high-level abstract information simultaneously. Meanwhile, this structure lacks flexibility, unable to dynamically adjust the contributions of different feature maps. To address these limitations, we introduce HCT-Unet, a hybrid CNN-Transformer model specifically designed for multi-organ medical images segmentation. HCT-Unet introduces a tunable hybrid paradigm that differs significantly from conventional hybrid architectures. It deploys powerful CNN to capture short-range information and Transformer to extract long-range information at each stage. Furthermore, we have designed a multi-functional multi-scale fusion bridge, which progressively integrates information from different scales and dynamically modifies attention weights for both local and global features. With the benefits of these two innovative designs, HCT-Unet demonstrates robust discriminative dependency and representation capabilities in multi-target medical image tasks. Experimental results reveal the remarkable performance of our approach in medical image segmentation tasks. Specifically, in multi-organ segmentation tasks, HCT-Unet achieved a Dice similarity coefficient (DSC) of 82.23%. Furthermore, in cardiac segmentation tasks, it reached a DSC of 91%, significantly outperforming previous state-of-the-art networks. Abstract:Erythrocytes and H-HeLa cells were treated with neuraminidase and then compared with untreated cells for their ability to adsorb to mycoplasma colonies or be agglutinated by suspensions of the mycoplasmas. Of the 17 mycoplasma serotypes examined, only 4 were found to use neuraminic acid receptors; these were Mycoplasma pneumoniae, M. gallisepticum, M. synoviae, and mycoplasma WR1. Not all strains of a serotype behaved alike. Thus, removal of receptors on erythrocytes for one strain of M. gallisepticum required at least 100 times the concentration of neuraminidase needed to remove them for another strain. The mechanism of attachment of erythrocytes to mycoplasma colonies does not appear to be the same as that for attachment to mycoplasmas in suspension.
TGDAUNet: Transformer and GCNN based dual-branch attention UNet for medical image segmentation

Abstract:Accurate and automatic segmentation of medical images is a key step in clinical diagnosis and analysis. Currently, the successful application of Transformers' model in the field of computer vision, researchers have begun to gradually explore the application of Transformers in medical segmentation of images, especially in combination with convolutional neural networks with coding-decoding structure, which have achieved remarkable results in the field of medical segmentation. However, most studies have combined Transformers with CNNs at a single scale or processed only the highest-level semantic feature information, ignoring the rich location information in the lower-level semantic feature information. At the same time, for problems such as blurred structural boundaries and heterogeneous textures in images, most existing methods usually simply connect contour information to capture the boundaries of the target. However, these methods cannot capture the precise outline of the target and ignore the potential relationship between the boundary and the region. In this paper, we propose the TGDAUNet, which consists of a dual-branch backbone network of CNNs and Transformers and a parallel attention mechanism, to achieve accurate segmentation of lesions in medical images. Firstly, high-level semantic feature information of the CNN backbone branches is fused at multiple scales, and the high-level and low-level feature information complement each other's location and spatial information. We further use the polarised self-attentive (PSA) module to reduce the impact of redundant information caused by multiple scales, to better couple with the feature information extracted from the Transformers backbone branch, and to establish global contextual long-range dependencies at multiple scales. In addition, we have designed the Reverse Graph-reasoned Fusion (RGF) module and the Feature Aggregation (FA) module to jointly guide the global context. The FA module aggregates high-level semantic feature information to generate an original global predictive segmentation map. The RGF module captures non-significant features of the boundaries in the original or secondary global prediction segmentation graph through a reverse attention mechanism, establishing a graph reasoning module to explore the potential semantic relationships between boundaries and regions, further refining the target boundaries. Finally, to validate the effectiveness of our proposed method, we compare our proposed method with the current popular methods in the CVC-ClinicDB, Kvasir-SEG, ETIS, CVC-ColonDB, CVC-300,datasets as well as the skin cancer segmentation datasets ISIC-2016 and ISIC-2017. N-Net: A novel dense fully convolutional neural network for thyroid nodule segmentation

Abstract:Medical image segmentation is an essential component of computer-aided diagnosis (CAD) systems. Thyroid nodule segmentation using ultrasound images is a necessary step for the early diagnosis of thyroid diseases. An encoder-decoder based deep convolutional neural network (DCNN), like U-Net architecture and its variants, has been extensively used to deal with medical image segmentation tasks. In this article, we propose a novel N-shape dense fully convolutional neural network for medical image segmentation, referred to as N-Net. The proposed framework is composed of three major components: a multi-scale input layer, an attention guidance module, and an innovative stackable dilated convolution (SDC) block. First, we apply the multi-scale input layer to construct an image pyramid, which achieves multi-level receiver field sizes and obtains rich feature representation. After that, the U-shape convolutional network is employed as the backbone structure. Moreover, we use the attention guidance module to filter the features before several skip connections, which can transfer structural information from previous feature maps to the following layers. This module can also remove noise and reduce the negative impact of the background. Finally, we propose a stackable dilated convolution (SDC) block, which is able to capture deep semantic features that may be lost in bilinear upsampling. We have evaluated the proposed N-Net framework on a thyroid nodule ultrasound image dataset (called the TNUI-2021 dataset) and the DDTI publicly available dataset. The experimental results show that our N-Net model outperforms several state-of-the-art methods in the thyroid nodule segmentation tasks.

Abstract:Accurate segmentation of thyroid nodules is essential for early screening and diagnosis, but it can be challenging due to the nodules' varying sizes and positions. To address this issue, we propose a multi-attention guided UNet (MAUNet) for thyroid nodule segmentation. We use a multi-scale cross attention (MSCA) module for initial image feature extraction. By integrating interactions between features at different scales, the impact of thyroid nodule shape and size on the segmentation results has been reduced. Additionally, we incorporate a dual attention (DA) module into the skip-connection step of the UNet network, which promotes information exchange and fusion between the encoder and decoder. To test the model's robustness and effectiveness, we conduct the extensive experiments on multi-center ultrasound images provided by 17 local hospitals. The model is trained using the federal learning mechanism to ensure privacy protection. The experimental results show that the Dice scores of the model on the data sets from the three centers are 0.908, 0.912 and 0.887, respectively. Compared to existing methods, our method demonstrates higher generalization ability on multi-center datasets and achieves better segmentation results.

Abstract:Accurate auto-medical image segmentation, which is essential in disease diagnosis and treatment planning, has been significantly prospered by recent advances in Convolution Neural Network (CNN) and Transformer. However, pure CNN-based or pure Transformer-base architectures exhibit limitations for segmentation tasks. For example, CNN fails to capture long-range relations due to fix receptive fields, and Transformer ignores pixel-level spatial details of features. To address these challenges, we propose a novel parallel hybrid architecture for medical image segmentation, which is named CPT-Unet (Cross Pyramid Transformer U-shape Network). CPT-Unet may be the first attempt to exploit both the advantages of Pyramid Vision Transformer (PVT) and CNN to the full by integrating them into the standard U-shape network to improve the segmentation performance and inference time. Specifically, we design a parallel dual branch encoder and decoder in CPT-Unet that consists of CNN and PVT. The input image is fed into the encoder of these two branches simultaneously to extract low-level spatial details and global contexts in a much shallower way. We design a novel fusion strategy to adequately utilize the multi-scale features extracted from CNN and PVT. We also add PVT in the decoder to better restore the segmentation map. Experiments on two public segmentation datasets demonstrate the improved performance of the proposed CPT-Unet over the comparison methods. FCG-Net: An innovative full-scale connected network for thyroid nodule segmentation in ultrasound images

Abstract:Ultrasonic imaging is more susceptible to poor contrast and speckle noise than other imaging modalities, which poses certain challenges to computer-aided automatic segmentation of thyroid nodules. Nine popular segmentation networks for thyroid nodule in ultrasound images were well trained with the same training strategy and the performances on the public dataset and the clinical dataset were well compared using different indicators. Among them, UNet3+ achieved the best performance. However, it brings more parameters compared to other lightweight networks, which limits its application in portable diagnosis. This paper proposed an innovative thyroid nodule segmentation network FCG-Net that using full-scale skip connection to extracted multi resolution features, and Ghost bottleneck was introduced in the encoder and decoder process to reduce computational complexity of feature maps. Compared to UNet3+, the parameters of the model were reduced from 27 million to 11 million, and the inference speed was improved by 1.07 FPS. Meanwhile, the segmentation performance of FCG-Net exceeds that of UNet3+. 729 ultrasound images from the public dataset and clinical ultrasound images were used to validate the performance of the network by 5-fold cross-validation. The accuracy, precision, sensitivity, specificity and Dice of the FCG-Net were 0.9488, 0.8579, 0.8984, 0.9627 and 0.8670 respectively. The network not only improves the accuracy of ultrasonic diagnosis but also reduces the requirements of hardware and calculation, which is significant to promote the development of portable ultrasonic diagnosis of thyroid nodule.

Abstract:Medical image segmentation is a crucial topic in medical image processing. Accurately segmenting brain tumor regions from multimodal MRI scans is essential for clinical diagnosis and survival prediction. However, similar intensity distributions, variable tumor shapes, and fuzzy boundaries pose severe challenges for brain tumor segmentation. Traditional segmentation networks based on UNet struggle to establish explicit long-range dependencies from the feature space due to the limitations of the CNN receptive field. This is particularly crucial for dense prediction tasks such as brain tumor segmentation. Recent works have incorporated the powerful global modeling capability of Transformer into UNet to achieve more precise segmentation results. Nevertheless, these methods encounter some issues: (1) the global information is often modeled by simply stacking Transformer layers for a specific module, resulting in high computational complexity and underutilization of the potential of the UNet architecture; (2) the rich boundary information of tumor subregions in multi-scale features is often overlooked. Motivated by these challenges, we propose an advanced fusion of Transformer with UNet by reexamining the core three parts (encoder, bottleneck, and skip connections). Firstly, we introduce a CNN-Transformer module in the encoder to replace the traditional CNN module, enabling the capture of deep spatial dependencies from input images. To address high-level semantic information, we incorporate a computationally efficient spatial-channel attention layer in the bottleneck for global interaction, highlighting important semantic features from the encoder path output. For irregular lesions, we fuse the multi-scale features from the encoder output and the decoder features in the skip connections by calculating cross-attention. This adaptive querying of valuable information from multi-scale features enhances the boundary localization ability of the decoder path and suppresses redundant features with low correlation. Compared to existing methods, our model further enhances the learning capacity of the overall UNet architecture while maintaining low computational complexity. Experimental results on the BraTS2018 and BraTS2020 datasets for brain tumor segmentation tasks demonstrate that our model achieves comparable or superior results compared to recent CNN or Transformer-based models. The average DSC and HD95 on the two datasets are 0.854, 6.688, and 0.862, 5.455 respectively. At the same time, our model achieves optimal segmentation of Enhancing tumors, showcasing the effectiveness of our method. Abstract:3D volumetric medical image segmentation is a crucial task in computer-aided diagnosis applications, but it remains challenging due to low contrast and boundary ambiguity between organs and surrounding tissues. Considering that accurate boundary voxels are of importance for organ segmentation, which relies on rich detailed features information. The most recent convolutional neural networks and transformer networks have attempted to enhance 2D boundaries during feature extraction. Few approaches focus on boundary voxels preservation for 3D scenarios. To address these issues, we propose the Hybrid Transformer-CNN with Boundary-awareness(HTCB-Net) network, which follows an encoder-decoder segmentation paradigm with learnable boundary modules. The 3D swin-transformer encoder is embedded with auxiliary object-related boundary map by designing a learnable boundary extracting module(BEM), which assists model in obtaining rich and discriminative feature representations. Our boundary map obtained from BEM supervises explicitly feature extraction process. Subsequently, boundary preserving module(BPM) adopts a novel fusion strategy, which integrates extracted boundary map and the corresponding encoder features. Through this module, the boundary position awareness is combined for feature enhancement with spatial complement and channel attention. We evaluate the performance of the proposed method with quantitative experiments on three public available datasets in both CT and MRI modalities: OAI-ZIB, Spleen, Pancreas. The comparative experimental results demonstrate that our HTCB-Net preserves more precise 3D boundaries and obtains significant improvements, particularly in terms of Average Symmetric Surface Distance(ASSD).

Abstract:Computer-aided thyroid CT image segmentation aims to provide imaging physicians and clinicians with auxiliary diagnostic suggestions and improve the efficiency of physicians in diagnosing the thyroid region. However, it is still a challenging task to distinguish the thyroid from other surrounding tissues due to adhesions in thyroid CT images caused by thyroid disease. To achieve accurate segmentation of thyroid CT images under the intervention of different types of thyroid nodules, we proposed a thyroid segmentation network named ResUnet by introducing the residual learning idea to UNet. Our network controls the gradient dispersion by incorporating a batch normalization operation and an intermediate layer regularization operation, then solves the degradation problem by introducing the residual connections into the convolution operation. Moreover, our ResUnet network can converge faster with the same number of layers, thus supporting a deeper design of the network. Extensive experiments also validated the high accuracy (94.10%), specificity (98.94%), and sensitivity (96.34%) of the proposed ResUnet for the segmentation of thyroid nodules, which can assist CT physicians in the diagnosis of the thyroid gland.

Abstract:Introduction: Ultrasound imaging has become a crucial tool in medical diagnostics, offering real-time visualization of internal organs and tissues. However, challenges such as low contrast, high noise levels, and variability in image quality hinder accurate interpretation. To enhance the diagnostic accuracy and support treatment decisions, precise segmentation of organs and lesions in ultrasound image is essential. Recently, several deep learning methods, including convolutional neural networks (CNNs) and Transformers, have reached significant milestones in medical image segmentation. Nonetheless, there remains a pressing need for methods capable of seamlessly integrating global context with local fine-grained information, particularly in addressing the unique challenges posed by ultrasound images. Methods: In this paper, to address these issues, we propose DDTransUNet, a hybrid network combining Transformer and CNN, with a dual-branch encoder and dual attention mechanism for ultrasound image segmentation. DDTransUNet adopts a Swin Transformer branch and a CNN branch to extract global context and local fine-grained information. The dual attention comprising Global Spatial Attention (GSA) and Global Channel Attention (GCA) modules to capture long-range visual dependencies. A novel Cross Attention Fusion (CAF) module effectively fuses feature maps from both branches using cross-attention. Results: Experiments on three ultrasound image datasets demonstrate that DDTransUNet outperforms previous methods. In the TN3K dataset, DDTransUNet achieves IoU, Dice, HD95 and ACC metrics of 73.82%, 82.31%, 16.98 mm, and 96.94%, respectively. In the BUS-BRA dataset, DDTransUNet achieves 80.75%, 88.23%, 8.12 mm, and 98.00%. In the CAMUS dataset, DDTransUNet achieves 82.51%, 90.33%, 2.82 mm, and 96.87%. Discussion: These results indicate that our method can provide valuable diagnostic assistance to clinical practitioners.

Abstract:Objectives The application of deep learning to medical image segmentation has received considerable attention. Nevertheless, when segmenting thyroid ultrasound images, it is difficult to achieve good segmentation results using deep learning methods because of the large number of nonthyroidal regions and insufficient training data. Methods In this study, a Super-pixel U-Net, designed by adding a supplementary path to U-Net, was devised to boost the segmentation results of thyroids. The improved network can introduce more information into the network, boosting auxiliary segmentation results. A multi-stage modification is introduced in this method, which includes boundary segmentation, boundary repair, and auxiliary segmentation. To reduce the negative effects of non-thyroid regions in the segmentation, U-Net was utilized to obtain rough boundary outputs. Subsequently, another U-Net is trained to improve and repair the coverage of the boundary outputs. Super-pixel U-Net was applied in the third stage to assist in the segmentation of the thyroid more precisely. Finally, multidimensional indicators were used to compare the segmentation results of the proposed method with those of other comparison experiments. Discussion The proposed method achieved an F1 Score of 0.9161 and an IoU of 0.9279. Furthermore, the proposed method also exhibits better performance in terms of shape similarity, with an average convexity of 0.9395. an average ratio of 0.9109, an average compactness of 0.8976, an average eccentricity of 0.9448, and an average rectangularity of 0.9289. The average area estimation indicator was 0.8857. Conclusion The proposed method exhibited superior performance, proving the improvements of the multi-stage modification and Super-pixel U-Net.

Abstract:Although histamine intolerance (HIT) is not very frequently encountered, it can have serious consequences. Food intolerance is a non allergic hypersensitivity to food that does not include the immune system even though the symptoms are similar to those of IgE-mediated allergic reactions. HIT apparently develops as a result of an impaired diamine oxidase (DAO) activity due to gastrointestinal disease or through DAO inhibition, as well as through a genetic predisposition which was proven in a number of patients. The intake of histamine-rich foods as well as alcohol or drugs which cause either the release of histamine or the blocking of DAO can lead to various disorders in many organs (gastrointestinal system, skin, lungs, cardiovascular system and brain), depending on the expression of histamine receptors. Dermatologic sequels can be rashes, itch, urticaria, angioedema, dermatitis, eczema and even acne, rosacea, psoriasis, and other. Recognizing the symptoms due to HIT is especially important in treating such patients. The significance of HIT in patients with atopic dermatitis in whom the benefit of a low histamine diet has been proven is becoming increasingly understood recently. Because of the possibility of symptoms affecting numerous organs, a detailed history of symptoms following the intake of histamine-rich foods or drugs that interfere with histamine metabolism is essential for making the diagnosis of HIT. Considering that such symptoms can be the result of multiple factors, the existence of HIT is usually underestimated, but considerable expectations are being made from future studies.

Abstract:Thyroid gland segmentation in ultrasound images is essential in thyroid disease diagnoses. However, the relatively low quality and the speckle noise in ultrasound images make the organ tissues ambiguous and inhomogeneous. Besides, the thyroid anatomical parts may be separated by a significant distance. In this paper, we proposed a novel Attention-guided Non-local Residual Network (ANR-Net) for thyroid segmentation in ultrasound image, which extracts the short-long relationship to boost thyroid gland segmentation performance. In the proposed network, we deploy two non-local modules to learn the global information of the image at shallow layers. Moreover, the residual features extracted in the first step are used to guide the training of the network in the second step. Experimental results on datasets demonstrate that our proposed network outperforms the state-of-the-art segmentation methods. Our code is available at https://github.com/zyairelu/ANR-Net.
