Abstract:Objective This study examined the prevalence of exposure to secondhand smoke, its correlates and its association with quality of life (QOL) among pregnant and postnatal Chinese women. Design This was a multicentre, cross-sectional study. Setting Participants were consecutively recruited from eight tertiary hospitals located in eight municipalities or provinces in China. Participants A total of 1140 women were invited to join this study and 992 (87.02%) completed all measures. Primary and secondary outcome Measures women's secondhand smoking behaviour (frequency and location of exposure to secondhand smoking), and their QOL measured by the WHO Quality of Life Questionnaire. Results A total of 211 women (21.3%, 95% CI 18.7% to 23.8%) had been exposed to secondhand smoking. Exposure to secondhand smoking was most common in public areas (56.4%), and residential homes (20.5%), while workplaces had the lowest rate of exposure (13.7%). Women with physical comorbidities were more likely to report secondhand smoking exposure, while older women, women living in urban areas, those with college or higher education level, and women in their second trimester were less likely to report exposure to secondhand smoking. Network analysis revealed that there were six significant links between secondhand smoke and QOL items. The strongest negative edge was the connection between secondhand smoke and QOL9 ('physical environment health', edge weight=–0.060), while the strongest positive edge was the connection between secondhand smoke and QOL3 ('pain and discomfort', edge weight=0.037). Conclusion The prevalence of exposure to secondhand smoking is becoming lower among pregnant and postnatal women in China compared with findings reported in previous studies. Legal legislation should be maintained and promptly enforced to establish smoke-free environments in both public and private urban/rural areas for protection of pregnant and postnatal women, especially those who are physically vulnerable and less educated.

Abstract:Background: Exposure to secondhand smoke among pregnant women is highly prevalent in India. Secondhand smoke exposure is linked with various serious deleterious health problems. Aim: The aim of this study was to assess knowledge, attitude, and behavior related to secondhand smoke exposure among pregnant women with smoking husbands in Davangere city. Materials and Methods: A descriptive cross-sectional questionnaire survey was conducted among 245 nonsmoking pregnant women with smoking husbands attending two antenatal clinics in Davanagere city using pretested, validated, investigator-administered closed-ended 18 item questionnaire. Responses to items were placed on a 3-point Likert scale. Statistical significance was set atP< 0.05. IBM SPSS Statistics for Windows, version 21 (IBM Corp., Armonk, N.Y., USA) was used for statistical analysis. Descriptive statistics were generated in terms of percentages. Results: The mean age of study participants was 26.22 ± 4.11 years. Majority of pregnant women were unaware of secondhand smoke exposure (>60%). They were unaware of its systemic complications such as cardiac problems, respiratory problems, and cancer. They were unaware of the harmful effects of secondhand tobacco smoke (SHS) on fetus/child (>60%) which indicated poor knowledge of participants toward harmful effects of secondhand smoke exposure majority of participants showed favorable attitude (>50%) and felt the need to get educated regarding SHS. Majority reported positive behavior by refraining themselves from secondhand smoke (>50%). Conclusions: Knowledge regarding secondhand smoke exposure among pregnant women with smoking husbands was poor. Inspite of having favorable attitude and behavior toward avoidance of smoke exposure, majority of women felt helpless in refraining their husbands from smoking.

Abstract:Acoustic stimulation evokes about 15 waves, the so-called auditory evoked potentials (AEP). According to their appearance in time there are three groups: early auditory evoked potentials, middle latency AEP and late latency AEP. The shorter the latency of a wave, the more precise the coordination to a definable structure in the CNS. Therefore only EAEP allow besides an evaluation of hearing threshold a differential diagnosis between peripheral, neural and central hearing disturbances. Still unclear are questions such as exact origin of waves II to V and the difference of response due to rarefaction and condensation stimuli. The test, however, is a sensitive indicator to evaluate hearing disturbances and brainstem disorders.
Association between secondhand smoke exposure in pregnant women and their socioeconomic status and its interaction with age: a cross-sectional study

Abstract:Background: Existing evidence highlights that exposure to secondhand smoke (SHS) is a risk factor for pregnant women's health and is possibly affected by individual characteristics. This study aimed to explore the effect of individual socioeconomic status (SES) on SHS exposure among pregnant women in the third trimester and the interaction effect of age. Methods: A total of 678 nonsmoking pregnant women with a median age of 29.0 years from 14 communities in a medium-sized city were recruited for this survey. Exposure to SHS was defined as the self-reported smoking habit of a spouse/partner. Individual SES characteristics consisted of marital status, educational attainment, employment and per capita monthly income. Results: There were 238 (35.1%) participants who suffered from SHS exposure. Compared to the pregnant women who were employed, those who were unemployed were more likely to suffer from SHS exposure (OR = 1.572, 95% CI: 1.013-2.441). Participants who had a high school or technical secondary school education were more likely to be exposed to SHS than those with a college education or above (OR = 1.601, 95% CI: 1.029-2.449). Advanced age was a protective factor for participants with a college education or above (OR = 0.939, 95% CI: 0.884-0.997), but age increased the risk of SHS exposure among women who had unstable marriages (OR = 1.256, 95% CI: 1.019-1.549). Conclusion: Exposure to SHS was very common among pregnant women in the third trimester. Pregnant women with a low SES and an older age should be considered a key population for the implementation of public health interventions.
Prenatal passive smoking at home: The experiences of women in Thailand

Abstract:Background: Passive smoking poses a threat to the well-being of an unborn baby; however, women's understanding and practice in this regard remain unclear. Objective: This study aimed to describe prenatal passive smoking at home based on the experiences of women in Thailand. Methods: A qualitative descriptive approach was employed. Data were collected between September 2020 and April 2022 through semi-structured in-depth interviews with 16 postpartum women purposively selected from five hospitals. Thematic analysis was used for data analysis. Results: Three themes emerged. The first theme, 'safeguard for an unborn baby,' was performed through smoking reduction, far distance, abstinence from smoke/odor inhalation, and residual elimination since harm was highly aware. However, justifying safety from the absence of smoke and foul odor was misperceived. The second theme, 'intervention for smoking cessation,' was attempted through various methods, including acquired empathy, persuasion, compromise, and supportiveness. They also needed family/friend assistance in warning and professional care in terms of advice, health education, learning materials, cessation therapy, care continuation, and dealing with complex situations. The last theme, 'maternal perseverance,' was acquired by the motives of safety goals, rights/responsibilities, changeable norms, and a golden period. Conclusion: Pregnant women persistently aimed to prevent their husbands from smoking and safeguard their unborn babies from passive smoking. However, misunderstandings and improper practices were observed. Healthcare and nursing services need comprehensive approaches to enhance understanding, perseverance, and tailored practices catering to women's needs.

Abstract:Second-Hand Smoke (SHS) is a significant health issue. For non-smoker pregnant women, SHS exposure can lead to harmful consequences on the fetus. This study aimed to examine the effectiveness of a health education program in decreasing SHS exposure and increasing pregnant women's knowledge of its harmful effects. A quasi-experimental (pretest-posttest control group) design and the second-hand exposure questionnaire (SS-A) were used. A convenience sample of 136 pregnant Jordanian women from the antenatal clinic was recruited and assigned to an intervention group (n = 70) and a control group (n = 66). The intervention group received a health education program, while the control group received the usual antenatal care. The data were collected from October to December 2018. The results revealed that the highest exposure to SHS was in the home setting, with an average of 8.7 ± 2.21 hours daily for both groups. After the educational program, there were significantly lower scores of exposure and a higher score of knowledge in the intervention group, compared to the control group (p < 0.001). There was also a significant decrease in the scores of exposure and a significant increase in the score of knowledge of the intervention group from pretest to posttest (p < 0.001). The study provides evidence about the importance of a program to educate pregnant women about the negative impact of SHS. Nurses need to assess SHS exposure and provide health education for pregnant women.
Insecticide Resistance Profiles and Synergism in Field Populations of the German Cockroach (Dictyoptera: Blattellidae) from Singapore

Abstract:ABSTRACT The resistance profiles of 22 field-collected populations of the German cockroach, Blattella germanica (L.) (Dictyoptera: Blattellidae), from various localities in Singapore were determined by topical bioassay against novel and conventional insecticides from six classes: 1) pyrethroid (beta-cyfluthrin, deltamethrin), 2) carbamate (propoxur), 3) organophosphate (chlorpyrifos), 4) phenyl pyrazole (fipronil), 5) neonicotinoid (imidacloprid), and 6) oxadiazine (indoxacarb). Compared with a laboratory susceptible strain, resistance levels ranged from 3.0 to 468.0× for the pyrethroids, from 3.9 to 21.5× for the carbamate, from 1.5 to 22.8× for the organophosphate, from 1.0 to 10.0× for phenyl pyrazole, and were absent or low for the neonicotinoid (0.8–3.8×) and the oxadiazine (1.4–5.3×). One strain demonstrated broad-spectrum resistance to most of the insecticides. Synergism studies using piperonyl butoxide (PBO) and S,S,S-tributylphosphorotrithioate (DEF) in combination with a discriminating dose (LD99) of selected insecticides were conducted to test for possible resistance mechanisms. Resistance to pyrethroid was reduced with PBO and DEF, suggesting the involvement of P450 monooxygenase and esterases in conferring resistance. Propoxur resistance also was suppressed with PBO and DEF, and coadministration of both synergists resulted in complete negation of the resistance, indicating the involvement of both P450 monooxygenase and esterase. In six B. geimanica field strains evaluated, esterases were found to play a role in chlorpyrifos resistance, whereas the P450 monoxygenase involvement was registered in three strains, Additional resistance mechanisms such as kdr-type and Rdl mutation contributing toward pyrethroid and fipronil resistance, respectively, also may be involved in some strains in which the resistance levels were not affected by the synergists. We conclude that insecticide resistance is prevalent in field German cockroach populations in Singapore.
Associations of Education and Income with Secondhand Smoke Exposure Among Non-smoking Pregnant Women in Japan: The Tohoku Medical Megabank Project Birth and Three-Generation Cohort Study

Abstract:Objectives: There is accumulating evidence that lower socioeconomic groups are more exposed to secondhand smoke (SHS) than higher ones in the general population. The objective of the study was to examine the associations of education and income with SHS exposure among non-smoking pregnant women in Japan. Methods: We analyzed data from 11,615 non-smoking pregnant women in Japan who participated in the Tohoku Medical Megabank Project Birth and Three-Generation Cohort Study from 2013 to 2017. SHS exposure was defined as indoor exposure to someone else's cigarette smoke ≥ 1 day/week during pregnancy. Multiple logistic regression analyses were conducted to examine whether pregnant women's educational attainment or equivalent household income was associated with SHS exposure, adjusting for covariates. Stratified analyses by work status were also conducted. Results: The prevalence of SHS exposure during pregnancy was 32.6%; 27.6% among non-working women and 35.8% among working women. Lower educational attainment was associated with an increased risk of SHS exposure; the odds ratio of high school education or lower compared with university education or higher was 1.87 (95% confidence interval, 1.66-2.10). Lower equivalent household income was associated with an increased risk of SHS exposure; the odds ratio of the lowest compared with the highest level of income was 1.53 (95% confidence interval, 1.35-1.74). These associations did not differ between non-working women and working women. Conclusions for practice: Associations of education and income with SHS exposure imply that educational interventions and financial incentives are needed for pregnant women and their household smokers.

Abstract:Secondhand smoke (SHS) is a major risk factor for poor health outcomes among women in China, where proportionately few women smoke. This is especially the case as it pertains to women's reproductive health, specifically migrant women who are exposed to SHS more than the population at large. There are several factors which may increase migrant women's risk of SHS exposure. This paper aims to investigate the prevalence and associated factors of SHS exposure among internal Chinese migrant women of reproductive age. The data used were derived from the 2014 Chinese Labor Dynamic Survey, a national representative panel survey. The age-adjusted rate of SHS exposure of women of reproductive age with migration experience was of 43.46% (95% CI: 40.73%-46.40%), higher than those without migration experience (35.28% (95% CI: 33.66%-36.97%)). Multivariate analysis showed that participants with a marital status of "Widowed" had statistically lower exposure rates, while those with a status of "Cohabitation" had statistically higher exposure. Those with an undergraduate degree or above had statistically lower SHS exposure. Those with increasing levels of social support, and those who currently smoke or drink alcohol, had statistically higher SHS exposure. Participants' different work-places had an effect on their SHS exposure, with outdoor workers statistically more exposed. Our findings suggest that urgent tobacco control measures should be taken to reduce smoking prevalence and SHS exposure. Specific attention should be paid to protecting migrant women of reproductive age from SHS.

Abstract:Objectives and Methods: A cross sectional study of HCPs working in public MCH clinics in Assiut city was conducted to explore their knowledge, attitudes and counselling practices regarding prevention of SHS exposure among pregnant women and children. Descriptive and regression analyses were performed. Results: 367 HCPs participated in the study, 12% of whom were smokers. The majority were nurses (45%). A considerable proportion of HCPs reported being exposed to SHS in workplace (70%) and home (52%). About half HCP reported high SHS knowledge (56%), supportive attitude towards preventing SHS exposure (53%), and having good counselling practice regarding SHS exposure (52%). Being a GP and serving urban communities were significantly associated with high knowledge. Being female, serving a rural population, receiving training on smoking cessation services, not being exposed to SHS at home, and having a supportive attitude towards prevention of SHS exposure were significantly associated with good counselling practice. Conclusion: Awareness, attitudes and counselling practice of HCPs should be improved. Training for HCPs and enforcement of smoke free polices are needed to improve awareness and facilitate changes in social norms.
Individual and City-Level Determinants of Secondhand Smoke Exposure in China

Abstract:Background: Second hand smoke (SHS) exposure is a severe public health problem, especially in low and middle countries, but no studies have examined both individual and city-level variables influencing exposure.Methods: A cross-sectional multistage sampling design was used to survey subjects from 21 cities in China. Using a standardized questionnaire individual level information was collected. City-level variables were retrieved from the National Bureau of Statistics database. Multilevel logistic regression analysis was used to assess SHS exposure variation at both the individual and city level.Results: SHS exposure prevalence among non-smokers was 28.1 % (95 % CI 27.1-29.0). At the individual level lower educational attainment and income and higher exposure to tobacco advertising were associated with higher SHS exposure. On the other hand richer cities, and those with more anti-smoking media news coverage, had less SHS exposure. The presence of city smokefree regulations was unrelated to exposure.Conclusions: Given its human and economic costs, reducing SHS exposure should receive greater priority than it does in China. The results point to the need for the enactment of national smokefree laws in order to combat unacceptably high levels of SHS exposure.
Preventable Deaths Attributable to Second-Hand Smoke in Southeast Asia—Analysis of the Global Burden of Disease Study 2019

Abstract:Objectives In addition to harms caused to individuals who smoke, second-hand smoke (SHS or passive smoke) is an important public health issue. We aim to estimate the extent of preventable deaths due to tobacco and SHS exposure in Southeast Asia. Methods Data were from the Global Burden of Disease Study 2019. We analysed data from Southeast Asia, including Cambodia, Indonesia, Laos, Malaysia, Maldives, Mauritius, Myanmar, Philippines, Seychelles, Sri Lanka, Thailand, Timor-Leste, and Vietnam. Results In 2019, there were 728,500 deaths attributable to tobacco in Southeast Asia, with 128,200 deaths attributed to SHS exposure. The leading causes of preventable deaths were ischemic heart disease, stroke, diabetes mellitus, lower respiratory infections, chronic obstructive pulmonary disease, tracheal, bronchus, and lung cancer. Among deaths attributable to tobacco, females had higher proportions of deaths attributable to SHS exposure than males in Southeast Asia. Conclusion The burden of preventable deaths in a year due to SHS exposure in Southeast Asia is substantial. The implementation and enforcement of smoke-free policies should be prioritized to reduce the disease burden attributed to passive smoking in Southeast Asia.

Assessment of Urinary Concentration of Cotinine in Chinese Pregnant Women Exposed to Environmental Tobacco Smoke

Abstract:Environmental tobacco smoke (ETS) is a well-known reason of many adverse health consequences in pregnant women. Exposure of ETS during pregnancy may increase the risk of some fetal diseases in pregnant women and/or birth defects in neonates. Many countries have implemented laws to control smoking and free of second hand smoke in all enclosed workplaces. In China, anti-smoking law was enforced in March 2010. We aimed to assess the exposure of pregnant women to cotinine after anti-smoking law was implemented in China. The urine samples were collected from 2,100 volunteers from Zhejiang Province, China and urinary concentration of cotinine was measured using gas chromatography and mass spectrometry. Cotinine was detected in 87 % of the pregnant women. The GM and 95th percentile concentration in pregnant women were 4.28 and 44.00 μg/L, respectively. Similarly, the GM and 95th concentration of cotinine were significantly higher in smoking group than in non-smoking group (P < 0.0001). Our results indicate that pregnant women in China are at the high risk of exposure ETS. Further attempts are required to make strategies to find out the sources of ETS exposure and prevent smoking at public places especially during pregnancy.
Smoked, smokeless, and poly-tobacco use during pregnancy in relation to infant mortality in Cambodia: Findings from a nationwide sample

Abstract:Introduction: Maternal cigarette smoking during pregnancy is an established risk factor for adverse maternal, fetal, and infant outcomes. In contrast, maternal smokeless tobacco use (i.e. e-cigarettes, snus, betel quid, iqmik) during pregnancy has a more complex risk profile due to its potential use as a smoking cessation aid or to reduce the harm from smoking tobacco. The overall aim of this study was to investigate the association between smoked, smokeless, and poly-tobacco (smoked + smokeless) use during pregnancy and infant mortality, in a national sample of women in Cambodia. Methods: The study used data from the National Adult Tobacco Survey of Cambodia (NATSC) that employed sampling methods and tobacco survey items from the CDC Global Adult Tobacco Survey but also included a supplement on reproductive health and birthing history. We selected 5342 women of the NATSC who reported complete data on at least one pregnancy, and our unit of analysis was the 15998 pregnancies from these women. We conducted a multivariable logistic regression to relate tobacco use to infant mortality. Taylor linearized variance estimators were used to account for clustering by sampling unit and mother. Results: We found that smokeless tobacco in the form of a betel quid was the most common form of tobacco used during pregnancy. In multivariable logistic regression, we found increased odds of infant death for all tobacco use categories (smoked, smokeless), but that the strongest effects were seen for habits that included smokeless tobacco (relative to never use of tobacco in any form): exclusive use of smokeless tobacco (adjusted odds ratio, AOR=2.08; 95% CI: 1.15-3.76), and poly-tobacco use (AOR=5.68; 95% CI: 1.03-31.46). In more detailed analyses that considered the composition of the betel quid (tobacco, areca nut/leaf, slaked lime), we found that even chewing of tobacco leaves with no processing or additives was associated with a three-fold increase in odds of infant death relative to a never user (AOR=3.05; 95% CI: 1.45-6.45). Conclusions: We found that even among those pregnant women who limited their nicotine habit to chewing tobacco leaves with no processing or additives, there remained higher odds of fetal or infant death from that pregnancy.

Abstract:A high-calorie nutrient supplement was administered orally prior to celiac angiography in 47 patients. A comparison of pre-and postprandial angiograms was made in 15. The rest of the patients were compared with another group who received nothing orally prior to arteriography. An increased number of patients demonstrated good to excellent opacification of the gastric wall and gastroepiploic vein after the meal. Demonstration of the right gastric artery occurred more often, and there was an increase in vessel diameter in some cases.

Abstract:Objectives: Second-hand smoke (SHS) exposure causes >600,000 deaths annually worldwide, however, information regarding SHS exposure in Lao People’s Democratic Republic (Lao PRD) is limited; we report SHS exposure prevalence at home, inside workplaces, and indoor public spaces in Lao PDR. Methods: Data were from the 2015 Lao National Adult Tobacco Survey, a nationally representative sample of 7,562 participants aged ≥15 years recruited through a stratified 2-stage cluster sampling approach. Results: 88.3% (83.9% of non-smokers) reported SHS exposure at home and 63.0% (54.0% of non-smokers) at workplaces. Among non-smokers, women had greater exposure at home than men (86.6 vs. 77.0%). Lower education levels were associated with exposure at home or the workplace. 99.2% reported SHS exposure at any public place; specifically for restaurants/food stores 57.7%, government offices 56.2%, public transport 31.6%, and health care facilities 11.7%. Conclusion: SHS exposure at home and workplace in Lao PDR is among the highest in South-East Asia. Comprehensive smoke-free policies at government-owned workplaces and facilities, stricter enforcement of these smoke-free policies, and strategies to encourage smoke-free environments at homes and in public places are urgently needed.

Abstract:INTRODUCTION: Currently, 40.7% of women in China are exposed daily to secondhand smoke (SHS); however, research on SHS exposure among women in Inner Mongolia is limited. We aimed to investigate the prevalence of SHS exposure, knowledge about the harms of smoking and SHS, and the association between sociodemographic factors and SHS exposure among never-smoking women in Inner Mongolia, Northern China.METHODS: This study was based on a survey of chronic disease and nutrition conducted among Chinese adults in Inner Mongolia during 2015, a cross-sectional study with multi-stage stratified cluster sampling. A total 2293 never-smoking women aged ≥18 years were included in the study. Face-to-face interviews were used to collect data of sociodemographic characteristics, SHS exposure, and related knowledge. Questionnaires were administered by trained investigators, and the reliability and validity of the questionnaires were high. We performed descriptive analysis and logistic regression. All analyses were performed by IBM SPSS Statistics version 19.0.RESULTS: Of the total, 69% of young women in Inner Mongolia in 2015 reported that they were exposed to SHS, the highest rate in comparison with older and middle-aged women. A total 49.90% of young women reported being exposed to SHS every day. Respondents had insufficient knowledge regarding smoking and SHS exposure. Compared with older never-smoking women, their young counterparts had a higher risk of SHS exposure, with an odds ratio of 2.143 (95% CI: 1.647-2.787). Education level and ethnicity were also significantly associated with SHS exposure.CONCLUSIONS: This study showed that the highest rates of SHS exposure were among young never-smoking women in Inner Mongolia, and women with high education levels were less likely to be exposed to SHS. Improved public health information is needed that prioritizes SHS exposure among young women in Inner Mongolia. Health education regarding SHS exposure should be widely implemented throughout communities in this region.
