Abstract:In this paper we investigate the use of Time Reversal applied to Impulse Radio Ultra Wide Band (IR-UWB) systems with multi user interference (MUI). It is known that the MUI distribution is often not Gaussian in classical IR-UWB. We show how the Time Reversal technique has an impact on the distribution of the multi-user interference (MUI) by making it even less Gaussian. So we show how performance may benefit from this MUI distribution change.

Abstract:Background Primary gleno-humeral osteoarthritis is associated with both excessive posterior humeral subluxation (PHS) and excessive glenoid retroversion in 40% of cases. These morphometric abnormalities are a particular issue since they may be responsible for a deterioration in long term clinical and radiological outcomes. The aim of this study was to perform a CT-analysis of patients who underwent Total Shoulder Arthroplasty (TSA) for primary osteoarthritis (OA) with B2-, B3- or C-type glenoids in whom an attempt was made to correct for excessive glenoid retroversion and excessive posterior humeral subluxation intraoperatively. Material We performed a retrospective, single-center study including 62 TSA patients with a pre-operative PHS of the glenohumeral joint (31 men, 31 women, 70±9 years) between January 2000 and January 2014. Glenoids were classified as B2 (32 cases), B3 (13 cases) or C (17 cases). Glenoid retroversion was corrected by anterior asymmetric reaming. Patients were reviewed for clinical and CT-scan assessment with a mean follow-up of 8.3 years (minimum 5 years). At final follow-up the CT-images were reconstructed in a scapular plan. A PHS index of >65% defined persistence. Results The revision free rate was estimated at 93%. Correlation between PHS and retroversion was moderate preoperatively (ρ=0.58) and strong at final follow-up (ρ=0.73). Postoperative CT-scans on average showed a surgical correction of PHS compared to preoperatively (79% vs 65% respectively, p<0.05) and retroversion (20° vs 10° respectively, p<0.05). At final follow-up, 25/62 patients had a persistence in 2D model and 41/62 in 2D corrected model. Persistence of PHS had no influence on clinical outcomes but did demonstrate a significantly higher glenoid loosening rate (20% vs 59%, p<0.05). Conclusion Correlation between PHS and retroversion was moderate preoperatively and strengthened at long-term follow up. Anterior asymmetric reaming allowed for a surgical improvement of both PHS and retroversion, but it was not sufficient to maintain a correction over time. Glenoid loosening was more frequent in case of PHS persistence but seemingly without clinical relevance.

Abstract:Background: Multiple modifications of reverse total shoulder arthroplasty (RTSA) since the first Grammont design have developed to improve range of motion (ROM) and avoid notching. The effect of these changes in shoulder kinematics and the best compromise for ROM is still under debate. This computer simulation study evaluates the influence of humeral design, humeral neck-shaft angle (NSA), glenoid lateralization, and glenoid eccentricity on ROM of RTSA. Methods: We created a 3-dimensional computer model from computed tomography scans of 13 patients with primary osteoarthritis simulating implantation of a standardized reverse shoulder arthroplasty. We analyzed the effect of 4 different variables on impingement-free ROM: humeral design (inlay vs. semi-inlay vs. onlay), humeral NSA (135° vs. 145° vs. 155°), glenoid lateralization, and glenoid eccentricity on ROM. Results: The use of different humeral stem designs did not have a significant effect on total global ROM. Reducing NSA demonstrated a significant increase in adduction, and external and internal rotation in adduction, whereas a decrease in abduction and external rotation in abduction. Glenosphere lateralization was the most effective method for increasing total global ROM (P < .0001); however, extreme lateralization (+12 mm) did not show significant benefit compared with moderate lateralization (+4 mm). Glenosphere eccentricity increased only adduction and internal rotation in adduction. Conclusion: Only glenoid lateralization has a significant effect on increasing total global ROM in RTSA. The use of the semi-inlay 145° model combined with 4 mm lateralization and 2 mm inferior eccentricity represents the middle ground and the most universal approach in RTSA.
Humeral position after reverse shoulder arthroplasty as measured by lateralization and distalization angles and association with acromial stress fracture; a case-control study

Abstract:BackgroundReverse Shoulder Arthroplasty (RSA) has become increasingly utilized in the surgical management for a growing number of indications in increasingly younger and active populations. The complication of Acromial Stress Fracture (ASF) has been described in various patient groups after RSA. Patients who experience ASF after RSA report worse clinical outcomes. Biomechanical studies and some clinical studies have implicated the degree of lateralization and distalization of the humerus as factors associated with development of ASF. Most prior studies utilize scalar or linear methods to measure humeral position following RSA. These scalar measurements do not account for the relative effects of variances in patient stature. Lateralization Shoulder Angle (LSA) and Distalization Shoulder Angle (DSA) are angular methods that have been described as a reproducible assessment of humeral position while controlling for variance in patient stature.MethodsA case-control study was performed where cases of ASF after RSA were identified after a retrospective review of all reverse shoulder arthroplasty cases at a single institution using a single implant type. A 1:3 case-control matching scheme was implemented based on age and gender. All surgeries were performed by one of four fellowship-trained Shoulder and Elbow surgeons. Preoperative and Postoperative radiographs were assessed and humeral position was measured using LSA and DSA. A Mixed Linear model was utilized to test the difference in means between cases and controls.ResultsBetween January 2012 and December 2015, 689 patients underwent primary RSA. 29 patients (4.2%) sustained acromial stress fracture. Acromial stress fracture occurred at a mean of 7.6 months after surgery (range 1.0-36.8 months). According to the Levy classification, 19 were type 1 fractures, 9 were type 2 and 1 was type 3. An additional 24 patients (3.5%) patients were noted to have experience an acromial stress reaction without evidence for fracture. Mean change in LSA (from preoperative to postoperative) was -19.78 o (SE 2.82) in cases (net medialization) and -12.18 o (SE 1.63) in controls (p=0.02). The mean change in DSA was 36.40 o (SE 2.09) in cases and 31.82o (SE 1.21) in controls (p=0.06)ConclusionHigher preoperative LSA (humeral lateralization) and greater decrease of LSA (greater net medialization) after surgery were noted to have an association with ASF. Humeral position after RSA as measured by DSA was not associated with radiographically evident ASF in this case-control study controlling for age and gender.

Abstract:Background Optimal glenosphere positioning in a lateralized reverse shoulder arthroplasty (RSA) to maximize functional outcomes has yet to be clearly defined. Center of rotation (COR) measurements have largely relied on AP radiographs which allow assessment of lateralization and inferior position, but ignore scapular Y radiographs which may provide an assessment of posterior and inferior position relative to the acromion. The purpose of this study was to evaluate the COR in the sagittal plane and assess the effect of glenosphere positioning with functional outcomes utilizing a 135° inlay stem with a lateralized glenoid. Methods A retrospective review was performed on a prospectively maintained multicenter database on patients who underwent primary RSA from 2015-2021 with a 135° inlay stem. The COR was measured on minimum 2-year postoperative sagittal plain radiographs using a perfect-circle fit method. A perfect circle was made on the glenosphere and the center was marked. From there, four measurements were made: 1) center to the inner cortex of the coracoid, 2) center to the inner cortex of the anterior acromion, 3) center to the inner cortex of the middle acromion, 4) center to the inner cortex of the posterior acromion. Regression analysis was performed to evaluate any association between the position of the COR relative to bony landmarks with functional outcomes. Results A total of 136 RSAs met the study criteria. There was no relation with any of the distances with outcome scores (ASES, VAS). In regards to range of motion (ROM), each distance had an effect on at least one parameter. The COR to coracoid distance had the broadest association with ROM with improvements in forward flexion (FF), external rotation (ER0), and internal rotation with arm at 90° (IR90) ( p = <0.001, 0.031, <0.001; respectively). The COR to coracoid distance was also the only distance to affect the final FF and IR90. For every 1 mm increase in this distance, there was a 1.8° increase in FF and 1.5° increase in IR90 (ß = 1.78; 95% CI 0.85 – 2.72, p = <0.001, ß = 1.53; 95% CI 0.65 – 2.41, p = <0.001; respectively). Conclusion Evaluating the COR following RSA in the sagittal plane suggests that posteroinferior glenosphere position may improve ROM when using a 135° inlay humeral component and a lateralized glenoid.

Abstract:Indications for Reverse Shoulder Arthroplasty (RSA) have been extended over the last 25 years, and RSA has become the most frequently implanted shoulder arthroplasty worldwide. The initial Grammont design with medialization of the joint center of rotation (JCOR), placement of the JCOR at the bone–implant interface, distalization and semi-constrained configuration has been associated with drawbacks such as reduced rotation and range of motion (ROM), notching, instability and loss of shoulder contour. This review summarizes new strategies to overcome these drawbacks and analyzes the use of glenoid-sided, humeral-sided or global bipolar lateralization, which are applied differently by surgeons and current implant manufacturers. Advantages and drawbacks are discussed. There is evidence that lateralization addresses the initial drawbacks of the Grammont design, improving stability, rates of notching, ROM and shoulder contour, but the ideal extent of lateralization of the glenoid and humerus remains unclear, as well as the maximal acceptable joint reaction force after reduction. Overstuffing and spine of scapula fractures are potential risks. CT-based 3D planning as well as artificial intelligence will help surgeons with planning and execution of appropriate lateralization in RSA. Long-term follow-up of lateralization with new implant designs and implantation strategies is needed.

Abstract:Background While forward elevation, abduction, and external rotation are often successfully restored, internal rotation (IR) is frequently not satisfactorily recovered following reverse shoulder arthroplasty (RSA), affecting patients' ability to perform certain daily activities. This study examined the prevalence of limited IR in patients receiving modern RSA and its impact on clinical outcomes and patient satisfaction with their RSA. Methods A cross-sectional study was conducted on patients who had undergone primary RSA using a modern RSA prosthesis with a minimum one-year follow-up. Patients returned for a research visit where they were examined for range of motion and surveyed for patient-reported outcome measures (PROMs) and subjective rating of the success level of their RSA. Postoperative range of motion data were compared with the preoperative data. The relationship of postoperative IR with PROMs and patients' subjective rating of RSA was analyzed. Logistic regression was performed to identify factors affecting patients' subjective rating of RSA. Results A total of 78 patients met the criteria and completed the study. A significant portion (59%) reported subjective IR limitations, and 41% showed objective IR limitations. Comparison of preoperative and postoperative IR showed that 24% had worsened IR, while 33% improved. Limited IR was associated with lower patients' subjective rating of RSA and negatively impacted PROMs, especially when the dominant side was operated on. Regression analysis showed that limited IR was the only independent determining factor that was significantly associated with lower subjective rating of RSA. Discussion Despite advancements in RSA design, limited IR remains a prevalent issue, significantly affecting patients' satisfaction and clinical outcomes. This study underscores the need for setting appropriate expectations regarding IR through patient education and focused efforts to improve IR limitations in RSA patients, particularly on the dominant side.

Abstract:Objective To evaluate the early outcomes and risk factors of reverse shoulder arthroplasty (RSA) in patients with rheumatoid arthritis (RA). Methods A retrospective study was performed on RA patients who had undergone RSA between January 2016 and January 2018. Preoperative glenohumeral joint damage was evaluated according to two radiographic classification systems. The severity of joint damage was estimated using Larsen's method, while the Levigne‐Franceschi method was used to assess the type of destruction. Further, we recorded intra‐ and postoperative complications. Visual Analogue Scale (VAS) was used to assess the degree of shoulder pain while shoulder function was evaluated with the American Shoulder and Elbow Surgeons (ASES) Shoulder Score. In addition, patients' subjective outcome and range of shoulder motion were recorded. Radiographs were taken and examined during the follow‐up period. Paired t‐test was used to determine the difference in measurement data between preoperative and the last follow‐up. VAS was analyzed using the Wilcoxon matched‐pairs signed‐rank test. Results A total of 14 patients with 14 shoulders were included. All the patients were female with an average age of 60.29 years (range, 49–71 years) at the time of surgery and an average RA disease duration of 24.57 years (range, 5–40 years). Seven of the 14 patients had a history of joint surgery related to RA. Meanwhile, 11 of the 14 shoulders showed glenoid bone defect, and eccentric reaming was performed intraoperatively to avoid base plate malposition. The mean follow‐up period for the 14 patients was 2.76 years (range, 2–4 years). The mean VAS decreased from a value of 5.71 ± 1.10 preoperatively to 1.36 ± 0.61 postoperatively (P < 0.001). On the contrary, the ASES score showed an increase from 33.93 ± 6.89 to 76.67 ± 5.23 (P < 0.001). An increase in active forward elevation, abduction, and external rotation with the arm in 90° of abduction from 85.71° ± 17.61°, 77.14° ± 19.43°, and 17.14° ± 10.97° to 126.43° ± 5.23°, 106.42° ± 11.72°, and 38.57° ± 14.57°, respectively, was observed (P < 0.001). Subjective outcome assessment showed that 13 of the 14 patients were very satisfied or satisfied with the operation, while one patient was uncertain due to co‐existing ipsilateral elbow lesion. Notably, one patient acquired a humeral periprosthetic fracture during the operation. In this study, no major complications such as periprosthetic joint infection and dislocation or implant loosening were observed. Further, no patients underwent revision for any reason at the end of the follow‐up. Conclusions RSA could achieve good early outcomes without high complication rates in patients with RA. Glenoid bone defects and adjacent joints involvement were common in this patient group, which might increase the risk of surgery and affect postoperative satisfaction.
Augmented Reality Assisted Intraoperative Navigation Increases Precision of Glenoid Inclination in Reverse Shoulder Arthroplasty

Abstract:Background: Reverse Total Shoulder Arthroplasty (RTSA) is the standard of care for patients with glenohumeral osteoarthritis and rotator cuff deficiency. Preoperative RTSA planning based on medical images and patient-specific instruments has been established over the last decade. This study aims to determine the effects of using augmented reality assisted intraoperative navigation (ARIN) for baseplate positioning in RTSA compared to preoperative planning. It is hypothesized that ARIN will decrease deviation between preoperative planning and postoperative baseplate positioning. Moreover, ARIN will decrease deviation between the (senior) more (> 50 RTSA/year) and less experienced (junior) surgeon (5-10 RTSA/year). Methods: Preoperative CT scans of sixteen fresh-frozen cadaveric shoulders were obtained. Baseplate placements were planned using a validated software. The data was then converted and uploaded to the augmented reality system (NextAR; Medacta International, Castel San Pietro, Switzerland). Each of the eight RTSAs were implanted by a senior and a junior surgeon, with four RTSAs utilizing ARIN and four without. A postoperative CT scan was performed in all cases. The scanned scapulae were segmented, and the preoperative scan was laid over the postoperative scapula by nearest iterative point cloud analysis. The deviation from the planned entry point and trajectory was calculated regarding the inclination, retroversion, medialization (reaming depth)/lateralization, antero-posterior position and superior/inferior position of the baseplate. Data are reported as mean ± standard deviation (SD) or mean and 95% confidence interval (CI). P values <0.05 were considered statistically significant. Results: The use of ARIN yielded a reduction in the absolute difference between planned and obtained inclination from 9° (SD: 4°) to 3° (SD°:2) (p=0.011). Mean difference in planned-obtained inclination between surgeons was 3° in free-hand surgeries (95%CI: -4,10, p=0.578), while this difference reduced to 1° (95%CI: -6, 7, p=0.996) using ARIN. Retroversion, medialization (reaming depth)/lateralization, antero-posterior position and superior/inferior position of the baseplate were not affected by using ARIN. Surgical duration was increased using ARIN for both the senior (10 mins) and junior (18 mins) surgeon. Conclusions: The implementation of augmented reality assisted intraoperative navigation (ARIN) leads to greater accuracy of glenoid component placement, specifically with respect to inclination. Further studies have to verify if this increased accuracy is clinical important. Furthermore, ARIN allows less experienced surgeons to achieve a similar level of accuracy in component placement comparable to more experienced surgeons. However, the potential advantages of ARIN in RTSA are counterbalanced by an increase in operative time.
Does computerized CT-based 3D planning of the humeral head cut help to restore the anatomy of the proximal humerus after stemless total shoulder arthroplasty?

Abstract:BackgroundRestoration of proximal humeral anatomy (RPHA) after total shoulder arthroplasty (TSA) has been shown to result in better clinical outcomes than is the case in non-anatomical humeral reconstruction. Preoperative virtual planning has mainly focused on glenoid component placement. Such planning also has the potential to improve anatomical positioning of the humeral head by more accurately guiding of the humeral head cut, and selection of anatomical humeral component sizing. Hypothesis of the study: The use of preoperative 3D planning helps to reliably achieve RPHA after stemless TSA.MethodsOne hundred consecutive stemless TSA (67 males, 51 right shoulder, mean age of 62 ±9.4 years) were radiographically assessed using pre- and postoperative standardized AP radiographs. The RPHA was measured with the so-called circle method described by Youderian et al. We measured deviation from the premorbid center of rotation (COR), and more than 3mm was considered as minimal clinical important difference. Additionally, pre- and postoperative humeral head diameter (HHD), head neck angle (HNA) and humeral head height (HHH) were measured to assess additional geometrical risk factors for poor RPHA.ResultsThe mean distance from of the premorbid to the implanted head COR was 4.3mm ± 3.1mm. Thirty five shoulders (35%) showed a deviation of less than 3mm (mean 1.9, ±1.1) and 65 shoulders (65%) a deviation of ≥3mm (mean 8.0 ± 3.7). Overstuffing was the main reason for poor RPHA (88%). The level of the humeral head cut was responsible for overstuffing in the 46 of the 57 overstuffed cases. The preoperative HHD, HHH and the HNA were significantly larger, higher and more in valgus angulation in the group with accurate compared to the group with poor RPHA (HHD of 61.1mm ±4.4 vs 55.9 ±6.6, p<0.001; HHH 8.6±2.2 vs 7.6±2.6 p=0.026, varus angulation of 134.7° ±6.4° vs. 131.0° ±7.91, p=0.010).ConclusionRestoration of proximal humeral anatomy after stemless TSA using CT-based 3D planning was not precise. A poorly performed humeral head cut was the main reason for overstuffing which was seen in 88% of the cases with inaccurate RPHA. Preoperative small HHD, low HHH and varus angulated HNA are risk factors for poor RPHA after stemless TSA.Clinical SignificanceWhile Preoperative CT-based 3-D planning gives insight into the proximal humeral anatomy, execution of this insight through visual based surgery without guides or navigation, does not appear to increase accuracy of RPHA.Level of evidenceLevel IV; Case Series; Treatment Study

Abstract:Avian leukosis virus, subgroup J (ALV-J), has a wide host range, preferentially infecting meat-type birds, and produces a high incidence of myelocytomatosis and nephromas. Using the published sequences from HPRS-103 (ALV-J isolated in 1989 in Great Britain), we designed a set of PCR primers that amplified proviral DNA from nine U.S. field samples. The primers were specific for ALV-J, not amplifying DNA from uninfected cells or cells infected with ALV subgroups A-E. These primers expanded a 2.4-kb fragment that encompasses gp85, gp37, the E element, and most of the 3' LTR. We also developed a set of PCR primers that amplified a 2.1-kb fragment from ALV-J-infected cells and a 1.6-kb fragment from uninfected ev- chicken embryo fibroblasts (Line 0). Upon cloning and DNA sequencing, we determined that the 2.1- and 1.6-kb fragments contained ALV-J gp85- and gp37-like sequences. Comparison of the amino acid sequences demonstrated that the Line 0 sequences were 97.5% identical with the gp85 and gp37 of HPRS-103 and somewhat less identical with the other nine U.S. isolates. This suggests that the envelope genes of ALV-J may have arisen as a result of a recombination event between exogenous ALV and Line 0-like sequences in the chicken. Phylogenetic analysis also showed that the U.S. field isolates were closely related to one another and more distantly related to the European HPRS-103. The pattern of mutations in the U.S. field isolates suggests that the U.S. strains are slowly drifting away from their progenitor Line 0-like sequences. The development of effective vaccines and diagnostic tests is likely to become more problematic as the viruses continue to mutate.
Accuracy of reverse shoulder arthroplasty angle according to the size of the baseplate

Abstract:Background: Glenoid inclination must be assessed precisely during preoperative planning for reverse shoulder arthroplasty (RSA) to position the glenoid baseplate correctly. We hypothesized that a more dynamic measurement method would better match the diversity of glenoid heights in the population and the variety of commercialized glenoid baseplates. Our purpose was to describe a new method to measure the RSA angle accounting for the baseplate size. Methods: Computed tomography scans of 50 shoulders that underwent RSA for primary osteoarthritis or cuff tear arthropathy between June 2019 and February 2020 were included (mean age, 76 years). Three variants of the RSA angle were measured: the RSA angle as originally described by Boileau et al, the relative RSA 25 angle (which simulates the implantation of a 25-mm baseplate), and the relative RSA 29 angle (which simulates the implantation of a 29-mm baseplate). Measurements in the 2-dimensional true reformatted scapular plane were made by 3 independent operators. Results: The mean R-S distance (ie, distance between point R [intersection of supraspinatus fossa line with glenoid surface] and point S [inferior border of glenoid]) was 24.2 ± 4.0 mm. The mean RSA angle was 20.3° ± 8.4°, whereas the mean relative RSA 25 angle was 19.3° ± 7.8° and the mean relative RSA 29 angle was 15.6° ± 7.6°. The mean difference between the RSA angle and the relative RSA 25 angle was 1.0° ± 4.1° (P = .16). The mean difference between the RSA angle and the relative RSA 29 angle was 4.7° ± 3.8° (P < .0001). In half of the shoulders in our series, the difference between the RSA angle and the RSA 29 angle exceeded 5°. Conclusion: The RSA angle is a reproducible measure of the inclination of the inferior part of the glenoid that is reliable in most cases for glenoid baseplates of 24-25 mm in height. However, surgeons should be aware that the RSA angle may overestimate the superior orientation of the inferior glenoid for baseplates of different sizes or for small- or large-stature patients. In these cases, the relative RSA angle adapted to the size of the baseplate more accurately evaluates the inclination of the inferior glenoid.

Abstract:Background: Reversed total shoulder arthroplasty (RTSA) is an established surgery for many pathologies of the shoulder and the demand continues to rise with an aging population. Preoperative planning is mandatory to support the surgeon's understanding of the patient's individual anatomy and, therefore, is crucial for the patient's outcome. Methods: In this observational study, we identified 30 patients who underwent RTSA with two- and three-dimensional preoperative planning. Each patient underwent new two-dimensional planning from a medical student and an orthopedic resident as well as through a mid-volume and high-volume shoulder surgeon, which was repeated after a minimum of 4 weeks. The intra- and interobserver reliability was then analyzed and compared to the 3D planning and the implanted prosthesis. The evaluated parameters were the size of the pegged glenoid baseplate, glenosphere, and humeral short stem. Results: The inter-rater reliability showed higher deviations in all four raters compared to the 3D planning of the base plate, glenosphere, and shaft. The intra-rater reliability showed a better correlation in more experienced raters, especially in the planning of the shaft. Conclusions: Our study shows that 3D planning is more accurate than traditional planning on plain X-rays, despite experienced shoulder surgeons showing better results in 2D planning than inexperienced ones.

Abstract:Background Reverse total shoulder arthroplasty (RSA) has been increasingly utilized for a variety of shoulder pathologies that are difficult to treat with anatomical shoulder arthroplasty (TSA). Few studies have compared the outcomes of TSA versus RSA in patients with cuff intact GHOA and poor preoperative forward elevation. This study aimed to determine whether there is a difference in functional outcomes and postoperative ROM between TSA and RSA in these patients. Methods This retrospective cohort study included 116 patients who underwent RSA or TSA between 2013 and 2022 for the treatment of rotator cuff intact primary osteoarthritis with restricted preoperative forward flexion (FF) and a minimum 1-year follow-up. Each arthroplasty group was divided into two subgroups: patients with preoperative FF between 91 and 120 degrees or FF lower than or equal to 90 degrees. Patients' clinical outcomes, including active range of motion (ROM), American Shoulder and Elbow Surgeons (ASES) score, Visual analog scale (VAS) for pain, and Subjective Shoulder Value (SSV) were collected. Clinical and radiographic complications were evaluated. Results There was no significant difference between RSA and TSA in terms of sex (58.3% male vs 62.2% male, p=0.692), or follow-up duration (20.1 months vs 17.7 months, p=0.230). However, the RSA cohort was significantly older (72.0± 8.2 vs 65.4 ± 10.6, p=0.012) and weaker in FF and ER before surgery (p<0.001). There was no difference between RSA (57 patients) and TSA (59 patients) in VAS pain score (1.2 ± 2.3 vs 1.3 ± 2.3, p=0.925), SSV score (90 ± 15 vs 90 ± 15, p=0.859) or ASES score (78.4 ± 20.5 vs 82.1 ± 23.2, p=0.476). Postoperative active ROM was statistically similar between RSA and TSA cohorts in FF (145 ± 26 vs 146 ± 23, p= 0.728), and ER (39 ± 15 vs 41 ± 15, p= 0.584). However, IR was lower in the RSA cohort (p <0.001). This was also true in each subgroup. RSA led to faster postoperative FF and ER achievement at 3 months (p<0.001). There was no statistically significant difference in complication rates between cohorts. Conclusion This study demonstrates that patients with GHOA who have a structurally intact rotator cuff but limited preoperative forward elevation can achieve predictable clinical improvement in pain, range of motion, and function after either TSA or RSA. Reverse arthroplasty may be a reliable treatment option in patients at risk for developing rotator cuff failure.

Abstract:Background: Recently, the issue of subacromial notching, caused by acromial impingement has been reported. The purpose of this study was to assess the impact of differences in the distance between the glenosphere center and the greater tuberosity (DGT) and the distance between the glenosphere center and the acromion (DA) on the closest distance between the greater tuberosity and the acromion during active abduction in shoulders with reverse total shoulder arthroplasty (RSA). Methods: Eleven shoulders with semiinlay RSA were analyzed. Subjects underwent fluoroscopy during active scapular plane abduction. Computed tomography of their shoulders was performed to create three-dimensional (3D) implant models at a mean of 16 months after surgery. Using model-image registration techniques, poses of 3D implant models were iteratively adjusted to match their silhouettes with the silhouettes in the fluoroscopic images (shape matching), and 3D kinematics of implants were computed. The closest distance between the acromion and greater tuberosity was computed at maximum abduction. DA and DGT were measured from 3D surface models. Shoulders were divided into two groups based on DA and DGT measurements and their closest distance data were compared between the groups. Results: There were 7 shoulders with DA ≥ DGT, and 4 shoulders with DA < DGT. Shoulders with DA ≥ DGT showed a significantly wider distance between the greater tuberosity and acromion at maximum abduction compared to those with DA < DGT (5.9 ± 2.4 mm vs. 0.6 ± 0.7 mm, respectively, P = .0021). There were no significant differences in maximum glenohumeral abduction angle and humeral abduction angle between the two groups. Although DA was significantly greater in shoulders with DA ≥ DGT than in those with DA < DGT (43.7 ± 4.4 mm vs. 35.1 ± 6.7 mm, respectively, P = .0275), there was no significant difference in DGT between the two groups. Conclusion: When DGT is less than DA in shoulders with RSA, the closest distance between the greater tuberosity and the acromion at maximum abduction is significantly wider compared to cases where DGT is greater than DA by 3D measurement. Therefore, acromial impingement is less likely to occur in shoulders with RSA when DA is greater than DGT. To avoid acromial impingement, it might be important to make DA greater than DGT.

Abstract:Background Optimal glenosphere positioning in reverse shoulder arthroplasty (RSA) remains highly debated. We aimed to characterize the association between the prosthesis scapular neck angle (PSNA) and postoperative range of motion (ROM) and clinical outcome scores. Methods A retrospective review of 284 RSAs using a single design with minimum 2-year follow-up was performed. Glenosphere tilt was measured postoperatively using PSNA. ROM and functional outcome scores were assessed preoperatively and at latest follow-up. The PSNA was dichotomized to inferior or superior groups (>90° vs ≤ 90°, respectively) and stratified into quartiles; ROM and outcome score measures were compared between groups controlling for inferior glenosphere overhang. Results No range of PSNA was consistently associated with superior ROM, clinical outcome scores, or rates of complications including scapular notching. However, greater preoperative to postoperative improvement in active FE was found for superiorly (PSNA ≤ 90°) versus inferiorly tilted glenospheres (37° ± 33° vs 53° ± 35°, P = 0.005) and the greater improvement in active FE (57° ± 35°, P = 0.004) was further isolated to the first quartile (mean 85.1° ± 3.5°). Discussion A lack of variation in clinical outcomes based on PSNA suggests negligible short-term (median follow-up 3.1 years) clinical significance when glenosphere tilt falls within the distribution of this cohort (mean 92.6° ± 6.2°).
