Abstract:BACKGROUND:Whether to prophylactically irradiate the ipsilateral internal mammary chain (IMC) in post-mastectomy radiotherapy (PMRT) remains controversial because of equivocal clinical benefits against the added toxicities. Our previous study revealed that the cardiac dose was decreased during left-sided breast radiotherapy with abdominal deep inspiration breath-hold (aDIBH) as compared with free-breathing (FB) and thoracic deep inspiration breath-hold (tDIBH). Here we present the dosimetric advantage of aDIBH for patients undergoing PMRT with IMC coverage.METHODS:We prospectively analyzed 19 patients with left-sided breast cancer who underwent PMRT. Patients underwent computed tomography (CT) simulation under both free-breathing (FB) and aDIBH. The heart, left anterior descending coronary artery (LAD), lungs, and the contralateral breast was defined as organs at risk (OARs). Three-dimensional conformal radiation therapy (3D-CRT), inverse planning intensity-modulated radiation therapy (IMRT), and volumetric modulated arc therapy (VMAT) were used to calculate the doses received by both the planning target volume (PTV) and OARs, which were compared using the Wilcoxon signed-rank test.RESULTS:Compared with FB, the Dmean of the heart and LAD were respectively reduced by 3.5 Gy (P<0.003) and 8.9 Gy (P<0.001) in 3D-CRT, 2.6 Gy (P<0.001), and 7.8 Gy (P=0.001) in IMRT, 1.5 Gy (P<0.001) and 4.5 Gy (P=0.001) in VMAT plans under aDIBH. Among all these plans, the Dmean of the heart was lowest in aDIBH IMRT and 1.3 Gy lower than in aDIBH VMAT (P=0.002). aDIBH IMRT also resulted in a significantly reduced dose to the ipsilateral lung than plans under FB (P<0.05). Dmean and V5 to the contralateral lung and breast were higher in VMAT plans (P<0.05).CONCLUSIONS:Using an immobilization-assisted aDIBH technique, radiation doses to the heart can be kept at reasonably low levels even if IMC is included in the clinical target volume (CTV). Among 3D-CRT, IMRT, and VMAT plans, IMRT plus aDIBH results in the best heart-sparing effect. We recommend that the aDIBH technique be routinely applied in suitable patients if the IMC is irradiated.

Abstract:Objective To compare the dose distribution of reserved planned tangential beam IMRT(2-field IMRT) with multifields beams IMRT(6-field IMRT) of the bulk-recurrent chest wall in postmastectomy breast cancer patients.Methods For 8 patients with chest wall in postmastectomy breast cancer bulk-recurrence,2-field IMRT and 6-field IMRT plans were generated on PTV in Pinnacle Planning System.The prescribed dose of PTV was 50 Gy in 25 fractions and GTV was 66-70 Gy which irradiated incrementally by subsequent plan in 8-10 fractions.The conformal index (CI) and homogeneity index (HI) of 95％ of prescribed dose over PTV were compared,while the dose distribution on ipsilateral lung and heart were evaluated.Results The CI of PTV by 6-field IMRT (0.66 ± 0.08) was higher than that of the2-field IMRT (0.53±0.10)(t=7.99,P＜0.05).The HI ofPTV by6-field IMRT (1.36±0.08)waslower than that of 2-field IMRT (2.19 ±0.78) (t =9.04,P ＜0.05).There was no statistical difference in V5,V10,V20 and V35 for ipsilateral lung and in D V35 and D for heart between two plans.Conclusions Compared with 2-field IMRT,6-field IMRT might have a better dose distribution on planning target volume(PTV) for chest wall in postmastectomy breast cancer bulk-recurrence patients,butthere is no significant difference in dose-volume of ipsilateral lung and heart between two plans.

Abstract:PurposeHypofractionated radiotherapy of prostate cancer is gaining clinical acceptance given its potential increase in therapeutic ratio and evidence for noninferiority and lack of added late toxicities compared to conventional fractionation. However, concerns have been raised that smaller number of fractions might lead to larger dosimetric influence by interfractional motion. We aim to compare the effect of these variations on hypofractionated pencil beam scanning (PBS) proton therapy and volumetric modulated arc therapy (VMAT) for localized prostate cancer.MethodsWeekly CT images were acquired for 6 patients participating in a randomized clinical trial. PBS plans featuring bilateral (BL) and a combination of lateral and anterior-oblique beams (AOL), and VMAT plans were created. All patients were treated to a conventional 79.2Gy total dose in 44 fractions. For this study, hypofractionated dose to the prostate gland was 51.6Gy in 12 fractions or 36.25Gy in 5 fractions, and 32.8, and 23.1Gy to proximal seminal vesicles, respectively. Patients were simulated with endorectal balloons to aid gland immobilization. Three fiducial markers were implanted for setup guidance. All plans were recomputed on the weekly CT images after aligning with the simulation CT. The entire set of 9 CT images was used for dose recalculation for 12-fraction and only 5 used for the 5-fraction case. Adaptive range adjustments were applied to anterior-oblique beams assuming clinical availability of invivo range verification. Fractional doses were summed using deformable dose accumulation to approximate the delivered dose. Biologically equivalent dose to 2Gy(EQD2) was calculated assuming / of 1.5Gy for prostate and 3Gy for bladder and rectum.ResultsThe median delivered prostate D-98 was 0.13/0.14/0.13Gy(EQD2) smaller than planned for PBS-BL, 0.13/0.27/0.17Gy(EQD2) for PBS-AOL and 0.59/0.66/0.59Gy(EQD2) for VMAT, for 44/12/5 fractions, respectively. The largest D-98 reduction was 1.5 and 3.5Gy(EQD2) for CTV1 and CTV2, respectively. Target dose degradation was comparable for all fractionation schemes within each modality. The maximum increase in rectum D-2 was 0.98Gy(EQD2) for a 5-fraction PBS case.ConclusionsThe robustness of PBS and VMAT were comparable for all patients for the studied fractionations. The delivered target dose generally remained within clinical tolerance and the deviations were relatively minor for both fractionation schemes. The delivered OAR dose stayed in compliance with the RTOG hypofractionation constraints for all cases.

Abstract:In this paper, we compare the performance of the different Model Predictive Control (MPC) approaches for power electronic and drive systems: Finite Control Set (FCS) MPC directly applies the voltage vectors and Continuous Control Set (CCS) MPC uses voltage modulation (PWM, SVM). A unified system formulation is proposed to design both FCS-MPC and CCS-MPC for three-phase resistive-inductive loads with an internal voltage source. The controllers are implemented with the explicit MPC technique and they are evaluated with a permanent-magnet-synchronous-machine drive system. The steady-state performance is assessed in terms of current quality and switching frequency and the dynamic performance is evaluated estimating the control bandwidth. Although CCS-MPC and FCS-MPC have different characteristics (harmonic spectrum, constant or variable switching frequency, THD, etc.), they achieve similar performance. However, CCS-MPC can work with larger sampling-times, i.e. it has lower requirements on the control architecture. This makes it more versatile for application on different systems and power levels.
Dosimetric Comparison for Volumetric Modulated Arc Therapy and Intensity-Modulated Radiotherapy on the Left-Sided Chest Wall and Internal Mammary Nodes Irradiation in Treating Post-Mastectomy Breast Cancer

Abstract:Abstract Background. The aim of the study was to evaluate the dosimetric benefit of applying volumetric modulated arc therapy (VMAT) on the post-mastectomy left-sided breast cancer patients, with the involvement of internal mammary nodes (IMN). Patients and methods. The prescription dose was 50 Gy delivered in 25 fractions, and the clinical target volume included the left chest wall (CW) and IMN. VMAT plans were created and compared with intensity-modulated radiotherapy (IMRT) plans on Pinnacle treatment planning system. Comparative endpoints were dose homogeneity within planning target volume (PTV), target dose coverage, doses to the critical structures including heart, lungs and the contralateral breast, number of monitor units and treatment delivery time. Results. VMAT and IMRT plans showed similar PTV dose homogeneity, but, VMAT provided a better dose coverage for IMN than IMRT (p = 0.017). The mean dose (Gy), V30 (%) and V10 (%) for the heart were 13.5 ± 5.0 Gy, 9.9% ± 5.9% and 50.2% ± 29.0% by VMAT, and 14.0 ± 5.4 Gy, 10.6% ± 5.8% and 55.7% ± 29.6% by IMRT, respectively. The left lung mean dose (Gy), V20 (%), V10 (%) and the right lung V5 (%) were significantly reduced from 14.1 ± 2.3 Gy, 24.2% ± 5.9%, 42.4% ± 11.9% and 41.2% ± 12.3% with IMRT to 12.8 ± 1.9 Gy, 21.0% ± 3.8%, 37.1% ± 8.4% and 32.1% ± 18.2% with VMAT, respectively. The mean dose to the contralateral breast was 1.7 ± 1.2 Gy with VMAT and 2.3 ± 1.6 Gy with IMRT. Finally, VMAT reduced the number of monitor units by 24% and the treatment time by 53%, as compared to IMRT. Conclusions. Compared to 5-be am step-and-shot IMRT, VMAT achieves similar or superior target coverage and a better normal tissue sparing, with fewer monitor units and shorter delivery time.

Abstract:Background: Volumetric modulated arc therapy (VMAT) is mostly considered due to its superior tumor coverage and sparing of organs at risk (OAR) with shorter treatment delivery time. Objective: This study aimed to explore the feasibility and potential benefits of VMAT with a constant dose rate (CDR). Material and methods: In this analytical study, 75 cancer patients (15 from each cancer) were selected. Step and shoot intensity-modulated radiation therapy (S&S IMRT), CDR, and VDR VMAT (variable dose rate VMAT) plans were generated for each patient using the Monte Carlo algorithm on the Monaco treatment planning system for 6 MV photon energy. For dosimetric comparison, some variables were compared, including doses to the planning target volume (PTV), OAR, homogeneity index, conformity index (CI), treatment delivery time, and monitor units. Results: CI was higher in CDR and VDR VMAT plans compared to IMRT without any significant variation for PTV coverage V95 and PTV mean dose. In the sparing of OAR, no significant variation was found between CDR, VDR, and IMRT for the brain, head-neck, oesophagus, lung, and prostate. The treatment delivery time was reduced more, i.e., by up to 72-80% in the CDR VMAT technique compared to IMRT. Conclusion: CDR VMAT technique generates a clinically acceptable plan in terms of PTV coverage, dose conformity, and OAR sparing as IMRT and VDR VMAT in all five cancer sites.

Abstract:The study was aimed at analysing the reciprocal relationships of several clinical and neurobiological items in order to predict alcohol misuse in patients with bulimia nervosa (BN). Seventy BN patients and 70 healthy controls were assessed for depression, impulsivity, borderline personality traits and self-defeating behaviours using specific scales; serum cortisol and 24-hour urinary excretion of serotonin and 5-hydroxiindolacetic acid were also assessed. The study confirmed the implications of these clinical factors for alcohol misuse in BN patients, but the results suggested that depressive symptoms and hypercortisolism could lie behind these relationships.

Abstract:Abstract Background To compare the dosimetric, normal tissue complication probability (NTCP), secondary cancer complication probabilities (SCCP), and excess absolute risk (EAR) differences of volumetric modulated arc therapy (VMAT) and intensity-modulated radiation therapy (IMRT) for left-sided breast cancer after mastectomy. Methods and materials Thirty patients with left-sided breast cancer treated with post-mastectomy radiation therapy (PMRT) were randomly enrolled in this study. Both IMRT and VMAT treatment plans were created for each patient. Planning target volume (PTV) doses for the chest wall and internal mammary nodes, PTV1, and PTV of the supraclavicular nodes, PTV2, of 50 Gy were prescribed in 25 fractions. The plans were evaluated based on PTV1 and PTV2 coverage, homogeneity index (HI), conformity index, conformity number (CN), dose to organs at risk, NTCP, SCCP, EAR, number of monitors units, and beam delivery time. Results VMAT resulted in more homogeneous chest wall coverage than did IMRT. The percent volume of PTV1 that received the prescribed dose of VMRT and IMRT was 95.9 ± 1.2% and 94.5 ± 1.6%, respectively ( p < 0.001). The HI was 0.11 ± 0.01 for VMAT and 0.12 ± 0.02 for IMRT, respectively ( p = 0.001). The VMAT plan had better conformity (CN: 0.84 ± 0.02 vs. 0.78 ± 0.04, p < 0.001) in PTV compared with IMRT. As opposed to IMRT plans, VMAT delivered a lower mean dose to the ipsilateral lung (11.5 Gy vs 12.6 Gy) and heart (5.2 Gy vs 6.0 Gy) and significantly reduced the V 5 , V 10 , V 20, V 30, and V 40 of the ipsilateral lung and heart; only the differences in V 5 of the ipsilateral lung did not reach statistical significance ( p = 0.409). Although the volume of the ipsilateral lung and heart encompassed by the 2.5 Gy isodose line (V 2.5 ) was increased by 6.7% and 7.7% ( p < 0.001, p = 0.002), the NTCP was decreased by 0.8% and 0.6%, and SCCP and EAR were decreased by 1.9% and 0.1% for the ipsilateral lung. No significant differences were observed in the contralateral lung/breast V 2.5 , V 5, V 10 , V 20 , mean dose, SCCP, and EAR. Finally, VMAT reduced the number of monitor units by 31.5% and the treatment time by 71.4%, as compared with IMRT. Conclusions Compared with IMRT, VMAT is the optimal technique for PMRT patients with left-sided breast cancer due to better target coverage, a lower dose delivered, NTCP, SCCP, and EAR to the ipsilateral lung and heart, similar doses delivered to the contralateral lung and breast, fewer monitor units and a shorter delivery time.

Abstract:This study aimed to compare different types of right breast cancer radiotherapy planning techniques and to estimate the whole-body effective doses and the critical organ absorbed doses. The three planning techniques are intensity-modulated radiotherapy (IMRT), volumetric modulated arc therapy (VMAT; two methods) and hybrid 3D-CRT/IMRT (three-dimensional conformal radiotherapy/intensity-modulated radiotherapy). The VMAT technique includes two methods to deliver a dose: non-continuous partial arc and continuous partial arc. A thermoluminescent dosimeter (TLD) is placed in the RANDO phantom to estimate the organ absorbed dose. Each planning technique applies 50.4 Gy prescription dose and treats critical organs, including the lung and heart. Dose-volume histogram was used to show the planning target volume (V95%), homogeneity index (HI), conformity index (CI), and other optimized indices. The estimation of whole-body effective dose was based on the International Commission on Radiation Protection (ICRP) Publication 60 and 103. The results were as follows: Continuous partial arc and non-continuous partial arc showed the best CI and HI. The heart absorbed doses in the continuous partial arc and hybrid 3D-CRT/IMRT were 0.07 ± 0.01% and 0% (V5% and V10%, respectively). The mean dose of the heart was lowest in hybrid 3D-CRT/IMRT (1.47 Gy ± 0.02). The dose in the left contralateral lung (V5%) was lowest in continuous partial arc (0%). The right ipsilateral lung average dose and V20% are lowest in continuous partial arc. Hybrid 3D-CRT/IMRT has the lowest mean dose to contralateral breast (organs at risk). The whole-body effective doses for ICRP-60 and ICRP-103 were highest in continuous partial arc (2.01 Sv ± 0.23 and 2.89 Sv ± 0.15, respectively). In conclusion, the use of VMAT with continuous arc has a lower risk of radiation pneumonia, while hybrid 3D-CRT/IMRT attain lower secondary malignancy risk and cardiovascular complications.

Abstract:BACKGROUND & OBJECTIVE:Breast-conserving therapy has become a widely accepted treatment option in the management of early-stage breast cancer. Postoperative radiotherapy is conventionally delivered with conventional wedged tangential fields. However, intensity modulation in radiotherapy helps to maximize tumor control while minimizing damage to normal tissues. This study was to evaluate the potential dosimetric benefits and optimal indications of intensity-modulated radiotherapy (IMRT) for the intact breast.METHODS:Ten patients with stage Tis-2N0M0 breast cancer, who received breast-conserving surgery, were selected for this study. A dosimetric comparison of forward planning IMRT with conventional wedged tangential technique was performed on each patient using three-dimensional treatment planning. The total prescribed dose for both plans was 5000 cGy/25 fractions. Dose volume histograms (DVH) were used to compare the planning target volume (PTV) and organs at risks (OARs) such as ipsilateral lung, contralateral breast, contralateral lung for all patients, and coronary arteries, heart for left sided patients, and liver for right sided patients.RESULTS:The PTV coverage in IMRT plan was similar to that of the conventional plan (97.7% vs. 98.3%). A better dose uniformity throughout the whole breast was achieved by IMRT plan. The percentage of PTV receiving less than 95% prescribed dose and more than 103% prescribed dose (inhomogeneity index, IHI) decreased from 29.9% to 2.9%; the percentage of PTV receiving more than 105% prescribed dose (V105%) decreased from 28.2% to 0.6%. A better amelioration of IHI and reduction of V105% in IMRT plans were observed in the relatively large PTV subgroup. Obvious reduction in the doses to OARs was achieved by IMRT plan. The maximum dose (D(max)) of coronary artery decreased from 5057.1 cGy to 4832.9 cGy, and the mean dose (D(mean)) of heart decreased from 629.8 cGy to 450.7 cGy; the Dmean of liver decreased from 283.9 cGy to 172.0 cGy for right sided patients; the Dmean and percentage of volume receiving more than 20 Gy (V20) of ipsilateral lung decreased from 925.2 cGy to 765.9 cGy, and from 16.0% to 15.3%, respectively; the Dmean and V20 of different central lung distance (CLD) subgroups decreased by 14.7% and 20.9%, 7.0% and 12.9%, respectively; the Dmean of contralateral breast decreased from 75.4 cGy to 20.3 cGy; the Dmean of contralateral lung decreased from 30.9 cGy to 16.1 cGy.CONCLUSION:Forward planning IMRT based on a standard tangential beam arrangement significantly improves the dose homogeneity throughout the target volume of intact breast, and reduces the dose to OARs, especially in patients with large breast volumes or exceeded CLD, who might be proposed as candidates of IMRT for intact breast.

Abstract:Objective To compute and compare the tumor control probability (TCP) of volumetric modulated arc therapy (VMAT) for breast cancer after conservative surgery based on two types of multileaf collimator (MLC) through a retrospective planning study. Methods For a group of 9 patients diagnosed with left breast cancer, VMAT plan based on Agility MLC and beam modulator (BM) MLC were designed. The prescription dose was 50 Gy covering at least 95% of the planning target volume, 2 Gy per fraction. TCPs were calculated according to dose-volume histogram (DVH) analysis. Results The TCP of the BM VMAT plan was slightly higher than that of the Agility VMAT plan (94.61% vs 94.23%) but was inferior with respect to delivery efficiency; the delivery time was reduced for Agility VMAT plan by 35% compared to BM VMAT plan. Conclusion For breast cancer radiation therapy after conservative surgery, BM VMAT plans provide slightly higher TCP while the delivery of Agility VMAT plans is significantly faster than the BM VMAT plans.

Abstract:Abstract Objective:.To explore the feasibility of volumetric-modulated arc therapy (VMAT) instead of intensity-modulated radiotherapy (IMRT) for primary tumors of advanced non–small-cell lung cancer (A-NSCLC). Methods:.We used propensity score matching (PSM) and multicenter retrospective analysis to study the efficacy and toxicity of VMAT technology in radiotherapy for primary tumors of A-NSCLC.We used the chi-squared test to analyze the response rate(RR), local control (LC), acute radiation injury, and dose-volume parameters; the Kaplan–Meier and log-rank tests were used to determine local-regional progression-free survival (LRPFS) and overall survival (OS). Results: LRPFS was significantly prolonged in stage III patients treated with IMRT before PSM (P< .05) and cases of grade 1 or 2 acute radiation esophagitis (RE) or radiation pneumonia(RP) in the IMRT than the VMAT(P< .05), but cases of grade 3 or 4 RE and RP were not significantly different between the groups (P> .05). Before PSM, there was no significant difference in the LRPFS and OS rates of the whole study group and the RR, LC, and stage IV subgroups, respectively (P> .05). After PSM, there was no significant difference in the RR, LC, LRPFS, OS, RP, or RE of patients treated with VMAT and IMRT (P> .05), normal whole-lung (V5, V20, MLD), heart (V30, V40, MHD), and equal dose-volume parameters were significantly lower in the VMAT group (P< .05).Conclusion:.Radiation therapy of A-NSCLC primary tumors using VMAT can achieve a similar efficacy to that of IMRT but with significantly lower low-dose volume parameters of normal tissues and organs; this is true especially in stage N2 cases, where the reduction in radiation damage may be more favorable.

Abstract:The host-guest assembly of CB7 with a series of alkyl(trimethyl)ammonium (C(n)TA(+)) surfactants of different chain lengths (n=6-18) has been studied. The complexation behaviour was investigated by NMR spectroscopy, isothermal titration calorimetry and kinetics measurements. The combined results of these techniques provided evidence for the formation of 1:1 inclusion and 2:1 external complexes in the cases of C(n)TA(+) with n=12-18. The binding constants for the 1:1 complexes are independent of the alkyl chain length of the surfactant, whereas a relationship between K(2:1) and the chain length of the surfactant was found for the 2:1 complexes.

Abstract:Background:Image-guided radiation therapy (IGRT) is the preferred method for curative treatment of localized prostate cancer, which could improve disease outcome and reduce normal tissue toxicity reaction. IGRT using cone-beam computed tomography (CBCT) in combination with volumetric-modulated arc therapy (VMAT) potentially allows smaller treatment margins and dose escalation to the prostate. The aim of this study was to compare the difference of dosimetric diffusion in conventional IGRT using 7-field, step-and-shoot intensity-modulated radiation therapy (IMRT) and hypofractionated IGRT using VMAT for patients with localized prostate cancer. Methods:We studied 24 patients who received 78 Gy in 39 daily fractions or 70 Gy in 28 daily fractions to their prostate with/without the seminal vesicles using IMRT (n = 12) or VMAT (n = 12) for prostate cancer between November 2013 and October 2015. Image guidance was performed using kilovoltage CBCT scans equipped on the linear accelerator. Offline planning was performed using the daily treatment images registered with simulation computed tomography (CT) images. A total of 212 IMRT plans in conventional cohort and 292 VMAT plans in hypofractionated cohort were enrolled in the study. Dose distributions were recalculated on CBCT images registered with the planning CT scanner. Results:Compared with 7-field, step-and-shoot IMRT, VMAT plans resulted in improved planning target volume (PTV) D95% (7663.17 ± 69.57 cGy vs. 7789.17 ± 131.76 cGy, P < 0.001). VMAT reduced the rectal D25 (P < 0.001), D35 (P < 0.001), and D50 (P < 0.001), bladder V50 (P < 0.001), D25 (P = 0.002), D35 (P = 0.028), and D50 (P = 0.029). However, VMAT did not statistically significantly reduce the rectal V50, compared with 7-field, step-and-shoot IMRT (25.02 ± 5.54% vs. 27.43 ± 8.79%, P = 0.087). Conclusions:To deliver the hypofractionated radiotherapy in prostate cancer, VMAT significantly increased PTV D95% dose and decreased the dose of radiation delivered to adjacent normal tissues comparing to 7-field, step-and-shoot IMRT. Daily online image-guidance and better management of bladder and rectum could make a more precise treatment delivery.

Abstract:Introduction: Breast cancer is the most common cancer amongst women, which is treated by a multi-modality approach consisting of surgery, chemotherapy, radiotherapy, hormonal therapy, targeted therapy and immunotherapy. Objective: The aim of the study is to compare the early and late treatment toxicities and the local recurrence following Conventional and Hypofractionated External Beam Radiotherapy in breast cancer patients after surgery. Methods: A randomized study was conducted for two years including 150 patients. The patients were divided into two arms, with Arm-A receiving a total dose of 50Gy/25# and the Arm-B receiving 40Gy/16#. Results: Early and late treatment toxicities were comparable in both the arms, except for skin toxicity which was more in the hypofractionated arm (Arm-B). Local Recurrence was observed in Arm-A in 1 patient at Month 17 of follow-up, and in Arm-B at Month 13 in 1 patient and at Month 21 in 1 patient. Conclusion: Hypofractionated radiotherapy given over a short period of time is comparable to Conventional fractionation radiotherapy without any significant statistical difference in terms of toxicities or local recurrence; with an added advantage of reduced treatment time, reduced duration of hospital stays, better patients’ compliance and reduced cost of treatment.

Abstract:To compare the dosimetric difference between three dimensional conformal radiotherapy using the field-in-field(FIF-CRT) and volumetric modulated arc therapy(VMAT) for whole breast irradiation with tumor bed boost after breast-conserving surgery, fifteen female patients with early left-sided breast cancer after breast-conserving surgery were selected. Treatment planning was performed by volumetric modulated arc therapy and three-dimensional conformal radiotherapy using the field-in-field on the same CT, respectively. The target dose distribution, homogeneity and the irradiation dose and volume for the lungs, heart, and contralateral breast were read in the dose volume histogram(DVH) and compared between FIF-CRT and VMAT. The treatment delivery monitor unit was also compared. In comparison with the FIF-CRT planning, the target coverage of PTV1 and PGTV was significantly higher by 5.62% and 10.64% in VMAT. The conformal index and homogeneity of planning target volume by the VMAT planning were better than those by FIF-CRT planning. There were no significant difference in the maximum dose(Dmax) and V10 for the heart and V20 for the left-lung between the VMAT and FIF-CRT planning, but the values of V10, V5, and Dmean for the left-lung; V5 for the heart; V5, Dmean for the contralateral breast and lung; Dmax for the spine cord were increased compared with those by the FIF-CRT planning. The V30, V40 of the left-lung and V20 of the heart by the VMAT planning were lower than those by FIF-CRT. The mean MU of the VMAT was 745 MU, which is significantly higher than that by FIF-CRT planning(250 MU, p0.001). The VMAT planning improves the dose distribution and homogeneity of PTV for breast cancer radiotherapy after breast—conserving surgery. However, it significantly enlarges the volume of normal tissues irradiated in low dose areas, and increases the MU value in comparison with FIF-CRT.

Abstract:Objective To evaluate the dosimetric difference between fixed-field static intensity-modulated radiotherapy (IMRT), fixed-field dynamic multileaf collimator (DMLC), and volumetric modulated arc therapy (VMAT), all of which involve supraclavicular and infraclavicular regions, in breast cancer patients after breast-conserving surgery.Methods This study included 14 female patients with breast cancer who received radiotherapy after breast-conserving surgery in our hospital from October 2012 to April 2016.The radiation field included the chest wall and supraclavicular and infraclavicular regions.IMRT, DMLC, and VMAT plans were generated for each patient while using identical optimization conditions.The doses to planning target volume (PTV) and organs at risk (OARs) were compared based on dose-volume histogram (DVH);one-way analysis of variance or nonparametric Wilcoxon rank test was used for comparison.Results For the dose distribution of PTV, VMAT achieved the best V95, V98, CI, and HI (P<0.009).Concerning the doses to OARs, VMAT achieved the best V5, V20, and Dmean of the ipsilateral lung and the best V5 and Dmean of the contralateral lung (P<0.022).Dmean of the spinal cord was significantly lower in VMAT than in IMRT and DMLC (P=0.004).Conclusions VMAT is preferred for the patients with breast cancer to be treated with radiotherapy involving supraclavicular and infraclavicular regions after breast-conserving surgery.It can improve the dose distribution of target and reduce the doses to organs at risk and radiotherapy toxicities.

Abstract:Purpose: The local management approach for node-positive breast cancer has undergone substantial evolution. Consequently, there exists a pressing need to enhance our treatment strategies by placing greater emphasis on planning and dosimetric factors, given the availability of more conformal techniques and delineation criteria, achieving optimal goals of radiotherapy treatment. The primary aim of this article is to discuss how the extent of regional nodal coverage influences the choice between IMRT and 3D radiation therapy for patients. Patients and methods: A total of 15 patients diagnosed with left breast cancer with disease involved lymph nodes were included in this study. Delivering the recommended dose required the use of a linear accelerator (LINAC) with photon beams energy of 6 mega voltage (6MV). Each patient had full breast radiation using two planning procedures: intensity-modulated radiotherapy (IMRT) and three-dimensional radiotherapy (3D conformal). Following the guidelines set forth by the Radiation Therapy Oncology Group (RTOG), the planned treatment coverage was carefully designed to fall between 95% and 107% of the recommended dose. Additionally, Dose Volume Histograms (DVHs) were generated the dose distribution within these anatomical contours. Results and conclusion: The DVH parameters were subjected to a comparative analysis, focusing on the doses absorbed by both Organs at Risk (OARs) and the Planning Target Volume (PTV). The findings suggest that low doses in IMRT plan might raise the risk of adverse oncological outcomes or potentially result in an increased incidence of subsequent malignancies. Consequently, the adoption of inverse IMRT remains limited, and the decision to opt for this therapy should be reserved for situations where it is genuinely necessary to uphold a satisfactory quality of life. Additionally, this approach helps in reducing the likelihood of developing thyroid problems and mitigates the risk of injuries to the supraclavicular area and the proximal head of the humerus bone.
