Abstract:OBJECTIVE: Autism Spectrum Disorders (ASD) are characterized by impairments in joint attention (JA) comprising two components: responding to JA (RJA) and initiating JA (IJA). RJA and IJA are considered two interrelated aspects of JA, related to different stages of infant development. While recent technologies have been used to characterize RJA emerging in earlier childhood, only a limited number of studies have attempted to explore IJA, which progressively becomes evident as a hallmark of ASD. This study aims to achieve the social recognition of both RJA and IJA by vision-based human behavior perception through a multi-modal framework automatically and comprehensively.METHODS: The first three layers of this framework leverage localization, feature extraction, and activity recognition. On this basis, three critical activities in JA are recognized: attention estimation, spontaneous pointing, and showing actions. Then different behaviors are linked through the fourth layer, semantic interpretation, to model the JA event. The proposed framework is evaluated on experiments of four groups: 7 children with ASD, 5 children with mental retardation (MR), 5 children with developmental language disorder (DLD), and 3 typically developed children (TD).RESULTS: Experimental results compared with human codings demonstrate recognition reliability with an intra-class coefficient of 0.959. In addition, statistical analysis suggests significant group difference and correlations.CONCLUSIONS: The multi-modal human behavior perception-based framework is a feasible solution for the recognition of joint attention in unconstrained environments.SIGNIFICANCE: Thus the proposed approach has the potential to improve the clinical diagnosis of autism by offering quantitative monitoring and statistical analysis.

Abstract:Deficits in responding to joint attention (RJA) are early symptoms of autism spectrum disorder (ASD). Currently, no automated tools exist for identifying and quantifying RJA behaviors. A few eye tracking studies have investigated RJA in ASD children but have produced conflicting results. In addition, little is known about the trajectory of RJA development through developmental age. Here, a new video was designed including 12 clips of an actor pointing to or looking at an object. Eye tracking technology was used to monitor RJA in three groups: 143 ASD children assessed with the Autism Diagnostic Interview-Revised (ADI-R) and the Autism Diagnostic Observation Schedule (ADOS) (4-7 years old), 113 age- and gender-matched typically developing children (TDC), and 43 typically developing adults (TDA) (19-32 years old). RJAfinder was developed in R and MATLAB to quantify RJA events from the eye tracking data. RJA events were compared among the three groups. Spearman correlation coefficients between total number of RJA events in ASD and the Social Responsiveness Scale (SRS) scores were calculated. A logistic regression model was built using the average valid sampling rate and the total number of RJA events as two predictive variables to classify ASD and TDC groups. ASD children displayed statistically significantly less RJA events than the TDC and TDA groups with medium-to-large-sized effects. ASD and TDC children both displayed more RJA events in response to pointing stimuli than to looking stimuli. Our logistic regression model predicted ASD tendency with 0.76 accuracy in the testing set. RJA ability improved more slowly between the ages of 4-7 years old in the ASD group than in the TDC group. In ASD children, RJA ability showed negative correlation with SRS total T-score as well as the scores of five subdomains. Our study provides an automated tool for quantifying RJA and insights for the study of RJA in ASD children, which may help improve ASD screening, subtyping, and behavior interventions.

Abstract:ObjectiveTo develop and validate deep learning (DL) methods for diagnosing autism spectrum disorder (ASD) based on conventional MRI (cMRI) and apparent diffusion coefficient (ADC) images.MethodsA total of 151 ASD children and 151 age-matched typically developing (TD) controls were included in this study. The data from these subjects were assigned to training and validation datasets. An additional 20 ASD children and 25 TD controls were acquired, whose data were utilized in an independent test set. All subjects underwent cMRI and diffusion-weighted imaging examination of the brain. We developed a series of DL models to separate ASD from TD based on the cMRI and ADC data. The seven models used include five single-sequence models (SSMs), one dominant-sequence model (DSM), and one all-sequence model (ASM). To enhance the feature detection of the models, we embed an attention mechanism module.ResultsThe highest AUC (0.824 ~ 0.850) was achieved when applying the SSM based on either FLAIR or ADC to the validation and independent test sets. A DSM using the combination of FLAIR and ADC showed an improved AUC in the validation (0.873) and independent test sets (0.876). The ASM also showed better diagnostic value in the validation (AUC = 0.838) and independent test sets (AUC = 0.836) compared to the SSMs. Among the models with attention mechanism, the DSM achieved the highest diagnostic performance with an AUC, accuracy, sensitivity, and specificity of 0.898, 84.4%, 85.0%, and 84.0% respectively.ConclusionsThis study established the potential of DL models to distinguish ASD cases from TD controls based on cMRI and ADC images.Key Points• Deep learning models based on conventional MRI and ADC can be used to diagnose ASD.• The model (DSM) based on the FLAIR and ADC sequence achieved the best diagnostic performance with an AUC of 0.836 in the independent test sets.• The attention mechanism further improved the diagnostic performance of the models.

Abstract:Objective: Machine learning (ML) is increasingly employed to diagnose medical conditions, with algorithms trained to assign a single label using a black-box approach. We created an ML approach using deep learning that generates outcomes that are transparent and in line with clinical, diagnostic rules. We demonstrate our approach for autism spectrum disorders (ASD), a neurodevelopmental condition with increasing prevalence. Methods: We use unstructured data from the Centers for Disease Control and Prevention (CDC) surveillance records labeled by a CDC-trained clinician with ASD A1-3 and B1-4 criterion labels per sentence and with ASD cases labels per record using Diagnostic and Statistical Manual of Mental Disorders (DSM5) rules. One rule-based and three deep ML algorithms and six ensembles were compared and evaluated using a test set with 6773 sentences (N = 35 cases) set aside in advance. Criterion and case labeling were evaluated for each ML algorithm and ensemble. Case labeling outcomes were compared also with seven traditional tests. Results: Performance for criterion labeling was highest for the hybrid BiLSTM ML model. The best case labeling was achieved by an ensemble of two BiLSTM ML models using a majority vote. It achieved 100% precision (or PPV), 83% recall (or sensitivity), 100% specificity, 91% accuracy, and 0.91 F-measure. A comparison with existing diagnostic tests shows that our best ensemble was more accurate overall. Conclusions: Transparent ML is achievable even with small datasets. By focusing on intermediate steps, deep ML can provide transparent decisions. By leveraging data redundancies, ML errors at the intermediate level have a low impact on final outcomes.

Abstract:Background Early identification of autism spectrum disorder (ASD) improves long-term outcomes, yet significant diagnostic delays persist. Methods A retrospective cohort of 449 children (ASD: 246, typically developing [TD]: 203) was used for model development. Eye-movement data were collected from the participants watching videos that featured eye-tracking paradigms for assessing social and non-social cognition. Five machine learning algorithms, namely random forest, support vector machine, logistic regression, artificial neural network, and extreme gradient boosting, were trained to classify children with ASD and TD. The best-performing algorithm was selected to build the final model which was further evaluated in a prospective cohort of 80 children. The Shapley values interpreted important eye-tracking features. Results Random forest outperformed other algorithms during model development and achieved an area under the curve of 0.849 (< 3 years: 0.832, ≥ 3 years: 0.868) on the external validation set. Of the ten most important eye-tracking features, three measured social cognition, and the rest were related to non-social cognition. A deterioration in model performance was observed using only the social or non-social cognition-related eye-tracking features. Limitations The sample size of this study, although larger than that of existing studies of ASD based on eye-tracking data, was still relatively small compared to the number of features. Conclusions Machine learning models based on eye-tracking data have the potential to be cost- and time-efficient digital tools for the early identification of ASD. Eye-tracking phenotypes related to social and non-social cognition play an important role in distinguishing children with ASD from TD children.

Abstract:This article aims to improve the detection of ASD in children using ET data and advanced ML techniques. ASD, a neurodevelopmental disorder characterized by impairments in social communication, interaction, and repetitive behaviors, typically manifests before the age of three. Early diagnosis is crucial for timely and effective interventions. ET data have revealed distinct gaze patterns in children with ASD, such as diminished attention to social cues and increased fixation on repetitive stimuli. However, current methods of ET data analysis are largely manual and lack scalability for widespread clinical screening. To address these limitations, we propose a novel, scanpath-based ASD detection method that identifies atypical gaze behaviors through dynamic changes in visual attention. We extract four sequential features from scanpaths and analyze variations in feature space across different ASD severity levels low, mild, moderate, and severe using the MultiMatch and DTW similarity metrics. Our analysis reveals that children with ASD exhibit unique and highly individualized gaze patterns when compared to TD children. Notable differences are observed in attention duration and vertical gaze distribution, providing key insights into ASD-related visual behaviors. For classification, we employ a hybrid CNN-RNN model, which significantly outperforms traditional ML methods. The CNN-RNN model achieves an accuracy of 97%, recall of 98.24%, and an F1-score of 97.04% using the feature set ( x , d , y ). In comparison, models based on GRU and 2-LSTM networks show competitive accuracies of 92% and 90%, respectively. However, RFC and XGBoost models underperform, with accuracies ranging between 70.25 and 80.80%. These findings demonstrate the efficacy of DL approaches, particularly the CNN-RNN hybrid model, in accurately classifying ASD based on ET data, emphasizing their potential to enhance diagnostic accuracy. The proposed scalable method for ASD detection holds promise for improving early diagnosis and intervention, which is critical for better long-term outcomes in children with ASD. Further evaluation on an independent dataset will assess the generalizability and precision of the model.

Abstract:Atypical visual attention in individuals with autism spectrum disorders (ASD) has been utilised as a unique diagnosis criterion in previous research. This paper presents a novel approach to the automatic and quantitative screening of ASD as well as symptom severity prediction in preschool children. We develop a novel computational pipeline that extracts learned features from a dynamic visual stimulus to classify ASD children and predict the level of ASD-related symptoms. Experimental results demonstrate promising performance that is superior to using handcrafted features and machine learning algorithms, in terms of evaluation metrics used in diagnostic tests. Using a leave-one-out cross-validation approach, we obtained an accuracy of 94.59%, a sensitivity of 100%, a specificity of 76.47% and an area under the receiver operating characteristic curve (AUC) of 96% for ASD classification. In addition, we obtained an accuracy of 94.74%, a sensitivity of 87.50%, a specificity of 100% and an AUC of 99% for ASD symptom severity prediction.

Identifying Visual Attention Features Accurately Discerning Between Autism and Typically Developing: a Deep Learning Framework

Abstract:Atypical visual attention is a hallmark of autism spectrum disorder (ASD). Identifying the attention features accurately discerning between people with ASD and typically developing (TD) at the individual level remains a challenge. In this study, we developed a new systematic framework combining high accuracy deep learning classification, deep learning segmentation, image ablation and a direct measurement of classification ability to identify the discriminative features for autism identification. Our two-stream model achieved the state-of-the-art performance with a classification accuracy of 0.95. Using this framework, two new categories of features, Food & drink and Outdoor-objects, were identified as discriminative attention features, in addition to the previously reported features including Center-object and Human-faces, etc. Altered attention to the new categories helps to understand related atypical behaviors in ASD. Importantly, the area under curve (AUC) based on the combined top-9 features identified in this study was 0.92, allowing an accurate classification at the individual level. We also obtained a small but informative dataset of 12 images with an AUC of 0.86, suggesting a potentially efficient approach for the clinical diagnosis of ASD. Together, our deep learning framework based on VGG-16 provides a novel and powerful tool to recognize and understand abnormal visual attention in ASD, which will, in turn, facilitate the identification of biomarkers for ASD. Abstract:BackgroundIn typical infant development, parents and their children jointly contribute to establishing frequent episodes of joint attention, which boost language acquisition and shape social cognition. Here we used novel live eye tracking technology to evaluate the degree to which Autism Spectrum Disorder (ASD) is related to reduced responding to others' joint attention bids in infancy (RJA) and to a reduced tendency to initiate joint attention episodes (IJA). Because young infants use their gaze for both RJA and IJA, this approach allowed us to quantify these elusive processes early in life.MethodsThe final sample consisted of 112 infants (54 boys, 58 girls), of which 81 were at familial risk for ASD and 31 were low-risk controls. At follow up (36 months), 22 children in the high risk group were diagnosed with ASD.ResultsAt 10 months, rates of IJA were lower in infants later diagnosed with ASD than in control groups (effect sizes d= 0.78-0.95), and followed an atypical developmental trajectory from 10 to 18 months (p < .002). RJA distinguished infants based on familial ASD risk, albeit not ASD diagnosis. The differences in IJA could not be explained by overall looking time, social preference, eye movement latencies or number of fixations.ConclusionsThis live eye tracking study suggests that during a critical period for the development of social cognition (10-18 months), infants later diagnosed with ASD show marked atypicalities in IJA, but not RJA. The results indicate that IJA is an important target for future prodromal intervention trials.

Abstract:One hundred and forty-five patients were identified with minor salivary gland tumours. General information and tumour-specific information on stage, grade of tumour, resection margins, recurrence and survival were collected. Data was analysed by both univariate and multivariate methods. Indices predicting tumour recurrence and survival were analysed. Forty-two benign lesions, mostly pleomorphic adenomas were identified, one recurred, all survived. One hundred and three malignant lesions were identified, mostly adenoid cystic carcinomas (70%) or mucoepidermoid carcinomas (19%). Late stage disease and the presence of neck node metastases predicted both early recurrence and high eventual mortality. Survival was favoured by the histological type (mucoepidermoid > adenoid cystic), site of primary (oral cavity and oropharynx > nose, sinuses and larynx) and good general condition. Many tumours recurred after 5 years of disease-free survival and late mortality was a feature (80% survival at 5 years, 20% at 20 years). Many patients survive some time with either local recurrence or distant metastases. Long-term follow-up is advocated as local or distant recurrence may be treatable. The value of super radical treatment of the primary is questionable given the likelihood of recurrence at distant sites.

Abstract:Autism Spectrum Disorder(ASD) is a type of neurological disorder that is common among children. The diagnosis of this disorder at an early stage is the key to reducing its effects. The major symptoms include anxiety, lack of communication, and less social interaction. This paper presents a systematic review conducted based on PRISMA guidelines for automated diagnosis of ASD. With rapid development in the field of Data Science, numerous methods have been proposed that can diagnose the disease at an early stage which can minimize the effects of the disorder. Machine learning and deep learning have proven suitable techniques for the automated diagnosis of ASD. These models have been developed on various datasets such as ABIDE I and ABIDE II, a frequently used dataset based on rs-fMRI images. Approximately 26 articles have been reviewed after the screening process. The paper highlights a comparison between different algorithms used and their accuracy as well. It was observed that most researchers used DL algorithms to develop the ASD detection model. Different accuracies were recorded with a maximum accuracy close to 0.99. Recommendations for future work have also been discussed in a later section. This analysis derived a conclusion that AI-emerged DL and ML technologies can diagnose ASD through rs-fMRI images with maximum accuracy. The comparative analysis has been included to show the accuracy range.

Abstract:Eye tracking is a promising tool for Autism Spectrum Disorder (ASD) detection in both children and adults. An important aspect of social communication is keeping eye contact, which is something that people with ASD frequently struggle with. Eye tracking can assess the duration of eye contact and the frequency and direction of gaze movements, offering quantifiable indicators of social communication deficits. People with ASD may also demonstrate other abnormalities in visual processing, such as an increased concentration on detail, sensory sensitivity, and trouble with complicated visual activities. These variations can be measured via Eye tracking, which offers critical information for the planning of therapy and diagnosis. The primary objective of this work is to provide a thorough description of the most recent studies that use Eye tracking combined with various Machine Learning (ML) and Deep Learning (DL) models for the detection of ASD. This will provide insights into the identification, and behavioral assessment, and distinguish between autistic people and those who are Typically Developing (TD). A detailed review of the various ML and DL models with their datasets and performance criteria is presented. Different types of eye movement datasets with diagnostic standards and eye tracker devices are also discussed. Finally, the study addresses the potential of gaze prediction in ASD patients for the design of interventions.

Abstract:BACKGROUND: Autism Spectrum Disorder (ASD) is a condition with social interaction, communication, and behavioral difficulties. Diagnostic methods mostly rely on subjective evaluations and can lack objectivity. In this research Machine learning (ML) and deep learning (DL) techniques are used to enhance ASD classification. OBJECTIVE: This study focuses on improving ASD and TD classification accuracy with a minimal number of EEG channels. ML and DL models are used with EEG data, including Mu Rhythm from the Sensory Motor Cortex (SMC) for classification. METHODS: Non-linear features in time and frequency domains are extracted and ML models are applied for classification. The EEG 1D data is transformed into images using Independent Component Analysis-Second Order Blind Identification (ICA-SOBI), Spectrogram, and Continuous Wavelet Transform (CWT). RESULTS: Stacking Classifier employed with non-linear features yields precision, recall, F1-score, and accuracy rates of 78%, 79%, 78%, and 78% respectively. Including entropy and fuzzy entropy features further improves accuracy to 81.4%. In addition, DL models, employing SOBI, CWT, and spectrogram plots, achieve precision, recall, F1-score, and accuracy of 75%, 75%, 74%, and 75% respectively. The hybrid model, which combined deep learning features from spectrogram and CWT with machine learning, exhibits prominent improvement, attained precision, recall, F1-score, and accuracy of 94%, 94%, 94%, and 94% respectively. Incorporating entropy and fuzzy entropy features further improved the accuracy to 96.9%. CONCLUSIONS: This study underscores the potential of ML and DL techniques in improving the classification of ASD and TD individuals, particularly when utilizing a minimal set of EEG channels.

Early identification of autism spectrum disorder by multi-instrument fusion: A clinically applicable machine learning approach

Abstract:Autism spectrum disorder (ASD), developmental language disorder (DLD), and global developmental delay (GDD) are common neurodevelopmental disorders in early childhood; however, the differential diagnosis of these disorders is difficult because of overlapping symptoms. Drawing on a cohort of 2004 children with ASD, DLD, or GDD, this study developed machine learning classifiers using decision trees, support vector machines, eXtreme gradient boosting (XGB), logistic regression, and neural networks by combining several easily accessible behavioral and developmental assessment instruments. The best-performing XGB model was further simplified into a two-stage decision model (TS-DM) to achieve better interpretability. Model performance was tested and compared with that of 12 pediatricians on an external dataset of 60 children. The accuracies of the resident pediatricians, senior pediatricians, TS-DM, and XGB were 53.3%, 66.7%, 75.0%, and 78.3%, respectively. Machine learning has the potential to identify these three neurodevelopmental disorders by integrating information from multiple instruments and thereby may increase our understanding of the roles of different behavioral and developmental characteristics in the different diagnoses.

This paper underscores the paramount significance of early detection and diagnosis as a pivotal precursor to effective intervention. To this end, integrating advanced technological tools, specifically eye-tracking technology and deep learning algorithms, is investigated for its potential to discriminate between children with ASD and their typically developing (TD) peers. By employing these methods, the research aims to contribute to refining early detection strategies and support mechanisms. This study introduces innovative deep learning models grounded in convolutional neural network (CNN) and recurrent neural network (RNN) architectures, employing an eye-tracking dataset for training. Of note, performance outcomes have been realised, with the bidirectional long short-term memory (BiLSTM) achieving an accuracy of 96.44%, the gated recurrent unit (GRU) attaining 97.49%, the CNN-LSTM hybridising to 97.94%, and the LSTM achieving the most remarkable accuracy result of 98.33%. These outcomes underscore the efficacy of the applied methodologies and the potential of advanced computational frameworks in achieving substantial accuracy levels in ASD detection and classification.
Learning Visual Attention to Identify People with Autism Spectrum Disorder.

Abstract:This paper presents a novel method for quantitative and objective diagnoses of Autism Spectrum Disorder (ASD) using eye tracking and deep neural networks. ASD is prevalent, with 1.5% of people in the US. The lack of clinical resources for early diagnoses has been a long-lasting issue. This work differentiates itself with three unique features: first, the proposed approach is data-driven and free of assumptions, important for new discoveries in understanding ASD as well as other neurodevelopmental disorders. Second, we concentrate our analyses on the differences in eye movement patterns between healthy people and those with ASD. An image selection method based on Fisher scores allows feature learning with the most discriminative contents, leading to efficient and accurate diagnoses. Third, we leverage the recent advances in deep neural networks for both prediction and visualization. Experimental results show the superior performance of our method in terms of multiple evaluation metrics used in diagnostic tests.
Objective Measurement of Social Communication Behaviors in Children with Suspected ASD During the ADOS-2

Abstract:ABSTRACTAutism spectrum disorder (ASD) is defined by persistent disturbances of social communication, as well as repetitive patterns of behavior. ASD is identified on the basis of expert, but subjective, clinician judgment during assessments such as the Autism Diagnostic Observation Schedule-2 (ADOS-2). Quantification of key social behavioral features of ASD using objective measurements would enrich scientific understanding of the disorder. The current pilot study leveraged computer vision and audio signal processing to identify a key set of objective measures of children's social communication behaviors during the ADOS-2 (e.g., social gaze, social smile, vocal interaction) that were captured with adult-worn camera-embedded eyeglasses. Objective measurements of children's social communicative behaviors during the ADOS-2 showed relatively low levels of association with the examiner-adjudicated ADOS-2 scores. Future directions and implications for the use of objective measurements in diagnostic and treatment monitoring are discussed.
Joint attention training for children with autism using behavior modification procedures

Abstract:Background: Deficits in joint attention are considered by many researchers to be an early predictor of childhood autism (e.g., Osterling & Dawson, 1994) and are considered to be pivotal to deficits in language, play, and social development in this population (Mundy, 1995). Although many researchers have noted the importance of joint attention deficits in the development of children with autism (e.g., Mundy, Sigman, & Kasari, 1994) and have called for intervention strategies (e.g., Mundy & Crowson, 1997), few studies have attempted to target joint attention. In this study, joint attention behaviors were taught to children with autism using a behavior modification procedure. Methods: A multiple-baseline design was implemented to evaluate intervention effects. The following target behaviors were included in the intervention: 1) Responding to showing, pointing, and gaze shifting of adult; 2) Coordinated gaze shifting (i.e., coordinated joint attention); and 3) Pointing (with the purpose of sharing, not requesting). Generalization to setting and parent, follow-up sessions, and social validation measures were also analyzed. Results: Joint attention behaviors were effectively trained and targeted behaviors generalized to other settings. In addition, positive changes were noted by naive observers using social validation measures. Conclusions: Integrating joint attention training into existing interventions may be important for children with autism. In addition, training parents in these techniques may help to maintain joint attention skills outside of the treatment setting.

Abstract:Currently, every 1 in 54 children have been diagnosed with Autism Spectrum Disorder (ASD), which is 178% higher than it was in 2000. An early diagnosis and treatment can significantly increase the chances of going off the spectrum and making a full recovery. With a multitude of physical and behavioral tests for neurological and communication skills, diagnosing ASD is very complex, subjective, time-consuming, and expensive. We hypothesize that the use of machine learning analysis on facial features and social behavior can speed up the diagnosis of ASD without compromising real-world performance. We propose to develop a hybrid architecture using both categorical data and image data to automate traditional ASD pre-screening, which makes diagnosis a quicker and easier process. We created and tested a Logistic Regression model and a Linear Support Vector Machine for Module 1, which classifies ADOS categorical data. A Convolutional Neural Network and a DenseNet network are used for module 2, which classifies video data. Finally, we combined the best performing models, a Linear SVM and DenseNet, using three data averaging strategies. We used a standard average, weighted based on number of training data, and weighted based on the number of ASD patients in the training data to average the results, thereby increasing accuracy in clinical applications. The results we obtained support our hypothesis. Our novel architecture is able to effectively automate ASD pre-screening with a maximum weighted accuracy of 84%.
