Abstract:Aortic dissection (AD) is a life-threatening cardiovascular disease with a high mortality rate. The accurate and generalized 3-D reconstruction of AD from CT-angiography can effectively assist clinical procedures and surgery plans, however, is clinically unavaliable due to the lacking of efficient tools. In this study, we presented a novel multi-stage segmentation framework for type B AD to extract true lumen (TL), false lumen (FL) and all branches (BR) as different classes. Two cascaded neural networks were used to segment the aortic trunk and branches and to separate the dual lumen, respectively. An aortic straightening method was designed based on the prior vascular anatomy of AD, simplifying the curved aortic shape before the second network. The straightening-based method achieved the mean Dice scores of 0.96, 0.95 and 0.89 for TL, FL, and BR on a multi-center dataset involving 120 patients, outperforming the end-to-end multi-class methods and the multi-stage methods without straightening on the dual-lumen segmentation, even using different network architectures. Both the global volumetric features of the aorta and the local characteristics of the primary tear could be better identified and quantified based on the straightening. Comparing to previous deep learning methods dealing with AD segmentations, the proposed framework presented advantages in segmentation accuracy.
Segmentation of the True Lumen of Aorta Dissection Via Morphology-Constrained Stepwise Deep Mesh Regression

Abstract:The lumen of aortic dissection (AD) has important clinical value for preoperative diagnosis, interoperative intervention, and post-operative evaluation of AD diseases. AD segmentation is challenging because (i) fitting its irregular profile by using traditional models is difficult, and (ii) the size of the AD image is usually so big that many algorithms have to perform down-sampling to reduce the computational burden, thereby reducing the resolution of the result. In this paper, an automatic AD segmentation algorithm, in which a 3D mesh is gradually moved to the surface of AD based on the offset estimated by a deep mesh deformation module, is presented. AD morphology is used to constrain the initial mesh and guide the deformation, which improves the efficiency of the deep network and avoids down-sampling. Moreover, a stepwise regression strategy is introduced to solve the mesh folding problem and improve the uniformity of the mesh points. On an AD database that involves 35 images, the proposed method obtains the mean Dice of 94.12% and symmetric 95% Hausdorff distance of 2.85 mm, which outperforms five state-of-the-art AD segmentation methods. The average processing time is 16.6 s, and the memory used to train the network is only 0.36 GB, indicating that this method is easy to apply in clinical practice.
Multi-Task Deep Convolutional Neural Network for the Segmentation of Type B Aortic Dissection

Abstract:Segmentation of the entire aorta and true-false lumen is crucial to inform plan and follow-up for endovascular repair of the rare yet life threatening type B aortic dissection. Manual segmentation by slice is time-consuming and requires expertise, while current computer-aided methods focus on the segmentation of the entire aorta, are unable to concurrently segment true-false lumen, and some require human interaction. We here report a fully automated approach based on a 3-D multi-task deep convolutional neural network that segments the entire aorta and true-false lumen from CTA images in a unified framework. For training, we built a database containing 254 CTA images (210 preoperative and 44 postoperative) obtained using various systems from 254 unique patients with type B aortic dissection. Slice-wise manual segmentation of the entire aorta and the true-false lumen for each 3-D CTA image was provided. Upon evaluation of another 16 CTA images (11 preoperative and 5 postoperative) with ground truth segmentation provided by experienced vascular surgeons, our method achieves a mean dice similarity score(DSC) of 0.910,0.849 and 0.821 for the entire aorta,true lumen and false lumen respectively.

Abstract:Accurate segmentation of cardiac tissues and organs based on cardiac computerized tomography angiography (CCTA) images has played an important role in biophysical modeling and medical diagnosis. The existing research on segmentation of cardiac tissues generally rely on limited public data, which may lead to unsatisfactory performance. In this paper, we first present a unique dataset of three-dimensional (3D) CCTA images collected from multiple centers to remedy this shortcoming. We further propose to efficiently create labels by solving the Laplace’s equation with given boundary conditions. The generated images and labels are confirmed by cardiologists. A deep learning algorithm, based on 3D-Unet model trained with a combined loss function, is proposed to simultaneously segment aorta, left ventricle, left atrium, left atrial appendage and myocardium from the CCTA images. Experimental evaluations show that the model trained with a proposed combined loss function can improve the segmentation accuracy and robustness. By efficiently producing a patient-specific geometry for simulation, we believe that this learning-based approach could provide an avenue to combine with biophysical modeling for the study of hemodynamics in cardiac tissues.
Automatic segmentation of thrombosed aortic dissection in post-operative CT-angiography images

Abstract:Purpose: The thrombus in the false lumen (FL) of aortic dissection (AD) patients is a meaningful indicator to determine aortic remodeling but difficult to measure in clinic. In this study, a novel segmentation strategy based on deep learning was proposed to automatically extract the thrombus in the FL in post-operative computed tomography angiography (CTA) images of AD patients, which provided an efficient and convenient segmentation method with high accuracy. Methods: A two-step segmentation strategy was proposed. Each step contained a convolutional neural network (CNN) to segment the aorta and the thrombus, respectively. In the first step, a CNN was used to obtain the binary segmentation mask of the whole aorta. In the second step, another CNN was introduced to segment the thrombus. The results of the first step were used as additional input to the second step to highlight the aorta in the complex background. Moreover, skip connection attention refinement (SAR) modules were designed and added in the second step to improve the segmentation accuracy of the thrombus details by efficiently using the low-level features. Results: The proposed method provided accurate thrombus segmentation results (0.903 ± 0.062 in dice score, 0.828 ± 0.092 in Jaccard index, and 2.209 ± 2.945 in 95% Hausdorff distance), which showed improvement compared to the methods without prior information (0.846 ± 0.085 in dice score) and the method without SAR (0.899 ± 0.060 in dice score). Moreover, the proposed method achieved 0.967 ± 0.029 and 0.948 ± 0.041 in dice score of true lumen (TL) and patent FL (PFL) segmentation, respectively, indicating the excellence of the proposed method in the segmentation task of the overall aorta. Conclusions: A novel CNN-based segmentation framework was proposed to automatically obtain thrombus segmentation for thrombosed AD in post-operative CTA images, which provided a useful tool for further application of thrombus-related indicators in clinical and research application.
Deep Learning-Based 3D Segmentation of True Lumen, False Lumen, and False Lumen Thrombosis in Type-B Aortic Dissection

Abstract:Patients with initially uncomplicated typeB aortic dissection (uTBAD) remain at high risk for developing late complications. Identification of morphologic features for improving risk stratification of these patients requires automated segmentation of computed tomography angiography (CTA) images. We developed three segmentation models utilizing a 3D residual U-Net for segmentation of the true lumen (TL), false lumen (FL), and false lumen thrombosis (FLT). Model 1 segments all labels at once, whereas model 2 segments them sequentially. Best results for TL and FL segmentation were achieved by model 2, with median (interquartiles) Dice similarity coefficients (DSC) of 0.85 (0.77-0.88) and 0.84 (0.82-0.87), respectively. For FLT segmentation, model 1 was superior to model 2, with median (interquartiles) DSCs of 0.63 (0.40-0.78). To purely test the performance of the network to segment FLT, a third model segmented FLT starting from the manually segmented FL, resulting in median (interquartiles) DSCs of 0.99 (0.98-0.99) and 0.85 (0.73-0.94) for patent FL and FLT, respectively. While the ambiguous appearance of FLT on imaging remains a significant limitation for accurate segmentation, our pipeline has the potential to help in segmentation of aortic lumina and thrombosis in uTBAD patients.Clinical relevance- Most predictors of aortic dissection (AD) degeneration are identified through anatomical modeling, which is currently prohibitive in clinical settings due to the timeintense human interaction. False lumen thrombosis, which often develops in patients with type B AD, has proven to show significant prognostic value for predicting late adverse events. Our automated segmentation algorithm offers the potential of personalized treatment for AD patients, leading to an increase in long-term survival.

Abstract:PURPOSE The purpose of this study, is to compare the sequences: 1) proton density (PD) BLADE with fat saturation (FS) coronal (COR), 2) PD FS COR, 3) multi-planar reconstruction (MPR) with 3mm slice thickness and 4) multi-planar reconstruction (MPR) with 1.5mm slice thickness, both derived from the T2 3D-double-echo steady state (DESS) with water excitation (WE) sagittal (SAG) sequence, regarding their abilities to identify changes in the femorotibial condyle cartilage in knee MRI examinations. METHODS Thirty three consecutive patients with osteoarthritis (18 females, 15 males; mean age 56 years, range 37-71 years), who had been routinely scanned for knee examination using the previously mentioned image acquisition techniques, participated in the study. A quantitative analysis was performed based on the relative contrast (ReCON) measurements, which were taken both on normal tissues as well as on pathologies. Additionally, a qualitative analysis was performed by two radiologists. Motion and pulsatile flow artifacts were evaluated. RESULTS The PD BLADE FS COR sequence produced images of higher contrast between Menisci and Cartilage, Fluid and Cartilage, Pathologies and Cartilage as well as of the Conspicuousness Superficial Cartilage and it was found to be superior to the other sequences (p<0.001). The sequences T2 3D DESS 1.5mm and T2 3D DESS 3mm were significantly superior to the PD BLADE FS COR and the PD FS COR sequences in the visualization of Bone, Cartilage and the Conspicuousness Deep Surface Cartilage. This pattern of results is also confirmed by the quantitative analysis. Also, in the PD BLADE FS COR sequence, the results of the Truncation Artifact, Pulsation Artifact, Edge Sharpness and Blurring Artifacts, were superior to the other sequences (p<0.001). CONCLUSION PD FS BLADE sequences are ideal for the depiction of the cartilage pathologies compared to the conventional PD FS and T2 3D DESS sequences.

Abstract:Background and objectiveAortic dissection is a severe cardiovascular pathology in which an injury of the intimal layer of the aorta allows blood flowing into the aortic wall, forcing the wall layers apart. Such situation presents a high mortality rate and requires an in-depth understanding of the 3-D morphology of the dissected aorta to plan the right treatment. An accurate automatic segmentation algorithm is therefore needed.MethodIn this paper, we propose a deep-learning-based algorithm to segment dissected aorta on computed tomography angiography (CTA) images. The algorithm consists of two steps. Firstly, a 3-D convolutional neural network (CNN) is applied to divide the 3-D volume into two anatomical portions. Secondly, two 2-D CNNs based on pyramid scene parsing network (PSPnet) segment each specific portion separately. An edge extraction branch was added to the 2-D model to get higher segmentation accuracy on intimal flap area.ResultsThe experiments conducted and the comparisons made show that the proposed solution performs well with an average dice index over 92%. The combination of 3-D and 2-D models improves the aorta segmentation accuracy compared to 3-D only models and the segmentation robustness compared to 2-D only models. The edge extraction branch improves the DICE index near aorta boundaries from 73.41% to 81.39%.ConclusionsThe proposed algorithm has satisfying performance for capturing the aorta structure while avoiding false positives on the intimal flaps.

Abstract:Purpose: Type A aortic dissection (TAAD) is a life-threatening aortic disease. The tear involves the ascending aorta and progresses into the separation of the layers of the aortic wall and the occurrence of a false lumen. Accurate segmentation of TAAD could provide assistance for disease assessment and guidance for clinical treatment. Methods: This study applied nnU-Net, a state-of-the-art biomedical segmentation network architecture, to segment contrast-enhanced CT images and quantify the morphological features for TAAD. CT datasets were acquired from 24 patients with TAAD. Manual segmentation and annotation of the CT images was used as the ground-truth. Two-dimensional (2D) nnU-Net and three-dimensional (3D) nnU-Net architectures with Dice- and cross entropy-based loss functions were utilized to segment the true lumen (TL), false lumen (FL), and intimal flap on the images. Four-fold cross validation was performed to evaluate the performance of the two nnU-Net architectures. Six metrics, including accuracy, precision, recall, Intersection of Union, Dice similarity coefficient (DSC), and Hausdorff distance, were calculated to evaluate the performance of the 2D and 3D nnU-Net algorithms in TAAD datasets. Aortic morphological features from both 2D and 3D nnU-Net algorithms were quantified based on the segmented results and compared. Results: Overall, 3D nnU-Net architectures had better performance in TAAD CT datasets, with TL and FL segmentation accuracy up to 99.9%. The DSCs of TLs and FLs based on the 3D nnU-Net were 88.42% and 87.10%. For the aortic TL and FL diameters, the FL area calculated from the segmentation results of the 3D nnU-Net architecture had smaller relative errors (3.89%-6.80%), compared to the 2D nnU-Net architecture (relative errors: 4.35%-9.48%). Conclusion: The nnU-Net architectures may serve as a basis for automatic segmentation and quantification of TAAD, which could aid in rapid diagnosis, surgical planning, and subsequent biomechanical simulation of the aorta.

Semi-supervised aortic dissections segmentation: A time-dependent weighted feedback fusion framework

Abstract:The segmentation of true lumen (TL) and false lumen (FL) plays an important role in the diagnosis and treatment of aortic dissection (AD). Although the deep learning methods have achieved remarkable performance for this task, a large number of labeled data are required for training. In order to alleviate the burden of doctors' labeling, in this paper, a novel time-dependent weighted feedback fusion based semi-supervised aortic dissections segmentation framework is proposed by effectively leveraging the unlabeled data. A feedback network is additionally extended to encode the predicted output from the backbone network into high-level feature space, which is then fused with the original feature information of the image to fix previous potential mistakes, thereby segmentation accuracy is improved iteratively. To utilize both labeled data and unlabeled data, the fused feature space flows into the network again to generate the second feedback and make sure consistency with the previous one. The utilization of image feature space provides better robustness and accuracy for the proposed structure. Experiments show that our method outperforms five existing state-of-the-art semi-supervised segmentation methods on both a type-B AD dataset and a public dataset.

Abstract:The {sup 154}Sm({sup 30}Si,5{ital n}){sup 179}Os reaction has been used to study rotational bands at high spin by means of extensive gamma-ray coincidence measurements. The new {alpha}={minus}1/2 signature partner of the 1/2{sup {minus}}(521) band has been found up to spin 31/2{sup {minus}} and the previously known {alpha}=+1/2 partner extended up to spin 45/2{sup {minus}}. The {alpha}={minus}1/2 and {alpha}=+1/2 components of the previously known 7/2{sup {minus}}(514) and the 9/2{sup +}(624) bands were extended up to spins 63/2{sup {minus}}, 53/2{sup {minus}} and 63/2{sup +},61/2{sup +}, respectively. The two components of the newly found band 5/2{sup {minus}}(512) have been established up to spins 59/2{sup {minus}} and 65/2{sup {minus}}. In addition, two other new bands were found, one of them with spins up to 61/2{sup +} decays to the 9/2{sup +}(624) band. Configurations have been proposed by comparing the signature splittings and the {ital B}({ital M}1)/{ital B}({ital E}2) ratios with the theoretical predictions. The latter points to rather strong Coriolis interaction between the 5/2{sup {minus}}(512) and 7/2{sup {minus}}(514) bands. The first band crossing observed in nine out of the ten bands is most probably due to the alignment of a pair of {ital i}{sub 13/2} neutrons, on the basis of a comparison of experimentalmore » Routhians with the cranking shell model calculations. In the three positive-parity sequences and in the {alpha}=+1/2 signature of the 5/2{sup {minus}}(512) band, second band crossings are also observed that are suggested to be caused by an alignment of two {ital h}{sub 9/2} protons.« less

Abstract:Objective To develop a machine learning algorithm to detect aortic dissection on non-contrast-enhanced CT and evaluate the diagnostic ability of the algorithm compared with those of radiologists. Methods This study developed a machine learning algorithm using single-center data collected between May 1st, 2022, and April 30th, 2023. Included in the study were 220 patients (110 with AD and 110 without AD). An AD detection algorithm was developed using a 3D full-resolution U-net architecture. We have continuously trained and developed an algorithm based on machine learning to segment the true and false lumens of the aorta and then determine whether there is aortic dissection. The diagnostic capabilities of our algorithm and three radiologists were also compared. Results The developed algorithm achieved an accuracy of 95.8%, a sensitivity of 93.2%, and a specificity of 92.6%. For radiologists, accuracy, sensitivity, and specificity were 88.6%, 90.6%, and 94.2%, respectively. The algorithm's performance was not significantly different from the mean performance of radiologists in terms of accuracy, sensitivity, or specificity. Conclusion The proposed algorithm showed comparable diagnostic performance to radiologists for detecting AD on non-contrast-enhanced CT, which suggests that the proposed algorithm has the potential to reduce misdiagnosis of AD to improve clinical outcomes.Aortic SegmentationAlgorithm development

Abstract:For the diagnosis and treatment of aortic dissection, where blood flows in between the layers of the aortic wall, the segmentation of true and false lumens is necessary. This is a challenging task because the intimal flap separating true and false lumens is thin, discontinuous and has a complex shape. In this paper we formulate lumen segmentation of aortic dissection as the extraction of aortic adventitia (the contour of aorta) and intima (the contour of true lumen). To this end, we propose a cascaded convolutional network for contour extraction on 2-D cross-section images, and then construct a 3-D adventitia and intima shape model. We performed a five-fold cross-validation on 45 aortic dissection CT volumes. The proposed method demonstrated a good performance for both aorta and true lumen segmentation, and the mean Dice similarity coefficient was 0.989 for aorta and 0.925 for true lumen.
Advanced Warning of Aortic Dissection on Non-Contrast CT: The Combination of Deep Learning and Morphological Characteristics

Abstract:Background: The identification of aortic dissection (AD) at baseline plays a crucial role in clinical practice. Non-contrast CT scans are widely available, convenient, and easy to perform. However, the detection of AD on non-contrast CT scans by radiologists currently lacks sensitivity and is suboptimal. Methods: A total of 452 patients who underwent aortic CT angiography (CTA) were enrolled retrospectively from two medical centers in China to form the internal cohort (341 patients, 139 patients with AD, 202 patients with non-AD) and the external testing cohort (111 patients, 46 patients with AD, 65 patients with non-AD). The internal cohort was divided into the training cohort ( n = 238), validation cohort ( n = 35), and internal testing cohort ( n = 68). Morphological characteristics were extracted from the aortic segmentation. A deep-integrated model based on the Gaussian Naive Bayes algorithm was built to differentiate AD from non-AD, using the combination of the three-dimensional (3D) deep-learning model score and morphological characteristics. The areas under the receiver operating characteristic curve (AUCs), accuracy, sensitivity, and specificity were used to evaluate the model performance. The proposed model was also compared with the subjective assessment of radiologists. Results: After the combination of all the morphological characteristics, our proposed deep-integrated model significantly outperformed the 3D deep-learning model (AUC: 0.948 vs. 0.803 in the internal testing cohort and 0.969 vs. 0.814 in the external testing cohort, both p < 0.05). The accuracy, sensitivity, and specificity of our model reached 0.897, 0.862, and 0.923 in the internal testing cohort and 0.730, 0.978, and 0.554 in the external testing cohort, respectively. The accuracy for AD detection showed no significant difference between our model and the radiologists ( p > 0.05). Conclusion: The proposed model presented good performance for AD detection on non-contrast CT scans; thus, early diagnosis and prompt treatment would be available.

Abstract:Background: Aortic dissection is a life-threatening clinical emergency, but it is often missed and misdiagnosed due to the limitations of diagnostic technology. In this study, we developed a deep learning-based algorithm for identifying the true and false lumens in the aorta on non-contrast-enhanced computed tomography (NCE-CT) scans and to ascertain the presence of aortic dissection. Additionally, we compared the diagnostic performance of this algorithm with that of radiologists in detecting aortic dissection. Methods: We included 320 patients with suspected acute aortic syndrome from three centers (Beijing Anzhen Hospital Affiliated to Capital Medical University, Fujian Provincial Hospital, and Xiangya Hospital of Central South University) between May 2020 and May 2022 in this retrospective study. All patients underwent simultaneous NCE-CT and contrast-enhanced CT (CE-CT). The cohort comprised 160 patients with aortic dissection and 160 without aortic dissection. A deep learning algorithm, three-dimensional (3D) full-resolution U-Net, was continuously trained and refined to segment the true and false lumens of the aorta to determine the presence of aortic dissection. The algorithm's efficacy in detecting dissections was evaluated using the receiver operating characteristic (ROC) curve, including the area under the curve (AUC), sensitivity, and specificity. Furthermore, a comparative analysis of the diagnostic capabilities between our algorithm and three radiologists was conducted. Results: In diagnosing aortic dissection using NCE-CT images, the developed algorithm demonstrated an accuracy of 93.8% [95% confidence interval (CI): 89.8-98.3%], a sensitivity of 91.6% (95% CI: 86.7-95.8%), and a specificity of 95.6% (95% CI: 91.2-99.3%). In contrast, the radiologists achieved an accuracy of 88.8% (95% CI: 83.5-94.1%), a sensitivity of 90.6% (95% CI: 83.5-94.1%), and a specificity of 94.1% (95% CI: 72.9-97.6%). There was no significant difference between the algorithm's performance and radiologists' mean performance in accuracy, sensitivity, or specificity (P>0.05). Conclusions: The algorithm proficiently segments the true and false lumens in aortic NCE-CT images, exhibiting diagnostic capabilities comparable to those of radiologists in detecting aortic dissection. This suggests that the algorithm could reduce misdiagnoses in clinical practice, thereby enhancing patient care.
Cyclic fatigue resistance of OneShape and WaveOne instruments using different angles of curvature.

Abstract:This study compared the cyclic fatigue resistance of OneShape (OS) instrument which used continuous rotation with WaveOne (WO) instrument which used reciprocating motion. OS size 25, 0.06 taper and WO primary size 25, 0.08 taper were used in simulated canals with 45°, 60°, and 90° angles of curvature until fracture. The number of cycles to fracture (NCF) was recorded to evaluate cyclic fatigue resistance. Data were statistically analyzed using two-way analysis of variance and Tukey's HSD test. Weibull analysis was performed on NCF data. WO had a significantly higher NCF than OS for all angles of curvature (p<0.05). According to Weibull distribution, WO instrument was predicted to have a higher number of cycles to survive than OS instrument. WO instrument which used the reciprocating motion had a greater resistance to cyclic fatigue than OS which used continuous rotation movement.
Deep-learning-based Image Segmentation for Image-based Computational Hemodynamic Analysis of Abdominal Aortic Aneurysms: A Comparison Study

Abstract:Computational hemodynamics is increasingly used to quantify hemodynamic characteristics in and around abdominal aortic aneurysms (AAA) in a patient-specific fashion. However, the time-consuming manual annotation hinders the clinical translation of computational hemodynamic analysis. Thus, we investigate the feasibility of using deep-learning-based image segmentation methods to reduce the time required for manual segmentation. Two of the latest deep-learning-based image segmentation methods, ARU-Net and CACU-Net, were used to test the feasibility of automated computer model creation for computational hemodynamic analysis. Morphological features and hemodynamic metrics of 30 computed tomography angiography (CTA) scans were compared between pre-dictions and manual models. The DICE score for both networks was 0.916, and the correlation value was above 0.95, indicating their ability to generate models comparable to human segmentation. The Bland-Altman analysis shows a good agreement between deep learning and manual segmentation results. Compared with manual (computational hemodynamics) model recreation, the time for automated computer model generation was significantly reduced (from ~2hr to ~10min). Automated image segmentation can significantly reduce time expenses on the recreation of patient-specific AAA models. Moreover, our study showed that both CACU-Net and ARU-Net could accomplish AAA segmentation, and CACU-Net outperformed ARU-Net in terms of accuracy and time-saving.
Automatic Segmentation of Coronary Lumen and External Elastic Membrane in Intravascular Ultrasound Images Using 8-Layer U-Net

Abstract:Background: Intravascular ultrasound (IVUS) is the golden standard in accessing the coronary lesions, stenosis, and atherosclerosis plaques. In this paper, a fully-automatic approach by an 8-layer U-Net is developed to segment the coronary artery lumen and the area bounded by external elastic membrane (EEM), i.e. cross section area (EEM-CSA). The database comprises of single-vendor and single-frequency IVUS data. Particularly, the proposed data augmentation of MeshGrid combined with flip and rotation operations is implemented, improving the model performance without pre- or post-processing of the raw IVUS images.Results: The mean intersection of union (mIoU) of 0.937 and 0.804 for the lumen and EEM-CSA respectively were achieved, which exceeded the manual labeling accuracy of the clinician.Conclusion: The accuracy shown by the proposed method is sufficient for subsequent reconstruction of 3D IVUS images, which is essential for doctors’ diagnosis in the tissue characterization of coronary artery walls and plaque compositions, qualitatively and quantitatively.

Abstract:Background & Objective Aortic dissection (AD) is a serious condition requiring rapid and accurate diagnosis. In this study, we aimed to improve the diagnostic accuracy of AD by presenting a novel method for aortic segmentation in computed tomography images that uses a combination of a transformer and a UNet cascade network with a Zoom-Out and Zoom-In scheme (ZOZI-seg). Methods The proposed method segments each compartment of the aorta, comprising the true lumen (TL), false lumen (FL), and thrombosis (TH) using a cascade strategy that captures both the global context (anatomical structure) and the local detail texture based on the dynamic patch size with ZOZI schemes. The ZOZI-seg model has a two-stage architecture using both a "3D transformer for panoptic context-awareness" and a "3D UNet for localized texture refinement." The unique ZOZI strategies for patching were demonstrated in an ablation study. The performance of our proposed ZOZI-seg model was tested using a dataset from Asan Medical Center and compared with those of existing models such as nnUNet and nnFormer. Results In terms of segmentation accuracy, our method yielded better results, with Dice similarity coefficients (DSCs) of 0.917, 0.882, and 0.630 for TL, FL, and TH, respectively. Furthermore, we indirectly compared our model with those in previous studies using an external dataset to evaluate its robustness and generalizability. Conclusions This approach may help in the diagnosis and treatment of AD in different clinical situations and provide a strong basis for further research and clinical applications.

Abstract:Objective. Automatic extraction of external elastic membrane border (EEM) and lumen-intima border (LIB) in intravascular ultrasound (IVUS) sequences aids atherosclerosis diagnosis. Existing IVUS segmentation networks ignored longitudinal relations among sequential images and neglected that IVUS images of different vascular conditions vary largely in intricacy and informativeness. As a result, they suffered from performance degradation in complicated parts in IVUS sequences.Approach. In this paper, we develop a 3D Pyramidal Densely-connected Network (PDN) with Adaptive learning and post-Correction guided by a novel cross-frame uncertainty (CFU). The proposed method is named PDN-AC. Specifically, the PDN enables the longitudinal information exploitation and the effective perception of size-varied vessel regions in IVUS samples, by pyramidally connecting multi-scale 3D dilated convolutions. Additionally, the CFU enhances the robustness of the method to complicated pathology from the frame-level (f-CFU) and pixel-level (p-CFU) via exploiting cross-frame knowledge in IVUS sequences. The f-CFU weighs the complexity of IVUS frames and steers an adaptive sampling during the PDN training. The p-CFU visualizes uncertain pixels probably misclassified by the PDN and guides an active contour-based post-correction.Main results. Human and animal experiments were conducted on IVUS datasets acquired from atherosclerosis patients and pigs. Results showed that the f-CFU weighted adaptive sampling reduced the Hausdorff distance (HD) by 10.53%/7.69% in EEM/LIB detection. Improvements achieved by the p-CFU guided post-correction were 2.94%/5.56%.Significance. The PDN-AC attained mean Jaccard values of 0.90/0.87 and HD values of 0.33/0.34 mm in EEM/LIB detection, preferable to state-of-the-art IVUS segmentation methods.
