Abstract:Background: Health care linen is a source of healthcare-associated infections. Hospital laundry services play a vital role in linen decontamination. Effective laundry is essential in preventing hospital-acquired infections, especially in multidrug-resistant organisms. Aims and Objectives: The study aimed to check the microbial load in linen after the laundry process and the during the supply chain in the hospital. We investigated the bacterial load in linen before and after the laundering process and the linen from stored linen banks. Materials and Methods: Linens ( n =125) from the hospital were collected in different stages of the laundry process [31 linen before decontamination; 94 linen after decontamination (Post washing = 30; Linen Bank = 34; Ward 30)]. A square inch of linen from each linen was vortexed in normal saline, and colony forming units (CFU)/inch of linen were calculated using appropriate media. The identification and susceptibility of organisms were performed using MALDI-TOF MS and CLSI disk diffusion methods, respectively. The difference in the load of the organism was analysed between different stages of the laundry and storage process. Results: We found that100% of pre-decontamination linen harbour viable organisms (22.59% including multidrug-resistant pathogenic organisms: MDROs), which decreased to 40% (no MDROs) post-decontamination. The overall load was reduced by a factor of 103.77. Conclusion: The microbial contamination of linen is common, and there is an urgent need to develop consensus on the microbial examination frequency and acceptable range of organisms on hospital linen worldwide.

Abstract:Antimicrobial textiles with improved functionality have a wide range of applications, including health and hygiene products, particularly garments worn next to the skin, and a number of medical applications, including infection control and barrier materials. Furthermore, these textiles are used in household materials, air filters, food packaging, athletic wear, storage, ventilation, and water purification systems. Cotton-based materials are hygienic and are commonly used in non-implantable hygiene medical textile products such as lint, gauze, bandages, wound dressing, and sanitary napkins. In terms of absorbency, comfort, and durability, non-implantable cotton fabric materials outperform standard clothing materials. Several studies show that depending on the type of components present in the plant extract, several plant extracts can be effective against both Gram-positive and Gram-negative bacteria. As a result, research into eco-friendly antimicrobial agents and their application on various textile products is gaining worldwide attention. Plants such as neem, eucalyptus, aloe vera, and clove contain natural antimicrobial compounds. Extracts of neem, eucalyptus, aloe vera, and clove are used in textile finishing. These bioactive substances could be used to impart antimicrobial properties to textiles for the development of health care and hygiene products. This review focuses on research into various natural antimicrobial agents derived from plant extracts and their applications as textile finishing agents for healthcare and hygiene applications.

Abstract:The colonization of textiles by axillary skin bacteria produces an unpleasant odour due to the rapid growth of a selective community of bacteria. Such colonized textiles subsequently act as vectors for transmitting nosocomial infections among healthcare workers and patients. An in-depth understanding of bacterial behaviour on soft surfaces like fabrics is necessary to mitigate the transmission of infections. This study examined the effect of artificial human sweat on biofilm formation by Staphylococcus aureus , Escherichia coli , Enterococcus faecalis , and Pseudomonas aeruginosa , on three fabrics, viz. polyester, cotton, and polyester-cotton (70:30) blend. Artificial sweat was constituted to replicate the natural human sweat on textiles. Using atomic force microscopy, the three-dimensional topography of the biofilm was determined, and scanning electron microscopy was employed to visualise the biofilm that had developed on the fabrics. All bacterial strains showed maximum growth on polyester fabric in the presence of sweat. P. aeruginosa and S. aureus were found to be strong biofilm producers, whereas E. coli and E. faecalis were moderate producers. The ability of the four bacterial strains to form biofilm was related to their production of extracellular polymeric substances (EPS). P. aeruginosa produced viscous EPS in contrast to the EPS produced by other bacterial strains. In conclusion, this study corroborates that sweat plays a major role in the colonization of textiles by bacteria. Regular practice of fabric hygiene, and the development of modified fabrics with anti-pathogen properties, could potentially reduce the prevalence of nosocomial infections in healthcare settings.

Scalable Ellipsoidal Classification for Entangled States

Abstract:The Separability Problem is approached from the perspective of Ellipsoidal Classification. A Density Operator of dimension N can be represented as a vector in a real vector space of dimension N−1, whose components are the projections of the matrix onto some selected basis. We suggest a method to test separability, based on successive optimization programs. First, we find the Minimum Volume Covering Ellipsoid that encloses a particular set of properly vectorized bipartite separable states, and then we compute the Euclidean distance of an arbitrary vectorized bipartite Density Operator to this ellipsoid. If the vectorized Density Operator falls inside the ellipsoid, it is regarded as separable, otherwise it will be taken as entangled. Our method is scalable and can be implemented straightforwardly in any desired dimension. Moreover, we show that it allows for detection of Bound Entangled States.

Abstract:Increasing numbers of hospital-acquired infections have generated much attention over the last decade. The public has linked the so-called 'superbugs' with their experience of dirty hospitals but the precise role of environmental cleaning in the control of these organisms remains unknown. Until cleaning becomes an evidence-based science, with established methods for assessment, the importance of a clean environment is likely to remain speculative. This review will examine the links between the hospital environment and various pathogens, including meticillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci, norovirus, Clostridium difficile and acinetobacter. These organisms may be able to survive in healthcare environments but there is evidence to support their vulnerability to the cleaning process. Removal with, or without, disinfectants, appears to be associated with reduced infection rates for patients. Unfortunately, cleaning is often delivered as part of an overall infection control package in response to an outbreak and the importance of cleaning as a single intervention remains controversial. Recent work has shown that hand-touch sites are habitually contaminated by hospital pathogens, which are then delivered to patients on hands. It is possible that prioritising the cleaning of these sites might offer a useful adjunct to the current preoccupation with hand hygiene, since hand-touch sites comprise the less well-studied side of the hand-touch site equation. In addition, using proposed standards for hospital hygiene could provide further evidence that cleaning is a cost-effective intervention for controlling hospital-acquired infection.

Abstract:Introduction: A nosocomial infection is contracted within a period of 48 hours following admission to a hospital or within 48 hours after being discharged from the hospital. Patients in the intensive care unit (ICU) are vulnerable to infections due to various factors, including their pre-existing medical conditions, medical procedures such as surgery, intubation, and catheterization, as well as their potential exposure to microorganisms from fellow patients. In addition to this, the presence of contaminated equipment represents a significant contributor to the occurrence of hospital-acquired infections caused by multidrug-resistant organisms among patients in critical areas. Methods: This cross-sectional study was conducted in coronary care and critical areas of Holy Family Hospital, Rawalpindi. A total of 415 samples were processed at the microbiology laboratory over a 4 month period from which 112 were swabs taken from equipment of the Medical, Surgical and Pediatric intensive care Units (MICU, SICU, and PICU) and the Coronary Care Unit. The remaining 303 samples were blood, urine, tracheal secretions/tips, pus and Foley tip cultures received in the microbiology laboratory from MICU, SICU, CCU and PICU. Samples were cultured on the appropriate media and observed after the required period. Results: High levels of contamination (58.03%) were identified on a wide range of healthcare equipment. The total of infected patients were 37.6%. Most common infection site was the respiratory tract. Acinetobacter spp. was the most predominant isolate among patients in MICU, SICU and CCU while Klebsiella spp. was the predominant isolate from the PICU. Conclusion: A significant degree of contamination is observed across a diverse array of healthcare equipment. Nevertheless, the prevalence of contaminated instruments and the subsequent potential for acquiring a healthcare-associated infection (HAI) can be significantly mitigated through the consistent implementation of cleaning, disinfection, and sterilization protocols for medical equipment. A novel comprehensive efficacy test for textiles intended for use in the healthcare setting

Abstract:Soft surfaces, including textiles are found throughout healthcare settings. Pathogens can survive for long periods of time on textiles, and can be transferred to and from the skin. Antimicrobial fabrics are used as an engineering control to prevent infection. Efficacy testing standards have limitations, including single microorganism challenges, multiple fabric plies tested, and lengthy contact times. We developed a novel method that better models in-use conditions through testing standardized mixtures of pathogens and normal skin microorganisms, artificial soils, and a 15-min contact time. Reproducible growth of all microorganisms from frozen stocks was achieved using this method. A novel rechargeable, monitorable N-halamine cotton cellulose fabric, containing 5885 ± 98 ppm of active chlorine, was evaluated with the new method using PBS, artificial sweat, and artificial sweat plus 5% serum as soil. Pathogens tested included Acinetobacter baumannii, Candida albicans, Escherichia coli, vancomycin-resistant Enterococcus faecalis, methicillin-resistant Staphylococcus aureus, methicillin-susceptible Staphylococcus aureus, and Pseudomonas aeruginosa. Each was tested singly and in the presence of a representative normal skin flora mixture, including: Acinetobacter lwoffii, Corynebacterium striatum, Micrococcus luteus, and Staphylococcus epidermidis. When tested singly, all microorganisms were reduced by 3.00 log10 or greater, regardless of artificial soil. In mixture, 4.00 log10 or greater reductions were achieved for all microorganisms. These results suggest that the novel testing method can be used to provide more comprehensive and realistic efficacy information for antimicrobial textiles intended for use in healthcare. Furthermore, the N-halamine fabric demonstrated efficacy against multiple pathogens, singly and in mixtures, regardless of the presence of artificial soils.

Abstract:Polymer-based textiles have a major impact on human well-being, as they provide the desired functional protection and aesthetic comfort when worn. However, natural and synthetic polymer-based textiles can also pose serious health risks, as they are surfaces that allow the adhesion of various bacteria, including pathogenic bacteria. To minimise these problems, antibacterial chemical treatments are generally applicable in the case of polymer-based textiles. However, to avoid the use of potentially toxic chemicals, sustainable approaches require the customised design of non-adhesive polymer-based textiles, considering their chemical, physicochemical, constructional, and textural properties. Before designing, several articles are required to gain sufficient knowledge of the described object. Despite the urgent need to combat bacteria (on polymer-based textiles), which pose a serious global health risk, only a few review articles have been published that address bacterial adhesion in the context of superhydrophobic and antibacterial textile materials, while only one review article holistically addresses the textile factors and their influence on this phenomenon. The aim of this review article is to expand the insufficient knowledge about bacterial adhesion to polymer-based textiles on the basis of theoretical findings and real examples through a high degree of structuring, simplification, holistic consideration, and visualization. Therefore, this review provides an insight into the mechanisms involved in bacterial adhesion and a comprehensive overview of the influence of different textile factors, such as chemical composition, hydrophilicity/hydrophobicity, surface charge, surface free energy, roughness, and porosity, on bacterial adhesion. To emphasise the importance of the synergistic effect of the combined textile factors, examples of the influence of hydrophilicity/hydrophobicity in combination with surface charge, surface roughness, and porosity are discussed. From the review, it can be concluded that the combination of hydrophilicity/hydrophobicity and the surface charge of textile fibres and bacteria is crucial for bacterial adhesion, with roughness and porosity being the most important factors among the constructive and textural properties of polymer-based textiles.

Abstract:Hospital-acquired (nosocomial) infections (HAIs) increase morbidity, mortality and medical costs. In the USA alone, nosocomial infections cause about 1.7 million infections and 99 000 deaths per year. HAIs are spread by numerous routes including surfaces (especially hands), air, water, intravenous routes, oral routes and through surgery. Interventions such as proper hand and surface cleaning, better nutrition, sufficient numbers of nurses, better ventilator management, use of coated urinary and central venous catheters and use of high-efficiency particulate air (HEPA) filters have all been associated with significantly lower nosocomial infection rates. Multiple infection control techniques and strategies simultaneously ('bundling') may offer the best opportunity to reduce the morbidity and mortality toll of HAIs. Most of these infection control strategies will more than pay for themselves by saving the medical costs associated with nosocomial infections. Many non-pharmacological interventions to prevent many HAIs will also reduce the need for long or multiple-drug antibiotic courses for patients. Lower antibiotic drug usage will reduce risk of antibiotic-resistant organisms and should improve efficacy of antibiotics given to patients who do acquire infections.

Abstract:Antibiotic-resistant bacteria, ESKAPE pathogens, present a significant and alarming threat to public health and healthcare systems. This study addresses the urgent need to combat antimicrobial resistance by exploring alternative ways to reduce the health and cost implications of infections caused by these pathogens. To disrupt their transmission, integrating antimicrobial textiles into personal protective equipment (PPE) is an encouraging avenue. Nevertheless, ensuring the effectiveness and safety of these textiles remains a persistent challenge. To achieve this, we conduct a comprehensive study that systematically compares the effectiveness and potential toxicity of five commonly used antimicrobial agents. To guide decision making, a MULTIMOORA method is employed to select and rank the optimal antimicrobial textile finishes. Through this approach, we determine that silver nitrate is the most suitable choice, while a methoxy-terminated quaternary ammonium compound is deemed less favorable in meeting the desired criteria. The findings of this study offer valuable insights and guidelines for the development of antimicrobial textiles that effectively address the requirements of effectiveness, safety, and durability. Implementing these research outcomes within the textile industry can significantly enhance protection against microbial infections, contribute to the improvement of public health, and mitigate the spread of infectious diseases.

Abstract:Personal protective equipment (PPE) is critical to protect healthcare workers (HCWs) from highly infectious diseases such as COVID-19. However, hospitals have been at risk of running out of the safe and effective PPE including personal protective clothing needed to treat patients with COVID-19, due to unprecedented global demand. In addition, there are only limited manufacturing facilities of such clothing available worldwide, due to a lack of available knowledge about relevant technologies, ineffective supply chains, and stringent regulatory requirements. Therefore, there remains a clear unmet need for coordinating the actions and efforts from scientists, engineers, manufacturers, suppliers, and regulatory bodies to develop and produce safe and effective protective clothing using the technologies that are locally available around the world. In this review, we discuss currently used PPE, their quality, and the associated regulatory standards. We survey the current state-of-the-art antimicrobial functional finishes on fabrics to protect the wearer against viruses and bacteria and provide an overview of protective medical fabric manufacturing techniques, their supply chains, and the environmental impacts of current single-use synthetic fiber-based protective clothing. Finally, we discuss future research directions, which include increasing efficiency, safety, and availability of personal protective clothing worldwide without conferring environmental problems.This article has not yet been cited by other publications.

Abstract:Textiles in all forms act as carriers in transmitting pathogens and provide a medium of microbial growth, especially in those fabrics which are used in sports, medical and innerwear clothing. More attention towards hygiene and personal healthcare made it a necessity to develop pathogen-free textiles. Synthetic and natural antimicrobial compositions are used to control and reduce microbial activity by killing or inhibiting microbial growth on textiles. Synthetic metallic nanoparticles of Ag, Zn, Cu Ti and Ga are the most commonly and recently used advanced nanocomposites. Synthetic organic materials such as triclosan, quaternary ammonium compounds, polyhexamethylene biguanide, and N-halamines have proven antimicrobial activity. Carbon quantum dots are one of the advanced nanomaterials prepared from different kinds of organic carbon material with photoluminescence efficiency also work efficiently in antimicrobial textiles. A greener approach for producing natural antimicrobial textiles has gained significant importance and demand for personal care due to their less toxic effects on health and the environment In comparison to synthetic. The naturally existing materials including extracts and essential oils of plants have significant applications for antimicrobial textiles. Additionally, a number of animal extracts are also used as antimicrobial agents include chitosan, alginate, collagen hydrolysate to prepare naturally treated antimicrobial textiles. This review focuses on the comparative performance of antimicrobial fabrics between synthetic and natural materials. Textiles with synthetic substances cause health and environmental concerns whereas textiles treated with natural compositions are more safe and eco-friendly. Finally, it is concluded that textiles modified with natural antimicrobial compositions may be a better alternative and option as functional textiles.
Field Study of Activity of Antimicrobial Polypropylene Textiles

Abstract:In this work, an in situ study is presented of the impact of textile materials used in healthcare facilities on microbial colonization of textile surfaces. The available literature describes antimicrobial active textiles and their effectiveness in laboratory conditions. However, the quantification of the impact on the microbiome of healthcare facilities has not been investigated so far. Polypropylene yarns doped with silver phosphate glass and zinc pyrithione were prepared and used for the production of bed sheets and clothing for healthcare personnel. Subsequently, measurements of airborne particles and viable microorganisms on given textiles were conducted in a private surgery clinic for 3 weeks, comparing the counts of viable microorganisms before and after replacing staff clothing and bedding on examination and the surgical bed with said polypropylene cloth. A significant reduction in airborne particles and viable microorganisms was expected based on previous studies on the use of polypropylene textiles in operating rooms. In this study, a significant reduction in viable airborne fungi and viable microorganisms on monitored textiles was observed by multiple methods. However, the effect on airborne microorganisms seems insignificant in areas with frequent patient traffic. The textile described here represents a new additional way of protecting patients and medical personnel from healthcare-associated infections while using a modification of proven production procedures and commercially usable materials without legislative restrictions.

During current COVID-19 crises, the antimicrobial textiles primarily those utilized in hospital by doctors and paramedical staff have become increasingly important. Thus, there is an unmet requirement to develop antimicrobial textiles for infection control and hygiene practices. Metallic nanoparticles exhibit great effectiveness towards resistant microbial species making them a potential solution to the increasing antibiotic resistance. Due to this, nanoparticles particularly copper and silver have become most prevalent forms of antibacterial finishing agents for the development of antimicrobial textiles. This review is mainly focused on the significance of copper and silver nanoparticles for the development of antimicrobial textiles. The comparative analysis of the antibacterial effectiveness of copper and silver nanoparticles as well as the possible physical and chemical interactions responsible for their antibacterial action are explained. The negative impact of pathogenic microbes on textiles and possible interactions of antimicrobial agents with microbes have also been highlighted. The significance of nanotechnology for the development of antimicrobial textiles and their applications in medical textiles domain have also been discussed. Various green synthesis and chemical methods used for the synthesis of Ag and Cu nanoparticles and their application on textile substrates to impart antimicrobial functionality have also been discussed. The various qualitative and quantitative standard testing protocols utilised for the antimicrobial characterization of textiles have also discussed in this review. The developed Cu and Ag coated textiles could be effectively applied in the field of hospital textiles for the preparation of antibacterial scrub suits, surgical gowns, panel covers, protective clothing, bedding textiles, coveralls, wound dressings, table covers, curtains, and chair covers etc.

The increasing usage of highly-developed medical devices, drugs and disposable products are a drain on natural resources as well as financial ones. In this situation, it is a major task for hospital epidemiologists to maintain high standards of hygiene while reducing environmental pollution, reducing consumption of limited natural resources, and minimizing costs. The reduction of hospital waste, the control of polluting and toxic emissions, the avoidance of unnecessary disinfection procedures and disposables, the implementation of energy and water saving technologies are practicable measures in hospital ecology. To realize a sustainable development within hospitals, it is necessary that the need to maintain a balance between effective infection control and a good ecological environment is recognized and supported by health-care workers and the hospital management.

Timing and route of contamination of hospitalized patient rooms with healthcare-associated pathogens

Abstract:Objective: To investigate the timing and routes of contamination of the rooms of patients newly admitted to the hospital. Design: Observational cohort study and simulations of pathogen transfer. Setting: A Veterans' Affairs hospital. Participants: Patients newly admitted to the hospital with no known carriage of healthcare-associated pathogens. Methods: Interactions between the participants and personnel or portable equipment were observed, and cultures of high-touch surfaces, floors, bedding, and patients' socks and skin were collected for up to 4 days. Cultures were processed for Clostridioides difﬁcile, methicillin-resistant Staphylococcus aureus (MRSA), and vancomycin-resistant enterococci (VRE). Simulations were conducted with bacteriophage MS2 to assess plausibility of transfer from contaminated floors to high-touch surfaces and to assess the effectiveness of wearing slippers in reducing transfer. Results: Environmental cultures became positive for at least 1 pathogen in 10 (59%) of the 17 rooms, with cultures positive for MRSA, C. difficile, and VRE in the rooms of 10 (59%), 2 (12%), and 2 (12%) participants, respectively. For all 14 instances of pathogen detection, the initial site of recovery was the floor followed in a subset of patients by detection on sock bottoms, bedding, and high-touch surfaces. In simulations, wearing slippers over hospital socks dramatically reduced transfer of bacteriophage MS2 from the floor to hands and to high-touch surfaces. Conclusions: Floors may be an underappreciated source of pathogen dissemination in healthcare facilities. Simple interventions such as having patients wear slippers could potentially reduce the risk for transfer of pathogens from floors to hands and high-touch surfaces.

Abstract:Summary Healthcare hygiene plays a crucial role in the prevention of healthcare-associated infections. Patients admitted to a room where the previous occupant had a multi-drug-resistant bacterial infection are at an increased risk of colonization and infection with the same organism. A 2006 systematic review by Kramer et al. found that certain pathogens can survive for months on dry surfaces. The aim of this review is to update Kramer et al. 's previous review and provide contemporary data on the survival of pathogens relevant to the healthcare environment. We systematically searched Ovid MEDLINE, CINAHL and Scopus databases for studies that described the survival time of common nosocomial pathogens in the environment. Pathogens included in the review were bacterial, viral, and fungal. Studies were independently screened against predetermined inclusion/exclusion criteria by two researchers. Conflicts were resolved by one of two senior researchers. A spreadsheet was developed for the data extraction. The search identified 1736 studies. Following removal of duplicates and application of the search criteria, the synthesis of results from 62 included studies were included. 117 organisms were reported. The longest surviving organism reported was Klebsiella pneumoniae which was found to have persisted for 600 days. Common pathogens of concern to infection prevention and control, can survive or persist on inanimate surfaces for months. This data supports the need for a risk-based approach to cleaning and disinfection practices, accompanied by appropriate training, audit and feedback which are proven to be effective when adopted in a 'bundle' approach.
