Abstract:Purpose: Simultaneous bilateral cochlear implantation is often seen as the gold-standard intervention for many people with bilateral deafness. However, this intervention is not as commonly performed in China. This paper presents the surgical technique and the peri- and postoperative safety results from simultaneous bilateral implantation and compares them with those from sequential bilateral implantation. Methods: Twenty nine children (aged 12‒18 months) met the inclusion/exclusion criteria and took part in this study. 10 participants received a unilateral cochlear implantation; 19 participants received simultaneous bilateral cochlear implantation. The same standard surgical technique (transmastoid facial recess approach with round window insertion) was used in all implantations. Safety was assessed via monitoring peri- and postoperative adverse events. The helpfulness of select surgical tools was assessed via questionnaire. Results: No adverse events were reported for any participants. All queried surgical tools were helpful in all 244 recorded responses. Conclusion: Simultaneous bilateral cochlear implantation can be performed using the same surgical technique as unilateral implantation and poses no increased safety risk for children aged 1‒2 years.
Comparison of Outcomes in Unilateral and Bilateral Pediatric Cochlear Implants: Our Experience

Abstract:The aim of our study is to compare the outcomes in unilateral and bilateral cochlear implants in pediatric age and also between simultaneous and sequential cochlear implant surgery. This retrospective study was carried out with 83 children aged between 12 months to 2.5 years which included 41 children with bilateral Cochlear implants and 42 with unilateral implants. Out of these 41 children, 21 were simultaneous and 20 were sequential cochlear implant. All the children were operated at civil hospital Gandhinagar, Gujarat, India. CAP, SIR, localization, traffic noise and speech in noise scores were assessed at regular intervals over the period of 4 years. Also the drug administration time, surgical time, operating room time were assessed for simultaneous and sequential cochlear implant surgery. Children with bilateral simultaneous implants fared significantly better with CAP, SIR, localization, speech noise and traffic noise scores than sequential bilateral implants and unilateral implants with a significant difference of means t tests between the two groups. Simultaneous cochlear implant surgery is associated with reduced surgical time, operating room time, it shortens the total in patient stay. There is less of drug administration and bilateral ones are stimulated simultaneously. Bilateral cochlear implants perform better with respect to auditory perception skills and spontaneous speech when compared with unilateral implants, but simultaneous surgery is better and safe option for pediatric cochlear implantation.

Abstract:The aim of this study was to determine whether children who received early, simultaneous, bilateral cochlear implants showed more rapid development of the central auditory pathways compared to children who received early, sequential, bilateral implants. In 20 children, over the first 15 months following bilateral implantation, we assessed longitudinal changes in the morphology and latency of the P1 cortical response, which is generated within the auditory cortex. Our results showed that by 3.5 months post-implantation, mean P1 latencies for both groups of children were within normal limits. Overall, the developmental trajectory of the P1 response did not differ significantly for the two groups over the 15-month period. Our results suggest that bilateral implantation, whether sequential or simultaneous, occurring within a sensitive period of 3.5 years, takes advantage of the high degree of plasticity in the developing central auditory nervous system.

Abstract:From a series of 460 dementia patients referred to a regional brain bank, 14 (3%) patients had a pathologic diagnosis of primary degeneration of the brain involving multiple sites (frontoparietal cortex, striatum, medial thalamus, substantia nigra, and hypoglossal nucleus), with cell loss and astrocytosis. There were no neuronal inclusions and essentially no senile plaques. This entity, which we have termed “dementia lacking distinctive histology” (DLDH), presented with memory loss and personality changes, and led to death, usually within 2 to 7 years. Dysarthria and dysphagia were prominent in the later phases of the illness in most patients. The psychometric findings of some of the patients were consistent with a “frontal” lobe dementia. A few patients had prominent caudate atrophy on CT as well as neuropathologically. Eight of our patients had positive family histories for neurologic disease, mainly dementia. DLDH, in addition to Pick's disease, is a major member of the frontal-lobe dementia group. In patients under age 70 years, the frontal lobe dementias represent an important diagnostic consideration.

Abstract:Introduction: Benefits of bilateral cochlear implants (CI) may be compromised by delays to implantation of either ear. This study aimed to evaluate the effects of sequential bilateral CI use in children who received their first CI at young ages, using a clinical set-up. Methods: One-channel cortical auditory evoked potentials and speech perception in quiet and noise were evoked at repeated times (0, 3, 6, 12 months of bilateral CI use) by unilateral and bilateral stimulation in 28 children with early-onset deafness. These children were unilaterally implanted before 3.69 years of age (mean ± SD of 1.98 ± 0.73 years) and received a second CI after 5.13 ± 2.37 years of unilateral CI use. Comparisons between unilaterally evoked responses were used to measure asymmetric function between the ears and comparisons between bilateral responses and each unilateral response were used to measure the bilateral benefit. Results: Chronic bilateral CI promoted changes in cortical auditory responses and speech perception performance; however, large asymmetries were present between the two unilateral responses despite ongoing bilateral CI use. Persistent cortical differences between the two sides at 1 year of bilateral stimulation were predicted by increasing age at the first surgery and inter-implant delay. Larger asymmetries in speech perception occurred with longer inter-implant delays. Bilateral responses were more similar to the unilateral responses from the first rather than the second CI. Conclusion: These findings are consistent with the development of the aural preference syndrome and reinforce the importance of providing bilateral CIs simultaneously or sequentially with very short delays. Abstract:Most physicians think of acute glomerulonephritis as a disease of children and young adults. Indeed, the majority of series show a strong predilection of this illness for the young age groups, 1-3 its incidence peaking between the ages of 3 and 7 years. 4 However, elderly patients are not immune to the disorder and acute glomerulonephritis has been reported to develop at a very late age. 5-8 Cases among the aged continue to occur, even in an era in which acute poststreptococcal glomerulonephritis is becoming a rarity in the pediatric services of developed countries 9 and when the incidence of streptococcus-related diseases seems to be declining. 10 The clinical features of acute postinfectious glomerulonephritis in the elderly do not differ appreciably from those in other age groups and basically reflect an acute nephritic syndrome with azotemia, oliguria, hypertension, edema, hematuria, and varying degrees of proteinuria. Abstract:Purpose The aim of the study was to assess whether sequential cochlear implantation (CI) with a prolonged interimplant interval (M = 15.2 years) between the first and second CIs benefited speech recognition and health-related quality of life. Method This prospective study included 14 prelingually deafened participants who received their second CI after a prolonged interimplant interval (M = 15.2 years). Additionally, speech recognition ability over a 12-month period of bilateral implant use was investigated. The results of the speech recognition test in both quiet and noisy conditions were statistically analyzed for each CI alone and both CIs together. Nijmegen Cochlear Implant Questionnaire scores were also collected at activation and at 12 months after activation. Results Improvements in speech recognition ability were observed following the use of the first implant alone and with the use of both implants together; however, progress was much slower with the use of the second implant alone, following its introduction. Furthermore, a significant difference in the trajectory of speech recognition ability was observed between the first and the second implanted ear. According to Nijmegen Cochlear Implant Questionnaire scores, all participants benefitted from bilateral CI after 12 months. Conclusions Prolonged interimplant intervals resulted in asymmetrical speech recognition abilities. A significant improvement in the speech recognition scores was observed with the first implanted ear, and much slower progress was observed with the second implanted ear. However, the "poorer" second implanted ear could provide a considerable beneficial effect on the improved speech recognition and health-related quality of life with the bilateral CI. Towards automated design improvement through combinatorial optimisation

Abstract:In this paper we present a novel approach to the problem of automated design improvement: by treating objectoriented design as a combinatorial optimisation of metrics we have developed a prototype software engineering tool capable of improving a design with respect to a conflicting set of goals. As the prioritisation of different goals is determined by weights associated with each metric, we also describe here a method of assigning coherent weights to a set of metrics based on object-oriented design heuristics. The combinatorial optimisation approach to automated design improvement is illustrated here by means of a simple case study, which shows the effect of applying our prototype tool to a small inheritance hierarchy. Results indicate that a balance between metrics has been achieved, as several potentially conflicting design goals are accommodated.
A Gly482Ser polymorphism of the peroxisome proliferator-activated receptor-gamma coactivator-1 (PGC-1) gene is associated with type 2 diabetes in Caucasians.

Abstract:The PGC-1 gene has been implicated in the regulation of several genes controlling energy metabolism. The prevalent Gly482Ser polymorphism of the PGC-1 gene has been shown to be associated with type 2 diabetes in some but not all studies. The aim of this study was to analyse whether the Gly482Ser variant is a risk factor for development of type 2 diabetes in Slovene population (Caucasians). Genotyping of the Gly482Ser polymorphism was performed for 545 subjects: 305 patients with type 2 diabetes and 240 non-diabetic controls. The Gly482Ser genotype distribution in patients with type 2 diabetes (AA = 11.5%, AG = 42.3%, GG = 46.2%) differed from genotype distribution in non-diabetic controls (AA = 6.3%, AG = 46.3%, GG = 47.5%), and the AA genotype was associated with 1.9-times increased risk of type 2 diabetes (95% confidence interval 1.0-3.6; P = 0.036). In conclusion, we suggest that the AA genotype of the Gly482Ser polymorphism of the PGC-1 gene should be considered as a risk factor for the development of type 2 diabetes in Caucasians.

Abstract:Tissues of 156 adult herring gulls (Larus argentatus) were sampled in the early 1990s from 11 colonies throughout the Laurentian Great Lakes and 2 reference colonies in Lake Winnipeg and the Bay of Fundy. Gulls from 1 or more Great Lakes differed from Lake Winnipeg or the Bay of Fundy for 17 of 19 clinical biochemical measures, whereas the freshwater and marine reference sites differed in only 3. Three differed with sex. There was little evidence to suggest that these differences reflect genotypic differences. Plasma thyroxine, albumin, calcium, magnesium, inorganic phosphorus, triglyceride, bile acids, total protein, uric acid, and urea nitrogen concentrations and aspartate aminotransferase (AST) activity were lower in gulls from one or more Great Lakes than for gulls from one or both reference sites, while those for globulins and glucose were higher. Highly carboxylated porphyrins accumulated in the livers of Great Lakes gulls and ethoxyresorufin O-deethylase (EROD) activity was induced. There was resistance to PCB/TCDD-induced EROD induction in the Lake Erie colonies. Gulls from five colonies were unable to obtain adequate food to maintain average body condition. Body condition was associated with seven biochemical measures. Colonies in designated Areas of Concern as well as those with high liver polychlorinated biphenyl (PCB) concentrations both differed for 50% of the biochemical measures. Associations between biochemical measures and δ15N-derived trophic position and/or contaminant levels in tissues suggest the effects may be toxicopathic responses. Associations were most frequently with PCBs and dioxin-like contaminants. The health of adult herring gulls varied with breeding location and “lifestyle” in the early 1990s, and Great Lakes gulls suffered from chemical and nutritional stressors that modulated physiological processes and endocrine function.
Inactivation of Aeromonas hydrophila by Fe(II)-related-radical generation in oxidizing groundwaters

Abstract:The survival of Aeromonas hydrophila AWWX1 in filter-sterilized phreatic groundwaters was studied by using viable counts. Aeromonas counts rapidly decreased 2 to 3 log units in oxidizing raw groundwaters from Snellegem and Beernem, Belgium (Snellegem-raw and Beernem-raw, respectively), containing high concentrations of Fe2+ (460 to 1,070 microM). The rapid decline in viable counts of Aeromonas cells in the oxidizing raw groundwater of Snellegem was prevented by the addition of an Fe2+ chelator (2,2'-dipyridyl) or compounds (i.e., ascorbic acid and catalase) that act on toxic oxygen species. The results suggest that free radicals, generated spontaneously in oxidizing Fe2+-containing groundwaters, caused the inactivation of A. hydrophila AWWX1. Evidence that free radicals are generated under the given conditions was provided by the observation that propylphosphonic acid, a compound which is very susceptible to radicals, was degraded upon addition to these waters. A. hydrophila PWBS, Pseudomonas fluorescens P17, Spirillum strain NOX, and heterotrophs showed decreases in culturability in filter-sterilized Snellegem-raw water similar to that shown by A. hydrophila AWWX1. These findings indicate that free radicals generated in Fe2+-containing groundwaters upon aeration are capable of inactivating various bacterial species.

Abstract:Objectives to develop a safe and effective way of qualifying children for a second device cochlear implantation. Material and methods. There were 50 children from two to twelve years old after unilateral cochlear implantation under our observation. During qualification, the following criteria were taken into account: audiometry results, use of the hearing aid in the non-implanted ear and benefit of the device, speech and hearing development after the first cochlear implantation. Results. According to our findings the second cochlear implantation was recommended for 12 (24%) patients; in 16 (32%) cases the second cochlear implantation was refused; in 22 (44%) cases it was recommended to postpone implantation of the second device. Conclusion. Our results demonstrate that the safe and effective way of qualifying for a second cochlear implant can restore binaural hearing that is crucial for the child's speech and hearing development and enhance integration into a world of peers.

Abstract:Objective: Although cochlear implantation (CI) is a relatively safe operation, postoperative complications sometimes occur. We reviewed the frequency and severity of complications of CI at our hospital. We compared our results with previously reported complications and considered measures to improve patient outcomes. Methods: This retrospective study examined the medical records of 70 patients who received CI between March 2005 and December 2018. We collected the following data: age at the time of the first surgery, etiology of hearing impairment, date of implantation, type of implanted devices, and complications. Surgical complications were divided by time into perioperative, early, and late, and by severity into major or minor. Results: Records of 38 adults and 32 children were analyzed. Bilateral CI was performed in 16 patients, 8 of whom were sequential, and unilateral CI was performed in 54 patients. The total number of operations was 78 for 86 CI. Complications were observed in 15 of 78 operations (19%), and the rates of minor and major complications were 15% and 4%, respectively. Complication rates were 21% (8/39) for children and 10% (4/39) for adults. All of the perioperative and early complications were minor. There were three major complications, all of which were infections presenting with mastoiditis and subcutaneous or subperiosteal abscesses. One case required reimplantation twice because of recurrent mastoiditis and temporal abscess. Conclusions: There was no significant difference in the incidence of complications between children and adults, but all major complications were infection in pediatric cases. Careful attention is needed to prevent postoperative infection.
Simultaneous Bilateral Cochlear Implantation in Very Young Children Improves Adaptability and Social Skills: A Prospective Cohort Study.

Abstract:OBJECTIVES:To investigate the value of using the Gesell Development Diagnosis Scale (GDDS) to predict developmental outcomes in very young children who undergo simultaneous bilateral cochlear implantation.DESIGN:In this prospective cohort study, a repeated-measures investigation was conducted in a tertiary referral hospital. A total of 62 children receiving simultaneous bilateral cochlear implantations were enrolled from April 2017 to August 2018. They were divided into 2 groups depending on the operative age: "Infants" group (6 to 12 months, N = 38) or "Children" group (12 to 36 months, N = 24). Data on the surgical outcomes, auditory development, speech production, and developmental indicators were collected until 2 years after the initial fitting. The primary outcome measure was the GDDS, a neuropsychological development examination. Secondary outcomes included the following: complication rate, aided pure-tone average, Infant-Toddler Meaningful Auditory Integration Scale, Categories of Auditory Performance-II, Meaningful Use of Speech Scale, Speech Intelligibility Rating, and the LittlEARS Auditory Questionnaire.RESULTS:The mean ages at implantation in infants and children groups were 9.2 ± 1.17 and 16.6 ± 3.60 months, respectively. Significant differences were found in the social skills ( p = 0.001) and adaptability ( p = 0.031) domains of GDDS. The younger the age of bilateral cochlear implants surgery, the higher developmental quotient of language, social skills, and adaptability the child could achieve after 2 years. The complication rates in the infants and children groups were 0% versus 2.1% ( p = 0.57). There was no surgical complication in the infants group. In the children group, 1 case with enlarged vestibular aqueduct and Mondini malformation had a receiver-implant misplacement on the right side (2%, 1/48). In the two groups, auditory performance and speech production had improved similarly. In the infants group, social skills developmental quotient at baseline had a significant positive relationship with Meaningful Use of Speech Scale after 2 years.CONCLUSIONS:Simultaneous bilateral cochlear implantation in younger children improves adaptability and social skills. GDDS is a sensitive tool of evaluating short-term effect of bilateral cochlear implants in neuropsychological development and constitutes a reliable predictor of speech production for the very younger pediatric cochlear implant users.

Abstract:BackgroundFunding for paediatric bilateral cochlear implantation became available in Ireland in 2014. Prior to this, children eligible for cochlear implantation received a unilateral implant.ObjectiveTo examine the cohort of children who received a unilateral cochlear implant in the 4 year period following bilateral cochlear implantation funding becoming available.MethodsA clinical audit of all children implanted for the first time between July 2014 and July 2018. The unilaterally implanted children (n = 105) were divided into 3 groups according to whether they met the audiometric thresholds for implantation in neither ear (Group 1), one ear (Group 2) or both ears (Group 3). One year post operative functional outcomes were examined for all 3 groups.ResultsAll 3 groups showed significant improvements in functional outcomes at 1 year post op. To date, 20% of the unilaterally implanted children have proceeded to get a sequential CI, often where there was no change in audiological status.ConclusionsThe number of children in Groups 1 and 2 highlighted how our decision making around cochlear implantation has changed in recent years. Unilateral cochlear implantation in certain circumstances is good practice, independent of the audiological profile when an experienced multi-disciplinary team (MDT) is involved in the decision making process. Decision making using a holistic model approach is key, including involving the parent/carer and, where appropriate, the child/teenager themselves. A staged bilateral cochlear implant is also a good option, where careful monitoring and support for the first implant has resulted in positive outcomes.

Abstract:With an aging population and increasing utilization of both cochlear implants (CI) to treat hearing loss and deep brain stimulation (DBS) to treat neurodegenerative movement disorders, more patients will be sequentially implanted with the two devices. As such, understanding both presurgical evaluations and surgical procedures will be of great importance to the treating cochlear implant surgeon and neurosurgeon in optimizing outcomes for both conditions. Two illustrative cases of sequential CI followed by DBS are reviewed. Relevant pre-, intra-, and postoperative issues are discussed for both DBS following CI surgery and CI following DBS surgery. Despite the potential for challenges, sequential CI and DBS (and vice versa) can provide substantial benefit to those individuals with severe hearing loss and movement disorders. A thorough understanding of both devices is critical to ensuring optimal outcomes.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Purpose: Cochlear implant (CI) surgery is conventionally done under general anesthesia (GA). However, many patients are unable to undergo GA due to various reasons, raising the need for an alternative safe option. CI under local anesthesia is feasible and safely done in patients who can't tolerate GA. This approach allows for device fitting immediately after surgery. This study aims to evaluate simultaneous bilateral cochlear implant (BiCI) surgery performed under local anesthesia with sedation in adults. To our knowledge, this is the largest cohort of patients who underwent this approach. Methods: This is a retrospective chart review in a tertiary center. We included all adult patients who underwent simultaneous BiCI under local anesthesia with sedation from 2018 to 2024. The feasibility of BiCI under local anesthesia with sedation was assessed through clinical, surgical, audiological, and patient questionnaire data. Results: Six patients underwent simultaneous BiCI with local anesthetic and sedation. Mean age was 41.7 ± 16.0, comprising 66.7% male and 33.3% female. No intraoperative problems were encountered. All woke up from sedation without any agitation or difficulties. Their recovery was uneventful. No dizziness, nausea, or vomiting were reported. The device was activated immediately postoperatively. Pure tone audiometry, speech reception threshold, and word recognition score were significantly improved. Positive experiences were reported in all patients. Conclusion: Bilateral simultaneous cochlear implantation under local anesthesia in adults is achievable, through a multidisciplinary approach. This approach is a potential alternative option for some patients and could optimize their hearing rehabilitation.

Sequential Bilateral Cochlear Implantation in Children: Outcome of the Second Implant and Long-Term Use

Abstract:Objectives: The aim of this retrospective cohort study was to assess speech perception outcomes of second-side cochlear implants (CI2) relative to first-side implants (CI1) in 160 participants who received their CI1 as a child. The predictive factors of CI2 speech perception outcomes were investigated. In addition, CI2 device use predictive models were assessed using the categorical variable of participant's decision to use CI2 for a minimum of 5 years after surgery. Findings from a prospective study that evaluated the bilateral benefit for speech recognition in noise in a participant subgroup (n = 29) are also presented. Design: Participants received CI2 between 2003 and 2009 (and CI1 between 1988 and 2008), and were observed from surgery to a minimum of 5 years after sequential surgery. Group A (n = 110) comprised prelingually deaf children (severe to profound) with no or little acquired oral language before implantation, while group B (n = 50) comprised prelingually deaf children with acquired language before implantation, in addition to perilingually and postlingually deaf children. Speech perception outcomes included the monosyllable test score or the closed-set Early Speech Perception test score if the monosyllable test was too difficult. To evaluate bilateral benefit for speech recognition in noise, participants were tested with the Hearing in Noise test in bilateral and "best CI" test conditions with noise from the front and noise from either side. Bilateral advantage was calculated by subtracting the Hearing in Noise test speech reception thresholds in noise obtained in the bilateral listening mode from those obtained in the unilateral "best CI" mode. Results: On average, CI1 speech perception was 28% better than CI2 performance in group A, the same difference was 20% in group B. A small bilateral speech perception benefit of using CI2 was measured, 3% in group A and 7% in group B. Longer interimplant interval predicted poorer CI2 speech perception in group A, but only for those who did not use a hearing aid in the interimplant interval in group B. At least 5 years after surgery, 25% of group A and 10% of group B did not use CI2. In group A, prediction factors for nonuse of CI2 were longer interimplant intervals or CI2 age. Large difference in speech perception between the two sides was a predictor for CI2 nonuse in both groups. Bilateral advantage for speech recognition in noise was mainly obtained for the condition with noise near the "best CI"; the addition of a second CI offered a new head shadow benefit. A small mean disadvantage was measured when the noise was located opposite to the "best CI." However, the latter was not significant. Conclusions: Generally, in both groups, if CI2 did not become comparable with CI1, participants were more likely to choose not to use CI2 after some time. In group A, increased interimplant intervals predicted poorer CI2 speech perception results and increased the risk of not using CI2 at a later date. Bilateral benefit was mainly obtained when noise was opposite to CI2, introducing a new head shadow benefit.
An Analysis of 368 Cases of Cochlear Implant Surgery

Abstract:Objective To investigate the surgical procedures for cochlear implantation in order to improve efficiency and safety of operation. Methods 368 cases of cochlear implantation through posterior tympanotomy approach without intraoperative facial nerve monitoring were studied. The electrode impedance and auditory nerve responses were tested during the operation. X-ray images were taken for all cases to locate the electrodes. The cochlear implants were activated 1 month after the surgery and the follow-up study was conducted afterwards. Results All of the electrodes were inserted successfully and all of the implants worked well. Three days after surgery, one patient had delayed facial paralysis. Forty-five days after surgery, one patient had external otitis in the implanted ear. Two months after surgery, 1 case suffered from mastoid subperiosteal abscesses. Seven months after implantation, the implant was damaged in one case because of a head injury. Their conditions were improved after active treatment. No permanent facial paralysis, meningitis, implant displacement or other serious surgical complications occured during a follow-up period of 24 months, but 3 cases of implant exposure and infection were reported 4 years after implantation. Conclusion Mastery of surgical techniques and individualized surgery can improve the success rate of cochlear implantation, enhance the auditory rehabilitation outcomes and reduce the postoperative complications. Long-term or even lifetime follow-up after surgery is necessary.
