Abstract:In women, prolactinomas (mainly microprolactinomas) are commonly diagnosed between 20-40-year old. In postmenopausal women, prolactinomas are rarely encountered and usually do not present with hyperprolactinemia-related symptoms as these are dependent on intact ovarian function. Therefore, the true incidence of prolactin (PRL)-secreting adenomas in postmenopausal woman is unknown. Our study objective was to characterize these rare and unique pituitary tumors. A retrospective study including a consecutive group of postmenopausal women followed and treated at 3 Endocrine academic clinics. Baseline clinical characteristics (PRL and gonadotropins levels, other pituitary hormones, adenoma size and invasiveness, visual fields) and response to treatment are reported. The cohort included 14 postmenopausal women with prolactinomas (mean age at diagnosis, 63.6 ± 7.1 years; range, 54-75 years). Mean adenoma size at presentation was 25.6 ± 12.4 mm (range, 8-50 mm). Six out of the 14 women had significant visual fields damage. Mean baseline PRL level was 1,783 ng/ml, and median PRL was 827 ng/ml (range, 85-6,732 ng/ml). Medical treatment with cabergoline was given to twelve of the patients. Cabergoline normalized/near-normalized PRL in eleven women; one woman was dopamine agonist-resistant. Five of the six subjects with visual disturbances normalized or improved their vision, and a pre-treatment diplopia in another patient disappeared. Two large pituitary tumors disappeared on MRI following long-term dopamine agonist therapy. All other treated prolactinomas, except the resistant adenoma, shrank following medical treatment. Prolactinomas are rarely diagnosed in postmenopausal women. These women usually harbor large and invasive macroadenomas, secreting high PRL levels, and usually respond to dopamine agonist treatment.
Prolactin-secreting tumors: what's new?

Abstract:Prolactinomas are a common cause of reproductive/sexual dysfunction. Once other causes of hyperprolactinemia have been excluded with a careful history and physical examination, routine chemistries, a pregnancy test and a thyroid-stimulating hormone, imaging with magnetic resonance imaging or computed tomography will delineate the size and extent of the tumor. Medical therapy with cabergoline or bromocriptine is the initial treatment of choice. When infertility is the primary indication for treatment, bromocriptine use has an extensive safety experience and is preferred by some clinicians. However, for other indications, cabergoline appears to be more efficacious and better tolerated. Transsphenoidal surgery remains an option, especially for patients with microadenomas, when medical therapy is ineffective or not tolerated.
Association of High Prolactin Level on Postoperative Day 1 and Tumor Invasion with Female Gonadal Dysfunction after Trans-Sphenoidal Surgery of Pituitary Adenomas.

Abstract:Background: The aim of this study was to evaluate the risk factors of gonadal dysfunction among Chinese women of reproductive age with pituitary adenomas (PAs) after trans-sphenoidal surgery. Material/Methods: We retrospectively evaluated 317 women (16-44 years old) who underwent gonadal function and hormone testing before and after trans-sphenoidal surgery for PAs during 2003-2012. Gonadal function was assessed on the basis of menstrual status. Results: Three women were excluded because of pre-existing gynecological diseases. Before trans-sphenoidal surgery, 34 (10.7%) women were eugonadal and 283 (89.3%) women had gonadal dysfunction. After trans-sphenoidal surgery, 130/189 (68.7%) women with follow-up menstruation data were eugonadal, and 59/189 (31.2%) women exhibited gonadal dysfunction. In addition, 67.4% women of reproductive age with PAs and gonadal dysfunction were restored by trans-sphenoidal surgery (P<0.01). Postoperative gonadal dysfunction was independently associated with high prolactin level at day 1 after trans-sphenoidal surgery (odds ratio (OR)=1.024; 95% confidence interval (CI)=1.005-1.043; P=0.012) and tumor invasion (OR=5.752; 95%CI=1.618-20.447; P<0.01). Based on the receiver operating characteristic (ROC) curve, prediction of gonadal dysfunction in women of reproductive age after trans-sphenoidal surgery for PAs using prolactin >46.82 mu g/L on postoperative day 1 had sensitivity of 88%, specificity of 95%, positive predictive value of 98%, and negative predictive value of 76%, and an area under the ROC curve of 0.701. Conclusions: Gonadal dysfunction is very common in Chinese women of reproductive age with PAs and can be effectively restored by trans-sphenoidal surgery. Prolactin >46.82 mu g/L at 1 day after trans-sphenoidal surgery and tumor invasion can predict postoperative gonadal dysfunction in these patients.
Predictive value of the official cancer alarm symptoms in general practice--a systematic review.

Abstract:INTRODUCTION The objective of this study was to investigate the evidence for positive predictive value (PPV) of alarm symptoms and combinations of symptoms for colorectal cancer, breast cancer, prostate cancer and lung cancer in general practice. METHODS This study is based on a literature search performed in PubMed, Embase, the Cochrane database and at ClinicalTrials.gov in accordance with the PRISMA guidelines. The main outcome measure used was PPV. RESULTS A total of 16 eligible studies were identified. The intervals in the brackets refer to the variation of the results in the studies. Colorectal cancer: The PPV of "rectal bleeding" was high for patients > 60 years (6.6-21.2%), but much lower in younger age groups. For "change in bowel habits" and "significant general symptoms", the PPV was 3.5-8.5%. Breast cancer: "Palpable suspected tumour" was well supported (8.1-24%). No studies on the predictive value of "pitting of the skin", "papil-areola eczema/ulceration" and "suspect axillary lymph nodes" were found. Prostate cancer: One study showed a high PPV for positive rectal examination (12%). The value for "lower urinary tract symptoms" was low (1.0-3.0%). PPV for "perianal pain" and "haemospermia" were not found. Lung cancer: For "haemoptysis" the PPV increased from 8.4 in patients aged 55 years to 20.4 at the age of > 85 years. PPV for "cough", "pain in the thorax", "dyspnoea" and "general symptoms" were low (0.4-1.1%). Using a new algorithm that estimates the PPV of combinations of symptoms and risk factors, a higher PPV may be achieved. CONCLUSION A few of the alarm symptoms show a high PPV, whereas the PPV for some symptoms currently remains unknown. To improve the GPs' diagnostic judgment, a new algorithm for calculating the PPV for combinations of symptoms and risk factors seems promising.
Hyperprolactinemia and hypogonadism in the human female

Abstract:Prolactin is a mammotropic hormone essential for the initiation of lactation. It also influences ovarian function; during hyperprolactinemia hypogonadism occurs. This is true for pathological forms of hyperprolactinemia but also for the early puerperium when there is physiological hyperprolactinemia. Amenorrhea is a better parameter of hyperprolactinemia than galactorrhea. The mechanisms by which prolactin disrupts ovarian function are not as yet fully understood; it probably alters hypothalamic neurotransmitter content through a direct feedback mechanism resulting in a decrease of Gn-RH. However, the direct effect of prolactin-producing pituitary tumors on the capacity of the gonadotrophs or a direct interference of prolactin at the gonadal level are also possibilities. Hyperprolactinemia can be treated very effectively with bromocriptine and this drug appears to have become the favorite form of treatment. In the case of obvious tumors hypophysectomy is indicated. When there are smaller tumors irradiation of the pituitary gland previous to bromocriptine treatment may prevent expansion of the gland during subsequent pregnancy.

Introduction: Silent pituitary adenomas secrete one or more anterior pituitary hormones or express their transcriptions factors, but do not secrete hormones at a clinically relevant level. Silent lactotroph adenomas are rare and often encountered as mixed somatotroph-lactotroph adenomas. Mono-hormonal silent lactotroph adenomas are exceedingly rare with the prevalence of 0.6-1.65% with most belonging to the sparsely granulated subtype. Clinical Case: A 32-year-old male presented with right visual problem over the last 7 months. Evaluation by Ophthalmology revealed trace afferent pupil defect of the right eye and bitemporal hemianopsia. MRI head showed an enhancing mass in the sellar and suprasellar regions collectively measured 2.5 x 2.0 x 1.8 cm, which upwardly displaced and distorted the optic chiasm. Biochemical workup showed normal prolactin 8.1 ng/ml (2.6-13.1) and unremarkable other pituitary hormone levels including ACTH 26 pg/ml (7-63), cortisol 16.6 ug/dl (6.7-22.6), growth hormone 0.08 ng/ml (0.00-2.47), LH 4.1 mIU/ml (1.2-8.6), FSH 5.8 mIU/ml (1.3-19.3), estradiol 27.9 pg/ml (15.0 - 31.5), total testosterone 527 ng/dl (240-950), free testosterone 16.1 ng/dl (4.85-19.0), SHBG 23 nmol/L (10-80), TSH 3.34 uIU/ml (0.34-5.60), free T4 0.84 ng/dl (0.58-1.64). He underwent transsphenoidal hypophysectomy. Final pathology showed densely granulated lactotroph, with extensively positive PRL in tumor cells, positive Pit-1 and negative ERa. Patient did well post-operatively. Conclusion: This case illustrates a rare case of a mono-hormonal silent pituitary lactotroph adenoma. The majority of lactotroph adenomas are sparely granulated, so the densely granulated histology shown here makes this case even more rare. Unfortunately, there is no data reported in relation to the association between this histological subtype and the invasiveness of tumor. Abstract:Objectives To investigate the clinical features of primary amenorrhea for pituitary prolactinoma and to evaluate the effectiveness of surgical treatment. Methods Clinical data of five patients with primary amenorrhea caused by pituitary prolactinoma were analyzed retrospectively. The tumors were classified into two types (E: enclosed in sella turcica; I: invasive) according to their invasiveness determined by the CT or MRI examinations. Transcranial and transsphenoidal approaches were employed to resect the tumors. Results Total removal were achieved in 2 intrasellar tumors and subtotal resection in the other 3 invasive tumors. Postoperatively, visual disturbances were improved markedly in all patients and serum PRL level reduced to some different extent, while in only one patient appeared normal menstrual cycle with dependence on bromocriptine. One patient (20 %) developed cerebrospinal fluid rhinorrhea after surgery and no other complications were observed. Conclusion Careful evaluation should be performed before surgically resecting the tumors because of its relatively low cure rate.
Clinical characteristics and prognostic factors of prolactinomas and growth hormone-secreting adenomas in premenopausal women in China

Abstract:Pituitary adenomas, including prolactinomas and growth hormone (GH)-secreting adenomas, are common intracranial tumors in premenopausal women. Few studies have systematically documented the profile of these tumors in premenopausal women in China. Herein, we analyzed the clinical features and prognostic factors of 132 prolactinomas and 39 growth hormone-secreting adenomas treated by transsphenoidal surgery in Chinese premenopausal women. Patients with prolactinomas had smaller tumors (12 mm), higher preoperative prolactin levels (181.9 μg/L), and were younger (28 years) than those with GH-secreting adenomas or with other types of pituitary adenomas. The preoperative level of prolactin positively correlated with tumor size (r = 0.247, P = 0.005) and negatively correlated with age (r = -0.257, P = 0.003) in patients with prolactinomas. The diagnostic cut-off value for the preoperative level of prolactin was 112.2 μg/L with an area under the curve (AUC) of 0.854, a sensitivity of 78%, a specificity of 83%, a positive predictive value (PPV) of 73%, and a negative predictive value (NPV) of 86%. The postoperative remission rate of patients with prolactinomas was 60.7%. Tumor size, preoperative prolactin level, prolactin level on day 1 postsurgery, the postoperative prolactin reduction rate, and the tumor invasiveness were correlated with postoperative remission. However, the predictive factor of positive outcome was only low prolactin level on day 1 postsurgery [odds ratio (OR) = 0.931; 95% confidence interval (CI) = 0.883-0.981] in patients with prolactinomas, with a cut-off value of 20.49 μg/L (AUC: 0.950; sensitivity: 88%; specificity: 95%; PPV: 98%; NPV: 76%) based on logistic regression and receiver-operator characteristic (ROC) analysis. Patients with GH-secreting adenoma had a higher preoperative level of GH (14.3 μg/L), larger tumors (22.5 mm), and were older (41 years) than patients with prolactinoma or with other types of pituitary adenomas. The diagnostic cut-off value for the preoperative level of GH was 2.6 μg/L (AUC: 0.809; sensitivity: 77%; specificity: 83%; PPV: 36%; NPV: 97%), and the postoperative remission rate was 60.7%. Thus, we can conclude that the clinical characteristics and outcomes of prolactinomas and GH-secreting adenomas in premenopausal women were not precisely the same as those in patients with general pituitary adenoma. Prolac tinoma should be suspected in premenopausal women with a prolactin level greater than 112.2 μg/L. A GH level greater than 2.6 μg/L is highly suggestive of GH-secreting adenomas. A prolactin level less than 20.49 μg/L on day 1 postsurgery is a strong independent predictor of remission in Chinese premenopausal women with prolactinoma.

Abstract:In psychological conditions of mourning and guilt, women who have lost children often attempt to quickly conceive a "replacement" child. This study examines the subsequent pregnancies of 32 women whose children died of sudden infant death syndrom (SIDS). The expected rate of infertility in a normal population is 10%. Spontaneous abortion has an incidence of 12% to 15%. Among the 32 women attempting to conceive after the loss of their child, 10 (31%) had spontaneous abortions and 11 (34%) could not conceive after attempts for at least one year. This association between psychological and biological phenomena require special recognition by physicians who are advising parents who have lost children to SIDS. The management of these families includes compassion, understanding, and regard for the psychological environment of the subsequent pregnancy.

Abstract:This Consensus Statement from an international, multidisciplinary workshop sponsored by the Pituitary Society offers evidence-based graded consensus recommendations and key summary points for clinical practice on the diagnosis and management of prolactinomas. Epidemiology and pathogenesis, clinical presentation of disordered pituitary hormone secretion, assessment of hyperprolactinaemia and biochemical evaluation, optimal use of imaging strategies and disease-related complications are addressed. In-depth discussions present the latest evidence on treatment of prolactinoma, including efficacy, adverse effects and options for withdrawal of dopamine agonist therapy, as well as indications for surgery, preoperative medical therapy and radiation therapy. Management of prolactinoma in special situations is discussed, including cystic lesions, mixed growth hormone-secreting and prolactin-secreting adenomas and giant and aggressive prolactinomas. Furthermore, considerations for pregnancy and fertility are outlined, as well as management of prolactinomas in children and adolescents, patients with an underlying psychiatric disorder, postmenopausal women, transgender individuals and patients with chronic kidney disease. The workshop concluded that, although treatment resistance is rare, there is a need for additional therapeutic options to address clinical challenges in treating these patients and a need to facilitate international registries to enable risk stratification and optimization of therapeutic strategies.

Granulomatous Mastitis: An Initial Presentation of Undiagnosed Prolactinoma

Abstract:Granulomatous lobular mastitis (GLM) is a rare, benign inflammatory disease of the breast that shares some physical diagnostic features with breast cancer. GLM has been rarely reported to be associated with prolactinoma. In this report, we present a case of undiagnosed prolactinoma in a 37-year-old woman with its initial presentation as GLM. We discuss the underlying pathophysiologic mechanisms for the development of GLM and the potential immunomodulatory role of prolactin in the development of GLM. We also highlight the need to assess for possible prolactinoma in GLM, which might go undiagnosed as in the case of our patient who did not seek medical attention for her amenorrhea, which is likely due to hyperprolactinemia that might also have other clinical implications on cardiovascular and bone health due to consequent estrogen deficiency.
Tumor expression of human growth hormone and human prolactin predict a worse survival outcome in patients with mammary or endometrial carcinoma.

Abstract:Context: Evidence suggests that human GH(hGH) and human prolactin (hPRL) possess an autocrine or paracrine oncogenic role in mammary and endometrial carcinoma. However, especially for hGH, the prognostic relevance of tumor expression of these hormones is not well defined. Objective: We investigated the potential association of tumor mRNA and protein expression of hGH and hPRL with the clinicopathological features of mammary and endometrial carcinoma. The prognostic relevance of the individual or combined expression of hGH and hPRL in mammary and endometrial carcinoma was also determined. Design: The expression of hGH and hPRL was analyzed in histopathological samples of mammary and endometrial carcinoma, and the respective normal tissues, by in situ hybridization and immunohistochemistry. Kaplan-Meier and Cox regression analysis was performed to examine the association of tumor hGH and hPRL expression with relapse-free survival and overall survival of patients. Results: hGH expression was significantly associated with lymph node metastasis, tumor stage, human epidermal growth factor receptor-2 status, and proliferative index in mammary carcinoma and with International Federation of Gynecology and Obstetrics grade, myometrial invasion, and ovarian metastases in endometrial carcinoma. hPRL expression was associated with lymph node metastasis, tumor grade, and tumor stage in mammary carcinoma and with International Federation of Gynecology and Obstetrics stage and myometrial invasion in endometrial carcinoma. Both hGH and hPRL expression, individually and combined, are associated with worse relapse-free survival and overall survival in patients with mammary or endometrial carcinoma. Conclusion: Tumor expression of both hGH or hPRL in mammary or endometrial carcinoma is associated with a large and significant difference in survival outcome for patients with these tumors. Abstract:Background and importance: Metastasis to the pituitary gland is uncommon in patients with systemic disseminated cancer. Individual articles have reported cases of pituitary metastasis mimicking a prolactinoma, but no case of a prolactin-secreting tumor metastasizing to the pituitary mimicking a prolactinoma has been reported so far. Clinical presentation: This article reports a 67-yr-old man with a recent onset of headaches, ophthalmoplegia, hypopituitarism, and hyperprolactinemia who was initially diagnosed with prolactinoma and given bromocriptine in the local hospital. Because of vomiting after taking drugs, he was transferred to our hospital for further diagnosis and treatment. Serum prolactin was elevated up to 1022 ng/mL, and pituitary magnetic resonance imaging revealed a 2.9 × 2.8 × 2.3 cm sellar mass with pituitary apoplexy, for which endoscopic transsphenoidal surgery was performed. Postoperative pathology and western blotting disclosed a prolactin-positive metastatic lung adenocarcinoma. Whole exome sequencing revealed a number of gene mutations including KRAS, PIK3CA, ALK, and CTNNB1. The patient died of deterioration of the lung disease 3 mo after the initial diagnosis. Conclusion: To the best of our knowledge, this is the first report of a prolactin-secreting tumor metastasizing to the pituitary mimicking a prolactinoma as confirmed by both immunohistochemistry and western blot. Prolactin secretion is rare and elusive, and may associate with specified gene mutations.
Risk of thromboembolic events in patients with prolactinomas compared with patients with nonfunctional pituitary adenomas

Abstract:Prolactin has been proposed as a potent coactivator of platelet aggregation, possibly contributing to thromboembolic events. The objective of the study was to evaluate the relationship between prolactinoma and deep vein thrombosis (DVT), pulmonary embolism (PE), and cerebrovascular accident (CVA). Subjects were identified from a prospectively maintained pituitary database at the Cleveland Clinic. We retrospectively reviewed the charts of 544 subjects: 347 patients with prolactinomas (prolactinoma group) and 197 patients with nonfunctional pituitary adenomas (control group). Main outcome measures were DVT, PE and CVA. We found that 19 (5.5%) patients in the prolactinoma group and five (2.5%) patients in the control group had documented DVT, PE, or CVA, but this difference was not significant (p = 0.109). However, the mean initial prolactin level was higher at the time of diagnosis among prolactinoma patients than among controls (815.23 ng/ml vs. 15.90 ng/ml; p < 0.001). Among prolactinoma patients, 15 (5.5%) of 275 patients who underwent medical treatment (with cabergoline, bromocriptine, pergolide and/or other drug) and 4 (5.6%) of 72 patients who underwent transsphenoidal surgery had documented DVT, PE, or CVA, which suggests that dopaminergic therapy did not influence the risk of thromboembolic events. Hyperprolactinemia per se does not appear to predispose to a hypercoagulable state.

However, for other indications, cabergoline appears to be more efficacious and better tolerated. Transsphenoidal surgery remains an option, especially for patients with microadenomas, when medical therapy is ineffective.

Abstract:An effective method for achieving a room-temperature multiwavelength erbium-doped fiber ring laser is presented. Simultaneous multiwavelength lasing with 0.5-nm intervals is achieved both experimentally and theoretically by addition of sinusoidal phase modulation in the ring cavity to prevent single-wavelength oscillation.

Abstract:Several subtypes of pituitary neuroendocrine tumors (PitNETs), such as acromegaly and Cushing's disease, can result in hypertension. However, whether prolactinoma is associated with this complication remains unknown. Moreover, the effect of treatment with surgery or drugs on blood pressure (BP) is unknown. Herein, a retrospective study reviewed 162 patients with prolactinoma who underwent transsphenoidal surgery between January 2005 and December 2022. BP measurements were performed 1 day before and 5 days after surgery. Accordingly, patients' medical characteristics were recorded. In addition, in situ rat and xenograft nude-mice prolactinoma models have been used to mimic prolactinoma. In vivo BP and serum prolactin (PRL) levels were measured after cabergoline (CAB) administration in both rats and mice. Our data suggest that surgery can effectively decrease BP in prolactinoma patients with or without hypertension. The BP-lowering effect was significantly associated with several variables, including age, sex, disease duration, tumor size, invasion, dopamine agonists (DAs)-resistance, recurrence, and preoperative PRL levels. Moreover, in situ and xenograft prolactinomas induced BP elevation, which was alleviated by CAB treatment without and with a statistical difference in rats and mice, respectively. Thus, surgery or CAB can decrease BP in prolactinoma, indicating that pre- and postoperative BP management becomes essential.
