Abstract:Purpose Even though phenotypes of disc degeneration vary on different sagittal magnetic resonance images (MRI), measurements typically are acquired on the mid-sagittal MRI. This study investigated the appropriateness of using the mid-sagittal MRI to measure various phenotypes of age-related disc degeneration. Methods Lumbar spine MRIs of 66 subjects (mean age 50.3 years, standard deviation 16.5 years, range 22–84 years) were studied. An image analysis program Spine Explorer was used to obtain quantitative measurements for disc height, bulging, and signal on para- and mid-sagittal T2-weighted MRIs. Measurements on para- and mid-sagittal MRIs and their associations with age were compared. Results Measurements of disc height, signal, and posterior disc bulging acquired on the mid-sagittal MRI were greater than those on the para-sagittal MRIs. Disc height measurements were not linearly associated with age. Greater age was correlated with greater anterior ( r = 0.45, P < 0.001) and posterior ( r = 0.33, P < 0.01) bulging on para-sagittal MRIs, but not posterior disc bulging on the mid-sagittal MRI ( r = − 0.10, P > 0.05). Disc signal intensity measurements on the mid-sagittal MRI had stronger correlations with age than those on para-sagittal MRIs. Mean and standard deviation of disc signal intensity acquired on the mid-sagittal MRI had the strongest correlations with age among all measures of disc degeneration studied ( r = − 0.50, − 0.67, respectively, P < 0.001 for both). Conclusions Disc signal measurements acquired on the mid-sagittal MRI were reliable and had strong correlations with age and thus can be used as an appropriate measure of disc degeneration. Disc bulging had better be measured on para-sagittal MRIs. Although severe disc narrowing clearly is a sign of severe disc degeneration, disc height was not linearly associated with age.
Identification of Cervical Disc Degeneration and Genes Associated with Immune Infiltration

Abstract:Abstract Objective: This study aims to identify new diagnostic genetic-biomarkers for early-stage cervical intervertebral disc degeneration (IDD) and analyze the potential association between key genes and infiltrating immune cells. Methods: Cervical disc specimens were collected from anterior cervical discectomy and fusion surgery, which were classified into IDD and controlled group depending on MRI disc degeneration grade. mRNA expression profile data was downloaded from the GEO database. Differentially expressed genes (DEGs) were determined from GEO database profile. Gene ontology (GO) functional annotation and Kyoto Encyclopedia of Genes and Genomes (KEGG) signaling pathway enrichment analysis were performed on DEGs. LASSO regression model and Support Vector Machine Recursive Feature Elimination (SVM-RFE) algorithm were employed to screen for potential biomarkers. The composition of 22 immune cell fractions in IDD patients was analyzed using CIBERSORT. Biomarker expression levels in cervical disc specimens were detected through quantitative PCR. Result: 71 DEGs were found, including 50 genes up-regulated and 21 genes down-regulated. KEGG pathway analysis revealed significant enrichment of inflammation-related pathways. Based on the results of two machine learning screening methods and validation in cervical disc specimens, CDKN3, SLC22A4, and SYDE1 were identified as key biomarkers for cervical IDD. Immune cell infiltration analysis showed significant differences, with higher rate of M0 macrophage and mast cell activation (P=0.004, P=0.004), and lower rate of T-cell CD8 and NK cell activation (P=0.045, P=0.024). Conclusion: CDKN3, SLC22A4 and SYD1 are associated with the occurrence and progression of cervical IDD through the regulation of certain immune cells. Therefore, they have the potential to be the biomarkers for the diagnosis of cervical IDD and play a role in monitoring the progression of cervical IDD.
Quantitative T2 Magnetic Resonance Imaging Compared to Morphological Grading of the Early Cervical Intervertebral Disc Degeneration: An Evaluation Approach in Asymptomatic Young Adults

Abstract:OBJECTIVE: The objective of this study was to evaluate the efficacy of quantitative T2 magnetic resonance imaging (MRI) for quantifying early cervical intervertebral disc (IVD) degeneration in asymptomatic young adults by correlating the T2 value with Pfirrmann grade, sex, and anatomic level.METHODS: Seventy asymptomatic young subjects (34 men and 36 women; mean age, 22.80±2.11 yr; range, 18-25 years) underwent 3.0-T MRI to obtain morphological data (one T1-fast spin echo (FSE) and three-plane T2-FSE, used to assign a Pfirrmann grade (I-V)) and for T2 mapping (multi-echo spin echo). T2 values in the nucleus pulposus (NP, n = 350) and anulus fibrosus (AF, n = 700) were obtained. Differences in T2 values between sexes and anatomic level were evaluated, and linear correlation analysis of T2 values versus degenerative grade was conducted.FINDINGS: Cervical IVDs of healthy young adults were commonly determined to be at Pfirrmann grades I and II. T2 values of NPs were significantly higher than those of AF at all anatomic levels (P<0.000). The NP, anterior AF and posterior AF values did not differ significantly between genders at the same anatomic level (P>0.05). T2 values decreased linearly with degenerative grade. Linear correlation analysis revealed a strong negative association between the Pfirrmann grade and the T2 values of the NP (P = 0.000) but not the T2 values of the AF (P = 0.854). However, non-degenerated discs (Pfirrmann grades I and II) showed a wide range of T2 relaxation time. T2 values according to disc degeneration level classification were as follows: grade I (>62.03 ms), grade II (54.60-62.03 ms), grade III (<54.60 ms).CONCLUSIONS: T2 quantitation provides a more sensitive and robust approach for detecting and characterizing the early stage of cervical IVD degeneration and to create a reliable quantitative in healthy young adults.

Abstract:Background: Magnetic resonance imaging (MRI) is a useful non-invasive tool for evaluating abnormalities of intervertebral discs. However, there are few studies which applied functional MRI techniques to investigate degenerative changes in cervical and cervicothoracic junction (CTJ) spine among adults. The aim of this study was to compare T2 relaxation time measurement evaluation with morphological grading for assessing cervical and CTJ intervertebral discs (IVD) in the patients suffering neck, shoulder, and upper back pain. Methods: Sixty-three patients (378 IVDs) and 60 asymptomatic volunteers (360 IVDs) of the cervical and CTJ discs were assessed using a 3.0 T magnetic resonance imaging (MRI) protocol, including an sagittal T2 relaxation time protocol. The relaxation time values of the nucleus pulposus (NP) were recorded and all discs were visually graded according to Pfirrman's grading system. The correlation between T2 relaxation time values and qualitative clinical grading of degeneration, patient age, sex and anatomic level were analyzed RESULTS: There is a clear trend of decreasing mean T2 values of the NP associate with increasing Pfirrmann grades (C2-T1) for both patients and asymptotic volunteers. Significant T2 differences were seen among grades I-V (P < 0.05). However, grade V was not observed in the CTJ. Linear correlation analysis revealed a strong negative association between T2 values of the NP and Pfirrmann grade (r = -0.588, r = -0.808) of C2-7 and C7T1. Age were also significantly correlated NP T2 values (r = -0.525, r = -0.723) for patients and volunteers. Moreover, the receiver operating characteristic analysis for average measures in a range from 0.70-0.79 (C2-7) to 0.84-0.89 (C7T1) for patients. Conclusions: T2 quantitation provides a more sensitive and robust approach for detecting and characterizing the early stage of IVD degeneration and age-associated disc changes.
Apparent Diffusion Coefficient of Diffusion-Weighted Imaging in Evaluation of Cervical Intervertebral Disc Degeneration: An Observational Study with 3.0 T Magnetic Resonance Imaging

Abstract:Aims: To investigate the correlation between the apparent diffusion coefficient (ADC) value and cervical intervertebral disc degeneration in adult symptomatic patients. Methods: A total of 52 symptomatic and 40 healthy volunteers were included. DWI and routine MRI examinations were performed to their cervical spines. The cervical discs (from C2-C3 to C6-C7) were graded according to the Pfirrmann grading system, and ADC values of the nucleus pulposus (NP) were measured. Differences of the ADC values between different genders and anatomic levels were analyzed; the correlation between the ADC value and the Pfirrmann grade was investigated. The cut-off ADC values of each Pfirrmann grade were calculated. Results: The mean ADC value of the NP decreased with increasing Pfirrmann grade (I-V) upon both patients and asymptotic volunteers. The ADC value decreased descendingly from C2-C3 to C5-C6 (P < 0.05) and then increased at C6-C7 (P < 0.05). Additionally, the comparison of the ADC values between different genders achieved statistical significance at each anatomical level (P < 0.05), except at C6-C7 (P > 0.05). Significant negative correlations between the ADC value and either age or Pfirrmann grade were observed. Conclusions: Our preliminary findings suggest that the ADC value obtained by DWI can provide a reliable indicator to evaluate the cervical disc degeneration.

Abstract:We study the effect of phase relaxation on coherent superpositions of rotating clockwise and anticlockwise wave packets in the regime of strongly overlapping resonances of the intermediate complex. Such highly excited deformed complexes may be created in binary collisions of heavy ions, molecules and atomic clusters. It is shown that phase relaxation leads to a reduction of the interference fringes, thus mimicking the effect of decoherence. This reduction is crucial for the determination of the phase–relaxation width from the data on the excitation function oscillations in heavy–ion collisions and bimolecular chemical reactions. The difference between the effects of phase relaxation and decoherence is discussed.

Abstract:The present study investigated the application of the T1ρ and T2 relaxation mapping magnetic resonance imaging (MRI) and diffusion-weighted imaging (DWI) in the evaluation of intervertebral disc degeneration (IDD). A total of 93 asymptomatic subjects were imaged with T1ρ and T2 mapping, as well as DWI. Pfirrmann grading was performed and correlation analysis was conducted for T1ρ, T2 and DWI results with the grading results and age. Pfirrmann grading indicated 69 cases of grade I, 240 cases of grade II, 101 cases of grade III, 43 cases of grade IV and 12 cases of grade V. MRI showed that the T1ρ values of the nucleus pulposi at L4/5 and L5/S1 were significantly reduced (P<0.05) and no significant differences were observed in the T2 values compared with the values of the nucleus pulposus at L1/2, L2/3 and L3/4. The apparent diffusion coefficient (ADC) values of L1/2 were significantly decreased from L2/3 and L3/4 (P<0.05). Correlation analysis revealed that the T1ρ, T, and ADC values were positively correlated with each other. Moreover, the T1ρvalues were significantly decreased with the increase of Pfirrmann grades (P<0.05), with the exception of grades IV and V. However, T2 and ADC values were not significantly different between grades I and II or IV and V. In addition, the T1ρ, T2 and ADC values were significantly decreased with the increase of age in patients with IDD (P<0.05). T1ρ and T2 mapping and DWI are promising techniques for the in vivo diagnosis of IDD, which may be useful in determining the appropriate prevention and treatment options for the disease.
Diurnal Variation in Hydration of the Cervical Intervertebral Disc Assessed Using T2 Mapping of Magnetic Resonance Imaging

Abstract:OBJECTIVE: The study aimed to investigate the diurnal variation in cervical disc hydration and its relationship with cervical degeneration.MATERIALS AND METHODS: C3-C7 discs of 86 prospectively enrolled participants (37 males, 49 females; mean age ± standard deviation, 23.5 ± 2.5 years) were assessed using T2 mapping in the morning and evening. All discs were stratified by Miyazaki grade or C2-C7 Cobb angle and T2 values (T2). The degree of diurnal T2 variation (T2-DDV), defined as (morning T2 - evening T2)/morning T2 × 100%, was measured for the entire disc, annulus fibrosus (AF), nucleus pulposus (NP), and endplate zones.RESULTS: T2 of the entire disc decreased significantly after the daytime load (p < 0.001), with a T2-DDV of 13.3% for all discs and 16.0%, 12.2%, and 13.0% for healthy (grade I), mild degenerative (grade II), and advanced degenerative (grade III/IV) discs, respectively. T2 of regional NPs and AFs decreased significantly from morning to evening (p ≤ 0.049) except in the healthy anterior inner AF (p = 0.092). Compared with healthy discs, mild degenerative discs displayed lower T2 and T2-DDV in regional NPs (p < 0.001). Advanced degenerative discs showed higher T2-DDV in the anterior inner AF compared with healthy discs (p = 0.050). Significant diurnal T2 changes in the endplate zones were observed only in healthy discs (p = 0.013). Cervical discs in the low Cobb angle group showed higher T2-DDV in the anterior AFs and anterior NP and lower T2-DDV in the posterior AF than those in the high Cobb angle group (p ≤ 0.041).CONCLUSION: This study characterized the diurnal variation in hydration of the cervical discs as assessed using T2 mapping and revealed early chemo-mechanical coupling dysfunction in degenerating discs. Cervical sagittal alignment on MRI can affect the diurnal stress patterns of the cervical discs. T2 mapping is sensitive to disc biomechanical dysfunction and offers translational potential from biomechanical research to clinical application.

Abstract:Objective: To investigate the relationship between degeneration of cervical intervertebral disc and degeneration of paravertebral muscles[multifidus (MF), cervical semispinalis (SCer), semispinalis capitis (SCap) and splenius capitis (SPL)]. Methods: 82 patients with chronic neck pain were randomly selected, including 43 males and 39 females, with 50.73 0.7.51 years old. All patients were scanned by 3.0T MRI Philips Ingenia performed conventional MRI sequence scanning and fat measurement sequence mDIXON-Quant scanning of cervical. Fat infiltration (FI) and cross-sectional area (CSA) of cervical paravertebral muscle (MF, SCer, SCap and SPL) at central level of C5–6 disc were measured by Philips 3.0T MRI image post-processing workstation. According to Pfirrmann grading system, there was no grade I in the included cases. The number of grade IIr IV cases were n=16, 40, 19 and 7 respectively. CSA and FI of cervical paravertebral muscles were compared with t test or one-way ANOVA, Spearman correlation analysis was used to evaluate the correlation between age, disc degeneration, and CSA, FI of cervical paravertebral muscles, and multiple linear regression analysis was used to analyze the independent influencing factors of CSA and FI. Results: CSA of cervical paravertebral muscles in male patients was significantly higher than that in female patients (all P 0.05). Age was weakly correlated with CSA of MF+SCer, moderately correlated with CSA of SCap and SPL (r=-0.256, -0.355 and -0.361, P<0.05), weakly correlated with FI of SCap and SPL (r= 0.182 and 0.264, P<0.001), moderately correlated with FI of MF+SCer (r=0.408, P<0.001). There were significant differences in FI with disc degeneration (P<0.001, P=0.028 and P=0.005). Further correlation analysis showed that disc degeneration was strongly correlated with FI of MF+SCer (r=0.629, P<0.001), and moderately correlated with FI of SCap and SPL (r=0.363, P=0.001; r=0.345, P=0.002). Multiple linear regression analysis showed that sex and age were the influencing factors of CSA of SCap and SPL, sex was the independent influencing factor of CSA of MF+SCer, and disc degeneration was the independent influencing factor of FI. Conclusions: Age is negatively correlated with CSA and positively correlated with FI. Disc degeneration was correlated with FI of paravertebral muscles, especially with FI of MF and SCer. Sex and age were the influencing factors of CSA, while disc degeneration was the independent influencing factor of FI.

Abstract:BACKGROUND AND OBJECTIVE: The intervertebral disc degeneration changes and paraspinal muscles changes are believed to be risk factors for lumbar degenerative spondylolisthesis (LDS). But there is limited quantitative information about this progression. This study is to reveal their changes the in the progression of LDS. METHODS: Data were gathered from 149 middle-aged degenerative spondylolisthesis patients and same amount of age-and sex-matched control group with both lumber spine MRI and X-ray. Narrowed disc space were measured in percent as anterior inferior disc height (DHIA)/anterior superior disc height (DHSA), inferior disc height (DHI)/superior disc height (DHS), and posterior inferior disc height (DHIP)/posterior superior disc height (DHSP). Signal intensity ratio of multifidus muscle (RM) and erector spinae (RES) to psoas muscle, muscle atrophy ratio of lean CSA (LCSA) to gross CSA (GCSA) of paraspinal muscles were calculated. RESULTS: In the case group the most common slipped vertebra was L4 (75.84%). Disc height (DHIA/DHSA, DHI/DHS) and multifidus muscle atrophy ratio (M-LCSA/M-GCSA) tended to be lower than those in the control group, whereas the disc degeneration degree and T2 signal intensity ratio (RM, RES) of the paraspinal muscles and erector spinae muscle atrophy ratio were higher than control group. The difference between the two groups was statistically significant (P < 0.05). Using multivariate logistic regression analysis, it was confirmed that ES-LCSA/ES-GCSA, especially RM are independent predisposing factors to lumbar spondylolisthesis (OR > 1, P < 0.05) while DHIA/DHSA, M-LCSA/M-GCSA are independent protective factors (OR < 1, P < 0.05). CONCLUSIONS: Decreased anterior disc height and multifidus muscle atrophy are found in the LDS patients and thy could be the cause of LDS. The presence of erector spinae hypertrophy could be a compensatory mechanism to compensate for the instability.

Abstract:BackgroundStudies have shown that lumbar disc herniation, degenerative lumbar instability, and other degenerative lumbar spinal diseases are often secondary to disc degeneration. By studying the intervertebral disc, researchers have clarified the pathological changes involved in intervertebral disc degeneration but have ignored the roles of biomechanical factors in the development of disc degeneration. This study aims to investigate the relationships among the location, scope, and extent of lumbar disc degeneration and sagittal spinal-pelvic parameters.MethodsA retrospective analysis was performed on the clinical data of 284 patients with lumbar degenerative disc diseases (lumbar disc herniation and degenerative lumbar instability) from January 2013 to December 2016. Statistics were calculated for the following: (1.) patients’ general information: name, sex, age, height, and weight. (2.) Measurements of sagittal parameters from total spinal radiographs: thoracic kyphosis (TK), Lumbar lordosis (LL), sacral slope (SS), pelvic tilt (PT), pelvic incidence (PI), sagittal vertical axis (SVA), T1 tilt angle (TA), and T1 pelvic angle (TPA). (3.) Location, scope, extent, and overall degree of lumbar disc degeneration. Parameters were analyzed in groups by sex, PI, and SVA, and a correlation analysis was performed for the location, scope, extent, and overall degree of lumbar intervertebral disc degeneration with 8 spinal-pelvic sagittal parameters.ResultsThe mean ages of the male and female patient groups were 59.00 and 53.28 years old, respectively (P < 0.05). The PT, location, scope, and overall degree of degradation were significantly different between the sexes (P < 0.05). Linear correlation analysis results showed that the overall degree and extent of degradation (r = 0.788, P < 0.01), LL and SS (r = 0.737, P < 0.01), PI and PT (r = 0.607, P < 0.01), and TPA and PT (r = 0.899, P < 0.01) were strongly correlated. The location values were 4.08 ± 0.72 in patients with PI≤50° and 3.62 ± 0.94 in patients with PI> 50° (P = 0.018). Different SVASVA groups differed in their overall degree of degeneration (P = 0.002).ConclusionsThe location of lumbar intervertebral disc degeneration is affected by spinal-pelvic sagittal morphology. Populations with small PI values tend to exhibit degeneration at the L4/5 and L5/S1 discs, and populations with large PI values tend to exhibit degeneration at the L3/4 and L4/5 discs. The SVA value and the overall degree of lumbar disc degeneration are positively correlated.

The measurement of normal lumbar intervertebral discs and its significance

Abstract:Objective Relevant parameters of normal lumbar intervertebral discs were measured, in order to provide basic data for designing artificial lumbar disc and artificial prosthetic nucleus that are suitable for Chinese as well as offer data of normal disc height as reference for clinical diagnosis of disc height loss.Methods MRI data of normal lumbar vertebrae of 300 Chinese adults(including 136 males and 164 females)who complained of low back pain were collected. The anterior height, the middle height, the posterior height and the intervertebral angle in segment L1/2~L5/S1, the sagittal and transverse diameter of disc, the sagittal and transverse diameter of nucleus pulposus in segment L2/3~L5/S1 were measured. Results No significance was detected in age(P>0.05). Significance was detected in gender(P<0.05) and data got from male was higher than that from female mostly. Significance was detected in segment(P<0.05). The anterior height and intervertebral angle increased with the increment of the segment number. The middle height, the posterior height, the sagittal and transverse diameter of disc, the sagittal and transverse diameter of nucleus pulposus increased with the increment of the segment number except for segment L5/S1. In each segment, There were linear relationships between the sagittal and transverse diameter of disc, with the same case in the sagittal and transverse diameter of nucleus pulposus. Conclusions Measurement of disc height is valuable to the diagnosis of lumbar spine disease. In the design of artificial lumbar disc and artificial prosthetic nucleus, gender and segmental difference should be considered. The linear relationship between the sagittal and transverse diameter of disc and nucleus pulposus should also be taken into account.

Abstract:0bjective To evaluate the distribution of Modic changes of cervical endplate in patients suffered neck pain and its related factors.Methods 1023 cases of patients suffered from neck pain without any pathological lesions were examined by MRi scan and X-ray.According to the criteria of Modic changes,the feature of distribution of Modic changes of cervical endplate on age,segment and grade of intervertebral disc degeneration was analyzed retrospectively.Results Among 6138 intervertebral disc of 1023 cases of patients,90(8.8%)patients and 108(1.8%)intervertebral discs were involved with Modic changes.32(3.1%) cases and 34(0.6%)discs were type I;55(5.4%)cases and 71(1.2%)discs were typeⅡ;3(0.3%)cases and 3(0.06%)discs were typeⅢ.According to the segment,the lesions involved C2-3. disc for 0(0%),C3-4 for 10(0.2%),C4-5 for 18(0.4%),C5-6 for 52(1.O%),C6-7 for 23(0.4%),C7-T1 for 5(0.1%).Modic changes was distributed mainly over the age of 40 and correlated with disc degeneration,disc level,condition of eervical curve and ages,moreover,disc degeneration played the most important role in the occurrence of Modic changes.Conclusion Modic changes of cervical endplate mainly OCCULT in C5-6 segment,then the C4-5 and C6-7 segment,and type Ⅱ is the most and type Ⅲ is the least.Modic changes is distributed mainlv over the age of 40 and correlated with disc degeneration,disc level condition of eervieal curve and ages,moreover,disc degeneration plays an important role in the occurrence of Modic changes.
Evaluation of lumbar intervertebral disc degeneration in ankylosing spondylitis patients with MRI

Abstract:Objective To evaluate the risk factors of intervertebral disc degeneration, MRI findings of lumbar intervertebral disc degeneration in ankylosing spondylitis (AS) patients and control patients were compared. Methods This study retrospec-tively analyzed 40 AS patients and 40 age-and sex-matched control patients without history of rheumatic disease. Four hundred in-tervertebral discs were assessed in terms of degeneration by analyzing signal intensity and morphology on T2weighted magnetic res-onance images. Degeneration degree of the intervertebral discs was compared between the two groups,and the risk factors of disc degeneration in AS patients were analyzed. Results The mean of total degeneration degrees for five lumbar intervertebral discs was significantly higher in AS patients compared to the control group(16.77 ± 4.67 vs 13.00 ± 4.08,P =0.001). With the ex-ception of L5/S1,disc degeneration was significantly higher in AS patients compared to the control group,when different segment intervertebral discs were analyzed respectively. Age,duration of disease and discitis were significantly associated with degenera-tion degree in AS patients( r =0.74,r =0.48,P <0.05). No significant correlation was determined between disc degeneration and vertebral inflammation ( r =0.15,P =0.43). Conclusion Magnetic resonance imaging is effective for evaluation of lum-bar discs in patients with AS. AS and its inflammatory effects on the discs are risk factors for disc degeneration. The duration of AS and impaired posture may aggravate disc degeneration.

Abstract:Background: Quantitative magnetic resonance imaging (MRI) has the function of noninvasive quantitative evaluation, providing unique advantages in intervertebral disc degeneration (IDD) assessment. Although studies exploring the field for domestic and international scholars are increasingly being published, there is a lack of systematic scientific measurement and clinical analysis of the literature in this field.Methods: Articles published from the respective database establishment to September 30, 2022, were obtained from the Web of Science core collection (WOSCC), PubMed database, and ClinicalTrials.gov. The scientometric software (VOSviewer 1.6.18, CiteSpace 6.1.R3, Scimago Graphica, and R software) were used for bibliometric and knowledge graph visualization analysis.Results: We included 651 articles from the WOSCC database and 3 clinical studies from ClinicalTrials.gov for literature analysis. With the passage of time, the number of articles in this field gradually increased. The United States and China were the top 2 countries in terms of the number of publications and citations, and Chinese publications lacked international cooperation and exchange. The author with the most publications was Schleich C, while the author with the most citations was Borthakur A, who have both made important contributions to research in this field. The journal publishing the most relevant articles was Spine, and the journal with the most mean times cited per study was Radiology, both of which are the authoritative journals in this field. Keyword co-occurrence, clustering, timeline view, and emergent analysis revealed that recent studies in this field have focused on quantifying the biochemical components of the degenerated intervertebral disc (IVD). There were few available clinical studies. The more recent clinical studies mainly used molecular imaging technology to explore the relationship between different quantitative MRI sequence values and the IVD biomechanical environment and biochemical components content.Conclusions: The study provided a knowledge map of quantitative MRI for IDD research in terms of countries, authors, journals, cited literature, and keywords through bibliometric analysis, and systematically sorted the current status, hotspots, and clinical research features in the field to provide a reference for future research.

Abstract:AIM: To prospectively evaluate the degeneration of intervertebral discs (IVDs) in adolescent idiopathic scoliosis (AIS) patients using diffusion tensor imaging (DTI). MATERIALS AND METHODS: Forty-three AIS patients and 12 healthy volunteers (HC) underwent 3 T magnetic resonance imaging (MRI) including DTI sequences. The mean diffusivity (MD) and fractional anisotropy (FA) values of lumbar IVDs were obtained and compared between HC and AIS patients with two independent samples t-tests. Further analyses were done and focused on degenerative and non-degenerative discs and different lumbar segmental with two independent samples t-tests. RESULTS: The MD and FA values of IVDs in AIS patients differed significantly with healthy controls (MD: 1.25 +/- 0.17 versus 1.56 +/- 0.23 x 10(-3) mm(2)/s, p<0.001; FA: 0.31 +/- 0.06 versus 0.28 +/- 0.08, p = 0.014). The lowest MD (1.13 +/- 0.17 x 10(-3) mm(2)/s, p = 0.001) and the highest FA values (0.33 +/- 0.05, p = 0.045) were obtained in degenerative IVDs compared with non-degenerative IVDs in AIS patients. The MD values of L4-5 IVDs were significantly higher than those of L1-3 IVDs in HC (p = 0.01), but there was no significant difference of MD values between L1-3 and L4-5 IVDs in AIS patients. CONCLUSION: DTI demonstrated degeneration of lumbar IVDs in AIS patients even without visible signs on T2-weighted images, and L4-5 IVDs were more prone to degenerate. DTI could serve as a sensitive technique to evaluate the degeneration of IVDs in AIS patients non-invasively and quantitatively. (C) 2018 The Royal College of Radiologists. Published by Elsevier Ltd. Abstract:Background: Lumbar intervertebral degenerative disc disease (IDD) is a multifaceted progressive condition that commonly occurs in conjunction with lumbar disc herniation (LDH). CXCL10 mRNA appears to be increased in both IDD and LHD. Objective: This study was performed to identify the relationship between serum CXCL10 levels and disease severity in patients with IDD. Methods: 136 IDD patients with low back pain, 127 asymptomatic volunteers and 120 healthy controls were enrolled. Serum CXCL10 protein concentrations were detected using commercial human CXCL10 ELISA Kits. Serum CXCL10 mRNA were examined using qRT-PCR. Clinical severity was assessed using the visual analog scale (VAS) and Oswestry Disability Index(ODI) scores. Radiographic severity was defined using the MRI-based Pfirrmann classification of disc degeneration. Receiver operating characteristic (ROC) curve analysis was used in estimating the correlation between CXCL10 and Pfirrmann grade. The cross-sectional area (CSA) of the lumbar multifidus muscle (LMM) and psoas major (PM) were calculated, and fat infiltration was evaluated by Ropponen-Kjaer criteria. Results: Serum CXCL10 concentrations were markedly raised in IDD patients with low back pain in contrast to asymptomatic individuals and healthy controls. Serum CXCL10 levels were positively associated with Pfirrmann grade. ROC curve analysis indicated that serum CXCL10 correlated well with Pfirrmann grade. In addition, serum CXCL10 concentrations were significantly higher in IDD patients with LMM and PM degeneration compared with IDD patients without degeneration. Increased CXCL10 levels positively correlated with VAS and ODI scores, as well as decreased CSA and fat filtration of the LMM and PM. Conclusion: Increased serum CXCL10 levels correspond to clinical severity and radiographic progression in IDD patients.

Abstract:Objective: to study the morphoscopic and morphometric characteristics of the LV-SI intervertebral disc according to magnetic resonance imaging. Materials and methods. The analysis of magnetic resonance imaging of 90 patients (66 men, 24 women) who did not have diseases of the lumbar spine was performed. Various morphometric parameters of the intervertebral disc (IVD) and nucleus pulposus (NP) at the LV-SI level were measured using software for processing images created on an MRI machine. The obtained data were compared between men and women, asthenics, normosthenics, hypersthenics and young (from 18 to 44 years old) and middle-aged (from 45 to 60 years old) people. In addition, morphoscopic characteristics were studied: the shape of the IVD and NP in the axial plane. Results. It was found that in normal IVD LV-SI is most often represented by elliptical (54,5%) less often kidney-shaped (45,5%) forms. Normally, the NP has an oval (57,8%) and kidney-shaped (42,2%) shape. It was shown that in men, the dorsal height of the IVD, its width, anteroposterior size, square as well as the longitudinal-transverse index of the NP were significantly higher. It has been proven that there are no significant differences in the shape of the IVD between asthenics, normo- and hypersthenics, while the height of the IVD in the center, its ventral, dorsal heights and most lateral IVD height are significantly greater in hypersthenics. It has been proved that the kidney-shaped form of NP was significantly more common in asthenics, and the oval form - in hypersthenics, while the morphometric parameters of NP did not significantly differ between extreme body types. A tendency to a decrease in IVD height in middle-aged people compared to young people was shown. Conclusion. The results obtained are necessary when planning the operation, as well as for the manufacture of rigid implants and artificial discs for arthroplasty and interbody fusion.
