Abstract:Abstract Study Objectives Obstructive sleep apnea (OSA) increases the risk of cognitive impairment. Measures of sleep microarchitecture from EEG may help identify patients at risk of this complication. Methods Participants with suspected OSA (n = 1142) underwent in-laboratory polysomnography and completed sleep and medical history questionnaires, and tests of global cognition (Montreal Cognitive Assessment, MoCA), memory (Rey Auditory Verbal Learning Test, RAVLT) and information processing speed (Digit–Symbol Coding, DSC). Associations between cognitive scores and stage 2 non-rapid eye movement (NREM) sleep spindle density, power, frequency and %-fast (12–16Hz), odds-ratio product (ORP), normalized EEG power (EEGNP), and the delta:alpha ratio were assessed using multivariable linear regression (MLR) adjusted for age, sex, education, and total sleep time. Mediation analyses were performed to determine if sleep microarchitecture indices mediate the negative effect of OSA on cognition. Results All spindle characteristics were lower in participants with moderate and severe OSA (p ≤ .001, vs. no/mild OSA) and positively associated with MoCA, RAVLT, and DSC scores (false discovery rate corrected p-value, q ≤ 0.026), except spindle power which was not associated with RAVLT (q = 0.185). ORP during NREM sleep (ORPNREM) was highest in severe OSA participants (p ≤ .001) but neither ORPNREM (q ≥ 0.230) nor the delta:alpha ratio were associated with cognitive scores in MLR analyses (q ≥ 0.166). In mediation analyses, spindle density and EEGNP (p ≥ .048) mediated moderate-to-severe OSA’s negative effect on MoCA scores while ORPNREM, spindle power, and %-fast spindles mediated OSA’s negative effect on DSC scores (p ≤ .018). Conclusions Altered spindle activity, ORP and normalized EEG power may be important contributors to cognitive deficits in patients with OSA.

Abstract:Abstract Introduction Obstructive sleep apnea (OSA) is associated with major neurocognitive sequalae including impaired memory. OSA patients are deficient in NREM sleep spindles (11 Hz to 16 Hz electroencephalography (EEG) sigma oscillations) which have been shown to promote memory. This finding is a result of less time spent in deeper sleep states in which most spindles occur but also a reduction in spindle density, particularly during the later portion of the night. We tested the hypothesis that spindle density deficiencies are driven by restriction of breathing preventing spindle generation. Methods 109 adults (ages 65-83, 53% female) with normal cognition underwent overnight polysomnography (PSG). Presence of sleep apnea was defined using AASM recommended criteria (apnea-hypopnea index >15, n=81[OSA] vs 28[controls]). The right occipital channel EEG was bandpass filtered between 0.3-35Hz. Following rejection of movement artifacts, spindles were detected in NREM sleep stages 2 and 3 (N2/N3) using complex demodulation to extract high sigma activity signals from background sigma noise. Spindles were classified depending on whether the patient was in eupnea or apnea-hypopnea. Results Overall spindle density in OSA patients did not differ from overall spindle density in controls (2.86 vs 3.17 spindles/minute, t(57.4)=0.49, p=0.14). Apnea-hypopnea spindle density in OSA patients did not differ from eupnea spindle density in controls (3.10 vs 3.17 spindles/minute, t(70.8)=0.31, p-value=0.76). Eupnea spindle density in OSA patients was significantly lower than eupnea spindle density in controls (2.57 vs 3.17 spindles/minute, t(54.4)=2.94, p-value< 0.01). Conclusion Contrary to our hypothesis, sleep spindle density was not reduced in OSA patients during apnea-hypopnea compared to healthy controls during eupnea, suggesting restriction of breathing does not affect spindle generation, but was reduced in OSA patients during eupnea. Whilst we did not observe an overall OSA-related spindle density deficit, our results suggest that previous findings of lower spindle density in OSA patients may in fact result from changes in spindle behaviour during eupnea. This observation raises the interesting possibility that using OSA interventions to prevent apnea-hypopnea occurring may not recover normal spindle density. Alternatively, our results could suggest that undetected N2/N3 sleep disruptions are occurring when OSA patients are breathing, skewing eupnea spindle density estimates. Sleep spindle metrics, including frequency (Hz), overall density (events/min), fast density (events/min), slow density (events/min), duration (sec) and amplitude (μV), were measured. Executive function was assessed using standardized neuropsychological tests. Associations between sleep spindle metrics, executive function, and demographic factors were analyzed using multivariate linear regression. Results: In fully adjusted linear regression models, higher overall sleep spindle density (TMT-A, B=− 1.279, p=0.009; TMT-B, B=− 1.813, p=0.008), fast sleep spindle density (TMT-A, B=− 1.542, p=0.048; TMT-B, B=− 2.187, p=0.036) and slow sleep spindle density (TMT-A, B=− 1.731, p=0.037; TMT-B, B=− 2.449, p=0.034) were associated with better executive function. And the sleep spindle duration both during N2 sleep time (TMT-A, B=− 13.932, p=0.027; TMT-B, B=− 19.001, p=0.034) and N3 sleep time (TMT-B, B=− 29.916, p=0.009; Stroop-incongruous, B=− 21.303, p=0.035) was independently associated with better executive function in this population. Additionally, age and education were found to be highly associated with executive function. Conclusion: Specific sleep spindle metrics, higher overall density, fast density and slow density during N2 sleep time, and longer duration during N2 and N3 sleep time, are independent and sensitive indicators of better executive function in young adult and middle-aged patients with OSA. Further research is needed to explore the underlying mechanisms and clinical implications of these findings. These occurrences lead to hypoxia, sleep fragmentation, and subsequent physiological damage. Epidemiological studies have revealed significant independent associations between OSA and conditions such as hypertension, coronary artery disease, heart failure, and cognitive impairment. 3–5 Epidemiology shows that OSA co-morbid with cognitive impairment can be observed in children, adolescents, and adults. Abstract:Background: Obstructive sleep apnea-hypopnea syndrome (OSAHS) is a sleep disorder causing cognitive impairments. Aims: We use the auditory verbal learning test (AVLT), clock drawing test (CDT), Wechsler intelligence scale for children (WISC) and Montreal cognitive assessment (MoCA) to evaluate the memory and spatial impairments of OSHAS in 6-12 years old children patients with different severity. Material and methods: A total of 137 children of snoring were enrolled following the inclusion criteria of this study. According to the apnea-hypopnea indices (AHI), they were divided into three groups. The AVLT, CDT, WISC and MoCA tests were executed by physicians. The self-rating depression scale (SDS) test was performed for depression screening. Results: Compared with the children in the primary snoring group, the other two groups had higher body mass index (BMI), longer periods of snoring and older age. The AHI, oxygen desaturation index (ODI) and 90% oxygen saturation (TS90%) showed increasing trends whereas the lowest blood oxygen saturation (LSaO2) showed a decreasing trend. Besides, compared with the primary snoring group, the two groups had lower immediate recall scores in AVLT. Conclusion: AVLT had clinical values for evaluation of impaired memory function in OSAHS children, suggesting a correlation between cognitive impairments and nocturnal hypoxia.

Abstract:OBJECTIVE:To explore the variability of event-related potentials P(300) and memory function in patients with obstructive sleep apnea syndrome(OSAS).METHODS:Polysomnography (PSG) event-related potentials (ERPs) P(300) and Wechsler Memory Scale (WMS) were detected in 30 subjects with OSAS and 20 normal controls.RESULTS:The memory quotient [(86 ± 13) scores] in the OSAS group was significantly lower than that in the control group (P < 0.01). The latency of P(300) [(410 ± 80) ms at Cz and (409 ± 80) ms at Pz] in the OSAS group was significantly longer than that in the control group (P < 0.01). In the OSAS group, the latency of P(300) was correlated negatively with the memory quotient and nocturnal hypoxemia, but positively with apea/hypopnea index (AHI, P < 0.01).CONCLUSION:OSAS has memory deficit, which can be evaluated comprehensively by P(300) and WMS. The longer latency of P(300) might be regarded as one of the electrophysiological markers to indicate the severity of cognition in OSAS. Recurrent attacks of nocturnal intermittent hypoxemia may cause the regional brain damage related to cognition defect.

Abstract:Abstract Introduction Obstructive sleep apnea (OSA) is associated with sleep disturbances, memory impairment, and dementia risk in older adults. The impact of OSA on sleep neurophysiology and related memory function remains unclear. We examined associations among OSA severity, non-rapid eye movement (NREM) sleep expression, and overnight memory retention in cognitively intact older adults at risk for Alzheimer’s disease (AD). Methods Overnight clinical polysomnography (PSG) with 256-channel electroencephalography (EEG) was conducted in 81 cognitively intact older adults (aged 61.68◻5.99 years, 49 female, Apnea/hypopnea Index, AHI: 7.61◻10.64 ), with word-pairs task (WPT) encoding and cued recall occurring prior to and following sleep. AHI, respiratory disturbance index (RDI), and oxygen desaturation index (ODI) were used as measures of OSA severity and cube-root transformed to normality. EEG data underwent preprocessing, artifact rejection, and segmentation into concatenated NREM epochs, and were then processed using multitaper spectral analysis with 11 tapers. Behavioral data, obtained as overnight proportion change in retention of word-pairs, were also cube-root transformed to enhance normality. Topographical correlations were then computed to examine associations between NREM sleep features and OSA severity, using 5000 permutations of threshold-free cluster enhancement (TFCE) for multiple comparisons correction. Multiple linear regression modeling was leveraged to examine whether spatiospectral NREM clusters associated with OSA severity were in turn predictive of overnight memory retention when adjusting for age and sex. Results Global, TFCE-significant (p< 0.05 corrected) negative correlations were seen between delta power (1-4.5Hz) and AHI and RDI, theta power (4.5-7.5Hz) and AHI and RDI, alpha power (7.5-11Hz) and RDI, and slow sigma power (11-13Hz) and ODI . Regression models (adjusted for age and sex) using cluster averages of spectral power from prior correlations to predict WPT performance further revealed significant effects for theta power (AHI cluster: B=0.077, p=0.007; RDI cluster: B=0.076, p=0.008), alpha power (RDI cluster: B=0.107, p=0.010), and for slow sigma power (ODI cluster: B=0.203, p=0.007), but not for delta power (both clusters: p>0.33). Conclusion These findings indicate that OSA is associated with globally disrupted NREM sleep expression, and that reduced theta, alpha, and spindle activity is further associated with impaired overnight episodic memory retention. Abstract:ObjectiveObstructive sleep apnea (OSA), a sleep disorder, results in decreased daytime alertness and neurocognitive dysfunction. Obesity is considered a major risk factor for the development and progression of OSA and the resulting cognitive dysfunction. However, the effect of obesity on neurocognitive dysfunction in OSA has been rarely investigated.MethodsEighty-three patients with moderate to severe OSA syndrome were recruited in our study. After matching for education, age, and body mass index (BMI), 40 patients were enrolled into our study with matched obese (BMI ≧ 30) and non-obese (BMI < 30) groups. All enrolled patients completed a polysomnographic study, sleepiness questionnaires, and attention, cognitive, and memory function tests.ResultsCompared to obese OSA patients, non-obese OSA patients had shorter reaction times in the psychomotor vigilance task but not the Flanker or Stroop cognitive tasks. Additionally, obese OSA patients had a reduced capacity for working memory relative to non-obese OSA patients.ConclusionsObesity had a significant effect on OSA patients in our study, including delayed reaction times in the psychomotor vigilance task and a decrease in working memory.

Abstract:Objective: The present study endeavored to investigate the interconnection between obstructive sleep apnea (OSA) and cognitive function, alongside the manifestations of depression and anxiety. Simultaneously, an analysis was conducted to discern the factors exerting influence upon cognitive function. Methods: A cohort of 102 patients, who had undergone polysomnography (PSG) at Binhu Hospital, Anhui Medical University, between January 2022 and June 2023, was encompassed in the study. Employing the PSG findings, these individuals were classified into two distinct categories: the grouping consisted of those with either negligible or mild OSA, and the other comprised individuals with moderate to severe OSA. Utilizing the Montreal Cognitive Assessment (MoCA-Beijing), Stroop Color and Word Test (SCWT), Digit Span Test (DST), Self-rating Depression Scale (SDS), and Self-rating Anxiety Scale (SAS), scores were recorded and analysed for each of the respective assessments. Additionally, discrepancies and associations between these groups were also scrutinized. Results: The group exhibiting moderate to severe OSA demonstrated significantly elevated measurements in parameters such as neck circumference, BMI, completion times for SCWT-A, B, C, Sleep Inefficiency Index (SIE), SAS, and SDS, in comparison to the No or Mild OSA group. Furthermore, the moderate–severe OSA group manifested notably diminished MoCA scores in areas of visual–spatial and executive function, memory, language, abstraction, delayed recall, orientation, total MoCA score, lowest oxygen saturation (LSaO2), average oxygen saturation, Digit Span Test-backward(DST-b), and Digit Span Test-forward(DST-f), as contrasted with the no-mild OSA group. These inter-group disparities exhibited statistical significance ( p < 0.05). The MoCA total score portrayed inverse correlations with age, Apnea-Hypopnea Index (AHI), BMI, SIE, SAS, SDS, CT90%, AHT90%, and Hypoxic Apnea Duration (HAD) (ranging from −0.380 to −0.481, p < 0.05). Conversely, it displayed positive correlations with DST-f, DST-b, LSaO2, and average oxygen saturation (ranging from 0.414 to 0.744, p < 0.05). Neck circumference, AHI, and SAS were autonomously linked to MoCA scores (OR = 1.401, 1.028, 1.070, p < 0.05), while AHI exhibited an independent correlation with SDS and SAS scores (OR = 1.001, p = 0.003). Conclusion: Patients grappling with moderate to severe OSA frequently reveal cognitive impairment and concomitant emotional predicaments encompassing depression and anxiety. These manifestations share an intimate association with AHI, LSaO2, and average oxygen saturation. Notably, anxiety, when coupled with OSA, emerges as an autonomous influential element impinging upon cognitive impairment.

Abstract:Objective: To investigate the alertness and task processing speed impairment status in young-mild aged men with obstructive sleep apnea hypopnea syndrome (OSAHS), and analyze its influencing factors. Methods: This prospective study recruited 251 snoring patients aged 18 to 59 (38.9±7.6) years in the Sleep Center of the Second Affiliated Hospital of Soochow University from July 2020 to September 2021 and all patients were diagnosed by polysomnography (PSG). Clinical information, Epworth Sleepiness Scale (ESS) and PSG date were collected. All patients were assessed with the Montreal Cognitive Assessment (MoCA) questionnaires, Mini-mental State Examination (MMSE) and Computerized Neurocognitive Assessment System which includes the reaction time of Motor Screening Task (MOT) for alertness, the reaction time of pattern recognition memory (PRM), spatial span (SSP) and spatial working memory (SWM) for task processing speed. Based on AHI tertiles, all patients were divided into Q1 group (AHI<15 times/h, n=79), Q2 group (15 times/h≤AHI<45 times/h, n=88), and Q3 group (AHI≥45 times/h, n=84). The characteristics of clinical information, ESS, PSG parameters and cognitive scores among three groups were compared. Multiple linear stepwise regression was conducted to analyze the influencing factors of cognitive impairment. Results: There were no statistically significant differences in age, years of education, history of smoking and drinking, and past disease history (except for the prevalence of hypertension) among the 3 groups (P>0.05). There were statistically significant among-group differences in the body mass index (BMI), ESS, prevalence of hypertension and complaints of daytime sleepiness (P<0.05). Compared with Q1 and Q2 group, the arousal index (ArI), oxygen desaturation index (ODI),the proportion of non-rapid eye movement phase 1 and 2 (N1+N2) and percentage of total sleep time with oxygen saturation level<90% (TS90) of Q3 group were higher (all P<0.05). In the cognitive assessment, there was no statistically significant difference in the MoCA total and individual scores and MMSE scores among the three groups (P>0.05). Compared with the Q1 group, the task processing speed and alertness were worse in Q3 group, as shown by slower PRM immediate and delayed reaction time, SSP reaction time and MOT reaction time (all P<0.05). The total time of SWM in Q2 group was slower than that in Q1 group (P<0.05). Multiple linear stepwise regression showed that years of education (β=-40.182, 95%CI:-69.847--10.517), ODI (β=3.539, 95%CI: 0.600-6.478) were the risk factors of PRM immediate reaction time. Age(β=13.303,95%CI: 2.487-24.119), years of education(β=-32.329, 95%CI:-63.162--1.497), ODI (β=4.515, 95%CI: 1.623-7.407) were the risk factors of PRM delayed reaction time. ODI was the risk factor of SSP reaction time (β=1.258, 95%CI: 0.379-2.137). TS90 was the risk factor of MOT reaction time (β=1.796, 95%CI: 0.664-2.928). Conclusions: The early cognitive impairment in young-mild aged OSAHS patients was manifested in decreased alertness and task processing speed, and intermittent nocturnal hypoxia was its influencing factor in addition to age and years of education.

Abstract:Background: We compared the density and duration of sleep spindles topographically in stage 2 and 3 of non-rapid eye movement sleep (N2 and N3) among adults diagnosed with Obstructive Sleep Apnea Syndrome (OSAS) and healthy controls. Materials and Methods: Thirty-one individuals with OSAS (mean age: 48.50 years) and 23 healthy controls took part in the study. All participants underwent a whole night polysomnography. Additionally, those with OSAS were divided into mild, moderate and severe cases of OSAS. Results: For N2, sleep spindle density did not significantly differ between participants with and without OSAS, or among those with mild, moderate and severe OSAS. For N3, post-hoc analyses revealed significantly higher spindle densities in healthy controls and individuals with mild OSAS than in those with moderate or severe OSAS. Last, in N2 a higher AHI was associated with a shorter sleep spindle duration. Conclusion: OSAS is associated with a significantly lower spindle density in N3 and a shorter spindle duration in N2. Our results also revealed that, in contrast to moderate and severe OSAS, the sleep spindle characteristics of individuals with mild OSAS were very similar to those of healthy controls.

However, the exact mechanisms of CI development in patients with OSA remains unclear. We investigated the neuropathological basis of CI development by examining changes in cerebral blood perfusion. Patients and Methods: Thirty-five patients with untreated OSA (15 with CI and 20 without CI [NCI]) and 15 good sleepers (GS) diagnosed using polysomnography were recruited. All participants underwent resting state brain scans in a Siemens 3.0 Tesla magnetic resonance imaging scanner with a pulsed arterial spin labeling sequence and completed a battery of neuropsychological tests. Results: Compared to the regional cerebral blood flow (rCBF) values in the GS group, both the CI and NCI groups exhibited lower rCBF values in the bilateral inferior temporal, left lingual, and right medial and paracingulate gyri, as well as higher rCBF values in the bilateral middle frontal gyrus ( p < 0.05 in all cases). Compared to the rCBF values in the NCI group, the CI group had lower rCBF values in the bilateral inferior temporal and left lingual gyri, and higher rCBF values in the right rectus and right middle orbital frontal gyri ( p < 0.05 in all cases). In the CI group, rCBF values in the bilateral inferior temporal (right, p = 0.025; left, p = 0.005) and left lingual gyri ( p = 0.018) were positively associated with the delayed memory scores, and rCBF values in the left inferior temporal gyrus positively correlated with the attention scores ( p = 0.011). Conclusion: Regions with abnormal perfusion in the NCI and CI groups were mostly memory-related. Blood perfusion in the bilateral inferior temporal and left lingual gyri decreased in the following order: GS > OSA-NCI > OSA-CI. These findings provide blood perfusion-level insights into the neuropathological basis of OSA-CI development. The dysfunction of cerebral vascular endothelial cells owing to blood pressure fluctuations in patients with OSA may further contribute to the abnormal cerebral blood flow perfusion in the affected brain areas, ultimately leading to structural 9 and functional 10,11 brain damage and cognitive function decline. 7 This suggests that cerebral blood perfusion may be used as an important indicator of neuropathological mechanisms of OSA-CI. Therefore, in the present study, we employed arterial spin labeling (ASL) 8 and examined local cerebral hemodynamic features to investigate neuropathological characteristics of patients with OSA-CI. Abstract:BACKGROUND The NHS and Community Care Act 1990, implemented from 1 April 1993, was expected to alter profoundly the provision of domiciliary services to old people. This change needs to be compared with what would have happened if no action had taken place. We sought to identify trends in community and residential care before and after 1993 to facilitate this comparison. METHODS Statistics were collated from diverse government publications and other authoritative sources. RESULTS Between 1985 and 1993 there was a progressive contraction of public community and residential services, as well as hospital beds available to the elderly. The concomitant huge increase in places publicly funded in private old people's and nursing homes appears to have been little affected by the implementation of the NHS and Community Care Act. CONCLUSIONS The NHS and Community Care Act has had little effect in curbing the institutionalization of old people, which has reached now the highest percentage this century at 7 per cent. A considerable expansion of community services would be required to return to the per capita provision of the early 1980s.

Abstract:Objective The aim of this study was to analyze the relationship between serum amyloid A (SAA) concentrations in patients with obstructive sleep apnea–hypopnea syndrome (OSAHS) and their magnetic resonance imaging (MRI) of resting brain function. Methods Male patients with OSAHS were enrolled from January to June 2019 in Suzhou Ninth People's Hospital Affiliated to Soochow University, and nineteen healthy male volunteers were selected as the normal control group. The patients with OSAHS were divided into mild, moderate, and severe groups according to their apnea-hypopnea index (AHI). Early in the morning after the polysomnography (PSG), blood samples were collected and serum levels of serum amyloid A (SAA) were measured by enzyme-linked immunosorbent assay. All subjects were scored by the Auditory Verbal Learning Test (AVLT) scale. Resting brain function images of healthy male volunteers and patients in the severe group were collected by 3.0 T magnetic resonance scanner. SPSS25.0 software was used for statistical analysis. Results The SAA of the OSAHS group ( n = 43) were higher than those of control group ( n = 19). The scores of AVLT-immediate and AVLT-delay in the severe OSAHS group were lower than those in the control group ( P < 0.05), and it was negatively correlated with SAA. In the severe OSAHS group, the rest state Function Connection (rsFC) in temporal lobe, marginal lobe, and frontal lobe was lower than that in the control group ( P < 0.05) and was significantly negatively correlated with SAA. The rsFC in bilateral parietal lobes was higher than that in the control group ( P < 0.05), was significantly positively correlated with SAA, and was negatively correlated with AVLT-delay. Conclusions The significant increase in SAA concentration in patients with OSAHS correlated with brain rsFC intensity, providing a reference role for the diagnosis, treatment, and prognosis of cognitive dysfunction in patients with OSAHS.

Abstract:Introduction: Sleep disturbances including obstructive sleep apnea (OSA) and poor sleep quality are common after stroke, while its association with cognitive changes following transient ischemic attack (TIA) or mild stroke remains unclear. We aim to determine whether sleep duration, OSA parameters, or nocturnal hypoxemia is associated with a greater cognitive decline after stroke. Methods: We prospectively followed-up patients with acute TIA/mild stroke [National Institute Health Stroke Scale (NIHSS) < 7] who underwent baseline sleep questionnaire [Pittsburgh Sleep Quality Index (PSQI)], and serial cognitive assessments [Montreal Cognitive Assessment (MoCA) 5-min, Stroop Test] at baseline and one-year. We also evaluated apnea-hypopnea index (AHI) and nocturnal hypoxemia by Home Sleep Apnea Test (HSAT) at one-year. Primary outcome was one-year change in MoCA 5-min score. Results: One hundred and five patients with TIA/mild stroke (mean age 63 years, 65 % male) were included. Baseline short sleep (< 6 hour/night) and AHI ≥ 20/hour at one-year were independently associated with a decline in the MoCA 5-min total score after covariates adjustment [short sleep: β = -2.36 95 % confidence interval (CI) (-4.13, -0.59), p = 0.009; AHI ≥ 20/hour: β = -1.79 (-3.26, -0.32), p = 0.017; remained significant after multiple comparisons correction]. A lower mean MinSpO2 was associated with a decline in executive function [Stroop interference index: β = 0.29 (0.04, 0.53), p = 0.021], but not with MoCA 5-min score at one-year. Moderation analysis indicated AHI ≥ 20/hour was associated with a pronounced decline in executive function only in men. Conclusions: Short sleep after stroke onset, AHI ≥ 20/hour and nocturnal hypoxemia at one-year contributed to an impaired cognitive trajectory at one-year following stroke in patients with TIA/mild stroke.

Abstract:Abstract Introduction Glymphatic cerebral waste-clearance system is activated during sleep, and obstructive sleep apnea (OSA) might have impact on the long-term disruption of the glymphatic system function and the development of neurodegenerative diseases. This study investigated the longitudinal association of OSA with changes in glymphatic system function and cognitive function domains. Methods Based on a community-based prospective cohort, participants who had both baseline and 4-year follow-up polysomnography, brain MRI with diffusion tensor imaging (DTI), and comprehensive cognitive assessment data were included. At both baseline and follow-up, participants were categorized as non-OSA (apnea-hypopnea index, AHI < 5), mild OSA (5≤AHI< 15), and moderate-to-severe OSA (AHI≥15) groups. According to the longitudinal change in the OSA severity, participants were categorized as OSA-free (non-OSA at both baseline and follow-up), improved OSA, stationary OSA, and progressed OSA groups. Glymphatic system function was measured using DTI analysis along the perivascular space (DTI-ALPS) index. Results 1,110 participants (mean [SD] age, 58.0 [6.0] years; 517 [46.6%] men) were included in the final analysis. At baseline, 621 participants were categorized as non-OSA, 338 participants as mild OSA, and remaining 151 participants as moderate-to-severe OSA. At follow-up, 458 participants were categorized as OSA-free, 114 participants as improved OSA, 303 participants as stationary OSA, and 235 participants as progressed OSA. At baseline, non-OSA group exhibited higher DTI-ALPS index compared to the remaining groups (both, P< 0.001) and AHI was inversely correlated with DTI-ALPS index (R=-0.169, P< 0.001). At follow-up, the change in the DTI-ALPS index (∆DTI-ALPS) was the highest in the improved OSA group, followed by the OSA free, stationary OSA, and progressed OSA groups. ∆DTI-ALPS over 4-year was inversely correlated with ∆AHI (R=-0.171, P< 0.001 for the entire study population and R=-0.218, P< 0.001 for the population excluding OSA-free subgroup). ∆DTI-ALPS over 4-year was correlated with the changes in the visual reproduction–immediate recall (R=0.233), delayed recall (R=0.178), and recognition (R=0.188) scores (all, P< 0.001). Conclusion The changes in the OSA status has impact on the longitudinal change in the glymphatic system function and this effect might be reversible. Sustained proper management of OSA might be important in the effective prevention and management of cognitive decline. Support (if any)

Abstract:Abstract Introduction Sleep disturbance from obstructive sleep apnea (OSA) impairs overnight memory processing which may be mediated by changes to slow wave sleep. Indeed, reduced region-specific EEG slow activity during non-REM sleep has been associated with worse overnight declarative, motor and spatial memory. Using a polysomnologically (PSG)-verified model of SWS-specific CPAP-withdrawal to create three conditions of 1) stable-SWS on CPAP, 2) SWS-fragmentation with intermittent hypoxemia (OSAsws), and 3) SWS-fragmentation with reduced hypoxemia (OSAsws+O2), we investigated whether CPAP withdrawal during SWS induces acute changes in EEG slow wave activity that is associated with overnight spatial navigational memory performance. Methods 19 channel EEG, re-referenced to average mastoids from 72 PSGs in 24 CPAP-adherent patients (7 female, average age 56yrs) with moderate-to-severe OSA was inspected visually for artifact removal prior to analysis using an automatic algorithm (DETOKS) for detection of relative slow wave (0.5-4Hz, SWA) and slow oscillation (0.6-1Hz, SOA) activity. During each PSG visit, participants explored one of three 3D maze environments, performing 3 timed trials before and after each of 3 randomized and counterbalanced SWS disruption conditions, 1)CPAP, 2)OSASWS & 3)OSASWS+O2. Frontal (Fz), central (Cz) and parietal (Pz) relative SWA, SOA and overnight change in maze completion times were compared according to PSG condition using Friedman Rank Sum and Conover’s tests. The relationship between SWA and maze performance was tested using Pearson’s correlation coefficient. Results No significant differences were observed between the three conditions for non-REM EEG slow activities or overnight change in maze completion times. In the CPAP condition only, greater EEG slow activity at Pz (SWA, r=0.57, p=0.01 and SOA, r=0.67, p=0.002) was associated with larger improvements in overnight maze completion times. Conclusion EEG slow wave activity and maze performance were not influenced by the recapitulation of OSA during SWS in CPAP-treated individuals. There was, however, a positive relationship between parietal-specific EEG slow oscillation activity and overnight maze performance during stable CPAP-treated slow wave sleep. These observations suggest that OSA of sufficient severity to impact SWA or SOA may negatively impact spatial navigational memory, but the selective withdrawal of CPAP in SWS only was not sufficient to do so. Abstract:STUDY OBJECTIVES: To investigate changes in regional cerebral blood flow (rCBF) in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS).DESIGN: We compared the 99mTc-ethylcysteinate dimer (ECD) single photon emission computed tomography (SPECT) images of patients with OSAHS with those of age- and sex-matched healthy volunteers.SETTING: University hospital.PATIENTS AND PARTICIPANTS: Twenty-seven patients with severe OSAHS and 27 healthy volunteers underwent 99mTc-ECD brain SPECT studies.INTERVENTION: For statistical parametric mapping analysis, all SPECT images were spatially normalized to the standard SPECT template and then smoothed using a 14-mm full-width at half-maximum Gaussian kernel. The Student t test was used for the statistical analysis.MEASUREMENTS AND RESULTS: The mean age of patients and subjects was 44.3 years (range 31-58). All patients underwent overnight polysomnography. The mean apnea-hypopnea index of patients was 60.4 +/- 17.6 per hour (range 33 -104), indicating severe OSAHS. All patients snored heavily and had daytime sleepiness (mean Epworth Sleepiness Scale score, 10.7 +/- 3.7, range 6-12). Statistical parametric mapping analysis showed that rCBF in patients with OSAHS was significantly reduced in bilateral parahippocampal gyri and in the right lingual gyrus, as compared with that of healthy volunteers (P < 0.05 with false discovery rate correction). Moreover, apnea-hypopnea indexes of patients were negatively correlated with rCBF in the right pericentral gyrus and right cuneus at uncorrected P < 0.001.CONCLUSIONS: Our results show the altered rCBF pattern in bilateral parahippocampal gyri, right lingual gyrus, pericentral gyrus, and cuneus in patients with severe OSAHS. These findings may partly explain the deficit in memory, spatial learning, executive function, and attention, which are frequently found in patients with OSAHS.

Abstract:To the Editor : Obstructive sleep apnea (OSA) can cause cognitive impairment, affect the quality of life of patients, and increase the burden on families and society. It has attracted the attention of more and more researchers, and there are also studies that point out the cognitive function of OSA. Most of them are mainly mild damage, and the specific mechanism is still unclear (1).With interest, we read the paper by Djonlagic and colleagues (2), which indicates that 3 months of nocturnal continuous positive airway pressure (CPAP) treatment can increase N3 sleep proportionally to the level of the healthy control group, thereby alleviating the declarative memory deficits of patients with OSA. From the data in the study, it can be seen that apnea–hypopnea index has dropped from 34.7 ± 7.5 to 3.6 ± 0.7 events/h, which reduces arousal and increases the time of the deep sleep N3 phase (mainly slow-wave sleep). The increase of slow-wave sleep helps the recovery of the key memory system in the brain.

Abstract:Abstract Introduction Impairments in the microstructure of the limbic tracts are seen in early stages of the Alzheimer’s disease (AD) spectrum. Disrupted sleep has been linked with AD. We examined associations between measures of limbic white matter tracts and objective sleep parameters in cognitively unimpaired older adults. Methods This cross-sectional study included 170 community-dwelling cognitively unimpaired older adults (mean±SD: age=67.2±5.2y) participating in NYU studies of sleep, aging, and memory. Subjects completed polysomnography (NPSG) and brain MRI. Sleep measures of interest included total sleep time (TST), NREM stages 2, 3 and REM sleep duration, sleep efficiency and fragmentation, and AHI4% and REM AHI4%. Microstructural properties of the cingulum, uncinate fasciculus (UF), and fornix were estimated using diffusional tensor imaging (DTI) metrics including radial kurtosis (RK), radial diffusivity (RD), and fractional anisotropy (FA). Linear mixed-effects regression models examined associations between sleep variables and DTI metrics. Models were adjusted for age, race, education, sex, time between NPSG and MRI. Results Participants were 71.2% female, 44.1% Black/African-American, and had 16.8±2.5y of education. In the right cingulum, higher AHI4% in REM was associated with decreased RK(β[IV]= β[SE}; β[AHI4%] =-0.0014[0.00054], p=0.01) and shorter stage 2 sleep duration was associated with decreased RK (β [stage 2 sleep duration] =0.00062[0.00026], p=0.02). In both hemispheres of the cingulum, increased sleep fragmentation in stage 3 was associated with increased RD (β [sleep fragmentation, right] = 0.0017[0.00080], p=0.03; β [sleep fragmentation, left] = 0.0026[0.00087], p=0.003), and higher AHI4% in REM was associated with reduced FA (β [AHI4% left] =-0.00039[0.00019], p=0.048; β [AHI4% right] =-0.00055[0.00017], p=0.02). In the right UF, with increasing age, reduced TST was associated with lower RK(β[TST]=0.0000047[0.000002], p=0.02). In the right UF, reduced time in REM sleep was associated with increased RD (β [REM duration] =-0.0022[0.00078], p=0.006). In the fornix, higher AHI4% was associated with lower FA(β[AHI4%] =-0.00061[0.00029], p=0.04). Conclusion In this sample of community-dwelling cognitively unimpaired older adults, reduced sleep duration and OSA severity (NREM and REM) were associated with limbic white matter tracts alterations. Longitudinal studies are needed to investigate the role of disrupted sleep in limbic white matter microstructure alterations in early stages of the AD spectrum.
