Abstract:We compared the incidence of bacterial pneumonia among 336 patients who discontinued trimethoprim-sulfamethoxazole (TMP-SMX) as prophylaxis against Pneumocystis carinii pneumonia (PCP) with that among 75 patients who fulfilled the criteria for discontinuation but continued receiving prophylaxis. The difference in the overall incidence rates for the 2 groups (1.2 events per 100 person-years) was not statistically significant. Discontinuation of TMP-SMX prophylaxis against PCP is not associated with a significant increase in the incidence of bacterial pneumonia among patients with a sustained CD4 cell count increase to >200 cells/microL.
Low serum levels of bone turnover markers are associated with the presence and severity of diabetic retinopathy in patients with type 2 diabetes mellitus

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Accumulating evidence demonstrates an association of type 2 diabetes and its microvascular complications with increased fracture risk. In this study, we aimed to evaluate the relationships between serum concentrations of bone turnover markers and the presence and/or severity of diabetic retinopathy (DR) among type 2 diabetic patients.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>A total of 285 type 2 diabetic patients comprising 168 patients without DR and 117 patients with DR were enrolled in the cross‐sectional study. In the latter group, patients were further divided into patients of mild and severe DR stages. The biochemical parameters and bone turnover markers were determined in all participants.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>This paper found that serum levels of procollagen type 1 N‐terminal propeptide (P1NP), a bone formation marker, and a bone resorption marker serum β‐crossLaps (β‐CTX) were decreased in diabetic patients with DR than those without DR, with differences remained significant (<i>P </i> < 0.05) in multivariate linear regression models after adjustments for multiple confounding factors. Osteocalcin and β‐CTX levels were further reduced along with the severity of DR among participants with DR. Moreover, multivariate logistic regression analysis revealed that lower serum levels of P1NP and β‐CTX were associated with higher odds for the presence of DR while β‐CTX was with the severity of DR. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>Our results suggested the development of DR might be involved in the progression in T2DM induced deficits in bone formation and resorption or vice versa. Follow‐up study and further research are necessary to validate the associations and elucidate the underlying mechanisms.</p></section>

Low Bone Turnover Markers in Young and Middle-Aged Male Patients with Type 2 Diabetes Mellitus

Abstract:Accumulating evidence has supported an increased risk of osteoporotic fracture in postmenopausal women and elderly men diagnosed with diabetes mellitus. However, it is not uncommon for young and middle-aged male patients diagnosed with type 2 diabetes mellitus (T2DM) to suffer from osteopenia or osteoporosis. Few studies focused on this population group are available. The aim of this study is to evaluate bone metabolic status and investigate the influence of T2DM on bone metabolism in 30-50-year-old men. Anthropometric assessment and blood samples were obtained from 160 patients with T2DM and 69 nondiabetic volunteers. Serum parathyroid hormone (PTH) and bone turnover markers (BTMs), including serum procollagen type I N-terminal peptide (PINP), osteocalcin (OC), and β-cross-linked C-telopeptide of type I collagen (β-CTX), were analysed. No significant differences were observed based on age, body mass index, systolic blood pressure, serum calcium, phosphorus, creatinine, total protein, and albumin levels when comparing T2DM and control groups. Fasting blood glucose, HbA1c, triglyceride (TG), total cholesterol, and low-density lipoprotein cholesterol were significantly increased, while high-density lipoprotein cholesterol was significantly decreased in the T2DM group. Compared with controls, diabetic patients showed lower serum PINP, OC, and PTH levels, whereas serum β-CTX levels were similar between the two groups. Moreover, HbA1c levels were positively correlated with PINP and inversely associated with PTH levels. TG levels were negatively correlated with OC or β-CTX levels. Furthermore, multiple linear regression revealed a positive correlation between HbA1c and PINP levels. These results also revealed a negative association between HbA1c and PTH, and between TG and OC levels, even after adjusting for expected confounder factors. Collectively, these findings indicated that young and middle-aged male patients with T2DM showed a lower turnover state resulting from bone formation inhibition. Glucose and lipid metabolic disorders may affect bone formation through different pathways.
THE INDEPENDENT EFFECTS OF TYPE 2 DIABETES MELLITUS AND CHRONIC KIDNEY DISEASE ON BONE TURNOVER MARKERS

Abstract:Background: Chronic kidney disease (CKD) and type 2 diabetes mellitus (T2DM) are associated with disturbed mineral homeostasis and serum bone biomarkers. The interplay between T2DM and CKD on serum bone turnover markers (BTM) is unclear. Our aim was to describe the BTM in patients with T2DM, CKD or both. Methods: In this observational, single-centre, prospective study, we included 320 patients over 40 years, divided into four groups: T2DM and normal kidney function (n=142), T2DM and CKD (n=36), CKD and normal glucose metabolism (n=29) and healthy controls (n=113). We excluded patients treated for osteoporosis and with secondary osteoporosis. Patients were compared by age, levels of glycated hemoglobin, PTH, alkaline phosphatase, osteocalcin (OC), CTx and 25 OH vitamin D. Results: Univariate analysis showed that GFR correlated significantly with PTH (r=0.37), OC (r=0.43) and CTX (r=0.45) in the diabetes group but only with PTH (r=0.34) in the non-T2DM group. Multivariate analysis showed that GFR remained significantly correlated with the same bone markers even after adjustment for age, sex or 25(OH)D levels. Diabetics seem to have lower levels of alkaline phosphatase (68±22.1 U/L) and CTX (0.37±0.24 ng/mL) than those without diabetes (76.7±29.6. U/L and 0.5±0.19 ng/mL, respectively). There was no correlation between BTM and glycated hemoglobin. Conclusions: Bone turnover markers correlate with GFR, particularly in patients with T2DM. However, alkaline phosphatase is lower in T2DM than in non-T2DM.
Relationships of Serum Bone Turnover Markers With Metabolic Syndrome Components and Carotid Atherosclerosis in Patients With Type 2 Diabetes Mellitus

Abstract:Objective This study aimed to investigate the association of serum bone turnover markers (BTMs) with metabolic syndrome components and carotid atherosclerosis in patients with type 2 diabetes mellitus (T2DM). Methods We performed a cross-sectional based study in T2DM populations. Serum BTMs including N-terminal osteocalcin (N-MID), β-cross-linked C-telopeptide of type I collagen (β-CTX), and procollagen type I N-terminal propeptide (PINP) were measured by immunoassay method. Carotid artery intima-media thickness and carotid artery plaque (CAP) were measured by B-mode ultrasound. Results The serum N-MID, PINP, and β-CTX levels significantly lower in the CAP group compared with the non-CAP group. N-MID and PINP levels were inversely associated with fasting blood glucose, HOMA-IR, CRP, eGFR, and triglycerides (all P < 0.05), whereas β-CTX levels were negatively associated with triglycerides (P < 0.05). After multiple adjustment, the odds ratios (ORs) were substantially higher for CAP with decreased N-MID level (OR = 0.958; 95% CI = 0.926–0.991; P = 0.013). However, serum levels of PINP and β-CTX were not associated with the presence of CAP. Multivariate logistic regression analysis further revealed that serum N-MID, PINP, and β-CTX levels were significantly associated with hypertriglyceridemia, whereas serum N-MID and β-CTX levels were associated with overweight/obesity risk. Conclusions These findings indicated that serum N-MID level was an independent risk factor for carotid atherosclerosis, whereas BTM levels were associated with other metabolic syndrome components in a T2DM population.

Abstract:<p>Recent advances indicate that bone and energy metabolism are closely related. However, little direct evidence on causality has been provided in humans. We aimed to assess the association of three bone‐related biomarkers 25 hydroxyvitamin D (25OHD), parathyroid hormone (PTH) and osteocalcin (OCN) with several metabolic phenotypes, and investigate any causal relevance to the associations using a Mendelian randomization (MR) study. Serum 25OHD, PTH and total OCN were measured at baseline in 5169 eligible Chinese participants in Changfeng study. Partial correlation and bivariate GREML analysis were used to estimate phenotypic and genetic correlations, respectively. Multiple linear regression and logistic regression were used to assess linear associations. Genome‐wide association analysis (GWAS) was performed. Bidirectional two sample MR analyses were conducted to examine causal relationships between OCN and body mass index (BMI), diastolic blood pressure (DBP), triglyceride (TG), high‐density lipoprotein cholesterol (HDL‐C), glycated hemoglobin A1c (HbA1c) and type 2 diabetes (T2DM), using our GWAS result of OCN and GWAS statistics from Biobank Japan project (BBJ) and the largest meta‐analysis of T2DM GWAS in East Asian population. Circulating OCN was significantly associated with higher DBP and HDL‐C, and decreased TG, blood glucose level, insulin resistance, liver fat content, bone mineral density, BMI, and a favorable body fat distribution pattern. GWAS identified one novel serum PTH locus and two novel serum OCN loci, explaining 0.81% and 1.98% of variances of PTH and OCN levels, respectively. MR analysis suggested a causal effect of T2DM on lower circulating OCN concentration (causal effect: ‐0.03; ‐0.05 to ‐0.01; p=0.006 for T2DM_BBJ and ‐0.03; ‐0.05 to ‐0.01; p=0.001 for T2DM_EAS). These findings indicate that T2DM might impact bone remodeling and provide a resource for understanding complex relationships between osteocalcin and metabolic (and related) traits in humans.</p>

Association between bone turnover markers, bone mineral density, and serum osteoglycine in middle-aged men with Type 2 Diabetes mellitus

Abstract:Background: Patients with Type 2 diabetes mellitus (T2DM) have decreased bone health. We aimed to investigate serum levels of bone turnover markers (BTMs) (markers of bone formation and bone resorption) and bone mineral density (BMD) at three sites (lumber, neck femur, and total femur) in middle-aged men with type 2 diabetes and to analyze the relationship between them. Also to evaluate serum osteoglycin as a novel marker and its relation to BTMs, BMD, and diabetic status. Methods: We recruited seventy-eight patients with T2DM and thirteen non-diabetic, male volunteers as a control group. BMD was measured using a DEXA scan. BTMs (carboxy-terminal crosslinking telopeptide of type 1 collagen [CTX] and procollagen type 1 N propeptide [P1NP]), osteoglycin, PTH, and vitamin D were estimated. Data was compared among subjects and statistical analysis was performed. Results: Most of the patients were having normal BMD with no significant difference between patients and the controls. BTMs and osteoglycin were significantly higher and vitamin D was significantly lower in the diabetic patients. Serum osteoglycin was positively correlated with DEXA Neck Femur (r = 0.233; p-value < 0.05). Conclusion: Body mass index and Serum osteoglycin have a significant positive effect on BMD. Both markers of bone formation and bone resorption were increased indicating a state of increased bone turnover in T2DM.
Diabetes Mellitus Type 2 Patients with Abdominal Obesity Are Prone to Osteodysfunction: A Cross-Sectional Study

Abstract:Introduction. The interaction between diabetes, obesity, and bone metabolism was drawing increasing public attention. However, the osteometabolic changes in diabetes mellitus type 2 (T2DM) patients with abdominal obesity have not been fully revealed. This study is aimed at investigating the association between abdominal obesity indices and bone turnover markers among T2DM participants. Methods. 4351 subjects were involved in the METAL study. Abdominal obesity indices included neck, waist, and hip circumference, visceral adiposity index (VAI), lipid accumulation product (LAP), waist-to-hip ratio (WHR), and Chinese visceral adiposity index (CVAI). They were applied to elucidate the nexus between β-C-terminal telopeptide (β-CTX), osteocalcin (OC), and intact N-terminal propeptide of type I collagen (P1NP). Results. Abdominal obesity indices were strongly negatively associated with β-CTX and OC. Among males, five indices were negatively correlated with β-CTX (BMI, WC, LAP, WHR, and CVAI) and OC (BMI, NC, WC, WHR, and CVAI). There were no significant associations with P1NP. Among females, all eight indices were negatively associated with β-CTX. Seven indices were negatively related to OC (BMI, NC, WC, HC, LAP, WHR, and CVAI). The VAI was negatively correlated with P1NP. Conclusions. The present study demonstrated that in T2DM, abdominal obesity had an obviously negative correlation with bone metabolism. Abdominal obesity indices were significantly negatively associated with skeletal destruction (β-CTX) and formation (OC). In routine clinical practice, these easily obtained indices could be used as a preliminary screening method and relevant factors for osteodysfunction incidence risk at no additional cost and may be of particular value for postmenopausal women in T2DM populations.

Abstract:Objective: This meta-analysis aims to examine differences in biochemical markers of bone metabolism between individuals with type 2 diabetes (T2DM) and non-T2DM control groups. Materials and methods: Two independent evaluators searched five databases: PubMed, EMBASE, EBSCOhost, Web of Science, and the Cochrane Library. We aimed to identify observational studies investigating the impact of T2DM on biochemical markers of bone metabolism. Literature retrieval covered the period from the establishment of the databases up to November 2022. Studies were included if they assessed differences in biochemical markers of bone metabolism between T2DM patients and non-T2DM control groups using cross-sectional, cohort, or case-control study designs. Results: Fourteen studies were included in the analysis, comprising 12 cross-sectional studies and 2 cohort studies. Compared to the non-T2DM control group, T2DM patients showed reduced levels of Osteocalcin and P1NP, which are markers of bone formation. Conversely, levels of Alkaline phosphatase and Bone-specific alkaline phosphatase, other bone formation markers, increased. The bone resorption marker CTX showed decreased levels, while TRACP showed no significant difference. Conclusion: In individuals with T2DM, most bone turnover markers indicated a reduced rate of bone turnover. This reduction can lead to increased bone fragility despite higher bone mineral density, potentially increasing the risk of osteoporosis. Abstract:The global incidence of Type 2 diabetes (T2D) is on the rise, fueled by factors such as obesity, sedentary lifestyles, socio-economic factors, and ethnic backgrounds. T2D is a multifaceted condition often associated with various health complications, including adverse effects on bone health. This study aims to assess key biomarkers linked to bone health and remodeling—Osteoprotegerin (OPG), Receptor Activator of Nuclear Factor Kappa-Β Ligand (RANKL), and Glycoprotein Non-Metastatic Melanoma Protein B (GPNMB)—among individuals with diabetes while exploring the impact of ethnicity on these biomarkers. A cross-sectional analysis was conducted on a cohort of 2083 individuals from diverse ethnic backgrounds residing in Kuwait. The results indicate significantly elevated levels of these markers in individuals with T2D compared to non-diabetic counterparts, with OPG at 826.47 (405.8) pg/mL, RANKL at 9.25 (17.3) pg/mL, and GPNMB at 21.44 (7) ng/mL versus 653.75 (231.7) pg/mL, 0.21 (9.94) pg/mL, and 18.65 (5) ng/mL in non-diabetic individuals, respectively. Notably, this elevation was consistent across Arab and Asian populations, except for lower levels of RANKL observed in Arabs with T2D. Furthermore, a positive and significant correlation between OPG and GPNMB was observed regardless of ethnicity or diabetes status, with the strongest correlation (r = 0.473, p < 0.001) found among Arab individuals with T2D. Similarly, a positive and significant correlation between GPNMB and RANKL was noted among Asian individuals with T2D (r = 0.401, p = 0.001). Interestingly, a significant inverse correlation was detected between OPG and RANKL in non-diabetic Arab individuals. These findings highlight dysregulation in bone remodeling markers among individuals with T2D and emphasize the importance of considering ethnic variations in T2D-related complications. The performance of further studies is warranted to understand the underlying mechanisms and develop interventions based on ethnicity for personalized treatment approaches.

Association of Insulin Resistance and β-cell Function With Bone Turnover Biomarkers in Dysglycemia Patients

Abstract:Background The interrelation between glucose and bone metabolism is complex and has not been fully revealed. This study aimed to investigate the association between insulin resistance, β-cell function and bone turnover biomarker levels among participants with abnormal glycometabolism. Methods A total of 5277 subjects were involved through a cross-sectional study (METAL study, http://www.chictr.org.cn , ChiCTR1800017573) in Shanghai, China. Homeostasis model assessment of insulin resistance (HOMA-IR) and β-cell dysfunction (HOMA-%β) were applied to elucidate the nexus between β-C-terminal telopeptide (β-CTX), intact N-terminal propeptide of type I collagen (P1NP) and osteocalcin (OC). β-CTX, OC and P1NP were detected by chemiluminescence. Results HOMA-IR was negatively associated with β-CTX, P1NP and OC (regression coefficient (β) -0.044 (-0.053, -0.035), Q4vsQ1; β -7.340 (-9.130, -5.550), Q4vsQ1 and β -2.885 (-3.357, -2.412), Q4vsQ1, respectively, all P for trend <0.001). HOMA-%β was positively associated with β-CTX, P1NP and OC (β 0.022 (0.014, 0.031), Q4vsQ1; β 6.951 (5.300, 8.602), Q4vsQ1 and β 1.361 (0.921, 1.800), Q4vsQ1, respectively, all P for trend <0.001). Conclusions Our results support that lower bone turnover biomarker (β-CTX, P1NP and OC) levels were associated with a combination of higher prevalence of insulin resistance and worse β-cell function among dysglycemia patients. It is feasible to detect bone turnover in diabetes or hyperglycemia patients to predict the risk of osteoporosis and fracture, relieve patients’ pain and reduce the expenses of long-term cure.

Positive association of glucagon with bone turnover markers in type 2 diabetes: A cross-sectional study

Abstract:Aims: The osteo-metabolic changes in type 2 diabetes (T2D) patients are intricate and have not been fully revealed. It is not clear whether glucagon is entirely harmful in the pathogenesis of diabetes or a possible endocrine counter-regulation mechanism to reverse some abnormal bone metabolism. This study aimed to investigate the association between glucagon and bone turnover markers (BTMs) in T2D patients. Methods: A total of 3984 T2D participants were involved in a cross-sectional study in Shanghai, China. Serum glucagon was measured to elucidate its associations with intact N-terminal propeptide of type I collagen (P1NP), osteocalcin (OC), and β-C-terminal telopeptide (β-CTX). Glucagon was detected with a radioimmunoassay. Propeptide of type I collagen, OC, and β-CTX were detected using chemiluminescence. The diagnosis of T2D was based on American Diabetes Association criteria. Results: The concentration of glucagon was positively correlated with two BTMs [OC-β: 0.034, 95% CI: 0.004, 0.051, p = 0.024; CTX-β: 0.035, 95% CI: 0.004, 0.062, p = 0.024]. The result of P1NP was [P1NP-regression coefficient (β): 0.027, 95% CI: -0.003, 0.049, p = 0.083]. In the glucagon tertiles, P for trend of the BTMs is [P1NP: 0.031; OC: 0.038; CTX: 0.020], respectively. Conclusions: Glucagon had a positive effect on bone metabolism. The concentrations of the three BTMs increased as glucagon concentrations rose. This implied that glucagon might speed up skeletal remodelling, accelerate osteogenesis, and promote the formation of mature bone tissue. At the same time, the osteoclastic process was also accelerated, providing raw materials for osteogenesis to preserve the dynamic balance. In view of the successful use of single-molecule as well as dual/triple agonists, it would be feasible to develop a preparation that would reduce osteoporosis in diabetic patients.

Abstract:Objective. This study is aimed at analyzing the association between bone metabolism indices and diabetic retinopathy (DR) in elderly patients with type 2 diabetes mellitus. Methods. Data of 352 men and 284 postmenopausal women, aged more than 50 years, with type 2 diabetes mellitus were retrospectively analyzed. Patients were divided into three groups based on the degree of DR: nondiabetic retinopathy (NDR) group, background diabetic retinopathy (BDR) group, and proliferative diabetic retinopathy (PDR) group. Results. (1) The diabetic duration and urinary albumin to creatinine ratio (UACR) were significantly higher in the PDR and BDR groups than in the NDR group ( P < 0.05 ). The level of beta-C-terminal telopeptide (β-CTX) in male patients was lower in the PDR and BDR groups than in the NDR group ( P < 0.05 ). In addition, the level of procollagen 1 intact N-terminal (P1NP) in female patients was higher in the PDR and BDR groups than in the NDR group ( P < 0.05 ). (2) For men and postmenopausal women, the proportion of vitamin deficiency was higher in the PDR and BDR groups than in the NDR group ( P < 0.05 ). (3) The logistic regression analysis in men and postmenopausal women showed that the diabetic duration and lower levels of UACR and 25(OH)D were independent risk factors for DR ( P < 0.05 ). (4) The diabetic duration was also an independent risk factor for PDR ( P < 0.05 ); however, no independent correlation was found between the level of 25(OH)D and PDR ( P > 0.05 ). Conclusions. A close association was observed between 25(OH)D level and DR in the elderly male patients and postmenopausal women with type 2 diabetes mellitus. P1NP and β-CTX levels might be closely related to DR in elderly male patients and postmenopausal women with type 2 diabetes mellitus.

Abstract:Abstract Type 1 diabetes mellitus (T1DM) has been linked to increased osteocyte apoptosis, local accumulation of mineralised lacunar spaces and microdamage suggesting an impairment of the mechanoregulation network in affected individuals. Diabetic neuropathy might exacerbate this dysfunction through direct effects on bone turnover, and indirect effects on balance, muscle strength, and gait. However, the in vivo effects of impaired bone mechanoregulation on bone remodelling in humans remain underexplored. This longitudinal cohort study assessed consenting participants with T1DM and varying degree of distal symmetric sensorimotor polyneuropathy (T1DM, n = 20, median age 46.5 years, 8 female) and controls (n = 9, median age 59.0 years, 4 female) at baseline and 4-year follow-up. Nerve conduction in participants with T1DM was tested using DPNCheck and bone remodelling was quantified with longitudinal high-resolution peripheral quantitative computed tomography (HR-pQCT, 82 μm) at the standard distal sites. Local trabecular bone formation (Tb.F) and resorption (Tb.R) sites were captured by implementing 3D rigid image registration of HR-pQCT images, and the mechanical environment across the bone microarchitecture at these sites was simulated using micro-finite element analysis. We calculated odds ratios to determine the likelihood of bone formation (ORF) and resorption (ORR) with increasing/decreasing strain in percent as markers for mechanoregulation. At the distal radius, Tb.F was 47% lower and Tb.R 59% lower in T1DM participants compared to controls (p < 0.05). Tb.F correlated positively with nerve conduction amplitude (R = 0.69, p < 0.05) in participants with T1DM and negatively with glycated haemoglobin (HbA1c) (R = -0.45, p < 0.05). Additionally, ORF was 34% lower and ORR was 18% lower in T1DM compared to controls (p < 0.05). Our findings represent in vivo evidence suggesting that bone remodelling in individuals with T1DM is in a state of low responsiveness to mechanical stimuli, resulting in impaired bone formation and resorption rates; these correlate to the degree of neuropathy and level of diabetes control.

Abstract:We analyzed prevalence rates of syphilis (positive Treponema pallidum hemagglutinin antigen/T. pallidum particle antigen and venereal disease research laboratory test) among police officers in Guinea-Bissau from 1990 to 2010 and found a significant decline from 4.5% to 0.4% (P = 0.0065). Our results are in line with other recent reports from West Africa. More research is needed to identify the reasons for this decline.
Diabetic Microvascular Disease is Associated with a Decreased Bone Mineral Density at Hip in Type 2 Diabetes Mellitus Patients in China: A Retrospective Single Center Study

Abstract:Backgroud: Diabetic microvascular disease(MVD) is thought to be deleterious on bone. We aimed to examine the link between the presence or number of MVD and bone mineral density (BMD) in a large cohort of Chinese patients with type 2 diabetes mellitus (T2DM).Methods: In this cross-sectional study of 10,924 patients with T2DM aged ≥50 years (5857 women), BMD was measured by dual-energy X-ray absorptiometry (DEXA). Indicators of diabetic MVD measured: diabetic retinopathy(DR), kidney disease (DKD), and peripheral neuropathy (DPN).Findings: Overall prevalence of osteopenia and osteoporosis was 39.7% and 11.9%. Lower T-score at hip (femoral neck or total hip) was associated with female sex, higher age, lower body mass index (BMI), no metformin treatment, the presence, and number of MVD. The lower T-score and higher incidence of osteoporosis or low BMD at hip was independently associated with the presence of DR (OR = 1.457, 95%CI: 1.192 – 1.781 and OR = 1.287, 95%CI: 1.159 – 1.429, both p < 0.001), or DKD (OR = 1.297, 95%CI: 1.098 – 1.533, p =0.002 and OR = 1.234, 95%CI: 1.125 – 1.354, p <0.001), DPN (OR = 1.147, 95%CI: 1.043 – 1.261, p =0.005 for low BMD only, not for osteoporosis), and the number of MVD (OR = 1.262, 95%CI: 1.138 – 1.399, and OR = 1.175, 95%CI: 1.114 – 1.239, both p < 0.001).Interpretation: The presence and number of MVD were significantly associated with lower BMD, higher incidence of osteoporosis and low BMD in dependent of sex, age, BMI, and metformin treatment.Funding: This study was funded by the National Natural Science Foundation of China (NSFC) 81870563.Declaration of Interest: The authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest.Ethical Approval: The Institutional Review Board of the hospital approved the present study. Abstract:Objective: Emerging evidences demonstrated that chronic inflammation can influence bone metabolism in type 2 diabetes mellitus (T2DM), leading to bone homeostasis imbalance. The aim of this study was to assess the correlations between novel pro-inflammatory indexes like monocyte to high-density lipoprotein (MHR), apolipoprotein A1 (MAR) ratios and bone mineral density (BMD), bone turnover markers in Chinese postmenopausal women with T2DM. Method: In this study, a total of 619 participants with complete data were included in the final analysis. Demographic and anthropometric information was collected. Biochemical parameters and bone turnover markers were determined by standard methods. BMD was measured by dual-energy x-ray absorptiometry. Correlation analysis and regression models were conducted to assess the associations between MHR, MAR and bone turnover markers, BMD. Multiple binomial logistic regression model was used to estimate the independent variables of MHR and MAR for osteoporosis. Results: Overall, the prevalence of osteoporosis was 38.3%. MHR and MAR were significantly correlated with C-terminal cross linking of type I collagen (β-CTX), L1-L4, femoral neck BMD and T scores. These correlations remained significant after adjustment for other confounding factors. Meanwhile, MHR and MAR were also significantly associated with higher odds of osteoporosis, the odds ratios (ORs) (95%CI) were 1.88 (1.49–2.38) and 2.30 (1.72–3.09) respectively. Furthermore, MHR and MAR seemed to have a good identifying value for osteoporosis. The area under the curve of MHR and MAR identifying osteoporosis were 0.791 (95% CI: 0.753–0.828) and 0.843 (95% CI: 0.809–0.877) respectively ( p < 0.001). The optimal cut-off values of MHR and MAR were 4.53 × 10 8 /mmol (sensitivity: 60.8%, specificity: 85.9%) and 4.74 × 10 8 /g (sensitivity: 71.7%, specificity: 89.3%) respectively. Conclusion: MHR and MAR were significantly associated with osteoporosis. These two novel pro-inflammatory indexes may be ideal markers to reflect bone homeostasis imbalance caused by chronic inflammation in Chinese postmenopausal women with T2DM.

Abstract:Surgeons occasionally note a discrepancy between preoperative assessment of middle‐ear effusion and operative observation during the placement of ventilating tubes in children's ears. This study was designed to determine whether this variance is secondary to the effects of inhalation anesthesia or misinterpretation of the preoperative exam. Observations were recorded by three distinctly different methods of measurement: the surgeon, the MD‐2 Impedance Analyzer, and the Acoustic Otoscope® immediately before and after induction of anesthesia. These results were then analyzed and compared with the operative findings. Anesthetic induction was shown to cause alteration in the presence of middle‐ear fluid in less than 10% of cases.
Clinical Multivariate Analysis of Osteoporosis in Elderly Patients with Type 2 Diabetes

Abstract:Objective To investigate the influencing factors of osteoporosis in elderly patients with type 2 diabetes(T2DM).Methods 298 patients with T2DM were divided into 3 groups(according to their bone mineral density):normal bone mineral density(NOP) group,low bone mineral density(LBMD) group,osteoprosis(OP) group.Age,diabetes duration,body mass index(BMI),Cystatin C(Cys C),transcutaneous tissue oxygen tension(TcPO2),glycosylated hemoglobin(HbA1c),urinary C-peptide(U-CP) were compared among three groups.The correlation analysis was used to analyze the factors of OP.Results Compared with NOP group,patients in LBMD and OP group had older age,longer diabetic course,lower U-CP.BMI,Cys C level of patients in OP group were lower than those in NOP group,while the difference in age,BMI was statistically significant between OP and LBMD group(P0.01 or P0.05).BMD in elderly patients with T2DM was negatively correlated with age,disease course,but positively correlated with BMI and U-CP.Meanwhile,BMD was independently correlated with U-CP.Conclusions Osteoporosis in elderly patients with T2DM is related to a number of risk factors,including advanced age,low body weight,long-term history of diabetes,insulin dysfunction.
Relevant Risk Factors for Type 2 Diabetes Mellitus Complicated by Osteoporosis

Abstract:Objective To analyze the changes in bone mineral density(BMD) and relevant bioch-emical markers in patients with type 2 diabetes mellitus(T2DM) complicated with osteoporosis(OP).Methods According to BMD values,93 patients were divided into OP group(n=59) and non-OP group(n=34).The age,body mass index(BMI),fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),urinary microalbumin,serum calcium,serum phosphorus and alkali phosphatase(ALP)were compared between the two groups.The correlation was analyzed.Results There were significant differences in the age,BMI,BMD,serum calcium,serum phosphorus,ALP and urinary microalbumin between OP group and non-OP group(P0.05).The BMD was negatively correlated with the age,HbA1c,urinary microalbumin and ALP,but positively correlated with BMI in patients with T2DM.There was no correlation between BMD and FBG.Conclusion The risk factors for T2DM complicated with OP include old age,weight loss,high ALP levels and diabetic nephropathy.
