Abstract:To study the effects of dietary composition and chronic ethanol ingestion on plasma proteinase inhibitor (PI) levels in intact animals, 192 male Wistar rats were divided into 4 groups, which received a standard diet (S), a fat-rich diet (F), a protein-rich diet (P), and a carbohydrate-rich diet (C), respectively, for 12 weeks. Half of the animals in each diet group had 15% ethanol as their drinking solution (A) during this diet period, and the rest drank tap water (W). FW and CW diets caused a significant decrease in the trypsin-inhibiting capacity (TIC) of plasma in comparison with the SW group (p less than 0.05 and p less than 0.001, respectively), and chronic ethanol ingestion in combination with P and C diets decreased plasma TIC levels significantly (p less than 0.01 and p less than 0.001) when compared with the corresponding water-drinking groups. The chymotrypsin-inhibiting capacity (CIC) of plasma behaved differently: in the FW, PW, and CW groups it was significantly higher than in the SW group (p less than 0.001). Chronic ethanol ingestion did not change plasma CIC levels significantly when compared with the corresponding water-receiving groups. In conclusion, dietary intake was found to alter plasma PI levels. Changes in the protein synthesis of the liver might be responsible for these alterations.
How to Diagnose Renal Artery Stenosis Correctly Using Ultrasound? Evaluation of Results of Renal Arteries Duplex Ultrasonography Examinations.

Abstract:Aim: Renal artery duplex ultrasonography (RDU) is an effective and non-invasive screening test in diagnosing renal artery stenosis. The discordance of results in multiple RDU is common. We aim to evaluate the discordance and the reasons for discordance between diagnoses and measurements from multiple RDU examinations.Material and method: A retrospective study was performed in 64 examinations of renal arteries from 32 patients that were referred for two or more RDU examinations and renal artery digital subtraction angiography (DSA) within six months, between August 2013 and January 2016. Using DSA as gold standard, we divided the renal arteries into three groups: discordant (one diagnosis of RDU was correct and one was wrong), misdiagnosed (neither RDU diagnosis was correct) and correct (both RDU diagnoses were correct) groups.We evaluated the discordance and reasons for discordance of diagnoses and measurements from multiple RDU examinations. Results: Among 64 renal arteries included in this study, 37 renal arteries had two correct diagnoses, 19 renal arteries had two discordant diagnoses, and eight renal arteries were misdiagnosed twice by RDU. The discordance of peak systolic velocity (PSV), the ratio between PSV in the renal artery with stenosis and PSV in the aorta (RAR), and tardus-parvus waveform measurements were clearly higher in the discordant diagnoses group than in the correctly diagnosed group. The most common reason for a discordant diagnosis was failure in obtaining correct tardus-parvus waveforms of the interlobar artery (26.31%). Themost common reason for misdiagnosis was the presence of an extremely severe stenosis with an atrophic kidney (31.25%). Overall, 87.50% of patients underwent RDU examinations had correct diagnoses of stenosis or occlusion at least once (including location and degree), as confirmed by DSA.Conclusions: Our study indicates that standard operating procedures and improvements in examination technique by ultrasound doctors could reduce the discordance between multiple tests.
Intra‐ and Interobserver Variability of 4D Ultrasound Examination of the Infrarenal Aorta

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objectives</h3><p>The four‐dimensional ultrasound (4D‐US) enables imaging of the aortic segment and simultaneous determination of the wall expansion. The method shows a high spatial and temporal resolution, but its in vivo reliability is so far unknown for low‐measure values. The present study determines the intraobserver repeatability and interobserver reproducibility of 4D‐US in the atherosclerotic and non‐atherosclerotic infrarenal aorta.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>In all, 22 patients with non‐aneurysmal aorta were examined by an experienced examiner and a medical student. After registration of 4D images, both the examiners marked the aortic wall manually before the commercially implemented speckle tracking algorithm was applied. The cyclic changes of the aortic diameter and circumferential strain were determined with the help of custom‐made software. The reliability of 4D‐US was tested by the intraclass correlation coefficient (ICC).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The 4D‐US measurements showed very good reliability for the maximum aortic diameter and the circumferential strain for all patients and for the non‐atherosclerotic aortae (ICC >0.7), but low reliability for circumferential strain in calcified aortae (ICC = 0.29). The observer‐ and masking‐related variances for both maximum diameter and circumferential strain were close to zero.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Despite the low‐measured values, the high spatial and temporal resolution of the 4D‐US enables a reliable evaluation of cyclic diameter changes and circumferential strain in non‐aneurysmal aortae independent from the observer experience but with some limitations for calcified aortae. The 4D‐US opens up a new perspective with regard to noninvasive, in vivo assessment of kinematic properties of the vessel wall in the abdominal aorta.</p></section>

Reproducibility of Heart Rate Variability Revealed by Repeated Measurements during and after Hemodialysis

Abstract:Background/aims: Trajectory of heart rate variability (HRV) represents a noninvasive real-time measure of autonomous nervous system (ANS) and carries the capability of providing new insights into the hemodynamic compensation reserve during hemodialysis (HD). However, studies on HRV reproducibility during HD are scarce and did not refer to different reading periods. In this observational study, we aimed to establish the best suited and most reliable and reproducible HRV index in routine HD treatments including different reading rates. Methods: HRV was characterized by standardized mathematical variation expressions of R/R' intervals: SD of all R/R' intervals (ms), square root of the root mean square of the sum of all differences between adjacent R/R' intervals (ms), percentage of consecutive R/R' intervals that differ by >50 ms (%), low-frequency spectral analysis HRV (LF, expressing sympathetic activity), and high-frequency HRV (HF, expressing parasympathetic activity). To compare robustness of these HRV indices during HD procedures, we compared HRV indices means between different HD sessions and controlled for association with clinical parameters. Results: In 72 HD treatments of 34 patients, we detected the highest reproducibility (89%) of HRV measures when analyzing the low-frequency to high-frequency (LF/HF) ratio in long-term (3 h) readings. Long-term LF/HF was able to discriminate -between patients with and without heart failure NYHA classes ≥3 (p = 0.009) and type 2 diabetes (p = 0.023). We were unable to study relationships between ANS and intradialytic complications because they did not appear in our cohort. Short-term readings of HRV indices did not show any significance of pattern change during HD. Conclusion: In summary, our data provide evidence for high robustness of long-term LF/HF in analyzing HRV in HD patients using future automated monitoring systems. For short-term analysis, mathematical real-time analysis must evolve.
Posttransplant renal monitoring with B-flow ultrasonography

Abstract:B-flow ultrasonography (BFU) is a new technology to detect blood circulation. It has been developed by using digitally encoded sonography, showing higher spatial and temporal resolution than Doppler imaging because of the clearer definition of the vessel lumen. Our study was performed on 55 patients, including 37 men (67.3%) and 18 women (32.7%), of overall mean age of 45.8 years (range = 16-60 who underwent kidney transplantation from deceased donors. The patients underwent an ultrasonography check postoperatively with successive daily controls to compare evaluations with both techniques: the combination of color and power Doppler versus BFU. In 25 patients (45.5%), visualization of cortical blood flow was clearer than with the standard techniques. The parameters of intrarenal circulation were easy to measure. In conclusion, BFU was effective to visualize hemodynamic flow and to detect stenotic lesions in the renal artery. Combined with the conventional B-mode technique, BFU seems to be useful in the evaluation of renal anasthamotic stenosis, especially among patients with vascular diseases.
Doppler ultrasound in the assessment of renal perfusion before and during continuous kidney replacement therapy in the pediatric intensive care unit

Abstract:Background: This study aimed to assess observer variability and describe renal resistive index (RRI) and pulsatility index (PI) before and after onset of continuous kidney replacement therapy (CKRT). A secondary objective was to correlate Doppler ultrasound findings with those from direct measurement of renal blood flow (RBF). Methods: This is a prospective observational study in hemodynamically stable Maryland piglets with and without acute kidney injury (AKI) and in hemodynamically unstable critically ill children requiring CKRT. Doppler-based RRI and PI were assessed for each subject. Measurements were made by two different operators (pediatric intensivists) before and after CKRT onset. Results: Observer variability assessment in the measurement of RRI and PI rendered a moderate correlation for both RRI (ICC 0.65, IQR 0.51-0.76) and PI (ICC 0.63, IQR 0.47-0.75). RRI and PI showed no correlation with RBF or urine output. Baseline RRI and PI were normal in control piglets [RRI 0.68 (SD 0.02), PI 1.25 (SD 0.09)] and those with AKI [RRI 0.68 (SD 0.03), PI 1.20 (SD 0.13)]. Baseline RRI and PI were elevated in critically ill children (RRI 0.85, PI 2.0). PI and RRI did not change with CKRT in any study group. Conclusions: Observer variability between inexperienced pediatric intensivists was comparable with that between senior and junior operators. Doppler-based calculations did not correlate with invasive measurements of RBF. RRI and PI were normal in hemodynamically stable piglets with and without AKI. RRI and PI were high in hemodynamically unstable patients requiring CKRT. RRI and PI did not change after CKRT onset, despite changes in hemodynamic status. Abstract:Journal for Vascular Ultrasound, Ahead of Print. Background: Duplex ultrasound (DUS) is a well-tolerated, noninvasive imaging technique. Renal artery stenosis (RAS)—a narrowing of the arteries that supply the kidney—is involved in the cause of renal failure, and its gold-standard method of detection is digital subtraction angiography (DSA). Whether DUS is a suitable alternative to DSA or other angiographic modalities has only been reviewed up to 2005, producing mixed results with studies based on older ultrasound technology without a measurement of study quality. Purpose: To provide an up-to-date review of the diagnostic accuracy of DUS for the diagnosis of RAS in comparison with angiography. Methods: Our registered systematic review (DOI: 10.17605/OSF.IO/SE9VN) examined articles post-2005. Studies must have compared RAS diagnosis between DUS and angiography. Sources were searched in November 2022 and included Scopus, EMBASE, MEDLINE, CINAHL, and Academic Search Premier (1749 citations; final: n = 34; DSA only: n = 9). Study quality was assessed using the Quality Assessment of Diagnostic Studies 2 tool and results are presented narratively. A total of 2968 (1281 females) patients were included. Duplex ultrasound exhibited moderately high agreement with DSA and other well-established angiographic criteria with the existing literature having low risk of bias and low concern for patient applicability. Studies exhibiting low agreement were generally in smaller samples or used unique definitions of stenosis. Conclusions: Improved study reporting, consistent definitions of stenosis, and a common statistical battery are needed.
Repeatability and Reproducibility of Retinal Fractal Dimension Measured with Swept-Source Optical Coherence Tomography Angiography in Healthy Eyes: A Proof-of-Concept Study

Abstract:The retinal vascular network fractal dimension (FD) could be a promising imaging biomarker. Our objective was to evaluate its repeatability and reproducibility in healthy eyes. A cross-sectional study was undertaken with young, healthy volunteers who had no reported cardiac risk factors or ocular disease history. For each participant, three SS-OCTA images (12 × 12 mm) were acquired using the Plex Elite 9000 (Carl Zeiss Meditec AG, Jena, Germany) by two ophthalmologists. Automated segmentation was obtained from both the superficial and deep capillary plexuses. FD was estimated by box counting. The intraclass correlation coefficients (ICC) were used as measures for repeatability and reproducibility. A total of 43 eyes of healthy volunteers were included. The mean ± standard deviation (SD) age was 30 ± 6.2 years. The results show good repeatability. The ICC was 0.722 (95% CI, 0.541–0.839) in the superficial capillary plexus and 0.828 (95% CI, 0.705–0.903) in the deep capillary plexus. For reproducibility, the ICC was 0.651 (95% CI, 0.439–0.795) and 0.363 (95% CI, 0.073–0.596) at the superficial and deep capillary plexus, respectively. In this study, the FD of the vascular network measured via SS-OCTA showed good repeatability and reproducibility in healthy participants.

Abstract:The Journal of Vascular Access, Ahead of Print. Background:EchoSure is an automated point-of-care 3D ultrasound (3DUS) designed to be used by dialysis technicians without advanced ultrasound training. The EchoMark/EchoSure System is a two-part system comprised of a bioresorbable implant, EchoMark, and a diagnostic ultrasound imaging platform, EchoSure. EchoSure was designed to enable any healthcare personnel in a dialysis clinic setting to obtain non-invasive, direct measurements of flow and vessel parameters that are critical quantifications used in the assessment of AVF maturation and readiness for hemodialysis cannulation.Objective:In this pilot feasibility study, we sought to explore whether use of an automated 3DUS could enable fistula assessment by non-experts and obtain measurement accuracy comparable to expert sonographer Duplex.Method:This was a 20 subject prospective multicenter trial conducted at four sites in the United States. All subjects had an EchoMark implanted under their AVF during the fistula creation procedure. Subjects were evaluated at 2-weeks post-op with EchoSure and Duplex, followed by monthly assessments until either the 4-month visit or until radiographic maturation was achieved, whichever was longer. Beyond monthly ultrasound assessments, clinical follow-up continued every 6 months between months 6 and 24.Result:Technical success of EchoMark implantation was demonstrated in 100% of subjects. Technical success of EchoSure was 99% across all scans. The mean percent difference in flow rate measurements between EchoSure and Duplex was −9.2% with a standard deviation of 38.1% compared to a mean percent difference of −15.7% with a standard deviation of 35.5% between Duplex measurements taken at the cannulation zone and brachial artery. Pearson correlation between EchoSure and Duplex was 0.708 versus 0.716 for the two Duplex measurements. Radiographic maturation was achieved in 70% of study subjects by 30 days and continued to rise to 90% through 90 days and 95% through 180 days after baseline.Conclusion:This study demonstrates the feasibility and accuracy of an automated 3D ultrasound system for assessment of maturation at the dialysis bedside by non sonographers.

Role of Renal Artery Doppler Ultrasonography in the Evaluation of Function of Transplanted Kidney

Abstract:Objective:To evaluate the clinical value of renal artery Doppler ultrasonography in the diagnosis of renal transplant dysfunction by detecting blood perfusion in transplanted kidney with ultrasound .Methods:A total of 107 renal transplantation patients ,aged (38 .8 ± 1 .6) years in average ,including 68 new cases and 39 follow‐ups ,were recruited .Peak systolic velocity (PSV) ,end‐diastolic velocity (EDV) and resistance index (RI) ,in main renal artery ,renal segmental arteries and interlobar arteries of transplanted kidneys ,were examined by color Doppler ultrasonography .Results:Hemorheologic alterations in renal segmental arteries and interlobar arteries were closely correlated with renal function .Correlation indexes between PSV and clearance of creatinine (Ccr) were r=0 .072(P>0 .05) ,r=0 .224 (P=0 .014) and r=0 .250 (P=0 .006) ,respectively ,in main renal artery ,renal segmental arteries and interlobar arteries .Correlation indexes between RI and Ccr were r= -0 .398 (P<0 .001) ,r= -0 .400(P<0 .001) and r= -0 .436(P<0 .001) ,respectively ,in main renal artery ,renal segmental arteries and interlobar arteries .Correlation indexes between EDV and Ccr were r=0 .446(P<0 .001) ,r=0 .659(P<0 .001) and r=0 .529(P< 0 .001) ,respectively ,in main renal artery ,renal segmental arteries and interlobar arteries .The above results indicated that ,the maintenance of diastolic blood flow in renal arteries was important for renal function .The increase of RI and decrease of EDV in arteries of transplanted kidney was characteristic manifestation of chronic allograft nephropathy and vasculitic rejection .Conclusions:Renal artery Doppler ultrasonography can effectively measure the hemorheologic alterations of transplanted kidney ,thus it is an important adjuvant diagnostic tool for clinical renal transplantation .
Repeatability, Reproducibility, and Observer Variability of Cortical T1 Mapping for Renal Tissue Characterization

Abstract:Background The global rise in kidney diseases underscores the need for reliable, noninvasive imaging biomarkers. Among these, renal cortical T1 has shown promise but further technical validation is still required. Purpose To evaluate the repeatability, reproducibility, and observer variability of kidney cortical T1 mapping in human volunteers without known renal disease. Study Type Prospective. Subjects Three cohorts without renal disease: 1) 25 volunteers (median age 38 [interquartile range, IQR: 28–42] years, female N = 11) for scan–rescan assessments on GE 1.5 T and Siemens 1.5 T; 2) 29 volunteers (median age 29 [IQR: 24–40] years, female N = 15) for scan–rescan assessments on Siemens 3 T; and 3) 16 volunteers (median age 34 [IQR: 31–42] years, female N = 8) for cross‐scanner reproducibility. Field Strength/Sequences 1.5 T and 3 T, a modified Look‐Locker imaging (MOLLI) sequence with a balanced steady‐state free precession (bSSFP) readout. Assessment Kidney cortical T1 data was acquired on GE 1.5 T scanner, Siemens 1.5 T and 3 T scanners. Within‐scanner repeatability and inter/intra‐observer variability: GE 1.5 T and Siemens 1.5 T, and cross‐scanner manufacturer reproducibility: Siemens 1.5 T–GE 1.5 T. Statistical Tests Bland Altman analysis, coefficient of variation (CoV), intra‐class coefficient (ICC), and repeatability coefficient (RC). Results Renal cortical T1 mapping showed high repeatability and reliability across scanner field strengths and manufacturers (repeatability: CoV 1.9%–2.8%, ICC 0.79–0.88, pooled RC 73 msec; reproducibility: CoV 3.0%, ICC 0.75, RC 90 msec). The method also showed robust observer variability (CoV 0.6%–1.4%, ICC 0.93–0.98, RC 22–48 msec). Data Conclusion Kidney cortical T1 mapping is a highly repeatable and reproducible method across MRI manufacturers, field strengths, and observer conditions. Abstract:PurposeTo prospectively determine the repeatability of noncontrast-enhanced renal arterial angiography with steady-state free precession (SSFP) and time-spatial labeling inversion pulse (Time-SLIP), and to compare the visibility of renal artery and its branches when different locations of tagging pulse were placed.MethodsThirty-six young healthy volunteers were enrolled in this study and were twice examined by noncontrast-enhanced renal arterial angiography with SSFP and Time-SLIP in 1.5T MR scanner. Measurement error and repeatability were assessed for each of the five major parameters [vessel-to-kidney ratio (VKR), grade of renal arterial branching, grading of image quality, diameter and area of the main renal artery] using the Bland–Altman plot. Two independent observers recorded the values of the parameters; Inter- and intra-observer agreement was assessed using the intraclass correlation coefficients (ICCs). The same parameters, acquired at the tagging pulse placed just above the superior poles of both kidneys or closer to the main renal arteries, were compared using the Wilcoxon signed-rank test.ResultsGrading of arterial branching by the Time-SLIP SSFP was satisfactorily reproducible with the mean score of greater 3.83 indicating the visibility of branches within the renal parenchyma. The image quality was excellent for Segment I (the main trunk of renal artery) and good for Segment II (segmental branches pre renal parenchyma) and III (vessels within the renal parenchyma) with a satisfying repeatability between two examinations and a good inter- and intra-observer agreement. The ICCs for the inter- and intra-observer measurements of both diameter and area of the main arteries ranged from 0.781 to 0.934, indicating very good agreement. The repeatability of VKR was poor between the two examinations and at the two different tagging pulse locations. The position of tagging pulse in the origination of the main renal arteries was better than in the superior poles of kidneys as it provided a better visualization of arterial branches.ConclusionNoncontrast-enhanced renal artery angiography with SSFP and Time-SLIP yields reliable and reproducible visualization of normal renal arteries. Localization of the tagging pulse closer to the main renal arteries provides better visibility of renal artery and its branches than the tag placement just above the superior poles of both kidneys.

Abstract:Objective: Volumetric flow measurement with Doppler ultrasound is useful in assessing blood flow as part of an evaluation of arteriovenous fistula maturity in patients undergoing hemodialysis. In this study, we assessed both accuracy and variability in volumetric flow measurements obtained using modern and commercially available ultrasound systems and an in vitro experimental setup. Methods: Volumetric flow measurements using duplex ultrasound were obtained by 3 users operating 5 different systems for randomized flow in the range of 100 to 1000 mL/min. Users performed 3 consecutive measurements at a given flow rate. Data were analyzed using statistical techniques to assess measurement accuracy and variability. Results: Over the span of flow rates studied, the root mean square error (RMSE) for the 5 ultrasound systems ranged from 38.8 to 79.7, 36.8 to 52.0, 73.0 to 85.3, 26.7 to 44.6, and 43.9 to 93.5 mL/min. Corresponding average RMSE values were 60.3, 42.7, 81.1, 37.2, and 64.4 mL/min, respectively. A linear regression analysis of mean interobserver measurements revealed an excellent correlation for all ultrasound systems (r(2) > 99.1%). Assessment of intraobserver measurements revealed no statistically significant differences for any ultrasound system evaluated (P > .94). Comparison of interobserver measurements indicates no statistically significant differences between any of the 5 systems (P > .14). Conclusions: Modern ultrasound systems are reasonably accurate in blood flow measurement in an experimental setup mimicking clinically relevant blood flow ranges in a hemodialysis fistula. Users need adequate training and experience to perform multiple measurements and use appropriate techniques to minimize errors in flow measurement.
Evaluating Renal Arterial Wall by Non-Enhanced 2D and 3D Free-Breathing Black-Blood Techniques: Initial Experience.

Abstract:Objectives: To evaluate the feasibility and reproducibility of 2D and 3D black-blood sequences in measuring morphology of renal arterial wall. Methods: The 2D and 3D imaging sequences used variable-refocusing-flip-angle and constant-low-refocusing-flipangle turbo spin echo (TSE) readout respectively, with delicately selected black-blood scheme and respiratory motion trigger for free-breathing imaging. Fourteen healthy subjects and three patients with Takayasu arteritis underwent renal artery wall imaging with 3D double inversion recovery (DIR) TSE and 2D Variable Flip AngleTSE (VFA-TSE) black-blood sequences at 3.0 T. Four healthy subjects were randomly selected for scan-rescan reproducibility experiments. Signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and morphology of arterial wall were measured and compared using paired-t-test or Wilcoxon signed-rank test between 2D and 3D sequences. The inter-observer, intra-observer and scan-rescan agreements of above measurements were determined using intraclass correlation coefficient (ICC). Results: The 2D and 3D imaging sequences showed similar morphological measurements (lumen area, wall area, mean wall thickness and maximum wall thickness) of renal arterial wall (all P > 0.05) and excellent agreement (ICC: 0.853-0.954). Compared to 2D imaging, 3D imaging exhibited significantly lower SNRlumen (P < 0.01) and SNRwall (P = 0.037), similar contrast-to-noise ratio (CNR) (P = 0.285), and higher CNR efficiency (CNReff) (P < 0.01). Both 2D and 3D imaging showed good to excellent inter-observer (ICC: 0.723-0.997), intra-observer (ICC: 0.749-0.996) and scan-rescan (ICC: 0.710-0.992) reproducibility in measuring renal arterial wall morphology, SNR and CNR, respectively. Conclusions: Both high-resolution free-breathing 2D VFA-TSE and 3D DIR TSE black-blood sequences are feasible and reproducible in high-resolution renal arterial wall imaging. The 2D imaging has high SNR, whereas 3D imaging has high imaging efficiency.
Intraobserver variation in Doppler ultrasound assessment of pulmonary artery pressure.

Abstract:Intraobserver variation associated with the non-invasive assessment of pulmonary artery pressure (PAP), using measurement of pulmonary artery Doppler derived systolic time intervals, was investigated. Forty pairs of independent ultrasound examinations of the pulmonary artery were performed by a single observer in 20 preterm infants, median gestation 27 weeks (range 24-31 weeks). Median age at study was 17 days (range 1-47 days). paired measurements of acceleration time (AT), ratio between acceleration time and right ventricular ejection time (AT:RVET), corrected AT, and corrected AT:RVET were compared to assess intraobserver agreement. For the corrected AT:RVET ratio, the mean percentage difference between observations was -0.9% (95% confidence intervals -5.0 to 3.1%). The limits of agreement for the two measurements were -26.3 to 24.5%. The coefficient of repeatability was 25.4%. Variation for other indices was similar. Non-invasive assessment of PAP using Doppler derived systolic time intervals is associated with considerable intraobserver variation.

Abstract:The Journal of Vascular Access, Ahead of Print. Background:An accessible tool is required to analyze volume flow trends in arteriovenous fistulas for hemodialysis. Earlybird, an easy-to-place ultrasound Doppler device, has shown comparable accuracy to duplex ultrasound. In this study, we compared volume flow measurements obtained with duplex ultrasound and the dilution technique to an enhanced earlybird device, featuring a dual Doppler probe system, eliminating the requirement for a known insonation angle.Methods:Nine patients with a distal radiocephalic arteriovenous fistula were monitored for 12 months with regular volume flow measurements. Correlation and inter- and intra-class reliability analyses were conducted.Results:An overall moderate correlation was observed between earlybird and duplex ultrasound or dilution technique (intraclass correlation coefficient = 0.606 (95% confidence interval 0.064, 0.721) and 0.581 (0.039, 0.739), respectively). Duplex ultrasound compared to dilution measurements, demonstrated an overall moderate correlation (0.725 (0.219, 0.843)). Correlation between earlybird and duplex ultrasound was stronger for the arteriovenous fistula (0.778 (0.016, 0.901)) than the brachial artery (0.381 (−0.062, 0.461)). For earlybird, inter-rater reliability was excellent for the arteriovenous fistula (0.907 (0.423, 0.930)) and poor for the brachial artery (0.430 (0.241, 0.716)). Duplex ultrasound showed a good inter-rater reliability (arteriovenous fistula: 0.843 (0.610, 0.871), brachial artery: 0.819 (0.477, 0.864)). The overall intra-rater reliability was good for duplex ultrasound (rater A: 0.893 (0.727, 0.911); rater B: 0.853 (0.710, 0.891)), while excellent for earlybird (rater A: 0.905 (0.819, 0.928); rater B: 0.921 (0.632, 0.969)).Conclusion:We observed a weaker correlation in the measurements of volume flow rates in arteriovenous fistulas when obtained using earlybird compared to dilution technique, unlike the comparison between duplex ultrasound and the dilution technique. However, inter-rater reliability for the arteriovenous fistula was excellent with earlybird and good with duplex ultrasound, indicating the potential of earlybird as a tool for frequent measurements, enabling trend surveillance and predicting adverse outcomes.

Abstract:Objectives: Strain echocardiography (StE) promises to be a new tool for quantitative assessment of cardiac function. Analysis of intra- and interobserver reliability is an important aspect in the process of developing these novel techniques from theory to the implementation into daily routine diagnostics.The purpose of the study was to estimate reliability of the segmental StE. Methods: Left ventricular strain analysis for radial strain (RS), circumferential strain (CS) and longitudinal strain (LS) was performed in 21 healthy volunteers. RS and CS values were obtained in the parasternal short axis at the level of the papillary muscles. LS values were determined in the apical 2-, 3- and 4-chamber views. Cine-loops were recorded and quantitative analyses were conducted on an off-line workstation. Results: Intraobserver reproducibility was highest using LS in the 4-chamber view (9 ± 13.6 % mean deviation, rho = 0.624, p = 0.003), followed by CS (13.3 ± 8.3 %, rho = 0.406, p = 0.068) and lowest in RS (26.3 ± 30.1 %, rho = 0.391, p = 0.080). Interobserver analyses of LS derived from 3-chamber view showed lowest deviation (11.9 ± 9.5 %, rho = 0.513, p = 0.017), followed by CS (15.2 ± 12.0 %, 0.263, p = 0.249) and the least consistent measurements in RS (35.9 ± 46.3 %, rho 0.382, p = 0.088). Conclusion: This study shows that the clinical utility of StE depends on the regional differences of LV wall motion and image quality. LS-values showed promising intra- and interobserver reproducibility values. For quantitative follow-up studies LS should be preferred.
Ultrasound localization microscopy of the human kidney allograft on a clinical ultrasound scanner

Abstract:Chronic kidney disease is a major medical problem, causing more than a million deaths each year worldwide. Peripheral kidney microvascular damage characterizes most chronic kidney diseases, yet noninvasive and quantitative diagnostic tools to measure this are lacking. Ultrasound Localization Microscopy (ULM) can assess tissue microvasculature with unprecedented resolution. Here, we optimized methods on 35 kidney transplants and studied the feasibility of ULM in seven human kidney allografts with a standard low frame rate ultrasound scanner to access microvascular damage. Interlobar, arcuate, cortical radial vessels, and part of the medullary organization were visible on ULM density maps. The medullary vasa recta can be seen but are not as clear as the cortical vessels. Acquisition parameters were derived from Contrast-Enhanced Ultrasound examinations by increasing the duration of the recorded clip at the same plane. ULM images were compared with Color Doppler, Advanced Dynamic Flow, and Superb Microvascular Imaging with a contrast agent. Despite some additional limitations due to movement and saturation artifacts, ULM identified vessels two to four times thinner compared with Doppler modes. The mean ULM smallest analyzable vessel cross section was 0.3 ± 0.2 mm in the seven patients. Additionally, ULM was able to provide quantitative information on blood velocities in the cortical area. Thus, this proof-of-concept study has shown ULM to be a promising imaging technique for qualitative and quantitative microvascular assessment. Imaging native kidneys in patients with kidney diseases will be needed to identify their ULM biomarkers.
