Abstract:Objective To explore the reliability of Modified Ashworth Scales(MAS) measuring muscular tone of lower extremities of children with spastic cerebral palsy(CP).Methods 31 children with spastic CP(19 males,12 females) were included in this study. The mean age for the children was (51.3±26.8) months; 20 diplegia,7 quadriplegia,4 hemiplegia. The muscle tone of hip adductor,hamstring,soleus muscles were tested bilaterally with MAS in lower extremity of the children. Two evaluators used the MAS to assesse the muscles tone of children. To compare with the relativity of the results of interrater and intrarater,the reliability was confirmed.Results The reliability of interrater was poor,the total ICC value was 0.68,while better for the reliability of intrarater,the total ICC value was 0.83.Conclusion It should be cautions in assessment of spasticity of muscles in the lower extremities of children with spastic CP by MAS.

Abstract:Introduction: The Fugl-Meyer Motor Assessment (FMMA) is recommended for evaluating stroke motor recovery in clinical practice and research. However, its widespread use requires refined reliability data, particularly across different health professions. We therefore investigated the interrater reliability of the FMMA scored by a physical therapist and a physician using video recordings of stroke patients. Methods: The FMMA videos of 50 individuals 3 months post stroke (28 females, mean age 71.64 years, median National Institutes of Health Stroke Scale score 3.00) participating in the ESTREL trial (Enhancement of Stroke Rehabilitation with Levodopa: a randomized placebo-controlled trial) were independently scored by two experienced assessors (i.e., a physical therapist and a physician) with specific training to ensure consistency. As primary endpoint, the interrater reliability was calculated for the total scores of the entire FMMA and the total scores of the FMMA for the upper and lower extremities using intraclass correlation coefficients (ICC). In addition, Spearman's rank order correlation coefficients (Spearman's rho) were calculated for the total score and subscale levels. Secondary endpoints included the FMMA item scores using percentage agreement, weighted Cohen's kappa coefficients, and Gwet's AC1/AC2 coefficients. Results: ICCs were 0.98 (95% confidence intervals (CI) 0.96–0.99) for the total scores of the entire FMMA, 0.98 (95% CI 0.96–0.99) for the total scores of the FMMA for the upper extremity, and 0.85 (95% CI 0.70–0.92) for the total scores of the FMMA for the lower extremity. Spearman's rho ranged from 0.61 to 0.94 for total and subscale scores. The interrater reliability at the item level of the FMMA showed (i) percentage agreement values with a median of 77% (range 44–100%), (ii) weighted Cohen's kappa coefficients with a median of 0.69 (range 0.00–0.98) and (iii) Gwet's AC1/AC2 coefficients with a median of 0.84 (range 0.42–0.98). Discussion and conclusion: The FMMA appears to be a highly reliable measuring instrument at the overall score level for assessors from different health professions. The FMMA total scores seem to be suitable for the quantitative measurement of stroke recovery in both clinical practice and research, although there is potential for improvement at the item level.

Abstract:Background/Objectives: This study aimed to examine intra- and inter-rater reliability of a myotonometer (MyotonPRO) in measuring upper and lower extremity mechanical properties in children with spinal muscular atrophy types I and II. Methods: Biceps brachii, triceps brachii, rectus femoris, and gastrocnemius muscle tone and stiffness in children (n = 21) were measured using the MyotonPRO device. Examiner 1 performed two sets of measurements in 60 min to determine intra-rater reliability. Examiner 2 performed measurements between Examiner 1's sets. Intra–interclass correlation coefficient, minimal detectable change, and standard error of measurement values were calculated to assess intra- and inter-rater reliabilities in this cross-sectional study. Results: The results showed excellent intra- and inter-rater reliability analyses for frequency and stiffness values except for the stiffness value of the gastrocnemius muscle, which presented good reliability (ICC = 0.71). Minimal detectable change values ranged from 0.59 to 1.98 Hz for muscle tone and 16.08 to 124.74 N/m for stiffness (for both intra- and inter-rater reliabilities). Conclusions: Our findings indicate that MyotonPRO is a reliable tool for quantifying upper and lower extremity mechanical properties within one session in children with spinal muscular atrophy types I and II. Mechanical properties of the extremity muscle can be determined using this easily applied tool in future studies.

Reliability of ankle dorsiflexor muscle strength, rate of force development, and tibialis anterior electromyography after stroke

Abstract:Background Measures of hemiparetic ankle dorsiflexor muscle strength and rate of force development (RFD) are often used to determine the efficacy of rehabilitation interventions after stroke. However, evidence supporting the reliability of these measures is limited. This brief report provides a secondary analysis investigating the between-session reliability of isometric ankle dorsiflexor muscle strength, rate of force development (RFD), and tibialis anterior electromyography (TA EMG), in people with chronic stroke. Method Participants (n=15) completed three maximal isometric contractions of the ankle dorsiflexor muscles as fast as possible using a rigid dynamometer. Tests were repeated seven days later. Outcomes included ankle dorsiflexor isometric maximal voluntary contraction (MVC), RFD in the first 200ms (RFD200ms), time to reach 90% MVC, and peak TA EMG. Data were analysed for 13 participants using intra-class correlation coefficients (ICC) and standard error of the measure percentage (SEM%). Results Reliability was higher when analysing the mean of three trials rather than the best of three trials. There was excellent reliability for isometric dorsiflexor MVC (ICC 0.97 [95% CI 0.92, 0.99], SEM% 7%). However, for other outcomes, while the ICC indicated good reliability, the lower bound of the 95% confidence interval of the ICC fell in the moderate range for TA EMG (ICC 0.86 [95% CI 0.60, 0.96], SEM% 25%) and time to reach 90% MVC (ICC 0.8 [95% CI 0.53, 0.93], SEM% 23%) and in the poor range for dorsiflexor RFD200ms (ICC 0.79 [95% CI 0.48, 0.92], SEM% 24%). Conclusion The findings raise concerns about the reliability of measures of rapid force production in the dorsiflexor muscles after stroke. Given the functional significance of the ankle dorsiflexors, larger studies should be conducted to further investigate these concerns and explore reliable methods for measuring rapid force production in the hemiparetic dorsiflexor muscles.

Abstract:BACKGROUND: The Active Movement Scale (AMS) is a brief screen for identifying movement impairments affecting functional task performance. OBJECTIVE: To assess inter-rater reliability, test-retest reliability, floor and ceiling effects, and concurrent validity of the AMS in adults with musculoskeletal disorders. METHODS: Fifty-five subjects (mean age: 61.3±15.3 years) transitioning from physical therapy attended two sessions. The 14-item AMS was assessed by two therapists, resulting in a total score (AMST) and two sub-scores: upper body (AMSU) and lower body (AMSL). Moreover, subjects completed PROMIS Physical Function-10a (PPF), Lifestyle Physical Function (LPF), Cardio Activity Level (CAL) surveys, Two Square Agility Test (TSAT), WorkAbility Rate of Manipulation Turning (WRMT) and Placing (WRMP), Grip Strength (GS), 10-Meter Walk Usual (GSU) and Fast (GSF) pace, and Grooved Pegboard Placing (GPP) and Remove (GPR) tests. RESULTS: Inter-rater reliability coefficients for AMST, AMSU, and AMSL were 0.96, 0.92, and 0.96, respectively, with individual item reliability ranging from 0.58 to 1.0. Test-retest reliability for these assessments yielded coefficients of 0.93, 0.84, and 0.94, with individual item reliability ranging from 0.47 to 0.88. No floor effects were observed, but mild ceiling effects were noted. AMST showed a high correlation with LPF (r = 0.72) and moderate correlation with PPF (r = 0.64) surveys. AMSU had moderate correlations with GPP (r = 0.61) and WRMP (r = 0.57) tests, while AMSL correlated moderately with GSF (r = 0.55) and TSAT (r = 0.50) tests. CONCLUSIONS: This study provides evidence of the reliability and validity of AMS in adults with musculoskeletal disorders to support transitions between rehabilitation and fitness programs.

Abstract:Background After stroke many patients remain with difficulties in using the upper limbs, balance, transfers, and walking. The Motor Assessment Scale (MAS) assess the movements needed to do those activities. Social restrictions to combat the Covid-19 pandemic have increased the telerehabilitation, but the remote assessment is also important for rural areas or geographic regions where neurorehabilitation specialists are scarce, and when patients have difficulties on transport to the clinic. Although the measurement properties of the MAS applied in person are established, the validity and reliability of the MAS applied via teleconsultation is unknown. Objectives To investigate the validity and reliability of the Motor Assessment Scale (MAS) when administered remotely by videoconference (Tele-MAS). Methods This is a study of investigation of measurement properties, following the recommendations of COSMIN, for validity and reliability of Tele-MAS. The sample was 18 participants with a diagnosis of stroke, Braztel-MMSE score ≥13 points and with internet access and mobile device. The order of the evaluations (remote or in person) was randomly defined. The application sequence of the MAS items was adapted to allow remote application in addition to verbal commands during the evaluation and a specific instruction manual for application by videoconference was developed. For assessment by videoconference (rater A and B) the participant was instructed to position the camera in a way that the therapist can observe from the ground to above the head. The raters are positioned similarly to the participant to demonstrate the items and score synchronously. The application in person takes place in the participant's house, by rater A. The three collections took place within a period of eight days, with an interval of 2 days. The validity between the in person and remote evaluation was analyzed by Pearson's correlation coefficient and the reliability between the remote and in person evaluation was analyzed using the Bland-Altman limits of agreement. The interrater reliability for the sum of the score of the items was analyzed by the Intraclass Correlation Coefficient (ICC) with a confidence interval of 95% (95%CI). Data were analyzed using MedCalc software®. Results A strong positive correlation (r=0.96; 95%CI=0.90-0.99) was found between the MAS and Tele-MAS scores. The analysis of the Bland-Altman graph for the total MAS scores indicated that the mean difference between in person and remote scores was -0.6 points. The 95% limits of agreement are +6.5 and -7.6 points. Excellent interrater reliability (ICC≤0.94; 95%CI =0.84-0.98) of the total Tele-MAS score. Conclusion The preliminary results indicate that Tele-MAS should be considered as a valid measure and has interrater reliability. Implications MAS is widely used in clinical practice, however, the measurement properties of the remote version applied by videoconference were unknown. The results of the final study should present the validity and reliability of Tele-MAS to remotely assess post-stroke motor function through videoconference.

Reliability of Surface Electromyography From the Lower-limb Muscles During Maximal and Submaximal Voluntary Isometric Contractions in In-bed Healthy Individuals and Patients With Subacute Stroke

Abstract:This study aims to develop maximal voluntary isometric contraction (MVIC) and submaximal voluntary isometric contraction (subMVIC) methods and to assess the reliability of the developed methods for in-bed healthy individuals and patients with subacute stroke. The electromyography (EMG) activities from the lower-limb muscles including the tensor fascia lata (TFL), rectus femoris (RF), tibialis anterior (TA), and gastrocnemius (GC) on both sides were recorded during MVIC and subMVIC using surface EMG sensors in 20 healthy individuals and 20 subacute stroke patients. In inter-trial reliability, both MVIC and subMVIC methods demonstrated excellent reliability for all the measured muscles at baseline and follow-up evaluations in both healthy individuals and stroke patients. In inter-day reliability, MVIC showed good reliability for the TFL and moderate reliability for the RF, TA, and GC, while subMVIC showed good reliability for the TFL, RF, and GC and poor reliability for the TA in healthy individuals. In conclusion, the MVIC and subMVIC methods of EMG activities were feasible in in-bed healthy individuals and patients with subacute stroke. The results can serve as a basis for the clinical evaluation of muscular activities using quantitative EMG signals on the lower-limb muscles in stroke patients with impaired mobility.
Site-specific skeletal muscle mass and functional prognosis in geriatric stroke patients

Abstract:Background: Evidence is scarce regarding prognostic value of site-specific muscle mass in geriatric stroke survivors. We aim to assess the association between four measures of muscle mass, namely the skeletal muscle mass index of the limbs (SMI), upper limb SMI (USMI), lower limb SMI (LSMI), and trunk muscle mass index (TMI), and the functional prognosis in stroke patients. Methods: This study conducted a retrospective cohort analysis involving post-acute stroke inpatients. Muscle mass data were obtained through bioelectrical impedance analysis and computed by dividing each muscle mass by the square of the height. The study outcomes included the Functional Independence Measure (FIM) motor at discharge and FIM-motor gain. Multiple regression analysis was conducted to assess the association between SMI, USMI, LSMI, and TMI with outcomes, while adjusting for confounding factors. Results: A total of 701 patients (mean age 72.8 years, 374 males) were analyzed. As a result, LSMI (β = 0.089, P = 0.003) and SMI (β = 0.083, P = 0.008) were significantly associated in the FIM-motor at discharge, with LSMI showing a stronger association. USMI (β = 0.019, P = 0.521) and TMI (β = 0.035, P = 0.231) showed no significant association. LSMI (β = 0.124, P = 0.003) and SMI (β = 0.116, P = 0.008) were significantly associated with FIM-motor gain; however, USMI (β = 0.027, P = 0.521) and TMI (β = 0.049, P = 0.231) showed no significant association with FIM-motor gain. Conclusions: Differential associations were observed between site-specific muscle mass and functional prognosis in post-stroke patients. Among these, lower limb muscle mass was most strongly associated with activities of daily living (ADL) recovery.

Abstract:Objective: To study the correlation between isokinetic parameters and modified Ashworth scores in spasticity assessment. Method: Twenty patients of spasticity after hemiplegia were studied. The muscle tone were assessed with modified Ashworth spasticity scale(mASS) at first. Then the isokinetic dynamometer (Biodex System-3) was used to measure the resistance torque of knee joint at 30°/s,120°/s and 180°/s. Two parameters: peak torque (PT) and peak torque/body weight (PT/BW) were recorded. The Spearman correlation analysis was studied between two tests. Result: The correlation coefficient was 0.633-0.962 among mASS and PT and PT/BW during isokinetic tests(P<0.01). Conclusion: The isokinetic parameters were closely related with mASS. Both of them could represent the extent of spasticity.

Abstract:Title Inter-Tester Reliability and Agreement of the Cumulated Ambulation Score in Stroke patients: The InTRO-CAS-stroke study Objective To examine the inter-tester reliability of the total Cumulated Ambulation Score (CAS) and the three activities of the CAS, and to define limits for the smallest change in basic mobility that indicates a real change in patients with stroke. Methods An intertester reliability study. Setting: Participants: 60 participants from a specialized stroke unit with a mean age of 69.10 ±13.23 years. The CAS describes a patient's independency in three activities – (1) getting in and out of bed, (2) sit to stand from a chair, and (3) walking ability and was assessed by two independent physiotherapists at median day 3 poststroke. Each activity was assessed on a three-point ordinal scale from 0 (not able to) to 2 (independent). The cumulated score for each activity provides a total CAS from 0 to 6, with 6 indicating independent basic mobility. Relative and absolute reliability was evaluated using weighted kappa, the standard error of measurement (SEM) and minimal detectable change (MDC). Results The weighted kappa for the total CAS score was 0.816 and ranged from 0.733 to 0.904 for the 3 CAS activities. The SEM and the MDC of the total CAS was 0.37 and 1.03 respectively. Conclusions The intertester reliability of the CAS is almost perfect, and a change of 1 CAS point for the total CAS indicates a real change in basic mobility, at group level and for an individual patient with stroke Clinicaltrials.gov identifier NCT05601089

The Reliability of Four Motor Function Tests for Assessing Upper Extremity Motor Function in Stroke Patients

Abstract:Objective To investigate the test-retest reliability and inter-rater reliability of the Fugl-Meyer assessment (FMA),Wolf's motor function test (WMFT),the box-block test (BBT) and the nine-hole peg test (NHPT) in assessing the upper limb motor function of stroke patients.Methods Eighteen stroke patients participated in the study.Each patient was assessed twice within 2 hours with the U-FMA,WMFT,the BBT and the NHPT.Two raters recorded each session separately.Test-retest reliability was investigated by comparing the assessment results of different sessions; interrater reliability was identified by analyzing the assessment results of different raters.Results The intraclass correlation coefficients (ICC) for the U-FMA were 0.988 on test-retest reliability and 0.959 on inter-rater reliability.The ICC of WMFT by mean and median time were respectively 0.996 and 0.393 on test-retest reliability and both 1.000 on inter-rater reliability.The ICC of WMFT by score mean was O.989 for test-retest reliability and 0.977 for inter-rater reliability.The ICC for test-retest reliability of the BBT using affected hand counts and the difference in counts between the affected and un-affected hand were respectively 0.975 and 0.982.The ICC of the NHPT using affected hand times and the difference between the affected hand and unaffected hand times were respectively 0.937 and 0.936 for test-retest reliability,and both 1.000 for inter-rater reliability.Conclusion The U-FMA,WMFT,the BBT and the NHPT are all measurements with good test-retest reliability and inter-rater reliability.They are reliable for assessing the upper extremity motor function of stroke patients.
Effects of Maitland mobilization technique on upper extremity function in stroke survivors with spasticity: An experimental study

Abstract:Background: The recovery of upper limb function is of great significance for stroke patients to regain their self-care ability, yet it is still a difficult point in clinical practice of neurological rehabilitation. This study aimed to investigate the effect of Maitland joint mobilization technique on the recovery of upper extremity function in patients with spasticity after stroke. Methods: From August to December 2023, 71 patients with upper extremity flexor spasm after stroke were recruited and randomly divided into experimental group (n = 35) and control group (n = 36). The control group was given conventional rehabilitation treatment, while the experimental group was treated with Maitland mobilization technique treatment of upper extremity joints on the basis of the control group. The experiment lasted for 8 weeks. Participants of the 2 groups were observed for Fugl-Meyer motor assessment-upper extremity (FMA-UE), box and block test (BBT) and Brunnstrom stage, modified Ashworth scale (MAS), and functional independence measure (FIM) at pre- and post-8 weeks study. Results: There was no significant difference in gender distribution, hemiplegic side, diagnosis, past history, age, duration, body mass index, and mini-mental state examination between the 2 groups (P > .05). After 8 weeks of intervention, both groups showed significant improvement in FMA-UE, Brunnstrom stage, BBT, FIM, and MAS of the shoulder (P .05). The posttreatment values showed a significant improvement in FMA-UE, BBT, and FIM in the experimental group compared to the control group. Comparing the changes in pretreatment and posttreatment, FMA-UE, BBT, and FIM in the experimental group were significantly improved compared with those in the control group (P < .05). Conclusion: Maitland joint mobilization can improve the motor function of upper extremity and the spasticity of shoulder joint complex in patients with stroke.

Measuring Exercise Self-Efficacy After Stroke: Validity and Reliability of Current Measures

Abstract:Background and purpose: Many physically capable stroke survivors are insufficiently active, with low self-efficacy considered an important contributor. However, validity and reliability of self-efficacy measures in stroke survivors have not been established. This research aims to evaluate the test-retest reliability and construct validity of 3 self-efficacy measures: Self-Efficacy for Exercise Scale (SEE), Spinal Cord Injury Exercise Self-Efficacy Scale (SCI-ESES), and Participation Strategies Self-Efficacy Scale (PS-SES). Methods: A repeated measures study with community-dwelling, independently mobile adult stroke survivors (n = 51, mean age 74 years, 45% female, median 22 months poststroke) was completed. Test-retest reliability was assessed using intraclass correlation coefficients and Bland-Altman analyses. Construct validity was assessed using 8 pre-determined hypotheses concerning physical activity level (subjective and objective), comorbidities, work and volunteering, and measures of function. Results: Retest reliability was established for the SEE (intraclass correlation coefficient, ICC = 0.77) and PS-SES (ICC = 0.78) but not for the SCI-ESES (ICC = 0.68). Bland-Altman analysis showed participants consistently scored higher on the second test for all measures. The SEE achieved construct validity by meeting 75% of hypotheses, whereas the PS-SES and SCI-ESES did not. Self-efficacy was positively related to steps/day, functional capacity, self-reported activity levels, and work or volunteering participation. Discussion and conclusions: The SEE was found to be the most appropriate tool to measure exercise self-efficacy in independently mobile chronic stroke survivors in terms of retest reliability and validity. Abstract:Motor disorders as a result of stroke impair the mobility of the patient, limiting his participation in daily activities. Spastic paresis is one of the most frequent consequences of the central nervous system damage. At present, a promising method of therapy for spastic paresis,due to its non-invasiveness and safety, is the application of focal muscle vibration (FMV) in stroke patients. Foreign studies demonstrate the successful use of FMV, while there are no mentions of the FMV application of in the Russian literature. Aim. To evaluate the effectiveness of the of FMV use in the rehabilitation of the upper limb in the early recovery period in patients who have suffered a cerebral stroke, with a degree of disability of 3 points on the Rankin scale. Material and methods. The study involved 40 patients of the Federal Center of Brain Research and Neurotechnologies: 20 patients of the experimental group (13 men, 51.4±12.85 years) and 20 patients of the control group (15 men, 53.0±11.34 years). Patients of the control group received a course of conventional physical therapy (individual physical therapy sessions). The participants of the experimental group, in addition to conventional physical therapy classes identical to those in the control group, underwent a training course on the «Vibramoov» (Techno Concept, France) apparatus for the upper limb. To assess the effectiveness of rehabilitation before and after the course, functional scales (MRC, MAS, FMA, FAT) were used to assess the degree of the upper limb functional recovery. Results and discussion. As a result, the patients of the experimental group showed improvements in both the proximal and distal parts of the upper limb, while the patients of the control group only in the proximal. Conclusion. Due to the results obtained after the course with the use of FMV, the range of motor skills has expanded in patients that will allow them to use the upper limb more effectively when performing household activities, such as eating, dressing, personal hygiene, etc.

Abstract:In spite of all the advances in sport nutrition and the importance of an adequate food intake in order to improve sport performance, both recreational and professional athletes forget frequently to include planning an optimum diet and fluid intake in their global strategy for performance. Physiological and metabolic adaptations produced as a consequence of physical exercise lead to the necessity of increasing caloric (in accordance to energy output) and protein (based on the trophic needs of the organism) intake. Likewise, paying major attention to vitamin and mineral intake, specifically B vitamins and zinc and chromium, is required, in order to optimize carbohydrate metabolism, the ultimate limiting factor for sport performance. During the training phase, 60% of calories should come from carbohydrates, protein intake should be 1.2-2 g/kg/day and athletes should follow the recommendations of the food guide pyramid. During the pre-, per- and post-competition phase the healthy aspect of the diet passes to a second level, in order to obtain good sport performance and to guarantee a fast and effective recovery. Again, carbohydrates with a high or medium glycaemic index and water are the nutrients which have to be calculated more thoroughly. In conclusion, athletes have to follow a diet that is adequate to their higher energy output and to their higher metabolic turnover. The food guide pyramid is a graphic expression which facilitates the comprehension and following of a healthy diet. In the present article, the authors introduce the pyramid adapted to the characteristics of sports nutrition, with easy-to-follow practical recommendations regarding the kind and amounts of foodstuffs that should be consumed in order to cover nutrient needs of people who exercise regularly.

Abstract:Stroke triggers a systemic inflammatory response over the ensuing days after the cerebral insult. The age and comorbidities of the stroke population make them a vulnerable population for low muscle mass and sarcopenia, the latter being another clinical condition that is closely associated with inflammation, as shown by increased levels of pro-inflammatory biomarkers, including neutrophil-to-lymphocyte ratio (NLR). In this study, we evaluated the relationship between post-stroke NLR changes and muscle mass in a prospective cohort of acute ischemic stroke patients (n = 102) enrolled in the Muscle Assessment in Stroke Study Turkey (MASS-TR). Admission lumbar computed tomography images were used to determine the cross-sectional muscle area of skeletal muscles at L3 vertebra level and calculate the skeletal muscle index (SMI). The median (IQR) SMI was 44.7 (39.1-52.5) cm2/m2, and the NLR at admission and follow-up were 4.2 (3.0-10.5) and 9.4 (5.7-16.2), respectively. While there was no relationship between SMI and admission NLR, a significant inverse correlation was observed between SMI and follow-up NLR (r = - 0.26; P = 0.007). Lower SMI remained significantly associated (P = 0.036) with higher follow-up NLR levels in multivariate analysis. Our findings highlight the importance of muscle mass as a novel factor related to the level of post-stroke stress response.

Abstract:This study aimed to explore the limitations of the Ashworth scale for measuring spasticity. An isokinetic dynamometer to quantify resistance to passive stretch and surface EMG was used to verify if a stretch response occurred and, if so, at what joint angle. The authors sought to determine which components of passive resistance (magnitude, rate of change, onset angle of stretch, or velocity dependence) were most related to Ashworth scores and which were related to motor function in cerebral palsy (CP). Twenty‐two individuals with spastic CP (11 males, 11 females; mean age 11.9 years, SD 4.3) and a comparison group of nine children without CP (four males, five females; mean age 11.3 years, SD 2.5) participated in the study. The group with CP included those with a diagnosis of spastic diplegia, hemiplegia, or quadriplegia, distributed across Gross Motor Functional Classification Levels. Procedures included: (1) clinical assessment at the knee joint, (2) functional assessments, and (3) isokinetic assessment of passive resistance torque in hamstrings and quadriceps at three velocities. EMG data were recorded simultaneously to identify stretch responses. Detecting stretch responses using the Ashworth scale compared with instrumented measures showed near complete agreement at extremes of the scale, with marked inconsistencies in mid‐range values. Ashworth scores were correlated with instrumented measures, particularly for the quadriceps, with higher correlations to the rate of change in resistance (stiffness) and onset angle of stretch than to peak resistance torque. Those with greater resistance tended to have poorer function with isokinetic relations typically stronger.

Investigating Muscle Function after Stroke Rehabilitation with 31P-MRS: A Preliminary Study

Abstract:Background: New evidence reveals significant metabolic changes in skeletal muscle after stroke. However, it is unknown if 31P magnetic resonance spectroscopy (31P-MRS) can evaluate these metabolic changes. Our objective here was to investigate: (a) if muscle energy metabolism changes in the affected side; (b) if muscle energy metabolism changes after rehabilitation; and (c) if energy metabolism measured by 31P-MRS can reflect changes in the Modified Modified Ashworth Scale (MMAS) and Fugl-Meyer assessment-lower extremity (FMA-LE) scores after rehabilitation. Material/Methods: We enrolled 13 patients with stroke symptoms and hemiplegia. Lower-limb motor status on the affected side was evaluated by FMA-LE and MMAS. The 31P-MRS measures included phosphocreatine (PCr), inorganic phosphate (Pi), PCr/Pi, and pH. We statistically compared these measures in the affected and unaffected lower leg muscles before rehabilitation and after rehabilitation on the affected side. Spearman correlational analyses was performed to determine correlations between change in energy metabolism and change in FMA-LE score and MMAS score after rehabilitation. Results: PCr and PCr/Pi were significantly lower in the affected muscle compared to the unaffected muscle; however, there were no significant differences in Pi or pH. After rehabilitation, PCr, Pi, PCr/Pi, and pH did not significantly change. However, FMA-LE and MMAS score improved significantly after rehabilitation. Changes in energy metabolism measured by 31P-MRS had no correlation with FMA-LE change after rehabilitation. However, changes in PCr and PCr/Pi were correlated with change in MMAS score after rehabilitation. Conclusions: 31P-MRS can evaluate changes in muscle energy metabolism in patients with stroke. PCr measured by 31P-MRS can reflect changes in MMAS after rehabilitation.
