Abstract:Aims: To test whether circulating neurotoxic autoantibodies increase in adult type 2 diabetes mellitus with Parkinson’s disease (PD) or dementia. To identify the G-protein coupled receptor on neuroblastoma cells mediating neural inhibitory effects in diabetic Parkinson’s disease plasma autoantibodies. To determine the mechanism of accelerated neuroblastoma cell death and acute neurite retraction induced by diabetic Parkinson’s disease and dementia autoantibodies. Methods: Protein-A eluates from plasma of twelve older adult male diabetic patients having Parkinson’s disease (n=10) or dementia (n=2), and eight age-matched control diabetic patients were tested for ability to cause accelerated N2A neuroblastoma cell death and acute neurite retraction. Specific antagonists of G protein coupled receptors belonging to the G alpha q subfamily of heterotrimetric G-proteins, the phospholipase C/inositol triphosphate/Ca2+ pathway, or the RhoA/Rho kinase pathway were tested for ability to block diabetic Parkinson’s disease/dementia autoantibodyinduced neurite retraction or N2A accelerated cell loss. Sequential Liposorber LA-15 dextran sulfate cellulose/protein-A affinity chromatography was used to obtain highlypurified fractions of diabetic Parkinson’s disease autoantibodies . Results: Mean accelerated neuroblastoma cell loss induced by diabetic Parkinson’s disease or dementia autoantibodies significantly exceeded (P = 0.001) the level of N2A cell loss induced by an identical concentration of the diabetic autoantibodies in control patients without these two comorbid neurodegenerative disorders. Co-incubation of diabetic Parkinson’s disease and dementia autoantibodies with two-hundred nanomolar concentrations of M100907, a highly selective 5-HT2AR antagonist, completely prevented autoantibody-induced accelerated N2A cell loss and neurite retraction. A higher concentration (500 nM-10μM) of alpha-1 adrenergic, angiotensin II type 1, or endothelin A receptor antagonists did not substantially inhibit autoantibody-induced neuroblastoma cell death or prevent neurite retraction. Antagonists of the inositol triphosphate receptor (2-APB, 50μM), the intracellular calcium chelator (BAPTA-AM, 30 μM) and Y27632 (10 μM), a selective RhoA/Rho kinase inhibitor, each completely blocked acute neurite retraction induced by sixty nanomolar concentrations of diabetic Parkinson’s disease autoantibodies. Co-incubation with 2-APB (1-2 μM) for 8 hours’ prevented autoantibody-induced N2A cell loss. The highly-purified fraction obtained after Liposorber LA/protein-A affinity chromatography in hypertriglyceridemic diabetic dementia and Parkinson’s disease plasmas had apparent MWs > 30 kD, and displayed enhanced N2A toxicity requiring substantially higher concentrations of 5-HT2AR antagonists (M100907, ketanserin, spiperone) to effectively neutralize. Conclusion: These data suggest increased autoantibodies in older adult diabetes with Parkinson’s disease or dementia cause accelerated neuron loss via the 5-hydroxytryptamine 2 receptor coupled to inositol triphosphate receptor-mediated cytosolic Ca2+ release.
Effect of chemoradiotherapy on nutrition status of patients with nasopharyngeal cancer

Abstract:We aimed to assess the effect of chemoradiotherapy on the nutritional status of patients with nasopharyngeal cancer (NPC) and to detect the risk factors for poor nutrition status in NPC patients after radiotherapy. A total of 104 NPC patients participated in this clinical observational study. Psychological distress and nutritional indicators were measured prior to chemoradiotherapy. During the course of radiation therapy, side effect symptoms were assessed weekly. At the end of radiotherapy, nutritional indicators were measured again. Logistic regression was used to identify the risk factors for poor nutritional status after radiotherapy. The values of the 9 nutritional indicators were significantly lower after radiotherapy (P < 0.001) than the initial values before treatment. After radiotherapy, 20.19% of patients had more than 10% weight loss. At a significance level of α = 0.05, the risk factors for poor nutritional status were old age (P = 0.042), female gender (P < 0.001), late stage of the disease (P = 0.013), depression (P = 0.024), high side effect score (P = 0.007), and moderate nutritional status before radiotherapy (P = 0.015). Radiotherapy affects the nutritional status of NPC patients. To prevent malnutrition during radiotherapy, nutritional assessment and intervention should be an integral part of treatment.
The prognostic value of weight loss during radiotherapy among patients with nasopharyngeal carcinoma: a large-scale cohort study

Abstract:Background We aim to investigate the prognostic value of weight loss during radiotherapy (RT) among patients with nasopharyngeal carcinoma (NPC). Methods A total of 1149 NPC patients who received radical RT were retrospectively analyzed. Patients' weight were measured at initiation of RT (W Pre-RT ) and every week during RT (W RT1,2,3,4,5,6,7 ). Percentage of weight loss (PWL) at 1st, 2nd, 3rd, 4th, 5th, 6th, and 7th week of RT (RT-PWL 1,2,3,4,5,6,7 ) were calculated using the following equation: (W Pre-RT –W RT1,2,3,4,5,6,7 )/W Pre-RT × 100%. The optimal threshold of RT-PWL 7 was determined by recursive partitioning analyses (RPAs). Our endpoints included disease-free survival (DFS), overall survival (OS), distant metastasis-free survival (DMFS), and locoregional relapse-free survival (LRRFS). Results The median RT-PWLs were 0, 0, 1.5, 2.9, 4.1, 5.5, 6.6% at 1st, 2nd, 3rd, 4th, 5th, 6th, and 7th week of RT, respectively. RT-PWL 7 optimal threshold with respect to DFS was 5.3% based on RPAs. Therefore, a consistent threshold of 5% ( ≥5%) was selected to classify NPC patients into low RT-PWL 7 and high RT-PWL 7 groups for survival analysis. Compared to high RT-PWL 7 (≥5%), patients with low RT-PWL 7 (< 5%) had significantly better ten-year DFS (61.2% vs 78.8%; P < 0.001), OS (70.1% vs 86.6%; P < 0.001), and DMFS (80.2% vs 88.5%; P = 0.007). However, no difference was observed between LRRFS groups (91.7% vs 94.3%; P = 0.173). In multivariate analysis, high RT-PWL 7 was an independent risk factor for DFS (HR, 1.56; 95%CI, 1.19-2.03; P = 0.001), OS (HR, 1.54; 95%CI, 1.11-2.15; P = 0.011), and DMFS (HR, 1.47; 95%CI, 1.03-2.10; P = 0.033) in patients with NPC. In addition, treatment strategy, plasma Epstein-Barr virus DNA, and N stage were associated with weight loss. Conclusions High RT-PWL 7 was significantly associated with decreased DFS, OS, and DMFS for NPC patients. Clinicians should continuously inform patients on the health impact of minimizing RT-PWL 7 under 5% during radiotherapy.

Abstract:To quantitate changes in regional left ventricular function induced by ischemia or scar, rest and exercise equilibrium radionuclide studies of 26 patients with one vessel coronary artery disease and 12 normal individuals were analysed with a new method for regional ejection fraction determination. A computer algorithm provided observer-independent segmental analysis from a centre of gravity of the left ventricular activity at end-diastole (left anterior oblique projection). Special segments were assigned for anteroseptal, inferoapical and posterolateral areas corresponding to the three main coronary arteries. Reproducibility using an unchanged camera positioning was excellent even for 2 min acquisition studies (r=0.93) and still good after repositioning (r=0.80 to 0.87). In normal areas, regional ejection fraction increased or showed no change during exercise. In contrast, it decreased significantly in regions supplied by stenosed coronary arteries (ischemia) and remained depressed in scar zones. The method proved to be valid for regional changes induced by left anterior descending and left circumflex coronary obstructions, but less for right coronary artery lesions. Global ejection fraction reflected a sum of all regional changes implying that regional analysis should be more sensitive in detecting coronary artery disease.
Weight Loss in Post-Chemoradiotherapy Head and Neck Cancer Patients

Abstract:Background: Weight loss is well-known among head and neck (HNC) patients during radiotherapy and could continue after the treatment is completed. Weight monitoring is essential for treatment outcomes and cancer surveillance. The purpose of the study is to evaluate the weight loss during and post-treatment among HNC patients. Methods: A total of 45 out of 50 patients post-treatment were included in this secondary treatment. Data were collected at baseline, at the completion of radiotherapy and one month after completion of radiotherapy. Results: The mean weight loss was 4.53 ± 2.87 kg (7.4%) during treatment and 1.38 ± 2.65 kg (2.1%) post-treatment. There were significant improvements one month after completion of radiotherapy in Patient-Generated Subjective Global Assessment (PGSGA), muscle mass, nutrition impact symptoms (NIS) score, energy and protein from oral intake (p < 0.0001). Energy and protein from oral nutritional supplements (ONS) decreased significantly (p < 0.0001). Conclusions: The results of this study underline the importance of early identification and monitoring post-treatment in HNC patients. The post-treatment recovery stage is very important for HNC patients to ensure a healing process.

Abstract:Isolated frog muscles were equilibrated in solutions of varying pH and were then analyzed for potassium, sodium and sometimes chloride, phosphate, or bicarbonate. It was found that acid applied outside the muscle causes in most cases an increased loss of K. Over the physiological pH range this was also true of Na in experiments in which the complication of the extracellular space was minimized by replacing 90% of the NaCl of the solution by sucrose. In the other experiments the variations were such that it was impossible to establish any regular effect of pH on the movements of sodium. Except at a pH below 2 or above 12 the Na did not appear to exchange with K in response to injury. It is suggested that the Na pump in muscle works better when the Na is being moved into a more acid solution. It is concluded that muscle neutralizes external acid in part by loss of K, sometimes by loss of Na, and to some extent by gain of chloride. There was a slightly increased loss of phosphate and bicarbonate from muscle in the more acid solutions but no evidence of an increase in the extracellular fluid as measured by inulin.
Critical weight loss during radiotherapy and its influencing factors in patients with head and neck cancer

Abstract:Objective:To observe the critical weight loss(weight loss rate>5%) in patients with head and neck cancer (HNC) during radiotherapy and analyze its influencing factors.Methods:Patients with head and neck cancer treated with radiotherapy in a cancer hospital in Beijing from March 2017 to September 2019 were recruited. The day before the beginning of radiotherapy, general information was collected, and Nutritional Risking Screening 2002 (NRS2002) was used as a nutritional screening tool. Malnutrition was assessed by criteria: 1.the score of normal nutritional status in NRS2002 was 3; 2.body mass index<18.5 kg/m 2 with poorer general condition; 3.Global leadership initiative on malnutrition criteria was used except whole body muscle mass measurement. Dietary intake was recorded by 24-hour dietary recall. The weight of patients before and after radiotherapy was recorded. Results:435 patients were completely investigated. The average weight of patients was 65.52 kg±12.20 kg before radiotherapy, and 61.01 kg±11.17 kg after radiotherapy. The weight loss was 4.50 kg±3.17 kg(-3.20~20.90 kg), and the weight loss rate was 6.72%±4.34 %(-5.19%~25.49%). 66.0% of patients had critical weight loss. Multifactor logistic regression analysis showed that the influencing factors of critical weight loss were planned radiotherapy dose ( OR=1.069, 95% CI=1.014~1.127, P=0.013), concurrent chemoradiotherapy ( OR=1.798, 95% CI=1.148~2.816, P=0.010), baseline malnutrition ( OR=2.693, 95% CI=1.223~5.933, P=0.014) and nasopharyngeal tumors ( OR=2.059, 95% CI=1.158~3.661, P=0.014). Conclusions:HNC patients experience significant critical weight loss during radiotherapy. Patients with more planned radiotherapy dose, concurrent chemoradiotherapy, nasopharyngeal tumors and without malnutrition at baseline are at high risk of having critical weight loss. It is suggested that clinical medical staff should pay more attention to patients' weight loss during radiotherapy, give corresponding preventive measures, strengthen nutrition education, and increase the intake of energy and protein and provide nutritional support when necessary to improve the critical weight loss.

Abstract:Background: Weight loss during chemoradiotherapy is a major problem in patients with head and neck cancer. The aim of this study was to evaluate the effect of ONS on weight, nutritional status and quality of life (QOL) in patients with loco-regionally advanced nasopharyngeal cancer (NPC) undergoing chemoradiotherapy.Methods: Patients with locally advanced NPC treated at a tertiary hospital in China prior to curative chemoradiotherapy were eligible for this exploratory randomized study. Patients were assigned to either the intervention or the control group based on a computer-generated randomization sequence. The intervention group commenced ONS at the start of chemoradiotherapy. Outcomes included body weight, BMI, nutritional status and QOL.Results: From June 2015 to June 2016, 50 patients with NPC were randomized to intervention and 50 to the control group. Patients in the ONS group had a higher body weight at the end of chemoradiotherapy (59.11kg vs 58.14kg, p=0.036). A higher BMI and prealbumin were observed in the ONS group (p=0.021 and p=0.048, respectively). No other differences were found for nutritional status, QOL or clinical outcomes.Conclusion: ONS had beneficial outcomes in terms of reducing weight loss, minimizing BMI decrease and increasing protein intake in loco-regionally advanced NPC patients during chemoradiotherapy.

Abstract:e18002 Background: To investigate the influence of neoadjuvant chemotherapy (NACT) plus concurrent chemoradiotherapy (CCRT) on nutritional status in locoregionally advanced nasopharyngeal carcinoma (LANPC) patients. Methods: Patients were recruited from ten hospitals. All patients received NACT plus CCRT. Nutrition related parameters including weight loss (WL), body mass index (BMI), Nutrition Risk Screening 2002 (NRS2002), Patient-Generated Subjective Global Assessment (PG-SGA), and Quality of life (QoL) score (EORTC QLQ-C30) as well as toxicity were recorded before, during, and after treatment. Statistical analyses are using SPSS 22.0. The study was registered on ClinicalTrials.gov, No. NCT02575547. Results: From Jun 2015 to Nov 2016, 186 patients enrolled, of whom 171 were eligible for analysis. The overall incidence of mild and severe malnutrition was 68.5% and 37.4%, as shown in Table 1. Patients with ≥ 2 grade mucositis have higher risks of WL ≥ 10% (P = 0.037), NRS2002 ≥ 3 points (P = 0.002) and PG-SGA ≥ 9 points (P < 0.001). 97.7%, 87.7% and 99.4% patients finished NACT, CCRT and radiotherapy, respectively. Patients with PG-SGA ≥ 9 points and C30-QoL < 50 points had poor chemotherapy compliance (P = 0.014, P < 0.001), and BMI < 18.5 had poor radiotherapy compliance (P = 0.036). Conclusions: Malnutrition was prevalent in LANPC patients treated with NACT plus CCRT, especially during CCRT, even lasted until 1 year after treatment. Its impact on clinical outcomes need long-term follow-up. Abstract:This study aimed to quantify dosimetric effects of weight loss for nasopharyngeal carcinoma (NPC) treated with intensity-modulated radiation therapy (IMRT). Overall, 25 patients with NPC treated with IMRT were enrolled. We simulated weight loss during IMRT on the computer. Weight loss model was based on the planning computed tomography (CT) images. The original external contour of head and neck was labeled plan 0, and its volume was regarded as pretreatment normal weight. We shrank the external contour with different margins (2, 3, and 5mm) and generated new external contours of head and neck. The volumes of reconstructed external contours were regarded as weight during radiotherapy. After recontouring outlines, the initial treatment plan was mapped to the redefined CT scans with the same beam configurations, yielding new plans. The computer model represented a theoretical proportional weight loss of 3.4% to 13.7% during the course of IMRT. The dose delivered to the planning target volume (PTV) of primary gross tumor volume and clinical target volume significantly increased by 1.9% to 2.9% and 1.8% to 2.9% because of weight loss, respectively. The dose to the PTV of gross tumor volume of lymph nodes fluctuated from −2.0% to 1.0%. The dose to the brain stem and the spinal cord was increased (p < 0.001), whereas the dose to the parotid gland was decreased (p < 0.001). Weight loss may lead to significant dosimetric change during IMRT. Repeated scanning and replanning for patients with NPC with an obvious weight loss may be necessary.

Abstract:Prader-Willi syndrome (PWS) is caused by the loss of expression of imprinted genes in chromosome 15q11-q13. Affected individuals exhibit neonatal hypotonia, developmental delay and childhood-onset obesity. Necdin, a protein implicated in the terminal differentiation of neurons, is the only PWS candidate gene to reduce viability when disrupted in a mouse model. In this study, we have characterized MAGEL2 (also known as NDNL1), a gene with 51% amino acid sequence similarity to necdin and located 41 kb distal to NDN in the PWS deletion region. MAGEL2 is expressed predominantly in brain, the primary tissue affected in PWS and in several fetal tissues as shown by northern blot analysis. MAGEL2 is imprinted with monoallelic expression in control brain, and paternal-only expression in the central nervous system as demonstrated by its lack of expression in brain from a PWS-affected individual. The orthologous mouse gene (Magel2) is located within 150 kb of NDN:, is imprinted with paternal-only expression and is expressed predominantly in late developmental stages and adult brain as shown by northern blotting, RT-PCR and whole-mount RNA in situ hybridization. Magel2 distribution partially overlaps that of NDN:, with strong expression being detected in the central nervous system in mid-gestation mouse embryos by in situ hybridization. We hypothesize that, although loss of necdin expression may be important in the neonatal presentation of PWS, loss of MAGEL2 may be critical to abnormalities in brain development and dysmorphic features in individuals with PWS.
The association analysis between fatigue and body composition loss in patients with nasopharyngeal carcinoma during radiotherapy: An observational longitudinal study

Abstract:Purpose: This study aimed to reveal the association of fatigue with weekly changes in the body composition in patients with nasopharyngeal carcinoma (NPC) and identified the independent strength. Methods: Four body composition indexes and fatigue were assessed before treatment (T0, baseline) and once a week throughout radiotherapy (T1-T7). Generalized additive mixed models (GAMMs) were used to explore the trajectories and longitudinal relationships of fatigue and weekly changes in body composition. The marginal structural model (MSM) was used to control the effect of depression and anxiety. Results: The trajectories of fatigue in 105 participants reached a peak in the fifth week, and changes in body composition started appearing from the second week. Four body composition indexes, weight, body mass index (BMI), body fat rate, and lean body weight loss weekly were positively associated with fatigue. High magnitude of effects was revealed when anxiety and depression were controlled as time-dependent confounders. The positive associations with fatigue were manifested in patients aged >53 years, those with senior high and above education, no drinking, >5000 Y/month of family inflow, ≥ stage III, or those receiving a dose of ≥70 Gy, ≥3 cycles of induced chemotherapy, and ≤1 cycle of concurrent chemotherapy. Conclusions: Loss of weight, BMI, body fat rate, and lean body weight could be used to independently evaluate the development of fatigue in patients with NPC during radiotherapy. Positive associations between fatigue and weekly body composition loss were found in patients with certain characteristics.
A prospective analysis of factors that influence weight loss in patients undergoing radiotherapy

Abstract:Malnutrition occurs frequently in patients with cancer. Indeed, a variety of nutritional and tumor-related factors must be taken into account in these patients. Recognizing this relationship, we aimed to prospectively evaluate the risk factors that influence weight loss in patients undergoing radiotherapy with oral nutritional supplementation and dietetic counseling. Weight loss of 74 patients during radiotherapy and 1 month after treatment was analyzed. Parameters such as age, gender, tumor location, tumor stage, Eastern Cooperative Oncology Group performance status (ECOG PS) score, and the use of chemotherapy were analyzed to evaluate their influence on weight loss. All patients underwent oral nutritional supplementation and dietetic counseling. Forty-six (65.7%) patients lost weight, with a mean weight loss of (4.73 ± 3.91) kg, during radiotherapy. At 1 month after treatment, 45 (66.2%) patients lost weight, presenting a mean weight loss of (4.96 ± 4.04) kg, corresponding to a (6.84 ± 5.24)% net reduction from their baseline weight. Head and neck cancer patients had a mean weight loss of (3.25 ± 5.30) kg, whereas the remaining patients had a mean weight loss of (0.64 ± 2.39) kg (P = 0.028) during radiotherapy. In the multivariate analysis, the head and neck tumor location (P = 0.005), use of chemotherapy (P = 0.011), and ECOG PS score of 2-3 (P = 0.026) were considered independent risk factors. Nutritional status and parameters, such as tumor location (especially the head and neck), the use of chemotherapy, and the ECOG PS score, should be evaluated before radiotherapy because these factors can influence weight loss during radiotherapy and 1 month after treatment.
Longitudinal Body Composition Changes and the Importance of Fat-Free Mass Index in Locally Advanced Nasopharyngeal Carcinoma Patients Undergoing Concurrent Chemoradiotherapy

Abstract:Aim: This was a prospective investigation of longitudinal body composition changes in patients with nasopharyngeal carcinoma undergoing concurrent chemoradiotherapy (CCRT) and a comparison of the Patient-Generated Subjective Global Assessment (PG-SGA) and the ESPEN (European Society for Clinical Nutrition and Metabolism) diagnostic criteria (EDC) as evaluation methods. Methods: All patients received standard CCRT according to 2 centers’ current practices. Body composition parameters were determined by bioelectrical impedance analysis and obtained weekly from baseline until the end of treatment. The nutritional status of all patients was evaluated by the PG-SGA and EDC. Results: Forty-eight patients were eligible for analysis. Most body composition parameters, including body cell mass, fat mass, fat-free mass, and skeletal mass, as well as body weight, body mass index, and PG-SGA score, significantly decreased during CCRT ( P = .00). The PG-SGA was shown to have better sensitivity than the EDC; however, the 2 different evaluation methods were found to have a perfect concordance at Week 4 and Week 6 (κ = 0.91 and 0.96, P = .00 and .00, respectively). Pearson correlation analyses showed that fat-free mass index and body weight were positively correlated with global quality of life score ( r = 0.81, P = .00; r = 0.78, P = .00, respectively). Conclusions: This study has shown that body composition parameters, especially fat-free mass index, are valuable for diagnosing malnutrition in patients with nasopharyngeal carcinoma receiving CCRT. We recommend that these bioelectrical impedance analysis techniques should be increasingly implemented in nutritional assessments.
Evaluating Nutritional Health in Cases of Head and Neck Cancer Receiving Radiotherapy

Abstract:Introduction Patients diagnosed with head and neck cancers (HNC) are highly susceptible to malnutrition, with initial rates ranging from 35% to 60%, which can further worsen during treatment, leading to significant weight loss in up to 80% of patients. Malnutrition often remains undetected, contributing to increased morbidity, mortality, and healthcare expenses. This study sought to evaluate the nutritional status of patients with head and neck cancer receiving radiotherapy and investigate its relationship with factors related to the disease and treatment.  Materials and Methods The study is a longitudinal observational research involving 31 patients who were treated at the Radiation Oncology Unit of the National Institute of Oncology in Rabat, Morocco, between October 2023 and March 2024. Patients underwent radiation therapy using the Volumetric Modulated Arc Therapy (VMAT) technique, either alone or in combination with chemotherapy. Data, including anthropometric measurements, PG- SGA assessments, and RTOG criteria evaluations, were gathered at the start of treatment, at the end of treatment, and three months after the treatment ended.  Results The cohort consisted of 28 males and 3 females, with a mean age of 56 ± 8 years. The majority of tumors were located in the larynx (32%) and nasopharynx (30%), with 78% of patients presenting with stage III or IV cancer. Initially, 88% of patients were well- nourished; however, by the end of treatment, 79% were malnourished. The average PG-SGA score rose from 2.8 to 11.3. There was a marked decrease in food intake, with only 40.2% of patients consuming more than half of their meal portions by the end of treatment. Weight loss occurred in 85% of patients, with 27% experiencing a loss of over 10% of their initial body weight. Patients with advanced-stage cancer, oral cavity and pharyngeal cancers, and those undergoing chemo-radiotherapy were more likely to be malnourished. Higher radiation doses were associated with increased pain and swallowing difficulties.  Conclusion Radiotherapy significantly worsens the nutritional status of HNC patients, particularly by the end of treatment. The most severe nutritional declines were observed in patients with oral cavity and pharyngeal cancers, advanced-stage disease, concurrent chemotherapy, and higher radiation toxicity. Incorporating routine nutritional assessments into clinical practice and assigning dedicated staff for these evaluations could enable timely nutritional interventions.

Abstract:6036 Background: We conducted a multicenter prospective study (NCT02575547) to investigate the association between longitudinal nutritional status and survival in locally advanced nasopharynx cancer (LA-NPC) treated with induction chemotherapy and concurrent chemoradiotherapy (IC-CCRT). Methods: All patients with biopsy-proven LA-NPC and planned for IC-CCRT were recruited from ten institutions. IC entailed 2 cycles of docetaxel 75mg/m 2 /3w and cisplatin 75mg/m 2 /3w; CCRT entailed 2-3 cycles cisplatin 100mg/m 2 /3w and IMRT (70-72Gy/30-32fr). Study parameters included weight loss (WL), % of ideal body weight (%IBW), body mass index (BMI), nutrition risk screening 2002 (NRS2002), patient-generated subjective global assessment (PG-SGA), and EORTC QLQ-C30 that were collected at the following time-points: baseline (T1), 1 w pre-2 nd IC (T2), 1 w pre-CCRT (T3), 4 w mid-CCRT (T4), end-CCRT (T5), 3 mo post-CCRT (T6), 1 y post-CCRT (T7), 2 y post-CCRT (T8). Results: 186 patients were recruited; 171 were eligible for analysis. Median follow-up was 35.8 mo (range 12.3-46.1 mo). Compliance rates were 97.7% (167/171) and 87.7% (150/171) for IC and CCRT, respectively; all except one completed RT. Longitudinal assessment indicated the worst nutritional status at T5, followed by recovery at T8: 27.1% with %IBW <90%; 69.2% with WL ≥5%, which was also associated with a worsened QOL (OR = 6.23 for QOL change ≥25.0, P = 0.012). Interestingly, T1 nutritional status was not associated with prognosis (P >0.05 for all). However, nutritional parameters at T5 were significantly associated with survival; %IBW <90% was the strongest predictor for inferior DMFS (HR = 2.669) and OS (HR = 4.661) among all parameters (multivariable-adjusted P <0.05). Subgroup analyses revealed that %IBW <90%, WL ≥10% at T5 represented the most adverse subset of patients (Table). Conclusions: Here, we show that poor nutrition despite systemic intensification leads to inferior QOL and disease control in LA-NPC patients. This is counter-intuitive and highlights the crucial importance of paracrine factors in optimising treatment efficacy. Clinical trial information: NCT02575547. [Table: see text]

Abstract:OBJECTIVE:To investigate the association of nutritional status with treatment compliance and toxicities in patients undergoing chemoradiation therapy (CRT) after gastrectomy. METHODS:From September 2010 to May 2012, 40 patients with gastric cancer received adjuvant CRT in the Department of Radiation, Shanghai Cancer Center. Data including clinical data, weight loss of perioperative period, dynamic changes of weight, NRS 2002 score, PG-SGA score, lymph cell count and serum albumin during CRT, toxic effects and nutritional interventions were collected. Treatment compliance of CRT and adjuvant chemotherapy was recorded. Associations among nutrition, toxicities and treatment compliance were statistically studied. RESULTS:Weight loss percentage from pre-operation to pre-CRT(T1-T2) was 10.0%, which was significantly higher than that of 4.3% during CRT(T3) (P<0.05). Adverse reaction incidence of digestive tract during T3 was 95.0% (38/40). Patients with weight loss >5% during T3 had higher ratio of >II degree digestive tract adverse reaction [91.3% (21/23) vs. 76.5% (13/17), P<0.01] and higher ratio of >3 symptoms of digestive tract[82.4% (14/17) vs. 39.1% (9/23), P<0.05] as compared to those with weight loss ≤5% during T3. Fourteen patients (35.0%) did not complete the synchronous CRT. Factors related to incompletion of CRT were weight loss >7% after surgery (T1) or >10% during T1-T2, malnourishment before CRT, dependence on nutritional support during CRT. Factors related to incompletion of adjuvant chemotherapy were weight loss >5% during CRT(T3), requirement for nutritional support and NRS 2002 score ≥5 at the end of radiation (all P<0.05). CONCLUSIONS:Nutritional deterioration before CRT may aggravate the toxicities and reduce compliance of CRT in patients with radical resection of gastric cancer. Malnutrition during CRT may impair compliance to adjuvant chemotherapy. Therefore, early and persistent nutritional interventions are crucial considerations of strategies of multidisciplinary treatment for patients with gastric cancer.

Abstract:Objective To investigate the relationship between nutritional status and quality of life ( QOL) in patients with nasopharyngeal carcinoma undergoing concurrent chemoradiotherapy, and to identify the optimal indices of body composition for evaluation of malnutrition. Methods Based on the European Society Parenteral and Enteral Nutrition ( ESPEN) consensus on the diagnosis of malnutrition and patient?generated subjective global assessment (PG?SGA), a prospective study was performed in 48 patients with nasopharyngeal carcinoma who received concurrent chemoradiotherapy in our hospital from 2014 to 2015. Changes in body composition and nutritional status were evaluated in those patients. The relationship between nutritional status and QOL was studied. The predictive factors for nutritional status in patients with nasopharyngeal carcinoma were explored. The correlation between datasets was evaluated by the Pearson correlation coefficient. The influencing factors for nutritional status were analyzed by Logistic regression. Results During concurrent chemoradiotherapy, some indices of body composition, including weight, body mass index ( BMI) , fat mass index ( FMI) , fat?free mass index ( FFMI) , body cell mass ( BCM) , skeletal muscle mass ( SM) , and phase angle ( PA) had different degrees of reduction ( P=0?00) , while the PG?SGA score gradually increased. The incidence of malnutrition in patients with nasopharyngeal carcinoma during radiotherapy was 2?1%?39?6% based on 2015 ESPEN consensus on the diagnosis of malnutrition and 12?5%?41?7% based on PG?SGA. These two methods showed good agreement at the fourth and sixth weeks of radiotherapy ( Kappa=0?911;Kappa=0?957) . The changes in QOL score were correlated with changes in FFMI and weight during radiotherapy ( r= 0?805, P= 0?00;r= 0?777, P= 0?00 ) . Logistic regression indicated that age, FMI, and FFMI were influencing factors for nutritional status ( P= 0?035, 0?013, 0?043) . Conclusions Patients with nasopharyngeal carcinoma have substantial nutritional deterioration during chemoradiotherapy. The nutritional status is closely associated with QOL. A prediction model of nutritional status can provide a comprehensive and accurate judgment of nutritional status in patients.

Abstract:Introduction This systematic review and meta-analysis evaluated the quality of life (QoL) for nasopharyngeal carcinoma (NPC) patients with radiotherapy. Methods A systematic literature search was performed to identify relevant studies published until March 2022. Quality evaluation and data extraction were performed for the included studies, and meta-analysis was performed using Stata. Results Nine studies, including 1659 patients, were eligible. Most QoL scales developed at the end of the treatment course and then followed by a gradual recovery to 1 year and more than 1 year after treatment. However, some items have not changed significantly and have a deteriorating trend. Items of cognitive functioning and constipation in EORTC QLQ-C30, and sexuality, felt ill, and weight gain in EORTC QLQ- H&N35 showed that scales with follow-up of more than 1 year were worse than those within 1 year but still better than those after treatment. In the intensity-modulated radiotherapy (IMRT) subgroup in EORTC QLQ-C30, cognitive functioning was similar to those before, and there was no significant change in insomnia. There was no significant change in the teeth item in EORTC QLQ- H&N35. In the IMRT subgroup, scales of swallowing, felt ill, and weight gain with follow-up of more than 1 year were worse than those within 1 year. Conclusion The QoL of patients with NPC is significantly impaired after radiotherapy-treated compared to baseline, and most of these items will gradually improve.
