Abstract:Objective:To analyze the common condition of medical examination in infant,and approach the effect factors of low birth weight on newborn. Methods:Age,nation,education,occupations,smoking and drinking,medical history,family history of high blood pressure,physical examination of parents,and progesterone production history,complication of pregnancy,delivery situation of pregnant woman,birth status and physical examination of newborn were investigated and described,and the affect factors of low birth weight were analyzed by adopt binary logistic regression model. Results:The result of single factor analysis shown: remote home address,national minority of mother,early termination of pregnancy time,low education level,physical work occupations and low BMI of parents,medical history of father,cooking oil type,eat uneven proportion fat and lean,lack of vitamin supplements,eat salt too much or very few,eat few eggs and fish every week,other family member smoking,bad along situation of mother in pregnancy and intrapartum complications,few prenatal examination times,low weight gain during pregnancy,twins or multiple birth were the affect factors of low birth weight.The result of multivariate logistic regression shown: few antenatal examination number,early termination of pregnancy time,disorder in pregnancy such as gestational hypertension and infection were the affect factors of low birth weight. Conclusion:Relevant factors of mother in pregnancy can effect low birth weight of infants.To improve medical examination during pregnancy and medical propaganda education can reduce the occurrence of low birth weight.
[An Epidemiologic Investigation of Newborns from Obstetric Departments in the Central South Region of China].

Abstract:OBJECTIVE:To investigate the birth information of newborn infants from obstetric departments in the Central South Region of China.METHODS:A retrospective investigation was carried out in 15582 newborns from obstetric departments of 23 hospitals in the Central South Region of China between January 1 and December 31 of 2005.RESULTS:The sex ratio (male/female) of neonates was 1.16∶1. The proportion of preterm infants was 8.11%. The very low birth weight infants accounted for 0.73%. The neonates born by spontaneous labor accounted for 57.52%. Cesarean sections accounted for 40.82% (social factor of cesarean section: 29.91%). The incidence of neonatal asphyxia was 3.78%, in which 0.75% of the cases were severe asphyxia. The mortality of newborn infants was 0.55%, in which the mortality of preterm infants was 5.56%.CONCLUSIONS:The proportion of preterm infants and the incidence of neonatal asphyxia is high in the Central South Region of China. The proportion of births delivered by cesarean section is high, and social factors are probably responsible for the high rate.

Abstract:BACKGROUND:With the progress of perinatal medicine and neonatal technology, more and more extremely low birth weight (ELBW) survived all over the world. This study was designed to investigate the short-term outcomes of ELBW infants during their Neonatal Intensive Care Unit (NICU) stay in the mainland of China.METHODS:All infants admitted to 26 NICUs with a birth weight (BW) < l000 g were included between January l, 2011 and December 31, 2011. All the data were collected retrospectively from clinical records by a prospectively designed questionnaire. The data collected from each NICU transmitted to the main institution where the results were aggregated and analyzed. Categorical variables were performed with Pearson Chi-square test. Binary Logistic regression analysis was used to detect risk factors.RESULTS:A total of 258 ELBW infants were admitted to 26 NICUs, of whom the mean gestational age (GA) was 28.1 ± 2.2 weeks, and the mean BW was 868 ± 97 g. The overall survival rate at discharge was 50.0%. Despite aggressive treatment 60 infants (23.3%) died and another 69 infants (26.7%) died after medical care withdrawal. Furthermore, the survival rate was significantly higher in coastal areas than inland areas (53.6% vs. 35.3%, P = 0.019). BW < 750 g and GA < 28 weeks were the largest risk factors, and being small for gestational age was a protective factor related to mortality. Respiratory distress syndrome was the most common complication. The incidence of patent ductus arteriosus, intraventricular hemorrhage, periventricular leukomalacia, bronchopulmonary dysplasia, retinopathy of prematurity was 26.2%, 33.7%, 6.7%, 48.1%, and 41.4%, respectively. Ventilator associated pneumonia was the most common hospital acquired infection during hospitalization.CONCLUSIONS:Our study was the first survey that revealed the present status of ELBW infants in the mainland of China. The mortality and morbidity of ELBW infants remained high as compared to other developed countries.
Analysis of trends of birth weight, and incidence of adverse birth outcomes and relevant factors from 2002 to 2015

Abstract:Objective To describe the trends of birthweight from 2002 to 2015,and analyze the incidence of adverse birth outcomes and to explore their relevant factors.Methods Datasets were obtained from decoded Electronic medical recorder system (EMRS) of Zhoushan,trends of birthweight were described,incidence of adverse birth outcomes were analyzed.Multivariate Logistic regression was used to analyze relevant factors.Results Mean birth weight of 37 141 newborns were (3 388 ± 462) g,and incidence of low birth weight,macrosomia,preterm birth were 2.34％,9.42％ and 3.61％,respectively.Girls had higher risk of low birth weight than boys.Pre-pregnancy underweight or overweight,high risk pregnancy (OR =1.80,95％ CI:1.42-2.28) were risk factors of low birth weight.High levels of maternal education reduced the risk of low birth weight;girls had lower risk of macrosomia than boys.Pre-pregnancy overweight and obese increased the risk of macrosomia.Born in summer or fall,maternal age ＜ 25 and pre-pregnancy underweight reduced the risk of macrosomia.Girls had lower risk of preterm birth than boys.Maternal age ＜ 25 reduced the risk of preterm,maternal age ＞30 (OR =1.38,95％ CI:1.19-1.60),born in winter (OR =1.24,95％ CI:1.06-1.45),pre-pregnancy overweight or obese and high risk pregnancy (OR =1.95,95％ CI:1.66-2.29) increased risk of preterm birth.Conclusions During 2002-2015,mean birth weight tends to decrease,incidence of low birth weight and preterm birth tend to increase.Incidence of macrosomia or low birth weight and preterm birth were relevant with newborn's gender,maternal age,prepregnancy body mass index (BMI) and high risk pregnancy.

Abstract:The aim of this study is to investigate the association between maternal gestational weight gain (GWG) and preterm birth according to pre-pregnancy body mass index (BMI) and maternal age. We did a cohort, hospital-based study in Quzhou, South China, from 1 Jan 2018 to 30 June 2019. We selected 4274 singleton live births in our analysis, 315 (7.4%) of which were preterm births. In the overall population, excess GWG was significantly associated with a decreased risk of preterm birth compared with adequate GWG (adjusted OR 0.81 [95% CI 0.72–0.91]), and the risk varied by increasing maternal age and pre-pregnancy BMI. Interestingly, underweight women who older than 35 years with excess GWG had significantly increased odds of preterm birth compared with adequate GWG in underweight women aged 20–29 years (2.26 [1.06–4.85]) and normal weight women older than 35 years (2.23 [1.13–4.39]). Additionally, low GWG was positively and significantly associated with preterm birth overall (1.92 [1.47–2.50]). Among normal weight women category, compared with adequate GWG women aged 20–29 years did, those older than 20 years with low GWG, had significantly higher odds of preterm birth, which increased with maternal age (1.80 [1.16–2.79] in 20–29 years, 2.19 [1.23–3.91] in 30–34 years, 3.30 [1.68–6.46] in ≫ 35 years). In conclusion, maternal GWG was significantly associated with the risk of preterm birth, but the risk varied by pre-pregnancy BMI and maternal age.
An epidemiological survey on low birth weight infants in China and analysis of outcomes of full-term low birth weight infants

Abstract:Background: Low birth weight (LBW) is one of the leading causes of adverse perinatal outcomes and is closely related to neonatal disease and death. The incidence of LBW has been increasing. The aim of this study was to investigate the current incidence rate and factors affecting low birth weight infants and perinatal outcomes of full-term low birth weight infants in mainland China. Methods: This paper describes a retrospective analysis of children born in 39 hospitals of different types in 14 different provinces, municipalities, and autonomous regions in seven districts within China throughout 2011. The data were first collected in hardcopy format and then entered into computer network databases. Data covering a total of 112,441 cases were collected. Cases were excluded if data were incomplete and in the case of miscarriage before 24 weeks of gestation, multiple pregnancies, or induction of labor due to fetal malformation, intrauterine death, and other reasons, leaving a total of 101,163 cases. SPSS 18.0 and SAS 9.2 statistical packages were used to analyze the collected data. Results: According to this research, the incidence of LBW in mainland China was 6.1%, which is higher than the 5.87% reported in 2000, and it varied across different areas. The incidence of LBW was significantly higher in tertiary care hospitals than in secondary care hospitals. LBW was found to be associated with maternal age of less than 20 years, low level of maternal education, previous histories of adverse pregnancies, and with pregnancy comorbidities and complications, such as hypertensive disorders during pregnancy, anemia, oligohydramnios, premature rupture of membranes, and gestational diabetes. The rates of stillbirths, severe neonatal asphyxia, and deaths among full-term LBW infants were 2.42%, 0.83%, and 3.49%, respectively. The rates of stillbirths and neonatal deaths among full-term LBW infants born by caesarean section were 0.5% and 1.0%, respectively, which was lower than vaginal delivery. Conclusions: The incidence of LBW has increased in China. LBW is a leading cause of adverse pregnancy outcomes. Health care during pregnancy and management of high-risk factors for LBW may reduce the incidence of LWB and the death rate of LBW infants.
Risk Factors of Low Birth Weight Infants:A Case-control Study in Shanghai

Abstract:[Objective]To study the risk factors of the term low birth weight infants in Changning District of Shanghai. [Methods] 88 term low birth weight single birth newborns,whose birth weight were less than 2500g and gestational weeks were more than 37 weeks,were taken as the case group.And 113 normal single birth infants,whose birth weight were more than 2500g and gestational weeks were more than 37 weeks,were taken as the control group.The case group and control group all bern in Health Care Center for Maternal and Children of Changning District between Jan.1~(st),2006 and Dec.31~(st),2006 were investigated based on questionnaire.A binomial Logistic regression was used to analyze the related factors of birth weight.[Results] The mean birth weight and the birth length of the case group were (2 285.75±220.00) g and (45.33±2.42) cm,respectively,whereas those of the control group were (3 346.65±421.78) g and (48.81±1.45) cm,respectively.Factors such as abnormality of umbilical stalk,a history of dysmenorrhea,a history of previous illnesses,pregnancy complications,drinking of boiled well water or fiver water and caught cold during pregnancy were associated with low birth weight after single factor Logistic regression analysis.However,after the muhifaetor Logistic regression analysis,abnormality of umbilical stalk,a history of dysmenorrhea,and drinking of boiled well water or river water were significantly related to term low birth weight infants(P0.05).[Conclusion] Abnormality of umbilical stalk,a history of dysmenorrhea,and drinking of boiled river or well water were factors possibly associated with low birth weight.
Perinatal factors and early neonatal outcomes of abnormal birthweight infants in Hangzhou, China, 2015-2021: a retrospective cohort study

Abstract:Background: This study aimed to investigate the perinatal factors and early neonatal outcomes of abnormal birth weight (ABW) in Hangzhou, China from 2015 to 2021. Methods: A retrospective cohort study was designed to analyse the data of 76 847 newborns, in which the case groups included 3042 cases of low birth weight (LBW) and 2941 cases of fetal macrosomia (MAC), and 70 864 cases of normal weight were as the reference group. Results: The incidence of LBW and MAC was 3.96% and 3.83% in Hangzhou, China from 2015 to 2021. Prematurity (<37 weeks), multiple births, hospitalisation >7 days, fetal anomalies, caesarean section, pregnancy complications, maternal coinfection with pathogens and summer births would be correlated with the incidence of LBW (ORs=43.50, 7.60, 2.09, 1.89, 1.57, 1.28, 1.19 and 1.18, all p<0.05). Factors such as post-term pregnancy (>41 weeks), scarred uterus, anterior vaginal incision and gravidity ≥2 were correlated with decreased incidence of LBW, with ORs of 0.05, 0.54, 0.65 and 0.80. Moreover, caesarean delivery, post-term pregnancy (> 41 weeks), parity ≥1, lateral vaginal incision, gravidity ≥2, hospitalisation >7 days, winter births and pregnancy complications also have association with the incidence of MAC (ORs=3.92, 2.73, 2.19, 1.87, 1.22, 1.20, 1.17 and 1.13, all p<0.05) while prematurity (<37 weeks), scarred uterus and anterior vaginal incision have close association with decreased incidence of MAC, with ORs of 0.07, 0.21 and 0.74 (all p<0.05). Conclusion: There was a trend of yearly increase in ABW in Hangzhou, China from 2015 to 2021. Several neonatal and maternal-related variables such as caesarean section, pregnancy complications and hospitalisation >7 days are associated with the odds of LBW and MAC, however, factors such as pregnancy with scarred uterus relate to the decrease of ABW. Close monitoring and intervention during pregnancy are essential to reduce the occurrence of ABW.

Abstract:Low birth weight (LBW), defined as a birth weight less than 2500 g, irrespective of gestational age, poses a significant health concern for newborns. Despite efforts, the incidence of LBW in sub-Saharan Africa has remained stagnant over the past decade, warranting attention from healthcare providers, policymakers, and researchers. This study aimed to identify factors associated with LBW among newborns delivered in public hospitals of North Shewa Zone, Amhara Region, Ethiopia, from May 2 to June 10, 2023. An unmatched case-control study was conducted from May 2 to June 10, 2023, involving 318 participants (106 cases and 212 controls). Data were collected using pretested interviewer-administered structured questionnaires, medical record reviews, and direct anthropometric measurements. Bivariate analyses were conducted, and variables with a p-value ≤ 0.25 were included in a multivariable logistic regression model to determine significant determinants of LBW. A significance level of p < 0.05 was used. A total of 309 newborns (103 cases and 206 controls) were included, yielding a response rate of 97.2%. Among the findings, females exhibited a higher risk of LBW (adjusted odds ratio [AOR]: 3.13, 95% CI: 1.34, 7.32, p = 0.008), as did mothers aged 20 or younger (AOR: 3.42, 95% CI: 1.35, 8.66, p = 0.009). Lack of formal education was associated with increased risk (AOR: 6.82, 95% CI: 2.94, 15.3, p < 0.001), as were unplanned pregnancies (AOR: 3.08, 95% CI: 1.38, 6.84, p = 0.006) and missed antenatal care visits (AOR: 2.74, 95% CI: 1.16, 6.49, p = 0.021). No significant associations were found with residency type or maternal age above 35. Mothers aged ≤ 20 years, with inadequate minimum dietary diversity, lack of antenatal care attendance, and unplanned pregnancies, faced heightened risks of LBW. Addressing these factors is vital for reducing LBW occurrences and improving newborn health outcomes in Ethiopia.

Abstract:【Objective】 To investigate the birth weights of live births in surveyed population,and to analyze the risk factors on the occurrence of low birth weight and macrosomia so as to provide effective interventional strategy.【Method】 A cross sectional survey of 2 977 women was conducted on their babies' birth weights and the parents' risk factors exposed before and during the pregnancy.【Results】 The average birth weight was 3 383.6 g.The incidence of low birth weight was 1.9%,and the incidence of the macrosomia was 7.4%.By the chi-square test,the mother with the history of preterm delivery,the preterm baby and the mother with the symptoms of threatened abortion were the risk factors of low birth weight.The mother suffered the anemia before the pregnancy or the diabetes during the pregnancy and the women with unhappy emotion during the pregnancy were the risk factors of macrosomia(P<0.05).Logistic regression analysis showed that the preterm infants(OR=37.457)was associated with low birth weight.Odds ratio for the mother with happy emotion was 0.737 for macrosomia.Odds ratios for the mothers suffered diabetes during pregnancy and the fathers smoking before pregnancy were 2.940 and 1.407 for macrosomia.【Conclusions】 The incidence of low birth weight is low,and the incidence of macrosomia is similar with the national level.They are affected by multi-factors and the preventions should be done in all aspects.

Abstract:As the major public health problem among under-5 children in the world, neonatal asphyxia (NA) contributes to 24% of the main causes of neonatal death. The effects of NA is not only limited to death but also has a long-term brain injury with lifelong adverse effects. Therefore, the goal of this study was to identify determinants of NA among newborns in the tropical province of China to guide early interventions and improve the survival and quality of life of these infants. A case control study was conducted at Hainan Women and Children's Medical Center from January 1 to December 31, 2021. A total of 255 newborns (85 cases and 170 controls, 1:2 case to control ratio) were enrolled in the study. A systematic random sampling approach was adopted based on hospital delivery registration. Structured questionnaires were used to collected data. The data was entered into statistical software SPSS version 20.0 for analysis. In the bivariable analysis, variables with P values less than .1 were entered into multivariable logistic regression analysis. At a P value of .05, a statistically significant level was reported. Amniotic fluid stained by meconium/blood (AOR=3.19, 95% confidence interval [CI]: 1.47-6.95), primiparity, fetal presentation of malpresentation (AOR=3.89, 95% CI: 1.25-12.09), and low birth weight (AOR=10.51, 95% CI: 3.02-36.55) were to be significantly associated with NA. This study identified that amniotic fluid stained by meconium/blood, primiparity, low birth weight were determinants of NA. Thus, preventive solutions such as close monitoring of fetus presentation, meliorating the obstetric care setup during antenatal care consultations should be stressed in China.
Contributions of Maternal Lifecourse Experience on Temporal Trends in Birthweight in China: an Age–period–cohort Analysis among 2·5 Million Newborns

Abstract:BackgroundSmall for gestational age (SGA), an indicator of small size at birth, can strongly predict mortality and morbidity in later life, and collective efforts to reduce the incidence of SGA or low birthweight can improve perinatal health. Previous studies have mainly focused on secular trends of birthweight by birth year, and few have explored the interaction between maternal lifecourse factors and trends in offspring birthweight. Rapid economic and social developments in China during past decades have profoundly affected women's health. In Guangzhou, a well developed city in China, rapid urbanisation along with an increased immigrant population provide a unique opportunity to explore the extent to which maternal lifecourse factors (age at delivery, period of delivery, and maternal birth cohort) independently contribute to trends in SGA incidence.MethodsData were obtained from the Guangzhou Perinatal Health Care and Delivery Surveillance System for 2 524 960 singleton livebirths with 24–42 weeks of gestation from 2001 to 2016. We used SGA as the birthweight indicator, defined as below the tenth percentile for sex-specific and gestational-age-specific birthweight standard from the INTERGROWTH-21st project. The age–period–cohort models were constructed to clarify the independent effects of age at delivery, period of delivery, and maternal birth cohort on SGA incidence trends. Analyses were also performed by stratifying maternal birthplace (western, central, and eastern China). This study was approved by the institutional ethical committee board of Guangzhou Women and Children's Medical Center. Data used in the study were anonymous, and individual identifiable information was not available for the analysis.FindingsThe SGA incidence declined from 10·4% in 2001 to 7·1% in 2016, with a 16-year overall incidence of 8·4%. In the age–period–cohort analysis, we observed strong effects of maternal age and birth cohort on SGA risk, while the effect of delivery period was mild. Age effect showed an L-shaped curve, with the highest risk (rates of >13%) for women younger than 20 years old irrespective of the delivery period. The effect of birth cohort on SGA incidence trends appeared to be linear. Compared with women born around 1961, those born around 1997 had a 61% decreased risk of SGA birth (95% CI 58–63). When the analyses were stratified by maternal birthplace, the effects of birth cohort seemed strongest among women who were born in central China compared with those from western and eastern China, whereas effects of maternal age and delivery period were similar among different birthplaces.InterpretationOur findings provide insight into the effects of maternal lifecourse factors on birthweight trends. Abstract:Low maternal prepregnancy BMI is associated with adverse birth outcomes, but the BMI at which risk increases is not well defined. We assessed whether the relationship between prepregnancy BMI and birth outcomes is influenced by the extent to which mothers were underweight in a prospective study in Anhui, China. The women (n = 575) were 20-34 y old, married, nulliparous and nonsmokers. All measures of infant growth increased with increasing maternal BMI until a plateau was reached at a BMI of 22-23 kg/m(2). Infants born to the 27% of women who were severely underweight before pregnancy (BMI less than or equal to 18.5 kg/m(2)) were at increased risk for fetal growth deficits associated with infant morbidity. Compared with a normal BMI, being severely underweight was associated with mean (+/- SEM) reductions of 219 +/- 40 g in infant birthweight and 6.7 +/- 1.3% in the birthweight ratio and an 80% increase in risk of intrauterine growth restriction [odds ratio (OR) 1.8; 95% Cl: 1.0, 3.3; P = 0.05]. Being severely underweight was also associated with smaller infant head circumference and lower ponderal index. Being moderately underweight (18.5 < BMI < 19.8 kg/m(2)) was not significantly associated with adverse pregnancy outcomes. Gestational age and risk of preterm birth were not associated with maternal BMI. More than half of the women in this study were underweight before pregnancy. Although being moderately underweight was not associated with increased risk of adverse pregnancy outcomes, being severely underweight was an important risk factor for reduced fetal growth.
Attendance at Prenatal Care and Adverse Birth Outcomes in China: A Follow-Up Study Based on Maternal and Newborn's Health Monitoring System.

Abstract:Objective: to evaluate the independent association between attendance at prenatal care and adverse birth outcomes in China, measured either as the occurrence of preterm birth or low birth weight. Design: a follow-up study. Setting: the data was collected from maternal and newborn's health monitoring system at 6 provinces in China. Participants: all pregnant women registered in the system at their first prenatal care visit. We included 40152 registered pregnant women who had delivered between October 2013 and September 2014. Measurements: attendance at prenatal care was evaluated using Kessner index chi(2) tests were used to examine the correlations between demographic characteristics and preterm birth or low birth weight. The associations between attendance at prenatal care and birth outcomes were explored using multilevel mixed-effects logistic regression models. Findings: the prevalence for preterm birth and low birth weight was 3.31% and 2.55%. The null models showed region clustering on birth outcomes. Compared with women who received adequate prenatal care, those with intermediate prenatal care (adjusted OR 1.62, 95% CI 1.37-1.92) or inadequate prenatal care (adjusted OR 2.78, 95% CI 2.24-3.44) had significantly increased risks for preterm birth, and women with intermediate prenatal care (adjusted OR 1.31, 95% CI 1.10-1.55) or inadequate prenatal care (adjusted OR 1.70, 95% CI 1.32-2.19) had significantly increased risks for low birth weight. We found very significant dose-response patterns for both preterm birth (p-trend < 0.001) and low birth weight (p-trend = 0.001). Key conclusions and implication for practice: This study shows that attendance at prenatal care in China has independent effects on both preterm birth and low birth weight. Appropriate timing and number of prenatal care visits can help to reduce the occurrence of preterm birth or low birth weight.
Association of Maternal Pre-Pregnancy Body Mass Index and Pregnancy History with Birthweight in Offspring

Abstract:Objective To investigate the association between maternal body mass index (BMI), pregnancy history and birthweight of offspring including low birthweight, macrosomia, small-for-gestational-age (SGA) and large-for-gestational-age (LGA) in Jiashan county of Zhejiang province. Methods We recruited 919 pairs of women and their singleton neonates from the participants of National Free Preconception Health Examination Project (NFPHEP) conducted in Jiashan county of Zhejiang province in 2014 and 2015. Relevant information were extracted from the dataset of NFPHEP. Chi-square test, linear regression, and logistic regression model were applied in statistics analyses. Results Of all the pregnant women, 21.00% (193) were overweight/obesity and 12.73% (117) were lean. For all the neonates, the mean birthweight was 3 339.90 ± 429.98 grams and the number of low birthweight, macrosomia, SGA, and LGA were 28, 57, 48, and 95, respectively. Linear regression analysis indicated that the neonates born to lean mothers had a birthweight of 65.44 (95% confidence interval [95% CI]: – 130.57 – – 0.31) grams lower than that of the neonates born to mothers with normal weight significantly (t = – 1.97, P Conclusion Maternal BMI before pregnancy is associated with the birthweight in offspring. The pre-pregnancy overweight/obese women have an increased risk of to give a birth to a macrosomia and the risk is modified by maternal age, education and pregnancy history of the women.
Impact of gestational hypertension and preeclampsia on low birthweight and small‐for‐gestational‐age infants in China: A large prospective cohort study

Abstract:<p>Studies have shown that maternal blood pressure level is associated with neonatal birthweight, but the results are not exactly consistent. As the most common hypertensive disorders during pregnancy, the mechanism of gestational hypertension and pre‐eclampsia that affect fetal growth remain unclear. Our objective was to examine the association of gestational hypertension and pre‐eclampsia with the risk of low birthweight (LBW) and small‐for‐gestational‐age (SGA). Data were obtained from the China–US Collaborative Project for Neural Tube Defects Prevention, a large population‐based cohort study. We selected participants who were registered in two southern provinces, had exact information on gestational blood pressure and pregnancy outcomes, and were not affected by chronic hypertension. Logistic regression was used to adjust for the effects of the main potential confounders, including age, body mass index, education, occupation, ethnicity, folic acid use, and parity. The overall incidences of LBW and SGA were 2.25% and 5.86%, respectively. The incidences of LBW/SGA were 3.58%/7.58% and 6.02%/10.67% for gestational hypertension and pre‐eclampsia group, relative to 2.11%/5.68% and 2.16%/5.74% for normal group. The adjusted odds ratios associated with gestational hypertension/pre‐eclampsia were 1.77 (95% CI: 1.63, 1.92)/3.01 (95% CI: 2.67, 3.40) for LBW and 1.40 (95% CI: 1.32, 1.48)/2.02 (95% CI: 1.84, 2.22) for SGA, respectively. The early onset of gestational hypertension/pre‐eclampsia appeared to be a relatively more detrimental exposure window for both LBW and SGA. Our results support an association between gestational hypertension or pre‐eclampsia and the increased risk of LBW and SGA.</p>
Influence of perinatal factors on full-term low-birth-weight infants and construction of a predictive model

Abstract:BACKGROUND Being too light at birth can increase the risk of various diseases during infancy. AIM To explore the effect of perinatal factors on term low-birth-weight (LBW) infants and build a predictive model. This model aims to guide the clinical management of pregnant women’s healthcare during pregnancy and support the healthy growth of newborns. METHODS A retrospective analysis was conducted on data from 1794 single full-term pregnant women who gave birth. Newborns were grouped based on birth weight: Those with birth weight < 2.5 kg were classified as the low-weight group, and those with birth weight between 2.5 kg and 4 kg were included in the normal group. Multiple logistic regression analysis was used to identify the factors influencing the occurrence of full-term LBW. A risk prediction model was established based on the analysis results. The effectiveness of the model was analyzed using the Hosmer–Leme show test and receiver operating characteristic (ROC) curve to verify the accuracy of the predictions. RESULTS Among the 1794 pregnant women, there were 62 cases of neonatal weight < 2.5 kg, resulting in an LBW incidence rate of 3.46%. The factors influencing full-term LBW included low maternal education level [odds ratio (OR) = 1.416], fewer prenatal examinations (OR = 2.907), insufficient weight gain during pregnancy (OR = 3.695), irregular calcium supplementation during pregnancy (OR = 1.756), and pregnancy hypertension syndrome (OR = 2.192). The prediction model equation was obtained as follows: Logit (P ) = 0.348 × maternal education level + 1.067 × number of prenatal examinations + 1.307 × insufficient weight gain during pregnancy + 0.563 × irregular calcium supplementation during pregnancy + 0.785 × pregnancy hypertension syndrome − 29.164. The area under the ROC curve for this model was 0.853, with a sensitivity of 0.852 and a specificity of 0.821. The Hosmer–Leme show test yielded χ 2 = 2.185, P = 0.449, indicating a good fit. The overall accuracy of the clinical validation model was 81.67%. CONCLUSION The occurrence of full-term LBW is related to maternal education, the number of prenatal examinations, weight gain during pregnancy, calcium supplementation during pregnancy, and pregnancy-induced hypertension. The constructed predictive model can effectively predict the risk of full-term LBW.

A Study on Determinants of Full-Term Low-Birth Weight Based on Hospital Records

Abstract:To describe the prevalence of full-term low-birth weight(TLBW)and to explore potential risk factors of TLBW,The data used were the medical records of the People's Hospital of Gansu Province,from January 1st,1995 to December 18th,2002.Cross-tables and multiple logistic regressions were used to identify the potential risk factors related to the outcomes and to estimate the association between TLBW and risk factors.Results show that the overall prevalence of full-term low-birth weight among 9017 live singleton births was 4.2%,of which,53.0% was TLBW.Compared with the professional women,mothers without a job and peasants were more likely to have a TLBW(OR 2.00,95%CI 1.17~3.42;OR 2.67,95%CI=1.52~4.69) after controlling the confounding factors.Mothers who did not have antenatal care were twice as much likely to have TLBW as their counterparts(95%CI 1.26~3.43).Placental insufficient functions were significantly associated with TLBW(OR8.04,95%CI 2.27~28.52),and anemia was somewhat associated with higher risk of TLBW(OR=2.70,95%CI=0.92~7.92).Infants born in Autumn were more likely to be TLBW than in winter(OR=1.78,95%CI=1.12~2.85).Female has odds ration of 1.34(95%CI=0.97~1.84).Other potential risk factors are abnormal presentations,but maternal age,gravidity and parity were not found to be related to TLBW.Hence,our conclusion is that mothers without a job and peasants were more likely to have a TLBW.Antenatal care,placental insufficiency,abnormal presentation and season are the main influential factors.Anemia and female baby might be potential risk factors.
Phthalate Levels and Low Birth Weight: A Nested Case-Control Study of Chinese Newborns

Abstract:Objective To assess maternal-fetal exposure to phthalates and investigate whether in utero phthalate exposure is associated with low birth weight (LBW). Study design A total of 201 newborn-mother pairs (88 LBW cases and 113 controls) residing in Shanghai were enrolled in this nested case-control study during 2005-2006. Maternal blood, cord blood, and meconium specimens were collected and analyzed for phthalates by high-performance liquid chromatography-mass spectrometry. Nonparametric tests were used to compare demographic characteristics in cases and controls. Conditional logistic regression and Spearman correlation were used to analyze the association between phthalate exposure and LBW. Results No significant differences in gestational age, prepregnancy body mass index, prenatal care, vitamin supplementation, or socioeconomic levels were found between the LBW and control infants. More than 70% of the biosamples had quantifiable levels of phthalates, with higher levels in the LBW infants compared with the controls. Prenatal di-n-butyl phthalate (DBP) exposure was associated with LBW, and di-2-ethylhexyl phthalate (DEHP) was negatively associated with birth length. After adjusting for the potential confounders, DBP concentrations in the highest quartile were associated with an increased risk of LBW. Conclusions Newborns in China are ubiquitously exposed to phthalates; significantly higher phthalate levels were detected in LBW cases compared with controls. In utero DBP and DEHP exposures were associated with LBW in a dose-dependent manner. Prenatal phthalate exposure may be a risk factor for LBW. (J Pediatr 2009,155:500-4).
Low Incidence of Maternal Near-Miss in Zhejiang, a Developed Chinese Province: A Cross-Sectional Study Using the WHO Approach

Abstract:Background and Aim: Maternal near-miss (MNM) surveillance has been developed to identify severe complications in pregnancy since 2011 in China. However, very little is known about MNM in China. This study aims to explore the prevalence of MNM, the risk factors, and perinatal outcomes using the WHO near-miss approach in a developed Chinese province. Materials and Methods: We used data from China's National Maternity Near Miss Obstetrics Surveillance System for the period 2012 to 2017, which included 18 hospitals in Zhejiang Province. Chi-squared tests were used to compare the socio-demographic factors, obstetric complications and perinatal outcomes between women with and without MNM. Logistic regression was used to examine the independent risk factors for MNM. Results: A total of 612,264 pregnant women were recruited. There were 3208 MNM cases and 34 maternal deaths. The MNM incidence ratio was low at 5.9 per 1,000 live births, with an MNM mortality ratio of 98:1 and mortality index of 1.1%. Among 3208 women with MNM, postpartum hemorrhage was the commonest cause at 76.3% followed by severe anemia at 23.7% and placenta previa at 23.0%. Embolism was identified as having the highest risk for MNM (AOR 46.0; 95% CI 19.1-110.7), followed by postpartum hemorrhage (AOR 41.0; 95% CI 35.7-47.0), and severe anemia (AOR 36.6; 95% CI 16.0-84.1). MNM cases were significantly associated with severe perinatal outcome, including premature birth, low birth weight, multiple fetuses, stillbirth rates and neonatal mortality. Conclusion: Overall near-miss indicators suggested a relatively high quality of maternal health care in a developed province of China. The identified risk factors may be helpful in developing targeted interventions for improving maternal safety.
