Abstract:To investigate the association of long-term exposure to ambient air pollution with the risk of allergic rhinitis (AR), we performed a longitudinal analysis of 379,488 participants (47.4% women) free of AR at baseline in the UK Biobank. The annual average concentrations of PM2.5, PMcoarse, PM10, NO2, and NOx were estimated by land use regression models. Cox proportional hazard models were used to calculate hazard ratios (HRs) and 95% confidence intervals (CIs). A weighted polygenic risk score was constructed. During a median follow-up period of 12.5 years, 3095 AR cases were identified. We observed significant associations between the risk of AR and PM2.5 (HR: 1.51, 95% CI: 1.27-1.79, per 5 μg/m3), PMcoarse (HR: 1.28, 95% CI: 1.06-1.55, per 5 μg/m3), PM10 (HR: 1.45, 95% CI: 1.20-1.74, per 10 μg/m3), NO2 (HR: 1.14, 95% CI: 1.09-1.19, per 10 μg/m3), and NOx (HR: 1.10, 95% CI: 1.05-1.15, per 20 μg/m3). Moreover, participants with high air pollution combined with high genetic risk showed the highest risk of AR, although no multiplicative or additive interaction was observed. In conclusion, long-term exposure to air pollutants was associated with an elevated risk of AR, particularly in high-genetic-risk populations, emphasizing the urgent need to improve air quality.
Respiratory symptoms among residents of a heavy-industry province in China: prevalence and risk factors.

Abstract:Objective: In China, significant levels of environmental pollution, substandard worksite quality and high rates of smoking predispose the population to potentially high risks of respiratory illnesses and other diseases. We assessed the prevalence of respiratory symptoms and their risks in relation to personal, occupational and environmental risk factors in a heavy-industry province of northeastern China. Methods: Lifestyle, health, residential and occupational data were obtained in 2002 from 31,704 adults of six cities in Liaoning, China, using self-assessment questionnaires. General linear and multi-level models were used to evaluate prevalence rates and risks of respiratory symptoms, related to both individual and combined exposures to environmental and occupational risk factors. Results: The crude prevalence rates (PRs) for persistent cough, persistent phlegm, wheeze and asthma were 2.3, 3.8, 2.1 and 1.0%, respectively. The odds ratios (ORs) of at[ four respiratory symptoms examined were increased by: smoking (ORs from 2.06 to 5.02), occupational dust (ORs from 1.35 to 1.72), occupational gas (ORs from 1.48 to 1.72) and presence of irritating smoke during cooking (ORs from 1.54 to 2.22). An index combining proximity of residence to road, factory or chimney, indoor coat use and presence of irritating smoke during cookingwas associated with up to 3.9-fold increased risks of all symptoms. Increasing values of each risk factor were generally associated with dose-response trends in prevalence rates and risks (at[ p for trend <0.01). Conclusion: The crude PRs of symptoms were tower than those reported by European and American studies but closer to those of previous Chinese studies. The risks of respiratory symptoms in this population were increased by smoking, occupational exposures to dust and gas, and combined residence-related exposures such as living close to a main road, factory or chimney, indoor coal use and the presence of irritating smoke during cooking, among other risk factors. Abstract:Air pollutants are a heterogeneous mixture of gases and particles including particulate matter <2.5 µm in size (PM2.5), particulate matter <10 µm in size (PM10), nitrogen dioxide (NO2), carbon monoxide (CO), sulfur dioxide (SO2) and ozone (O3). Air pollutants have proinflammatory effects and are therefore among the environmental factors contributing to the generation of inflammation and autoimmunity. However, there is scanty data supporting the correlation between air pollution and systemic autoimmune rheumatic diseases (SARDs), which mainly include systemic lupus erythematosus (SLE), rheumatoid arthritis (RA), systemic sclerosis and autoimmune myositis. We investigated the relationship between air pollutants exposure and SARDs in a nationwide population-based case–control study. Abstract:The pathogenesis of systemic autoimmune rheumatic diseases (SARDs) is complex and remains insufficiently understood. It is commonly accepted that both intrinsic and extrinsic environmental factors interact to induce a self-reactive immune response. Case reports and observational studies have revealed an association between SARDs and specific airborne environmental factors, but the heterogeneity of the published studies hampers clear conclusions. The aim of this review is to provide an overview of the available epidemiological evidence on the relationship between airborne pollutants and SARDs. We performed a narrative review using the PubMed database. Observational studies have shown significant associations between airborne pollutants and SARDs. Cigarette smoking is strongly associated with the development of rheumatoid arthritis (RA) while the association between cigarette smoke and the development of other SARDs remains controversial. For decades, silica exposure has been linked to systemic sclerosis (SSc), RA and systemic lupus erythematosus (SLE). There is also strong evidence for a link between solvents and SSc. Recent observations even suggest that ambient air pollution is associated with the development of SARDs. Some studies have shown associations between asbestos, organic dust, metals and pesticides and SARDs, but more studies are needed to confirm these findings. Increasing evidence has linked airborne pollutants to SARDs. Although more studies are needed to understand the potential mechanisms by which these environmental agents contribute to disease pathogenesis, awareness of the link between environmental agents and SARDs is important to recognize and prevent work-related and environmentally induced diseases.

Causal association between air pollution and autoimmune diseases: a two-sample Mendelian randomization study

Abstract:Background: In recent years, an increasing number of observational studies have reported the impact of air pollution on autoimmune diseases (ADs). However, no Mendelian randomization (MR) studies have been conducted to investigate the causal relationships. To enhance our understanding of causality, we examined the causal relationships between particulate matter (PM) and nitrogen oxides (NO x ) and ADs. Methods: We utilized genome-wide association study (GWAS) data on PM and NO x from the UK Biobank in European and East Asian populations. We also extracted integrated GWAS data from the Finnish consortium and the Japanese Biobank for two-sample MR analysis. We employed inverse variance weighted (IVW) analysis to assess the causal relationship between PM and NO x exposure and ADs. Additionally, we conducted supplementary analyses using four methods, including IVW (fixed effects), weighted median, weighted mode, and simple mode, to further investigate this relationship. Results: In the European population, the results of MR analysis suggested a statistically significant association between PM 2.5 and psoriasis only (OR = 3.86; 95% CI : 1.89–7.88; P IVW < 0.00625), while a potential association exists between PM 2.5–10 and vitiligo (OR = 7.42; 95% CI : 1.02–53.94; P IVW < 0.05), as well as between PM 2.5 and systemic lupus erythematosus (OR = 68.17; 95% CI : 2.17–2.1e+03; P IVW < 0.05). In East Asian populations, no causal relationship was found between air pollutants and the risk of systemic lupus erythematosus and rheumatoid arthritis ( P IVW > 0.025). There was no pleiotropy in the results. Conclusion: Our results suggest a causal association between PM 2.5 and psoriasis in European populations. With the help of air pollution prevention and control, the harmful progression of psoriasis may be slowed.

Abstract:Rheumatoid arthritis is a common rheumatic disease, and its onset is closely related to genetic and environmental factors, however, the relationship between air pollution and RA is still hotly debated. Further investigation of the relationship between air pollution and rheumatoid arthritis is conducive to a comprehensive understanding of the risk factors of the disease, providing certain value for the clinical prevention and treatment of RA. We used a Two-Sample Mendelian Randomization approach, integrating the large-scale public genomewide association study, to assess the genetically predicted causal effect of air pollution (including: PM2.5, PM2.5–10, PM10, nitrogen dioxide, nitrogen oxides) on RA in European and European East Asian populations, respectively. Indicators related to air pollution (2,505 individuals to 423,796 individuals), including European and East Asian populations were obtained from the Integrative Epidemiology Unit open GWAS project. Published East Asian RA data were also obtained from the IEU open GWAS project (212,453 individuals), while large-scale publicly available European RA data were obtained from finngen R10 (13,621 cases and 262,844 controls). Inverse variance weighting was used as the primary analytical method, complemented by MR-egger, Weighed median, and Weighted mode results. Cochran Q tested for heterogeneity, and MR-Egger regression analyses were performed to test for multiplicity. leave-one-out analysis allowed for the robustness and reliability were assessed. No statistically significant effects of PM2.5, PM2.5–10, PM10, nitrogen dioxide, nitrogen oxides and RA were observed in either European or East Asian populations. Results from European data: PM2.5 (IVW OR: 0.71; 95% CI: 0.27–1.91; p = 0.498; number of SNPs: 5), PM2.5–10 (IVW OR: 1.20; 95% CI: 0.61–2.40; p = 0.596; number of SNPs: 15), PM10 (IVW OR: 1.69; 95% CI: 0.84–3.39; p = 0.142; number of SNPs: 9), nitrogen dioxide (IVW OR: 3.88; 95% CI: 0.19–77.77; p = 0.375; number of SNPs: 2), nitrogen oxides (IVW OR: 0.51; 95% CI: 0.16–1.67; p = 0.268; number of SNPs: 4). East Asian data results: PM2.5 (IVW OR: 1.16; 95% CI: 0.98–1.38; p = 0.086; number of SNPs: 4), PM2.5–10 (IVW OR: 1.14; 95% CI: 0.95–1.38; p = 0.166; number of SNPs: 2), PM10 (IVW OR: 0.95; 95% CI: 0.81–1.11; p = 0.503; number of SNPs: 3), nitrogen dioxide (IVW OR: 0.87; 95% CI: 0.76–1.00; p = 0.051; number of SNPs: 6), nitrogen oxides (IVW OR: 0.96; 95% CI: 0.82–1.14; p = 0.671; number of SNPs: 3). No signs of pleiotropy or heterogeneity were observed in the MR-Egger intercept, MR-PRESSO and Cochrane's Q (p>0.05). In addition, no outliers were found in the MR-PRESSO analysis. The results were further validated by leave-one-out tests, confirming the robustness of the findings. We performed transethnic MR analysis suggesting that there may not be a genetically predicted causal relationship between air pollution and RA.

Association between long-term exposure to air pollution and immune-mediated diseases: a population-based cohort study

Abstract:Objective Environmental air pollution has been associated with disruption of the immune system at a molecular level. The primary aim of the present study was to describe the association between long-term exposure to air pollution and risk of developing immune-mediated conditions. Methods We conducted a retrospective observational study on a nationwide dataset of women and men. Diagnoses of various immune-mediated diseases (IMIDs) were retrieved. Data on the monitoring of particulate matter (PM)10 and PM2.5 concentrations were retrieved from the Italian Institute of Environmental Protection and Research. Generalised linear models were employed to determine the relationship between autoimmune diseases prevalence and PM. Results 81 363 subjects were included in the study. We found a positive association between PM10 and the risk of autoimmune diseases (ρ+0.007, p 0.014). Every 10 μg/m 3 increase in PM10 concentration was associated with an incremental 7% risk of having autoimmune disease. Exposure to PM10 above 30 μg/m3 and PM2.5 above 20 μg/m 3 was associated with a 12% and 13% higher risk of autoimmune disease, respectively (adjusted OR (aOR) 1.12, 95% CI 1.05 to 1.20, and aOR 1.13, 95% CI 1.06 to 1.20). Exposure to PM10 was associated with an increased risk of rheumatoid arthritis; exposure to PM2.5 was associated with an increased risk of rheumatoid arthritis, connective tissue diseases (CTDs) and inflammatory bowel diseases (IBD). Conclusion Long-term exposure to air pollution was associated with higher risk of developing autoimmune diseases, in particular rheumatoid arthritis, CTDs and IBD. Chronic exposure to levels above the threshold for human protection was associated with a 10% higher risk of developing IMIDs.

Ambient PM2.5, O₃, and NO₂ Exposures and Associations with Mortality over 16 Years of Follow-Up in the Canadian Census Health and Environment Cohort (CanCHEC)

Abstract:Background: Few studies examining the associations between long-term exposure to ambient air pollution and mortality have considered multiple pollutants when assessing changes in exposure due to residential mobility during follow-up. Objective: We investigated associations between cause-specific mortality and ambient concentrations of fine particulate matter (≤ 2.5 μm; PM2.5), ozone (O3), and nitrogen dioxide (NO2) in a national cohort of about 2.5 million Canadians. Methods: We assigned estimates of annual concentrations of these pollutants to the residential postal codes of subjects for each year during 16 years of follow-up. Historical tax data allowed us to track subjects' residential postal code annually. We estimated hazard ratios (HRs) for each pollutant separately and adjusted for the other pollutants. We also estimated the product of the three HRs as a measure of the cumulative association with mortality for several causes of death for an increment of the mean minus the 5th percentile of each pollutant: 5.0 μg/m3 for PM2.5, 9.5 ppb for O3, and 8.1 ppb for NO2. Results: PM2.5, O3, and NO2 were associated with nonaccidental and cause-specific mortality in single-pollutant models. Exposure to PM2.5 alone was not sufficient to fully characterize the toxicity of the atmospheric mix or to fully explain the risk of mortality associated with exposure to ambient pollution. Assuming additive associations, the estimated HR for nonaccidental mortality corresponding to a change in exposure from the mean to the 5th percentile for all three pollutants together was 1.075 (95% CI: 1.067, 1.084). Accounting for residential mobility had only a limited impact on the association between mortality and PM2.5 and O3, but increased associations with NO2. Conclusions: In this large, national-level cohort, we found positive associations between several common causes of death and exposure to PM2.5, O3, and NO2. Abstract:Introduction Rheumatoid arthritis (RA) is a common autoimmune disease with a complex and poorly understood etiology that includes genetic, hormonal, and environmental factors. Objective Our objective was to assess current literature that investigated the association between exposure to environmental and occupational air pollutants and RA-related biomarkers rheumatoid factor (RF) and anti-citrullinated peptide antibody (ACPA). Design PubMed and Web of Science were used to identify epidemiological studies that measured or estimated air pollution and at least one RA biomarker. Information was charted for comparison of evidence, including pollutant(s) studied, exposure assessment, biomarker measurement, analysis method, study population, size, and dates, covariates, and findings. Results Several common air pollutants (including two mixtures) and a few dozen occupational inhalants were assessed in 13 eligible studies. Associations between industrial sulfur dioxide and particulate matter less than 2.5 μm in diameter with ACPA were observed most frequently, including dose-response associations between residential proximity to pollution sources and ACPA concentration. Consistency of associations with other pollutants was either not observed or limited to single studies. Three studies evaluated the modifying impact of SE alleles (a genetic factor associated with RA) and found that pollutant associations were stronger among participants positive for SE alleles. Conclusion Based on mixed results, there was no consistent link between any single pollutant and RA-related biomarker outcomes. Comparisons across studies were limited by differences in study populations (e.g., by RA status, by sociodemographic groups) and study design (including designs focused on different sources of air pollution, methodological approaches with varying levels of potential exposure misclassification, and assessments of inconsistent biomarker cut-points). However, given that multiple studies reported associations between exposure to air pollution and RA biomarkers, continued exploration utilizing studies that can be designed with a more robust causal framework, including continued consideration of effect modification by genetic status, may be necessary.

Health risk of air pollution on people living with major chronic diseases: a Canadian population-based study

Abstract:OBJECTIVES: The objective of this study was to use health administrative and environmental data to quantify the effects of ambient air pollution on health service use among those with chronic diseases. We hypothesised that health service use would be higher among those with more exposure to air pollution as measured by the Air Quality Health Index (AQHI).SETTING: Health administrative data was used to quantify health service use at the primary (physician office visits) and secondary (emergency department visits, hospitalisations) level of care in Ontario, Canada.PARTICIPANTS: We included individuals who resided in Ontario, Canada, from 2003 to 2010, who were ever diagnosed with one of 11 major chronic diseases.OUTCOME MEASURES: Rate ratios (RR) from Poisson regression models were used to estimate the short-term impact of incremental unit increases in AQHI, nitrogen dioxide (NO2; 10 ppb), fine particulate matter (PM2.5; 10 µg/m(3)) and ozone (O3; 10 ppb) on health services use among individuals with each disease. We adjusted for age, sex, day of the week, temperature, season, year, socioeconomic status and region of residence.RESULTS: Increases in outpatient visits ranged from 1% to 5% for every unit increase in the 10-point AQHI scale, corresponding to an increase of about 15,000 outpatient visits on a day with poor versus good air quality. The greatest increases in outpatient visits were for individuals with non-lung cancers (AQHI:RR=1.05; NO2:RR=1.14; p<0.0001) and COPD (AQHI:RR=1.05; NO2:RR=1.12; p<0.0001) and in hospitalisations, for individuals with diabetes (AQHI:RR=1.04; NO2:RR=1.07; p<0.0001) and COPD (AQHI:RR=1.03; NO2:RR=1.09; p<1.001). The impact remained 2 days after peak AQHI levels.CONCLUSIONS: Among individuals with chronic diseases, health service use increased with higher levels of exposure to air pollution, as measured by the AQHI. Future research would do well to measure the utility of targeted air quality advisories based on the AQHI to reduce associated health service use.

Abstract:OBJECTIVE To obtain the glucose metabolic profile of patients with essential hypertension in China. METHODS This is an epidemiologic cross-sectional survey and screening study, 5 sites in China participated in. Patients with essential hypertension and without known diabetes mellitus(DM) answered the questionnaire and received in-site fasting blood glucose (FBG) determination and simple oral glucose tolerance test (OGTT). RESULTS A total of 1,421 patients had the data of FBG and OGTT 2-hour glucose. Among them, The prevalence rates of newly diagnosed DM and impaired glucose regulation (IGR) were 22.9% and 64.4% respectively. Patients with newly diagnosed DM have higher systolic blood pressure, waistline, waist-to-hip ratio and plasma triglyceride. CONCLUSION Newly diagnosed disturbed glucose metabolic status is common among patients with essential hypertension without DM history, which had very low diagnose rate in routine work. We must pay more attention to the glucose metabolic status of patients with hypertension, especially the one with metabolic syndrome, OGTT should be as a routine procedure in the diagnosis of such patients.
Individual and joint exposure to air pollutants and patterns of multiple chronic conditions

Abstract:Existing research on the detrimental effects of air pollution and its mixture on multiple chronic conditions (MCC) is not yet fully recognized. Our objective was to examine if individual and joint exposure to air pollution is associated with the incidence and patterns of MCC. Totally 10,231 CHARLS 2015 participants aged over 45 years and 1,938 without MCC were followed up in 2018 and 2020. Residential-levelcumulative personal exposure concentrations of PM1, PM10, PM2.5, CO, O3, NO2, SO2, NO3-, Cl-, NH4+, and SO42- at the residential level were determined utilizing a spatio-temporal random forest model with a spatial resolution of 0.1° × 0.1°. In the cross-sectional and longitudinal research, logistic regression, cox regression analysis, and quantile g-computation were utilized to estimate the single and joint effect with MCC and its patterns, respectively. Interaction analyses and stratified analyses were also performed. A correlation was observed between the prevalence of cardiovascular illnesses and the presence of all 11 major air pollutants. PM2.5, PM10, NH4+, NO3-, CO, and SO42- are associated with an increased frequency of respiratory disorders. An increase of PM2.5, PM1, PM10, NO2, and SO2 (a 10 µg/m3 rise), CO (a 0.1 mg/m3 rise), and PMCs (Cl-, NH4+, NO3-, and SO42-) (a 1 µg/m3 rise) corresponded to the HRs (95% CI) for developing MCC of 1.194 (95% CI: 1.043, 1.367), 1.362 (95% CI: 1.073, 1.728), 1.115 (95% CI: 1.026, 1.212), 1.443 (95% CI: 1.151, 1.808), 3.175 (95% CI: 2.291, 4.401), 1.272 (95% CI: 1.149,1.410), 1.382 (95% CI: 1.011, 1.888), 1.107 (95% CI: 1.003, 1.222), 1.035 (95% CI: 0.984, 1.088), and 1.122 (95% CI: 1.086, 1.160), respectively. SO2 was the predominant contributor to the combined effect (HR: 2.083, 95% CI: 1.659-2.508). Gender, age, drinking, and health status could modify the effects of air pollutants on MCC patterns. Long-term exposure to air pollution is correlated to the incidence and patterns of MCC in middle-aged and elderly Chinese individuals. Preventive methods are essential to safeguarding those susceptible to MCC.

Abstract:Background: The associations of long-term exposure to air pollutants in the presence of asthmatic symptoms remain inconclusive and the joint effects of air pollutants as a mixture are unclear. Objective: We aimed to investigate the individual and joint associations of long-term exposure to ambient fine particulate matter (PM2.5) and daily 8-hour maximum ozone concentrations (MDA8 O3) in the presence of asthmatic symptoms in Chinese adults. Methods: Data were derived from the World Health Organization Study on Global Ageing and Adult Health (WHO SAGE) cohort study among adults aged 50 years or older, which was implemented in 1 municipality and 7 provinces across China during 2007-2018. Annual average MDA8 O3 and PM2.5 at individual residential addresses were estimated by an iterative random forest model and a satellite-based spatiotemporal model, respectively. Participants who were diagnosed with asthma by a doctor or taking asthma-related therapies or experiencing related conditions within the past 12 months were recorded as having asthmatic symptoms. The individual associations of PM2.5 and MDA8 O3 with asthmatic symptoms were estimated by a Cox proportional hazards regression model, and the joint association was estimated by a quantile g-computation model. A series of subgroup analyses was applied to examine the potential modifications of some characteristics. We also calculated the population-attributable fraction (PAF) of asthmatic symptoms attributed to PM2.5 and MDA8 O3. Results: A total of 8490 adults older than 50 years were included, and the average follow-up duration was 6.9 years. During the follow-up periods, 586 (6.9%) participants reported asthmatic symptoms. Individual effect analyses showed that the risk of asthmatic symptoms was positively associated with MDA8 O3 (hazard ratio [HR] 1.12, 95% CI 1.01-1.24, for per quantile) and PM2.5 (HR 1.18, 95% CI 1.05-1.31, for per quantile). Joint effect analyses showed that per equal quantile increment of MDA8 O3 and PM2.5 was associated with an 18% (HR 1.18, 95% CI 1.05-1.33) increase in the risk of asthmatic symptoms, and PM2.5 contributed more (68%) in the joint effects. The individual PAFs of asthmatic symptoms attributable to PM2.5 and MDA8 O3 were 2.86% (95% CI 0.17%-5.50%) and 4.83% (95% CI 1.42%-7.25%), respectively, while the joint PAF of asthmatic symptoms attributable to exposure mixture was 4.32% (95% CI 1.10%-7.46%). The joint associations were greater in participants with obesity, in urban areas, with lower family income, and who used unclean household cooking fuel. Conclusions: Long-term exposure to PM2.5 and MDA8 O3 may individually and jointly increase the risk of asthmatic symptoms, and the joint effects were smaller than the sum of individual effects. These findings informed the importance of joint associations of long-term exposure to air pollutants with asthma.

Abstract:Abstract Background The prevalence of allergic respiratory disease (ARD) is increasing worldwide during the last few decades, causing a great disease burden especially for children. Air pollution has been increasingly considered as a potential contributor to this trend, but its role in ARD induced by house dust mite (HDM-ARD) remains unclear, especially in time-series study. Methods A positive reporting of respiratory allergy to named allergens was included by serum specific IgE testing. A time series Quasi-Poisson regression with distributed lag non-linear model, combined with generalized linear model was used to examine the effects of air pollutants on ARD, HDM-ARD and ARD induced by non-house dust mite (NHDM-ARD). Results A total of 16,249 cases of ARD, including 8,719 HDM-ARD and 8,070 NHDM-ARD from 1 Jan 2013 to 31 Dec 2017 were involved in this study. Air pollutants were significantly associated with clinical visits for childhood ARD and HDM-ARD. Exposure to higher O 3 and interquartile range (IQR) increment in O 3 (40.6 μg/m 3 ) increased the risks of clinical visits for childhood HDM-ARD (RR lag0-5 for the 95th percentile of O 3 : 1.26, 95% confidence interval (CI): 1.03, 1.55; RR lag0-5 for IQR increment (40.6 μg/m 3 ): 1.09, 95% CI: 1.01, 1.17) and ARD (RR lag0-5 for the 95th percentile of O 3 : 1.19, 95% CI: 1.03, 1.38; RR lag0-5 for IQR increment (40.6 μg/m 3 ): 1.06, 95% CI: 1.01, 1.12). In addition, higher O 3 was associated with increased RR of boys with ARD (RR lag0-5 for the 95th percentile: 1.26, 95% CI: 1.05, 1.51; RR lag0-5 for IQR increment (40.6 μg/m 3 ): 1.09, 95% CI: 1.02, 1.16) and HDM-ARD (RR lag0-5 for the 95th percentile: 1.36, 95% CI: 1.06, 1.75; RR lag0-5 for IQR increment (40.6 μg/m 3 ): 1.11, 95% CI: 1.02, 1.22), but not in girls. Conclusions Exposure to O 3 appeared to be a trigger of clinical visits for childhood ARD, especially for HDM-ARD and boys. These findings provide novel evidence on the impact of air pollution on HDM-ARD, which may have significant implications for designing effective intervention programs to control and prevent childhood ARD, especially HDM-ARD, in China and other similar developing countries.

Abstract:Abstract Air pollution (AP) is an alarming public health crisis worldwide. Common ambient pollutants include sulfur dioxide (SO2), carbon monoxide (CO), ozone (O3), coarse particulate matter (PM10), fine particulate matter (PM2.5), and nitrogen dioxide (NO2). Although previous studies have shown that AP has been associated with various cardiovascular, respiratory and inflammatory diseases, the relationship between AP and psoriasis remains unknown. This nationwide, population-based case–control study aims to explore the relationship between AP and incident psoriasis, using data from the National Health Insurance Research Database of Taiwan between 2012 and 2018. Patients with newly diagnosed psoriasis were included, which was confirmed by board-certified dermatologists or rheumatologists on at least three occasions. Exclusion criteria included patients with a diagnosis of psoriasis before 2012, patients under 20 years of age in 2012, or incomplete AP data. For each patient with psoriasis, four controls were matched considering age, sex, urbanization level, index date and comorbidities. AP data were retrieved from 73 general air quality monitoring stations operated by the Taiwan Environmental Protection Administration. Exposure to air pollutants (SO2, CO, O3, PM10, PM2.5 and NO2) was analysed in the 5 years preceding psoriasis diagnosis and in controls. The mean pollution levels were further divided into four quartiles (1 = lowest and 4 = highest) to assess the dose–response effect. A conditional logistic regression model was used to assess the association. Our study included 45 093 patients with psoriasis and 180 372 matched controls. No significant intergroup difference was found in age, sex, urbanization level or comorbidities. After adjusting for age, sex, urbanization, index date and comorbidities, the second and third concentration quartiles of CO exposure were associated with a decreased psoriasis incidence [adjusted odds ratio (aOR) 0.94, 95% confidence interval (CI) 0.91–0.97; and aOR 0.91, 95% CI 0.89–0.94, respectively), while the third concentration quartile of PM10 (aOR 1.05, 95% CI 1.02–1.08) and the second concentration quartile of PM2.5 (aOR 1.08, 95% CI 1.04–1.11) were associated with a slightly increased incidence of psoriasis. Using a model incorporating all air pollutants, we found that the levels of PM10 (quartile 4: aOR 1.09, 95% CI 1.03–1.16), PM2.5 (quartile 2: aOR 1.08, 95% CI 1.04–1.13) and NO2 (quartile 4: aOR 1.14, 95% CI 1.05–1.23) were associated with a slight increase in the incidence of psoriasis. Our study shows that although exposure to certain PM10, PM2.5 and NO2 levels was significantly associated with incident psoriasis, the increased risks were trivial, with no dose–response relationship.

Abstract:Few studies have investigated the associations between ambient air pollution and mental disorders (MDs), especially in developing countries. We conducted a time-series study to explore the associations between six criteria air pollutants and daily hospital admissions for MDs in Shanghai, China, from 2013 to 2015. The MDs data were derived from the Shanghai Health Insurance System. We used over-dispersed, generalized additive models to estimate the associations after controlling for time trend, weather conditions, day of the week, and holidays. In addition, we evaluated the effect of modification by age, sex, and season. A total of 39,143 cases of hospital admissions for MDs were identified during the study period. A 10-μg/m3 increase in 2-day, moving-average concentration of inhalable particulate matter, sulfur dioxide (SO2), and carbon monoxide was significantly associated with increments of 1.27% [95% confidence interval (CI): 0.28%, 2.26%], 6.88% (95% CI, 2.75%, 11.00%), and 0.16% (95% CI: 0.02%, 0.30%) in daily hospital admissions for MDs, respectively. We observed positive but insignificant associations of fine particulate matter, nitrogen dioxide and ozone. The estimated association of SO2 was relatively robust to the adjustment of simultaneous exposure to other pollutants. We found generally stronger associations of air pollutants with MDs in warm seasons than in cool seasons. There were no significant differences in the associations between different sex and age groups. This study suggested that short-term exposure to air pollution, especially to sulfur dioxide, was associated with increased risk of hospital admissions for MDs in Shanghai, China.

Abstract:Exposure to outdoor air pollution may affect incidence and severity of coronavirus disease 2019 (COVID-19). In this retrospective cohort based on patient records from the Greater Manchester Care Records, all first COVID-19 cases diagnosed between March 1, 2020 and May 31, 2022 were followed until COVID-19 related hospitalization or death within 28 days. Long-term exposure was estimated using mean annual concentrations of particulate matter with diameter <2.5 μm (PM 2.5 ), <10 μm (PM 10 ), nitrogen dioxide (NO 2 ), ozone (O 3 ), sulphur dioxide (SO 2 ) and benzene (C 6 H 6 ) in 2019 using a validated air pollution model developed by the Department for Environment, Food and Rural Affairs (DEFRA). The association of long-term exposure to air pollution with COVID-19 hospitalization and mortality were estimated using multivariate logistic regression models after adjusting for potential individual, temporal and spatial confounders. Significant positive associations were observed between PM 2.5 , PM 10, NO 2, SO 2, benzene and COVID-19 hospital admissions with odds ratios (95% Confidence Intervals [CI]) of 1.27 (1.25–1.30), 1.15 (1.13–1.17), 1.12 (1.10–1.14), 1.16 (1.14–1.18), and 1.39 (1.36–1.42), (per interquartile range [IQR]), respectively. Significant positive associations were also observed between PM 2.5 , PM 10 , SO 2 , or benzene and COVID-19 mortality with odds ratios (95% CI) of 1.39 (1.31–1.48), 1.23 (1.17–1.30), 1.18 (1.12–1.24), and 1.62 (1.52–1.72), per IQR, respectively. Individuals who were older, overweight or obese, current smokers, or had underlying comorbidities showed greater associations between all pollutants of interest and hospital admission, compared to the corresponding groups. Long-term exposure to air pollution is associated with developing severe COVID-19 after a positive SARS-CoV-2 infection, resulting in hospitalization or death.

Abstract:Background: Air pollution and genetic risk have been found to contribute to both onset and development of psoriasis. However, the extent to which genetic susceptibility modifies the effects of air pollutants on the risk of incident psoriasis remains unknown. Objectives: Our study aimed to assess the association between joint exposure to multiple air pollutants and the risk of psoriasis and the modification by the genetic susceptibility. Methods: This prospective study included 451,064 participants with complete air pollution data and free of psoriasis at baseline from the UK Biobank. All participants were enrolled from 2006 to 2010 and followed up to 2022. The air pollution score (APS) was calculated to assess the joint exposure to multiple air pollutants, including particulate matter (PM) with diameters ≤ 2.5 μm (PM2.5), between 2.5 and 10 μm (PM2.5-10), and ≤ 10 μm (PM10), as well as nitrogen dioxide (NO2) and nitrogen oxides (NOx). To evaluate the genetic risk, the polygenic risk score (PRS) for psoriasis was constructed. The Cox proportional hazard models were used to assess the association of air pollution and genetic susceptibility with the risk of psoriasis. Stratified analyses were conducted based on the individual characteristics. Results: During a median follow-up of 13.79 years, 4414 psoriasis events were recorded. The hazard ratios (HRs) [95% confidence intervals (CIs)] for psoriasis were 1.036 (0.936-1.147), 1.091 (0.987-1.206), 1.159 (1.048-1.283), and 1.163 (1.052-1.286) in higher quintile groups compared with the lowest quintile of APS (P trend <0.05). When considering genetic susceptibility, participants with high PRS and high APS had the greatest risk of incident psoriasis [HR (95% CI): 1.962 (1.630-2.362)] than those with low PRS and low APS. The HRs (95% CIs) for PM2.5-10, NOx, PM2.5 absorbance, PM2.5, NO2, and PM10 in the group with high exposure level and genetic risk were 1.831 (1.537-2.181), 1.722 (1.431-2.073), 1.698 (1.416-2.037), 1.619 (1.353-1.938), 1.504 (1.252-1.806), and 1.425 (1.192-1.704), respectively. Conclusions: Long-term exposure to various air pollutants is positively associated with an increased risk of incident psoriasis, particularly in individuals with high genetic risk. More comprehensive measures are needed to reduce the air pollution levels for better prevention of psoriasis.
