Olea europaea L. and Hyphaene thebaica L. combination, from traditional utilization to rational formulation: Preclinical and clinical efficacy on hypertensives patients

Abstract:Ethnopharmacological relevance Hyphaene thebaica L. (HT) pericarp is usually used by traditional healers in Africa to cure and sometimes to prevent hypertension, alone or in a mixture of a polyherbal preparation (PHP). Aim of the study The first objective of this study was to test the antihypertensive effect of HT alone and in combination with Olea europaea L. leaf (OEL) in PHP formulation. The secondary objective was to assess the clinical benefits of the oral new formulation of PHP (Ifanosine). Materials and methods The antihypertensive effect of the aqueous extracts of HT, OEL and Ifanosine, was studied in vitro on rat aorta using an isometric transducer and the rationalized formulation was realized using a functional approach. To evaluate intestinal bioavailability of HT, OEL and PHP, Ussing chambers were used to measure mucosal to serosal fluxes and total transepithelial electrical conductance (Gt). The phytochemical composition of OEL and HT extracts was assessed by HPLC. Clinical benefits of the new formulation were evaluated in an observational study, a cohort of 32 hypertensives patients received a fixed oral dose of 3.5 mg of PHP, three times a day for 30 days. Clinical benefits were evaluated on blood pressure (SBP, DBP), hematological and biochemical parameters, such as triglycerides, HDL-c, LDL-c, VLDL-c, Liver and kidney enzymes (ALT, AST) and, creatinine. Results The crude aqueous extract of HT, EOL and PHP induced dose-dependent relaxation of rat segment isolated aorta rings after precontraction induced by 20 nM of phenylephrine (PHE), with an IC 50 of HT and OEL were respectively 44.76 ± 1.35 ng/mL and 58.67 ± 1.02 ng/mL, whereas that of PHP was 29,46 ± 0.26 ng/mL. Calculated Schild plot of PHE concentration-response curve in the presence of a different concentration of HT and OEL indicate that the p A 2 value of OEL and HT were respectively 1 and 0.6 whereas that of PHP was 0.06. The intestinal bioavailability of different preparation was respectively P app = 7.77 ± 0.72 × 10-6 cm/s for OEL, P app = 7.92 ± 0.43 × 10 -6 cm/s for HT, and P app = 9.68 ± 0.58 × 10 -6 cm/s for PHP when compared to the positive control of prazosin (PRZ) P app = 12.44 ± 1.27 × 10 -6 cm/s. The most dominant active constituents are flavonoids and phenolic acids such as Oleuropein, Oleuropein aglucone, Oleacein, Hydroxytyrosol, Naringenin, Naringin, and Luteolin 7-O-rutinoside. Clinical benefits were observed in humans after utilization of PHP which reduction on SBP (−12.29 mmHg), DBP (−6.37 mmHg), LDL-c (−16.50 mg/dl), VLDL-c (−6.90 mg/dl) and increase of HDL-c (+4.5 mg/dl) without overt adverse effects. Conclusion this study confirms the traditional use of HT to treat or prevent hypertension and the use of alternative and combinatorial polyphytotherapy (ACPP) to better promote traditional recipes.
