Abstract:Purpose: Treatment plans are used for patients under radiotherapy in clinics. Before execution, these plans are checked for safety and quality by human experts. A few of them were identified with flaws and needed further improvement. To automate this checking process, an unsupervised learning method based on an autoencoder was proposed. Methods: First, features were extracted from the treatment plan by human experts. Then, these features were assembled and used for model learning. After network optimization, a reconstruction error between the predicted and target signals was obtained. Finally, the questionable plans were identified based on the value of the reconstruction error. A large value of the reconstruction error indicates a longer distance from the standard distribution of normal plans. A total of 576 treatment plans for breast cancer patients were used for the test. Among them, 19 were questionable plans identified by human experts. To evaluate the performance of the autoencoder, it was compared with four baseline detection algorithms, namely, local outlier factor (LOF), hierarchical density-based spatial clustering of applications with noise (HDBSCAN), one-class support vector machine (OC-SVM), and principal component analysis (PCA). Results: The results showed that the autoencoder achieved the best performance than the other four baseline algorithms. The AUC value of the autoencoder was 0.9985, while the second one was 0.9535 (LOF). While maintaining 100% recall, the average accuracy and precision of the results by the autoencoder were 0.9658 and 0.5143, respectively. While maintaining 100% recall, the average accuracy and precision of the results by LOF were 0.8090 and 0.1472, respectively. Conclusion: The autoencoder can effectively identify questionable plans from a large group of normal plans. There is no need to label the data and prepare the training data for model learning. The autoencoder provides an effective way to carry out an automatic plan checking in radiotherapy.

Abstract:<h3>Purpose/Objective(s)</h3> To evaluate the feasibility and safety of creating radiation treatment plans based on a convolutional neural network auto-contouring (AC) model for patients receiving radiation for localized prostate cancer with a radio-opaque rectal spacer hydrogel. <h3>Materials/Methods</h3> The deep learning model produces ACs for target volumes (prostate, proximal seminal vesicle), OARs (rectum, bladder, penile bulb, femoral heads), and rectal hydrogel spacers. ACs were compared to manual contours (MC) by MDs for clinical use. Individual volumes and overall performance (composite score) were previously evaluated and reported, qualitatively using a 1 (minor discrepancy), 2 (moderate discrepancy), 3 (significant discrepancy), and 4 (rejected, gross error) scale and quantitatively using Dice Similarity Coefficient (DSC) and mean distance to agreement. VMAT treatment plans were created on unedited AC volumes from five composite 1 (C1) cases and five composite 2 (C2) cases and dose volume metrics were computed for the final MCs. Dosimetry comparison was performed to assess the safety of delivering a treatment plan created from unedited ACs. Cases were planned to 50.4 Gy in 28 fractions to the SVs, with a SIB of 70 Gy the prostate. PTV expansions were CTV+5 mm (CTV+4 mm posteriorly). <h3>Results</h3> Of 62 initial cases evaluated, 11 were scored as a C1, 28 were scored as a C2, and 23 received a C3. 10 cases were randomly selected for treatment planning. For prostate (CTV P) in selected C1 and C2 cases, mean score was 1 and 1.8 and mean DSC was 0.919 and 0.876, respectively. Clinical goals for planning and dosimetric data from treatment plans are shown in Table 1. The mean V70 Gy for prostate CTV/PTV was 99.83%/90.05% for C1 cases and 99.46%/89.15% for C2 cases. All clinical goals for OARs were met in both cohorts. <h3>Conclusion</h3> 63% of all cases evaluated received a C1 or C2 score. Our results indicate the treatment plans generated from unedited ACs on these cases respected all clinical goals for OARs. Although C1 plans produced better PTV coverage than C2, almost all plans had sub-optimal PTV coverage, indicating the need for MDs to edit target volumes before treatment planning.

Abstract:As an unsupervised data representation neural network, auto-encoder (AE) has shown great potential in denoising, dimensionality reduction, and data reconstruction. Many AE-based background (BKG) modeling methods have been developed for hyperspectral anomaly detection (HAD). However, their performance is subject to their unbiased reconstruction of BKG and target pixels. This article presents a rather different low rank and sparse matrix decomposition (LRaSMD) method based on AE, named auto-encoder and independent target (AE-IT), for hyperspectral anomaly detection. First, the encoder weight matrix, obtained by a designed AE network, is utilized to construct a projector for generating a low-rank component in the encoder subspace. By adaptively and reasonably determining the number of neurons in the latent layer, the designed AE-based method can promote the reconstruction of BKG. Second, to ensure independence and representativeness, the component in the encoder orthogonal subspace is made into a sphere and followed by finding of unsupervised targets to construct an anomaly space. In order to mitigate the influence of noise on anomaly detection, sparse cardinality (SC) constraint is enforced on the component in the anomaly space for obtaining the sparse anomaly component. Finally, anomaly detector is constructed by combining Mahalanobi distance and multi-components, which include encoder component and sparse anomaly component, to detect anomalies. The experimental results demonstrate that AE-IT performs competitively compared to the LRaSMD-based models and AE-based approaches.

Abstract:The unsupervised anomaly detection task based on high-dimensional or multidimensional data occupies a very important position in the field of machine learning and industrial applications; especially in the aspect of network security, the anomaly detection of network data is particularly important. The key to anomaly detection is density estimation. Although the methods of dimension reduction and density estimation have made great progress in recent years, most dimension reduction methods are difficult to retain the key information of original data or multidimensional data. Recent studies have shown that the deep autoencoder (DAE) can solve this problem well. In order to improve the performance of unsupervised anomaly detection, we propose an anomaly detection scheme based on a deep autoencoder (DAE) and clustering methods. The deep autoencoder is trained to learn the compressed representation of the input data and then feed it to clustering approach. This scheme makes full use of the advantages of the deep autoencoder (DAE) to generate low-dimensional representation and reconstruction errors for the input high-dimensional or multidimensional data and uses them to reconstruct the input samples. The proposed scheme could eliminate redundant information contained in the data, improve performance of clustering methods in identifying abnormal samples, and reduce the amount of calculation. To verify the effectiveness of the proposed scheme, massive comparison experiments have been conducted with traditional dimension reduction algorithms and clustering methods. The results of experiments demonstrate that, in most cases, the proposed scheme outperforms the traditional dimension reduction algorithms with different clustering methods.

Abstract:Appropriate dosing of radiation is crucial to patient safety in radiotherapy. Current quality assurance depends heavily on a peer-review process, where the physicians' peer review on each patient's treatment plan, including dose and fractionation. However, such a process is manual and laborious. Physicians may not identify errors due to time constraints and caseload. We designed a novel prescription anomaly detection algorithm that utilizes historical data to predict anomalous cases. Such a tool can serve as an electronic peer who will assist the peer-review process providing extra safety to the patients. In our primary model, we created two dissimilarity metrics, R and F. R defining how far a new patient's prescription is from historical prescriptions. F represents how far away a patient's feature set is from the group with an identical or similar prescription. We flag prescription if either metric is greater than specific optimized cut-off values. We used thoracic cancer patients (n=2356) as an example and extracted seven features. Here, we report our testing f1 score, between 75%-94% for different treatment technique groups. We also independently validate our results by conducting a mock peer review with three thoracic specialists. Our model has a lower type 2 error rate compared to manual peer-review physicians. Our model has many advantages over traditional machine learning algorithms, particularly in that it does not suffer from class imbalance. It can also explain why it flags each case and separate prescription and non-prescription-related features without learning from the data.

Abstract:Background Quality assurance (QA) of patient‐specific treatment plans for intensity‐modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) necessitates prior validation. However, the standard methodology exhibits deficiencies and lacks sensitivity in the analysis of positional dose distribution data, leading to difficulties in accurately identifying reasons for plan verification failure. This issue complicates and impedes the efficiency of QA tasks. Purpose The primary aim of this research is to utilize deep learning algorithms for the extraction of 3D dose distribution maps and the creation of a predictive model for error classification across multiple machine models, treatment methodologies, and tumor locations. Method We devised five categories of validation plans (normal, gantry error, collimator error, couch error, and dose error), conforming to tolerance limits of different accuracy levels and employing 3D dose distribution data from a sample of 94 tumor patients. A CNN model was then constructed to predict the diverse error types, with predictions compared against the gamma pass rate (GPR) standard employing distinct thresholds (3%, 3 mm; 3%, 2 mm; 2%, 2 mm) to evaluate the model's performance. Furthermore, we appraised the model's robustness by assessing its functionality across diverse accelerators. Results The accuracy, precision, recall, and F1 scores of CNN model performance were 0.907, 0.925, 0.907, and 0.908, respectively. Meanwhile, the performance on another device is 0.900, 0.918, 0.900, and 0.898. In addition, compared to the GPR method, the CNN model achieved better results in predicting different types of errors. Conclusion When juxtaposed with the GPR methodology, the CNN model exhibits superior predictive capability for classification in the validation of the radiation therapy plan on different devices. By using this model, the plan validation failures can be detected more rapidly and efficiently, minimizing the time required for QA tasks and serving as a valuable adjunct to overcome the constraints of the GPR method.

Abstract:Background and purpose: Artificial intelligence (AI) based treatment planning tools are being implemented in clinic. However, human interactions with such AI tools are rarely analyzed. This study aims to comprehend human planner's interaction with the AI planning tool and incorporate the analysis to improve the existing AI tool. Materials and methods: An in-house AI tool for whole breast radiation therapy planning was deployed in our institution since 2019, among which 522 patients were included in this study. The AI tool automatically generates fluence maps of the tangential beams to create an AI plan. Human planner makes fluence edits deemed necessary and after attending physician approval for treatment, it is recorded as final plan. Manual modification value (MMV) maps were collected, which is the difference between the AI-plan and the final plan. Subsequently, a human-AI interaction (HAI) model using full scale connected U-Net was trained to learn such interactions and perform plan enhancements. The trained HAI model automatically modifies the AI plan to generate AI-modified plans (AI-m plan), simulating human editing. Its performance is evaluated against original AI-plan and final plan. Results: AI-m plan showed statistically significant improvement in hotspot control over the AI plan, with an average of 25.2cc volume reduction in breast V105% (p=0.011) and 0.805% decrease in Dmax (p<.001). It also maintained the same PTV coverage as the final plan, demonstrating the model has captured the clinic focus of improving PTV hot spots without degrading coverage. Conclusions: The proposed HAI model has demonstrated capability of further enhancing the AI plan via modeling human-AI tool interactions. This study shows analysis of human interaction with the AI planning tool is a significant step to improve the AI tool.

Infants' Consumption of a New Food Enhances Acceptance of Similar Foods

Abstract:The number of feedings needed to increase intake of a novel target food was investigated, and whether exposure effects generalized to other foods in a sample of 4 to 7-month-old infants (N=39). Other foods varied in their similarity to the target food, including the same food prepared by another manufacturer, similar foods (other fruits for infants receiving a target fruit) and a different food (e. g. vegetables for infants receiving a target fruit). Infants were fed the target food once a day for 10 days. Intake was used to indicate acceptance. Results revealed that exposure dramatically increased infants' intake of the target food, from an average of 35-72 g. Intake of the different food was unchanged. Same and similar food intake increased with target food exposure. Intake of the target, same and similar foods nearly doubled to 60 g after one exposure to the target food. These rapid increases in intake contrast the slower changes seen in young children. Results for the other foods suggest that infants may have difficulty discriminating among many foods.

Abstract:Accurate response prediction allows for personalized cancer treatment of locally advanced rectal cancer (LARC) with neoadjuvant chemoradiation. In this work, we designed a convolutional neural network (CNN) feature extractor with switchable 3D and 2D convolutional kernels to extract deep learning features for response prediction. Compared with radiomics features, convolutional kernels may adaptively extract local or global image features from multi-modal MR sequences without the need of feature predefinition. We then developed an unsupervised clustering based evaluation method to improve the feature selection operation in the feature space formed by the combination of CNN features and radiomics features. While normal process of feature selection generally includes the operations of classifier training and classification execution, the process needs to be repeated many times after new feature combinations were found to evaluate the model performance, which incurs a significant time cost. To address this issue, we proposed a cost effective process to use a constructed unsupervised clustering analysis indicator to replace the classifier training process by indirectly evaluating the quality of new found feature combinations in feature selection process. We evaluated the proposed method using 43 LARC patients underwent neoadjuvant chemoradiation. Our prediction model achieved accuracy, area-under-curve (AUC), sensitivity and specificity of 0.852, 0.871, 0.868, and 0.735 respectively. Compared with traditional radiomics methods, the prediction models (AUC=0.846) based on deep learning-based feature sets are significantly better than traditional radiomics methods (AUC=0.714). The experiments also showed following findings: 1) the features with higher predictive power are mainly from high-order abstract features extracted by CNN on ADC images and T2 images; 2) both ADC_Radiomics and ADC_CNN features are more advantageous for predicting treatment responses than the radiomics and CNN features extracted from T2 images; 3) 3D CNN features are more effective than 2D CNN features in the treatment response prediction. The proposed unsupervised clustering indicator is feasible with low computational cost, which facilitates the discovery of valuable solutions by highlighting the correlation and complementarity between different types of features.

Iterative Dataset Optimization in Automated Planning: Implementation for Breast and Rectal Cancer Radiotherapy

Abstract:PURPOSE:To develop a new automated treatment planning solution for breast and rectal cancer radiotherapy.METHODS:The automated treatment planning solution developed in this study includes selection of the iterative optimized training dataset, dose volume histogram (DVH) prediction for the organs at risk (OARs), and automatic generation of clinically acceptable treatment plans. The iterative optimized training dataset is selected by an iterative optimization from 40 treatment plans for left-breast and rectal cancer patients who received radiation therapy. A two-dimensional kernel density estimation algorithm (noted as two parameters KDE) which incorporated two predictive features was implemented to produce the predicted DVHs. Finally, 10 additional new left-breast treatment plans are re-planned using the Pinnacle3 Auto-Planning (AP) module (version 9.10, Philips Medical Systems) with the objective functions derived from the predicted DVH curves. Automatically generated re-optimized treatment plans are compared with the original manually optimized plans.RESULTS:By combining the iterative optimized training dataset methodology and two parameters KDE prediction algorithm, our proposed automated planning strategy improves the accuracy of the DVH prediction. The automatically generated treatment plans using the dose derived from the predicted DVHs can achieve better dose sparing for some OARs without compromising other metrics of plan quality.CONCLUSIONS:The proposed new automated treatment planning solution can be used to efficiently evaluate and improve the quality and consistency of the treatment plans for intensity-modulated breast and rectal cancer radiation therapy.
Rethinking Autoencoders for Medical Anomaly Detection from A Theoretical Perspective

Abstract:Medical anomaly detection aims to identify abnormal findings using only normal training data, playing a crucial role in health screening and recognizing rare diseases. Reconstruction-based methods, particularly those utilizing autoencoders (AEs), are dominant in this field. They work under the assumption that AEs trained on only normal data cannot reconstruct unseen abnormal regions well, thereby enabling the anomaly detection based on reconstruction errors. However, this assumption does not always hold due to the mismatch between the reconstruction training objective and the anomaly detection task objective, rendering these methods theoretically unsound. This study focuses on providing a theoretical foundation for AE-based reconstruction methods in anomaly detection. By leveraging information theory, we elucidate the principles of these methods and reveal that the key to improving AE in anomaly detection lies in minimizing the information entropy of latent vectors. Experiments on four datasets with two image modalities validate the effectiveness of our theory. To the best of our knowledge, this is the first effort to theoretically clarify the principles and design philosophy of AE for anomaly detection. PatchCL-AE: Anomaly detection for medical images using patch-wise contrastive learning-based auto-encoder

Abstract:Anomaly detection is an important yet challenging task in medical image analysis. Most anomaly detection methods are based on reconstruction, but the performance of reconstruction-based methods is limited due to over-reliance on pixel-level losses. To address the limitation, we propose a patch-wise contrastive learning-based auto-encoder for medical anomaly detection. The key contribution is the patch-wise contrastive learning loss that provides supervision on local semantics to enforce semantic consistency between corresponding input-output patches. Contrastive learning pulls corresponding patch pairs closer while pushing non-corresponding ones apart between input and output, enabling the model to learn local normal features better and improve discriminability on anomalous regions. Additionally, we design an anomaly score based on local semantic discrepancies to pinpoint abnormalities by comparing feature difference rather than pixel variations. Extensive experiments on three public datasets (i.e., brain MRI, retinal OCT, and chest X-ray) achieve state-of-the-art performance, with our method achieving over 99% AUC on retinal and brain images. Both the contrastive patch-wise supervision and patch-discrepancy score provide targeted advancements to overcome the weaknesses in existing approaches.

Abstract:Abstract Using deep learning models to analyze patients with intracranial tumors, to study the image segmentation and standard results by clinical depiction complications of cerebral edema after receiving radiotherapy. In this study, patients with intracranial tumors receiving computer knife (CyberKnife M6) stereotactic radiosurgery were followed using the treatment planning system (MultiPlan 5.1.3) to obtain before-treatment and four-month follow-up images of patients. The TensorFlow platform was used as the core architecture for training neural networks. Supervised learning was used to build labels for the cerebral edema dataset by using Mask region-based convolutional neural networks (R-CNN), and region growing algorithms. The three evaluation coefficients DICE, Jaccard (intersection over union, IoU), and volumetric overlap error (VOE) were used to analyze and calculate the algorithms in the image collection for cerebral edema image segmentation and the standard as described by the oncologists. When DICE and IoU indices were 1, and the VOE index was 0, the results were identical to those described by the clinician.The study found using the Mask R-CNN model in the segmentation of cerebral edema, the DICE index was 0.88, the IoU index was 0.79, and the VOE index was 2.0. The DICE, IoU, and VOE indices using region growing were 0.77, 0.64, and 3.2, respectively. Using the evaluated index, the Mask R-CNN model had the best segmentation effect. This method can be implemented in the clinical workflow in the future to achieve good complication segmentation and provide clinical evaluation and guidance suggestions.

Abstract:Quantitative medical image computing (radiomics) has been widely applied to build prediction models from medical images. However, overfitting is a significant issue in conventional radiomics, where a large number of radiomic features are directly used to train and test models that predict genotypes or clinical outcomes. In order to tackle this problem, we propose an unsupervised learning pipeline composed of an autoencoder for representation learning of radiomic features and a Gaussian mixture model based on minimum message length criterion for clustering. By incorporating probabilistic modeling, disease heterogeneity has been taken into account. The performance of the proposed pipeline was evaluated on an institutional MRI cohort of 108 patients with colorectal cancer liver metastases. Our approach is capable of automatically selecting the optimal number of clusters and assigns patients into clusters (imaging subtypes) with significantly different survival rates. Our method outperforms other unsupervised clustering methods that have been used for radiomics analysis and has comparable performance to a state-of-the-art imaging biomarker.
Clustering-Based Deep Autoencoders for Network Anomaly Detection

Abstract:AbstractA novel hybrid approach between clustering methods and autoencoders (AEs) is introduced for detecting network anomalies in a semi-supervised manner. A previous work has developed regularized AEs, namely Shrink AE (SAE) and Dirac Delta Variational AE (DVAE) that learn to represent normal data into a very small region being close to the origin in their middle hidden layers (latent representation). This work based on the assumption that normal data points may share some common characteristics, so they can be forced to distribute in a small single cluster. In some scenarios, however, normal network data may contain data from very different network services, which may result in a number of clusters in the normal data. Our proposed hybrid model attempts to automatically discover these clusters in the normal data in the latent representation of AEs. At each iteration, an AE learns to map normal data into the latent representation while a clustering method tries to discover clusters in the latent normal data and force them being close together. The co-training strategy can help to reveal true clusters in normal data. When a querying data point coming, it is first mapped into the latent representation of the AE, and its distance to the closest cluster center can be used as an anomaly score. The higher anomaly score a data point has, the more likely it is anomaly. The method is evaluated with four scenarios in the CTU13 dataset, and experiments illustrate that the proposed hybrid model often out-performs SAE on three out of four scenarios.

Abstract:Anomaly detection is an important task for medical image analysis, which can alleviate the reliance of supervised methods on large labelled datasets. Most existing methods use a pixel-wise self-reconstruction framework for anomaly detection. However, there are two challenges of these studies: 1) they tend to overfit learning an identity mapping between the input and output, which leads to failure in detecting abnormal samples; 2) the reconstruction considers the pixel-wise differences which may lead to an undesirable result. To mitigate the above problems, we propose a novel heterogeneous Auto-Encoder (Hetero-AE) for medical anomaly detection. Our model utilizes a convolutional neural network (CNN) as the encoder and a hybrid CNN-Transformer network as the decoder. The heterogeneous structure enables the model to learn the intrinsic information of normal data and enlarge the difference on abnormal samples. To fully exploit the effectiveness of Transformer in the hybrid network, a multi-scale sparse Transformer block is proposed to trade off modelling long-range feature dependencies and high computational costs. Moreover, the multi-stage feature comparison is introduced to reduce the noise of pixel-wise comparison. Extensive experiments on four public datasets (i.e., retinal OCT, chest X-ray, brain MRI, and COVID-19) verify the effectiveness of our method on different imaging modalities for anomaly detection. Additionally, our method can accurately detect tumors in brain MRI and lesions in retinal OCT with interpretable heatmaps to locate lesion areas, assisting clinicians in diagnosing abnormalities efficiently.

Application of Artificial Intelligence in Radiotherapy of Nasopharyngeal Carcinoma with Magnetic Resonance Imaging

Abstract:The value of automatic organ-at-risk outlining software for radiotherapy is based on artificial intelligence technology in clinical applications. The accuracy of automatic segmentation of organs at risk (OARs) in radiotherapy for nasopharyngeal carcinoma was investigated. In the automatic segmentation model which is proposed in this paper, after CT scans and manual segmentation by physicians, CT images of 147 nasopharyngeal cancer patients and their corresponding outlined OARs structures were selected and grouped into a training set (115 cases), a validation set (12 cases), and a test set (20 cases) by complete randomization. Adaptive histogram equalization is used to preprocess the CT images. End-to-end training is utilized to improve modeling efficiency and an improved network based on 3D Unet (AUnet) is implemented to introduce organ size as prior knowledge into the convolutional kernel size design to enable the network to adaptively extract features from organs of different sizes, thus improving the performance of the model. The DSC (Dice Similarity Coefficient) coefficients and Hausdorff (HD) distances of automatic and manual segmentation are compared to verify the effectiveness of the AUnet network. The mean DSC and HD of the test set were 0.86 ± 0.02 and 4.0 ± 2.0 mm, respectively. Except for optic nerve and optic cross, there was no statistical difference between AUnet and manual segmentation results ( P > 0.05). With the introduction of the adaptive mechanism, AUnet can achieve automatic segmentation of the endangered organs of nasopharyngeal carcinoma based on CT images more accurately, which can substantially improve the efficiency and consistency of segmentation of doctors in clinical applications.

Predicting treatment plan approval probability for high-dose-rate brachytherapy of cervical cancer using adversarial deep learning

Abstract:Objective: Predicting the probability of having the plan approved by the physician is important for automatic treatment planning. Driven by the mathematical foundation of deep learning that can use a deep neural network to represent functions accurately and flexibly, we developed a deep-learning framework that learns the probability of plan approval for cervical cancer high-dose-rate brachytherapy (HDRBT). Approach: The system consisted of a dose prediction network (DPN) and a plan-approval probability network (PPN). DPN predicts OAR D2cc and CTV D90% of the current fraction from the patient's current anatomy and prescription dose of HDRBT. PPN outputs the probability of a given plan being acceptable to the physician based on the patient's anatomy and the total dose combining HDRBT and external beam radiotherapy sessions. Training of the networks was achieved by first training them separately for a good initialization, and then jointly via an adversarial process. We collected approved treatment plans of 248 treatment fractions from 63 patients. Among them, 216 plans from 54 patients were employed in a four-fold cross validation study, and the remaining 32 plans from other 9 patients were saved for independent testing. Main results: DPN predicted equivalent dose of 2 Gy for bladder, rectum, sigmoid D2cc and CTV D90% with a relative error of 11.51±6.92%, 8.23±5.75%, 7.12±6.00%, and 10.16±10.42%, In a task that differentiates clinically approved plans and disapproved plans generated by perturbing doses in ground truth approved plans by 20%, PPN achieved accuracy, sensitivity, specificity, and area under the curve 0.70, 0.74, 0.65, and 0.74. Significance: We demonstrated the feasibility of developing a novel deep-learning framework that predicts a probability of plan approval for HDRBT of cervical cancer, which is an essential component in automatic treatment planning.
