The clinical pregnancy rate (CPR), implantation rate (IR), live birth rate (LBR), miscarriage rate, multiple pregnancy rate (MPR), preterm birth rate, and low-birth-weight rate were compared between the 3 groups. Transfers of 2 high-quality embryos resulted in significant differences in CPR, IR, LBR, MPR, and neonatal weight among the 3 groups, but no differences in premature birth and abortion rates were observed. Transfers with 1 high-quality and 1 fair-quality embryo resulted in significant differences in CPR, IR, and LBR among the 3 groups. Comparison of transfers of 2 high-quality embryos with 1 high-quality and 1 fair-quality embryo showed that the CPR and LBR were significantly lower for the latter in Groups A and C, but Group B had no salient changes. Higher IR and LBR and lower MPR may be achieved by selection of optimal embryo types for patients of different ages. Two high-quality embryos need to be transferred in women older than 38 years. For women aged 36 to 37 years, 1 high-quality embryo or 1 high-quality plus 1 fair-quality embryo should be singled out for transfer. For women younger than 35 years, a single high-quality embryo should be selected for transfer.

[Study of the Best Embryo Transfer Strategy in Frozen-Thawed Embryo Transfer Cycle].

Abstract:OBJECTIVES:To study the relationship of the number and quality of embryos transferred with clinical pregnancy rate and multiple pregnancy rate in frozen-thawed embryo transfer (FET) cycle. METHODS:Retrospective analysis of the clinical data of 995 FET cycles. Patients were stratified as age < 35 or > or = 35, Both groups' clinical pregnancy rate and multiple pregnancy rate were compared according to the number of embryos and high-quality embryos transferred. RESULTS:(1) The clinical pregnancy rates were 50.0%, 56.6% and 56.5%, and twin pregnancy rates were 6.3%, 43.8% and 30.6% respectively in < 35 group when one, two or three embryos were transferred. There is not any significantly statistic difference among these subgroups (P > 0.05). However, the twin pregnancy rate in one-embryo transferred subgroup was significantly lower than that of two-embryo transferred subgroup (P < 0.05). Moreover, the clinical pregnancy rate was not different among single high-quality embryo transferred subgroup and other high-quality embryo transferred subgroups, the multiple pregnancy rate was lower than the others. (2) The clinical pregnancy rates were 0, 47.3% and 53.8%, and twin pregnancy rates were 0, 25.7% and 25.7% respectively in > or = 35 group when one, two or three embryos were transferred. The clinical pregnancy rate and twin pregnancy rate between two-embryo and three-embryo transferred subgroups were not significantly different (P > 0.05). Moreover, the clinical pregnancy rate was not different among two-embryo transferred containing one high-quality embryo subgroup and other high-quality embryo transferred subgroups, the multiple pregnancy rate was lower than the others. CONCLUSIONS:There is a close relationship between the number and quality of embryos transferred and clinical pregnancy rate, multiple pregnancy rate in FET cycles. For the patient aged < 35, we recommend performing single high-quality embryo transfer in order to reduce the multiple pregnancy. For the patient aged > or = 35, two-embryo transfer containing one high-quality embryo is enough to obtain a satisfied clinical pregnancy rate and an acceptable multiple pregnancy rate.

Abstract:Abstract Background Blastocyst development by extended culture in vitro allows the embryos to ‘select’ themselves, thus successful growth to the blastocyst stage is a reflection of the developmental competence of cleavage stage embryos in a cohort. The study aims to determine whether the number of frozen blastocysts is associated with live birth rates of the transferred fresh embryos. Method The retrospective study included 8676 cycles of first fresh embryo transfer from January 2016 to June 2019 at a fertility center of a university hospital. The patients with ≥ 10 oocytes retrieved were divided into three groups according to the number of frozen blastocysts: 0 (group 1), 1–2 (group 2), and ≥ 3 (group 3). The primary outcome measure was the live birth. The secondary outcome measures included clinical pregnancy rates and implantation rates. Logistic regression analysis was also performed. Results Live birth rates in patients with ≥ 3 and 1–2 frozen blastocysts were 47.6% and 46.1%, respectively, which were significantly higher than that in patients without blastocyst (36.0%). The clinical pregnancy rate in group 3 was 65.1%, which was also significantly higher than the other two groups (47.0% and 59.2%). The implantation rates were 35.5%, 47.6%, and 56.0% in the three groups, respectively (P < 0.001). Compared with groups of frozen blastocysts, 0 frozen blastocyst yielded a lower rate of live birth (the adjusted odds ratio: 0.526, 95% CI: 0.469, 0.612). Conclusion In patients with optimal ovarian response that retrieved ≥ 10 oocytes, fresh embryos transfer followed by having blastocysts frozen is a strong indicator of pregnancy achievement, but the number of frozen blastocysts (if not = 0) has limited value in predicting live birth rates.

Abstract:BACKGROUND:Elective single embryo transfer (eSET) has been increasingly advocated, but concerns about the lower pregnancy rate after reducing the number of embryos transferred have encouraged transfer of multiple embryos. Extended embryo culture combined with electively freezing all embryos and undertaking a deferred frozen embryo transfer might increase pregnancy rate after eSET. We aimed to establish whether elective frozen single blastocyst transfer improved singleton livebirth rate compared with fresh single blastocyst transfer.METHODS:This multicentre, non-blinded, randomised controlled trial was undertaken in 21 academic fertility centres in China. 1650 women with regular menstrual cycles undergoing their first cycle of in-vitro fertilisation were enrolled from Aug 1, 2016, to June 3, 2017. Eligible women were randomly assigned to either fresh or frozen single blastocyst transfer. The randomisation sequence was computer generated, with block sizes of two, four, or six, stratified by study site. For those assigned to frozen blastocyst transfer, all blastocysts were cryopreserved and a delayed frozen-thawed single blastocyst transfer was done. The primary outcome was singleton livebirth rate. Analysis was by intention to treat. This trial is registered at the Chinese Clinical Trial Registry, number ChiCTR-IOR-14005405.FINDINGS:825 women were assigned to each group and included in analyses. Frozen single blastocyst transfer resulted in higher rates of singleton livebirth than did fresh single blastocyst transfer (416 [50%] vs 329 [40%]; relative risk [RR] 1·26, 95% CI 1·14-1·41, p<0·0001). The risks of moderate or severe ovarian hyperstimulation syndrome (four of 825 [0·5%] in frozen single blastocyst transfer vs nine of 825 [1·1%] in fresh single blastocyst transfer; p=0·16), pregnancy loss (134 of 583 [23·0%] vs 124 of 481 [25·8%]; p=0·29), other obstetric complications, and neonatal morbidity were similar between the two groups. Frozen single blastocyst transfer was associated with a higher risk of pre-eclampsia (16 of 512 [3·1%] vs four of 401 [1·0%]; RR 3·13, 95% CI 1·06-9·30, p=0·029).INTERPRETATION:Frozen single blastocyst transfer resulted in a higher singleton livebirth rate than did fresh single blastocyst transfer in ovulatory women with good prognosis. The increased risk of pre-eclampsia after frozen blastocyst transfer warrants further studies.FUNDING:The National Key Research and Development Program of China.
Association of the duration of post-thaw culture with clinical outcome after vitrified-warmed day 3 embryo transfer in 10,464 cycles: A retrospective cohort study.

Abstract:This study aimed to investigate the effect of the duration of embryo culture on clinical outcome in vitrified-warmed cycles. This retrospective cohort study enrolled 10,464 infertile patients, with a total of 18,843 vitrified-warmed day 3 embryos from 2012 to 2017 at a single center. The patients were divided into 2 groups: 9470 cycles in the short-term culture group (0.5-8 hours of post-thaw culture) and 994 cycles in the 48 to 72 hours culture group. The independent effect of the following variables on clinical outcomes was determined: duration of post-thaw culture, maternal age, transferred embryos, embryo quality, and endometrial thickness. We found that the pregnancy rate was positively associated with the post-thaw culture time. Ordinary least square regression analyses showed that the duration of post-thaw culture was positively associated with implantation and live birth rates overall. However, the implantation and live birth rates were not significantly associated with the post-thaw culture time in the short-term culture group. Additionally, maternal age and the number of transferred embryos were independent predictors of the implantation and live birth rates. Moreover, the duration of post-thaw culture did not affect live birth weight. These results indicated that the pregnancy rate is positively associated with the duration of post-thaw culture. Therefore, under the condition of not affecting work shifts, properly prolonging the duration of post-thaw culture to improve the outcome of frozen-thawed embryo transfer should be considered.
Influence of Cleavage-Stage Embryo Quality on the In-Vitro Fertilization Outcome after Single Embryo Transfer in Fresh Cycles.

Abstract:Objective: Embryo quality is crucial for determining the outcome of embryo implantation. This study aimed to assess the impact of embryo quality on the outcome of in vitro fertilization/single-embryo transfer (IVF-SET). Materials and methods: This retrospective study included 2531 fresh IVF-SET cycles, including 277 poor-quality and 2254 top-quality embryos. The clinical pregnancy rate, miscarriage rate, live birth, implantation rate, pregnancy outcome and complication were analyzed and compared. Risk factors associated with miscarriage rate and pregnancy complication were identified using logistics regression analysis. Results: Top-quality embryos resulted in higher clinical pregnancy rate (30.5% vs. 12.6%, P < 0.001) and live birth rate (23.9% vs. 9.7%, P < 0.001) compared with poor-quality embryos. Logistics regression analysis revealed that embryo quality was not correlated with miscarriage rate (95% CI 0.33-1.89) and pregnancy complications (95% CI 0.12-7.84). Maternal age and body mass index was a risk factor for miscarriage rate (95% CI 1.05-1.22) and pregnancy complication (95% CI 1.01-1.29), respectively. Conclusion: Clinical miscarriage rate and pregnancy complication were embryo quality independent. Maternal age was the risk factor for miscarriage rate. Prognostic Factors of Frozen Thawed Embryo Transfer: Analysis on 2433 Cycles

Abstract:Objective To investigate the factors influencing the prognosis of frozen thawed embryo transfer(FET)cycles.Methods Totally 2433 FET cycles were analyzed,955 of them were pregnant,while 626 were live birth,and 1478 non pregnant.They were assigned to natural cycle,ovulation induction cycle or hormone therapy(HT)cycle.Results Total clinical pregnancy rate was 35.2%(856/2433)and live birth rate was 25.7%(626/2433)in our study.There were 6203 embryos transferred to form 1026 gestational sac.The implantation rate was 16.5%(1026/6203).The pregnancy rate was 40.0%(758/1897),37.8%(70/185),and 36.2%(127/351)in natural cycle,ovulation induction cycle,and HT cycle,respectively(χ2=1.94,P=0.38),no significant differences were found in these cycles.Multiple pregnancy rates were 3.1%(1/32),14.7%(34/231)and 38.3%(227/593)when one,two,or three embryos were transferred,while the implantation rate and live birth rate were similar.Through Logistic regression analysis,we found that the numbers of high quality embryos transferred and patients' age played important roles in their clinical results,especially the number of high quality embryos transferred(OR=1.41,95% CI:1.11-1.79).It appeared a downtrend of pregnancy and live birth rate with the age.Conclusions The clinical outcomes of FET are not related to natural or ovulation induction,or HT cycles.The quality of embryos,especially the numbers of high quality embryos transferred,shows close correlation with FET outcomes.The pregnancy rate must be elevated and multiple conceive be reduced through transferring high quality embryos.
Analysis of Factors Influencing Pregnancy Rate in Frozen- Thawed Embryo Transfer

Abstract:Objective To analyse factors influencing the outcome of frozen-thawed embryo transfer (FET)Method A retrospective analysis was performed in our center on 129 thawing cycles from March 2001 to April 2003. The related parameters were compared between conceived and non-conceived cycles.Results There were totally 129 clinical pregnancies in these transfers (pregnancy rate: 27.1％). Frozen-thawed embryos were transferred to natural cycles and CC cycling and hormone replacement treatment had equal success. Groups of IVF and ICSI did not differ significantly in pregnancy rates (P＞0. 05). The pregnancy rates for one, two, three and four pre-embryos transfer were 0, 20.0％,44.1％ and 75.0％,respectively (P＜0. 05). There were statistical differences between pregnancy group or non- pregnancy group in the endometrial thickness, CES, CES/No. Of embryo. A higher pregnancy rate was observed in embryo transfers which had at least one 4-cell grade I embryo (d 2)(P＜0.01). Conclusions The most important factors influencing the implantation rate and pregnancy rate of frozen-thawed embryo transfer are age, endometrium thickness, and the number, morphology and growth rate of transferred frozen embryos of women participants.
A Clinical Analysis of Frozen/thawed Embryo Transfer in Spontaneous Cycles

Abstract:【Objective】To analyze the impact of a number of factors on the pregnancy rate of frozen/thawed embryo transfer in spontaneous cycles【.Methods】A database was analyzed retrospectively concerning frozen/thawed embryo transfer in spontaneous cycles from January 2003 to January 2004 that covered 268 cycles in 233 patients.The factors investigated were maternal age, endometrial thickness, echogenic pattern, hormone levels of luteinizinghormone (LH) surge day, the number of embryos transferred and pregnancy rate per embryo transfer【.Results】Thepregnancies were occurred in 58 cycles of all 268 natural cycles, the pregnancy rate per embryo transfer was21.6%. The mean age of the patients with pregnancy were statistically significant younger than that of the patientswithout pregnancy (30.8±3.0 years vs 31.9±4.0 years, P=0.035). The endometrial thickness on the day of LH surgeat the age of less than 35 years was significant thicker than that at the age of more than 35 years (10.2±1.8 mm vs9.3±2.0 mm, P=0.007). The pregnancy rates were 13.3%, 18.2%, and 23.8% in the cycle of transferred 1, 2, and 3embryos, respectively. However, there was no significant difference in these three groups. There was no significantdifference of endometrial thickness and echogenic pattern, between the pregnancy and the non-pregnancy group.【Conclusion】Our study suggests that the age of female partners can significantly affect the pregnancy rate of frozen/thawed embryo transfer in spontaneous cycles. The pregnancy rate was not influenced by the number of embryos transferred per cycle, endometrial thickness nor echogenic pattern.
Clinical Analysis of 12 491 Cycles Treated in Embryo Transfer Program

Abstract:OBJECTIVE:To analyze the clinical outcomes of patients treated with in vitro fertilization and embryo transfer (IVF-ET) and influence factors of pregnancy rate.METHODS:We retrospectively analyzed the clinical data of 12,491 cycles, including 6,832 fresh IVF/intracytoplast single sperm injection (ICSI) cycles and 5,659 frozen embryo transfer (FET) cycles from 2005 to 2007.RESULTS:The clinical pregnancy rate per cycle was 32.99% (2254/6832) in fresh embryo transfer program, and the live birth rate was 25.75% (1394/5413); the early pregnant loss rate was 9.36% (211/2254), and the prenatal defect rate was 1.45% (25/1722). Through analysis of these patients' basic data, we found that the patients' age, causes for infertility, egg retrieval and cycle number affected the pregnancy rate. Using logistic regression method, we found that patients' age was the most important factor affecting pregnancy outcome. In FET cycles, the clinical pregnancy rate was 38.08% (2155/5659), significantly higher than fresh embryo transfer cycles.CONCLUSIONS:IVF-ET treatment is a safe and effective method for infertility couples. However, the female age and poor ovarian response are the main factors affecting pregnancy rate. Thawed embryo transfer can increase the accumulated pregnancy rate effectively.
Factors Influencing the Outcome of Embryo Freezing and Thawing Program

Abstract:Objective: To investigate the factors that might influence the success of an embryo freezing and thawing program. Method: The relationship between the pregnancy rate in 73 cycles of embryo, freezing and thawing program and the following factors was analyzed: maternal age, E 2 level at the time of HCG trigger, embryo storage time, number of thawed embryos transferred, presence of sponsoring embryos and intact embryos. And the survival rate of thawed embryos with different morphology, cell stage and storage time was evaluated. Result: Transfer with three or more than three thawed embryos resulted in pregnancy rates of 38.5% and 35.7%, respectively, compared with 5.3% for transfer of fewer than three embryos. The presence of sponsoring embryos and intact embryos significantly increases pregnancy rate in embryo freezing and thawing program. No other factor examined had any effect on pregnancy outcome. The survival rate of good morphology embryos was higher than poor ones, but was not influenced by cell stage and storage time. Conclusion: Embryo morphology before freezing, number of thawed embryos transferred and the presence of intact embryos are important to the outcome of embryo freezing and thawing program.

Abstract:Our objective was to assess whether there is a difference in the pregnancy outcomes in the natural cycle (NC) with spontaneous LH rise compared with modified natural cycle controlled by hCG for final oocyte maturation and ovulation after frozen-thawed embryo transfer (FET). In this retrospective cohort study, we analyzed the clinical outcomes of a total of 1937 patients undergoing FET followed by endometrial preparation with the natural cycle and modified natural cycle. The primary outcome was live birth, and secondary outcomes included miscarriage rate, clinical pregnancy rate, preterm birth rate, and ectopic pregnancy rate. The type of endometrial preparation did not impact live birth (adjusted odds ratio [aOR] 0.92; 95% confidence interval [CI], 0.69–1.23), miscarriage (aOR 0.83; 95%CI, 0.50–1.39), clinical pregnancy (aOR 0.88; 95%CI, 0.66–1.18), preterm birth (aOR 0.91; 95%CI, 0.56–1.50), or ectopic pregnancy (aOR 1.06; 95%CI, 0.29–3.94). In conclusion, in women undergoing FET, natural cycles and modified natural cycles resulted in comparable clinical outcomes.

Research has suggested that frozen ET might be beneficial for certain populations, such as hyper-responders. This study aimed to compare outcomes of pregnancies between frozen and fresh embryo transfer cycles in patients with endometrial hyperplasia and carcinoma.METHODS:This retrospective cohort study was conducted at a high-volume reproductive center from January 2010 to January 2022. Patients who were diagnosed with endometrial hyperplasia with atypia and endometrial carcinoma were included. They all underwent in vitro fertilization after conservative treatment. The primary outcome was live birth after frozen and fresh embryo transfer cycles, and secondary outcomes included perinatal complications and other pregnancy outcomes.RESULTS:Overall, 259 ET cycles (130 fresh and 129 frozen) were included. The rate of live births per embryo transfer cycle of the whole cohort was 20.8% (54/259), and no significant between-group difference was found after adjusting for potential confounding factors (23.8% vs. 17.8%; adjusted OR, 0.47; 95% CI, 0.21-1.06; p=0.068). Compared to fresh embryo transfer group, the incidence of total maternal complications in the frozen embryo transfer group was significantly higher (30.4% vs. 6.5%, p=0.019). Analyzing each complication as a separate entity, patients in the frozen embryo transfer group had a higher incidence of hypertensive disorders of pregnancy (p=0.028). Multiple logistic regression analysis showed that frozen embryo transfer was related with an increased occurrence of maternal complications (OR, 6.68, 95% CI, 1.01-44.19, p=0.040).CONCLUSIONS:Among patients with endometrial hyperplasia and carcinoma, the rate of live births was comparable between both embryo transfer procedures, while frozen embryo transfer might be associated with a higher risk of maternal complications compared to that with fresh embryo transfer.

Abstract:Introduction: The utilization of frozen embryo transfer not only enhances reproductive outcomes by elevating the likelihood of live birth and clinical pregnancy but also improves safety by mitigating the risks associated with ovarian hyperstimulation syndrome (OHSS) and multiple pregnancies. There has been an increasing debate in recent years regarding the advisability of making elective frozen embryo transfer the standard practice. Our study aims to determine the optimal choice between fresh and frozen embryo transfer, as well as whether the transfer should occur at the cleavage or blastocyst stage. Method: In this retrospective cohort study conducted in Taiwan, data from the national assisted reproductive technology (ART) database spanning from January 1st, 2013, to December 31st, 2017, were analyzed. The study included 51,762 eligible female participants who underwent ART and embryo transfer. Pregnancy outcomes, maternal complications, and singleton neonatal outcomes were evaluated using the National Health Insurance Database from January 1st, 2013, to December 31st, 2018. Cases were categorized into groups based on whether they underwent fresh or frozen embryo transfers, with further subdivision into cleavage stage and blastocyst stage transfers. Exposure variables encompassed clinical pregnancy rate, live birth rate, OHSS, pregnancy-induced hypertension, gestational diabetes mellitus (DM), placenta previa, placental abruption, preterm premature rupture of membranes (PPROM), gestational age, newborn body weight, and route of delivery. Results: Frozen blastocyst transfers showed higher rates of clinical pregnancy (CPR) and live births (LBR) compared to fresh blastocyst transfers. Conversely, frozen cleavage stage transfers demonstrated lower rates of clinical pregnancy and live birth compared to fresh cleavage stage transfers. Frozen embryo transfers were associated with reduced risks of OHSS but were linked to a higher risk of pregnancy-induced hypertension compared to fresh embryo transfers. Additionally, frozen embryo transfers were associated with a higher incidence of large for gestational age infants and a lower incidence of small for gestational age infants. Conclusion: The freeze-all strategy may not be suitable for universal application. When embryos can develop to the blastocyst stage, FET is a favorable choice, but embryos can only develop to the cleavage stage, fresh embryo transfer becomes a more reasonable option.

Outcomes of Frozen-Thawed Embryo Transfer in Overweight and Obese Infertile Women

Abstract:Background: This study aimed to retrospectively analyze the effects of different endometrial preparation protocols on live birth rate (LBR) of overweight/obese patients in frozen-thawed embryo transfer (FET) cycles. Methods and Results: The study population comprised women who received IVF treatment between June 2013 and December 2020 in the Reproductive Medical Centre. We collected 529 patients with BMI >= 24 kg/m(2) according to the inclusion criteria. The patients were divided into four groups according to the different endometrial preparation protocols in the freeze-thaw cycle: modified natural cycle group (135 cases), stimulated cycle group (124 cases), artificial cycle group (85 cases), and artificial cycle with down-regulation group (185 cases). There was no statistical difference in LBR, clinical pregnancy rate, miscarriage rate, or ectopic pregnancy rate among different endometrial preparation protocols. Multivariate logistic regression analysis showed that only the duration of infertility was related to the LBR (P=0.048). Conclusion: For overweight and obese people who want to receive FET, different endometrial preparation protocols may not affect the LBR.

Abstract:Abstract This study was to retrospectively analyze the effect of the age of embryos transfer and oocyte retrieval on the clinical pregnancy outcome in patients with simple tubal factor infertility (TFI) who received frozen-thawed embryo transfer. Patients (n = 3619) with simple TFI who underwent in vitro fertilization (IVF)/ intracytoplasmic sperm injection (ICSI) frozen-thawed embryo transfer at our hospital were enrolled. Univariate logistic regression analysis, categorical multivariate logistic regression analysis, curve fitting and threshold effect analysis were performed. Age of embryo transfer was a significant ( P < 0.05) independent risk factor affecting the clinical pregnancy, live birth, and miscarriage rates. The Clinical pregnancy outcome declined significantly after the age of 34 years. After limiting the female oocyte retrieval age to ≤ 34 years, no significant change was detected in the clinical pregnancy, live birth, or miscarriage rate with increase of transplantation age. In conclusion, in patients with simple TFI undergoing IVF/ICSI frozen-thawed embryo transfer, age is a significant independent risk factor affecting the clinical pregnancy, live birth, and miscarriage rate. Aging of oocytes has a greater impact on the clinical pregnancy in women with simple TFI than the aging of the body. Patients with TFI can freeze embryos in advance to preserve fertility.

Abstract:Abstract Study question Which factors affect the clinical pregnancy rate (CPR) after single euploid frozen embryo transfers (FET), when the blastocyst is transferred in the upper uterine cavity area? Summary answer Blastocyst quality, embryo transfer difficulty and endometrial thickness affect the CPR in FET. What is known already There is a limited understanding of the factors affecting success rates after FET. The most important factors influencing implantation rates are patient characteristics, type of endometrial preparation, embryo quality and transfer difficulty. It has been shown that the position of the euploid blastocyst, measured as distance from the fundus (DFF) of the uterine cavity (mm), affects the implantation potential. Although the ideal location within the uterine cavity is still being debated in very heterogeneous patient populations, most studies have found that the highest pregnancy rates are obtained when the embryo is placed in the upper area of the uterine cavity. Study design, size, duration This single center retrospective cohort study included a total of 603 single euploid FET cycles, in the upper half of the uterine cavity, between January 2019 and November 2020 in ART Fertility Clinic Abu Dhabi, UAE. Participants/materials, setting, methods Trophectoderm biopsy samples were subjected to Next Generation Sequencing to screen the ploidy state. Vitrification and warming were performed using the Cryotop method (Kitazato, Biopharma). The full length of the uterine cavity and the longitudinal distance between the fundal endometrial surface and the air bubble after transfer were measured. Main results and the role of chance The patients were on average 33.9 (19–46) years old. The FET was performed in a natural cycle (NC) (n = 278) or hormone replacement therapy (HRT) (n = 325). Of the 603 transfers which had been performed in the upper half of the uterus, 412 (68.3%) resulted in a pregnancy and 311 (51.5%) in a clinical pregnancy. After bivariate analysis, the clinical pregnancy rate was significantly higher for high quality blastocysts (grade 1–2 versus 3–4) (p < 0.001), after easy embryo transfers (p = 0.001) and for higher endometrial thickness (p = 0.027). After performing a multivariate logistic regression analysis to consider the effect of all explanatory variables (age, Anti Müllerian hormone, body mass index, endometrial thickness, quality of the blastocyst, difficulty of the transfer [requirement of additional instrumentation], presence of mucus or blood on the transfer catheter, day 5 or day 6 biopsy, FET endometrial preparation), the clinical pregnancy was affected by the endometrial thickness: OR 1.20 [1.05–1.37], p = 0.007; transfer difficulty: OR 0.44 [0.25–0.79], p = 0.006; blastocyst quality 3: OR 0.38 [0.18–0.79], p = 0.01 and blastocyst quality 4: OR 0.15 [0.06–0.37], p < 0.0001. Age did not affect the clinical pregnancy after transferring a single euploid blastocyst: OR 1.03 [1.00–1.06], p = 0.052. Limitations, reasons for caution The limitation of this study was its retrospective nature and the small sample size. Other parameters may be important in live birth outcomes. Wider implications of the findings: Optimization of clinical pregnancy outcomes after FET depends on multiple factors. Even after transfer of euploid blastocysts in the upper uterine cavity, the endometrial thickness, transfer difficulty and blastocyst quality will still affect the clinical pregnancy outcomes. Abstract:Abstract Background Owing to the crucial role the endometrium plays in embryo implantation, the four main endometrial preparation protocols have become important factors in the study of pregnancy outcomes in the FET cycles. Previous studies have shown that the best of these four protocols remains controversial for women undergoing FET. Methods A total of 10333 FET cycles from January 2018 to December 2018 were analyzed in this study. They were categorized into four groups according to endometrial preparation regimen: natural cycles (Group NC,n = 815), hormone replacement therapy cycles (Group HRT ,n = 6434), GnRH agonist artificial cycles (Group GAC,n = 1392) and ovarian stimulation cycles (Group OC, n = 1692). All patients were followed up for at least 1 year. Pregnancy outcomes were compared between the four groups and multiple logistic regression models were used to adjust for the effects of confounding factors. Results The ectopic pregnancy rate (P = 0.627) and miscarriage rate (P = 0.164) were not statistically significant between the four groups. Moreover, biochemical pregnancy rate, clinical pregnancy rate and live birth rate in the NC group were not statistically significant compared to the other three groups. After adjusting for covariates, multiple logistic regression analysis showed no statistical significance in pregnancy outcomes in the HRT, GAC and OC groups compared to the NC group.And the adjusted OR for live births was 0.988 (95 % CI0.847-1.152) for the HRT group, 0.955 (95 % CI0.795-1.146) for the GAC group,0.898 (95 % CI0.754-1.070) for the OC group. Conclusions Our study showed that natural cycles have similar pregnancy outcomes in terms of clinical pregnancy and live birth to the other three endometrial preparation options. As it has other advantages, the natural cycle protocol can therefore be the recommended option for endometrial preparation in the FET population.
