Abstract:Collagen fibrils are the most important structural component of tendons. Their crimped structure and parallel arrangement within the tendon lead to a distinctive non-linear stress-strain curve when a tendon is stretched. Microstructural models can be used to relate microscale collagen fibril mechanics to macroscale tendon mechanics, allowing us to identify the mechanisms behind each feature present in the stress-strain curve. Most models in the literature focus on the elastic behaviour of the tendon, and there are few which model beyond the elastic limit without introducing phenomenological parameters. We develop a model, built upon a collagen recruitment approach, that only contains microstructural parameters. We split the stress in the fibrils into elastic and plastic parts, and assume that the fibril yield stretch and rupture stretch are each described by a distribution function, rather than being single-valued. By changing the shapes of the distributions and their regions of overlap, we can produce macroscale tendon stress-strain curves that generate the full range of features observed experimentally, including those that could not be explained using existing models. These features include second linear regions occurring after the tendon has yielded, and step-like failure behaviour present after the stress has peaked. When we compare with an existing model, we find that our model reduces the average root mean squared error from 4.15MPa to 1.61MPa, and the resulting parameter values are closer to those found experimentally. Since our model contains only parameters that have a direct physical interpretation, it can be used to predict how processes such as ageing, disease, and injury affect the mechanical behaviour of tendons, provided we can quantify the effects of these processes on the microstructure.

Structural Changes of Active Skeletal Muscles: Modelling, Validation and Numerical Experiments

Abstract:The purpose of this study was to report numerical validation of a 3D finite element model of contracting muscle. The model was based on continuum theory for fibre-reinforced composite materials. Here we simulated contractions for an idealized medial gastrocnemius muscle in man, using the model. Simulations were performed to test the force-length relation of the whole muscle, to evaluate the changes in internal fascicle geometry during contractions, and to assess the importance of material formulations for the aponeurosis and tendon. The simulation results were compared to previously published experimental values. The force-length profile for the whole muscle showed a realistic profile. As the muscle contracted the fascicles curved into S-shaped trajectories and curled around 3D paths, both of which matched previous experimental findings. As the fascicles shortened they increased in their cross-sectional area, but this increase was asymmetric with the smaller increase occurring within the fascicle-plane: the Poisson's ratio in this plane matched that previously shown from ultrasound imaging. The distribution of strains in the aponeurosis and tendon was shown to be a function of their material properties. This study demonstrated that the model could replicate realistic patterns of whole muscle-force, and changes to the internal muscle geometry, and so will be useful for testing mechanisms that affect the structural changes within contracting muscle.

Abstract:Achilles tendon (AT) rupture leads to long-term structural and functional impairments. Currently, the predictors of good recovery after rupture are poorly known. Thus, we aimed to explore the interconnections between structural, mechanical, and neuromuscular parameters and their associations with factors that could explain good recovery in patients with non-surgically treated AT rupture. A total of 35 patients with unilateral rupture (6 females) participated in this study. Muscle-tendon structural, mechanical, and neuromuscular parameters were measured 1-year after rupture. Interconnections between the inter-limb differences (Δ) were explored using partial correlations, followed by multivariable linear regression to find associations between the measured factors and the following markers that indicate good recovery: 1) tendon length, 2) tendon non-uniform displacement, and 3) flexor hallucis longus (FHL) normalized EMG amplitude difference between limbs. Δmedial gastrocnemius (MG) (β = -0.12, p = 0.007) and Δlateral gastrocnemius (β = -0.086, p = 0.030) subtendon lengths were associated with MG tendon Δstiffness. MG (β = 11.56, p = 0.003) and soleus (β = 2.18, p = 0.040) Δsubtendon lengths explained 48 % of variance in FHL EMG amplitude. Regression models for tendon length and non-uniform displacement were not significant. Smaller inter-limb differences in Achilles subtendon lengths were associated with smaller differences in the AT stiffness between limbs, and a smaller contribution of FHL muscle to the plantarflexion torque. In the injured limb, the increased contribution of FHL appears to partially counteract a smaller contribution from MG due to the elongated tendon, however the role of FHL should not be emphasized during rehabilitation to allow recovery of the TS muscles.

Abstract:This study aimed to establish a finite element model that vividly reflected the anterior cruciate ligament (ACL) geometry and investigated the ACL stress distribution under different loading conditions. The ACL's three-dimensional finite element model was based on a human cadaveric knee. Simulations of three loading conditions (134 N anterior tibial load, 5 Nm external tibial torque, 5 Nm internal tibial torque) on the knee model were performed. Experiments were performed on a knee specimen using a robotic universal force/moment sensor testing system to validate the model. The simulation results of the established model were in good agreement with the experimental results. Under the anterior tibial load, the highest maximal principal stresses (14.884 MPa) were localized at the femoral insertion of the ACL. Under the external and internal tibial torque, the highest maximal principal stresses (0.815 MPa and 0.933 MPa, respectively) were mainly concentrated in the mid-substance of the ACL and near the tibial insertion site, respectively. Combining the location of maximum stress and the location of common clinical ACL rupture, the most dangerous load during ACL injury may be the anterior tibial load. ACL injuries were more frequently loaded by external tibial than internal tibial torque.
Murine Hind Limb Explant Model for Studying the Mechanobiology of Achilles Tendon Impingement

Abstract:Tendon impingement upon bone generates a multiaxial mechanical strain environment with markedly elevated transverse compressive strain, which elicits a localized fibrocartilage phenotype characterized by accumulation of glycosaminoglycan (GAG)-rich matrix and remodeling of the collagen network. While fibrocartilage is a normal feature in impinged regions of healthy tendons, excess GAG deposition and disorganization of the collagen network are hallmark features of tendinopathy. Accordingly, impingement is clinically recognized as an important extrinsic factor in the initiation and progression of tendinopathy. Nevertheless, the mechanobiology underlying tendon impingement remains understudied. Prior efforts to elucidate the cellular response to tendon impingement have applied uniaxial compression to cells and excised tendon explants in vitro. However, isolated cells lack a three-dimensional extracellular environment crucial to mechanoresponse, and both in vitro and excised explant studies fail to recapitulate the multiaxial strain environment generated by tendon impingement in vivo, which depends on anatomical features of the impinged region. Moreover, in vivo models of tendon impingement lack control over the mechanical strain environment. To overcome these limitations, we present a novel murine hind limb explant model suitable for studying the mechanobiology of Achilles tendon impingement. This model maintains the Achilles tendon in situ to preserve local anatomy and reproduces the multiaxial strain environment generated by impingement of the Achilles tendon insertion upon the calcaneus during passively applied ankle dorsiflexion while retaining cells within their native environment. We describe a tissue culture protocol integral to this model and present data establishing sustained explant viability over 7 days. The representative results demonstrate enhanced histological GAG staining and decreased collagen fiber alignment secondary to impingement, suggesting elevated fibrocartilage formation. This model can easily be adapted to investigate different mechanical loading regimens and allows for the manipulation of molecular pathways of interest to identify mechanisms mediating phenotypic change in the Achilles tendon in response to impingement.

Abstract:Three experiments were performed to study the effects of bilateral olfactory bulbectomy upon active and passive avoidance conditioning. In the first experiment, olfactory bulbectomized rats were found not to be impaired on a one-way active avoidance task but showed a fear retention deficit. In Experiment 2, sham-operated rats demonstrated a degree of fear retention that was a positive linear function of the number of shock presentations; olfactory bulbectomized rats showed a total absence of fear retention. In the third experiment olfactory bulbectomized rats failed to demonstrate the inverse relationship between shock intensity and two-way avoidance performance normally evidenced. The pattern of data obtained suggests that while bulbectomized rats do not show any general avoidance impairment they do seem to be unaffected by the suppressive effects of shock upon ongoing behavior.
A Pilot Study of Musculoskeletal Abnormalities in Patients in Recovery from a Unilateral Rupture-Repaired Achilles Tendon

Abstract:The purpose of this study was to compare the inter-limb joint kinematics, joint moments, muscle forces, and joint reaction forces in patients after an Achilles tendon rupture (ATR) via subject-specific musculoskeletal modeling. Six patients recovering from a surgically repaired unilateral ATR were included in this study. The bilateral Achilles tendon (AT) lengths were evaluated using ultrasound imaging. The three-dimensional marker trajectories, ground reaction forces, and surface electromyography (sEMG) were collected on both sides during self-selected speed during walking, jogging and running. Subject-specific musculoskeletal models were developed to compute joint kinematics, joint moments, muscle forces and joint reaction forces. AT lengths were significantly longer in the involved side. The side-to-side triceps surae muscle strength deficits were combined with decreased plantarflexion angles and moments in the injured leg during walking, jogging and running. However, the increased knee extensor femur muscle forces were associated with greater knee extension degrees and moments in the involved limb during all tasks. Greater knee joint moments and joint reaction forces versus decreased ankle joint moments and joint reaction forces in the involved side indicate elevated knee joint loads compared with reduced ankle joint loads that are present during normal activities after an ATR. In the frontal plane, increased subtalar eversion angles and eversion moments in the involved side were demonstrated only during jogging and running, which were regarded as an indicator for greater medial knee joint loading. It seems after an ATR, the elongated AT accompanied by decreased plantarflexion degrees and calf muscle strength deficits indicates ankle joint function impairment in the injured leg. In addition, increased knee extensor muscle strength and knee joint loads may be a possible compensatory mechanism for decreased ankle function. These data suggest patients after an ATR may suffer from increased knee overuse injury risk.

Abstract:Objective To develop a three dimension finite element model of the human knee joint using MRI in order to evaluate the change in stress at the patellar tendon proximal medial/lateral insertion under the different loading on the vastus medialis oblique/vastus lateralis. Methods A volunteer accepted MRI scan with his knee 60° flexion. MRI was used to reconstruct a three dimension model by Mimics 8.1. After optimized by Magic RP, the tetrahedron model was meshed by MSC.PATRAN. Patellar tendon was simulated, and material attribute was selected. Based on the model, the Von Mises stress of the patellar tendon proximal medial/lateral insertion was calculated under the loading of 0N, 33N, 66N and 100N on the vastus medialis oblique. Results When the loading on vastus medialis oblique increased from 0N to 100N, the stress of medial patellar tendon decreased, and the stress of medial patellar tendon increased. The maximal stress of the patellar tendon proximal medial insertion decreased from 2.41MPa to 1.41MPa, and the maximal stress of the patellar tendon proximal lateral insertion increased from 2.90MPa to 5.03MPa. Conclusion With the knee 60° flexion, when the loading on vastus medialis oblique increased, the maximal stress of the patellar tendon proximal lateral insertion increased, and that of the patellar tendon medial proximal insertion decreased.
Injuries in Muscle-Tendon-Bone Units: A Systematic Review Considering the Role of Passive Tissue Fatigue

Abstract:Background: Low-cycle fatigue damage accumulating to the point of structural failure has been recently reported at the origin of the human anterior cruciate ligament under strenuous repetitive loading. If this can occur in a ligament, low-cycle fatigue damage may also occur in the connective tissue of muscle-tendon units. To this end, we reviewed what is known about how, when, and where injuries of muscle-tendon units occur throughout the body. Purpose: To systematically review injuries in the muscle-tendon-bone complex; assess the site of injury (muscle belly, musculotendinous junction [MTJ], tendon/aponeurosis, tendon/aponeurosis–bone junction, and tendon/aponeurosis avulsion), incidence, muscles and tendons involved, mechanism of injury, and main symptoms; and consider the hypothesis that injury may often be consistent with the accumulation of multiscale material fatigue damage during repetitive submaximal loading regimens. Methods: PubMed, Web of Science, Scopus, and ProQuest were searched on July 24, 2019. Quality assessment was undertaken using ARRIVE, STROBE, and CARE (Animal Research: Reporting In Vivo Experiments, Strengthening the Reporting of Observational Studies in Epidemiology, and the Case Report Statement and Checklist, respectively). Results: Overall, 131 studies met the inclusion criteria, including 799 specimens and 2,823 patients who sustained 3,246 injuries. Laboratory studies showed a preponderance of failures at the MTJ, a viscoelastic behavior of muscle-tendon units, and damage accumulation at the MTJ with repetitive loading. Observational studies showed that 35% of injuries occurred in the tendon midsubstance; 28%, at the MTJ; 18%, at the tendon-bone junction; 13%, within the muscle belly and that 6% were tendon avulsions including a bone fragment. The biceps femoris was the most injured muscle (25%), followed by the supraspinatus (12%) and the Achilles tendon (9%). The most common symptoms were hematoma and/or swelling, tenderness, edema and muscle/tendon retraction. The onset of injury was consistent with tissue fatigue at all injury sites except for tendon avulsions, where 63% of the injuries were caused by an evident trauma. Conclusion: Excluding traumatic tendon avulsions, most injuries were consistent with the hypothesis that material fatigue damage accumulated during repetitive submaximal loading regimens. If supported by data from better imaging modalities, this has implications for improving injury detection, prevention, and training regimens.

Novel mechanical impact simulator designed to generate clinically relevant anterior cruciate ligament ruptures

Abstract:Background: Over 250,000 anterior cruciate ligament ruptures occur each year; therefore, it is important to understand the underlying mechanisms of these injuries. The objective of the current investigation was to develop and analyze an impact test device that consistently produces anterior cruciate ligament failure in a clinically relevant manner. Method: A mechanical impact simulator was developed to simulate the ground reaction force impulse generated from landing in a physiologic and clinically relevant manner. External knee abduction moment, anterior shear, and internal tibial rotation loads were applied to the specimen via pneumatic actuators. The magnitudes of applied loads were determined in vivo from a cohort of healthy athletes. Loads were systematically increased until specimen failure was induced. Three cadaveric lower extremity specimens were tested and clinically assessed for failure. Knee specimens were physically and arthroscopically examined at baseline and at post-injury by a board certified orthopedic surgeon. Findings: All three specimens experienced failure at either the midsubstance or the femoral insertion site. The mean peak strain prior to failure was 18.8 (6.2)%, while the mean peak medial collateral ligament strain was 7.9 (5.9)%. Interpretation: A board certified orthopedic surgeon confirmed observed rupture patterns were representative of clinical cases. Peak strains were consistent with literature. The novel mechanical impact simulator will allow researchers to assess clinically relevant patterns of rupture and the data generated will inform clinician decisions. This novel machine presents the ability to assess healthy specimens as well as differences in the function of deficient and reconstructed knees.
Quantitative comparison of three rat models of Achilles tendon injury: A multidisciplinary approach

Abstract:The Achilles tendon, while the strongest and largest tendon in the body, is frequently injured. Inconclusive evidence exists regarding treatment strategies for both complete tears and partial tears. Well-characterized animal models of tendon injury are important for understanding physiological processes of tendon repair and testing potential therapeutics. Utilizing three distinct models of rat Achilles tendon injury, the objective of this study was to define and compare the effects and relative impact on tendon properties and ankle function of both tear severity (complete tear versus partial tear, both with post-operative immobilization) and immobilization after partial tear (partial tear with versus without immobilization). We hypothesized that a complete tear would cause inferior post-injury properties compared to a partial tear, and that immediate loading after partial tear would improve post-injury properties compared to immobilization. All models were reproducible and had distinct effects on measured parameters. Injury severity drastically influenced tendon healing, with complete tear causing decreased ankle mobility and tendon mechanics compared to partial tears. One week of plantarflexion immobilization had a strong effect on animals receiving a partial tear. Tendons with partial tears and immobilization failed early during fatigue cycling three weeks post-injury. Partial tear without immobilization had no effect on ankle range of motion through dorsiflexion at any time point compared to the pre-surgery value, while partial tear with immobilization demonstrated diminished function at all post-injury time points. All three models of Achilles injury could be useful for tendon healing investigations, chosen based on the prospective applications of a potential therapeutic.
Early response to tendon fatigue damage accumulation in a novel in vivo model

Abstract:This study describes the development and application of a novel rat patellar tendon model of mechanical fatigue for investigating the early in vivo response to tendon subfailure injury. Patellar tendons of adult female Sprague-Dawley rats were fatigue loaded between 1-35N using a custom-designed loading apparatus. Patellar tendons were subjected to Low-, Moderate- or High-level fatigue damage, defined by grip-to-grip strain measurement. Molecular response was compared with that of a laceration-repair injury. Histological analyses showed that progression of tendon fatigue involves formation of localized kinked fiber deformations at Low damage, which increased in density with presence of fiber delaminations at Moderate damage, and fiber angulation and discontinuities at High damage levels. RT-PCR analysis performed at 1- and 3-day post-fatigue showed variable changes in type I, III and V collagen mRNA expression at Low and Moderate damage levels, consistent with clinical findings of tendon pathology and were modest compared with those observed at High damage levels, in which expression of all collagens evaluated were increased markedly. In contrast, only type I collagen expression was elevated at the same time points post-laceration. Findings suggest that cumulative fatigue in tendon invokes a different molecular response than laceration. Further, structural repair may not be initiated until reaching end-stage fatigue life, where the repair response may unable to restore the damaged tendon to its pre-fatigue architecture.
Development of a continuum damage model to predict accumulation of sub-failure damage in tendons

Abstract:Many painful and physically debilitating conditions involve sub-failure mechanical damage to seemingly intact connective tissues such as tendons and ligaments. We found that the amount of denatured collagen in rat tail tendon (RTT) fascicles increased over experiments of cyclic loading to a constant load level (creep cyclic fatigue) with fluorescently tagged collagen hybridizing peptides (CHPs) that bind to denatured collagen. To better understand tendon sub-failure damage progression, computational modeling of tendon materials via finite element analysis in FEBio has been conducted. The objective of this project was to develop, implement, and test the ability of a new continuum damage mechanics (CDM) model in FEBio to represent the sub-failure damage behavior seen in our RTT fascicle creep cyclic fatigue experimental data. There appeared to be two distinct mechanisms responsible for the creep cyclic fatigue softening behavior of RTT fascicles over the number of cycles to failure: the preconditioning effect and overall collagen damage. In our finite element (FE) models, the RTT fascicle undamaged elastic constitutive material was composed of a matrix and fibers described by the Coupled Veronda-Westmann and exponential-linear materials. This undamaged elastic material was convolved with a modified CDM model adapted from Balzani et al., in 2012. The novelty of the Balzani damage model is the inclusion of two interrelated mechanisms described as continuous and discontinuous damage. The continuous damage formulation calculates damage accumulation during the loading and reloading of each new cycle, while the discontinuous damage approach accumulates damage from the maximum strain over the loading history to the current time. We modified the Balzani damage model formulations to represent exponential and sigmoidal increases in damage marked by the preconditioning effect and collagen damage in RTT fascicles as functions of continuous and discontinuous damage. The original Balzani damage model was first verified, then the modified CDM model was implemented into FEBio and used to reproduce the sample specific experimental creep cyclic fatigue stress-strain data as well as predict incremental cyclic fatigue. The resulting model will be useful for future experimental and computational studies of damage mechanics to understand tendon pathologies.

Abstract:The knee meniscus plays a key role in shock absorption, joint stability, and congruity at the knee joint. However, due to several reasons, meniscal tears are increasing that in turn causes osteoarthritis. The objective of the current study was to evaluate the stresses on the meniscus during kinematic activities and correlate them with the potential tears observed clinically. From the MRI data of a healthy volunteer, 3D models of the menisci were segmented in Mimics®, post-processed in 3-matics®, meshed in HyperMesh® and were analyzed in Abaqus® for stresses arising due to compressive loads from both femoral and tibial ends; under various angles of flexion. The critical regions with higher stress distribution were correlated with the potential meniscal tears. It was observed that the stresses were higher when loads were applied from the tibial end than femoral end. Though the loads were applied proportional to the area, higher stresses were observed in the lateral meniscus. Results of the study show that at higher knee flexion angles, loading from tibial end leads to longitudinal tear in region E1–E2 (as per Cooper's Classification) on the lateral meniscus and horizontal tear in A-1 region of the medial meniscus. Longitudinal, horizontal, and root detachment tears reported in the literature, coincide with our predictions. The novelty of the study is the estimation of stresses due to forces from the tibial end and the correlation of the stresses to the type of meniscal tears observed clinically.

Abstract:Tendons of the body differ dramatically in their function, mechanics and range of motion, but all connect to bone via an enthesis. Effective force transfer at the enthesis enables joint stability and mobility, with strength and stiffness arising from a fibrous architecture. However, how enthesis toughness arises across tendons with diverse loading orientations remains unclear. To study this, we performed simultaneous imaging of the bone and tendon in entheses that represent the range of tendon-to-bone insertions and extended a mathematical model to account for variations in insertion and bone geometry. We tested the hypothesis that toughness, across a range of tendon entheses, could be explained by differences observed in interactions between fibre architecture and bone architecture. In the model, toughness arose from fibre reorientation, recruitment and rupture, mediated by interactions between fibres at the enthesis and the bony ridge abutting it. When applied to tendons sometimes characterized as either energy-storing or positional, the model predicted that entheses of the former prioritize toughness over strength, while those of the latter prioritize consistent stiffness across loading directions. Results provide insight into techniques for surgical repair of tendon-to-bone attachments, and more broadly into mechanisms for the attachment of highly dissimilar materials.

Reconstruction of the human gastrocnemius force-length curve in vivo: part 1-model-based validation of method

Abstract:The muscle fiber force-length relationship has been explained in terms of the cross-bridge theory at the sarcomere level. In vivo, for a physiologically realistic range of joint motion, and therefore range of muscle fiber lengths, only part of the force-length curve may be used; that is, the section of the force-length curve expressed can vary. The purpose of this study was to assess the accuracy of a method for determining the expressed section of the force-length curve for biarticular muscles. A muscle model was used to simulate the triceps surae muscle group. Three model formulations were used so that the gastrocnemius operated over different portions of the force-length curve: the ascending limb, the plateau region, and the descending limb. Joint moment data were generated for a range of joint configurations and from this simulated data the region of the force- length relationship that the gastrocnemius muscle operated over was successfully reconstructed using the algorithm of Herzog and ter Keurs (1988a). Further simulations showed that the correct region of the force-length curve was accurately reconstructed even in the presence of random and systematic noise generated to reflect the effects of sampling errors, and incomplete muscle activation.

Abstract:Digital human body models are used to simulate injuries that occur as a result of vehicle collisions, vibration, sports, and falls. Given enough time the body's musculature can generate force, affect the body's movements, and change the risk of some injuries. The finite-element code LS-DYNA is often used to simulate the movements and injuries sustained by the digital human body models as a result of an accident. In this work, we evaluate the accuracy of the three muscle models in LS-DYNA (MAT_156, EHTMM, and the VEXAT) when simulating a range of experiments performed on isolated muscle: force-length-velocity experiments on maximally and sub-maximally stimulated muscle, active-lengthening experiments, and vibration experiments. The force-length-velocity experiments are included because these conditions are typical of the muscle activity that precedes an accident, while the active-lengthening and vibration experiments mimic conditions that can cause injury. The three models perform similarly during the maximally and sub-maximally activated force-length-velocity experiments, but noticeably differ in response to the active-lengthening and vibration experiments. The VEXAT model is able to generate the enhanced forces of biological muscle during active lengthening, while both the MAT_156 and EHTMM produce too little force. In response to vibration, the stiffness and damping of the VEXAT model closely follows the experimental data while the MAT_156 and EHTMM models differ substantially. The accuracy of the VEXAT model comes from two additional mechanical structures that are missing in the MAT_156 and EHTMM models: viscoelastic cross-bridges, and an active titin filament. To help others build on our work we have made our simulation code publicly available.

Abstract:The supraspinatus tendon plays a crucial role in shoulder abduction, making it one of the common structures affected by injury. Clinically, crescent-shaped tears are the most commonly seen tear shape. By developing six specimen-specific, three-dimensional, supraspinatus-infraspinatus finite element model with heterogeneous material properties, this study aimed to examine the changes in tissue deformation (maximum principal strain) of the supraspinatus tendon due to specimen-specific material properties and rotator cuff tear size. FE models with small- and medium-sized full-thickness crescent-shaped tears were subjected to loads seen during activities of daily living and physiotherapy. Six fresh-frozen cadaveric shoulders were dissected to mechanically test the supraspinatus tendon and develop and validate FE models that can be used to assess changes in strain due to small (< 1 cm, equivalent to 20-30% of the tendon width) and medium-sized (1–3 cm, equivalent to 40–50% of the tendon width) tears that are located in the middle and posterior regions of the supraspinatus tendon. FE predictions of maximum principal strain at the tear tips were examined to determine whether failure strain was reached during activities of daily living (drinking and brushing teeth) and physiotherapy exercises (prone abduction and external rotation at 90° abduction). No significant differences were observed between the middle and posterior tear failure loads for small- and medium-sized tears. For prone abduction, there was a potential risk for tear progression (exceeded failure strain) for medium-sized tears in the supraspinatus tendon's middle and posterior regions. For external rotation at 90° abduction, one model with a middle tear and two with posterior tears experienced failure. For all daily activity loads, the strain never exceeded the failure strain. Our three-dimensional supraspinatus-infraspinatus FE model shows that small tears appear unlikely to progress based on the regional strain response; however, medium-sized tears are at higher risk during more strenuous physiotherapy exercises. Furthermore, differences in patient-specific tendon material properties are important in determining whether the tear will progress. Therefore, patient-specific management plans based on tear size may be beneficial to improve clinical outcomes.
