Abstract:Systemic hypertension has been considered mainly as an adult health issue for a long time, but it is now being increasingly acknowledged as a significant problem also among pediatric patients. The frequency of pediatric hypertension has grown mostly because of increases in childhood obesity and sedentary lifestyles, but secondary forms of hypertension play a role as well. Considering that unaddressed hypertension during childhood can result in enduring cardiovascular complications, timely identification and intervention are essential. Strategies for addressing this disease encompass not only lifestyle adjustments, but also the use of medications when needed. Lifestyle modifications entail encouraging a nutritious diet, consistent physical activity, and the maintenance of a healthy weight. Moreover, educating both children and their caregivers about monitoring blood pressure at home can aid in long-term management. Thus, the aim of this review is to discuss the etiologies, classification, and principles of the treatment of hypertension in pediatric patients.

Loss classification in optical burst switching networks using machine learning techniques: improving the performance of TCP

Abstract:Optical burst switching (OBS) is considered as a contending technology for the core of the Internet in future. However, due to lack of the buffers, losses occur due to contention among simultaneously arriving bursts at the core nodes. Contention losses do not necessarily indicate a situation of congestion in the network. Thus differentiation (classification) of losses is essential in many applications to avoid false identification of congestion. In this paper, we propose a loss classification technique for the OBS networks based on machine learning techniques. We devise a new measure to differentiate between congestion and contention losses, which is derived from the observed losses, called the number of bursts between failures (NBBF). We observe that the NBBF follows a Gaussian distribution with different parameters for contention and congestion losses. This feature is used in differentiation. We use both a supervised learning technique (hidden Markov model (HMM)) and an unsupervised learning technique (expectation maximization (EM) clustering) on the observed losses and classify them into a set of states (clusters) after which an algorithm differentiates between the congestion and contention losses. We also demonstrate the use of loss differentiation in improving the performance of transport control protocol (TCP) over OBS networks. We modify congestion control mechanism of TCP suitably to arrive at two variants of TCP, HMM-TCP and EM-TCP. Their performance is compared with TCP NewReno, TCP SACK, and Burst TCP (X. Yu et al., Mar. 2004). Simulation results demonstrate the effectiveness and accuracy of the loss classification technique in different network scenarios.

Abstract:Hypertension is a real problem in children. It shows a tracking behaviour, representing a key risk factor for hypertension, cardiovascular disease, and end-organ failure in adulthood. However, the importance of addressing arterial hypertension in children is not limited to its risk of tracking into adulthood. Thus, early detection and management are crucial. Hypertension may be primary or due to secondary causes, and identification of this distinction is very important for the treatment setting. Importantly, the management of hypertension in children is crucial to prevent the well-known cardiovascular effects in adulthood. As demonstrated in the literature, healthy eating habits, together with regular physical activity, can have a major impact on reducing high blood pressure and preventing organ damage in children and adolescents. However, suppose these are not sufficient to treat hypertension. In that case, if patients are symptomatic and/or have additional metabolic conditions such as obesity, type diabetes mellitus, or chronic kidney disease, anti-hypertensive medication must be started. Recent guidelines have provided clear guidance on the treatment of hypertension and hypertensive crisis in pediatric age. On the other hand, there are currently few specific recommendations on the treatment of isolated nocturnal hypertension and treatment-resistant hypertension. This review aims to summarize the most recent recommendations for the treatment of hypertension and hypertensive crisis in children and the last years' knowledge and experience in treating childhood isolated nocturnal hypertension and resistant hypertension of childhood.

Abstract:Hypertension, Ahead of Print. The overall prevalence of hypertension in childhood is 2% to 5%, and the leading type of childhood hypertension is primary hypertension, especially in adolescence. As in adults, the leading risk factors for children with primary hypertension are excess adiposity and suboptimal lifestyles; however, environmental stress, low birth weight, and genetic factors may also be important. Hypertensive children are highly likely to become hypertensive adults and to have measurable target organ injury, particularly left ventricular hypertrophy and vascular stiffening. Ambulatory and home blood pressure monitoring may facilitate diagnosis. Primordial prevention of hypertension through public health implementation of healthier diet and increased physical activity will reduce the prevalence of primary hypertension, and evidence-based treatment guidelines should be implemented when hypertension is diagnosed. Further research to optimize recognition and diagnosis and clinical trials to better define outcomes of treatment are needed.

The effect of retirement on alcohol consumption: results from the US Health and Retirement Study.

Abstract:BACKGROUND Prior research examining the association between retirement and alcohol consumption is inconsistent with respect to salience, direction and magnitude. Reasonable conceptual arguments for both positive (e.g. coping, introduction of leisure time) and negative (e.g. severance of work-related social relationships) changes further complicate investigations of this critical association, as do differences in study design, national setting and measurement of alcohol use. METHODS This prospective longitudinal study analyses 2-year wave-pairs drawn from seven waves (14 years) of data from the US Health and Retirement Study to assess the effect of complete retirement on weekly alcohol consumption (n = 9979 observations; 4674 unique participants). We use multiple regression analysis in a two-period follow-up design and account for potential selection bias and reverse causality not addressed in prior research on this topic. RESULTS We find that retirement is positively associated with subsequent weekly alcohol consumption for men who reported drinking at both follow-up and the prior study wave (β = 1.9, 95% confidence interval = 0.43-3.36). No association was observed among women. CONCLUSION Our results suggest that health care professionals should monitor the drinking habits of retired men, as older individuals are particularly susceptible to the adverse effects of heavy alcohol use.

Abstract:Purpose of ReviewPediatric hypertension is increasing in incidence with concomitant increases in childhood obesity. Target organ damage from primary or secondary pediatric hypertension is a growing concern, as the pathogenesis of vascular disease, typically observed later in life, may begin at a young age. While hypertensive complications are described extensively in adults, much less is known about end organ damage from elevated blood pressure in children and adolescents.Recent FindingsThis review highlights the recent advances describing the target organ damage in the macro- and microvasculature resulting from hypertension. Persistently elevated blood pressure in childhood is associated with vascular changes affecting the heart, brain, kidneys, and retina. Emerging research shows that elevated blood pressure in children has effect on neurocognitive function in specific domains such as executive functioning, attention, and memory. Microalbuminuria is an early marker of kidney disease and a sign of glomerular injury and endothelial dysfunction that increases the risk of kidney damage and is associated with future kidney and cardiovascular events. Screening with ambulatory blood pressure monitoring to improve blood pressure control and echocardiogram may aid in early prevention of end organ damage.SummaryIn this review, we summarize and critically evaluate recent findings, describe ongoing studies, and address future research questions. Implications for diagnosis, prognosis, and need for better control of hypertension are also discussed.

Abstract:1. By the use of Signal Detection Theory techniques resistive load detection sensitivity was estimated in ten normal subjects during loading of the whole of inspiration and compared with loading restricted to the initial, accelerating phase of inspiratory air flow. 2. Detection sensitivity during the accelerating-flow phase of inspiration was not significantly different from that in the control conditions. 3. These results suggest that the information for detection is generated in the accelerating phase of inspiratory air flow. 4. We propose that the stimulus for resistive load detection is the ratio of the rate of change of pressure to the rate of change of flow.

Abstract:High blood pressure (BP) still remains one of the most relevant cardiovascular risk factors, also due to its persistently high prevalence and growing incidence in the general adult and elderly population. Since almost all hypertension-related cardiovascular complications, mostly including coronary artery disease, myocardial infarction, ischemic stroke, and congestive heart failure, occurred in adult and elderly individuals, evidence on both prevalence and clinical management of hypertension in young individuals are lacking. Therefore, the clinical impact of high BP levels in young populations remains to be explored. In the recent years, the attitude of the scientific community has changed and more attention was devoted to young individuals with hypertension, also in view of the fact that early identification of these subjects may prevent developing of established hypertension in adulthood. In addition, unhealthy lifestyle habits have progressively involved children and adolescents worldwide, thus contributing to further increase the risk of developing hypertension in young individuals. On the basis of these considerations, the present review is aimed at providing a brief reappraisal of the major aspects of hypertension in the young age, as well as at promoting interest and discussion on this important issue.

Abstract:Hypertension is much less common in children than in adults. The group of experts decided to perform a review of the literature to draw up a position statement that could be used in everyday practice. The group rated recommendations using the GRADE approach. All children over the age of 3 years should have their blood pressure measured annually. Due to the lack of data on cardiovascular morbidity and mortality associated with blood pressure values, the definition of hypertension in children is a statistical value based on the normal distribution of blood pressure in the paediatric population, and children and adolescents are considered as having hypertension when their blood pressure is greater than or equal to the 95th percentile. Nevertheless, it is recommended to use normative blood pressure tables developed according to age, height and gender, to define hypertension. Measuring blood pressure in children can be technically challenging and several measurement methods are listed here. Regardless of the age of the child, it is recommended to carefully check for a secondary cause of hypertension as in 2/3 of cases it has a renal or cardiac origin. The care pathway and principles of the therapeutic strategy are described here.

We examine its etiology, progression, and long-term cardiovascular implications. The review covers definitions and recommendations of blood pressure classifications, recent developments in measurement, epidemiological trends, findings from observational and clinical studies, and prevention and treatment, while identifying gaps in understanding and suggesting future research directions.

Abstract:Background: Hypertension is one of the most prevalent diseases in the United States, affecting an estimated 3.5% of children and adolescents. It can be adversely affect most organ systems but is particularly detrimental to the heart and vascular systems. The repercussions can be gauged through well-established measures of cardiovascular function including left ventricular mass index (LVMI), left ventricular hypertrophy (LVH), carotid intima media thickness (cIMT), and aortic stiffness. Cardiovascular function is also affected by underlying etiologies of hypertension including chronic kidney disease, polycystic kidney disease, coarctation of the aorta, adrenal disorders, renal artery stenosis, obstructive sleep apnea, as well as various drugs and medications (decongestants, stimulants, Non-steroidal Anti-inflammatory Drugs (NSAIDs), and steroids). Methods: An exhaustive literature search was conducted for clinical data regarding pediatric hypertension. Sixty-seven articles were incorporated with data on 189,477 subjects total. The data was then extracted and categorized as relating to hypertension incidence, LVMI, LVH, cIMT, and/or aortic stiffness. Results: The prevalence of pediatric ( < 18 years) hypertension extracted from 47 studies from 1994 to 2018 averaged 4%. The LVMI assessed over 7 studies (n = 661) averaged 39.3 g/ m 2.7 in the hypertensive cohort and 30.1 g/ m 2.7 in the control cohort. The cIMT assessed over 7 studies (n = 580) averaged 0.55 mm in the hypertensive cohort and 0.49 mm in the control cohort. Ambulatory arterial stiffness parameters assessed over 5 studies (n = 573) in the normotensive cohort averaged 99.73 mmHg, 69.81 mmHg, 76.85 mmHg, and 46.90 mmHg, for SBP, DBP, MAP, and PP respectively. Ambulatory arterial stiffness parameters assessed over 5 studies (n = 573) in the hypertensive cohort averaged 129.56 mmHg, 73.69 mmHg, 95.08 mmHg, and 56.80 mmHg, for SBP, DBP, MAP, and PP respectively. Conclusions: The significance of pediatric hypertension is emphasized by evidence of early cardiovascular disease as demonstrated by non-invasive measures including cIMT and arterial stiffness parameters, and target organ damage and including LVH and LVMI factors. Thus, early diagnosis and treatment of high blood pressure is paramount for improving long term cardiovascular health and preventing long term morbidity and mortality.
Retrospective analysis of etiology and target organ damage in children with hypertension

Abstract:Objective To analyze the clinical data of children with primary hypertension and secondary hypertension in order to provide reference for accurate diagnosis and for early detection of target organ damage in case of primary hypertension.Methods All the children who were diagnosed with hypertension and hospitalized between January 1992 and May 2016 were enrolled in this study.Hypertension was classified as primary and secondary based on the etiology of hypertension.Data was analyzed with SPSS 17.0 software.Results (1) The average age of primary hypertension patients was 11.76 years old,and the mean BMI was 26.90 kg/m2,which were higher than those of secondary hypertension patients with statistically significant difference (P =0.000).(2) The composition ratio of secondary hypertension during infancy,preschool period,school age and adolescence was 94.7％、48.3％、17.0％ and 8.6％ respectively.Adolescents were more vulnerable to primary hypertension than children of other ages.(3) 59.42％ of secondary hypertension patients had kidney disease.Blood pressure was higher in secondary hypertension patients.(4) Target organ damage was present during diagnosis in most of the patients with primary hypertension.Elevated levels of serum or urine β2-microglobulin could be an earlier indicator of kidney damage.In the target organ damage group,the mean diastolic blood pressure and insulin level were higher.Conclusions Patients with secondary hypertension were younger than patients with primary hypertension,and blood pressure was higher.The causes of secondary hypertension are mainly kidney and vascular lesions.Target organ damage can be detected during diagnosis in most of the patients with primary hypertension.The higher diastolic blood pressure and insulin levels,the more likely target organ damage can be.
Etiology and clinical analysis of 232 hospitalized children with hypertension

Abstract:Objective To analyze the etiology and clinical characteristics of hypertension in children;to ana-lyze the risk factors for primary hypertension with target organ damage,and to provide evidence for clinical diagnosis of hypertension in children. Methods Two hundred and thirty-two hospitalized children with hypertension at Children＇s Hospital Institute of Pediatrics from April 2013 to December 2017 were retrospectively analyzed,including general situa-tion,clinical manifestation,laboratory examination,auxiliary examination and damage assessment of target organs. Results (1)There were totally 232 eligible cases,consisting of 184 males(79. 3%)and 48 females(20. 7%). Among these children:38 cases(16. 4%),126 cases(54. 3%),and 68 cases(29. 3%)were diagnosed as prehypertension,hy-pertension in stageⅠ,and hypertension in stageⅡ,respectively.(2)The etiological composition of hypertension:there were 181 cases(78%)diagnosed as primary hypertension and 51 cases(22%)diagnosed as secondary hypertension, respectively;the proportion of the primary hypertension children during the period of preschool,school-age,and ado-lescent was 1. 6%(3 cases),21. 0%(38 cases)and 77. 4%(140 cases),respectively. The etiology of secondary hy-pertension was mostly of the renal parenchymal hypertension and renal vascular hypertension(43. 1%).(3)The clinical manifestations of primary hypertension were mostly of asymptomatic or mild symptoms,totally 148 cases(64%),and then dizziness,headache,chest pain,chest tightness,palpitation,blurred vision,and other clinical symptoms.(4)The high risk factors for primary hypertension with target-organ damage included cesarean section,glucose metabolism dis-turbance and body mass index >24 kg/m2 . Conclusions In recent years,the proportion of primary hypertension among hospitalized children has increased. The changes in the etiology of secondary hypertension are not found. Primary hypertension children are often accompanied with target organ damage. It is of great significance to make blood pressure monitoring as a routine physical examination for primary and middle school students,so as to detect hypertension in children earlier,and make active intervention and improve the prognosis of hypertension in children.

Abstract:Hypertension in children and adolescents is defined as systolic blood pressure(SBP)and/ or dias-tolic blood pressure(DBP)≥95th percentile for age,gender and height,on at least 3 occasions. Primary hypertension is more common among children of older age or adolescents,while secondary hypertension accounts for more cases for younger children. Among causes of secondary hypertension,renovascular diseases,renal parenchymal diseases,cardio-vascular diseases,and endocrine diseases are common. An initial evaluation can be reached after history taking and physical examination,to decide whether it should be primary or secondary hypertension. Laboratory tests and procedures can further confirm the classification and etiology. There is an increase in prevalence of hypertension in children and adolescents,and an in - time diagnosis and evaluation of hypertension is important to help patients receive a better management of their conditions.

Abstract:Objectives: To analyze the etiology and clinical characteristics of hypertension in hospitalized children, and to provide basis for clinical diagnosis and treatment of the children hypertension. Methods: From April 2013 to December 2017, clinical data of children with “hypertension” in the children's hospital affiliated to the capital institute of pediatrics was respectively analyzed. We calculated the proportion of primary hypertension and secondary hypertension. Results: A total of 262 children were enrolled in this study, and 30 cases were rejected by the exclusion criteria, leaving 232 eligible cases finally; 181 cases (78%) and 51 cases (22%) were diagnosed as primary hypertension and secondary hypertension, respectively, and 106 cases (59%) showed target organ damage in the 181 primary hypertension cases; 148 cases were asymptomatic or had mild symptoms (64%), 84 cases showed typical clinical symptoms - dizziness and headache (36%); Analyze the concomitant diseases: 145 cases were with obesity (63%), 67 cases were with sugar metabolic abnormalities (29%); Summarize the etiology of secondary hypertension. Conclusion: In this study, the cause categories of secondary hypertension did not change significantly compared with previous studies. However, we found that the age of secondary hypertension was becoming younger, the younger age, the greater the proportion. The primary hypertension was usually found in routine physical examination without obvious clinical symptoms. On the other hand, once symptoms begin to appear, target organ damage may have already occurred. Therefore, taking blood-pressure monitoring as a routine examination is significant for primary school and middle school students, which is in favor of early detection, intervention and improved outcomes of children hypertension.
Evolution of isolated systolic hypertension with normal central blood pressure in adolescents-prospective study

Abstract:Background: The clinical significance of isolated systolic hypertension with normal central blood pressure known as spurious hypertension (sHT) in adolescents and its evolution over time is not known. Methods: The aim of this study was to analyze changes in office, ambulatory blood pressure (ABPM), central systolic blood pressure (cSBP), hemodynamic parameters, and target organ damage (TOD) over a 1-year follow-up in a group of non-obese children with sHT. Results: Of 294 patients referred for primary hypertension, 138 patients (31 girls; 22%) had hypertension confirmed by ABPM. 48/138 (35%) patients (7 girls; 15%) were diagnosed with sHT (elevated office and ambulatory systolic BP, but normal cSBP); 43 of them (6 girls; 14%) were followed for 12 ± 3 months during non-pharmacological therapy. At baseline 7 (16%) patients had borderline values of cIMT or LVMi indicating mild TOD. After 12 months, 10/43 (3 girls; 23%) patients developed sustained HT (elevated office, ambulatory BP and cSBP), 11/43 (1 girl; 26%) maintained sHT, and 22/43 (2 girls; 51%) evolved to white coat hypertension or normotension. The cSBP values increased in 27 patients (4 girls; 63%), but the group average remained in the normal range. Prevalence of TOD did not change during observation. The multivariate regression analysis showed that the only predictor of cSBP change over time was a change in serum uric acid level. Conclusions: In conclusion, after 1 year of non-pharmacological treatment, 23% of adolescents with sHT developed sustained hypertension, with the main predictor of cSBP change being the change in serum uric acid.

Abstract:BACKGROUND We evaluated children with hypertension and compared those with essential and secondary (including renal and non-renal) hypertension. METHODS This retrospective study analyzed data from hypertensive children (age, 0-18 years) referred for treatment between January 2008 and December 2015. Demographic factors, causative factors, and medical treatments were evaluated. Treatment failure was defined as a systolic or diastolic blood pressure ≥95th percentile for age, gender, and height on three separate occasions, despite treatment. All patients not meeting the failure criteria were considered to have controlled hypertension. The control rate was defined as the proportion of patients with controlled blood pressure. RESULTS Among 172 consecutive patients, 28% had essential hypertension and 72% had secondary hypertension. As compared with children with secondary hypertension, those with essential hypertension had a higher frequency of family history of hypertension (P<0.001), a higher body mass index (BMI) (P=0.001), lower frequency of proteinuria (P=0.003), lower uric acid (P=0.04), and lower triglyceride (P=0.048). The medications used in the controlled group were similar to those used in the uncontrolled group. Angiotensin-converting enzyme inhibitors (ACEIs) were only used in nephrogenic patients, and a higher rate of ACEI use seemed to increase control rates. Control rates did not significantly differ by age, number of drugs, or cause of hypertension. CONCLUSIONS As compared with children with secondary hypertension, those with essential hypertension were more likely to have a family history of hypertension and had a higher BMI, lower frequency of proteinuria, and lower uric acid and triglyceride concentrations. Treatment guidelines for essential and secondary hypertension should be established for children of all ages.

Abstract:Over the past 60 years there has been a dramatic increase in the prevalence of overweight in children and adolescents, ranging from 4% in 1975 to 18% in 2016. Recent estimates indicate that overweight or obese children and adolescents are more than 340 million. Obesity is often associated with hypertension, which is an important cardiovascular risk factor. Recent studies show that the presence of hypertension is a frequent finding in the pediatric age. Hypertensive children easily become hypertensive adults. This phenomenon contributes to increasing cardiovascular risk in adulthood. Primary hypertension is a growing problem especially in children and adolescents of western countries, largely because of its association with the ongoing obesity epidemic. Recently, it has been hypothesized that a dietary link between obesity and elevated blood pressure (BP) values could be simple carbohydrate consumption, particularly fructose, both in adults and in children. Excessive intake of fructose leads to increased serum uric acid (SUA) and high SUA values are independently associated with the presence of hypertension and weaken the efficacy of lifestyle modifications in children. The present review intends to provide an update of existing data regarding the relationship between BP, simple carbohydrates (particularly fructose), and uric acid in pediatric age. In addition, we analyze the national policies that have been carried out over the last few years, in order to identify the best practices to limit the socio-economic impact of the effects of excessive sugar consumption in children.

Abstract:The excitation of whispering-gallery-mode resonances of cylindrical microcavities, using narrow fiber tapers, produces simultaneously the excitation of spiral modes. The power coupled to the spiral modes leads to a significant broadening of the dips observed in the spectrum of light transmitted through the taper. We demonstrate that whispering-gallery modes resonances of cylindrical microcavities can also be interrogated by measuring the reflection spectrum. Unlike the dips observed in transmission, the peaks in the reflection spectrum are symmetric and narrower than the corresponding transmission dips.
