Abstract:Medical data on chronic diseases are rife with the private information of patients. These data are often stored in many isolated institutions and cannot be shared due to privacy and security concerns. Federated learning provides a privacy protection solution for collaboration among multiple medical institutions by exchanging model parameters instead of private data. However, in real-world healthcare scenarios, the quality of medical data owned by different medical institutions is highly distinct, which poses a critical challenge for the application of federated learning. To tackle this problem, we propose a client level division based federated learning algorithm that estimates the quality of all data sources to fairly coordinate the training of the federated model among multiple participants. Specifically, we quantify the value of clients by testing their local models. Then we divide all clients into several levels based on their data quality and enable clients with high-quality data to have a higher priority in federated learning. Our method provides an insight into which participants are more valuable for the prediction task, thereby facilitating the model training process. Experiments on the CHRALS dataset demonstrate that our method accelerates the convergence of federated learning and improves the performance of the federated model.

Abstract:Federated learning (FL) is a promising technical support to the vision of ubiquitous artificial intelligence in the sixth generation (6G) wireless communication network. However, traditional FL heavily relies on a trusted centralized server. Besides, FL is vulnerable to poisoning attacks, and the global aggregation of model updates makes the private training data under the risk of being reconstructed. What's more, FL suffers from efficiency problem due to heavy communication cost. Although decentralized FL eliminates the problem of the central dependence of traditional FL, it makes other problems more serious. In this paper, we propose BlockDFL, an efficient fully peer-to-peer (P2P) framework for decentralized FL. It integrates gradient compression and our designed voting mechanism with blockchain to efficiently coordinate multiple peer participants without mutual trust to carry out decentralized FL, while preventing data from being reconstructed according to transmitted model updates. Extensive experiments conducted on two real-world datasets exhibit that BlockDFL obtains competitive accuracy compared to centralized FL and can defend against poisoning attacks while achieving efficiency and scalability. Especially when the proportion of malicious participants is as high as 40 percent, BlockDFL can still preserve the accuracy of FL, which outperforms existing fully decentralized FL frameworks.

Abstract:The use of machine learning (ML) with electronic health records (EHR) is growing in popularity as a means to extract knowledge that can improve the decision-making process in healthcare. Such methods require training of high-quality learning models based on diverse and comprehensive datasets, which are hard to obtain due to the sensitive nature of medical data from patients. In this context, federated learning (FL) is a methodology that enables the distributed training of machine learning models with remotely hosted datasets without the need to accumulate data and, therefore, compromise it. FL is a promising solution to improve ML-based systems, better aligning them to regulatory requirements, improving trustworthiness and data sovereignty. However, many open questions must be addressed before the use of FL becomes widespread. This article aims at presenting a systematic literature review on current research about FL in the context of EHR data for healthcare applications. Our analysis highlights the main research topics, proposed solutions, case studies, and respective ML methods. Furthermore, the article discusses a general architecture for FL applied to healthcare data based on the main insights obtained from the literature review. The collected literature corpus indicates that there is extensive research on the privacy and confidentiality aspects of training data and model sharing, which is expected given the sensitive nature of medical data. Studies also explore improvements to the aggregation mechanisms required to generate the learning model from distributed contributions and case studies with different types of medical data.

Abstract:In the era of data-driven healthcare, the amalgamation of blockchain and Federated Learning (FL) introduces a paradigm shift towards secure, collaborative, and patient-centric data-sharing. This paper pioneers the exploration of the conceptual framework and technical synergy of FL and blockchain for decentralized data-sharing, aiming to strike a balance between data utility and privacy. FL, a decentralized machine learning paradigm, enables collaborative AI model training across multiple healthcare institutions without sharing raw patient data. Combined with blockchain, a transparent and immutable ledger, it establishes an ecosystem fostering trust, security, and data integrity. The paper elucidates the technical foundations of FL and blockchain, unravelling their roles in reshaping healthcare data-sharing. The paper vividly illustrates the potential impact of this fusion on patient care. The proposed approach preserves patient privacy while granting healthcare providers and researchers access to diversified datasets, ultimately leading to more accurate models and improved diagnoses. The findings underscore the potential acceleration of medical research, improved treatment outcomes, and patient empowerment through data ownership. Abstract:The healthcare industry is one of the most vulnerable to cybercrime and privacy violations because health data is very sensitive and spread out in many places. Recent confidentiality trends and a rising number of infringements in different sectors make it crucial to implement new methods that protect data privacy while maintaining accuracy and sustainability. Moreover, the intermittent nature of remote clients with imbalanced datasets poses a significant obstacle for decentralized healthcare systems. Federated learning (FL) is a decentralized and privacy-protecting approach to deep learning and machine learning models. In this paper, we implement a scalable FL framework for interactive smart healthcare systems with intermittent clients using chest X-ray images. Remote hospitals may have imbalanced datasets with intermittent clients communicating with the FL global server. The data augmentation method is used to balance datasets for local model training. In practice, some clients may leave the training process while others join due to technical or connectivity issues. The proposed method is tested with five to eighteen clients and different testing data sizes to evaluate performance in various situations. The experiments show that the proposed FL approach produces competitive results when dealing with two distinct problems, such as intermittent clients and imbalanced data. These findings would encourage medical institutions to collaborate and use rich private data to quickly develop a powerful patient diagnostic model.

Abstract:Background: The integrity and reliability of clinical research outcomes rely heavily on access to vast amounts of data. However, the fragmented distribution of these data across multiple institutions, along with ethical and regulatory barriers, presents significant challenges to accessing relevant data. While federated learning offers a promising solution to leverage insights from fragmented data sets, its adoption faces hurdles due to implementation complexities, scalability issues, and inclusivity challenges. Objective: This paper introduces Federated Learning for Everyone (FL4E), an accessible framework facilitating multistakeholder collaboration in clinical research. It focuses on simplifying federated learning through an innovative ecosystem-based approach. Methods: The "degree of federation" is a fundamental concept of FL4E, allowing for flexible integration of federated and centralized learning models. This feature provides a customizable solution by enabling users to choose the level of data decentralization based on specific health care settings or project needs, making federated learning more adaptable and efficient. By using an ecosystem-based collaborative learning strategy, FL4E encourages a comprehensive platform for managing real-world data, enhancing collaboration and knowledge sharing among its stakeholders. Results: Evaluating FL4E's effectiveness using real-world health care data sets has highlighted its ecosystem-oriented and inclusive design. By applying hybrid models to 2 distinct analytical tasks-classification and survival analysis-within real-world settings, we have effectively measured the "degree of federation" across various contexts. These evaluations show that FL4E's hybrid models not only match the performance of fully federated models but also avoid the substantial overhead usually linked with these models. Achieving this balance greatly enhances collaborative initiatives and broadens the scope of analytical possibilities within the ecosystem. Conclusions: FL4E represents a significant step forward in collaborative clinical research by merging the benefits of centralized and federated learning. Its modular ecosystem-based design and the "degree of federation" feature make it an inclusive, customizable framework suitable for a wide array of clinical research scenarios, promising to revolutionize the field through improved collaboration and data use. Towards Fair and Privacy Preserving Federated Learning for the Healthcare Domain

Abstract:Federated learning enables data sharing in healthcare contexts where it might otherwise be difficult due to data-use-ordinances or security and communication constraints. Distributed and shared data models allow models to become generalizable and learn from heterogeneous clients. While addressing data security, privacy, and vulnerability considerations, data itself is not shared across nodes in a given learning network. On the other hand, FL models often struggle with variable client data distributions and operate on an assumption of independent and identically distributed data. As the field has grown, the notion of fairness-aware federated learning mechanisms has also been introduced and is of distinct significance to the healthcare domain where many sensitive groups and protected classes exist. In this paper, we create a benchmark methodology for FAFL mechanisms under various heterogeneous conditions on datasets in the healthcare domain typically outside the scope of current federated learning benchmarks, such as medical imaging and waveform data formats. Our results indicate considerable variation in how various FAFL schemes respond to high levels of data heterogeneity. Additionally, doing so under privacy-preserving conditions can create significant increases in network communication cost and latency compared to the typical federated learning scheme.

Abstract:Federated learning (FL) is a system in which a central aggregator coordinates the efforts of multiple clients to solve machine learning problems. This setting allows training data to be dispersed in order to protect privacy. The purpose of this paper is to provide an overview of FL systems with a focus on healthcare. FL is evaluated here based on its frameworks, architectures, and applications. It is shown here that FL solves the preceding issues with a shared global deep learning (DL) model via a central aggregator server. This paper examines recent developments and provides a comprehensive list of unresolved issues, inspired by the rapid growth of FL research. In the context of FL, several privacy methods are described, including secure multiparty computation, homomorphic encryption, differential privacy, and stochastic gradient descent. Furthermore, a review of various FL classes, such as horizontal and vertical FL and federated transfer learning, is provided. FL has applications in wireless communication, service recommendation, intelligent medical diagnosis systems, and healthcare, all of which are discussed in this paper. We also present a thorough review of existing FL challenges, such as privacy protection, communication cost, system heterogeneity, and unreliable model upload, followed by future research directions.

Abstract:Facilitating large-scale, cross-institutional collaboration in biomedical machine learning projects requires a trustworthy and resilient federated learning (FL) environment to ensure that sensitive information such as protected health information is kept confidential. In this work, we introduce APPFLx, a low-code FL framework that enables the easy setup, configuration, and running of FL experiments across organizational and administrative boundaries while providing secure end-to-end communication, privacy-preserving functionality, and identity management. APPFLx is completely agnostic to the underlying computational infrastructure of participating clients. We demonstrate the capability of APPFLx as an easy-to-use framework for accelerating biomedical studies across institutions and healthcare systems while maintaining the protection of private medical data in two case studies: (1) predicting participant age from electrocardiogram (ECG) waveforms, and (2) detecting COVID-19 disease from chest radiographs. These experiments were performed securely across heterogeneous compute resources, including a mixture of on-premise high-performance computing and cloud computing, and highlight the role of federated learning in improving model generalizability and performance when aggregating data from multiple healthcare systems. Finally, we demonstrate that APPFLx serves as a convenient and easy-to-use framework for accelerating biomedical studies across institutions and healthcare system while maintaining the protection of private medical data.

Abstract:Federated learning (FL) provides a distributed machine learning system that enables participants to train using local data to create a shared model by eliminating the requirement of data sharing. In healthcare systems, FL allows Medical Internet of Things (MIoT) devices and electronic health records (EHRs) to be trained locally without sending patients data to the central server. This allows healthcare decisions and diagnoses based on datasets from all participants, as well as streamlining other healthcare processes. In terms of user data privacy, this technology allows collaborative training without the need of sharing the local data with the central server. However, there are privacy challenges in FL arising from the fact that the model updates are shared between the client and the server which can be used for re-generating the client's data, breaching privacy requirements of applications in domains like healthcare. In this paper, we have conducted a review of the literature to analyse the existing privacy and security enhancement methods proposed for FL in healthcare systems. It has been identified that the research in the domain focuses on seven techniques: Differential Privacy, Homomorphic Encryption, Blockchain, Hierarchical Approaches, Peer to Peer Sharing, Intelligence on the Edge Device, and Mixed, Hybrid and Miscellaneous Approaches. The strengths, limitations, and trade-offs of each technique were discussed, and the possible future for these seven privacy enhancement techniques for healthcare FL systems was identified.

Abstract:Machine Learning (ML) algorithms are generally designed for scenarios in which all data is stored in one data center, where the training is performed. However, in many applications, e.g., in the healthcare domain, the training data is distributed among several entities, e.g., different hospitals or patients' mobile devices/sensors. At the same time, transferring the data to a central location for learning is certainly not an option, due to privacy concerns and legal issues, and in certain cases, because of the communication and computation overheads. Federated Learning (FL) is the state-of-the-art collaborative ML approach for training an ML model across multiple parties holding local data samples, without sharing them. However, enabling learning from distributed data over such edge Internet of Things (IoT) systems (e.g., mobile-health and wearable technologies, involving sensitive personal/medical data) in a privacy-preserving fashion presents a major challenge mainly due to their stringent resource constraints, i.e., limited computing capacity, communication bandwidth, memory storage, and battery lifetime. In this paper, we propose a privacy-preserving edge FL framework for resource-constrained mobile-health and wearable technologies over the IoT infrastructure. We evaluate our proposed framework extensively and provide the implementation of our technique on Amazon's AWS cloud platform based on the seizure detection application in epilepsy monitoring using wearable technologies.

Abstract:The advent of Federated Learning has enabled the creation of a high-performing model as if it had been trained on a considerable amount of data. A multitude of participants and a server cooperatively train a model without the need for data disclosure or collection. The healthcare industry, where security and privacy are paramount, can substantially benefit from this new learning paradigm, as data collection is no longer feasible due to stringent data policies. Nonetheless, unaddressed challenges and insufficient attack mitigation are hampering its adoption. Attack surfaces differ from traditional centralized learning in that the server and clients communicate between each round of training. In this paper, we thus present vulnerabilities, attacks, and defenses based on the widened attack surfaces, as well as suggest promising new research directions toward a more robust FL.

Abstract:Artificial intelligence (AI) shows potential to improve health care by leveraging data to build models that can inform clinical workflows. However, access to large quantities of diverse data is needed to develop robust generalizable models. Data sharing across institutions is not always feasible due to legal, security, and privacy concerns. Federated learning (FL) allows for multi-institutional training of AI models, obviating data sharing, albeit with different security and privacy concerns. Specifically, insights exchanged during FL can leak information about institutional data. In addition, FL can introduce issues when there is limited trust among the entities performing the compute. With the growing adoption of FL in health care, it is imperative to elucidate the potential risks. We thus summarize privacy-preserving FL literature in this work with special regard to health care. We draw attention to threats and review mitigation approaches. We anticipate this review to become a health-care researcher's guide to security and privacy in FL.

Abstract:The proliferation of Internet and Communication Technologies (ICTs) has ushered in a period often referred to as Industry 5.0. The subsequent development is accompanied by the healthcare industry coining Healthcare 5.0. Healthcare 5.0 incorporates the Internet of Things (IoT), enabling medical imaging technologies to facilitate early diagnosis of diseases and enhance the quality of healthcare facilities' service. Nevertheless, the healthcare sector is currently experiencing a delay in adopting Artificial Intelligence (AI) and big data technologies compared to other sectors under the umbrella of Industry 5.0. This delay may be attributed to the prevailing concerns about data privacy within the healthcare domain. In recent times, there has been a noticeable increase in the use of Machine Learning (ML) enabled adaptive Internet of Medical Things (IoMT) systems with different technologies for medical applications. ML is an essential component of the IoMT system, as it optimizes the trade-off between delay and energy consumption. The issue of data fraud in classical learning models inside the distributed IoMT system for medical applications remains a significant research challenge in practical settings. This paper proposes Federated Learning and Blockchain-Enabled Privacy-Preserving (FL-BEPP) for Fraud Prevention and Security (FPS) in the IoMT framework. The system incorporates numerous dynamic strategies. This research examines the medical applications that exhibit hard constraints, such as deadlines, and soft constraints, such as resource consumption, when executed on distributed fog and cloud nodes. The primary objective of FL-BEPP is to effectively detect and safeguard the confidentiality and integrity of data across many tiers, including local fog nodes and faraway clouds. This is achieved by minimizing power use and delay while simultaneously meeting the time constraints associated with healthcare workloads.

Abstract:Data privacy has become a major concern in healthcare due to the increasing digitization of medical records and data-driven medical research. Protecting sensitive patient information from breaches and unauthorized access is critical, as such incidents can have severe legal and ethical complications. Federated Learning (FL) addresses this concern by enabling multiple healthcare institutions to collaboratively learn from decentralized data without sharing it. FL's scope in healthcare covers areas such as disease prediction, treatment customization, and clinical trial research. However, implementing FL poses challenges, including model convergence in non-IID (independent and identically distributed) data environments, communication overhead, and managing multi-institutional collaborations. A systematic review of FL in healthcare is necessary to evaluate how effectively FL can provide privacy while maintaining the integrity and usability of medical data analysis. In this study, we analyze existing literature on FL applications in healthcare. We explore the current state of model security practices, identify prevalent challenges, and discuss practical applications and their implications. Additionally, the review highlights promising future research directions to refine FL implementations, enhance data security protocols, and expand FL's use to broader healthcare applications, which will benefit future researchers and practitioners.

Abstract:Federated Learning (FL) is a distributed machine learning paradigm designed for privacy-sensitive applications that run on resource-constrained devices with non-Identically and Independently Distributed (IID) data. Traditional FL frameworks adopt the client-server model with a single-level aggregation (AGR) process, where the server builds the global model by aggregating all trained local models received from client devices. However, this conventional approach encounters challenges, including susceptibility to model/data poisoning attacks. In recent years, advancements in the Internet of Things (IoT) and edge computing have enabled the development of hierarchical FL systems with a two-level AGR process running at edge and cloud servers. In this paper, we propose a Secure Hierarchical FL (SHFL) framework to address poisoning attacks in hierarchical edge networks. By aggregating trained models at the edge, SHFL employs two novel methods to address model/data poisoning attacks in the presence of client adversaries: 1) a client selection algorithm running at the edge for choosing IoT devices to participate in training, and 2) a model AGR method designed based on convex optimization theory to reduce the impact of edge models from networks with adversaries in the process of computing the global model (at the cloud level). The evaluation results reveal that compared to state-of-the-art methods, SHFL significantly increases the maximum accuracy achieved by the global model in the presence of client adversaries applying model/data poisoning attacks.

Abstract:Federated Learning (FL) has emerged as a transformative approach for enabling distributed machine learning while preserving user privacy, yet it faces challenges like communication inefficiencies and reliance on centralized infrastructures, leading to increased latency and costs. This paper presents a novel FL methodology that overcomes these limitations by eliminating the dependency on edge servers, employing a server-assisted Proximity Evaluation for dynamic cluster formation based on data similarity, performance indices, and geographical proximity. Our integrated approach enhances operational efficiency and scalability through a Hybrid Decentralized Aggregation Protocol, which merges local model training with peer-to-peer weight exchange and a centralized final aggregation managed by a dynamically elected driver node, significantly curtailing global communication overhead. Additionally, the methodology includes Decentralized Driver Selection, Check-pointing to reduce network traffic, and a Health Status Verification Mechanism for system robustness. Validated using the breast cancer dataset, our architecture not only demonstrates a nearly tenfold reduction in communication overhead but also shows remarkable improvements in reducing training latency and energy consumption while maintaining high learning performance, offering a scalable, efficient, and privacy-preserving solution for the future of federated learning ecosystems.

Abstract:In the context of distributed machine learning, the concept of federated learning (FL) has emerged as a solution to the privacy concerns that users have about sharing their own data with a third-party server. FL allows a group of users (often referred to as clients ) to locally train a single machine learning model on their devices without sharing their raw data. One of the main challenges in FL is how to select the most appropriate clients to participate in the training of a certain task. In this paper, we address this challenge and propose a trust-based deep reinforcement learning approach to select the most adequate clients in terms of resource consumption and training time. On top of the client selection mechanism, we embed a transfer learning approach to handle the scarcity of data in some regions and compensate potential lack of learning at some servers. We apply our solution in the healthcare domain in a COVID-19 detection scenario over IoT devices. In the considered scenario, edge servers collaborate with IoT devices to train a COVID-19 detection model using FL without having to share any raw confidential data. Experiments conducted on a real-world COVID-19 dataset reveal that our solution achieves a good trade-off between detection accuracy and model execution time compared to existing approaches.
