Abstract:Background Chronic obstructive pulmonary disease (COPD) is characterized by progressive airflow obstruction and lung hyperinflation due to loss of elastic recoil and air trapping. These changes are related to an altered pattern of ventilatory muscle recruitment. Respiratory muscle training aims to alter respiratory muscle recruitment to reduce dyspnea, hyperinflation, improve respiratory muscle performance, and optimize thoracoabdominal movement. Objectives To analyze the effect of respiratory muscle training in people with Chronic Obstructive Pulmonary Disease at different stages. Methods This is a systematized integrative literature review, carried out in the PubMed, SciELO and Physiotherapy Evidence Database (PEDro) databases, using the following search strategy: “breathing exercises” AND “maximal respiratory pressures” AND “chronic obstructive pulmonary disease”. Searches were carried out limited to the years 2019 to 2023. The inclusion criteria were randomized controlled clinical trials, carried out in humans, in Portuguese and/or English, available in full and related to the proposed topic. The selection of studies was performed by two reviewers simultaneously and independently. The research was conducted following four stages: 1) reading of titles and abstracts; 2) analysis of duplicates; 3) reading the articles in full and 4) extracting the main information. The risk of bias in clinical trials was assessed using the Cochrane Risk of Bias Tool and the quality of evidence using the Grading of Recommendations Assessment, Development and Evaluation (GRADE). Results After searching the databases, 880 articles were found. Of these, 7 articles were selected to compose the present review because they met the objective and determined criteria. The respiratory muscle training protocols consisted of fast and deep breathing training with Voldyne, inspiratory and expiratory muscle training with Threshold, pursed lip breathing, respiratory muscle training with Powerbreathe, deep breathing and blowing straw in a glass of water. A total of 472 individuals, with mild to severe COPD, were analyzed. Intervention duration and weekly frequency ranged from 1 to 18 months, 3 to 7 days a week, respectively. All studies show that respiratory muscle training improved symptoms of dyspnea and fatigue, increased muscle strength, FEV1, FVC, functional capacity and improved quality of life (p<0.05). About risk of bias, studies ranged from low to moderate and the quality of evidence ranged from moderate to very low. Conclusion It was observed that respiratory muscle training significantly contributed to improving symptoms of shortness of breath, fatigue and increased muscle strength, quality of life and functional capacity. Implications Respiratory muscle training seems to be a good treatment for patients in different stages of COPD and at any level of care, whether at home, outpatient, or hospital, in addition to improving the symptoms of the disease and the patient's quality of life.

Abstract:Noninvasive ventilation is an increasingly disseminated therapeutic option, which is explained by increases in the prevalence of chronic respiratory diseases, life expectancy, and the effectiveness of this type of respiratory support. This literature review observes that upon returning home after hospital discharge, there are difficulties in adhering to and maintaining this therapy. The aim of this study is to identify nursing interventions for client and family training in the proper use of noninvasive ventilation in the transition from hospital to community. A scoping review was carried out by searching MEDLINE, CINAHL, Scopus, and Web of Science. The articles were selected by two independent reviewers by applying the predefined eligibility criteria. Regarding transitional care, the authors opted to include studies about interventions to train clients and families during hospital stay, hospital discharge, transition from hospital to home, and the first 30 days after returning home. The eight included publications allowed for identification of interventions related to masks or interfaces, prevention of complications associated with noninvasive ventilation, leakage control, maintenance and cleaning of ventilators and accessories, respiratory training, ventilator monitoring, communication, and behavioral strategies as transitional care priority interventions to guarantee proper training in the transition from hospital to community.

Abstract:Objective: evidence is inconsistent for the effect of non-invasive ventilation (NIV) for individuals with chronic obstructive pulmonary disease (COPD) during exercise training. This review aimed to determine the effect of NIV in COPD individuals during exercise training on exercise capacity, quality of life, functional performance and symptoms. Material and methods: we searched for studies evaluating the effect of NIV on COPD individuals during exercise training published until May 2020 in 6 electronic databases (PubMed, Embase, Cochrane Library, Web of Science, clinical trial registers and Wanfang). The included studies were appraised with the Cochrane Risk of Bias tool and Downs and Black criteria. The primary outcomes were improvement in 6-min walking distance and quality of life. Mean difference (MD) or standardized mean difference (SMD) with 95% confidence intervals (CIs) was calculated. Results: among 855 identified articles, reports for 15 studies with heterogeneous populations were eligible, with 520 individuals: 257 in the NIV group and 263 in the control group. Across studies, NIV intervention during exercise training affected exercise performance (6-min walking distance: SMD: 0.33, 95% CI: 0.06; 0.59, P=0.02; quality of life: SMD: −0.77, 95% CI: −1.01; −0.53, P<0.001). In the analysis of dyspnea, pooled estimates demonstrated improvement in the NIV versus control group. NIV intervention was also better than exercise alone in ameliorating oxygen saturation, PaO2, PaCO2, blood lactate level and breath rate. The groups did not differ in duration of exercise, BODE index, minute ventilation, heart rate and systolic blood pressure. Conclusions: our review suggests that NIV is a relevant adjuvant for exercise training in COPD individuals because the intervention could improve exercise performance and quality of life. The current results also demonstrate the importance of further investigations of higher methodological quality to assess the effect on exercise capacity and quality of life.
Mobility impact and methods of diaphragm monitoring in patients with chronic obstructive pulmonary disease: a systematic review

Abstract:The objectives of the study were to identify the factors that limit diaphragmatic mobility and evaluate the therapeutic results of the monitoring methods previously used in patients with chronic obstructive pulmonary disease. The PubMed, Web of Science, Scopus, and LILACS databases were used. A gray literature search was conducted with Google scholar. PRISMA was used, and the bias risk analysis adapted from the Cochrane Handbook for clinical trials and, for other studies, the Downs and Black checklist were used. Twenty-five articles were included in the qualitative synthesis analysis on physiotherapeutic techniques and diaphragmatic mobility. Eight clinical trials indicated satisfactory domains, and on the Downs and Black scale, 17 cohort studies were evaluated to have an acceptable score. Different conditions must be observed; for example, for postoperative assessments the supine position is suggested to be the most appropriate position to verify diaphragm excursion, although it has been shown to be associated with difficulty of restriction and matching in samples. Therefore, we identified the need for contemporary adjustments and strategies that used imaging instruments, preferably in the dorsal position. Therapeutic evidence on the association between the instrumental method and diaphragmatic mobility can be controversial. The ultrasound measurements indicated some relevance for different analyses, for pulmonary hyperinflation as well as diaphragm thickness and mobilization, in COPD patients. In particular, the study suggests that the ultrasound technique with B-mode for analysis and M-mode for diaphragmatic excursion be used with a 2 - 5 MHz with the patient in the supine position. However, the methods used to monitor diaphragm excursion should be adapted to the conditions of the patients, and additional investigations of their characteristics should be performed. More selective inclusion criteria and better matching in the samples are very important. In addition, more narrow age, sex and weight categories are important, especially in patients with chronic obstructive pulmonary disease.

Abstract:BackgroundThere is a controversy about the additional benefit of various supplemental interventions used in clinical practice to further enhance the effectiveness of respiratory rehabilitation in patients with Chronic obstructive pulmonary disease (COPD). The aim of this research was to assess randomised controlled trials (RCTs) testing the additional benefit of supplemental interventions during respiratory rehabilitation in COPD patients.MethodsSystematic review with literature searches in six electronic databases, extensive hand-searching and contacting of authors. Two reviewers selected independently eligible RCTs, rated the methodological quality and extracted the data, which were analyzed considering the minimal important difference of patient-important outcomes where possible.FindingsWe identified 20 RCTs whereof 18 provided sufficient data for analysis. The methodological quality was low and sample sizes were too small for most trials to produce meaningful results (median total sample size = 28). Data from five trials showed that supplemental oxygen during exercise did not have clinically meaningful effects on health-related quality of life while improvements of exercise capacity may be even larger for patients exercising on room air. RCTs of adding assisted ventilation, nutritional supplements or a number of anabolically acting drugs do not provide sufficient evidence for or against the use any of these supplemental interventions.InterpretationThere is insufficient evidence for most supplemental interventions during respiratory rehabilitation to estimate their additional value, partly due to methodological shortcomings of included RCTs. Current data do not suggest benefit from supplemental oxygen during exercise, although the methodological quality of included trials limits conclusions. To appropriately assess any of the various supplemental interventions used in clinical practice, pragmatic trials on respiratory rehabilitation of COPD patients need to consider methodological aspects as well as appropriate sample sizes.

Abstract:this article, we have investigate a Taylor collocation method, which is based on collocation method for solving fractional pantograph equation. This method is based on first taking the truncated fractional Taylor expansions of the solution function in the mathematical model and then substituting their matrix forms into the equation. Using the collocation points, we have the system of nonlinear algebraic equation. Then, we solve the system of linear algebraic equation using Maple 14 and we obtain the coefficients of Taylor expansion. In addition illustrative example is presented to demonstrate the effectiveness of the proposed method. Invasive versus non-invasive ventilation for acute respiratory failure in neuromuscular disease and chest wall disorders

Abstract:Background: Acute respiratory failure is a common life-threatening complication of acute onset neuromuscular diseases, and may exacerbate chronic hypoventilation in patients with neuromuscular disease or chest wall disorders. Standard management includes oxygen supplementation, physiotherapy, cough assistance, and, whenever needed, antibiotics and intermittent positive pressure ventilation. Non-invasive mechanical ventilation (NIV) via nasal, buccal or full-face devices has become routine practice in many centres. Objectives: The primary objective of this review was to compare the efficacy of non-invasive ventilation with invasive ventilation in improving short-term survival in acute respiratory failure in people with neuromuscular disease and chest wall disorders. The secondary objectives were to compare the effects of NIV with those of invasive mechanical ventilation on improvement in arterial blood gas after 24 hours and lung function measurements after one month, incidence of barotrauma and ventilator-associated pneumonia, duration of mechanical ventilation, length of stay in the intensive care unit and length of hospital stay. Search methods: We searched the following databases on 11 September 2017: the Cochrane Neuromuscular Specialised Register, CENTRAL, MEDLINE and Embase. We also searched conference proceedings and clinical trials registries. Selection criteria: We planned to include randomised or quasi-randomised trials with or without blinding. We planned to include trials performed in children or adults with acute onset neuromuscular diseases or chronic neuromuscular disease or chest wall disorders presenting with acute respiratory failure that compared the benefits and risks of invasive ventilation versus NIV. Data collection and analysis: Two review authors reviewed searches and independently selected studies for assessment. We planned to follow standard Cochrane methodology for data collection and analysis. Main results: We did not identify any trials eligible for inclusion in the review. Authors' conclusions: Acute respiratory failure is a life-threatening complication of acute onset neuromuscular disease and of chronic neuromuscular disease and chest wall disorders. We found no randomised trials on which to elaborate evidence-based practice for the use of non-invasive versus invasive mechanical ventilation. For researchers, there is a need to design and conduct new randomised trials to compare NIV with invasive ventilation in acute neuromuscular respiratory failure. These trials should anticipate variations in treatment responses according to disease condition (acute onset versus acute exacerbation on chronic neuromuscular diseases) and according to the presence or absence of bulbar dysfunction.
Influence of Mechanical Ventilation Modes on the Efficacy of Nebulized Bronchodilators in the Treatment of Intubated Adult Patients with Obstructive Pulmonary Disease

Abstract:Background: Little has been reported in terms of clinical outcomes to confirm the benefits of nebulized bronchodilators during mechanical ventilation (MV). Electrical Impedance Tomography (EIT) could be a valuable method to elucidate this gap. Objective: The purpose of this study is to evaluate the impact of nebulized bronchodilators during invasive MV with EIT by comparing three ventilation modes on the overall and regional lung ventilation and aeration in critically ill patients with obstructive pulmonary disease. Method: A blind clinical trial in which eligible patients underwent nebulization with salbutamol sulfate (5 mg/1 mL) and ipratropium bromide (0.5 mg/2 mL) in the ventilation mode they were receiving. EIT evaluation was performed before and after the intervention. A joint and stratified analysis into ventilation mode groups was performed, with p < 0.05. Results: Five of nineteen procedures occurred in controlled MV mode, seven in assisted mode and seven in spontaneous mode. In the intra-group analysis, the nebulization increased total ventilation in controlled (p = 0.04 and ⅆ = 2) and spontaneous (p = 0.01 and ⅆ = 1.5) MV modes. There was an increase in the dependent pulmonary region in assisted mode (p = 0.01 and ⅆ = 0.3) and in spontaneous mode (p = 0.02 and ⅆ = 1.6). There was no difference in the intergroup analysis. Conclusions: Nebulized bronchodilators reduce the aeration of non-dependent pulmonary regions and increase overall lung ventilation but there was no difference between the ventilation modes. As a limitation, it is important to note that the muscular effort in PSV and A/C PCV modes influences the impedance variation, and consequently the aeration and ventilation values. Thus, future studies are needed to evaluate this effort as well as the time on ventilator, time in UCI and other variables.

More recently, inspiratory muscle training (IMT) is becoming an effective complementary treatment with positive effects on muscle strength and exercise capacity. We assessed the effectiveness of IMT on the cardiovascular system through autonomic function modulation via heart rate variability and arterial blood pressure. Methods: Randomized controlled trials (RCTs) were identified from searches of The Cochrane Library, MEDLINE and EMBASE to November 2018. Citations, conference proceedings and previous reviews were included without population restriction, comparing IMT intervention to no treatment, placebo or active control. Results: We identified 10 RCTs involving 267 subjects (mean age range 51–71 years). IMT programs targeted maximum inspiratory pressure (MIP) and cardiovascular outcomes, using low ( n =6) and moderate to high intensity ( n =4) protocols, but the protocols varied considerably (duration: 1–12 weeks, frequency: 3–14 times/week, time: 10–30 mins). An overall increase of the MIP (cmH 2 O) was observed (−27.57 95% CI −18.48, −37.45, I 2 =64%), according to weighted mean difference (95%CI), and was accompanied by a reduction of the low to high frequency ratio (−0.72 95% CI−1.40, −0.05, I 2 =50%). In a subgroup analysis, low- and moderate-intensity IMT treatment was associated with a reduction of the heart rate (HR) (−7.59 95% CI −13.96, −1.22 bpm, I 2 =0%) and diastolic blood pressure (DBP) (−8.29 [−11.64, −4.94 mmHg], I 2 =0%), respectively. Conclusion: IMT is an effective treatment for inspiratory muscle weakness in several populations and could be considered as a complementary treatment to improve the cardiovascular system, mainly HR and DBP. Abstract:OBJECTIVE This article will describe the elements of performing a thorough venous ultrasound evaluation of the lower extremity in patients with manifestations of chronic venous disorder. The emphasis will be on the evaluation of superficial venous reflux. Only the specific aspects of the evaluation of the deep system pertaining to chronic venous disease will be discussed. CONCLUSION Duplex ultrasound requires the examiner to solve a puzzle to explain the patient's clinical manifestations. Patients who have been treated with surgery, thermal ablation, or ultrasound-guided sclerotherapy will require duplex ultrasound after treatment to identify complications, gauge the extent of treatment success, and evaluate the cause for any recurrence.
Lung Expansion Therapy for the Potential Populations: A Literature Review

Abstract:Secretion retention, atelectasis, and reduced lung compliance occur when endotracheal or tracheostomised patients cough ineffectively. There is a possibility of infection if the collapsed regions of the lungs are not reinflated. Therefore, to improve clinical outcomes, such as diaphragm mobility and thickness, lung volume, and thickness of the abdominal muscles, and decrease the length of hospitalizations, mechanical and manual techniques, such as balloon blowing exercises and incentive spirometer, are required. PubMed, Google Scholar, Pedro, Clinical Keys, Helinet, ProQuest, and Science Direct databases were used for the literature search considering the inclusion and exclusion criteria. The several manual and mechanical methods that were employed for lung expansion treatment for the potential populations are highlighted in this overview of the literature. Ten studies were considered in this review: five on balloon-blowing exercises, four on tracheostomy with incentive spirometry exercises, and one on incentive spirometry with balloon-blowing exercises. The effects were examined on individual outcomes that included rate of perceived exertion (RPE), diaphragm mobility, pulmonary function, volume of breath, length of hospitalization, and postoperative day complications. The structured protocols proved to be effective in improving lung expansion and pulmonary function for the potential population that involved healthy adults, noncritical COVID-19 adults, smokers, thoracotomy patients, and tracheostomised patients. The road to recovery is yet unexplored and underachieved because of the lack of evidence.
Effects of inspiratory muscle training and breathing exercises in children with asthma: a systematic review

Abstract:Introduction: Asthma is characterized by a narrowing and inflammation of the bronchi, with symptoms of dyspnea, fatigue and exercise limitation. Physical therapy includes inspiratory muscle training and breathing exercises, given that an increase in inspiratory muscle strength and resistance can improve the symptoms of the disease. Objective: To describe the effects of inspiratory muscle training (IMT) and breathing exercises in children with asthma. Methods: This is a systematic review of the literature using the Cochrane, PubMed Scopus e Web of Science databases. The following descriptors were used: asthma, inspiratory muscle training, breathing exercises and child in Portuguese, English and Spanish. Two independent evaluators screened studies that used breathing exercises and IMT in children with asthma. Results: Of a total of 312 titles, eight studies were included, of which six are randomized clinical trials and two are observational studies All the studies included breathing exercises, with the objective of adjusting breathing patterns and pulmonary ventilation, reducing pulmonary hyperinflation, bronchospasm and sensation of dyspnea. However, as these exercises were not performed solely, the effects of this intervention could not be verified. Two studies performed IMT and showed an increase in maximal respiratory pressure. Conclusion: Breathing exercises are widely used in clinical practice as part of the management of asthma in children; however it is not possible to measure the effects in this population. IMT seems to improve inspiratory and expiratory muscle strength, but its indication in the pediatric population is not a standard procedure.
Effects of different types and frequencies of early rehabilitation on ventilator weaning among patients in intensive care units: A systematic review and meta-analysis

Abstract:This study aimed to investigate the effects of different types and frequencies of physiotherapy on ventilator weaning among patients in the intensive care unit (ICU) and to identify the optimal type and frequency of intervention. PubMed, Cochrane Library, EMBASE, and Airiti Library. Randomized controlled trials that provided information on the dosage of ICU rehabilitation and the parameters related to ventilator weaning were included. Treatment types were classified into conventional physical therapy, exercise-based physical therapy, neuromuscular electrical stimulation (NEMS), progressive mobility, and multi-component. The frequencies were divided into high (≥ 2 sessions/day or NEMS of > 60 minutes/day), moderate (one session/day, 3–7 days/week or NEMS of 30–60 minutes/day), and low (one session/day, < 3 days/week, or NEMS of < 30 minutes/day). Twenty-four articles were included for systematic review and 15 out of 24 articles were analyzed in the meta-analysis. Early rehabilitation, especially the progressive mobility treatment exerted an optimal effect in reducing the ventilator duration in patients in the ICU (standardized mean difference [SMD] = 0.91; 95% confidence interval [CI] = 0.23–1.58; P < 0.01). Regarding the treatment frequency, the high-frequency intervention did not result in a favorable effect on ventilator duration compared with the moderate frequency of treatment (SMD = 0.75; 95% CI = -1.13–2.64; P = 0.43). Early rehabilitation with progressive mobility is highly recommended to decrease the ventilation duration received by patients in the ICU. Depending on clinical resources and the tolerance of patients, the frequency of interventions should reach moderate-to-high frequency, that is, at least one session per day and 3 days a week. Effect of prophylactic non-invasive mechanical ventilation on functional capacity after heart valve replacement: a clinical trial

Abstract:Objective: During cardiac surgery, several factors contribute to the development of postoperative pulmonary complications. Non-invasive ventilation is a promising therapeutic tool for improving the functionality of this type of patient. The aim of this study is to evaluate the functional capacity and length of stay of patients in a nosocomial intensive care unit who underwent prophylactic non-invasive ventilation after heart valve replacement. Method: The study was a controlled clinical trial, comprising 50 individuals of both sexes who were allocated by randomization into two groups with 25 patients in each group: the control group and experimental group. After surgery, the patients were transferred to the intensive care unit and then participated in standard physical therapy, which was provided to the experimental group after 3 applications of non-invasive ventilation within the first 26 hours after extubation. For non-invasive ventilation, the positive pressure was 10 cm H2O, with a duration of 1 hour. The evaluation was performed on the 7th postoperative day/discharge and included a 6-minute walk test. The intensive care unit and hospitalization times were monitored in both groups. Brazilian Registry of Clinical Trials (REBeC): RBR number 8bxdd3. Results: Analysis of the 6-minute walk test showed that the control group walked an average distance of 264.34±76 meters and the experimental group walked an average distance of 334.07±71 meters (p=0.002). The intensive care unit and hospitalization times did not differ between the groups. Conclusion: Non-invasive ventilation as a therapeutic resource was effective toward improving functionality; however, non-invasive ventilation did not influence the intensive care unit or hospitalization times of the studied cardiac patients.

We extracted and integrated the context–mechanism–outcome strings of each study to form the theoretical framework.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Six theoretical mechanisms that affected the success of the intervention were articulated: Wide acceptance of training methods, Integration of the intervention with life, Self‐management of the participants, Confidence in controlling symptoms, Participation and support of practitioners, Motivation for intervention. Effectiveness of functional training on cardiorespiratory parameters: a systematic review and meta-analysis of randomized controlled trials

Abstract:Background: Functional training is a new training vision that was prepared from the gesture imitation of daily activities. Although your use has become popular in clinical practice, the influence of the several cardiorespiratory adjustments performed during the functional training in different populations and conditions is unknown. So, the aim of this systematic review was to gather information in the literature regarding the influence of functional training on cardiorespiratory parameters. Methods: We conducted search strategies on MEDLINE, PEDro, EMBASE, SportDiscus and Cochrane to identify randomized controlled trials investigating the effects of functional training on cardiorespiratory parameters. Methodological quality of the included studies was assessed using the PEDro scale. Grading of Recommendations Assessment, Development and Evaluation (GRADE) summarized the evidence. Results: Five original studies were included. Effects favoured functional training on oxygen consumption (VO2 ) at intermediate-term follow-up: weighted mean difference -1·0 (95% CI: 5·4-3·3), P = 0·642, and a small and not clinically important effect observed on VO2 favouring control at intermediate-term follow-up (i.e. mean difference of 1·30 (95% CI 1·07-1·53), P<0·001). Conclusion: According to the GRADE system, there is very low quality evidence that functional training is better than other interventions to improve cardiovascular parameters. This result encourages new searches about the theme.

Abstract:Background: Chronic Obstructive Pulmonary Disease (COPD) is a preventable and treatable lung inflammatory disease in which professionals can provide patients and caregivers with a specific treatment plan consisting of pharmacological and/or exercise therapy. COPD patients usually control exacerbations and maintain functional status through muscarinic antagonists, either long-acting (LAMA) or short-acting (SABA). One of the functional outcomes of a successful treatment is exercise tolerance, also called subjectively perceived effort during exercise, which is the target of Respiratory Rehabilitation provided by Rehabilitation Nurses. The relationship between pharmacological and nursing interventions in COPD is extremely important for the effectiveness of treatments. It is important to understand how they are associated, when they are used and what results can be obtained in disease management. Aim: Synthesize the relationship between rehabilitation nursing interventions and LAMA/SABA use in COPD management. Materials and Methods: Narrative review in MEDLINE, CINAHL and Cochrane databases, in which, through the screening process, a total of 15 studies were selected. Whittemore and Knafl’s method was used. Results and Conclusions: LAMA and LABA should be used in combination, but efficacy on exercise tolerance levels reduces if there’s no respiratory rehabilitation associated. Nurses should provide counseling and patient education to promote therapy adherence and motivate exercise therapy throughout treatment.
The Implementation of Inspiratory Muscle Training to Enhance Weaning from Mechanical Ventilation: A Systematic Literature Review

Abstract:Background: Mechanical ventilation is very important for the world of critical care. However, the use of mechanical ventilation also affected respiratory muscle weakness. Objective: The purpose of this study was to identify the implementation of Inspiratory muscle training for weaning mechanical ventilation in patients with mechanical ventilation in the Intensive Care Unit. Design: A systematic literature review study design was used. Data Sources: The databases used are CINAHL, PubMed, MEDLINE, Sage Journal, and ScienceDirect. Review Methods: Articles published from 2009-2020 with the type of randomized research-controlled trial (RCT) in English. The keywords "Inspiratory Muscle Exercise", "Inspiratory Muscle Training", "weaning", "mechanically ventilated", "critical care", and "intensive care". Results: From four articles analyzed, we found an effect of inspiratory muscle training on ventilator weaning, which was carried out on therapy five times a week, and some were carried out for seven days for two weeks and increased respiratory muscle strength. Conclusion: Inspiratory muscle training in mechanically ventilated patients is a nursing intervention that requires evidence-based preparation of guidelines to provide best practice. The optimalization of inspiratory muscles exercise can help the weaning process and be implemented further.

Abstract:Background: Inspiratory muscle training (IMT) is a simple and well-tolerated physical therapy that increases respiratory muscle strength and relieving the degree of dyspnea and fatigue. Therefore, it may be used as a transitional modality before exercise training or as a specific physical therapy intervention for those who are diagnosed with respiratory muscle weakness. However, the current evidence on IMT in pulmonary hypertension (PH) patients is inconclusive. The purpose of this systematic review and meta-analysis was to summarize the current role of IMT in this group of patients. Methods: PubMed, EMBASE, and Cochrane databases were searched through May 2022. Trials examining the feasibility and effectiveness of IMT in PH patients. Outcome measures included adverse events, training adherence and compliance, maximum inspiratory pressure (MIP), maximum expiratory pressure (MEP), forced vital capacity (FVC%), forced expiratory volume in 1 s (FEV 1 %), FEV 1 /FVC%, 6 min walk distance (6MWD), Peak VO 2 , dyspnea, and fatigue perception after the IMT training program. Only randomized controlled trials were included. The Cochrane Risk of Bias tool for controlled trials was adopted to assess study quality. Statistical heterogeneity was evaluated with the chi-square test and I 2 statistic. Mean differences and 95% confidence intervals (CIs) were estimated. Results: We ultimately identified four studies that met the criteria. These studies comprised 80 patients with 16 males and 64 females. The mean age was 53.25. The main types of PH were group I (pulmonary arterial hypertension, 95%) and group IV (chronic thromboembolic PH, 5%). No severe adverse events were reported in the included studies. IMT had a significant effect on improving MIP (18.89 cmH 2 O; 95% CI: 9.43–28.35, P < 0.001) and MEP (8.06 cmH 2 O; 95% CI: 2.39–13.73; P = 0.005), increase in the 6MWD (30.16 m; 95% CI: 1.53–58.79; P = 0.04). No significant improvement was found in pulmonary function ( P > 0.05), and uncertain effect on the quality of life (QoL) score. Conclusion: Based on currently limited evidence, IMT is an effective physical therapy for increasing respiratory muscle function and exercise capacity, but still a lack of evidence on dyspnea and fatigue levels, pulmonary function, and QoL in PH patients. There are reasons to believe that IMT is a promising intervention in PH patients, enriching rehabilitation options and serving as a bridge before formal exercise training. It is expected that IMT will play an important role in the future clinical pathway of physical therapy for this group of patients. Breathing techniques to reduce symptoms in people with serious respiratory illness: a systematic review

Abstract:Background In adults with serious respiratory illness, breathlessness is prevalent and associated with reduced health-related quality of life. The aim of this review was to assess the impact of breathing techniques on breathlessness in adults with serious respiratory illness. Methods Electronic databases were searched to identify randomised controlled trials testing breathing techniques (techniques that aim to alter the respiratory pattern, excluding respiratory muscle training) in people with serious respiratory illness. The primary outcome was breathlessness and secondary outcomes were health-related quality of life and adverse events. Two authors independently screened for inclusion, evaluated risk of bias and extracted data. Results 73 randomised controlled trials were included with 5479 participants, most with COPD or asthma. Breathing exercises (pursed lip and/or diaphragmatic breathing) reduced breathlessness measured by the modified Medical Research Council scale compared to usual care (mean difference (MD) –0.40 points, 95% CI –0.70– –0.11, eight studies, n=323), although the effect did not exceed the minimal important difference. Yoga breathing also improved modified Medical Research Council score compared to usual care (MD –1.05 points, 95% CI –2.45–0.35, three studies, n=175). Breathing techniques consistently improved health-related quality of life in people with COPD and asthma on multiple health-related quality of life measures in comparison to usual care, with effects that generally exceeded the minimal important difference. No adverse events related to breathing techniques were reported. Conclusion Breathing techniques may improve breathlessness, and consistently improve health-related quality of life, in people with serious respiratory illness. These findings support the use of breathing exercises in the care of people with serious respiratory illness.
