Abstract:R ed blood cells (RBCs) are nature’s long-circulating delivery vehicles. They possess various remarkable properties and continue to inspire the design and engineering of man-made delivery systems. Inherently suited for intravascular delivery, RBCs are intrinsically biocompatible, biodegradable, and nonimmunogenic. They form natural compartments capable of protecting encapsulated cargos, and this allows them to circulate in the bloodstream for a long period of time (up to 120 days). In addition, their semipermeable membrane affords sustained release to smallmolecule drugs, yet they are ideal for retaining large proteins while providing them access to the substrates. Delivery vehicles possessing one or several of these properties have long been desired for efficacious therapeutics. A long-sought strategy for RBC-mimicking delivery vehicles is to load natural RBCs with therapeutic agents without compromising the structural integrity and biological functions of the RBCs. Various loading techniques, including automated loading devices, have been developed to enable the encapsulation of payloads with molecular weights of over 180 kDa into RBCs with maintenance of the carriers’ biological competence. In addition to the interior of RBCs, their exterior surface has also been coupled with therapeutic molecules, either covalently or physically, for various delivery applications. These RBC-based delivery vehicles, namely carrier RBCs, have been developed for the delivery of numerous therapeutic agents, including proteins, nucleic acids, and small-molecule drugs. Several of them have entered clinical tests to treat various diseases, including cancers and enzyme deficiencies. Meanwhile, advances in molecular biology have provided unprecedented understanding of the connections between the physicochemical characteristics of RBCs and their biological functions. This understanding, in turn, has inspired researchers to model drug carriers after RBCs. Designs that mimic the physicochemical characteristics and biological functions of RBCs, particularly those that enable their passage through narrow constrictions while maintaining a long in vivo survival, have been integrated into the engineering of drug carriers and have resulted in novel delivery systems with improved drug tolerability, circulation lifetime, and efficacy. Recently, the pursuit of RBC-mimicking nanoparticles led us to develop an intriguing approach for functionalizing synthetic nanoparticles with natural RBC membranes. In this approach, we first collected intact cellular membranes and then coated them onto polymeric nanoparticle cores, such as those made from poly(lactic-co-glycolic acid) (PLGA); this resulted in red-blood-cell-membrane–coated nanoparticles (RBC-NPs; Figure 1). Our aim was to fabricate cellmimicking nanoparticles through a top-down approach that bypassed the labor-intensive processes of protein identification, purification, and conjugation. The natural membranes also provide a bilayered medium for transmembrane protein anchorage while preventing common chemical modifications that could compromise the integrity and functionalities of these proteins. The independent preparation of cellular membranes and particle cores before coating offers a new level of engineering flexibility toward highly functional biomimetic nanoparticles. Since their initial development, RBC-NPs have provided an unprecedented capability for harnessing the natural functionalities of native cells that would otherwise be difficult to replicate. They have since inspired us to develop novel nanotherapeutics for better disease intervention. In this Perspective, we review our recent progress in developing RBC-NPs for three distinct biomedical applications: long-circulating nanocarriers for drug delivery, biomimetic nanosponges for detoxification, and nanotoxoids for safe and effective toxin vaccination.
Assessment of Children With Suspected Auditory Processing Disorder: A Factor Analysis Study

Abstract:Objectives: To identify the factors that may underlie the deficits in children with listening difficulties, despite normal pure-tone audiograms. These children may have auditory processing disorder (APD), but there is no universally agreed consensus as to what constitutes APD. The authors therefore refer to these children as children with suspected APD (susAPD) and aim to clarify the role of attention, cognition, memory, sensorimotor processing speed, speech, and nonspeech auditory processing in susAPD. It was expected that a factor analysis would show how nonauditory and supramodal factors relate to auditory behavioral measures in such children with susAPD. This would facilitate greater understanding of the nature of listening difficulties, thus further helping with characterizing APD and designing multimodal test batteries to diagnose APD. Design: Factor analysis of outcomes from 110 children (68 male, 42 female; aged 6 to 11 years) with susAPD on a widely used clinical test battery (SCAN-C) and a research test battery (MRC Institute of Hearing Research Multi-center Auditory Processing “IMAP”), that have age-based normative data. The IMAP included backward masking, simultaneous masking, frequency discrimination, nonverbal intelligence, working memory, reading, alerting attention and motor reaction times to auditory and visual stimuli. SCAN-C included monaural low-redundancy speech (auditory closure and speech in noise) and dichotic listening tests (competing words and competing sentences) that assess divided auditory attention and hence executive attention. Results: Three factors were extracted: “general auditory processing,” “working memory and executive attention,” and “processing speed and alerting attention.” Frequency discrimination, backward masking, simultaneous masking, and monaural low-redundancy speech tests represented the “general auditory processing” factor. Dichotic listening and the IMAP cognitive tests (apart from nonverbal intelligence) were represented in the “working memory and executive attention” factor. Motor response times to cued and noncued auditory and visual stimuli were grouped in the “processing speed and alerting attention” factor. Individuals varied in their outcomes in different tests. Poor performance was noted in different combinations of tests from the three factors. Impairments solely related to the “general auditory processing” factor were not common. Conclusions: The study identifies a general auditory processing factor in addition to two other cognitive factors, “working memory and executive attention” and “processing speed and alerting attention,” to underlie the deficits in children with susAPD. Impaired attention, memory, and processing speed are known to be associated with poor literacy and numeracy skills as well as a number of neurodevelopmental disorders. Individuals with impairments in the “general auditory processing” tests along with tests from the other two cognitive factors may explain the co-occurrence of APD and other disorders. The variation in performance by individuals in the different tests noted was probably due to a number of reasons including heterogeneity in susAPD and less-than ideal test–retest reliabilities of the tests used to assess APD. Further research is indicated to explore additional factors, and consensus is needed to improve the reliability of tests or find alternative approaches to diagnose APD, based on the underlying factors.
RGD Modified Protein-Polymer Conjugates for Ph-Triggered Targeted Thrombolysis

Abstract:It holds great promise to develop thrombolytic agent delivery systems with prolonged circulation time, minimized adverse effect, and preferential thrombolytic activity at the thrombus site. In this work, a pH-triggered delivery system for the thrombolytic agent was synthesized. Fluorescein isothiocyanate-labeled urokinase-type plasminogen activator (uPA) was conjugated to oxidized dextran (Oxd) via the pH-sensitive imine linkage, and then the conjugation was modified with RGD peptide. The uPA-Oxd conjugates displayed enhanced stability to resist enzymatic hydrolysis in comparison with the native uPA. Meanwhile, the bioactivity of uPA in the uPA-Oxd conjugates was masked at physiological pH and regenerated under weak acidic condition due to the release of uPA via the hydrolysis of imine bond in the conjugates. RGD modification endowed the conjugates with targeting ability to the thrombus site due to the specific peptide-GP IIb/IIIa binding and enhanced the thrombolysis efficacy via an endogenous low-pH triggered uPA release at the thrombus site, reducing the risk of acute hemorrhage complication. Thus, the RGD modified pH-sensitive uPA-Oxd conjugates provide a promising potential of the system in local thrombolysis therapy.
Platelet membrane-functionalized nanoparticles with improved targeting ability and lower hemorrhagic risk for thrombolysis therapy

Abstract:Intravenous injection of thrombolytic drugs is the most effective strategy for the treatment of thrombotic diseases. However, the clinical application of most thrombolytic drugs is limited by hemorrhagic risks and narrow therapeutic index. The targeted drug delivery systems may help to address these problems. Inspired by the crucial role of platelets in the process of thrombus, Platelet membrane-coated PLGA cores loading lumbrokinase (PNPs/LBK) were designed for effective thrombolysis with reduced hemorrhagic risk. Using a mouse carotid thrombosis model, the affinity of platelet membrane-coated nanoparticles to the thrombus was confirmed. Also, the PNPs/LBK exhibited excellent thrombolytic efficacy at a low dose, compared with free LBK. More importantly, PNPs/LBK showed less adverse effect on the function of the coagulation system, and thus reduced hemorrhagic risk. These results indicated that a promising thrombus-targeted drug delivery system was achieved by coating PLGA nanoparticles with platelet membrane. Such rationally designed drug delivery system will provide a broad platform for thrombus treatment.

Most thrombolytic effects of traditional treatments or nanomedicine are not efficient or safe enough. Therefore, we designed a nanoparticle (NP) with a combination of a phase transition material and thrombolytic drugs for efficient and safe thrombolysis. Methods: A thrombus fibrin-targeted and phase transition NP was designed and contained perfluorohexane (PFH) and the thrombolytic drug rtPA core, with CREKA polypeptides attached to the shell of the PLGA NPs. Characterization of the phase transition and ultrasound imaging of the NPs was carried out under low-intensity focused ultrasound (LIFU). LIFU-responsive drug release in vitro was also explored. Under the synergistic effect of PFH and rtPA, the efficient thrombolysis ability of the NPs was studied in vitro and in vivo. In vivo monitoring of thrombosis and biosafety were also verified. Results: The PPrC NPs had good ultrasound imaging ability under LIFU irradiation and were related to the phase transition characteristics of the NPs. CREKA polypeptides can effectively increase the aggregation of the NPs on thrombi. Under static and dynamic conditions in vitro, the "liquid to gas" transformation effect of PFH can perform the destruction function of the excavator at the thrombus site and promote the specific release of rtPA, and the subsequent rtPA drug thrombolysis can further fully dissolve the thrombus. In vivo experiments showed that the NPs can monitor the formation of thrombi and have good thrombolytic effects, with significantly reduced bleeding side effects. The biochemical indexes of the rats were within normal limits after treatment. Conclusion: PPrC NPs loaded with PFH and rtPA combining a mechanical way of blasting with thrombolytic drugs may be a promising new and reliable approach for thrombus monitoring and treatment. Abstract:Thrombolytic agents have thus far yielded limited therapeutic benefits in the treatment of thrombotic disease due to their short half-life, low targeting ability, and association with serious adverse reactions, such as bleeding complications. Inspired by the natural roles of platelets during thrombus formation, we fabricated a platelet-based delivery system (NO@uPA/PLTs) comprising urokinase (uPA) and arginine (Arg) for targeted thrombolysis and inhibition of re-embolism. The anchoring of uPA to the platelet surface by lipid insertion increased the thrombotic targeting and in vivo circulation duration of uPA without disturbing platelet functions. Nitric oxide (NO) generated by the loaded Arg inhibited platelet aggregation and activation at the damaged blood vessel, thereby inhibiting re-embolism. NO@uPA/PLTs effectively accumulated at the thrombi in pulmonary embolism and carotid artery thrombosis model mice and exerted superior thrombolytic efficacy. In addition, the platelet delivery system showed excellent thrombus recurrence prevention ability in a mouse model of secondary carotid artery injury. The coagulation indicators in vivo showed that the platelet-based uPA and NO co-delivery system possessed a low hemorrhagic risk, providing a promising tool for rapid thrombolysis and efficient inhibition of posttreatment re-embolism.

Bioresponsive Polyphenol-Based Nanoparticles as Thrombolytic Drug Carriers

Abstract:Thrombolytic (clot-busting) therapies with plasminogen activators (PAs) are first-line treatments against acute thrombosis and ischemic stroke. However, limitations such as narrow therapeutic windows, low success rates, and bleeding complications hinder their clinical use. Drug-loaded polyphenol-based nanoparticles (NPs) could address these shortfalls by delivering a more targeted and safer thrombolysis, coupled with advantages such as improved biocompatibility and higher stability in vivo. Herein, a template-mediated polyphenol-based supramolecular assembly strategy is used to prepare nanocarriers of thrombolytic drugs. A thrombin-dependent drug release mechanism is integrated using tannic acid (TA) to cross-link urokinase-type PA (uPA) and a thrombin-cleavable peptide on a sacrificial mesoporous silica template via noncovalent interactions. Following drug loading and template removal, the resulting NPs retain active uPA and demonstrate enhanced plasminogen activation in the presence of thrombin (1.14-fold; p < 0.05). Additionally, they display lower association with macrophage (RAW 264.7) and monocytic (THP-1) cell lines (43 and 7% reduction, respectively), reduced hepatic accumulation, and delayed blood clearance in vivo (90% clearance at 60 min vs 5 min) compared with the template-containing NPs. Our thrombin-responsive, polyphenol-based NPs represent a promising platform for advanced drug delivery applications, with potential to improve thrombolytic therapies.

Drug delivery under cover of erythrocytes extends drug half-life: A thrombolytic targeting therapy utilizing microenvironment-responsive artificial polysaccharide microvesicles

Abstract:The development of thrombolytic drug carriers capable of thrombus-targeting, prolonged circulation time, intelligent responsive release, and the ability to inhibit thrombotic recurrences remains a promising but significant challenge. To tackle this, an artificial polysaccharide microvesicle drug delivery system (uPA-CS/HS@RGD-ODE) was constructed. It is composed of cationic chitosan and anionic heparin assembled in a layer by layer structure, followed by surface modification using RGD peptide and 2-(N-oxide-N,N-diethylamino) ethylmethacrylate (ODE) before encapsulation of urokinase-type plasminogen activator (uPA). The effect of chitosan on the basic performances of uPA-CS/HS@RGD-ODE was estimated. The in vitro results suggest the uPA carrier, CS/HS@RGD-ODE, displayed outstanding targeting specific to activated platelets (61 %) and microenvironment-responsiveness at pH 6.5, facilitating thrombus-targeting and a controlled drug release, respectively. Most importantly, in vivo experiment suggests ODE from uPA-CS/HS@RGD-ODE substantially extends the half-life of uPA (120 min), as uPA-CS/HS@RGD-ODE can adhere onto erythrocytes and deliver uPA under cover of erythrocytes enabling a prolonged circulation time in the bloodstream. Further tail vein and abdominal aorta thrombosis models confirmed uPA-CS/HS@RGD-ODE exhibited superior targeting and thrombolysis capabilities compared to systemic administration of free uPA. To the knowledge of authors, this may be the first study to develop new drug carriers for delivery of thrombolytic drugs under the cover of erythrocytes for extended drug half-lives.
Physical Activity Patterns in Recent College Alumni

Abstract:Within this sample of recent college alumni, a high proportion indicated that they enjoy exercise (66.1%) and felt confident in setting their own fitness programs (79.4%). Yet, only 34.3% reported engaging in recommended levels of moderate or vigorous PA. In contrast to previous reports, rates of participation in vigorous, moderate, and strengthening-type PA did not differ between men and women. Alumni who reported that they engaged in adequate levels of moderate or vigorous PA had gained significantly less weight in the years following graduation than those who reported being insufficiently active (3.0 kg vs. 6.1 kg). Last, we noted a strong positive association between PA patterns as college seniors and current PA behavior as recent alumni.
Recombinant Tissue Plasminogen Activator-conjugated Nanoparticles Effectively Targets Thrombolysis in a Rat Model of Middle Cerebral Artery Occlusion

Abstract:The efficacy and safety of recombinant tissue plasminogen activator (rtPA) need to be improved due to its low bioavailability and requirement of large dose administration. The purpose of this study was to develop a fibrin-targeted nanoparticle (NP) drug delivery system for thrombosis combination therapy. We conjugated rtPA to poly(ethylene glycol)- poly(e-caprolactone) (PEG-PCL) nanoparticles (rtPA-NP) and investigated its physicochemical characteristics such as particle size, zeta potential, enzyme activity of conjugated rtPA and its storage stability at 4°C. The thrombolytic activity of rtPA-NP was evaluated in vitro and in vivo as well as the half-life of rtPA-NP, the properties to fibrin targeting and its influences on systemic hemostasis in vivo. The results showed that rtPA-NP equivalent to 10% of a typical dose of rtPA could dissolve fibrin clots and were demonstrated to have a neuroprotective effect after focal cerebral ischemia as evidenced by decreased infarct volume and improved neurological deficit (P<0.001). RtPA-NP did not influence the in vivo hemostasis or coagulation system. The half-life of conjugated rtPA was shown to be approximately 18 times longer than that of free rtPA. These experiments suggested that rtPA-conjugated PEG-PCL nanoparticles might be a promising fibrin-targeted delivery system for a combination treatment of thrombosis.

Abstract:Thrombosis is a principle cause of various life-threatening cardiovascular diseases. However, current antithrombotic treatments using drugs only offer limited efficacy due to short half-life, low targeting ability to the thrombus site, and unexpected bleeding complications. Taking into account of the biological characteristics of thrombus including upregulation of hydrogen peroxide (H2O2) and abundance of fibrin, we engineered a H2O2-responsive nanocarrier for thrombus-targeting delivery of an antithrombotic agent (i.e., tirofiban). The nanocarrier was composed of a drug-conjugated dextran nanocore and a red blood cell (RBC) membrane shell, and its surface was functionalized with a fibrin-targeting peptide, CREKA. Tirofiban was conjugated to dextran through a H2O2-cleavable phenylboronic ester linkage. The fibrin-targeting RBC membrane-cloaked dextran-tirofiban conjugate nanoparticles (i.e., T-RBC-DTC NPs) can scavenge H2O2 and provide controlled release of tirofiban to achieve site-specific antithrombotic effects. In RAW 264.7 cells and HUVECs, the T-RBC-DTC NPs effectively scavenged H2O2 and protected cells from H2O2-induced cytotoxicity. In the ferric chloride-induced carotid thrombosis mouse model, the T-RBC-DTC NPs efficiently accumulated at the injured carotid artery and exhibited significantly enhanced antithrombotic activity compared to free drug. The T-RBC-DTC NPs also exhibited good biocompatibility according to histology analysis. Overall, our results indicated that this bioengineered nanocarrier offers a promising therapeutic strategy for thrombotic disorders.

Abstract:A thrombus‐targeting peptide derived from phage display is conjugated onto silica‐coated bismuth‐cored nanoparticles, which are then loaded with a chemotherapeutic drug. Under the guidance of the peptide, they can selectively target and bind to the thrombus. They effectively combat the thrombus by photothermal and chemo‐therapy. Moreover, they enhance the contrast in computed tomography imaging, facilitating the detection of the thrombus. Thrombotic disease poses a significant threat to human health as it blocks blood vessels and leads to severe symptoms. Effective treatment requires targeted therapy and precise localization of the thrombus, but traditional drugs are limited in their targeting ability and ability to locate the thrombus. To overcome these issues, a nanoparticle capable of both thrombus targeting and computed tomography (CT) imaging is developed. Phage display technology is used to screen for the thrombus‐targeting peptide termed GK, which is then linked to the surface of macroporous silica (Mp‐SiO2) with a Bi core to create Mp‐Bi@SiO2‐GK. The large pores of Mp‐Bi@SiO2‐GK enable the transport of the drug Urokinase (UK), while the Bi core provides the capabilities of CT imaging and photothermal therapy. The Mp‐Bi@SiO2‐GK nanoparticles precisely target thrombi in mouse carotid arteries and locate them via CT imaging. Furthermore, the combination of Bi‐enabled photothermal therapy and UK‐induced chemotherapy enhance the thrombolysis efficiency. Treatment with Mp‐Bi@SiO2‐GK nanoparticles do not harm tissues/organs or affect liver/kidney metabolism. These results show that Mp‐Bi@SiO2‐GK exhibits precise thrombus targeting and efficient imaging/treatment capability, making it a promising tool for the diagnosis and treatment of thrombosis.

The Inter-Rater Reliability and Diagnostic Accuracy of Patellar Mobility Tests in Patients with Anterior Knee Pain

Abstract:Abstract Background: Patients with patellofemoral pain syndrome (PFPS) often have nonspecific findings on clinical examination. A tight knee and diminished patellar mobility in particular, may contribute to anterior knee pain. Methods: Qualifying patients with anterior knee pain of > 4 weeks were sequentially examined by 2 physicians who used the patellar mobility tests. The tests evaluated for diminished patellar translation superior-inferiorly and medial-laterally, diminished patellar tendon mobility, and absent inferior pole tilt. Each of the 4 tests was scored as 1 (diminished/absent) or 0 (nondiminished/normal), with the sum of the scores determining the patellar mobility scale. Inter-rater reliability and diagnostic accuracy were determined and analyzed. Receiver operating characteristic analysis was used to determine the capacity of the patellar mobility scale to predict PFPS. Results: 98 patients participated in the study. The inter-rater reliability for the 4 individual patellar mobility tests was moderately strong, with diminished medial-lateral patellar mobility demonstrating the strongest reliability (Kappa value [K], 0.59; 95% confidence interval [CI], 0.42−0.72), followed by diminished superior-inferior mobility (K), 0.55; 95% CI, −0.37 to 0.69), inferior pole tilt (K, 0.48; 95% CI, −0.28 to 0.61), and patellar tendon mobility (K, 0.45; 95% CI, −0.27 to 0.56). The diagnostic accuracy of the individual patellar mobility tests in diagnosing PFPS was fair to moderate, with the individual diminished patella tendon mobility test having the highest level of diagnostic accuracy (sensitivity, 49%; specificity, 83%; likelihood ratio [LR] ± 2.8; 95% CI, 1.3−7.3) and also having the highest positive predictive value (0.88). Diminished medial-lateral mobility was second in diagnostic accuracy (sensitivity, 54%; specificity, 69%; LR ± 1.8; 95% CI, 0.9−3.6). Receiver operating characteristic analysis showed only moderate levels of diagnostic capacity (area under the curve, 0.65) because combining all 4 patellar mobility tests to form the patellar mobility scale did not improve the diagnostic accuracy. Conclusion: The clinical importance of diminished patellar and/or patellar tendon mobility findings in the evaluation of patients with anterior knee pain remains unclear. We determined that individual patellar mobility tests had moderate levels of inter-rater reliability, the best agreement found with medial-lateral mobility. The accuracy of the patellar mobility scale for diagnosing PFPS was marginal. As a tool, this cluster of examination techniques may be used by physicians to better understand their patients' conditions and descriptively communicate their findings. However, the patellar mobility scale cannot be used alone for determining the diagnosis of PFPS, as defined in our study. Future studies aim to determine whether post-treatment symptomatic improvements correlate with an improvement on the patellar mobility scale.
Platelet Membrane Biomimetic Magnetic Nanocarriers for Targeted Delivery and in Situ Generation of Nitric Oxide in Early Ischemic Stroke.

Abstract:Early diagnosis and treatment of acute ischemic stroke poses a significant challenge due to its suddenness and short therapeutic time window. Human endogenous cells derived biomimetic drug carriers have provided new options for stroke theranostics since these cells have higher biosafety and targeting abilities than artificial carriers. Inspired by natural platelets (PLTs) and their role in targeting adhesion to the damaged blood vessel during thrombus formation, we fabricated a biomimetic nanocarrier comprising a PLT membrane envelope loaded with l-arginine and γ-Fe2O3 magnetic nanoparticles (PAMNs) for thrombus-targeted delivery of l-arginine and in situ generation of nitric oxide (NO). Results demonstrate that the engineered 200 nm PAMNs inherit the natural properties of the PLT membrane and achieve rapid targeting to ischemic stroke lesions under the guidance of an external magnetic field. Subsequent to the release of l-arginine at the thrombus site, endothelial cells produce NO, which promotes vasodilation to disrupt the local PLT aggregation. Rapid targeting of PAMNs to stroke lesions as well as in situ generation of NO prompts vasodilation, recovery of blood flow, and reperfusion of the stroke microvascular. Thus, these PLT membrane derived nanocarriers are diagnostically beneficial for localizing stroke lesions and a promising modality for executing therapies.

Abstract:In oral and maxillofacial surgery, flap repair is essential to the quality of postoperative life. Still, thrombosis is fatal for the survival of the flaps. Besides, some postoperative thrombotic diseases, such as pulmonary embolism, also intimidate patients' life. The traditional diagnostic methods are still limited by a large amount of hardware and suffer from inconvenience, delay, and subjectivity. Moreover, the treatments mainly rely upon thrombolytics, such as urokinase (UK) plasminogen activator, which may cause bleeding risk, especially intracerebral hemorrhage. Herein, a kind of poly (lactic-co-glycolic acid) (PLGA) nanoparticles (NPs) containing a first near-infrared window (NIR-I) phototheranostic agent Y8 and urokinase plasminogen activator (UK) as the core, and modified with the fibrin-targeting peptide Gly–Pro–Arg–Pro–Pro (GPRPP) were developed for the flap and postoperative thromboembolism treatment (named GPRPP-Y8U@P). The conjugated molecule Y8 endows GPRPP-Y8U@P with the capacity of NIR-II imaging and excellent photothermal/photodynamic therapeutic effects. In vivo experiments demonstrated that GPRPP-Y8U@P could quickly locate thrombus by NIR-II fluorescence imaging, and semi-quantitative analysis of the embolized blood vessels' paraffin section verified its thrombolytic efficiency. Additionally, the urokinase trapped in the NPs would not result in nonspecific bleeding, tremendously improving physical security and curative effects with minimizing side effects. Overall, the advantages of GPRPP-Y8U@P, such as precise localization of the thrombus, thrombus ablation in the site, and mild side effects, demonstrated the attractiveness of this approach for effective clinical monitoring of thrombus therapy.

Abstract:Ischemic stroke caused by a thrombus clog and ischemia is one of the most lethal and disabling cerebrovascular diseases. A sequentially targeted delivery system is highly desired to deliver thrombolytics and neuroprotectant to the site of the thrombus and ischemic penumbra, respectively, to pursue a maximized combinational effect. Inspired by the vital roles that platelets play in thrombus formation, herein, we develop a bioengineered "nanoplatelet" (tP-NP-rtPA/ZL006e) for sequentially site-specific delivery of recombinant tissue plasminogen activator (rtPA) and neuroprotectant (ZL006e) for ischemic stroke treatment. The tP-NP-rtPA/ZL006e consists of a ZL006e-loaded dextran derivative polymeric nanoparticle core and platelet membrane shell conjugated with thrombin-cleavable Tat-peptide-coupled rtPA. Mediated by the cloak of the platelet membrane, tP-NP-rtPA/ZL006e targets the thrombus site and rtPA is triggered to release by the upregulated thrombin. Subsequently, the in situ exposed Tat peptide enhanced penetration of the "nanoplatelet" across the blood-brain barrier into ischemic brain for ZL006e site-specific delivery. From the in vitro and in vivo evaluation, tP-NP-rtPA/ZL006e is demonstrated to significantly enhance the anti-ischemic stroke efficacy in the rat model with middle cerebral artery occlusion, showing a 63 and 72% decrease in ischemic area and reactive oxygen species level compared to that with free drug combination, respectively.

Abstract:Intravenous injection of thrombolytic drugs is the most feasible strategy for the clinical treatment of thrombotic diseases, however, its applications are yet limited by narrow therapeutic index, short half-life, and hemorrhagic risk. Inspired by the excessive ROS and inflammatory factors at the thrombus site, we manufactured a thrombus microenvironment-modulated nanosystem , which achieved the targeted delivery of thrombolytic drugs and ROS scavenging agents. Ir monomers (Ir-PVP) with enzyme-mimetic activity were prepared and then wrapped with lumbrokinase (LK) by HA materials of thrombo-inflammatory affinity, forming multifunctional thrombolytic agents could prolong the biological half-life of LK and target the thrombus site to release LK with reduced bleeding risk. Moreover, Ir-NPs, as both CT imaging contrast agent and ROS scavenger, not only allowed us to observe the progression of thrombolytic treatment through CT imaging in real time, but also played an important role in elimination of excessive ROS at thrombus site. Taken together, the in vitro and in vivo experiments illustrated that with CT function displayed an excellent efficiency in thrombolysis, anti-inflammation and ROS scavenging. Our work may encourage further exploration of thrombus microenvironment-modulated theranostic platforms.

Abstract:Thrombotic diseases cause significant risks of disability and death worldwide. However, conventional thrombolytic drugs have short half-lives, poor targeting capabilities, limited therapeutic effects, and narrow therapeutic windows. Moreover, most basic studies focus only on thrombolytic efficiency and ignore the inflammatory factors that promote thrombosis progression, such as reactive oxygen species (ROS)-mediated oxidative stress. Therefore, to achieve comprehensive and effective thrombolytic management, both targeted thrombolytic therapy and the elimination of inflammatory factors must be achieved. In this study, we developed a platelet membrane (PM)-functionalized hollow mesoporous Prussian blue nanomedicine (HMPB-rtPA@PM), which was innovatively coassembled from two FDA-approved drugs (Prussian blue (PB) and alteplase (rtPA)) and showed marked advantages in both thrombi targeting and photothermal/pharmacological synergistic fibrinolysis. Moreover, HMPB-rtPA@PM exhibited the potential to inhibit platelet aggregation and regulate the inflammatory microenvironment by scavenging ROS. As expected, this nanomedicine not only achieved more efficient thrombolysis in both thrombus models but also delayed thrombosis progression through therapeutic visualization in the black tail model. Such rational therapeutic principle provides a broad platform for the comprehensive management of thrombotic diseases.

Abstract:It is of great significance to find effective thrombolytic treatments due to the harm caused by thrombosis to human health. Based on the formation mechanism and complex microenvironment of a thrombus, polydopamine nanomotors (PDANMs) modified by the peptide of Arg-Gly-Asp (RGD) and loaded with urokinase (UK) were designed and prepared. A polydopamine (PDA) substrate has a good photothermal conversion effect. Under near-infrared (NIR) light irradiation, it can not only perform photothermal therapy (PTT) on thrombus, but also provide the driving force of PDANMs. Thrombolytic drug UK was loaded in the mesoporous structure of the PDA substrate and can be released at the thrombus site for drug therapy. The modified RGD can target the thrombus site, moreover, benefiting from the guanidine group of L-arginine in the peptide chain, and RGD can interact with reactive oxygen species (ROS) in the thrombus microenvironment to produce nitric oxide (NO). NO not only propelled the movement of nanomotors, but also promoted the growth of vascular endothelial cells to repair damaged blood vessels. The experimental results show that NIR and NO can provide dual driving sources for the nanosystem to achieve continuous and deep penetration of the drug-loaded nanomotors at the thrombus site, while realizing the photothermal and drug synergistic therapy to enhance the therapeutic effect and promote the growth of vascular endothelium cells. This kind of thrombus treatment strategy based on nanomotor drug delivery systems will provide good technical support for the clinical treatment of inferior vena cava thrombus.

Abstract:Global public health is seriously threatened by thrombotic disorders because of their high rates of mortality and disability. Most thrombolytic agents, especially protein-based pharmaceuticals, have a short half-life in circulation, reducing their effectiveness in thrombolysis. The creation of an intelligent drug delivery system that delivers medication precisely and releases it under regulated conditions at nearby thrombus sites is essential for effective thrombolysis. In this article, we present a unique medication delivery system (MCRUA) that selectively targets platelets and releases drugs by stimulation from the thrombus' microenvironment. The thrombolytic enzyme urokinase-type plasminogen-activator (uPA) and the anti-inflammatory medication Aspirin (acetylsalicylic acid, ASA) are both loaded onto pH-sensitive CaCO 3 /cyclodextrin crosslinking metal-organic frameworks (MC) that make up the MCRUA system. c(RGD) is functionalized on the surface of MC, which is functionalized by RGD to an esterification reaction. Additionally, the thrombus site's acidic microenvironment causes MCRUA to disintegrate to release uPA for thrombolysis and aiding in vessel recanalization. Moreover, cyclodextrin-encapsulated ASA enables the treatment of the inflammatory environment within the thrombus, enhancing the antiplatelet aggregation effects and promoting cooperative thrombolysis therapy. When used for thrombotic disorders, our drug delivery system (MCRUA) promotes thrombolysis, suppresses rethrombosis, and enhances biosafety with fewer hemorrhagic side effects.
