Abstract:Identifying arrhythmias from electrocardiogram(ECG) signals remains an intractable challenge. This study aims to develop an effective and non-invasive approach to realize the recognition of arrhythmias based on 12-lead ECG for the PhysioNet/Computing in Cardiology Challenge2020. To this end, we propose a deep learning-based diagnosis approach, called EASTNet which captures the characteristics of cardiac abnormalities and correlation between heartbeats sampled randomly from 12-lead ECG records by a 34-layer 1D-deep squeeze-and-excitation network. Experimenting in the multi-label arrhythmia classification task, our team, EASTBLUE, was unable to rank and score in the hidden validation and test sets, but achieved diagnostic performance with 0.7030 ± 0.0090 metric score using 5-fold cross-validation on the training set. We also investigate the effect of beat sampling on diagnostic performance, and find that the beat sampling plays a role in data augmentation that effectively alleviates network overfitting. These results demonstrate that our approach has good potential application prospects in clinical practice, especially in the auxiliary diagnosis of abnormalities.
Automated multilabel diagnosis on electrocardiographic images and signals

Abstract:The application of artificial intelligence (AI) for automated diagnosis of electrocardiograms (ECGs) can improve care in remote settings but is limited by the reliance on infrequently available signal-based data. We report the development of a multilabel automated diagnosis model for electrocardiographic images, more suitable for broader use. A total of 2,228,236 12-lead ECGs signals from 811 municipalities in Brazil are transformed to ECG images in varying lead conformations to train a convolutional neural network (CNN) identifying 6 physician-defined clinical labels spanning rhythm and conduction disorders, and a hidden label for gender. The image-based model performs well on a distinct test set validated by at least two cardiologists (average AUROC 0.99, AUPRC 0.86), an external validation set of 21,785 ECGs from Germany (average AUROC 0.97, AUPRC 0.73), and printed ECGs, with performance superior to signal-based models, and learning clinically relevant cues based on Grad-CAM. The model allows the application of AI to ECGs across broad settings.

Abstract:Even before Medawar and colleagues published their seminal description of long-term skin transplant survival in mice in the early 1950s, the field of transplantation was fascinated by the concept of tolerance. Following this pioneering work, many thought that clinical tolerance was just around the corner. However, the first report of clinical tolerance was not published until 1975. It took another 10-20 years before advances in immunology suggested approaches that could be exploited to induce tolerance in humans. These advances included protocols in rodents that induced a state of mixed hematopoietic chimerism and subsequent tolerance to transplanted organs and the description of T cells with suppressive or regulatory properties. In this issue of HEPATOLOGY, Todo et al. report 10 patients undergoing living donor liver transplantation who received an infusion of an ex vivo-generated cell product enriched for regulatory cells with the aim of inducing tolerance. Tapering of immunosuppression began at 6 months, with complete withdrawal achieved by 18 months. This was successful in 7 of the 10 patients, with the three failures occurring in individuals with autoimmune liver diseases. At the time of this report, the duration of tolerance ranged from 16 to 33 months. Importantly, the cell infusion was not associated with any adverse events, and all treated patients—including the 3 who failed weaning of immunosuppressive drugs—have normal graft function without histologic evidence of injury. So why is the Todo et al. study significant? Despite over two decades of work aimed at characterizing regulatory cells, determining how they mediate their suppressive effects, and developing protocols to expand them, this study is the first to demonstrate that they can be exploited to induce tolerance after organ transplantation. This is also the first study to intentionally induce tolerance in patients undergoing liver transplantation. How does this work compare with and advance previous studies of tolerance in liver transplant recipients? Previous studies have capitalized on the relatively high incidence of spontaneous tolerance after liver transplantation to permit the gradual withdrawal of immunosuppression in liver transplant recipients initially treated with conventional immunosuppressive regimens. These studies have found that the success of weaning recipients from immunosuppressive drugs was strongly correlated with increasing time since transplantation. The advantage of the approach described by Todo et al. is that it allows for earlier withdrawal of the drugs, thereby saving patients years of the morbidities associated with immunosuppression. Several groups have reported strategies that induce tolerance to transplanted kidneys with significant success. Each of these protocols use the infusion of various populations of donor cells in the setting of recipient conditioning consisting of irradiation and nonmyeloablative chemotherapeutic agents to induce variable states of chimerism. It remains to be determined whether these differences between the strategies for inducing tolerance following liver and kidney transplantation reflect fundamental organ-specific differences in the mechanisms responsible for the development and maintenance of tolerance or are artifacts related to differences in the patient populations studied, study design, and the choices of the relatively small number of investigative teams. ECG-Based Heart Arrhythmia Diagnosis Through Attentional Convolutional Neural Networks

Abstract:Electrocardiography (ECG) signal is a highly applied measurement for individual heart condition, and much effort have been endeavored towards automatic heart arrhythmia diagnosis based on machine learning. However, traditional machine learning models require large investment of time and effort for raw data preprocessing and feature extraction, as well as challenged by poor classification performance. Here, we propose a novel deep learning model, named Attention-Based Convolutional Neural Networks (ABCNN) that taking advantage of CNN and multi-head attention, to directly work on the raw ECG signals and automatically extract the informative dependencies for accurate arrhythmia detection. To evaluate the proposed approach, we conduct extensive experiments over a benchmark ECG dataset. Our main task is to find the arrhythmia from normal heartbeats and, at the meantime, accurately recognize the heart diseases from five arrhythmia types. We also provide convergence analysis of ABCNN and intuitively show the meaningfulness of extracted representation through visualization. The experimental results show that the proposed ABCNN outperforms the widely used baselines, which puts one step closer to intelligent heart disease diagnosis system.

Abstract:The artificial intelligence (AI) system has achieved expert-level performance in electrocardiogram (ECG) signal analysis. However, in underdeveloped countries or regions where the healthcare information system is imperfect, only paper ECGs can be provided. Analysis of real-world ECG images (photos or scans of paper ECGs) remains challenging due to complex environments or interference. In this study, we present an AI system developed to detect and screen cardiac abnormalities (CAs) from real-world ECG images. The system was evaluated on a large dataset of 52,357 patients from multiple regions and populations across the world. On the detection task, the AI system obtained area under the receiver operating curve (AUC) of 0.996 (hold-out test), 0.994 (external test 1), 0.984 (external test 2), and 0.979 (external test 3), respectively. Meanwhile, the detection results of AI system showed a strong correlation with the diagnosis of cardiologists (cardiologist 1 (R=0.794, p<1e-3), cardiologist 2 (R=0.812, p<1e-3)). On the screening task, the AI system achieved AUCs of 0.894 (hold-out test) and 0.850 (external test). The screening performance of the AI system was better than that of the cardiologists (AI system (0.846) vs. cardiologist 1 (0.520) vs. cardiologist 2 (0.480)). Our study demonstrates the feasibility of an accurate, objective, easy-to-use, fast, and low-cost AI system for CA detection and screening. The system has the potential to be used by healthcare professionals, caregivers, and general users to assess CAs based on real-world ECG images.

Abstract:Electrocardiogram (ECG) is a widely used reliable, non-invasive approach for cardiovascular disease diagnosis. With the rapid growth of ECG examinations and the insufficiency of cardiologists, accurate and automatic diagnosis of ECG signals has become a hot research topic. Deep learning methods have demonstrated promising results in predictive healthcare tasks. In this paper, we developed a deep neural network for multi-label classification of cardiac arrhythmias in 12-lead ECG recordings. Experiments on a public 12-lead ECG dataset showed the effectiveness of our method. The proposed model achieved an average area under the receiver operating characteristic curve (AUC) of 0.970 and an average F1 score of 0.813. The deep model showed superior performance than 4 machine learning methods learned from extracted expert features. Besides, the deep models trained on single-lead ECGs produce lower performance than using all 12 leads simultaneously. The best-performing leads are lead I, aVR, and V5 among 12 leads. Finally, we employed the SHapley Additive exPlanations (SHAP) method to interpret the model's behavior at both patient level and population level. Abstract:Automated electrocardiogram (ECG) diagnosis could be a useful aid for clinical use. We applied a deep learning method to build a system for automated detection and classification of ECG signals. We first trained a convolutional neural network (CNN) to detect cardiovascular disease in ECG signals using a training data set of 259,789 ECG signals collected from the cardiac function rooms of a tertiary care hospital. The CNN classification was validated using an independent test data set of 18,018 ECG signals. The labels used covered >90% of clinical diagnoses. The system grouped ECGs into 18 classifications-17 different types of abnormalities and normal ECG. The overall accuracy of the model was tested and found to be close to 95%; the accuracy for diagnosis of normal rhythm/atrial fibrillation was 99.15%. The proposed CNN model could help reduce misdiagnosis and missed diagnosis in primary care settings and also improve efficiency and save manpower cost for large general hospitals.

Abstract:Heart disease is one of the most common diseases causing morbidity and mortality. Electrocardiogram (ECG) has been widely used for diagnosing heart diseases for its simplicity and non-invasive property. Automatic ECG analyzing technologies are expected to reduce human working load and increase diagnostic efficacy. However, there are still some challenges to be addressed for achieving this goal. In this study, we develop an algorithm to identify multiple abnormalities from 12-lead ECG recordings. In the algorithm pipeline, several preprocessing methods are firstly applied on the ECG data for denoising, augmentation and balancing recording numbers of variant classes. In consideration of efficiency and consistency of data length, the recordings are padded or truncated into a medium length, where the padding/truncating time windows are selected randomly to sup-press overfitting. Then, the ECGs are used to train deep neural network (DNN) models with a novel structure that combines a deep residual network with an attention mechanism. Finally, an ensemble model is built based on these trained models to make predictions on the test data set. Our method is evaluated based on the test set of the First China ECG Intelligent Competition dataset by using the F1 metric that is regarded as the harmonic mean between the precision and recall. The resultant overall F1 score of the algorithm is 0.875, showing a promising performance and potential for practical use.

Abstract:Objective: Arrhythmia detection and classification are challenging because of the imbalanced ratio of normal heartbeats to arrhythmia heartbeats and the complicated combinations of arrhythmia types. Arrhythmia classification on wearable electrocardiogram monitoring devices poses a further unique challenge: unlike clinically used electrocardiogram monitoring devices, the environments in which wearable devices are deployed are drastically different from the carefully controlled clinical environment, leading to significantly more noise, thus making arrhythmia classification more difficult. Methods: We propose a novel hierarchical model based on CNN+BiLSTM with Attention to arrhythmia detection, consisting of a binary classification module between normal and arrhythmia heartbeats and a multi-label classification module for classifying arrhythmia events across combinations of beat and rhythm arrhythmia types. We evaluate our method on our proprietary dataset and compare it with various baselines, including CNN+BiGRU with Attention, ConViT, EfficientNet, and ResNet, as well as previous state-of-the-art frameworks. Results: Our model outperforms existing baselines on the proprietary dataset, resulting in an average accuracy, F1-score, and AUC score of 95%, 0.838, 0.906 for binary classification, and 88%, 0.736, 0.875 for multi-label classification. Conclusions: Our results validate the ability of our model to detect and classify real-world arrhythmia. Our framework could revolutionize arrhythmia diagnosis by reducing the burden on cardiologists, providing more personalized treatment, and achieving emergency intervention of patients by allowing real-time monitoring of arrhythmia occurrence.
Anomaly Detection in Electrocardiograms: Advancing Clinical Diagnosis Through Self-Supervised Learning

Abstract:The electrocardiogram (ECG) is an essential tool for diagnosing heart disease, with computer-aided systems improving diagnostic accuracy and reducing healthcare costs. Despite advancements, existing systems often miss rare cardiac anomalies that could be precursors to serious, life-threatening issues or alterations in the cardiac macro/microstructure. We address this gap by focusing on self-supervised anomaly detection (AD), training exclusively on normal ECGs to recognize deviations indicating anomalies. We introduce a novel self-supervised learning framework for ECG AD, utilizing a vast dataset of normal ECGs to autonomously detect and localize cardiac anomalies. It proposes a novel masking and restoration technique alongside a multi-scale cross-attention module, enhancing the model's ability to integrate global and local signal features. The framework emphasizes accurate localization of anomalies within ECG signals, ensuring the method's clinical relevance and reliability. To reduce the impact of individual variability, the approach further incorporates crucial patient-specific information from ECG reports, such as age and gender, thus enabling accurate identification of a broad spectrum of cardiac anomalies, including rare ones. Utilizing an extensive dataset of 478,803 ECG graphic reports from real-world clinical practice, our method has demonstrated exceptional effectiveness in AD across all tested conditions, regardless of their frequency of occurrence, significantly outperforming existing models. It achieved superior performance metrics, including an AUROC of 91.2%, an F1 score of 83.7%, a sensitivity rate of 84.2%, a specificity of 83.0%, and a precision of 75.6% with a fixed recall rate of 90%. It has also demonstrated robust localization capabilities, with an AUROC of 76.5% and a Dice coefficient of 65.3% for anomaly localization.

Abstract:Cardiac arrhythmias are the leading cause of death and pose a huge health and economic burden globally. Electrocardiography (ECG) is an effective technique for the diagnosis of cardiovascular diseases because of its noninvasive and cost-effective advantages. However, traditional ECG analysis relies heavily on the clinical experience of physicians, which can be challenging and time-consuming to produce valid diagnostic results. This work proposes a new hybrid deep learning model that combines convolutional neural network (CNN) and bidirectional gated recurrent unit (BiGRU) with multi-head attention (CBGM model). Specifically, the model consists of seven convolutional layers with varying filter sizes (4, 16, 32, and 64) and three pooling layers, respectively, while the BiGRU module includes two layers with 64 units each followed by multi-head attention (8-heads). The combination of CNN and BiGRU effectively captures spatio-temporal features of ECG signals, with multi-head attention comprehensively extracted global correlations among multiple segments of ECG signals. The validation in the MIT-BIH arrhythmia database achieved an accuracy of 99.41%, a precision of 99.15%, a specificity of 99.68%, and an F1-Score of 99.21%, indicating its robust performance across different evaluation metrics. Additionally, the model's performance was evaluated on the PTB Diagnostic ECG Database, where it achieved an accuracy of 98.82%, demonstrating its generalization capability. Comparative analysis against previous methods revealed that our proposed CBGM model exhibits more higher performance in automatic classification of arrhythmia and can be helpful for assisting clinicians by enabling real-time detection of cardiac arrhythmias during routine ECG screenings.

Abstract:Background: Automatic abnormalities detection based on Electrocardiogram (ECG) contributes greatly to early prevention, computer aided diagnosis, and dynamic analysis of cardiovascular diseases. In order to achieve cardiologist-level performance, deep neural networks have been widely utilized to extract abstract feature representations. However, the mechanical stacking of numerous computationally intensive operations makes traditional deep neural networks suffer from inadequate learning, poor interpretability, and high complexity. Method: To address these limitations, a clinical knowledge-based ECG abnormalities detection model using dual-view CNN-Transformer and external attention mechanism is proposed by mimicking the diagnosis of the clinicians. Considering the clinical knowledge that both the detailed waveform changes within a single heartbeat and the global changes throughout the entire recording have complementary roles in abnormalities detection, we presented a dual-view CNN-Transformer to extract and fuse spatial-temporal features from different views. In addition, the locations of the ECG where abnormalities occur provide more information than other areas. Therefore, two external attention mechanisms are designed and added to the corresponding views to help the network learn efficiently. Results: Experiment results on the 9-class dataset show that the proposed model achieves an average F1-score of 0.854±0.01 with a higher interpretability and a lower complexity, outperforming the state-of-the-art model. Conclusions: Combining all these excellent features, this study provides a credible solution for automatic ECG abnormalities detection.

Abstract:Multi-label electrocardiogram (ECG) classification is of great significance for the diagnosis of cardiovascular diseases, and it is also a challenging task because of the various of pathological patterns across different cardiac abnormalities. This paper analyzes the complex spatial-temporal variations information contained in the 12-lead ECG from a new perspective, and proposes an advanced deep neural network framework to accurately identify various multi-label ECG abnormalities. Firstly, the 12-lead ECG was modeled as 3-dimensional (3-D) spatial-temporal variations involving the intra-period, inter-period temporal dimensions, and cross-lead spatial dimension. Then, a 3-D spatial-temporal learning network (3D-STLNet) was carefully designed to extract the complex pathological features from the 3-D spatial-temporal variations. Secondly, the multi-head attention (MHA) mechanism was introduced into the proposed network to extract global temporal dependencies in ECG segments, which further improved the performance of the proposed network. By integrating a residual backbone network, a 3D-STLNet, and the MHA module, our proposed network allows end-to-end learning and prediction in a unified framework. The influence of several classic threshold selection strategies on the performance of multi-label ECG classification algorithm was also analyzed, which provides a reference for subsequent research on optimal threshold selection for multi-label ECG classification. Finally, our proposed network was evaluated on multiple large multi-label ECG public datasets, and the performance of our network outperforms that of current state-of-the-art networks, which demonstrates that our network has great competitiveness in identifying a wider range of multi-label ECG abnormalities.

Abstract:Arrhythmia accounts for more than 80% of sudden cardiac death, and its incidence rate has increased rapidly recently. Nowadays, many studies have applied artificial intelligence (AI) methods to arrhythmia detection. Deep learning approaches can improve model performance but lack the capability to mine the relationship between the electrocardiogram (ECG) samples and final results. Furthermore, most arrhythmia diagnosis methods aim at only multiclassification problems. However, in many cases, an ECG recording may have more than one arrhythmia disease. In this article, we develop an accurate and interpretable model for multilabel ECG signals, which is called dual-level attentional convolutional long short-term memory neural network (DLA-CLSTM). Convolutional layers, bidirectional long short-term memory layers, and attention mechanisms are used for extracting beat- and rhythm-level information. Notably, the attention mechanism helps the model be more interpretable. In addition, we design a weighted multilabel classifier to address multilabel and imbalance problems. Finally, the MIT-BIH Arrhythmia Database and the 1st China Physiological Signal Challenge dataset are utilized to evaluate DLA-CLSTM. Compared with existing methods, DLA-CLSTM can improve the accuracy by 22.50% and the F1-macro by 20.51% on average. Further analysis shows that the areas focused by DLA-CLSTM match the clinical judgment basis to some extent. Collectively, DLA-CLSTM has good potential for assisting clinicians in real applications.

This article analyzes the complex spatial-temporal variations information contained in the 12-lead ECG from a new perspective and proposes an advanced deep neural network (DNN) framework to accurately identify various multilabel ECG abnormalities. First, the 12-lead ECG was modeled as 3-D spatial-temporal variations involving the intraperiod and interperiod temporal dimensions and cross-lead spatial dimension. Then, a 3-D spatial-temporal learning network (3D-STLNet) was carefully designed to extract the complex pathological features from the 3-D spatial-temporal variations. Second, the multihead attention (MHA) mechanism was introduced into the proposed network to extract global temporal dependencies in ECG segments, which further improved the performance of the proposed network. By integrating a residual backbone network, a 3D-STLNet, and the MHA module, our proposed network allows end-to-end learning and prediction in a unified framework. The influence of several classic threshold selection strategies on the performance of multilabel ECG classification algorithm was also analyzed, which provides a reference for subsequent research on optimal threshold selection for multilabel ECG classification. Finally, our proposed network was evaluated on multiple large multilabel ECG public datasets, and the performance of our network outperforms that of current state-of-the-art networks, which demonstrates that our network has great competitiveness in identifying a wider range of multilabel ECG abnormalities.
A Multi-label Learning Method to Detect Arrhythmia Based on 12-Lead ECGs.

Abstract:Cardiovascular disease (CVD) is one of the most serious diseases that harm human life and gives a huge burden to the health care system. Recent advances in deep learning have achieved great success in object detection, speech and image recognition. Although deep learning has been applied to the detection of arrhythmia, detection accuracy is limited because of three major issues: 1. Each ECG signal maybe contains more than one-label information; 2. It is hard to classify ECG with different lengths; 3. Data imbalance problem is severe for arrhythmia detection. In this paper, we present a multi-label learning algorithm to address the class imbalance and detection on ECGs with different durations. We utilize Deep Convolutional Generative Adversarial Networks (DCGANs) and Wasserstein GAN-Gradient Penalty (WGAN-GP) to generate new positive samples and use two losses to balance the importance between positive samples and negative samples. Moreover, we construct a Squeeze and Excitation-ResNet (SE-ResNet) module for normal rhythm and arrhythmia detection. In order to solve the multi-label classification problem, we train nine different binary classifiers for each category and determine which types of rhythm the ECG signals belong to. Experimental results on The ECG Intelligence Challenge 2019 dataset demonstrate that our multi-label learning method achieves competitive performance in multi-label ECGs classification.

Abstract:BackgroundElectrocardiogram automated arrhythmia detection plays a crucial role in the early prevention and diagnosis of cardiovascular diseases. However, previous research relies on noise removal algorithms and extracting solid features from raw ECGs. Besides, existing heartbeat classifiers ignore underlying complementary information of various scales, and intra-patient paradigms often lead to biased results.MethodsWe constructed a novel end-to-end Multi-Scale Convolutional Neural Network-Sequence to Sequence architecture for heartbeat classification to address these issues. We have verified this approach on the clinical data collected by wearable devices and two heterogeneous datasets.ResultsThe proposed model can effectively capture multi-granularity frequency and longitudinal temporal information by fusion representation and sequence learning. The overall F1 score of our approach was achieved at 99.57%, which exceeded the reference pure cascade model by 4.36%.

Abstract:Background Deep learning has demonstrated diagnostic performance in electrocardiogram (ECG) analysis comparable to that of clinicians. However, existing models primarily excel in diagnosing common conditions such as arrhythmias and heart blocks, which are easily identifiable using standard predictor variables. Conversely, models aiming to uncover novel insights, such as detecting structural changes or the presence of myocardial scar, often lack generalizability due to insufficient data, model architecture limitations, and other factors. Moreover, these models frequently encounter challenges, such as a decline in performance when applied to external datasets. To address these issues comprehensively, we propose the ECGWiz — a unified model capable of tackling diverse ECG-related problems with consistently high performance. Objective To develop a generalizable Foundation AI model for ECG diagnosis capable of detecting diverse cardiac abnormalities and diseases using few training samples. Methods We utilized publicly available datasets like PTB-XL, CPSC, etc. To build our model, totaling approximately 80,000 ECGs. These 12 lead ECGs are scaled and filtered before using them for training. The AI model is pretrained using Self Supervised Learning and Contrastive Learning strategies. Further, the model undergoes supervised training using a hierarchy of labels that increase in granularity, encompassing arrhythmias, heart blocks, other cardiac abnormalities, heart failure, sudden cardiac death and more. Lastly, the model is prepared for Few-Shot Learning — where the model learns from a minimal set of samples (5 to 10 ECGs) of the target disease. For testing the model, we are using unique datasets like PROSE which consists of non-ischemic cardiomyopathy patients with and without myocardial scaring detected using cardiac MRI imaging at the Johns Hopkins University. Results Our model, trained on a dataset of only 5 ECG recordings, demonstrated a 74% accuracy in identifying myocardial scarring as detected by LGE MRI, utilizing a standard 12-lead ECG. This performance is comparable to that of conventional deep learning methods, which necessitate datasets containing upwards of 120 ECG recordings. Conclusion Our model represents a promising stride in creating a unified and adaptable foundation AI model for comprehensive ECG-based diagnosis. Its capacity to learn from a minimal set of examples signifies a robust framework that can evolve and adapt to new challenges in ECG diagnosis. Ongoing improvements, particularly through dataset expansion, underscore its potential to enhance cardiovascular diagnostics, offering precise and efficient patient care.

Abstract:Cardiovascular disease is a major global public health problem, and intelligent diagnostic approaches play an increasingly important role in the analysis of electrocardiograms (ECGs). Convenient wearable ECG devices enable the detection of transient arrhythmias and improve patient health by making it possible to seek intervention during continuous monitoring. We collected 658,486 wearable 12-lead ECGs, among which 164,538 were annotated, and the remaining 493,948 were without diagnostic. We present four data augmentation operations and a self-supervised learning classification framework that can recognize 60 ECG diagnostic terms. Our model achieves an average area under the receiver-operating characteristic curve (AUROC) and average F1 score on the offline test of 0.975 and 0.575. The average sensitivity, specificity and F1-score during the 2-month online test are 0.736, 0.954 and 0.468, respectively. This approach offers real-time intelligent diagnosis, and detects abnormal segments in long-term ECG monitoring in the clinical setting for further diagnosis by cardiologists.
