Abstract:Background: Transcranial direct current stimulation (tDCS) is used to modulate neuronal activity, but the exact mechanism of action (MOA) is unclear. This study investigates tDCS-induced modulation of the corticospinal excitability and the underlying MOA. By anesthetizing the scalp before applying tDCS and by stimulating the cheeks, we investigated whether stimulation of peripheral and/or cranial nerves contributes to the effects of tDCS on corticospinal excitability. Materials and methods: In a randomized cross-over study, four experimental conditions with anodal direct current stimulation were compared in 19 healthy volunteers: 1) tDCS over the motor cortex (tDCS-MI), 2) tDCS over the motor cortex with a locally applied topical anesthetic (TA) on the scalp (tDCS-MI + TA), 3) DCS over the cheek region (DCS-C), and 4) sham tDCS over the motor cortex(sham). tDCS was applied for 20 minutes at 1 mA. Motor evoked potentials (MEPs) were measured before tDCS and immediately, 15, 30, 45, and 60 minutes after tDCS. A questionnaire was used to assess the tolerability of tDCS. Results: A significant MEP amplitude increase compared with baseline was found 30 minutes after tDCS-MI, an effect still observed 60 minutes later; no time∗condition interaction effect was detected. In the other three conditions (tDCS-MI + TA, DCS-C, sham), no significant MEP modulation was found. The questionnaire indicated that side effects are significantly lower when the local anesthetic was applied before stimulation than in the other three conditions. Conclusions: The significant MEP amplitude increase observed from 30 minutes on after tDCS-MI supports the modulatory effect of tDCS on corticospinal neurotransmission. This effect lasted one hour after stimulation. The absence of a significant modulation when a local anesthetic was applied suggests that effects of tDCS are not solely established through direct cortical stimulation but that stimulation of peripheral and/or cranial nerves also might contribute to tDCS-induced modulation.
Electric fields of motor and frontal tDCS in a standard brain space: A computer simulation study

Abstract:The electric field produced in the brain is the main physical agent of transcranial direct current stimulation (tDCS). Inter-subject variations in the electric fields may help to explain the variability in the effects of tDCS. Here, we use multiple-subject analysis to study the strength and variability of the group-level electric fields in the standard brain space. Personalized anatomically-accurate models of 62 subjects were constructed from T1- and T2-weighted MRI. The finite-element method was used to computationally estimate the individual electric fields, which were registered to the standard space using surface based registration. Motor cortical and frontal tDCS were modelled for 16 electrode montages. For each electrode montage, the group-level electric fields had a consistent strength and direction in several brain regions, which could also be located at some distance from the electrodes. In other regions, the electric fields were more variable, and thus more likely to produce variable effects in each individual. Both the anode and cathode locations affected the group-level electric fields, both directly under the electrodes and elsewhere. For motor cortical tDCS, the electric fields could be controlled at the group level by moving the electrodes. However, for frontal tDCS, the group-level electric fields were more variable, and the electrode locations had only minor effects on the group average fields. Our results reveal the electric fields and their variability at the group level in the standard brain space, providing insights into the mechanisms of tDCS for plasticity induction. The data are useful for planning, analysing and interpreting tDCS studies.

Abstract:Transcranial Direct Current Stimulation (tDCS) has benefits for motor rehabilitation in stroke patients, but its clinical application is limited due to inter-individual heterogeneous effects. Recently, optimized tDCS that considers individual brain structure has been proposed, but the utility thereof has not been studied in detail. We explored whether optimized tDCS provides unique electrode positions for each patient and creates a higher target electric field than the conventional approach. A comparative within-subject simulation study was conducted using data collected for a randomized controlled study evaluating the effect of optimized tDCS on upper extremity function in stroke patients. Using Neurophet tES LAB 3.0 software, individual brain models were created based on magnetic resonance images and tDCS simulations were performed for each of the conventional and optimized configurations. A comparison of electrode positions between conventional tDCS and optimized tDCS was quantified by calculation of Euclidean distances. A total of 21 stroke patients were studied. Optimized tDCS produced a higher electric field in the hand motor region than conventional tDCS, with an average improvement of 20% and a maximum of 52%. The electrode montage for optimized tDCS was unique to each patient and exhibited various configurations that differed from electrode placement of conventional tDCS. Optimized tDCS afforded a higher electric field in the target of a stroke patient compared to conventional tDCS, which was made possible by appropriately positioning the electrodes. Our findings may encourage further trials on optimized tDCS for motor rehabilitation after stroke.

Abstract:Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation technique that has been utilized for treating brain disorders and improving cognitive function. In order to achieve targeted tDCS, many optimization methods of montages and electric currents have been proposed. However, these methods have some limitations. Most of them were proposed for single-objective optimization (focality or intensity) and have no constrain with the number of electrodes (Most devices only have less than 8 electrodes currently). In this study, we proposed an operational optimization approach for well-targeted tDCS, which aims to optimize for two objectives of electric field (EF) intensity and focality with constraints on the number of electrodes. Compared with traditional tDCS in our cohort (10 subjects), our method significantly improves the EF focality. When compared to commonly used 4×1 high-definition tDCS (HD-tDCS), our method can achieve higher EF intensity in the target region with less than 8 electrodes. Our method can balance the two objectives of EF and shorten optimization time, which is convenient for practical application.

Abstract:Over the last decade, Transcranial Direct Current Stimulation (tDCS) has been applied not only to modulate local cortical activation, but also to address communication between functionally-related brain areas. Stimulation protocols based on simple two-electrode placements are being replaced by multi-electrode montages to target intra- and inter-hemispheric neural networks using multichannel/high definition paradigms. Objective: This study aims to investigate the characteristics of electric field (EF) patterns originated by tDCS experiments addressing changes in functional brain connectivity. Methods: A previous selection of tDCS experimental studies aiming to modulate motor-related connectivity in health and disease was conducted. Simulations of the EF induced in the cortex were then performed for each protocol selected. The EF magnitude and orientation are determined and analysed in motor-related cortical regions for five different head models to account for inter-subject variability. Functional connectivity outcomes obtained are qualitatively analysed at the light of the simulated EF and protocol characteristics, such as electrode position, number and stimulation dosing. Main findings: The EF magnitude and orientation predicted by computational models can be related with the ability of tDCS to modulate brain functional connectivity. Regional differences in EF distributions across subjects can inform electrode placements more susceptible to inter-subject variability in terms of brain connectivity-related outcomes. Significance: Neuronal facilitation/inhibition induced by tDCS fields may indirectly influence intra and inter-hemispheric connectivity by modulating neural components of motor-related networks. Optimization of tDCS using computational models is essential for adequate dosing delivery in specific networks related to clinically relevant connectivity outcomes.

Abstract:Introduction: The conventional method of placing transcranial direct current stimulation (tDCS) electrodes is just above the target brain area. However, this strategy for electrode placement often fails to improve motor function and modulate cortical excitability. We investigated the effects of optimized electrode placement to induce maximum electrical fields in the leg regions of both M1 and SMA, estimated by electric field simulations in the T1and T2-weighted MRI-based anatomical models, on motor performance and cortical excitability in healthy individuals. Methods: A total of 36 healthy volunteers participated in this randomized, triple-blind, sham-controlled experiment. They were stratified by sex and were randomly assigned to one of three groups according to the stimulation paradigm, including tDCS with (1) anodal and cathodal electrodes positioned over FCz and POz, respectively, (A-P tDCS), (2) anodal and cathodal electrodes positioned over POz and FCz, respectively, (P-A tDCS), and (3) sham tDCS. The sit-to-stand training following tDCS (2 mA, 10 min) was conducted every 3 or 4 days over 3 weeks (5 sessions total). Results: Compared to sham tDCS, A-P tDCS led to significant increases in the number of sit-to-stands after 3 weeks training, whereas P-A tDCS significantly increased knee flexor peak torques after 3 weeks training, and decreased short-interval intracortical inhibition (SICI) immediately after the first session of training and maintained it post-training. Discussion: These results suggest that optimized electrode placement of the maximal EF estimated by electric field simulation enhances motor performance and modulates cortical excitability depending on the direction of current flow.
The Pursuit of DLPFC: Non-neuronavigated Methods to Target the Left Dorsolateral Pre-frontal Cortex With Symmetric Bicephalic Transcranial Direct Current Stimulation (tDCS)

Abstract:Background: The dose of transcranial direct current stimulation (tDCS) is defined by electrode montage and current, while the resulting brain current flow is more complex and varies across individuals. The left dorsolateral pre-frontal cortex (lDLPFC) is a common target in neuropsychology and neuropsychiatry applications, with varied approaches used to experimentally position electrodes on subjects. Objective: To predict brain current flow intensity and distribution using conventional symmetrical bicephalic frontal 1 × 1 electrode montages to nominally target lDLPFC in forward modeling studies. Methods: Six high-resolution Finite Element Method (FEM) models were created from five subjects of varied head size and an MNI standard. Seven electrode positioning methods, nominally targeting lDLPFC, were investigated on each head model: the EEG 10-10 including F3-F4, F5-F6, F7-8, F9-F10, the Beam F3-System, the 5-5 cm-Rule and the developed OLE-System were evaluated as electrode positioning methods for 5 × 5 cm(2) rectangular sponge-pad electrodes. Results: Each positioning approach resulted in distinct electrode positions on the scalp and variations in brain current flow. Variability was significant, but trends across montages and between subjects were identified. Factors enhancing electric field intensity and relative targeting in lDLPFC include increased inter-electrode distance and proximity to thinner skull structures. Conclusion: Brain current flow can be shaped, but not focused, across frontal cortex by tDCS montages, including intensity at lDLPFC. The OLE-system balances lDLPFC targeting and reduced electric field variability, along with clinical ease-of-use.

Abstract:Transcranial direct current stimulation (tDCS) is increasingly being used as a possible tool to enhance plasticity in brain regions that might undergo remodeling after a stroke. Our aim was to determine whether dose and montage of tDCS leads to differential changes in regional cerebral blood flow (rCBF) and differential effect on a finger sequence motor learning experiment. Methods: A total of 20 right-handed healthy subjects participated in 6 imaging and 6 motor learning sessions. Brain images of non-invasive blood flow (using arterial spin labelling) were obtained before (6min), during (10min), as well as after (8min) tDCS was applied within the MRI scanner. For the motor sequence learning experiment as well as for the imaging studies, we used three different dose levels (Sham, 2mA, and 4mA) and two different electrode montages (unihemispheric or bihemispheric). In both montages, the anodal electrode (excitability increasing) was placed over the right motor region (C4) while the cathodal electrode (excitability decreasing) was placed either over a left supraorbital region (unihemispheric montage) or over the left motor region (C3; bihemispheric montage). Imaging data were processed and analyzed using SPM12. For the finger sequence learning task, subjects were asked to type a randomly chosen sequence of 7 items (using numbers 2-5 on a computer keyboard) with digits 2-5 of each hand several times before and immediately after a 10 min stimulation period. Results: After unihemispheric and bihemispheric stimulation, left hand showed more improvement in finger sequence learning than right hand, although a linear trend as a function of dose was observed in both montage conditions. rCBF increased during the Stimulation condition and showed a clear linear effect of dose in the peri-rolandic region. Bihemispheric stimulation showed similar, but stronger effects and revealed additional activation of frontomesial regions in the 4mA condition. Conclusion: tDCS dose has an effect on regional surrogate markers of neuronal/synaptic. Effects in a finger sequence learning showed a similar dose effect. Montage (unihemispheric versus bihemispheric) shows differential brain effects, but based on our preliminary analysis no clear behavioral effects.

Abstract:Abstract Transcranial direct current stimulation (tDCS) is an increasingly popular tool to investigate the involvement of the cerebellum in a variety of brain functions and pathologies. However, heterogeneity and small effect sizes remain a common issue. One potential cause may be interindividual variability of the electric fields induced by tDCS. Here, we compared electric field distributions and directions between two conventionally used electrode montages (i.e., one placing the return electrode over the ipsilateral buccinator muscle and one placing the return electrode [25 and 35 cm 2 surface area, respectively] over the contralateral supraorbital area; Experiment 1) and six alternative montages (electrode size: 9 cm 2 ; Experiment 2) targeting the right posterior cerebellar hemisphere at 2 mA. Interindividual and montage differences in the achieved maximum field strength, focality, and direction of current flow were evaluated in 20 head models and the effects of individual differences in scalp–cortex distance were examined. Results showed that while maximum field strength was comparable for all montages, focality was substantially improved for the alternative montages over inferior occipital positions. Our findings suggest that compared to several conventional montages extracerebellar electric fields are significantly reduced by placing smaller electrodes in closer vicinity of the targeted area.

Abstract:Transcranial direct current stimulation (tDCS) has been applied in numerous brain and neurological studies over recent years. However, the understand about the mechanisms of tDCS is limited. This paper presents the method to obtain a realistic CT imaging-derived finite element computional model through reverse engineering. The model can be used to analyze the distributions of electric field in tDCS with different electrode configuration. Based on the model, current density distributions of tDCS with two different size of electrodes were calculated to compare the focality and stimulation depth. The results shows that high current density were found under the electrodes, and electrode configuration effects current density distribution significantly. The conclusion may guide the optimization of tDCS parameters for clinical applications.

Abstract:Transcranial direct current stimulation (tDCS) is a technique that has been intensively investigated in the past decade as this method offers a non-invasive and safe alternative to change cortical excitability. The effects of one session of tDCS can last for several minutes, and its effects depend on polarity of stimulation, such as that cathodal stimulation induces a decrease in cortical excitability, and anodal stimulation induces an increase in cortical excitability that may last beyond the duration of stimulation. These effects have been explored in cognitive neuroscience and also clinically in a variety of neuropsychiatric disorders--especially when applied over several consecutive sessions. One area that has been attracting attention of neuroscientists and clinicians is the use of tDCS for modulation of pain-related neural networks. Modulation of two main cortical areas in pain research has been explored: primary motor cortex and dorsolateral prefrontal cortex. Due to the critical role of electrode montage, in this article, we show different alternatives for electrode placement for tDCS clinical trials on pain; discussing advantages and disadvantages of each method of stimulation.

Abstract:Electrode montage optimization for transcranial electric stimulation (tES) is a challenging topic for targeting a specific brain region. Targeting the deep brain region is difficult due to tissue inhomogeneity, resulting in complex current flow. In this study, a simplified protocol for montage optimization is proposed for multichannel tES (mc-tES). The purpose of this study was to reduce the computational cost for mc-tES optimization and to evaluate the mc-tES for deep brain regions. Optimization was performed using a simplified protocol for montages under safety constraints with 20 anatomical head models. The optimization procedure is simplified using the surface EF of the deep brain target region, considering its small volume and non-concentric distribution of the electrodes. Our proposal demonstrated that the computational cost was reduced by >90%. A total of six–ten electrodes were necessary for robust EF in the target region. The optimization with surface EF is comparable to or marginally better than using conventional volumetric EF for deep brain tissues. An electrode montage with a mean injection current amplitude derived from individual analysis was demonstrated to be useful for targeting the deep region at the group level. The optimized montage and injection current were derived at the group level. Our proposal at individual and group levels showed great potential for clinical application.

Abstract:Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation method.The electrode positions and the current intensity are optimized using a finite element analysis of a four layer spherical model based on the human body anatomy to determine the electric field distribution deep in the brain and on the brain surface.The results show that the electric field intensity increases with the stimulation current increase.The electrode area is linearly related to the stimulation effective area ratio in the brain.Smaller electrodes produce a higher proportion of electric field distribution on the cortex than in the deep brain.This model can be used to select the electrode area and current intensity for the desired dose and target location.

Abstract:We used three dose levels (Sham, 2mA, and 4mA) and two different electrode montages (unihemispheric or bihemispheric) to examine DOSE and MONTAGE effects on regional cerebral blood flow (rCBF) as a surrogate marker of neural activity, and on a finger sequence task, as a surrogate behavioral measure drawing on brain regions targeted by transcranial direct current stimulation (tDCS). We placed the anodal electrode over the right motor region (C4) while the cathodal or return electrode was placed either over a left supraorbital region (unihemispheric montage) or over the left motor region (C3 in the bihemispheric montage). Performance changes in the finger sequence task for both hands (left hand: p = 0.0026, and right hand: p = 0.0002) showed a linear tDCS dose response but no montage effect. rCBF in the right hemispheric perirolandic area increased with dose under the anodal electrode (p = 0.027). In contrast, in the perirolandic ROI in the left hemisphere, rCBF showed a trend to increase with dose (p = 0.053) and a significant effect of montage (p = 0.00004). The bihemispheric montage showed additional rCBF increases in frontomesial regions in the 4mA condition but not in the 2mA condition. Furthermore, we found correlations between rCBF changes in the right perirolandic region and improvements in the finger sequence task performance (FSP) for the left and right hand. Our data support not only a strong direct tDCS dose effect for rCBF and FSP as surrogate measures of targeted brain regions but also indirect effects on rCBF in functionally connected regions (e.g., frontomesial regions), particularly in the higher dose condition and on FSP of the ipsilateral hand (to the anodal electrode). At a higher dose and irrespective of polarity, a wider network of sensorimotor regions is positively affected by tDCS.

Abstract:Abstract Background Multichannel transcranial electrical stimulation (tES) modeling and optimization have been widely studied in recent years. Its theoretical bases include quasi-static assumption and linear superposition. However, there is still a lack of direct in vivo evidence to validate the simulation model and theoretical assumptions. Methods We directly measured the multichannel tES-induced voltage changes with implanted stereotactic-electroencephalographic (sEEG) electrodes in 12 epilepsy subjects. By combining these measured data, we investigate the linear superposition and prediction accuracy of simulation models for multi-electrode stimulation and further compare the induced EF differences between transcranial direct current stimulation (tDCS) and transcranial alternating current stimulation (tACS). Results Our in vivo measurements demonstrated that the multi-electrode tES-induced voltages were almost equal to the sum of the voltages generated independently by bipolar stimulation. Both measured voltages and electric fields obtained in vivo were highly correlated with the predicted values in our cohort (Voltages: r = 0.92, p < 0.001; electric fields: r = 0.74, p < 0.001). Under the same stimulation intensity, the tDCS-induced peak-zero voltages were highly correlated with the values of tACS ( r = 0.99, p < 0.001; s = 0.99). Conclusions The in vivo measurements provides confirmatory results for linear superposition and quasi-static assumption within the human brain. Furthermore, we found that the individualized simulation model reliably predicted the multi-electrode tES-induced electric fields.

In a randomized cross-over design, 21 participants underwent 10 min of anodal tDCS with 0.5, 1.0, 1.5, or 2.0 mA or sham. Compared to sham, all active conditions increased the size of motor evoked potentials (MEP) normalized to the pre-tDCS baseline, irrespective of anterior or posterior magnetic test stimuli. The electric field calculated in the motor cortex of each participant had a nonlinear effect on the normalized MEP size, but its effects were small compared to those of other participant-specific factors. The findings support the efficacy of anodal tDCS in enhancing the MEP size but do not demonstrate any benefits of personalized electric field modeling in explaining tDCS response variability.

Abstract:Background: Although tDCS constitutes a non-invasive neuromodulation technique with promising results in a great variety of applications, its clinical implementation is compromised by the high inter-subject variability reported. This study aims to analyze the inter-subject variability in electric fields (E-fields) over regions of the cortical motor network under two electrode montages: the classical C3Fp2 and an alternative P3F3, which confines more the Efield over this region. Methods: Computational models of the head of 98 healthy subjects were developed to simulate the E-field under both montages. E-field parameters such as magnitude, focality and orientation were calculated over three regions of interest (ROI): M1S1, SMA and preSMA. The role of anatomical characteristics as a source of inter-subject variability on E-field parameters was addressed using linear mixed-effect models. Results: P3F3 showed a more confined E-field distribution over M1S1 than C3Fp2; the latter elicited higher E-fields over supplementary motor areas. Both montages showed high inter-subject variability, especially for the normal component over C3Fp2. Skin, bone and CSF ROI volumes showed a negative association with E-field magnitude irrespective of montage. Grey matter volume and montage were the main sources of variability for focality. The curvature of gyri was found to be significantly associated with the variability of normal E-fields. Conclusions: Computational modelling proves useful in the assessment of E-field variability. Our simulations predict significant differences in E-field magnitude and focality for C3Fp2 and P3F3. However, anatomical characteristics were also found to be significant sources of E-field variability irrespective of electrode montage.

The effects of transcranial direct current stimulation on corticospinal and cortico-cortical excitability and response variability: conventional versus high-definition montages

Abstract:Abstract Response variability following transcranial direct current stimulation (tDCS) highlights need for exploring different tDCS electrode montages. This study compared corticospinal excitability (CSE), cortico-cortical excitability and intra-individual variability following conventional and HD anodal (a-tDCS) and cathodal (c-tDCS) tDCS. Fifteen healthy young males attended four sessions at least one-week apart: conventional a-tDCS, conventional c-tDCS, HD-a-tDCS, HD-c-tDCS. TDCS was administered (1mA, 10-minutes) over the primary motor cortex (M1), via 6×4cm active and 7×5cm return electrodes (conventional tDCS) and 4×1 ring-electrodes 3.5cm apart in ring formation around M1 (HD-tDCS). For CSE, twenty-five single-pulse transcranial magnetic stimulation (TMS) peak-to-peak motor evoked potentials (MEP) were recorded at baseline, 0-minutes and 30-minutes post-tDCS. For cortico-cortical excitability, twenty-five paired-pulse MEPs with 3-millisecond (ms) inter-pulse interval (IPI) and twenty-five at 10ms assessed short-interval intracortical inhibition (SICI) and intracortical facilitation (ICF) respectively. MEP standardised z-values standard deviations represented intra-individual variability. No significant differences were reported in CSE between conventional and HD a-tDCS, but significant differences between conventional and HD c-tDCS 0-minutes post-tDCS. Intra-individual variability was significantly reduced in conventional tDCS compared to HD-tDCS for a-tDCS (0-minutes) and c-tDCS (30-minutes). No significant changes were reported in SICI and ICF. These novel findings highlight current technical issues with HD-tDCS, suggesting future tDCS studies should utilise conventional tDCS to minimise intra-individual variability, ensuring tDCS after-effects are true changes in CSE and cortico-cortical excitability.
