Abstract:Sir:FigureThere have been only a few studies conducted regarding the posttraumatic functional and cosmetic outcomes of young children postoperatively.1,2 In this article, special attention is paid to growth potential and growth outcome of transferred toes. Between 1992 and 2002, four children with a mean age of 4 years (range, 1 to 7 years) underwent second toe–to-hand transfers for thumb or finger reconstructions in our hospital. All children were followed for at least 4 years. The radiographic phalanx length was measured from the standard view. The actual length of each phalanx was measured by the actual distance between the transverse creases of each finger. Results of the mean length of each transferred phalanx were compared with those of the contralateral toes and normal finger as an evaluation of growth potential and eventual growth in transplanted toes. The average total active motion of the proximal interphalangeal joints was 72 degrees (interphalangeal joint in the thumb). The length of the three transferred phalanges was similar to the span of the thumb. The mean radiographic length and actual length of the distal and middle phalanges were over 90 percent that of the contralateral toe (Table 1), but the growth of the proximal phalange in case 2 (Figs. 1 and 2) was far in excess of that of the contralateral toe (40 and 45 percent, respectively).Table 1: Radiographic and Actual Growth Potential of Each Phalanx in Transferred ToesFig. 1: Radiographs showing a boy's right hand in the first year postoperatively.Fig. 2: Oblique and posteroanterior radiographs of the right hand in the seventh year postoperatively.The growth potential of a transferred toe has always played a key role in the evaluation of toe-to-hand transfer despite the fact that many studies3,4 have not provided details regarding the growth potential and eventual growth of these revascularized epiphyseal bones. We recorded the postoperative radiographic length and actual length of all phalanges of every transferred toe. The mean length of each transferred phalanx was compared with that of the contralateral toe and also with that of the normal side hand. The difference between case 2 and the others was whether transfer was performed with the proximal epiphyseal plates and base bone. A comparison of our results with similar reports1,2,5 on toe transfer for reconstruction of traumatic finger and thumb amputations is shown in Table 2. Trauma was the cause in only five of 40 patients (12.5 percent) in the report by Kay et al.1 The average age of the patients in the report by Wei et al.2 was 12 years. Gülgönen and Gudemez5 reported only one child among their five cases.
